Umbilical Cord Blood Transplant (UCBT) is a type of allogenic haematopoetic stem cell transplant. Stem cell transplantation as a mode of treatment of diseases was first successfully done by the pioneering work of a Nobel Prize Winner in Physiology or Medicine, E. Donnall Thomas in 1957. Throughout the past 50 years, numerous other researchers have improved the techniques of this previously highly risky procedure. Among the notable landmark in haematopoetic stem cell transplant include: advancement in immunogenetics of HLA, better immunosuppressive preparative regimen, improved control of infections, appearance of donor registries throughout the world and successful transplantation stories using haematopoetic stem cells from peripheral blood and the cord blood. These developments lead to a change in terminology from marrow transplantation to haematopoetic stem cell transplantation .

Polyfluoroalkyl and perfluoroalkyl substances (PFAS) are persistent organic pollutants and exposure have been suggested with the risk of developing preeclampsia. Yet, evidence on the associations of PFAS with preeclampsia is still conflicting. Thus, the current study conducted a systematic review and meta-analysis of the epidemiological evidence linking maternal PFAS exposure to preeclampsia. This research methodology involved searching three electronic databases for epidemiological studies, and then conducting a meta-analysis using a random-effects model to analyse the heterogeneity between the studies. The quality and strength of evidence for each exposure-outcome pair was also evaluated, as well as the risk of bias. The search identified 10 potentially eligible studies related to maternal PFAS blood level with preeclampsia, which 7 were ultimately selected. Meta-analysis demonstrated evidence of association between combined PFAS compounds in pregnant mother with preeclampsia with zero heterogeneity (I2=0.0%, Q= 3.09, df= 6, p=0.798). Preeclampsia was found to have moderate association with maternal perfluorooctanoic acid (PFOA) exposure (Test for overall effect: z=2.2, p=0.03; Test for heterogeneity: I2=0.0%, Q= 3.49, df= 6, p=0.745) as well as maternal perfluorooctane sulfonate (PFOS) exposure (Test for overall effect: z=2.5, p=0.01; Test for heterogeneity: I2=0.0%, Q= 3.70, df= 6, p=0.718). This study showed significant associations between PFOA and PFOS exposure with the risk of preeclampsia. However, in-depth investigation is imperative to elucidate the impact of the different concentration and types of PFAS on preeclampsia risk.

PURPOSE: Surface micropatterning is being explored as a strategy to mitigate thrombus formation and reduce long-term anticoagulation requirements in left ventricular assist devices (LVADs). This study investigated whether specific micro-topographies can modulate platelet deposition under LVAD supraphysiological wall shear stress (WSS) conditions.

MATERIALS AND METHODS: A custom microfluidic platform was developed to generate a linear WSS gradient from 16 to 130 Pa. Microchannels were patterned with reverse cones and hemispheres in small (1-3 µm) and large (3-9 µm) sizes using two-photon polymerization and nanoimprinting. Human blood was perfused through the microchannels, and platelet deposition was quantified over time as the area coverage ratio (Aₚₗₜ/Aₜₒₜ) and area under the curve (AUC). Platelet detachment events were counted as an indicator of thrombus stability, and computational simulations supported the interpretation of local shear conditions.

RESULTS: Consistent trends emerged, although no statistical differences were observed relative to flat controls. Aₚₗₜ/Aₜₒₜ increased with WSS for all surfaces. At 16 Pa, small and large cones reduced platelet adhesion by approximately 84 and 98%, respectively, compared to flat controls. At 49 Pa, the reduction was about 95% for small cones and 80% for large cones. Conical geometries also promoted platelet washout at higher WSS. Small hemispherical features showed more than 50% lower platelet adhesion than flat surfaces for WSS > 16 Pa, with limited thrombus growth.

CONCLUSION: Conical micropatterns may be most effective at limiting platelet adhesion at lower WSS, while small hemispheres may perform better at higher WSS. These trends suggest a surface-shear interaction that warrants further investigation for LVAD surface optimization.

Patients with thrombocytopenia requiring ongoing platelet transfusion support may develop inadequate platelet count increments, referred to as platelet refractoriness (PR), which further complicates their care. The underlying etiologies of PR can be broadly divided into immune and nonimmune causes. A high index of suspicion is required to initiate testing for alloimmunization, and the leading culprit in immune PR is the development of class I HLA antibodies. The approach to diagnosis of immune PR has changed over recent years with new technologies, but questions regarding the clinical significance and interpretation of these methods have not been conclusively answered. The provision of HLA-matched platelets requires close and timely coordination between transfusion services and clinical teams; however, the true impact of their provision on clinical outcomes is not clear. This paper reviews diagnostic and management challenges, appraises the existing data available to support treatment options, and identifies research gaps.

Polycystic ovarian syndrome (PCOS) is an endocrine disease characterized by hormonal imbalances, metabolic inefficiency, and infertility problems. Furthermore, anti-Müllerian hormone (AMH), testosterone, and insulin are PCOS biomarkers that need to be detected accurately for early diagnosis and treatment. A narrative review discusses current improvements in nanomaterial-enhanced biosensors that detect biomarkers with high sensitivity and specificity. In addition, nanomaterials such as graphs and quantum dots have large surface areas and unique physicochemical properties that make them more effective biosphere. In addition, wearables and lab-on-chip platforms benefit from these features because they can detect in real time. In addition, artificial intelligence (AI) and machine learning (ML) are investigated to increase data interpretation and risk stratification using optical and electrochemical biosensors. Consequently, these biosensor technologies provide molecular insights into the underlying causes of PCOS, such as hyperandrogenism, insulin resistance, and chronic inflammation. By developing a short and portable biosensor, we can bridge the bridge between laboratory research and clinical practice and provide user-friendly diagnosis. Along with increasing clinical accuracy, the nanomaterial-based biosensor is considered a platform to learn more about PCOS pathology. Finally, their integration into clinical practice can contribute to the development of individual treatment methods in reproductive endocrinology and encourage research in the future.

Small noncoding microRNAs (miRNAs) are known as noninvasive biomarkers for early detection in various cancers. In fact, miRNAs have key roles in carcinogenicity process such as proliferation, apoptosis and metastasis. After cardiovascular disease, cancer is the second cause of death in the world with an estimated 9.6 million deaths in 2018. So, early diagnosis of cancer is critical for successful treatment. To date, several selective and sensitive laboratory-based methods have been applied for the detection of circulating miRNA, but a simple, short assay time and low-cost method such as a biosensor method as an alternative approach to monitor cancer biomarker is required. In this review, we have highlighted recent advances in biosensors for circulating miRNA detection.

An accurate, practical, and low-cost method for predicting parturition is urgently needed in the dairy industry. The objective of this study was to evaluate changes in plasma progesterone concentration ([prog]) and glucose concentration in whole blood ([gluc]b) and plasma ([gluc]p) as predictors of parturition within 6, 12, and 24 h in primiparous and multiparous Holstein cows. Blood samples were obtained daily at approximately 0900 h from 34 primiparous and 72 multiparous Holstein cows in late gestation and the time of calving recorded to the nearest hour. Plasma [prog] was measured using an ELISA, and [gluc]b and [gluc]p using a low-cost point-of-care glucose meter. The optimal cut-point for predicting parturition was determined using binomial logistic regression with general estimating equations, because the data set consisted of repeated measures for each cow. Diagnostic test performance was evaluated by comparing the area under the receiver operating characteristic curve (AUC) and calculating the sensitivity, specificity, and κ at the optimal cut-point for predicting parturition. Plasma [prog] was the most accurate predictor of parturition within 24 h (AUC = 0.96) and 12 h (AUC = 0.93), whereas [gluc]b was the most accurate predictor of parturition within 6 h (primiparous, AUC = 0.96; multiparous, AUC = 0.86). We conclude that a decrease in plasma [prog] is currently the most accurate test for predicting calving within 24 h. Measurement of [gluc]b is a promising new test for the cow-side prediction of parturition in dairy cows due to its accuracy, practicality, and low cost.

We assessed the analytical performance of the Abbott IMxTM immunoassay analyser for total beta human chorionic gonadotropin (Beta hCG), follicle-stimulating hormone (FSH) and luteinising hormone (LH). The within-run CVs for various analyte concentrations were 2% to 6% while those for between-run imprecision in routine assay ranged from 4% to 10%. IMxTM values correlated well with radioimmunoassay for beta hCG, and immunoradiometric assay for FSH and LH; the correlation coefficients (r) were 0.97, 0.99 and 0.98 for total beta hCG, FSH and LH respectively. The average sensitivities were approximately 3.1, 0.2 and 0.5 iu/l for beta hCG, FSH and LH, respectively. Sample carry-over was not detected and there was negligible cross-reaction between LH and beta hCG in the respective assays. The automatic sample dilution protocol for beta hCG was superior to the manual procedure. The IMxTM is easy to operate and is able to process 24 samples in 40-45 minutes.

The tumour marker CA19-9 is a sensitive marker for pancreatic, gastric and hepatobiliary malignancies. High CA 19-9 level indicates unresectable lesions and a poor prognosis. The objective of the study was to determine the significance and implications of elevated CA 19-9 levels in the serum. A one-year retrospective review of all patients who had CA19-9 measured in our Medical Centre was undertaken; 69 patients were found to have CA 19-9 level above the cut-off value (37 U/ml). Thirty-six patients had malignant and the remaining 33 had benign lesions. CA 19-9 was found to be elevated in malignancies of pancreas, colorectum, lung, liver and ovary. Benign conditions associated with elevation of CA 19-9 included disease of the hepatobiliary system, pneumonia, pleural effusion, renal failure and SLE. In two individuals, there was no obvious cause for the elevation of this marker. CA 19-9 levels were significantly lower in benign than in malignant conditions. In conclusion, elevated CA 19-9 may be found in patients with benign as well as malignant disease. Therefore, it is important (1) that elevated levels of CA 19-9 are interpreted in the light of the clinical presentation of the patient and (2) to be aware of the benign conditions that can be associated with increased levels of this marker. With these factors in mind, CA 19-9 can be used to assist in the diagnosis of pancreatic cancer and assessment of resection adequacy post-operatively.

Blood gastrin and pepsinogen responses of native village goats in Malaysia to a single dose of 10,500 infective Haemonchus contortus larvae were investigated. Both blood values were significantly elevated within a week of infection and exhibited a highly significant correlation during the study. The magnitude of the blood gastrin response was, however, significantly greater than that of pepsinogen during the period that both blood values were elevated. It is suggested that blood gastrin assay may be of particular value in the diagnosis of chronic haemonchosis in animals harbouring relatively light worm burdens.

The finding of a prolonged bleeding time in a patient whose platelet count is normal suggests some abnormality in the function of the platelets. This situation may be due to an inherent platelet defect or to the deficiency of a plasma factor necessary for some aspect of platelet function. Defects of platelet function are also seen in a wide range of disease states and may be caused by many different classes of drugs. Simple methods are available to identify abnormalities in platelet numbers and/or function. Tests of platelet aggregation, using the aggregometer, are now widely used to investigate patients suspected of having platelet disorders. We observed in the last four years that thrombocythopenia is quite common amongst children and bleeding from platelet dysfunction, congenital or acquired, is not rare in Malaysia. This study was done to assess the prevalence of platelet abnormality in a section of the general 'healthy' population of school-going children and to study the possible causes of any abnormalities detected.

Postprandial changes in plasma total cholesterol (TC) are minimal and there is essentially no difference between fasting vs random TC concentrations, as reflected in the small diurnal coefficient of variation (CV) for TC of 2.5%. Similarly, a cholesterol-rich meal within the last 24 hours lacked an impact on plasma TC. Thus, random specimens are acceptable in blood cholesterol screening. The intraindividual biological CV (CVb) for plasma TC as measured over a long period was estimated from the data of several published studies to be 6.0%, which, when combined with a probable analytical CV (CVa) of 5% during screening, gave a total intraindividual CV (CVt) of about 8% for the single cholesterol assay. There is consensus that 'high TC values' acquired during screening should be confirmed under the conventional laboratory setting capable of CVa of 3% or less.

The effect of human normal serum (HNS) on Pseudomonas pseudomallei was determined. It is apparent from our data that the organism is resistant to the normal serum bactericidal mechanism. Ancillary experiments to confirm this serum-resistant property of P. pseudomallei were done by examining the effects of growth phase conditions of the bacteria (i.e., logarithmic and stationary phases) and different buffered systems used as diluent in our bactericidal assay. Results obtained showed similar degree of resistance to serum bactericidal killing by 5 strains of the organisms tested. The possible survival advantage of serum-resistance property to P. pseudomallei as bacterial pathogens known to invade the blood stream is discussed.

Skin biopsies from 100 patients with untreated lepromatous leprosy from Malaysia, India, Africa, and South America were examined with particular regard to pathological changes in intima, media, or adventitia of blood vessels and to the presence of leprosy bacilli in these layers. Bacilli were found in capillaries, venules, or arterioles in all cases, and in many instances they were present in endothelial lining cells or smooth muscle in large masses (globi). In several cases, solid-staining bacilli in endothelial lining cells were especially prominent. The findings are discussed in relation to a) the continuous bacteremia of lepromatous leprosy, b) the role of endothelial cells in phagocytosis, c) smooth muscle cells of the media as a site in which bacilli may persist, and d) the transmission of human leprosy by biting arthropods.
