    Hepatocellular carcinoma (HCC) is a malignant tumour of the hepatocyte. It is a common malignancy worldwide and causes almost half a million deaths annually. Asia is a high risk area. Although surgery (hepatectomy or liver transplantation) is the main form of curative treatment, the majority of patients are not eligible for surgery due to extent of tumour and dysfunction of liver. Radiopharmaceuticals used for transarterial treatment of HCC were Yttrium-90 microspheres, Iodine-131 lipiodol, Rhenium-188 lipiodol, and Holmium-166 Chitosan complex. Yittrium-90 microspheres are glass or resin microspheres of mean sphere diameter of 20 to 30 micrometre. The activity administered was about 4 GBq. Reported response rate was about 20%, and median survival was 54 weeks. On inoperable tumours, reported objective response of I-131 lipiodol was 40 to 70%, and median survival was six to nine months. It showed efficacy similar to TACE. In adjuvant treatment following curative resection of HCC, reported three year survival was 86% compared with 46% for the control group. The administered activity in both adjuvant and inoperable HCC was about 2 GBq (55 mCi). Rhenium-188 lipiodol is a new radioconjugate, and using it we treated 70 patients with inoperable HCC. This treatment was a part of a multi-centre trial sponsored by the International Atomic Energy Agency. Partial response was obtained in 17% of cases, while 49% had stable disease at three months, and 34% showed disease progression. In terms of survival, 19% survived one year, 60% for six months, and 90% for three months. The mean activity was about 4.6 GBq (124 mCi). This method was safe and free from adverse effects.

    BACKGROUND AND OBJECTIVE: 18F-Fluorocholine has been suggested as one of the reputable imaging tracers for diagnosis of prostate tumour in Positron Emission Tomography / Computed Tomography (PET/CT) modality. Nevertheless, it has never been synthesised in Malaysia. We acknowledged that the major problem with 18F-Fluorocholine is due to its relatively low radiochemical yield at the end of synthesis (EOS). Therefore, this article presents improved 18FFluorocholine radiochemical yields after carrying out optimisation on azeotropic drying of 18F-Fluorine.

    METHODS: In the previous study, the azeotropic drying of non-carrier-added (n.c.a) 18F-Fluorine in the reactor was conducted at atmospheric pressure (0 atm) and shorter duration time. In this study, however, the azeotropic drying of non-carried-added (n.c.a) 18FFluorine was made at a high vacuum pressure (- 0.65 to - 0.85 bar) with an additional time of 30 seconds. At the end of the synthesis, the mean radiochemical yield was statistically compared between the two azeotropic drying conditions so as to observe whether the improvement made was significant to the radiochemical yield.

    RESULTS: From the paired sample t-test analysis, the improvement done to the azeotropic drying of non-carrier-added (n.c.a) 18F-Fluorine was statistically significant (p < 0.05). With the improvement made, the 18F-Fluorcholine radiochemical yield was found to have increase by one fold.

    CONCLUSION: Improved 18F-Fluorocholine radiochemical yields were obtained after the improvement had been done to the azeotropic drying of non-carrier-added (n.c.a) 18F-Fluorine. It was also observed that improvement made to the azeotropic drying of non-carrier-added (n.c.a) 18F-Fluorine did not affect the 18F-Fluorocholine quality control analysis.

    A cylindrical gamma-ray 60Co source of activity alpha is predicted to produce an exposure rate X at a distance d in vacuum, given by X = gamma(T)(alpha/d2), where gamma(T) is the specific gamma-ray constant. It has been documented that this formula may be used to approximate X with an accuracy of 1% from a source of length l, provided that d/l > or = 5. It is shown that the formula is accurate to 0.1% under these conditions, provided that the distance is measured from the centre of the source. When absorption in the source and scattering in the collimator are considered, the position of the origin d = 0 can shift by a distance of the order of centimetres. Absorption in air between the source and the ionization chamber adds an exponential factor to the formula. It is shown that even when these modifications are included the discrepancy in the results, although generally less than 1%, is still large compared with the measurement errors. Some suggestions are made for the origin of this discrepancy.

    For the specific absorbed dose constant for 60Co photons, three values quoted directly in the literature and two derived indirectly from published information are reported. The three publications giving the direct values mentioned no medium of absorption, whereas the other two specify tissue. A database of the specific absorbed dose constant is generated for each of 14 media namely air, water, bone and 11 types of soft tissue. These values are consistent with the three directly quoted values plus one of the indirectly obtained values. Air is found to be unlikely as the medium for the first three; and appropriate media for these are suggested. For the other two values, the generated database suggests that one is too small to be accurate; while the other is correct for tissue (as stated in the publication). An apparent error of 10(3) is identified in one of the values directly quoted.

    Fish has been known as a source of nonoccupational mercury exposure to fish-consuming population groups. In this study, hair samples collected from fishermen and their families residing in an industrialized area in Penang and a nonindustrialized area in Terengganu were analyzed for mercury by neutron activation. The range, arithmetic mean, geometric mean, and median of the mercury concentrations for the groups in Penang and in Terengganu were 0.45-16.68, 3.61, 3.49, and 2.96 and 6.79-18.31, 12.08, 11.69, and 12.05 mg/kg, respectively. Somewhat lower values than from the Penang group were found in a group from Selangor consisting mainly of office workers. The group in Penang took about 40-100 g of fish/d, whereas the group in Terengganu consumed twice as much. This shows that hair mercury levels depend on a fish consumption pattern, and not on the location of the population. The levels of mercury found in this study were similar to those reported by other workers for fish-consuming population groups worldwide.

    To evaluate the effects of simple compensated fixed-dose iodine-131 therapy for thyrotoxicosis, the long-term results for 74 patients treated with a fixed dose of iodine-131 ranging from 5 to 12 mCi (185 to 444 MBq) were evaluated in the first 2 years of a trial. The dose selected was loosely based on the gross size of the thyroid gland. Routine antithyroid drug therapy was given for a minimum of 3 months after iodine-131 therapy. The mean (+/- SD) duration of follow-up was 74.5 +/- 42 months. The results indicated that roughly 25% of patients treated in this way will become hypothyroid after 5 years and that 85% are cured (need no further therapy during the follow-up period) using a single dose of iodine-131. Of those cured using a single iodine-131 dose, 81% were no longer receiving drugs after 6 months and 85% after 1 year. Such a regimen seems currently to be among the best available where prolonged periods of medication-free euthyroidism after therapy are sought.

    Batu Dam is of considerable importance to the metropolis of Kuala Lumpur, its existence and the quality assessment of its waters being essential in helping to maintain the lives of a large sector of the Malaysian population. Concerning the level of naturally occurring radioactivity contained within its waters, a well characterised HPGe γ-ray technique has been used in making measurements of the concentrations of primordial radionuclides in samples of surface water from the Dam. Based on the mean individual daily consumption of dam water, estimation has been made of the concomitant radiation dose. Activity concentrations, in units of Bq l-1, have been found to be in the range 2.4-3.2 for 226Ra, 1.1-1.3 for 232Th and 22.7-40.7 for 40K, in line with literature data for surface waters. The total annual ingestion dose for infants (<1 y) and adolescents (12-17 y) are found to be significant and greater than the World Health Organization recommended maximum dose of 0.1 mSv y-1 from the imbibing of drinking water. However, the Dam water does not pose a threat to public health, the Dam water not being used as the sole source of drinking water. Noting that this is the only known study of water from Batu Dam, the reported levels allow for evaluation of future changes in the natural radioactivity profile.

    Textitis is a new term used to refer to the degenerative-strain osteoarthritis that comes from excessive use of a smart phone. 18F-NaF is increasingly used in diagnosing skeletal pain that is not identified on radiographs. We report a case of a 26-y-old woman with left breast cancer referred for 18F-NaF PET/CT, who was complaining of right thumb and wrist pain. Findings were negative for bone secondaries. Dedicated hands views were acquired on a positron emission mammography scanner and showed focal uptake at the first carpometacarpal and second metacarpophalangeal joints. On the basis of the strong history, the findings were likely due to active arthritic changes caused by repetitive strain injury from excessive text messaging.

    The Jos Plateau has been reported to have elevated levels of natural background radiation. A few earlier studies have measured the levels of natural radioactivity for specific locations in the area. Our interest is to investigate how geology of the study area influences the activity concentrations of natural radionuclides. Thus, the activity concentrations of terrestrial radionuclides in soil samples collected across the geological formations of the Jos Plateau were determined by gamma spectrometry technique. The mean activity concentrations of 226Ra, 232Th and 40K were found to exceed their corresponding world reference values of 35, 40 and 400 Bq kg-1, respectively. Data were compared using statistical methods, analysis of variance (ANOVA) and post hoc tests. The results revealed in some instances significant influences of geological types on the activity concentrations in the area. The spatial distribution maps of activity concentrations of 226Ra, 232Th and 40K were geostatistically interpolated by ordinary Kriging method using ArcGIS software.

    We aimed to investigate the validity of the partition model (PM) in estimating the absorbed doses to liver tumour ([Formula: see text]), normal liver tissue ([Formula: see text]) and lungs ([Formula: see text]), when cross-fire irradiations between these compartments are being considered. MIRD-5 phantom incorporated with various treatment parameters, i.e. tumour involvement (TI), tumour-to-normal liver uptake ratio (T/N) and lung shunting (LS), were simulated using the Geant4 Monte Carlo (MC) toolkit. 108track histories were generated for each combination of the three parameters to obtain the absorbed dose per activity uptake in each compartment ([Formula: see text], [Formula: see text], and [Formula: see text]). The administered activities, A were estimated using PM, so as to achieve either limiting doses to normal liver, [Formula: see text] or lungs, [Formula: see text] (70 or 30 Gy, respectively). Using these administered activities, the activity uptake in each compartment ([Formula: see text], [Formula: see text], and [Formula: see text]) was estimated and multiplied with the absorbed dose per activity uptake attained using the MC simulations, to obtain the actual dose received by each compartment. PM overestimated [Formula: see text] by 11.7% in all cases, due to the escaped particles from the lungs. [Formula: see text] and [Formula: see text] by MC were largely affected by T/N, which were not considered by PM due to cross-fire exclusion at the tumour-normal liver boundary. These have resulted in the overestimation of [Formula: see text] by up to 8% and underestimation of [Formula: see text] by as high as  -78%, by PM. When [Formula: see text] was estimated via PM, the MC simulations showed significantly higher [Formula: see text] for cases with higher T/N, and LS  ⩽  10%. All [Formula: see text] and [Formula: see text] by MC were overestimated by PM, thus [Formula: see text] were never exceeded. PM leads to inaccurate dose estimations due to the exclusion of cross-fire irradiation, i.e. between the tumour and normal liver tissue. Caution should be taken for cases with higher TI and T/N, and lower LS, as they contribute to major underestimation of [Formula: see text]. For [Formula: see text], a different correction factor for dose calculation may be used for improved accuracy.

    This study entails the measurement of the specific activity of natural radionuclides (226Ra, 40K and 232Th) in 18 tooth samples obtained from the clinic of the Universiti Sains Malaysia (USM), Penang, by using an HPGe detector. The specific activity of 226Ra, 40K and 232Th was measured to estimate the hazard index of the radionuclides, radium equivalent activities (Raeq), external, internal hazard indices (Hex, Hin), and absorbed dose (Dout, Din). The maximum values of concentration of 226Ra, 232Th and 40K in the tooth samples were found to be 60.82, 60.29 and 594.22 Bq kg-1, respectively. Maximum values of Raeq, Hex, Hin, Dout and Din were found to be 192.78 Bq kg-1, 0.520, 0.685, 89.29 and 169.81 nGy h-1, Iγ and Iα as 0.702 and 0.304, respectively. The results were lower than the average world value (UNSCEAR). In addition, a strong correlation was found between the concentrations of 226Ra and Raeq, between energy and net area, as well as between radionuclides (226Ra, 40K and 232Th) in tooth samples and age of volunteers. This study showed that the concentrations and hazard indices of tooth samples are below the recommended safe levels; therefore, the study area is considered safe in terms of radiological health hazards.

    Uranium, thorium and potassium are the most abundant naturally occurring radioactive materials (NORMs) found in soils and other environmental media including foodstuffs. Since the human exposures to NORMs is an unavoidable phenomenon, in such a way that they can easily find their way to human being via food chain, detailed knowledge on their presence in foodstuffs is necessary to assess the radiation dose to the population. Thus, the present study concerns the assessment of natural radioactivity in maize, a staple foodstuff for Nigerian, via HPGe gamma-ray spectrometry. Activity concentrations (Bq/kg) in the maize samples were found to be in the range of 6.1 ± 0.6-8.2 ± 1.3, 2.2 ± 0.4-5.1 ± 0.7 and 288 ± 16-401 ± 24 for 226Ra, 232Th and 40K, respectively. Measured data for 226Ra and 232Th show below the world average values of 67 Bq/kg and 82 Bq/kg, respectively, while the activity of 40K exceeds the global average of 310 Bq/kg. The annual effective dose via the maize consumption was found to be far below the UNSCEAR recommended ingestion dose limit of 290 μSv/y, and the estimated lifetime cancer risk show lower than the ICRP (1991) cancer risk factor of 2.5 × 10-3 based on the additional annual dose limit of 1 mSv for general public, thus pose no adverse health risk to the Nigerian populace.

    Brachytherapy is commonly used in treatment of cervical, prostate, breast and skin cancers, also for oral cancers, typically via the application of sealed radioactive sources that are inserted within or alongside the area to be treated. A particular aim of the various brachytherapy techniques is to accurately transfer to the targeted tumour the largest possible dose, at the same time minimizing dose to the surrounding normal tissue, including organs at risk.     Study is made of the radioactivity in the beach sands of Langkawi island, a well-known tourist destination. Investigation is made of the relative presence of the naturally occurring radionuclide 40K and the natural-series indicator radionuclides 226Ra and 232Th, the gamma radiation exposure also being estimated. Sample quantities of black and white sand were collected for gamma ray spectrometry, yielding activity concentration in black sands of 226Ra, 232Th and 40K from 451±9 to 2411±65Bqkg-1 (mean of 1478Bqkg-1); 232±4 to 1272±35Bqkg-1 (mean of 718Bqkg-1) and 61±6 to 136±7Bqkg-1 (mean of 103Bqkg-1) respectively. Conversely, in white sands the respective values for 226Ra and 232Th were appreciably lower, at 8.3±0.5 to 13.7±1.4Bqkg-1 (mean of 9.8Bqkg-1) and 4.5±0.7 to 9.4±1.0Bqkg-1 (mean of 5.9Bqkg-1); 40K activities differed insubstantially from that in black sands, at 85±4 to 133±7Bqkg-1 with a mean of 102Bqkg-1. The mean activity concentrations of 226Ra and 232Th in black sands are comparable with that of high background areas elsewhere in the world. The heavy minerals content gives rise to elevated 226Ra and 232Th activity concentrations in all of black sand samples. Evaluation of the various radiological risk parameters points to values which in some cases could be in excess of recommendations providing for safe living and working. Statistical analysis examines correlations between the origins of the radionuclides, also identifying and classifying the radiological parameters. Present results may help to form an interest in rare-earth resources for the electronics industry, power generation and the viability of nuclear fuels cycle resources.

    Understanding the public awareness concerning the Lynas Advanced Material Plant (LAMP), an Australian rare earths processing plant located in Malaysia, a radiological study in soil and water samples collected at random surrounding the LAMP environment was undertaken using HPGe gamma-ray spectrometry. The mean soil activities for (226)Ra, (232)Th, and (40)K were found to be 6.56 ± 0.20, 10.62 ± 0.42, and 41.02 ± 0.67 Bq/kg, respectively, while for water samples were 0.33 ± 0.05, 0.18 ± 0.04, and 4.72 ± 0.29 Bq/l, respectively. The studied areas show typical local level of radioactivity from natural background radiation. The mean gamma absorbed dose rate in soils at 1 m above the ground was found to be 11.16 nGy/h. Assuming a 20 % outdoor occupancy factor, the corresponding annual effective dose showed a mean value of 0.014 mSv year(-1), significantly lower than the worldwide average value of 0.07 mSv year(-1) for the annual outdoor effective dose as reported by UNSCEAR (2000). Some other representative radiation indices such as activity utilization index (AUI), H ex, H in, excess lifetime cancer risk (ELCR), and annual gonadal dose equivalent (AGDE) were derived and also compared with the world average values. Statistical analysis performed on the obtained data showed a strong positive correlation between the radiological variables and (226)Ra and (232)Th.

    Computed tomography (CT) derived Monte Carlo (MC) phantoms allow dose determination within small animal models that is not feasible with in-vivo dosimetry. The aim of this study was to develop a CT-derived MC phantom generated from a mouse with a xenograft tumour that could then be used to calculate both the dose heterogeneity in the tumour volume and out of field scattered dose for pre-clinical small animal irradiation experiments. A BEAMnrc Monte-Carlo model has been built of our irradiation system that comprises a lead collimator with a 1 cm diameter aperture fitted to a Cs-137 gamma irradiator. The MC model of the irradiation system was validated by comparing the calculated dose results with dosimetric film measurement in a polymethyl methacrylate (PMMA) phantom using a 1D gamma-index analysis. Dose distributions in the MC mouse phantom were calculated and visualized on the CT-image data. Dose volume histograms (DVHs) were generated for the tumour and organs at risk (OARs). The effect of the xenographic tumour volume on the scattered out of field dose was also investigated. The defined gamma index analysis criteria were met, indicating that our MC simulation is a valid model for MC mouse phantom dose calculations. MC dose calculations showed a maximum out of field dose to the mouse of 7% of Dmax. Absorbed dose to the tumour varies in the range 60%-100% of Dmax. DVH analysis demonstrated that tumour received an inhomogeneous dose of 12 Gy-20 Gy (for 20 Gy prescribed dose) while out of field doses to all OARs were minimized (1.29 Gy-1.38 Gy). Variation of the xenographic tumour volume exhibited no significant effect on the out of field scattered dose to OARs. The CT derived MC mouse model presented here is a useful tool for tumour dose verifications as well as investigating the doses to normal tissue (in out of field) for preclinical radiobiological research.

    Coastal Mangroves are facing growing threats due to the harmful consequences of human activities. This first-ever detailed study of natural radioactivity in soil samples collected from seven tourist destinations within the Sundarbans, the world's largest mangrove forest, was conducted using HPGe gamma-ray spectrometry. Although the activity levels of 226Ra (11 ± 1-44 ± 4 Bq/kg) and 232Th (13 ± 1-68 ± 6 Bq/kg) generally align with global averages, the concentration of 40K (250 ± 20-630 ± 55 Bq/kg) was observed to surpass the worldwide average primarily due to factors like salinity intrusion, fertilizer application, agricultural runoff, which suggests the potential existence of potassium-rich mineral resources near the study sites. The assessment of the hazard parameters indicates that the majority of these parameters are within the recommended limits. The soil samples do not pose a significant radiological risk to the nearby population. The results of this study can establish important radiological baseline data before the Rooppur Nuclear Power Plant begins operating in Bangladesh.

    The concentrations of primordial radionuclides (226Ra, 232Th and 40K) in commonly used building materials (brick, cement and sand), the raw materials of cement and the by-products of coal-fired power plants (fly ash) collected from various manufacturers and suppliers in Bangladesh were determined via gamma-ray spectrometry using an HPGe detector. The results showed that the mean concentrations of 226Ra, 232Th and 40K in all studied samples slightly exceeded the typical world average values of 50 Bq kg(-1), 50 Bq kg(-1) and 500 Bq kg(-1), respectively. The activity concentrations (especially 226Ra) of fly-ash-containing cement in this study were found to be higher than those of fly-ash-free cement. To evaluate the potential radiological risk to individuals associated with these building materials, various radiological hazard indicators were calculated. The radium equivalent activity values for all samples were found to be lower than the recommended limit for building materials of 370 Bq kg(-1), with the exception of the fly ash. For most samples, the values of the alpha index and the radiological hazard (external and internal) indices were found to be within the safe limit of 1. The mean indoor absorbed dose rate was observed to be higher than the population-weighted world average of 84 nGy h(-1), and the corresponding annual effective dose for most samples fell below the recommended upper dose limit of 1 mSv y(-1). For all investigated materials, the values of the gamma index were found to be greater than 0.5 but less than 1, indicating that the gamma dose contribution from the studied building materials exceeds the exemption dose criterion of 0.3 mSv y(-1) but complies with the upper dose principle of 1 mSv y(-1).

    A 57-year-old woman received radioiodine therapy post total thyroidectomy for pT3aNxMx follicular thyroid carcinoma. Posttherapy 131I whole-body scan showed 131I concentration in the chest, mediastinum, and left upper thigh with stimulated thyroglobulin (Tg) of 89 μg/L. Subsequent radioiodine therapies showed persistent 131I accumulation in the anterior mediastinal soft tissue lesions and a hypodense segment VII liver lesion visualized on SPECT/CT, suggestive of iodine-avid metastatic disease despite the undetectable serum Tg (<1.0 μg/L) with no Tg antibody interference. Biopsy of the liver lesion revealed liver cyst, and consequent removal of the mediastinal lesions showed benign thymic cysts.
