    Many receiver-based Preamble Sampling Medium Access Control (PS-MAC) protocols have been proposed to provide better performance for variable traffic in a wireless sensor network (WSN). However, most of these protocols cannot prevent the occurrence of incorrect traffic convergence that causes the receiver node to wake-up more frequently than the transmitter node. In this research, a new protocol is proposed to prevent the problem mentioned above. The proposed mechanism has four components, and they are Initial control frame message, traffic estimation function, control frame message, and adaptive function. The initial control frame message is used to initiate the message transmission by the receiver node. The traffic estimation function is proposed to reduce the wake-up frequency of the receiver node by using the proposed traffic status register (TSR), idle listening times (ILTn, ILTk), and "number of wake-up without receiving beacon message" (NWwbm). The control frame message aims to supply the essential information to the receiver node to get the next wake-up-interval (WUI) time for the transmitter node using the proposed adaptive function. The proposed adaptive function is used by the receiver node to calculate the next WUI time of each of the transmitter nodes. Several simulations are conducted based on the benchmark protocols. The outcome of the simulation indicates that the proposed mechanism can prevent the incorrect traffic convergence problem that causes frequent wake-up of the receiver node compared to the transmitter node. Moreover, the simulation results also indicate that the proposed mechanism could reduce energy consumption, produce minor latency, improve the throughput, and produce higher packet delivery ratio compared to other related works.

    OBJECTIVE: Breath sound has information about underlying pathology and condition of subjects. The purpose of this study was to examine asthmatic acuteness levels (Mild, Moderate, Severe) using frequency features extracted from wheeze sounds. Further, analysis was extended to observe behaviour of wheeze sounds in different datasets.

    METHODS: Segmented and validated wheeze sounds was collected from 55 asthmatic patients from the trachea and lower lung base (LLB) during tidal breathing maneuvers. Segmented wheeze sounds have been grouped in to nine datasets based on auscultation location, breath phases and a combination of phase and location. Frequency based features F25, F50, F75, F90, F99 and mean frequency (MF) were calculated from normalized power spectrum. Subsequently, multivariate analysis was performed.

    RESULTS: Generally frequency features observe statistical significance (p < 0.05) for the majority of datasets to differentiate severity level Ʌ = 0.432-0.939, F(12, 196-1534) = 2.731-11.196, p < 0.05, ɳ2 = 0.061-0.568. It was observed that selected features performed better (higher effect size) for trachea related samples Ʌ = 0.432-0.620, F(12, 196-498) = 6.575-11.196, p < 0.05, ɳ2 = 0.386-0.568.

    CONCLUSIONS: The results demonstrated dthat severity levels of asthmatic patients with tidal breathing can be identified through computerized wheeze sound analysis. In general, auscultation location and breath phases produce wheeze sounds with different characteristics.

    The need for implementing reliable data transfer in resource-constrained cognitive radio ad hoc networks is still an open issue in the research community. Although geographical forwarding schemes are characterized by their low overhead and efficiency in reliable data transfer in traditional wireless sensor network, this potential is still yet to be utilized for viable routing options in resource-constrained cognitive radio ad hoc networks in the presence of lossy links. In this paper, a novel geographical forwarding technique that does not restrict the choice of the next hop to the nodes in the selected route is presented. This is achieved by the creation of virtual clusters based on spectrum correlation from which the next hop choice is made based on link quality. The design maximizes the use of idle listening and receiver contention prioritization for energy efficiency, the avoidance of routing hot spots and stability. The validation result, which closely follows the simulation result, shows that the developed scheme can make more advancement to the sink as against the usual decisions of relevant ad hoc on-demand distance vector route select operations, while ensuring channel quality. Further simulation results have shown the enhanced reliability, lower latency and energy efficiency of the presented scheme.

    BACKGROUND: Evidence underscores that empowerment is central to improve the elderly residents' quality of life. In truth, empowerment is a process through which individuals gain better control over their life. The aim of this study was to explore how perceived empowerment influence on the quality of life among elderly Malay residents.

    MATERIALS AND METHODS: A focus ethnographic approach was employed in a Malaysian residential home between May 2011 and January 2012. Data were gathered from participant observations, field notes, in-depth interviews, and exploring related documents.

    RESULTS: The analysis of the data gathered in the current study resulted in the development of three themes - social life and its requirements, caregivers' skills empowerment, and listening and supporting.

    CONCLUSIONS: Findings of the study provide new insights that are useful in charting new guideline for care providers and policy makers to improve the elderly residents' quality of life.

    Pneumonic pasteurellosis is an economically important infectious disease in the small ruminant industry which causes sudden death and loss for farmers. Nonetheless, this disease is still a common sight in sheep and goats in Malaysia, probably due to the unpopular usage of pasteurellosis vaccine or inappropriate vaccination practices. The aim of this study was designed to classify the severity of pneumonia via the establishment of auscultation scoring method and to quantify the acute phase proteins and heat shock proteins responses from vaccinated and non-vaccinated goats. Goat farms, consist of vaccinated and non-vaccinated farms, were selected in this study: where 15 clinically normal healthy goats and 9 pneumonic goats were selected from vaccinated farms whereas 15 clinically normal healthy goats and 31 pneumonic goats from non-vaccinated farms were selected for this study. Crackle lung sounds were not detected in both vaccinated and non-vaccinated normal goats.     An 8-year-old spayed female Shih Tzu was referred to University Veterinary Hospital (UVH) with history of chronic coughing for more than a year duration. Dry, hacking cough was heard and bilateral wheezing lung sound was noted upon physical examination. Auscultation of heart revealed left apical systolic heart murmur Grade III/VI. A soft, intermittent swelling was observed at ventral neck, cranial to thoracic inlet (protruded upon expiration and collapsed upon inspiration). Thoracic radiography taken showed presence of apical radiolucency at cervical region and bronchial pattern at caudodorsal lungs with left atrium enlargement and right-sided heart enlargement. Echocardiographic examination revealed myxomatous mitral valve disease (MMVD) with mild regurgitation. Based on diagnostic imaging, this case was diagnosed as dynamic lung lobe herniation secondary to chronic coughing concurrent with myxomatous mitral valve disease. Other differential diagnosis that may lead to chronic cough such as of tracheal collapsed and bronchiectasis was not rule out.

    An elderly gentleman with a known history of
    well-controlled hypertension presented with
    a three-week history of hoarseness associated
    with mild breathlessness. There was no episode
    of cyanosis, no noisy breathing, and no
    reduction in effort tolerance. There was also no
    history of chest pain or orthopnea. He denied
    any feeling of food stuck in his throat or chest,
    and he had no history of choking sensations
    during meals. He, however, was unable to
    count from 1 to 10 in one breath, and lung
    auscultation revealed reduced air entry on both

    One of the major causes of death all over the world is heart disease or cardiac dysfunction. These diseases could be identified easily with the variations in the sound produced due to the heart activity. These sophisticated auscultations need important clinical experience and concentrated listening skills. Therefore, there is an unmet need for a portable system for the early detection of cardiac illnesses. This paper proposes a prototype model of a smart digital-stethoscope system to monitor patient's heart sounds and diagnose any abnormality in a real-time manner. This system consists of two subsystems that communicate wirelessly using Bluetooth low energy technology: A portable digital stethoscope subsystem, and a computer-based decision-making subsystem. The portable subsystem captures the heart sounds of the patient, filters and digitizes, and sends the captured heart sounds to a personal computer wirelessly to visualize the heart sounds and for further processing to make a decision if the heart sounds are normal or abnormal. Twenty-seven t-domain, f-domain, and Mel frequency cepstral coefficients (MFCC) features were used to train a public database to identify the best-performing algorithm for classifying abnormal and normal heart sound (HS). The hyper parameter optimization, along with and without a feature reduction method, was tested to improve accuracy. The cost-adjusted optimized ensemble algorithm can produce 97% and 88% accuracy of classifying abnormal and normal HS, respectively.

    Running in high heat and humidity increases psychophysiological strain, which typically impairs running performance. Listening to synchronous music has been shown to provide psychophysiological benefits, which may enhance running performance. The present randomized, crossover study examined effects of listening to synchronous music on psychophysiological parameters and running performance in hot and humid conditions. Twelve male runners (21.7 ± 2.2 y; 166.17 ± 7.18 cm; 60.32 ± 9.52 kg; 59.29 ± 5.95 ml kg-1 min-1) completed two running trials in simulated conditions (31°C and 70% humidity) with and without synchronous music. Participants ran on a treadmill inside a climatic chamber for 60 min at 60%

    O2max. Time-to-exhaustion under the synchronous music condition was 66.59% longer (mean = 376.5 s vs. 226.0 s, p = 0.02, d = 0.63) compared to the no music condition. Ratings of perceived exertion were significantly lower for the synchronous music condition at each time point (15, 30, 45, and 60 min) of the steady state portion of the running trials. Small differences in heart rate were detected between conditions. No significant between-condition differences were found in urine specific gravity, percentage of body weight loss, thermal comfort, and blood lactate. Findings suggest that listening to synchronous music is beneficial to running performance and perceived exertion in hot and humid conditions.

    Introduction: Presentation of Dermatomyositis with Interstitial Lung Disease (ILD) is not uncommon. Case descrip-tion: A 50 years old lady with dyspnoea of 1 month. On examination patient had heliotrope rash around face with gottron papules and fine crepitations bibasally over lung auscultation with proximal weakness of the limbs. High Resolution Computer Tomography of Chest(HRCT of Chest) revealed ground glass appearance with fibrotic changes in both lungs. Blood test serum ANA 1:160, Anti MDA5 strong positive, Anti Ro 52 positive with Serum dsDNA nega-tive. Skin Biopsy-compatible with cutaneous manifestation of dermatomyositis. EMG revealed there is electrophysio-logical cause of myopathy. Lung function test was unable to perform due to severe pain. Investigate for the possibility of underlying malignancy were taken with Serum Tumour Markers, Nasoscopy, ultrasound of pelvis, OGDS and colonoscopy. Did not reveal lesions suspicious of malignancy. Patient treated by using tablet prednisolone 50mg once daily with tapering done and tablet azathioprine 100mg once daily. Planned for IV Methylprednisolone if wors-ening of disease. Patient passed away due to rapidly worsening ILD. Conclusion: Amyotrophic Dermatomyositis with interstitial lung disease(ILD) diagnosis can be confidently diagnosed with clinical correlation and Serum AntiMDA5.

    An 11-year-old intact male Shih Tzu dog was referred to the University Veterinary Hospital (UVH) with history of coughing and laboured breathing. Auscultation revealed that the dog had a left systolic heart murmur grade V/VI at the base of the heart. Radiography showed findings of cardiomegaly and pulmonary oedema. Echocardiography findings revealed that dog has a myxomatous mitral valve disease. The mitral valve was severely thickened and prolapsed into the left atrium. Congestive heart failure (CHF) was treated with an angiotensin converting enzyme (ACE) inhibitor and diuretic. An anti-mucolytic agent was prescribed as a symptomatic treatmentfor the coughing that could be due to mild bronchitis. Dog did not response well with treatment as the frequency of cough was not reduced and the duration was longer each time. Pimobendan was then added on to the initial treatment and there was a tremendous improvement seen from the patient after that. Episodes of productive cough were noticed greatly reduced and dog was much more active at home post two weeks of ACE inhibitor, diuretic and dual-acting inodilator. We successfully maintained the dog with minimal coughing episodes. However, the prognosis is moderate to poor in this case due to possibilities of refraction towards medications.
