    The principle of ferrule effect is of prime importance when restoring an endodontically treated tooth. A severely broken down tooth due to subgingival caries almost always end up with extraction as inadequate ferrule effect would compromise the predictability of restorative treatment. This clinical case report describes a treatment approach that combines non-surgical endodontic treatment, orthodontic extrusion and prosthetic rehabilitation to restore the function and aesthetic aspect of an extensively carious premolar with compromised prognosis. One year follow-up indicated stable periodontal health with evidence of periapical healing radiographically.

    OBJECTIVES: The aim of this study was to investigate the recovery of cuspal stiffness and fracture resistance in endodontically treated maxillary premolars restored with bonded ceramic inlays and onlays of various designs.
    METHODS: Seventy intact premolars were selected for this study; six cavity designs were investigated: (i) mesio-occlusal-distal (MOD) inlay (I), (ii) MOD inlay with palatal cusp coverage (IPC), (iii) MOD onlay (O), (iv) MOD inlay with pulp chamber extension (IPE), (v) MOD inlay with palatal cusp coverage and pulp chamber extension (IPCPE), and (vi) MOD onlay with pulp chamber extension (OPE). Intact teeth acted as control. Strain gauges were attached to the buccal and palatal surfaces of the teeth to measure cuspal stiffness under static loading. All specimens were eventually subjected to compressive load to failure. Cuspal stiffness and fracture resistance data were analyzed using ANOVA and Tukey test.
    RESULTS: The I and IPE restorations restored cuspal stiffness to 75% of the sound tooth value. The O and OPE restored teeth had stiffness values greater than that of a sound tooth. The I, IPC, O, IPE, IPCPE and OPE restored teeth demonstrated fracture strength values of 938N±113 N (s.d.), 1073N±176 N and 1317N±219 N, 893N±129 N, 1062N±153 N and 1347N±191 N respectively.
    CONCLUSIONS: Within the limitations of this study, it was concluded that the all-ceramic onlay or inlay with palatal cusp coverage provided best biomechanical advantage in restoring an endodontically treated maxillary premolar tooth.
    CLINICAL SIGNIFICANCE: The onlay approach which is more conservative compared to full coverage restoration is considered an appropriate approach to the restoration of endodontically treated maxillary premolars. The addition of a pulpal extension to the all-ceramic restorations, apart from being technically challenging, was not found to offer any biomechanical advantage to the restored teeth.

    Objective: Endodontically treated teeth may require posts for retaining the core and replacing the coronal structures that have been lost. The objective of this study was to evaluate and compare the push-out bond strength between different types of post cemented with different types of luting cement at different types of root level.

    Materials and Methods: In this in-vitro study, a total of 48 single-rooted permanent human teeth were decoronated, and the roots were treated endodontically. Following post space preparation, the sample was divided into four groups (n= 12 each) based on the types of post and cement. Two different types of post [GC everStick®POST (ES) and Parapost® Fiber LuxTM (PF)], and two different types of cement [G-CEMTM (G), and RelyXTM Unicem (R)] were used according to the manufacturer's instructions. All roots were sectioned at the coronal and middle thirds with a thickness of 3±0.1mm. The Push-out bond strength (PBS) test was performed using a universal testing machine at a cross-head speed of 0.5mm/ min. The bond strength values were recorded, and the data were analyzed using the SPSS program. Apart from descriptive statistics, three-way ANOVA was used for the interaction of the independent variables (post, cement, and root level). For differences between the groups, the Mann-Whitney U test was used. A P-value of less than 0.05 was considered significant for all analyses.

    Results: Push-out bond strength of samples at the middle level (11.38±10.31 MPa), with PF posts (11.18±9.98 MPa), and of those luted with RelyXTM Unicem cement (13.26±8.73 MPa) was higher than that of their counterparts. The PBS means of RelyXTM Unicem cement at both root levels were much higher than PBS means of G-CEMTM cement. Three-way ANOVA test revealed a significant effect for each variable with a higher effect of cement (Sum of Squares= 1310.690; P< 0.001). No significant difference (P= 0.153) was found between the coronal and middle parts and between ES and PF posts (P= 0.058). However, a highly significant difference (P< 0.001) was found between RelyXTM Unicem and G-CEMTM cements.

    Conclusion: The type of cement had a significant effect on push-out bond strength with RelyXTM Unicem which had higher values than G-CEMTM. However, the type of post and root level had no significant effect on PBS, although Parapost® Fiber LuxTM and middle root level had higher values than their counterparts.

    Restoring extensively damaged endodontically treated posterior teeth is always a challenge in dentistry. The use of endocrowns has gained popularity in restoring severely damaged endodontically treated teeth (ETT) in recent years. In this clinical report, a structurally compromised mandibular second molar with symptomatic irreversible pulpitis and normal apical tissue was endodontically treated. Surgical crown lengthening was attempted thereafter to increase the crown height. However, marginal periodontal tissue re-growth occurred after surgical crown lengthening. The tooth was subsequently restored with endocrown which was fabricated using computer-aided design and computer-aided manufacturing-based hybrid-ceramic. In conclusion, endocrown can be a viable restorative modality for ETT with compromised clinical crown height.

    Endodontically treated teeth are generally weaker than sound teeth. The study objective was to compare the fracture resistance of endodontically treated teeth restored with different restorative techniques. Fifty extracted human maxillary central incisors of similar size were divided into five groups of 10 teeth. Group 1 was left intact as the control group. Other groups (Groups 2, 3, 4 and 5) were all endodontically treated followed by restorations using different restorative techniques; light cured composite resin (CR), CR and crown, post and CR, and post-CR core and crown respectively. The specimens were loaded in a universal testing machine with a static force at a crosshead speed of 0.5mm/min at 135 ° to the long axis of the root until failure. The means and the standard deviations of the maximum load at failure for groups 1, 2, 3, 4 and 5 were 1259.11N (379.12N), 578.63N (196.70N), 667.13N (298.72N), 1247.65N (294.48N) and 623.60N (193.75N) respectively.     Objectives: The aim of this study is to determine the success rate of the endodontically treated teeth in patients attending the Polyclinic, Kulliyyah of Dentistry, International Islamic University Malaysia (IIUM), from 2012 to 2015.

    Materials and Methods: A retrospective study involved endodontically treated teeth of patients attending the Polyclinic, Kulliyyah of Dentistry, IIUM, from 2012 to 2015. Clinical and radiographic data were recorded and classified as successful or failed, and further analyzed by Fisher's exact test to measure the correlation between the variables using SPSS software version 16.0. Kappa test was used to measure the overall relationship between clinical and radiographic findings.

    Results: A total of sixty teeth were evaluated clinically and radiographically, the overall success rate was 85% (n = 51). Correlation between the variables showed nonsignificant (P > 0.05) in the success rate among age, gender, and race, upper and lower arches and between anterior and posterior teeth at the time of treatment. At postendodontic fixed restorations, the variables showed statistically significant relationship with the success rate (P < 0.05).

    Conclusions: Patients with no signs and symptoms and with no radiographical changes at the the time of clinical examination, showed the highest percentage of success rate (85%) of postendodontic fixed restorations. Age, gender, and race have no significant relations with the success rate of endodontically treated teeth.

    PURPOSE: To investigate the level and distribution of stresses in endodontically treated maxillary premolar teeth restored using various cavity designs of bonded all-ceramic restorations. The hypothesis tested was that the various all-ceramic approaches, including incorporating a pulp chamber extension in the restoration, had no influence on the stresses in the restored tooth unit.
    METHODS: Finite element packages Patran and Abaqus were used for the stress analysis. The cavity designs investigated include: (1) inlay (I); (2) inlay with palatal cusp coverage (IPC); (3) onlay (O); (4) inlay with pulp chamber extension (IPE); (5) inlay with palatal cusp coverage and pulp chamber extension (IPCPE); and (6) onlay with pulp chamber extension (OPE).
    RESULTS: In each case, tensile stresses were found to be concentrated subjacent to the occlusal fossa. Peak tensile stress and peak shear stress values along the tooth/restoration interface for IPC, O IPCPE and OPE cavity designs were found to be associated with the axiogingival line angle. Overall, the order of the various forms of restoration investigated in terms of the maximum principal stress (from greatest to lowest) was as follows: IPE > IPCPE > OPE > I > IPC > O.

    The introduction of an intraradicular composite reinforcing technique, in conjunction with the reestablishment of matching post canal spaces, has allowed compromised, root-filled teeth to be restored with functional, esthetic post crowns. This clinical case report suggests that reconstituted post canals, in accurately adapting to passive, parallel-sided, matching, and well-fitting posts, can enhance the retention of post crowns. Other factors of clinical importance relating to the resin-reinforced technique are discussed, including fracture resistance, depth of polymerization, dentin adhesion, polymerization shrinkage, and coronal microleakage.

    AIM: The purpose of this study was to obtain data on current approaches for the restoration of endodontically treated teeth among general dental practitioners (GDPs) in Manchester, United Kingdom.

    METHOD: A questionnaire containing 19 single-answer, multiple-choice type questions was mailed to 503 GDPs practising in the Greater Manchester area in January 2002. An explanatory covering letter and a stamped addressed return envelope were enclosed. The data obtained were processed using SPSS statistical software.

    RESULTS: Three hundred and fifty-one (70%) of the practitioners responded to the questionnaire. The restoration of root-filled teeth was normally undertaken within 1-2 weeks of completing root canal therapy by 63% of the practitioners. Only 35% of the GDPs used posts routinely in the restoration of root-filled anterior teeth; the corresponding figure for posterior teeth was 15%. While a cast, precious metal post was the preferred choice in the restoration of anterior teeth, the use of prefabricated posts and related techniques predominated in the restoration of posterior teeth. Composite resin was the most popular choice of material for core build-up procedures in anterior teeth. Amalgam tended to be favoured for core build-ups in posterior teeth. The majority of the practitioners (56%) routinely restored root-filled anterior teeth by means of porcelain-fused-to-metal crowns. Seventy-three per cent of the GDPs preferred to restore root-filled posterior teeth by means of a full veneer crown.

    CONCLUSIONS: The results of this study suggest that the practitioners surveyed had a sound understanding of the principles involved in the restoration of endodontically treated teeth, with the possible exception of the need to establish a durable coronal seal as soon as possible after the placement of a root filling.

    PURPOSE: To evaluate fracture resistance and failure mode of endodontically-treated mandibular molars restored with different intra-radicular techniques.

    METHODS: 75 human mandibular molars were randomly divided into five equal groups. Teeth were standardized, endodontically-treated and restored according the assigned group as follows: amalgam core only, prefabricated titanium post in the distal canal and amalgam core, composite core only; fiber post in the distal canal and composite core. One group of untreated sound teeth was used as a control. Non-precious metal crowns were fabricated and cemented on the prepared specimens with Rely X U200 resin cement. All specimens were subjected to a compressive load at crosshead speed 0.5 mm/minute, 25° to the long axis of the tooth. Failure loads and modes were recorded.

    RESULTS: Mean failure loads among the groups were significantly different (P= 0.035). Post-hoc multiple pair-wise comparisons revealed the amalgam core and composite core groups produced significantly lower fracture resistance than the control group (P= 0.041 and P= 0.025, respectively) and no significant differences among the different intra-radicular techniques (P> 0.05). The composite core with fiber post and amalgam core with titanium posts showed the highest percentage of favorable failures (67%) and non-favorable failures (87%) respectively.

    CLINICAL SIGNIFICANCE: The composite core with fiber post is the most appropriate intraradicular restoration in cases of severely compromised molars.

    Existing literature suggests a relationship between the amount of remaining tooth structure and the fracture resistance of the restored endodontically treated tooth. This study investigated the amount of tooth structure remaining following various tooth preparations used in the restoration of the endodontically treated maxillary second premolar. Illustrations of the maxillary second premolar in buccopalatal, mesiodistal and occlusal sections were drawn to scale. Outlines of various intra- and extracoronal preparations were superim-posed on the illustrations to reveal the amount of tooth tissue remaining in each case. Preparations for a ceramic inlay, inlay with palatal cusp coverage and onlay left 2.0-2.5mm of tooth structure buccally and palatally. Following preparation for a metal-ceramic crown, approximately 1.0mm of tooth structure remained buccally, and between 1.6mm-1.8mm palatally. Preparation for an all-ceramic crown was observed to leave 1.0mm-1.2mm of tooth structure surrounding what remained of the endodontic access cavity. It was concluded that decisions as to the type of definitive restoration to restore the endodontically treated maxillary second premolar may be influenced, amongst other factors, by information on the amount of tooth tissue remaining following preparation.

    Revascularization endodontic procedures commonly require multiple treatment sessions. However, single visit procedures may be advantageous from the clinical and patient management standpoints. The purpose of this review was to evaluate the outcomes of single-visit revascularization endodontic procedures for the management of immature permanent teeth with non-vital pulp. Two electronic databases (Scopus and PubMed) were searched, from their inception to July 2018, for studies that assessed clinical and/or radiographic and/or histologic outcomes of single-visit revascularization endodontic procedures performed in immature permanent teeth with non-vital pulp. Case reports, animal studies and clinical trials were included. The quality of case reports was appraised by Joanna Briggs Institute Critical Appraisal Checklist for Case Reports. The quality of randomized clinical trials was assessed by the revised Cochrane risk of bias tool for randomized trials. The risk of bias for animal studies was assessed using SYRCLE's risk of bias tool. Tabulation followed by narrative synthesis was used to draw conclusions. Seven studies satisfied the selection criteria. Five were classified as case reports, one as a randomized clinical trial and one as animal study. The latter presented with a high risk of bias, whilst the remaining showed a low risk. The evidence supporting the potential use of single-visit revascularization endodontic procedures is scarce. Successful single-visit revascularization endodontic procedures commonly include the use of high concentrations of sodium hypochlorite and EDTA combined with the use of agitation systems. Further clinical trials with long term follow up are needed to confirm the results of the current review.

    BACKGROUND: There is no sufficient literature on the effect of post location on endodontically treated premolar teeth with 2 roots.

    OBJECTIVES: The aim of the study was to evaluate the effect of fiber post location on fracture resistance and failure mode of endodontically treated premolars with 2 roots.

    MATERIAL AND METHODS: Fifty extracted maxillary first premolars with 2 roots were divided randomly into 5 groups. Group 1 was comprised of sound teeth, which received only metal crowns (control). Teeth from groups 2, 3, 4, and 5 were decoronated 2 mm above the cementoenamel junction (CEJ) and were endodontically treated. No post was placed in group 2 teeth. Teeth from groups 3, 4 and 5 were given a fiber post placed in the buccal canal, palatal canal, and both buccal and palatal canals, respectively. All teeth in groups 2, 3, 4, and 5 were built up with composite and full coverage metal crowns. A compressive static load was applied at an angle of 25° to the crowns with a crosshead speed of 0.5 mm/min, until fracture.

    RESULTS: One-way analysis of variance (ANOVA) showed significant differences among the groups (p = 0.002). A post hoc test showed significantly lower fracture resistance of group 4 compared to group 5 (p = 0.011). Furthermore, group 2 had significantly less fracture resistance compared to group 1 (p = 0.021) and group 5 (p = 0.002). According to Fisher's exact test, different post locations are non-significantly associated with fracture mode (p = 0.256).

    CONCLUSIONS: Fiber post location has a significant effect on fracture resistance of severely damaged, endodontically treated maxillary premolars with 2 roots. However, post placement in the palatal root is preferred, as it maintains the restorability of the tooth.

    The aim of this study was to investigate the simultaneous influence of various dental posts and cementation materials on the fracture resistance and failure mode of the endodontically-treated teeth. Sixty endodontically treated upper central incisors were randomly divided into two main groups, each consisted of three subgroups restored with titanium, fiber and stainless steel posts. The posts in the first and second groups were luted with zinc phosphate and composite resin cements, respectively. Composite cores were built-up over the specimens and then retained with nickel-chromium crowns. Specimens were thermocycled and then loaded at 135o until failures were observed. The obtained data of fracture resistances and failure modes were analyzed using Two-way ANOVA and the Chi-Square tests, respectively. The results showed that the zinc phosphate cement resulted in relatively higher fracture resistances. However, luting of dental posts with composite resin provided more restorable failures in endodontically-treated teeth. Moreover, the teeth restored by fiber posts exhibited desirable fracture resistances with more restorable failure modes, compared with those restored by titanium or stainless steel posts.

    BACKGROUND: This study aimed to evaluate the load capacity of maxillary central incisors with simulated flared root canal restored with different fiber-reinforced composite (FRC) post cemented with either self-adhesive or self-etch resin cement and its mode of fracture.

    METHODS: Sixty-five extracted maxillary incisors were decoronated, its canal was artificially flared and randomly categorized into group tFRC (tapered FRC post) (n = 22), mFRC (multi-FRC post) (n = 21), and DIS-FRC (direct individually shaped-FRC (DIS-FRC) post) (n = 22), which were further subdivided based on cementation resin. The posts were cemented and a standardized resin core was constructed. After thermocycling, the samples were loaded statically and the maximum load was recorded.

    RESULTS: The load capacity of the maxillary central incisor was influenced by the different FRC post system and not the resin cement (p = 0.289), and no significant interaction was found between them. Group mFRC (522.9N) yielded a significantly higher load capacity compared to DIS-FRC (421.1N). Overall, a 55% favorable fracture pattern was observed, and this was not statistically significant.

    CONCLUSION: Within the limitation of the study, it can be concluded that prefabricated FRC posts outperform DIS-FRC posts in terms of the load capacity of a maxillary central incisor with a simulated flared root canal. The cementation methods whether a self-adhesive or self-etch resin cement, was not demonstrated to influence the load capacity of a maxillary central incisor with a flared root canal. There were no significant differences between the favorable and non-favorable fracture when FRC post systems were used to restored a maxillary central incisor with a flared root canal.

    This case report demonstrates sequential periodontic, orthodontic and prosthodontic treatment modalities to save and restore deep horizontally fractured maxillary central incisor. The location of fracture was deep in the mucosa which reveals less than 2 mm of tooth structure to receive the crown. The procedures like surgical crown lengthening, endodontic post placement, orthodontic forced eruption, core build-up and metal-ceramic crown restoration were sequentially performed to conserve the fractured tooth. Forced eruption is preferred to surgical removal of supporting alveolar bone, since forced eruption preserves the biologic width, maintains esthetics, and at the same time exposes sound tooth structure for the placement of restorative margins.
