    Rainfall-runoff information is critical for water resource and river basin management. Runoff can be estimated by using two methods; gauged method (direct measurement) and ungauged method (indirect formula and equation). The in-situ measurement provides real-time and accurate yet required time-consuming operation and inaccessibility topography. Therefore, the runoff estimation modelling and equation was developed to overcome the limitation of in-situ measurement. SCS-CN is a simple model of ungauged method, where runoff volume (Q) resulting from rainfall (P) is formulated using equation of (Q= (P-Ia) 2 / (P-Ia + S). It was known as the best technique to be adopted for large basin study where time and manpower also accessibility are limited. SCS-CN method also is widely use in prediction software as it taken into consideration of the effects of soil, properties, land cover and antecedent moisture. Curve Number is well developed in USA for the agriculture purpose with many investigations to validate and calibrate the values of curve number. It was applied in numerous river basins in temperate and other regions e.g. US, Argentina, India, China, South Korea, Palestine and Malaysia. However, the reliability of the CN in the tropics is doubtable due to different land use characteristics, soil type, climate, geological features and rainfall pattern and variability. Based on the reviewed conceptual and applications of SCS-CN in temperate and tropics, numerous studies found the SCS-CN method is reliable and practical for runoff estimation in tropics region.

    Widespread applications of phenol in manufacturing industries and oil refineries had resulted in unprecedented leakage of phenol into the environment, which can cause serious health effects such as tissue necrosis and cardiac arrhythmia upon contact or ingestion. Plants exposed to phenol had reduced seed germination index, inhibited growth or even fatality. There are many technologies currently practised to remediate phenol pollution such as physiochemical methods (adsorption to activated carbon and chemical oxidation), biological methods (biodegradation by bacteria or fungus, and soil bioaugmentation), and phytoremediation method (using hairy roots of plants). As physiochemical and microbial phenol degradation are destructive and costly, phytoremediation is widely studied as an alternative phenol remediator which is environmental friendly and cost effective. Microorganisms can detoxify the aromatic xenobiotic through the aerobic or anaerobic pathway. Aerobic degradation of phenol is through either the meta- or ortho-pathway of catechol cleavage while anaerobic degradation occurs through the benzoate pathway. In plants, degradation of phenol is also through catechol cleavage as in microorganisms. However, different enzyme systems were utilised in the different pathways involved.

    Diabetes mellitus has a high prevalence in Malaysia and is expected to rise in the future due to lifestyle changes. Thus, consumers are turning to alternative methods in the prevention and cure of the disease. The Momordica charantia has been studied for its anti-diabetic activity both in vitro and in vivo and is shown to be effective in inhibiting glucose absorption. Therefore, the MC is used as a main ingredient in the development of health beverages to offer alternatives for patients or the health conscious. The microbial quality of the product is examined to ensure the safety of the product and to find methods to enhance its shelf life.

    Herbicides are inevitable inputs to control excessive weed in crop land, particularly where modern agricultural practices such as conservation tillage, are opted. Intensive farming has increased the market value of herbicides among the other pesticides. Although herbicides are effective in controlling weed population, administration of this synthetic chemicals may alter the soil microbial community causing potential increase of plant pathogens. Moreover, herbicides may also have nontarget effects on the cultivated crops making them more susceptible to diseases. Actions of herbicides in soil that either stimulate microbial growth or wipe out some microbial population may create space for the thrivial of opportunistic fungi. Previous studies showed that white rot fungi are more tolerant to herbicides as they produce lignin degrading enzymes that are highly oxidative, non-specific and are able to transform a wide range of herbicides. Besides that, this group of fungi can grow on agricultural waste substrates. Influence of these herbicides on soil microbial ecosystem and interactions of plants and pathogenic white rot fungi modulate disease development in plant hosts.

    Porous ceramic components with decently controlled porosity offers remarkable advantages in industrial and structural applications such as fluid filtration, thermal insulation and scaffolds for bone tissue engineering. In this review study of porous ceramic components, requisite processing techniques necessary for the development of porous ceramics with imbued microstructural model intended for a specific application. An appraisal of the fabrication was made with respect to their economic viability wherein cost effective methods having great potentials in decently controlling the pore network imbued within the host ceramic matrix was preferred over the capital intensive counterparts.

    Recently research in creative leadership has ballooned to encompass the essential qualities in organizational leaders (Petrie, 2014). In this study, Generativity Theory was utilized to explore the concept of creativity in leaders. Epstein (1999) presented four main core skills and abilities that assist people in expressing creativity individually which are capturing, challenging, broadening and surrounding. Model of high performing school by Shannon and Bylsma (2007) was utilized to measure school effectiveness. The purpose of this study is to determine teacher’s perceptions related to principals’ creative leadership practices towards school effectiveness in secondary schools in Klang District. Simple random sampling was used to select ten secondary schools from three different clusters of schools in the Klang District. A total of 250 teachers from these ten schools participated in this study. The study used self-administered questionnaires. The data are collected and analyzed using descriptive statistics (mean, standard deviation) and inferential statistics (Pearson correlation and Multiple Regressions). This research endeavoured to answer the research questions through employing quantitative research method. This study discovered that the level of school effectiveness as practiced by secondary school teachers was overall high level. On the other hand, it was concluded that the level of creative leadership practiced by school leaders is at moderate level. This study also found out a moderate correlation between school principals’ creative leadership practice and school effectiveness (r = .617, p < .01).     Intraductal Papillary Mucinous Neoplasm (IPMN) of the pancreas in association with Autosomal Dominant Polycystic Kidney Disease (ADPKD) is extremely rare, even though 10% of ADPKD patients may develop simple pancreatic cyst. The first case report was published by Yasunori Sato from Japan in 2009. Since then less than 10 case reports are available worldwide to describe about this condition. We reported such a rare case of a 67-year-old man with ADPKD who was referred to our centre because of chronic abdominal pain and diagnosed as IPMN based on the serial imaging procedures. Despite of the high risk comorbidities, he successfully underwent pylorus preserving total pancreaticoduodenectomy with splenectomy.

    Enterobacter gergoviae is a gram negative rod-shaped opportunistic organism reported to cause urinary and respiratory tract infections, but peritonitis caused by this organism is unknown. We report a case of 50-year-old patient on peritoneal dialysis (PD) presented with Enterobacter gergoviae peritonitis with septic shock. Despite Intraperitoneal (IP) cloxacillin 250mg qid and IP ceftazidime 1gram q24h and subsequent escalation with IP amikacin 2mg/kg q24h and IP vancomycin 15mg/kg q24h within the next 48 hours, his peritonitis remained refractory and required catheter removal. Although Enterobacter gergoviae is naturally sensitive to aminoglycosides, carbapenems and quinolones, it reacts differently to the beta lactam antibiotics. Their resistance to third-generation cephalosporins is fast emerging and treatment with third-generation cephalosporins may cause AmpC-overproducing mutants. The majority of
    Enterobacteriaceae, including Extended-spectrum beta-lactamases producers, remain susceptible to carbapenems. Our report provides an unfavourable course of E. gergoviae peritonitis likely due to acquired secondary drug resistance during the therapy period.

    Multiple myeloma is a blood dyscrasias that accounts of almost 10% of all hematological malignancy. The presentation of myeloma kidney is highly variable and it often presents as renal insufficiency, renal tubular dysfunction and proteinuria of various types. In Malaysia the true incidence of myeloma kidney is unknown. Often the diagnosis of myeloma kidney was missed out despite the patient has sought medical treatment early. A high index of suspicion is required when the middle to elderly age patients present with unexplained renal impairment and enlarged kidneys. We present here the presentation of a rare subtype of myeloma in a relatively young patient whereby the patient presented with nephrotic syndrome and aoztemia.

    Free light chains (FLCs) are tumour markers of monoclonal gammopathies. Detection of urinary FLC or also known as Bence-Jones protein through urinary protein and its immunofixation electrophoreses (UPE and uIFE, respectively) have been considered the gold standard for its biochemical diagnosis. This is mainly due to their superior detection limits compared to their counterpart investigations in serum. However, urinalysis is limited in many ways. The emergence of serum FLC assay with markedly improved detection limit circumvents many of these problems and has gained much importance in biochemical investigations of monoclonal gammopathies. Nevertheless, they are not without limitations. This review discusses the advantages and limitations of serum and urinary FLC assays.

    Introduction: Microbial keratitis (MK) is an important cause for corneal blindness and understanding its risk factors enable us to improve management and minimise its complications. Methods: In this retrospective case review, medical records of all patients treated for MK from 2006 to 2013 was analysed to know the socio-demography, preceding risk factors, clinical characteristics, causative organisms and final visual outcome. Results: A total of 174 patients (180 eyes) were included in this study. Identifiable ocular risk factors included contact lens (CL) usage (85, 47.2%), ocular trauma (50, 27.8%), ocular surface disease (21, 11.6%), steroid use (6, 3.3%) and immuno-compromy (19, 10.5%).     Introduction: Self-harm is an important global health concern as it is a strong predictor and risk factor for completed suicide. This paper is aimed to estimate the incidence rate of self-harm and to describe the profiles of people who self-harmed in 2011. Methods: This is a descriptive study based on secondary data retrieved from the Ministry of Health (MOH) of Malaysia hospitals’ discharge records from the 1st of January to 31st of December 2011. Results: The overall self-harm rate was 16.6 per 100 000 population (n= 4 815). Adults in the state of Johor had the highest rate of self-harm. There were more female self-harmers within the peak age group of 20-29 years. Indians, Muslims and those who were unmarried had the highest rates of self-harm. Male mostly used unspecified chemical (45.89%) while female used non-opioid analgesics (24.79%). Conclusions: In 2011, the rate of self-harm was 16.6 per 100 000 population with slightly higher rate observed among adult females compared to males and twice as high among girls than boys. Less lethal methods of self-harm were employed.

    Introduction: Suspicious adnexal masses need to be investigated thoroughly as it may represent ovarian cancer, which is the fourth most common gynaecological cancer in Malaysia. Conventional cross sectional imaging may reveal non-specific findings, thus lead to unnecessary biopsies. 18F-Fluorodeoxyglucose positron emission tomography/ computed tomography (18F-FDG PET/CT) has emerged as a useful tool, for characterization of indeterminate adnexal masses. Most studies have been conducted in Western population, and little information is available in Asian population in general and Malaysian population in particular. Methods: Prospective study of women with suspicious adnexal masses, referred to the Centre for Nuclear Diagnostic Imaging, Universiti Putra Malaysia to undergo pre-operative whole-body contrast-enhanced 18F-FDG PET/CT scans from January 2014 to January 2016. Subjects underwent Contrast-Enhanced Computed Tomography (CECT) scans followed by positron emission tomography (PET) scans using a hybrid scanner. Two radiologists analyzed the CECT and PET/CT images by consensus; blinded to the HPE results. Then the PET/CT findings were correlated with HPE results as the gold standard. Results: 11 whole-body PET/CT scans and 18 adnexal masses (12 HPE-proven malignant lesions and 6 benign lesions) were analyzed. The sensitivity, specificity, PPV, and NPV of CECT alone compared to PET/CT was 91.7%, 50.0%, 78.6%, and 75.0% vs. 91.7%, 100%, 100% and 85.7% respectively. Conclusions: Improved diagnostic accuracy for characterizing benign and malignant adnexal masses can be achieved using contrast-enhanced 18F-FDG PET/CT, making it a potential investigation of choice which can help in treatment planning.

    Introduction: Open globe injury may lead to devastating visual outcome. Urgent management and subsequent follow-up is important to maximise recovery. More data on open globe injury in Malaysia is needed to identify risk groups and prognostic factors. This study was carried out to investigate open globe injury, the socio-epidemic profile, clinical characteristics and outcome of the open globe injury in the Hospital Serdang, Malaysia. Methods: All patients managed for open globe injury to Hospital Serdang from January 2006 to December 2013 were included in this retrospective case review. Student’s T-test was used to determine difference between means and chi-square for categorical data. P value of less than 0.05 was regarded as statistically significant. Results: There were 155 patients managed for open globe injury with three of them had bilateral involvement. As such, there were 158 cases eligible for analysis in this study. The average age was 32.2 ± 16.5 years (mean ± standard deviation, SD). Male was at higher risk for open globe injury.     Introduction: Osteoarthritis (OA) is the main cause of knee pain. It also affects individual’s physical functioning. Anti- inflammatory drugs and knee replacement are the mainstay methods in the management of knee OA in Malaysia. However, patients with knee OA often suffer pain. The general objective of the study is to evaluate the effectiveness of a cognitive behavioural intervention module on knee pain, functional disability and psychological outcomes among knee OA patients attending Orthopedics Clinics in Hospital Putrajaya and Hospital Serdang, Malaysia. This study aims to determine the baseline level of knee pain and functional disability among knee OA patients.
    Methods: Baseline results on the knee pain and functional disability were obtained from a two arm parallel- group randomized controlled study. Three hundred patients aged 35 to 75 years diagnosed with knee OA were recruited. A set of pre tested and validated Knee injury and Osteoarthritis Outcome Score (KOOS) questionnaire was used in this study.
    Results: Mean baseline of KOOS pain was 56.09 (SD=21.75) and 52.26 (SD=22.08) for the intervention and control groups respectively. Participants in the intervention and control groups had a mean KOOS function in daily living of 61.11 (SD=21.20) and 56.72 (SD=22.13) respectively. Overall mean baseline of KOOS function in sport and recreation was 35.30 (SD=27.38).
    Conclusions: Majority of participants had moderate level of knee pain and functional disability in daily living. However, participants had more extreme symptoms of functional disability in sport and recreation. Therefore, interventions to reduce knee pain and functional disability symptoms in knee OA are needed.

    Introduction: The Hedgehog (Hh) signalling pathway is a developmental signalling pathway involved in normal mammalian developmental and homeostasis of adult renewable tissues. In most adult tissues, this pathway remains silent and previous studies have shown that constitutive activation of Hedgehog signalling pathway leads to various types of malignancies including medulloblastomas, basal cell carcinoma, gastrointestinal, breast and prostate cancer. The purpose of this study was to investigate the immunohistochemical expression of Hedgehog pathway proteins in Diffuse Large B-cell Lymphoma and determine their association with overall survival (OS). Methods: Positive control using normal tonsils were included in each batch of immunohistochemical staining procedure. Results: PTCH1 proteins were highly expressed in DLBCL and showed strong staining intensity in 107 (100%) cases and SMO proteins were expressed in 105 (98.1%) cases. PTCH1 proteins were localised in the nucleus of tumour cells, whereas SMO proteins were mainly localised in the cytoplasm of tumour cells. Positive expression of PTCH1 and SMO proteins and overall survival of DLBCL patients were correlated with age, gender, race and tumour location. There was no significant correlation between the expression of these two proteins with any of the parameters. PTCH1 expression showed significant association with SMO expression (P=0.03). Conclusions: Our findings suggest that high expression of both PTCH1 and SMO may be important in the pathogenesis of DLBCL. However, additional mechanisms that may contribute to the activation of HH signalling in DLBCL needs to be further explored.

    Introduction: The high amounts of polyunsaturated fatty acid (PUFA) in kenaf seed oil (KSO) is susceptible to oxidation. However, KSO is rapidly oxidised due to its high PUFA content.

    Methods: In order to prevent oxidation, kenaf seed oil was encapsulated using a co-extrusion technique to produce microencapsulated kenaf seed oil (MKSO). The shell materials used were formulated from alginate with high methoxy pectin and chitosan. MKSO was freeze-dried and in vitro behaviour of MKSO was investigated and compared with oven-dried MKSO. After in vitro digestion, the antioxidant and bioactive compounds in freeze-dried MKSO were investigated.

    Results: Freeze-dried MKSO released more oil (95.35%) compared to oven-dried MKSO (83.88%) after in vitro digestion. Total phenolic content (TPC) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) radical scavenging activities showed decreases, compared to before in vitro digestion while 2,2’-azino-bis(3- ethylbenzothiazoline-6-sulphonic acid) (ABTS) radical-scavenging assay and tocopherol content showed increases in released oil from MKSO after in vitro digestion compared to before in vitro digestion. For phytosterol composition, the decreases in released oil were not significant.

    Conclusion: The studies showed that microencapsulation allowed for sitecontrolled oil delivery and protected the bioactive compounds.

    Introduction: This study aimed to investigate the effect of saffron extract and crocin on blood biomarkers associated with obesity using the rat model.

    Methods: Obesity was induced by feeding a high-fat diet to 42 male Sprague-Dawley rats for 12 weeks, after which they were equally distributed into seven groups. Three groups served as controls namely, normal diet (ND), high-fat diet (HFD), and high-fat diet plus orlistat (HFD + ORL), while the remaining four treatment groups consisted of HFD added low or high dose (40 and 80 mg/kg/day) of either saffron extract or crocin in the food. At the end of 8 weeks, blood samples were collected by cardiac puncture for biochemical analysis.

    Results: Obese rats treated with a high dose of saffron extract and crocin showed significantly lower plasma glucose levels (5.26 and 5.67 mmol/L respectively) than the HFD rats (6.92 mmol/L). Saffron extract and crocin at a high dose showed significantly lower levels of plasma insulin (3.97 and 3.88 ng/mL respctively) compared to HFD control (5.41 ng/mL). Adiponectin levels significantly increased in obese rats fed saffron extract and crocin at high doses (7.44 and 7.92 µg/mL respectively) compared to HFD control (5.34 µg/mL). Ghrelin level significantly increased from 419.10 to 284.10 pg/mL,while leptin level significantly decreased from 8.08 to 5.68 ng/mL for the high dose crocin groups compared to HFD control. No significant differences in plasma serotonin levels were found among the groups.

    Conclusion: Saffron extract and crocin show potential in reducing blood biomarkers associated with obesity as well as anti-inflammatory and regulatory potential of adipocytokines in an animal model.

    Introduction: Functional constipation is a widespread gastrointestinal ailment associated with infrequent defecation, unfavorably affecting quality of life. However, it has not been well studied worldwide. This study aimed to investigate the effects of black bean vinegar (black bean and black bean yeast) consumption in alleviating functional constipation.

    Methods: A randomised, double-blind, placebo-controlled, parallel group study involving 70 adults with functional constipation was conducted in Peninsular Malaysia from May till July 2016. Subjects aged 18-60 years diagnosed with a score of more than 15 based on the Constipation Scoring System were included. A third-party-controller assigned black bean vinegar and placebo drinks to two groups with 1:1 ratio using randomisation software. Participants received once-daily doses of black bean vinegar (20 ml daily) or fruit-flavored squash placebo (20 ml daily) which contained vitamins, minerals, trace elements and carbohydrates, without black bean and black bean yeast, for four weeks. Analysis was conducted on an on-treatment basis (i.e., all randomised participants conformed to the study protocol by consuming 20 ml of drinks daily, with less than 10% leftover drinks at post-intervention assessment).     Introduction: By the end of 2015, about 72,152 children with learning disabilities were registered under the Malaysian Welfare Service Department (JKM). Malnutrition has been found to be a common setback among children with learning disability (LD). This study presents available evidence on the prevalence and risk factors associated with malnutrition in children with LD.

    Methods: A framework suggested by Arksey & O`Male (2005) was used to carry out this scoping review. Published articles, reviews and reports were identified through a complete search. Inclusion criteria for the search were English articles related to LD, published from 2005 to 2016.

    Results: Seventeen international studies published from 2005 until 2015 with a total of 318,596 participants and one study involving 281 participants from Malaysia, were identified and included in this review (n=18). The target age range of the sample in these 18 studies was 2 - 20 years, with a mean age of 3.2 - 14.2 years. The prevalence of underweight among children with LD was 3.4 - 36%, overweight 7.6 - 37% and obesity 5.7 - 52%. Several studies reveal that malnutrition risk among children with LD is significantly associated with gender, age, genetic syndrome, type of disability, medication used, and country economic status.

    Conclusion: A number of studies show that children with LD have a higher prevalence of being overweight and obese than typically developing children and the risk associated with obesity significantly increases with age.
