    Workplace violence become a global concern and perceived as a very serious safety and health hazard especially in healthcare setting. The aim of this study was to determine the prevalence of workplace violence and its associated risk factors among healthcare workers in public hospital. Methods: This was a cross-sectional study which involved 136 randomly selected respondents among doctors and nurses in a public hospital in Kuala Lumpur. Data was gathered through a self-administered questionnaire consisted of two standardized questionnaire; Workplace Violence (WPV) and Job Content Questionnaire (JCQ-27). Results: The response rate was 91%. The prevalence of reported WPV was 71.3% where nurses (73.2%) had slightly higher prevalence than doctors (69.2%). The most common forms of WPV was verbal abuse (70.6%), followed by bullying/mobbing (29.4%), physical violence (11.0%), and sexual harassment (6.6%). The perpetrators were mostly among relatives of patients and visitors followed by the patients. Multiple logistic regression shows that respondents working in Accident and Emergency (A&E) Department was 17 times more likely to report workplace violence than those working in Pediatric Department. Also, for every 1 year younger, respondents were 5 times more likely to experience workplace violence controlling for other factors. Conclusion: The prevalence of workplace violent among respondents were high and most common among young workers, especially nurses and those working in A&E Department. Hence, further assessment should be carried out to reduce the identified risk factors and to find ways of solving this issue.

    Flood is a natural disaster cause by heavy rainfall and high tide phenomenon. In the event of flood, substantial amount of wastes tend to be moved away and washed with mud that cause the task of waste recovering extremely challenging. The objective of the study was to assess flood victims participation, duration and major problem arise during post flood waste clean-up activity together with respondent’s knowledge and psychological importance in the recovery process. Methods: A cross sectional survey using interview and self-administered questionnaire was conducted involving 150 flood victims in Kuala Krai, Kelantan, who were heavily affected by the recent flood. Results: Respondents participation in the clean-up process was high (N = 126, 84%). Average 1 to 3 months were taken for the clean-up activity. Majority of the respondents had low and moderate satisfaction on the cleaning service provided by the authority (N = 84, 56%). Major problem arises from the past flood waste clean up activity was the biased waste removal process (N = 124, 83%), victims have to find alternative ways for disposal (N = 108, 72%) and the temporary disposal site were located near to the housing area that creates discomfort (N = 105, 70%). The correlation test obtained significant relationship between knowledge and education level (X2 = 0.203, p=0.013). But no significant relationship was obtained between knowledge with gender, age, monthly income, duration of clean-up process, respondents participation and satisfaction. Conclusion: Disaster debris management and disposal is critically important to support the victims in their recovery process.

    Leptospirosis is an infectious and zoonotic disease caused by pathogenic bacteria called leptospires which can be transmitted through the exposure to the urine or excretion of infected animals, and also water or soil that being contaminated with secretion of the infected animals. Leptospirosis remains a concern in Malaysia, as the number of cases had steadily increased. This study aims to determine the knowledge, attitude and practice (KAP) among final year students of selected programmes in Universiti Putra Malaysia (UPM). Methods: A cross sectional study was conducted among students of Environmental and Occupational Health (ENVOCH) (57 students), Nursing (22 students) and Veterinary Medicine (97 students).     Electronic Cigarette (EC) usage has been gaining acceptance in Malaysia despite its lack of analytical evidence on the chemical constituents of its liquid formulations. This study aims to evaluate the chemical concentrations of nicotine, propylene glycol (PG) and selected Tobacco-Specific Nitrosamines (TSNAs); 4-(methylnitrosamino)-1-(3-pyridyl)-1-butanone (NNK) and N’-nitrosonornicotine (NNN) in e-liquids locally sourced from the Malaysian market. Methods: A total of 17 e-liquids from a variety of flavours and brands were purchased from local EC retailers in Klang Valley, Malaysia. Nicotine and PG concentrations were assessed using Gas Chromatography-Flame Ionization Detector (GC-FID) while NNK and NNN were quantified using Liquid Chromatography-Tandem Mass Spectrometry (LC-MS/MS). The concentrations of nicotine and PG (mg/mL) were described in comparison with the levels indicated on the labels when present while levels of TSNAs were descriptively explained. Results: Nicotine was detected in all e-liquid samples, despite several samples being declared as nicotine-free. The average (standard deviation) level of nicotine, PG, NNN and NNK were 3.26 (1.04) mg/mL, 484.10 (98.24) mg/mL, 0.383(0.288) μg/L and 0.086 (0.057) μg/L, respectively. Labelling discrepancies (when indicated on the label) of nicotine and PG were between the range of 27%-73% and 3%-63%, respectively. Conclusion: The concentrations of nicotine and PG in local e-liquids were varied. There were evidences of labelling discrepancy in that local e-liquids. TSNAs were detected in all samples of e-liquids. This study brought forth strong evidence on the need for the implementation of regulation on e-liquid manufacturing and sales, particularly on the accuracy of labelling and licensing to protect the public health.

    Mental health in policing has been widely studied but incidence of mental health illnesses among them keep increasing. This study aimed to analyse generic and specific work stressors among police officers and their differences between urban and sub-urban police population. Methods: This was a cross-sectional study conducted in Kuala Lumpur and in nine sub-urban provinces in Pahang and Negeri Sembilan which involved 328 traffic police officers recruited by universal sampling. Data was collected by using self-administered questionnaire consisted of Police Stress Questionnaire (PSQ), the Job Content Questionnaire (JCQ), the Work Family Conflicts (WFC), the International Union against Tuberculosis and Lung Disease (IUATLD), and the General Health Questionnaire (GHQ-12). Results: The response rate was 71.30%. Majority of respondents were male (86.59%) with average age of 38.84 years old. The prevalence of probable mental health illnesses was 29.80% in sub-urban and 44.30% in urban. Job demand, role ambiguity, family to work conflicts and almost all specific works stressors were significantly higher among urban respondents.     Cement industry contributes to the major constituent of airborne dust in the atmosphere. This study aims to determine the level of respirable cement dust exposure associated with workers’ respiratory health. A cross-sectional study was carried out among 84 administration workers as the comparative group and 84 manufacturing workers as the exposed group. Method: A set of validated questionnaires was used to obtain some pertained background information as well as respiratory symptoms among the respondents. Personal Air Sampling Pump was used for assessing personal exposure towards cement dust in 8 hours. For lung function performance, a Spirometry test was carried out and Fractional Exhaled Nitric Oxide (FENO) test was conducted to assess airway inflammation. Results: The median for personal exposure level to respirable dust of manufacturing workers was 2.68 (5.90) mg/m3 with range 0.351 to 10.60 mg/m3. The lung function abnormality among the manufacturing workers was FVC% (PR=3.82, 95% CI=1.52-9.58) and FEV1% (PR=5.16, 95% CI=1.65-16.10). Cough was reported to occur the most likely among the manufacturing workers (PR=2.40, 95% CI=1.12-5.15). After adjusting the smoking status, the prevalence of phlegm and cough were 35.7% and 29.8% respectively. 16.7% of manufacturing workers recorded a high level of FENO. The increasing exposure to respirable dust significantly reduces the FVC% of manufacturing workers (r=-0.36, p=0.05). Conclusion: The personal exposure to respirable cement dust increases the risk of lung impairment by highly developing respiratory health symptoms, reducing lung function level and increasing the higher level of airway inflammation among highly exposed workers.

    The production of ammonia has been increasing over the past few years. Unfortunately, the production does not follow the safety control of ammonia on workers. Indonesia still adopts chemical standard from other countries. Therefore, it requires an ammonia standard at the highest dose without effect or no observed adverse effect level (NOAEL) in the workplace. This research aims to determine standard at the highest dose of without effect through the expression of CD8 cells as well as analysis of histological alteration CD8 lymphocyte between exposed to ammonia group and control. Methods: The study was a laboratory experimental research with a post-test only control group design. The research used Rattus novergicus species as many as 24. NOAEL was determined by middle dose with a location between the smallest and the largest dose. The doses of ammonia were given through inhalation. The histological alteration of CD8 between ammonia in exposed and the control group were analyzed by using the Kruskal Wallis test. Results: NOAEL was found through CD8 located in group 3 with 0.0154 dose mg/kg body weight. There was a differential expression of CD8 lymphocyte cells in the white mice lung between exposed to ammonia group and control (p=0.042). Conclusion: The expression of CD8 lymphocyte cells in the white mice lung exposed to ammonia differs significantly with the number of the expression of CD8 lymphocyte cells in white mice lung at control group. NOAEL was 0.0154 mg/kg body weight of white mice.

    This study aimed to identify the exposure levels of traffic air pollutants specifically PM10, benzene, toluene, and xylene (BTX) among traffic policemen and the risks to their respiratory health. A cross-sectional comparative study was conducted among 42 traffic policemen and 42 desk-bound policemen as the exposed and comparative groups respectively. Methods: The questionnaire adapted from the American Thoracic Society for Adult Respiratory Health Disease (ATS-DLD) to obtain socio-demographic and respiratory symptoms data. A spirometer (Chestgraph Hi-105) was used to perform lung function test. A personal air sampling pump was used to measure the personal exposure level to PM10. A Ppbrae 3000 was used to measure the outdoor and indoor concentration of BTX during morning and afternoon peak hours respectively. Results: The mean personal exposure level of PM10 among the traffic policemen was 150.14 ± 130.66 μg/m3 compared to only 84.14 ± 94.11 μg/m3 in the comparative group. The short exposures to BTX at the roadsides were found to be slightly higher in the afternoons than in the mornings. Indoor offices air concentrations were only detectable for benzene while the mornings and afternoons values for toluene and xylene were below the detection limits. A median concentration of benzene documented significantly higher at the selected of sampling roadsides areas (median=0.157 ppm) than indoor office areas (median=0.071 ppm). Conclusion: The respiratory symptoms were significantly higher in the exposed group compared to the comparative group which they were 3.9, 4.1, and 3.5 times more likely to develop cough, wheezing, and breathlessness respectively.

    Air pollutants that possessed genotoxic properties have the potential to induce genetic damage. Micronuclei (MN) frequency is used as an indicator for identifying potential genotoxic exposures. A comparative cross-sectional study was carried out among primary school children in a petrochemical industrial area (N=111, Kemaman) and a rural (N=65, Dungun) area in Terengganu. Methods: Validated questionnaires were distributed to obtain the respondents’ socio-demographic data, previous exposure and reported respiratory illness. The frequency of micronuclei was assessed in collected buccal mucosa samples of children. The air monitoring was also carried out at 6 selected schools. Results: Results from the statistical analysis carried out showed significant differences with p=0.001 for all parameters assessed between areas, which included ultrafine particles, UFP (z = -4.842), PM2.5 (z = -10.392), PM10 (z= -11.074) NO2 (z = -11.868), SO2 (z = -5.667), relative humidity (z = -5.587). The MN frequency was statistically significant with PM2.5 (χ2= 17.78, p=0.001) and PM10 (χ2= 15.429, p =0.001). The statistical analysis also showed a significant association between UFP and coughing (PR=2.965, 95% CI=1.069-8.225). The multiple logistic regression analysis showed that the main pollutants influencing MN frequencies were UFP and NO2 with UFP (PR=1.877, 95%CI= 1.174-3.002) and NO2 (PR=1.008, 95%CI= 1.001-1.015). Conclusion: This study demonstrated that exposure to air pollutants may increase the risk of respiratory illness and may induce MN formation among children.

    Extensive agriculture activities in the upstream area causes pollutants flow to the downstream area and contaminate the drinking water resources. Methods: Water samples were collected from 27 sampling points of Bertam River in Cameron Highlands, during wet season in September 2017 for physicochemical properties and heavy metals analyses. Potential health risk was calculated based on the heavy metals concentration detected. Results: The pH value of the river ranged from neutral to slightly acidic (6.15-7.01). The overall mean level of turbidity (109.94 ± 160.73 NTU), DO (7.86 ± 0.71 mg/L), E. coli (5191.00 ± 14937.42 CFU/100 mL), and NH3-N (0.85 ± 0.54 mg/L) were exceeded the National Standard for Drinking Water Quality (NSDWQ) (Ministry of Health Malaysia). The concentration of heavy metals were in the following order; Fe>Al>Cu>Zn>Pb>Cd. The overall mean concentration of Cd (0.015 ± 0.007 mg/L), Fe (0.442 ± 0.191 mg/L) and Pb (0.021 ± 0.005 mg/L) were exceeded the national standard (NSDWQ). The non-carcinogenic health risk for adult and children from daily water consumption was within an acceptable risk. There is a potential cancer risk through Cd exposure in the drinking water where the risk higher among children (4.92 x 10-3), followed by adult male (3.06 x 10-3) and adult female (2.98 x 10-3). Conclusion: Significant heavy metals contamination and health risk through water consumption were observed in the highland river watershed, possibly due to the anthropogenic activities.

    The repeatedly heating cooking oil in preparation of fried food has become a main dietary habit among Malaysian which can cause health impacts on humans. The peroxide value (PV) can be applied to identify cooking oil’s quality through the oxidative change that takes place in fats or oils. High peroxide value indicates bad quality of cooking oil. This research was conducted to identify the awareness and practice of repeatedly heating cooking oil among food premise operators and to determine the peroxide value in repeatedly heating cooking oil samples. Methods: A total of 124 food premise operators in Bukit Mertajam, Penang, participated in this research. A face-to-face interview was carried out by using the structured questionnaire for data collection to obtain socio-demographics information as well as awareness and practice of repeatedly heating cooking oil among respondents. The peroxide values analysis was performed on five samples of respondent’s most favourable brands of cooking oil by using iodometric titration method. Results: Majority of respondents had moderate awareness (53.2%) and practice (50.0%) level regarding repeatedly heating cooking oil. The PV analysis showed that the peroxide value begin to exceed the AOCS standard limit of 10 meqO2/kg following the 5th cycles of frying. Conclusion: Relevant actions need to be taken by the governmental food authorities to address this issue and ensure safe consumption of fried foods by consumers.

    Depressive disorders are common. As compared to the general population, healthcare personnel are hypothesized to have an above than average risk to develop this condition. The aim of this study is to assess the prevalence of this condition and its contributing factors amongst primary healthcare personnel.
    Methods: A cross-sectional study was conducted by universally sampling 179 primary healthcare personnel at nine primary healthcare centers across Kuala Lumpur and Selangor in May 2015. Depressive symptoms were screened using self-administered Patient Health Questionnaire-9 (PHQ-9) questionnaire. Socio-demographic and work profile data were also assessed.
    Results: 92.7% of the 179 personnel agreed to participate. Near half of the respondents were staff nurses (49%), followed by doctors (22%), assistant medical officers (10%), attendants (10%) and the remaining were pharmacists, dieticians, and laboratory technicians (9%). Depression was found to be present in 38% of the participants with one-third found to have moderate to severe depression. Bivariate analysis show that males (p=0.043), assistant medical officers (p=0.048), and working more than 10 hours per day (p=0.019) are significant risk factors. Further analysis by logistic regression shows that working more than 10 hours per day increases the odds of depression by 3.1 (OR 3.1, 95% CI 1.1-8.7, p=0.03).
    Conclusions: A high prevalence of depression was found within the healthcare personnel population at the primary healthcare centres studied. Being a male, employed as an assistant medical officer, and, prolonged hours at the workplace is a significant risk factor for depression.

    Traffic policemen are exposed to polluted air for a long time in high concentrations since they are working outside for most of the time which is bad for their health. Hence, this study was to determine the prevalence of respiratory symptoms and pulmonary function among traffic police in Kuala Lumpur and Johor Bahru. Methods: A pulmonary function test using spirometer was used to measure the pulmonary function of subjects. A questionnaire on respiratory symptoms translated version from IUALTD was used. The questionnaire includes background data, occupational and health history. Results: The traffic policemen were determined as having lower lung function parameters; low FVC% predicted (89.6%) and low FEV1% predicted (94%) due to their nature of work and the environment. Coughing was present at the highest (33.6%) among them, whereas wheezing was found the least (15.7%) of the workers. Conclusions: Findings from this study, indicated that there is a development of respiratory diseases and deterioration of lung function among traffic policemen. These baseline data can serve as a reference to the top management of traffic police officers in order to develop an occupational safety and health guideline for police officers as they are not covered by Occupational Safety and Health Act (OSHA, Act 514 1994).

    Bisphenol A (BPA) is a controversial plastics ingredient used mainly in the production of polycarbonate plastics (PC) and epoxy resins that widely used nowadays in food and drink packaging. Even though BPA is not involved in polyethylene terephthalate (PET) manufacturing, recent study had reported the present of BPA in PET water bottle. This study was conducted to investigate effects storage conditions on release of BPA from PC and PET bottled water as well as to assess health risks associated with consumption. Methods: Solid phase extraction (SPE) was used to extract the samples, followed by analysis using ultra high performance liquid chromatography with fluorescence detector (UHPLC-FLD). The possibility of developing chronic non-carcinogenic health risk among consumers of bottled water was evaluated using hazard quotient (HQ). Results: Results showed that BPA migrated from PC and PET water bottles at concentrations ranging from 9.13 to 257.67 ng/L and 11.53 ng/L to 269.87 ng/L respectively. Concentrations of BPA were higher in PET bottled water compared to PC bottled water across all storage conditions. Higher storage temperature and longer storage duration increased BPA concentrations in PC and PET bottled water. Concentrations of BPA in bottled water which were kept in a car and were exposed to sunlight were higher than control samples which were stored indoor at room temperature. Conclusion: No significant chronic non-carcinogenic health risks were calculated for daily ingestion of BPA-contaminated bottled water; calculated HQ was less than one.

    Calcium Carbide (CaC2) use for fruit ripening become dubious in recent years when various health related issues began to arise. This study aims to assess knowledge, attitude and practises (KAP) and health related to CaC2 among mango farmers, staff/workers working in mango farms and fruits traders in Perak, Kedah and Perlis. Methods: All potential respondents in the study locations was invited to participate. Yes-no questions were used to assess KAP while structured questions were used to obtain socio-demographic data through face-to face interview. Results: The response rate for this study was 67.8% (n = 200). All respondents were male with the average age of 40.45 years old. Majority of them (67.5%) completed secondary school as their highest education level. Overall, less than half of the total respondents have appropriate KAP. Only job tenure significantly associated with the knowledge on the type of suitable PPE (χ2 = 16.421). Respondents with secondary or tertiary school of education level (χ2= 9.154) and those with 1-5 years of job tenure (χ2 = 10.932) showed significantly better attitude on CaC2 handling. Also, the total scores of knowledge significantly associated with attitude (χ2 = 26.801) and application of glove (χ2= 12.551). In multiple logistic regression only respondents with the knowledge on type of PPE (OR = 6.30) will persist to use CaC2. Conclusion: The KAP related to CaC2 among respondents is low and their knowledge was significantly associated with their attitude and practice. Therefore, effective training and communication approach is warranted.

    Introduction: The Penan people are largely settled in rural and remote areas of
    Sarawak with high rates of undernutrition among the children. The study aimed to
    determine the nutritional status and infant and young child feeding (IYCF) practices
    of Penan children. Methods: Subjects consisted of 121 children, aged between 0-23
    months, from 15 Penan longhouses in Belaga district. Malnutrition was assessed
    using anthropometric measurements of children and categorised according to the
    World Health Organization (WHO) Growth Standards (2006). Feeding practices were
    assessed using questionnaires based on WHO IYCF Indicators (2008). Results:
    The prevalence of underweight was 29.8%; stunting 43.0% and wasting 5.8%.
    Prevalence of exclusive breastfeeding under 6 months was 44.4%. About 86% of
    infants aged 6-8 months were already given solid, semi-solid and soft foods. The
    proportion of children aged 6-23 months achieving minimum dietary diversity
    (MDD) was 76.6%, while minimum meal frequency (MMF) was 83.0% (breastfed and
    non-breastfed). Among the children who achieved MDD, 64.9% of their meals were
    derived from 4-5 food groups. More than half (55.3%) of all subjects (breastfed and
    non-breastfed) received minimum acceptable diet (MAD). Achievement of iron-rich
    foods (IRF) indicator was 77.7%. There was no significant association between the
    IYCF indicators of MDD, MMF and MAD and underweight, stunting and wasting.
    Conclusion: A high prevalence of poor nutritional status was recorded among
    the Penan children. While the IYCF indicators apparently showed satisfactory
    complementary feeding practices, future studies should assess quantitatively the

    Dengue is a neglected disease caused by the infection of dengue virus which is transmitted
    by Aedes mosquitoes and to some, it could be fatal. Regardless of the enormous work devoted to
    research for the treatment of dengue, to this day there is no cure, and treatment is solely limited to
    supportive care by treating the symptoms. The inhibition of the viral RNA non-structural enzymes
    has been the most popular approach amongst the strategies applied to the search and development
    of dengue antivirals. This review is a compact digest of what is already known of the roles and the
    prospects of the dengue virus non-structural proteins NS1, NS2BNS3, NS4A, NS4B and NS5 as the
    targets for antiviral studies including the recent progress that has been published regarding their
    roles.

    Background: Many questionnaires have been developed to measure dental anxiety and
    fear. Among them is the Index of Dental Anxiety and Fear Scale (IDAF-4C+), which consists of a
    dental anxiety and fear module (IDAF-4C), a phobia module (IDAF-P) and a stimulus module
    (IDAF-S). The objective of this research was to report the adaptation and validation of the IDAF4C+ for Malaysian secondary school children.
    Methods: This was a cross-sectional validation study. The original English version of the
    IDAF-4C+ was translated into Malay, back-translated, and then sent for content validation via an
    expert validation and face validation by the target student population. Three hundred and seventy
    questionnaires were then distributed among 16-year-old school children. Confirmatory factor
    analysis (CFA) was conducted for the IDAF-4C module using a bootstrapped maximum likelihood
    estimator. Spearman’s rank correlation was used to assess the relationship between the IDAF-S
    and IDAF-4C modules. Intraclass correlation (ICC) was used to determine the stability of the
    IDAF-S and IDAF-4C modules, while kappa values were used for the IDAF-P module.
    Results: The response rate was 86.5% for CFA and 76.9% for stability. CFA showed the
    existence of only one factor with a reliability estimate of 0.921, obtained via Raykov’s procedure.
    All items in the IDAF-S module were significantly correlated with the IDAF-4C module (P <
    0.001). The IDAF-S and IDAF-4C modules were stable, as determined via a two-way mixed model
    with absolute agreement, a single measure and a Case 3 ICC (A, 1). The IDAF-P module showed
    satisfactory stability, as assessed via kappa values.
    Conclusion: The Malay version of the IDAF-4C+ is valid and reliable in measuring dental
    anxiety and fear among Malaysian secondary school children.

    Hypervirulent Klebsiella pneumoniae strain is a major cause of liver abscess and this bacteria has metastatic properties. This distinct liver abscess syndrome has been increasingly reported in Asia over the past two decades, but is emerging as a global disease. We described an 81-year-old lady, who presented to the emergency department with 1-week prior duration of fever and right eye swelling. She had been admitted for recurrent liver abscess prior to this. Hence, she was initially treated as sepsis secondary to recurrent liver abscess. Meanwhile, she was managed concurrently by ophthalmology team for endophtalmitis. Despite the initiation of treatment with antibiotics, she did not show any improvement and required right eye evisceration to treat the sepsis. Invasive liver abscess syndrome with metastatic endophthalmitis should be screened in patient with Klebsiella liver abscess. They should be monitored for ocular symptoms as early recognition can prohibit delays in treatment, which has debilitating consequences.

    Microperforate hymen is a rare vaginal anomaly that is usually reported in childhood due to recurrent urogenital infections. Unlike imperforate hymen where the presenting complaints are classical due to complete vaginal obstruction, the less profound and varied presentation of microperforate hymen may go unnoticed. We report a case of a 39-year-old, with a background history of amenorrhea, who presented with acute abdomen suggestive of tubo-ovarian abscess, and was finally diagnosed to have microperforate hymen. She underwent hymenectomy to correct the anomaly. To our knowledge, this is the oldest age of presentation of a congenital form of microperforate hymen. A high index of suspicion and early detection of microperforate hymen and properly timed intervention is essential to prevent acute symptoms and long term detrimental sequalae to women’s reproductive and psychosexual health.
