    Manipulating intracellular signals by interaction with transmembranal G-protein-coupled receptors (GPCRs) is the way of action of more than 30% of available medicines. Designing molecules against GPCRs is most challenging due to their flexible binding orthosteric and allosteric pockets, a property that lead to different mode and extent of activation of intracellular mediators. Here, in the current study we aimed to design N-substituted tetrahydro-beta-carbolines (THβC's) targeting Mu Opioid Receptors (MORs). We performed ligand docking study for reference and designed compounds against active and inactive states of MOR, as well as the active state bound to intracellular mediator of Gi. The reference compounds include 40 known agonists and antagonists, while the designed compounds include 25,227 N-substituted THβC analogues. Out of the designed compounds, 15 compounds were comparatively having better extra precision (XP) Gscore and were analyzed for absorption, distribution, metabolism, and excretion-toxicity (ADMET) properties, drug-likness, and molecular dynamic (MD) simulation. The results showed that N-substituted tetrahydro-beta-carbolines with and without C6-methoxy group substitutions (THBC/6MTHBC) analogues of A1/B1 and A9/B9 have relatively acceptable affinity and within pocket-stability toward MOR compared to the reference compounds of morphine (agonist) and naloxone (antagonist). Moreover, the designed analogues interact with key residue within the binding pocket of Asp 147 that is reported to be involved in receptor activation. In conclusion, the designed THBC analogues represent a good starting point for designing opioid receptor ligands other than morphinan scaffold, that have good synthetic accessibility which promotes feasible structural manipulation to tailor pharmacological effects with minimal side effects.

    Solid-state fermentation (SSF), which is an excellent alternative for industrial enzyme production, entails the exploitation of cheap agro-industrial residues (low priced culture media). This paper delves into the cultivation of mould (GRAS Penicillium candium PCA 1/TT031) using wheat bran (WB) as the culture media. The parameters for crude enzymatic extraction of lipase production were optimized to achieve the highest possible lipase activity. An incubation period of 7 days (3.06 U/g WB), 2% of tributyrin (5.43 U/g WB), meat peptone equal to 2% (5.2 U/g WB), moisture content of 50% (v:w) (6.6 U/g WB), initial pH of 9.0( 8.6 U/g WB), inoculum size of 5×106 spore/g of WB (11.3 U/g WB), an incubation temperature of 20°C (13.6 U/g WB), and an extractant consisting of phosphate buffer pH 7.0 (14.7 U/g WB) were the conditions applied for this study.

    BACKGROUND: Oral lichen planus (OLP) is a relatively common inflammatory disease, with unclear etiology. A number of studies have linked Epstein-Barr virus (EBV) with OLP. The present systematic review and meta-analysis aimed to evaluate the available evidence regarding the potential association between EBV and OLP.

    METHODS: Online databases (PubMed, Scopus, Web of Science, ProQuest, and Google Scholar) were searched from date of inception till May 2020. Studies were included if they met the following criteria: 1) observational studies that assessed the relationship between EBV and OLP, 2) the study comprised OLP patients and control subjects, 3) diagnosis of OLP was confirmed histopathologically, and 4) articles were in English. Studies without control groups, experimental studies, case reports, and reviews were excluded. The fixed-effects model was performed for meta-analyses using RevMan 5.3 software.

    RESULTS: A total of 10 studies comprising 386 OLP cases and 304 controls were included. Of these, only 8 studies were eligible for the meta-analysis. The results of the quality assessment showed that only 2 studies were of high quality, while the remaining studies were of moderate quality.     AIM: This study aimed to assess the quality and readability of the available health information on the net regarding denture hygiene.

    MATERIALS AND METHODS: Three search engines (Google, Yahoo!, and Bing) were searched. The first 20 consecutive websites from each engine were obtained and checked for eligibility. For the quality of the websites, the Health on the Net Foundation Code of Conduct (HONcode), the DISCERN tool, the Journal of the American Medical Association (JAMA) Benchmarks, and Google PageRank were used for the assessment of the included websites. For readability, an online web tool was used, including well-known analyzing indices [Flesch Kincaid grade level (FKGL), Simple Measure of Gobbledygook (SMOG), and Flesch Reading Ease (FRE)]. The acceptable readability level was set to be ≥80.0 for the FRE and <7 for the FKGL and SMOG. The data were presented in frequencies and percentages.

    RESULTS: Out of the 60 screened websites, 14 websites were eligible for analysis. There was only one (7.1%) website that had the HONcode seal. The mean score of all websites based on the DISCERN tool was 29.6 ± 12.1, with no website achieved the high score (≥65). Only one (7.1%) website scored >5 based on Google PageRank. Regarding JAMA benchmarks, all websites achieved a mean score of 2.57 ± 1.1. The mean grade level based on the FKGL was 8.4 ± 6.3. All websites had a score of <7 according to the SMOG index. The mean score of the readability ease index was 90.5 ± 16.4.

    CONCLUSION: Most of the dental health information on denture hygiene available on the Arabic websites did not have the required level of quality, regardless of being readable and comprehensible by most of the general people.

    CLINICAL SIGNIFICANCE: Directing the patients to the appropriate websites related to their cases is the responsibility of the dentists.

    OBJECTIVES: We critically evaluated the quality of evidence and quality of harm reporting in clinical trials that evaluated the effectiveness of hydroxychloroquine (HCQ) or chloroquine (CQ) for the treatment of coronavirus disease 2019 (COVID-19).

    STUDY DESIGN AND SETTING: Scientific databases were systematically searched to identify relevant trials of HCQ/CQ for the treatment of COVID-19 published up to 10 September 2020. The Cochrane risk-of-bias tools for randomized trials and non-randomized trials of interventions were used to assess risk of bias in the included studies. A 10-item Consolidated Standards of Reporting Trials (CONSORT) harm extension was used to assess quality of harm reporting in the included trials.

    RESULTS: Sixteen trials, including fourteen randomized trials and two non-randomized trials, met the inclusion criteria. The results from the included trials were conflicting and lacked effect estimates adjusted for baseline disease severity or comorbidities in many cases, and most of the trials recruited a fairly small cohort of patients. None of the clinical trials met the CONSORT criteria in full for reporting harm data in clinical trials. None of the 16 trials had an overall 'low' risk of bias, while four of the trials had a 'high', 'critical', or 'serious' risk of bias. Biases observed in these trials arise from the randomization process, potential deviation from intended interventions, outcome measurements, selective reporting, confounding, participant selection, and/or classification of interventions.

    CONCLUSION: In general, the quality of currently available evidence for the effectiveness of CQ/HCQ in patients with COVID-19 is suboptimal. The importance of a properly designed and reported clinical trial cannot be overemphasized amid the COVID-19 pandemic, and its dismissal could lead to poorer clinical and policy decisions, resulting in wastage of already stretched invaluable health care resources.

    In the title compound, C(13)H(23)N(2)O(2) (+)·Br(-), the octyl chain has an all-trans conformation. In the crystal, the cations are linked by C-H⋯O bonds into a zigzag chain along the b axis. The bromide anions further link the chains via C-H⋯Br inter-actions into a two-dimensional array parallel to the ab plane. An O-H⋯Br interaction is also observed.

In the crystal, adjacent mol-ecules are linked through C-H⋯O hydrogen bonds into infinite layers parallel to the ac plane. The layers are further connected into a three-dimensional netweork via π-π inter-actions formed between pairs of anti-parallel arranged mol-ecules, with a centroid-centroid distance between the central six-membered ring and the benzene ring of 3.4349 (9) Å.

    In the title compound, C(29)H(24)N(4)O(4)S(3), the two N-tosyl-benzimidazolyl unit are connected through a -S-CH(2)- fragment, the dihedral angle between the benzimidazole rings being 76.09 (5)°. The methyl-thio group is disordered with respect to exchange of the S and C atoms in a 0.547 (4):0.453 (4) ratio. In the crystal, C-H⋯O and C-H⋯π inter-actions connect adjacent mol-ecules into infinite layers parallel to the ab plane. The crystal packing is further stabilized by a π-π inter-action [centroid-centroid separation = 3.5187 (4) Å].

    Geriatric medicine practice requires a multidimensional and multidisciplinary assessment to provide a holistic overview of the older patients. During the current COVID-19 pandemic time, it becomes more critical to ensure that the elderly patients continue to receive regular geriatric care for their pre-existing chronic illness and at the same time avoid unnecessary exposure to COVID-19 virus. Geriatric telemedicine clinic provides a convenient solution to ensure continuity of care for the older patients. Careful patient selection, technical requirement, geriatric assessment via audio-visual communication, and caretaker involvement were among the important issues discussed in this article.

    In this approach, a batch reactor was employed to study the degradation of pollutants under natural sunlight using TiO2 as a photocatalyst. The effects of photocatalyst dosage, reaction time and pH were investigated by evaluating the percentage removal efficiencies of total organic carbon (TOC), chemical oxygen demand (COD), biological oxygen demand (BOD) and biodegradability (BOD/COD). Design Expert-Response Surface Methodology Box Behnken Design (BBD) and MATLAB Artificial Neural Network - Adaptive Neuro Fuzzy Inference system (ANN-ANFIS) methods were employed to perform the statistical modelling. The experimental values of maximum percentage removal efficiencies were found to be TOC = 82.4, COD = 85.9, BOD = 30.9% and biodegradability was 0.070. According to RSM-BBD and ANFIS analysis, the maximum percentage removal efficiencies were found to be TOC = 90.3, 82.4; COD = 85.4, 85.9; BOD = 28.9, 30.9% and the biodegradability = 0.074, 0.080 respectively at the pH 7.5, reaction time 300 min and photocatalyst dosage of 4 g L-1. The study reveals both models found to be well predicted as compared with experimental values. The values of R2 for RSM-BBD (0.920) and for ANFIS (0.990) models were almost close to 1. The ANFIS model was found to be marginally better than that of RSM-BBD.

    Water quality plays a significant role as a key factor in water resource management. The photocatalytic method is widely used for the removal of recalcitrant pollutants present in seawater. Photocatalysis is a cost-effective technology, sustainable, and environmentally friendly treatment process. In the current approach, a batch reactor was utilized experimentally to study the degradation of contaminants present in seawater by utilizing ZnO as a photocatalyst under natural sunlight. The performance of the process was studied by measuring the percentage removal efficiencies of total organic carbon (TOC), chemical oxygen demand (COD), biological oxygen demand (BOD), and biodegradability with respect to photocatalyst dosage, reaction time and pH of the solution. Biodegradability is defined as the ratio of BOD to COD and this parameter significantly removes pollutants from seawater. The higher the biodegradability, the better the performance of the treatment technology. It also significantly reduces the fouling characteristics of seawater during the desalination process. According to experimental values, the maximum percentage removal efficiencies were found to be TOC = 45.6, COD = 65.4, BOD = 20.01% and biodegradability = 0.038 with respect to the initial values of the seawater sample. The response surface methodology based on Box Behnken design (RSM-BBD) and a predictive model based on the MATLAB adaptive neuro-fuzzy inference system (ANFIS) tool were employed for modeling, optimizing, and evaluating the effects of parameters. According to the RSM-BBD and ANFIS models, the determination coefficients were R2 = 0.959 and R2 = 0.99, respectively, which was very close to 1. The maximum percentage removal efficiencies according to the RSM-BBD design were found to be TOC = 40.3; COD = 61.9; BOD = 18.8% and BOD/COD = 0.0390, whereas for the ANFIS model, the maximum reduction were found to be TOC = 46.5; COD = 65.4; BOD = 20.4% and BOD/COD = 0.040. In process optimization, the ANFIS model was shown better prediction than RSM-BBD in the process's optimization.

    5-Fluororaucil (5-FU) as anti-cancer drug was reported to induce thymidine synthase (TS) overexpression and cancer cell resistance. To improve its therapeutic efficacy and selective targeting, here we developed a targeted delivery system mediated by the active ligand-folate receptor chemistry to deliver the 5-FU drug selectively into the tumor microenvironment. The preparation was achieved by exploring chitosan (CS)-biopolymer based system with folic acid (FA)-conjugation. The 5-FU@FACS-Mn:ZnS quantum dots (QDs) based on the histological assessment conducted in the 4T1 challenged mice showed an improved tumor remission in the liver, spleen and lungs. The 5-FU@FACS-Mn:ZnS composite induced anti-proliferative properties in these organs as compared to the free 5-FU drug. Unlike the 5-FU@FACS-Mn:ZnS treated groups which showed some specific morphological changes such as cell shrinkage without obvious presence of adipocytes, the excised section of the tumor in the untreated control group and the free 5-FU drug treated group showed necrotic and degenerated cells; these cells are multifocally distributed in the tumor mass with evidence of widely distributed adipocytes within the tumor mass. These findings suggest that the 5-FU@FACS-Mn:ZnS composite has a superior role during the induction of apoptosis in the 4T1 cells as compared to the free 5-FU drug treated groups. The results of the study therefore suggest that the impregnation of 5-FU anti-cancer drug within the FACS-Mn:ZnS system significantly improves its selective targeting efficacy, in addition to improving the anti-proliferative properties and attenuate possible tumor resistances to the 5-FU drug. The work discusses about the anti-metastatic effects of folic acid-bound 5-Fluororacil loaded Mn:ZnS quantum dots towards 4T1 cell line proliferation in mice based on the histological analysis.
