    Two hundred and ninety-three bronchoscopies were done for 285 patients (78% males, 22% females) at Hospital University Sains Malaysia between 1984 and 1988. The mean age was 56.4 years (range 13 to 90 years). 70.2% of patients underwent bronchoscopies to confirm or exclude the diagnosis of carcinoma of the bronchus, out of which 58% were confirmed to have bronchial carcinoma. 77% of the 98 patients with visible endobronchial tumours had biopsy specimens diagnostic of malignancy. Brushing and washing cytology increased the positive yield to 92%. The commonest histological type of bronchial carcinoma identified was squamous cell carcinoma (48.1%), followed by small cell carcinoma (27.1%), anaplastic/undifferentiated carcinoma (12.9%), adenocarcinoma (9.4%) and large cell carcinoma (2.4%). Bronchoscopy for the investigation of haemoptysis identified the commonest cause as 'bronchitis'. There were no complications noted in our series. Notable differences of our experience compared to that of the western series were the high percentage of bronchoscopy done for infective respiratory disorders and the younger age of our patients.

    We report our experience on the use of fibreoptic bronchoscopy in the diagnosis of pulmonary tuberculosis. The case records of 1,274 patients who underwent fibreoptic bronchoscopy at the National Tuberculosis Centre, Kuala Lumpur, Malaysia during a three-year period were reviewed. In 120 of them the final diagnosis was tuberculosis. Bronchoscopy confirmed the diagnosis in 37 patients (30.8%). This was achieved by bronchial aspiration for culture in 26 patients (70.3%) and bronchial biopsy for histopathology in 11 patients (29.7%). It was the exclusive method of confirmation in 16 patients (13.3%). Sputum culture was positive in 62 patients (51.7%) including 41 patients (34.2%) in whom bronchoscopy was unhelpful. Six patients had diagnosis confirmed by other means while in 36 others (30%) it was based on clinical features and supportive basic investigations. There were no complications noted. We conclude that while sputum examination remains the mainstay for diagnosing pulmonary tuberculosis, fibreoptic bronchoscopy serves as a safe and useful adjunct.

    Lignocaine spray for anaesthetising the nasal mucosa for fibreoptic bronchoscopy often causes discomfort to the patient. We compared two techniques of applying nasal topical anaesthesia using either lignocaine spray (group A: 25 patients) or gel (group B: 30 patients) to assess patients' tolerance to the procedure. Both groups received 100 mg of lignocaine in the nostril, 40-50 mg to oropharynx, 120 mg to vocal cords and 40-100 mg to trachea and bronchi. Throat anaesthesia was the most common unpleasant part experienced by both groups of patients (34.5%), followed by examination of bronchi (30.6%) and nasal anaesthesia (21.8%). Significantly more patients in group A experienced discomfort or pain during nasal anaesthesia as compared to group B (p < 0.001). Patients' tolerance to the bronchoscopy was similar in both groups and the examination was performed satisfactorily in all patients. Thus, lignocaine gel is a simple technique, effective and less irritating as compared to lignocaine spray for topical nasal anaesthesia.

    Fibreoptic intubation has been established as a major advance in the management of difficult or failed intubation in the awake patient. If necessary, it may be performed under general anaesthesia with either spontaneous or controlled ventilation. This should be considered early in the management of failed intubation, before multiple attempts with other techniques lead to haemorrhage and oedema in the airway. We describe here selected case reports to illustrate this in 8 different situations. This is followed by a brief review of the technique and indications of fibreoptic intubation.

    This paper describes a system using lensed optical fiber sensors that are arranged in the form of two orthogonal projections. The sensors are placed around a process vessel for upstream and downstream measurements. The purpose of the system is for on-line monitoring of particles and droplets being conveyed by a fluid. The lenses were constructed using a custom heating fixture. The fixture enables the lenses to be constructed with similar radii resulting in identical characteristics with minimum differences in transmitted intensity and emission angle. By collimating radiation from two halogen bulbs, radiation can be obtained by the sensors with radiation intensity related to the nature of the media. Each sensor interrogates a finite section of the measurement section. Each sensor provides a view. Parallel sensors provide a projection. Signal processing is carried out on the measured data in the time and frequency domains to investigate the latent information present in the flow signals.

    Percentage depth doses for 6 and 10 MV x-ray beams from a linear accelerator were measured using approximately 1 cm long (approximately 0.3 mg) Ge-doped optical fibre as a thermoluminescence dosimeter for two field sizes, 5 x 5 and 10 x 10 cm2. The results indicate that the Ge-doped optical fibre dosimeter is in good agreement with the results from a PTW 30001 cylindrical ionisation chamber and TLD-100. For 6 MV x-ray beams we observe that the depth of maximum dose d(max) is 1.5 and 2 cm for field sizes of 5 x 5 and 10 x 10 cm2 respectively. For 10 MV d(max) is 2 cm for a field size of 5 x 5 cm2 and 2.5 cm for a 10 x 10 cm2 field.

    We demonstrate a multiple-wavelength Brillouin comb laser with cooperative Rayleigh scattering that uses Raman amplification in dispersion-compensating fiber. The laser resonator is a linear cavity formed by reflector at each end of the dispersion-compensating fiber to improve the reflectivity of the Brillouin Stokes comb. Multiple Brillouin Stokes generation has been improved in terms of optical signal-to-noise ratio and power-level fluctuation between neighboring channels. Furthermore, the linewidth of the Brillouin Stokes is uniform within the laser output bandwidth.

    Fatty hydroxamic acid (FHA) immobilized in polyvinyl chloride (PVC) has been studied as a sensor element of an optical fibre chemical sensor for V(V). By using this instrument, V(V) in solution has been determined in the log concentration range of 0-2.5 (i.e. 1.0-300 mg/L). The detection limit was 1.0 mg/L. The relative standard deviation (R.S.D.) of the method for the reproducibility study at V(V) concentration of 200 mg/L and 300 mg/L were calculated to be 2.9% and 2.0%, respectively. Interference from foreign ions was also studied at 1:1 mole ratio of V(V):foreign ions. It was found that, Fe(III) ion interfered most in the determination of vanadium(V). Excellent agreement with ICP-AES method was achieved when the proposed method was applied towards determination of V(V).

    This paper presents the implementing multiple fan beam projection technique using optical fibre sensors for a tomography system. From the dynamic experiment of solid/gas flow using plastic beads in a gravity flow rig, the designed optical fibre sensors are reliable in measuring the mass flow rate below 40% of flow. Another important matter that has been discussed is the image processing rate or IPR. Generally, the applied image reconstruction algorithms, the construction of the sensor and also the designed software are considered to be reliable and suitable to perform real-time image reconstruction and mass flow rate measurements.

    A configuration for linear cavity Brillouin fiber laser (BFL) generation is demonstrated using a standard single-mode fiber, two optical circulators, a 3 dB coupler, and a 95/5 coupler to allow high efficiency. With a Brillouin pump (BP) power of 13 dBm, the laser peak power is 12.3 dB higher than a conventional linear cavity BFL at an upshifted wavelength of 0.086 nm from the BP wavelength. In addition, it is revealed that the BFL peak power can be higher than the transmitted BP peak power when the BP power exceeds the second Brillouin Stokes threshold power.

    We demonstrate an enhanced multiwavelength L-band Brillouin-erbium fiber laser (BEFL), in which the Brillouin pump is pre-amplified before entering the single-mode fiber. The Brillouin pump pre-amplification provided by the Erbium-doped fiber has created higher intensity of Brillouin Stokes line generated in the single-mode fiber that leads to the homogenous gain saturation. Thus the built-up of self-lasing cavity modes is suppressed in a wider wavelength range. In contrary to the conventional linear-cavity BEFL, the number of output channels is enhanced within the same tuning range.

    A multi-wavelength laser comb is demonstrated using a nonlinear effect in a backward pumped Bismuth-based Erbium-doped fiber (Bi-EDF) for the first time. It uses a ring cavity resonator scheme containing a 215 cm long highly nonlinear Bi-EDF, optical isolators, polarisation controller and 10 dB output coupler. The laser generates more than 10 lines of optical comb with a line spacing of approximately 0.41 nm at 1615.5 nm region using 146 mW of 1480 nm pump power.

    The method of lines (MoL) has been developed to study coupling efficiency on hemispherical lens. In this paper, the physical shape of the lens is approximated by cascading a number of straight waveguide segments. The perfectly matched layer (PML) is applied as an absorber for the MoL to reduce numerical reflection in the simulation region. Analysis is done by calculating coupling efficiency at the plane of integration where the coupling efficiency is an overlap integral between laser diode field and fiber field. The result of coupling efficiency in this analysis is compared to the experiment and ABCD matrix. It is found that MoL gives good result accuracy.

    We experimentally demonstrate a simple widely tunable multiwavelength Brillouin/Erbium fiber laser that can be tuned over the entire C-band, thereby greatly improving the tuning range limitation faced by the previous Brillouin-erbium fiber laser architectures. Tuning range of 39 nm from 1527 nm to 1566 nm, which is only limited by the amplification bandwidth of the erbium gain was successfully achieved. At Brillouin pump wavelength of 1550 nm and 1480 nm laser pump and Brillouin pump powers of 130 mW and 2 mW respectively, all the generated output channels have peak power above 0 dBm, with the first output channel having a peak power of 8.52 dBm. The experimental set up that consists of only 4 optical components, is simple, devoid of the complex structure employed previously to enhance the tunability and feedback mechanism normally associated with multiwavelength Brillouin-erbium fiber laser sources. The generated output channels are stable, rigidly separated by 10 GHz (0.08 nm).

    We demonstrate a simple configuration for generating a double Brillouin frequency shift through the circulation of an odd-order Brillouin Stokes signal. It is operated based on cascaded Brillouin scattering in single-mode optical fibers that behave as the Brillouin gain media. A four-port circulator is incorporated into the setup to circulate the odd-order Brillouin Stokes signal in the fiber. It thus initiates a higher order Brillouin Stokes signal, which is double Brillouin frequency downshifted from the input signal. For the 5 km long fiber, the Brillouin pump power at 23 mW gives a clean output spectrum with 30 dB sideband suppression ratio. The output signal is 0.174 nm or approximately 21.7 GHz downshifted from the input signal.

    This paper describes a preliminary study of the thermoluminescence (TL) response of doped SiO(2) optical fibres subjected to (241)AmBe neutron irradiation. The TL materials, which comprise Al- and Ge-doped silica fibres, were exposed in close contact with the (241)AmBe source to obtain fast neutron interactions through use of measurements obtained with and without a Cd filter (the filter being made to entirely enclose the fibres). The neutron irradiations were performed for exposure times of 1-, 2-, 3-, 5- and 7-days in a neutron tank filled with water. In this study, use was also made of the Monte Carlo N-particle (MCNP) code version 5 (V5) to simulate the neutron irradiations experiment. It was found that the commercially available Ge-doped and Al-doped optical fibres show a linear dose response subjected to fast neutrons from (241)AmBe source up to seven days of irradiations. The simulation performed using MCNP5 also exhibits a similar pattern, albeit differing in sensitivity. The TL response of Ge-doped fibre is markedly greater than that of the Al-doped fibre, the total absorption cross section for Ge in both the fast and thermal neutrons region being some ten times greater than that of Al.

    We demonstrate a multiwavelength Brillouin-erbium fiber laser with double-Brillouin-frequency spacing. The wider channel spacing is realized by circulating the odd-order Stokes signals in the Brillouin gain medium through a four-port circulator. The circulated odd-order Stokes signals are amplified by the Brillouin gain and thus produce even-order Stokes signals at the output. These signals are then amplified by erbium gain block to form a ring-cavity laser. Ten channels with 0.174 nm spacing that are generated at 0.5 mW Brillouin pump power and 150 mW pump power at 1480 nm can be tuned from 1556 nm to 1564 nm. The minimum optical signal-to-noise ratio of the generated output channels is 30 dB with maximum power fluctuations of ±0.5 dB.

    Effects of immersing a microfiber knot resonator (MKR) in liquid solutions that have refractive indices close to that of silica are experimentally demonstrated and theoretically analyzed. Significant improvement in resonance extinction ratio within 2 to 10 dB was observed. To achieve a better understanding, a qualitative analysis of the coupling ratio and round-trip attenuation of the MKR is performed by using a curve-fitting method. It was observed that the coupling coefficient at the knot region increased when immersed in liquids. However, depending on the initial state of the coupling and the quantity of the increment in the coupling coefficient when immersed in a liquid, it is possible that the MKR may experience a deficit in the coupling parameter due to the sinusoidal relationship with the coupling coefficient.

    This paper details a theoretical modeling of Brillouin ring fiber laser which incorporates the interaction between multiple Brillouin Stokes signals. The ring cavity was pumped at several Brillouin pump (BP) powers and the output was measured through an optical coupler with various coupling ratios. The first-order Brillouin Stokes signal was saturated with the presence of the second-order Stokes signal in the cavity as a result of energy transfer between them. The outcome of the study found that the optimum point for the first-order Stokes wave performance is at laser power reduction of 10%. Resultantly, at the optimum output coupling ratio of 90%, the BFL was able to produce 19.2 mW output power at BP power and Brillouin threshold power of 60 and 21.3 mW respectively. The findings also exhibited the feasibility of the theoretical models application to ring-type Brillouin fiber laser of various design parameters.
