A study was done in 1987 to find out the attitudes and perceptions of patients toward treatment in the Hospital Universiti Sains Malaysia (HUSM). A similar study was repeated five years later covering both patients and relatives of patients. In 1987, 17% of patients were not satisfied with the treatment mainly because of poor patient-staff relationship. In 1991, 5% of the patients were dissatisfied. The reasons attributed for their dissatisfaction were rudeness of staff, the presence of medical students and attitude of the patients and public of the nurses. There was a positive change of attitude of the aptients and public towards the hospital services after five years. Factors that lead to this improvement like education and better public relations is discussed.
Analysis of the nutritional status and its related factors of three different geographic areas was conducted. The areas were Kampong Sungai Gulang-Gulang, a traditional village in Kuala Selangor, vegetable farming area in Kuala Terla, Cameron Highland and housing flats in Kuala Lumpur. Assessment of nutritional status was done using anthropometric, clinical, biochemical, dietary and stool examination. The results show that 13% of the children in traditional village were stunted, 8% were wasted and 17% were underweight. In vegetable farming area the prevalence were 16.5%, 10.2% and 20.2% respectively. The prevalence of anaemia among toddler was 81.0% in traditional village, 77.2% in vegetable farming area and 55.3% in urban flats. Anaemia in the three areas was strongly associated with inadequate intake of iron.
Three iIIustrative cases of children with depression managed at the University Hospital, Kuala Lumpur; showed similar features of depressive illness and the association with family problems and depressed parents as described in Western countries. The Multi-Axia System of classification (MAS), as suggested in a WHO publication provided a useful means of classification as well as management. Two of the axes are biological and psycho-social factors in the aetiology of psychiatric syndromes (the third axis) and the intellectual level (the second axis of the child). Appropriate counselling of the parents is an important part in the management of depression in children.
Background: Impaired fasting glucose (IFG) poses a higher risk of diabetes. Honey has been reported to improve metabolic abnormalities including lowering hyperglycemia. This study is sought at determining the effect of Malaysian Kelulut honey (KH) on fasting glucose levels and metabolic parameters in IFG patients.
Methods: This quasi-experimental intervention study of 30-day duration was conducted among 60 adult patients with IFG. They were allocated into taking 30 g/day KH group (experimental group, n=30) and not taking KH group (control group, n=30). Body mass index (BMI), waist circumference, blood pressure (BP), fasting glucose, and lipid profile levels (total cholesterol, triglyceride, high-density lipoprotein, and low-density lipoprotein) were measured before and after treatment.
Results: There was no significant difference in all measured variables at day 30 compared to day 1 within both groups. Similarly, all measured variables neither at day 1 nor at day 30 had shown a statistically significant difference between the groups.
Conclusions: Daily intake of 30 g KH for 30 days resulted in insignificant effect on fasting glucose, fasting lipid profiles, and other metabolic parameters in patients with IFG. Further studies that employ longer study duration are needed to ascertain the finding.

In paddy cultivation, harvesting is the most important operation, which needs suitable machinery. Thus, this study was carried out to compare field performances and energy and environmental effect between the conventional 5 m cutting width NEW HOLLAND CLAYSON 8080, 82 kW@2500 rpm combine harvester running on a total net area of 42.78 ha of plots for two rice (Oryza sativa L.) cultivation seasons and the new mid-size 2.7 m cutting width WORLD STAR WS7.0, 76 kW@2600 rpm combine harvester running on a total net area of 16.95 ha of plots for two rice cultivation seasons. The conventional combine as compared to mid-size combine showed 14.4% greater mean fuel consumptions (21.13 versus 18.46 l/ha), 31.1% greater mean effective field capacity (0.69 versus 0.53 ha/h), 5.23% greater cornering time (turning time) percentage of total time (8.28% versus 3.05%) and 1.41% greater reversing time percentage of total time (7.2% versus 5.79%) but 20.90% lesser mean operational speed (3.24 versus 4.10 km/h), 11.69% lesser effective time percentage of total time (60.0%versus 71.69%h/ha), 10.8% lesser mean field efficiency (64.3% versus 72.1%). In terms of total energy use the conventional combine showed 24.64% greater mean total energy use in the harvesting operation (1445.81 versus 1160.00 MJ/ha), 14.46% greater mean fuel energy (1010.014 versus 882.39 MJ/ha), 56.47% greater mean machinery energy (431.32 versus 275.65 MJ/ha) and 59.25% greater mean human energy (3.48 and 2.18 MJ/ha), this cause 26.12% greater mean total Green House Gas emission (GHG) than the mid-size combine. The results revealed that the mid-size combine is more suitable in conducting the harvest operation in rice field in Malaysia than the conventional combine.

Homelessness continues to exist in our country even though we hear of various initiatives that have been put in place to solve this ongoing issue. The objectives of this study are, first, to explore factors that lead to homelessness, second, to determine the associated medical conditions and third, to shed light on the views of homeless individuals regarding their future perspectives.
Psychiatric community-based services are being developed in Malaysia currently to ensure more comprehensive mental health care to especially patients with severe mental illness. Assertive Community Treatment (ACT) as one of the earliest component of community-based services has been observed to be useful and able to provide favourable outcomes in this group of patients. Though the paradigm shift has gradually occurred among mental health practitioners and policy makers, challenges are mainly in the implementation process. This article will present the discussion and academic view on various aspects of ACT including the rationale, elements, clinical and psychosocial impacts on patients as well as the current challenges in the Malaysian context. The use of Electro-Convulsive Therapy (ECT) has again risen, and so have the restrictions laid in the path for its use. This communication serves to discuss the thoughts and practices of ECT in our setting and that in the West, which we all know vary from center to center and individual to individual. A reminder guide, incorporating those ideas along with a simple risk-benefit assessment checklist, is suggested to induce an approved perspective that will enable the drawing-up of a standard, sanctioned guideline on the applications of ECT in our region. Aflatoxin B1(AFB1) is a toxic compound commonly found in some crops with an adverse health effect on human and animals. Some beneficial microorganisms (or probiotics) such as lactic acid bacteria have shown the ability to reduce the bioavailability of aflatoxins and its intestinal absorption. However, the dose and duration of aflatoxins exposure and probiotic treatment can influence the ability of probiotics to remove aflatoxins. Therefore, this research aimed to investigate the efficacy of oral probiotic Lactobacillus casei Shirota strain (LcS) induction in an acute exposure to AFB1 in rats. Experimentally, Sprague Dawley rats were divided into three groups: AFB1 only (n = 9); AFB1 treated with LcS (n = 9); and control (no AFB1 exposure) (n = 6) groups. The blood AFB1 level of rats treated with LcS was slightly lower than the untreated AFB1 induced rats (11.12 ± 0.71 vs 10.93 ± 0.69 ng g-1). Also, LcS treatment slightly moderated the liver and kidney biomarkers in AFB1 induced rats. However, a trend for a significant difference was only observed in ALT of AFB1 induced rats treated with LcS compared to their counterparts (126.11 ± 36.90 vs 157.36 ± 15.46, p = 0.06). Rats' body weight decreased in all animals force-fed with AFB1 with no significant difference between LcS treatment compared to the counterpart. In conclusion, this experiment indicated that probiotic LsC was able to slightly ameliorate the adverse effect of an acute exposure to AFB1 in rats. However, future studies with longer probiotics treatment or higher probiotics dose is required to confirm these findings.
The phenomenon of 'lost fetus' to orang bunian is quite commonly heard of in our culture. It may present in psychiatric settings and may potentially create confusion and difficulties in diagnostic and management aspects. A case of feigned full-term pregnancy followed by a pregnancy loss is described. This review emphasizes the need to recognize this phenomenon of 'lost advanced pregnancy' in context of the Malay belief. Possible etiological factors, diagnostic indicators of factitious disorder and management strategies are discussed.
