    Sixty patients with brain abscess were treated at the Neurosurgical Unit of the Department of Surgery, Hospital Universiti Sains Malaysia between January 1990 and December 1996. A retrospective study was done and data were collected from the computerise d registry of the Record Unit of Hospital Universiti Sains Malaysia. Good results were achieved in patients who were both treated surgically and medically. There were only twelve deaths in this group. The main factor that influences morbidity and mortality of brain abscess is the clinical presentation on admission. The mortality was high in patients treated solely by medical means. Death was common in patients who presented with acute onset of symptoms of less than one week duration and those with poor mental status. Brain abscess is common in the East Coast population of peninsular Malaysia, probably due in part to lower socioeconomic status. Efforts should be directed towards prevention of infection and early recognition and management.

    The first endoscopic procedure done in Malaysia using the Caemaert-Abdullah method is reported and the followup results showed an excellent neurosurgical outcome. A 16-year-old girl with an aqueduct stenosis was operated on using a free-hand, computer-assisted endoscopic method where a third ventriculostomy was done. This was the first case being carried out in the Hospital Universiti Sains Malaysia. The next two cases were a suprasellar pituitary cyst in an elderly man and a child with an obstructive hydrocephalus who was previously shunted which became infected. Both endoscopic procedures, extirpation of the cyst using a Nd:Yag laser and a third ventriculostomy, respectively, were done under general anaesthesia in the supine position. Follow-up revealed a transient diabetes insipidus in all three cases up to 48 hours after the operation which resolved spontaneously. The fourth case involved an endoscopic removal of retained ventricular catheter after rectal migration of a shunt in an eight-year-old girl with congenital hydrocephalus. The fifth was a free-hand endoscopy with perforation of multiple brain septae in a ten-month-old baby with hydrocephalus secondary to meningitis. The final outcomes for all the cases were favourable hence we conclude that endoscopic neurosurgery is a safe procedure and hope that more neurosurgeons will continue to use this method, especially for the management of intraventricular cyst and hydrocephalus and especially in South East Asia.

    Vehicular ad hoc networks (VANET) are also known as intelligent transportation systems. VANET ensures timely and accurate communications between vehicle to vehicle (V2V) and vehicle to infrastructure (V2I) to improve road safety and enhance the efficiency of traffic flow. Due to its open wireless boundary and high mobility, VANET is vulnerable to malicious nodes that could gain access into the network and carry out serious medium access control (MAC) layer threats, such as denial of service (DoS) attacks, data modification attacks, impersonation attacks, Sybil attacks, and replay attacks. This could affect the network security and privacy, causing harm to the information exchange within the network by genuine nodes and increase fatal impacts on the road. Therefore, a novel secure trust-based architecture that utilizes blockchain technology has been proposed to increase security and privacy to mitigate the aforementioned MAC layer attacks. A series of experiment has been conducted using the Veins simulation tool to assess the performance of the proposed solution in the terms of packet delivery ratio (PDR), end-to-end delay, packet loss, transmission overhead, and computational cost.

    Malaysia is a tropical country that is highly dependent on surface water for its raw water supply. Unfortunately, surface water is vulnerable to pollution, especially in developed and dense urban catchments. Therefore, in this study, a methodology was developed for an extensive temporal water quality index (WQI) and classification analysis, simulations of various pollutant discharge scenarios using QUAL2K software, and maps with NH3-N as the core pollutant using an integrated QUAL2K-GIS. It was found that most of the water quality stations are categorized as Class III (slightly polluted to polluted). These stations are surrounded by residential areas, industries, workshops, restaurants and wet markets that contribute to the poor water quality levels. Additionally, low WQI values were reported in 2010 owing to development and agricultural activities. However, the WQI values improved during the wet season. High concentrations of NH3-N were found in the basin, especially during dry weather conditions. Three scenarios were simulated, i.e. 10%, 50% and 70% of pollution discharge into Skudai river using a calibrated and validated QUAL2K model. Model performance was evaluated using the relative percentage difference. An inclusive graph showing the current conditions and pollution reduction scenarios with respect to the distance of Skudai river and its tributaries is developed to determine the WQI classification. Comprehensive water quality maps based on NH3-N as the core pollutant are developed using integrated QUAL2K-GIS to illustrate the overall condition of the Skudai river. High NH3-N in the Skudai River affects water treatment plant operations. Pollution control of more than 90% is required to improve the water quality classification to Class II. The methodology and analysis developed in this study can assist various stakeholders and authorities in identifying problematic areas and determining the required percentage of pollution reduction to improve the Skudai River water quality.

    MAIN CONCLUSION: X-ray microtomography results revealed that delignification process damaged the oil palm fibers, which correlated well with reduction of lignin components and increase of the phenolic content. Biodegradation investigation of natural fibers normally focuses on physico-chemical analysis, with less emphasis on physical aspect like fiber structures affect from microbial activity. In this work, the performance of Pycnoporus sanguineus to delignify oil palm empty fruit bunch fibers through solid-state fermentation utilizing various ratio of POME sludge was reported. In addition to tensile testing, physico-chemical and X-ray microtomography (µ-CT) analyses on the oil palm fibers were conducted to determine the effectiveness of the degradation process. The best ratio of fiber to fungi (60:40) was chosen based on the highest lignin loss and total phenolic content values and further investigation was performed to obtain fermentation kinetics data of both laccase and manganese peroxidase. µ-CT results revealed that delignification process damaged the pre-treated and untreated fibers structure, as evident from volume reduction after degradation process. This is correlated with reduction of lignin component and increase of the phenolic content, as well as lower stress-strain curves of the pre-treated fibers compared to the untreated ones (from tensile testing). It is suggested that P. sanguineus preferred to consume the outer layer of the fiber, before it penetrates through the cellular structure of the inner fiber.

    The need for the sensing of environmental pollutants cannot be overemphasized in the twenty-first century. Herein, a sensor has been developed for the sensitive and selective detection of copper (Cu2+), lead (Pb2+) and cadmium (Cd2+) as major heavy metals polluting water environment. A screen-printed carbon electrode (SPCE) modified by fluorescent carbon dots (CDs) and gold nanoparticles (AuNPs) was successfully fabricated for sensing Cu2+, Pb2+ and Cd2+. Differential pulse voltammetry (DPV) and cyclic voltammetry (CV) were deployed for the analysis of ternary analytes. CV was set at a potential range of - 0.8 to + 0.2 V at a scan rate of 100 mV/s, and DPV at a potential range of - 0.8 to + 0.1 V, scan rate of 50 mV/s, pulse rate of 0.2 V and pulse width of 50 ms. DPV technique was applied through the modified electrode for sensitive and selective determination of Cu2+, Pb2+ and Cd2+ at a concentration range of 0.01 to 0.27 ppm for Cu2+, Pb2+ and Cd2+. Tolerance for the highest possible concentration of foreign substances such as Mg2+, K+, Na+, NO3-, and SO42- was observed with a relative error less than ± 3%. The sensitivity of the modified electrode was at 0.17, 0.42 and 0.18 ppm for Cd2+, Pb2+ and Cu2+, respectively, while the limits of detection (LOD) achieved for cadmium, lead and copper were 0.0028, 0.0042 and 0.014 ppm, respectively. The quality of the modified electrode for sensing Cu2+, Pb2+ and Cd2+ at trace levels is in accordance with the World Health Organization (WHO) and Environmental Protection Agency (EPA) water regulation standard. The modified SPCE provides a cost-effective, dependable and stable means of detecting heavy metal ions (Cu2+, Pb2+ and Cd2+) in an aqueous solution. Graphical abstract .

    In this work, a novel electrochemical sensor was fabricated for determination of amoxicillin in bovine milk samples by decoration of carboxylated multi-walled carbon nanotubes (MWCNTs) with gold nanoparticles (AuNPs) using ethylenediamine (en) as a cross linker (AuNPs/en-MWCNTs). The constructed nanocomposite was homogenized in dimethylformamide and drop casted on screen printed electrode. Field emission scanning electron microscopy (FESEM), energy dispersive X-Ray (EDX), X-Ray diffraction (XRD) and cyclic voltammetry were used to characterize the synthesized nanocomposites. The results show that the synthesized nanocomposites induced a remarkable synergetic effect for the oxidation of amoxicillin. Effect of some parameters, including pH, buffer, scan rate, accumulation potential, accumulation time and amount of casted nanocomposites, on the sensitivity of fabricated sensor were optimized. Under the optimum conditions, there was two linear calibration ranges from 0.2-10 µM and 10-30 µM with equations of Ipa (µA) = 2.88C (µM) + 1.2017; r = 0.9939 and Ipa (µA) = 0.88C (µM) + 22.97; r = 0.9973, respectively. The limit of detection (LOD) and limit of quantitation (LOQ) were calculated as 0.015 µM and 0.149 µM, respectively. The fabricated electrochemical sensor was successfully applied for determination of Amoxicillin in bovine milk samples and all results compared with high performance liquid chromatography (HPLC) standard method.

    Subtentorial subdural empyema is a rare and life threatening intracranial suppuration. It is usually an intracranial complication of otogenic infections. Early diagnosis and surgical drainage are the most important factors determining prognosis. The high mortality reported in the literature reflects the severity of subtentorial subdural empyema if proper management is delayed. Intracranial infections usually require between 4 to 6 weeks of intravenous antibiotics therapy. However, the prolonged duration of hospitalization as well as requirement for neurosurgically inserted indwelling devices may predispose these patients to new nosocomial infections.

    Shunt surgery is frequently chosen to manage periventricular metastasis of pineal region tumours which obscured the floor of the third ventricle. However, this procedure falls short due to distant metastasis. Neuronavigation-guided endoscopic surgery offers a viable alternative.

    Newcastle disease virus (NDV) is a virus of paramyxovirus family and lately has been studied for the treatment of cancer in human. In this study, we successfully determined the oncolysis potential of NDV vaccine, V4UPM tested on the human glioblastoma multiform cell line (DBTRG.05MG) and human glioblastoma astrocytoma cell line (U-87MG) in vitro and in vivo. The V4UPM strain is a modified V4 strain developed as thermostable feed pellet vaccine for poultry.

    Frequent loss of heterozygosity (LOH) and mutations of the tumor suppressor gene PTEN (phosphatase and tensin homologue deleted from chromosome 10) have been found in sporadic gliomas. The most documented regions of allelic losses include 9p21, 10q23-25 and 17p1 3 whereas PTEN aberrations are preferentially found in glioblastoma multiformes. This research aimed to detect the incidence of allelic losses on chromosomes 10q, 9p, 17p and 13q and mutations on exons 5, 6 and 8 of PTEN in malignant gliomas. Malignant glioma specimens obtained were classified histopathologically according to the WHO criteria. Each tumor was then subjected to polymerase chain reaction (PCR)-LOH analysis using microsatellite markers and single-stranded conformational polymorphism (SSCP) analysis. Twelve of 23 (52%) malignant glioma cases showed allelic losses whereas 7 of 23 (30%) samples showed aberrant band patterns and mutations of PTEN. Four of these cases showed LOH in 10q23 and mutations of PTEN. The data on LOH indicated the involvement of different genes in the genesis of glioma whereas mutations of PTEN indicated the role of PTEN tumor suppressor gene in the progression of glioma in Malay population.

Here, we report the complete genome sequence of M. tuberculosis clinical isolate spoligotype SIT745/EAI1-MYS, which was isolated from a Malaysian tuberculosis patient.

    Nonchromaffin paragangliomas are unusual tumours arising from widely distributed paraganglionic tissues probably of neural crest origin. In the head and neck region they are usually seen as carotid body or jugulotympanic tumours. Other rarely reported sites in the head and neck region are the orbit, nose and larynx. This report deals with a case of sinonasal paraganglioma which was initially treated with surgery and radiotherapy. Twenty two years later the tumour recurred and showed a rapid growth due to malignant transformation which we believe is late effect of radiotherapy. The clinical features, histopathology and role of radiotherapy in sinonasal paragangliomas together with a review of the medical literature have been discussed.

    INTRODUCTION: Chiari malformation (CM) is a disorder of mesodermal origin and is commonly associated with syringomyelia. Foramen magnum decompression is the first-line of standard treatment in symptomatic patients with a confirmed radiographic diagnosis. Magnetic resonance (MR) cine allows accurate evaluation of cerebrospinal fluid (CSF) physiology at the craniovertebral junction but often this is under-utilised in Malaysia.

    METHODS: In this series, we looked into nine cases of CM with syringomyelia from clinical and radiological perspective before and after surgery. The radiological parameters were herniated tonsillar length, syrinx: cord ratio, syrinx length and diameter. Flow velocity and morphologic changes in Chiari were illustrated.

    RESULTS: Seven patients showed either reduction in syrinx length, syrinx: cord ratio or both postoperatively. Clinical recovery somewhat varied in motor and sensory symptoms. Four patients gained better functional grade in modified Rankin scale (MRS) while the rest remained similar. The study highlighted the advantage of CSF flow dynamics information over MR anatomical radiographic improvement in addressing the neurologic and functional recovery. We also discussed the practicality of cine sequence in preoperative patient selection, syrinx analysis and postoperative flow evaluation in anticipation of clinical outcome.

    CONCLUSION: Phase-contrast cine MRI is a useful tool dictated by resource availability. We recommend its routine use in preoperative analysis and subsequent observational follow-up after surgery.

    BACKGROUND: To date, BRAF mutations in brain tumor patients have not been characterized in the Malaysian population. Based on the numerous reported studies, there are main mutations that exist in BRAF gene in various types of cancers. A missense mutation in codon 600 of the BRAF nuclear oncogene (BRAFV600E) is the most prevalent hotspot point mutation that has been identified in multiple human malignancies.

    AIM: We here aimed to find out the frequency of BRAFV600E mutation in a series of Malaysian patients with brain tumors and if any association exists between BRAFV600E mutation and clinicopathological features of patients.

    MATERIAL AND METHODS: Fresh frozen tumor tissue samples from 50 Malaysian brain tumor patients were analyzed for BRAFV600E mutational status, and its correlation with clinicopathological features (including age, gender, and tumor localization such as intra-axial: within the brain substance or extra-axial: outside the brain substance) was examined.

    RESULTS: The overall BRAFV600E mutation frequency was determined to be 22% (in 11 of 50 patients). BRAFV600E was significantly correlated with the tumor location group, which shows BRAFV600E was more frequent in the intra-axial tumor than the extra-axial tumor group. In this study, we also observed that male patients were slightly more susceptible to BRAFV600E mutation, and this mutation was predominant in patients of the age group

    There is no report in the English literature on the criteria for neuroablation or neuroaugmentation for the treatment of Parkinson's disease in a developing country like Malaysia. A prospective study of patients with Parkinson's disease from the north-eastern peninsular Malaysia was done to assess their suitability of surgery. Age, race, duration of illness and dementia were considered important factors towards the success of such surgical procedures. A mathematical model is suggested for future cases deemed to be suitable for neuroaugmentative or ablative surgery.

    This research enhances crowd analysis by focusing on excessive crowd analysis and crowd density predictions for Hajj and Umrah pilgrimages. Crowd analysis usually analyzes the number of objects within an image or a frame in the videos and is regularly solved by estimating the density generated from the object location annotations. However, it suffers from low accuracy when the crowd is far away from the surveillance camera. This research proposes an approach to overcome the problem of estimating crowd density taken by a surveillance camera at a distance. The proposed approach employs a fully convolutional neural network (FCNN)-based method to monitor crowd analysis, especially for the classification of crowd density. This study aims to address the current technological challenges faced in video analysis in a scenario where the movement of large numbers of pilgrims with densities ranging between 7 and 8 per square meter. To address this challenge, this study aims to develop a new dataset based on the Hajj pilgrimage scenario. To validate the proposed method, the proposed model is compared with existing models using existing datasets. The proposed FCNN based method achieved a final accuracy of 100%, 98%, and 98.16% on the proposed dataset, the UCSD dataset, and the JHU-CROWD dataset, respectively. Additionally, The ResNet based method obtained final accuracy of 97%, 89%, and 97% for the proposed dataset, UCSD dataset, and JHU-CROWD dataset, respectively. The proposed Hajj-Crowd-2021 crowd analysis dataset and the model outperformed the other state-of-the-art datasets and models in most cases.

    We studied the efficacy of two surgical methods used for the treatment of intracranial subdural empyema (ISDE) at our centre. A cross-sectional study (1999-2005) of 90 patients with non-traumatic supratentorial ISDE revealed that the two surgical methods used for empyema removal were burr hole/s and drainage (50 patients, 55.6%) and a cranial bone opening procedure (CBOP) (40 patients, 44.4%). Patients in the CBOP group had a better result in terms of clinical improvement (chi-squared analysis, p=0.006) and clearance of empyema on brain CT scan (chi-squared analysis, p<0.001). Reoperation was more frequent among patients who had undergone burr hole surgery (multiple logistic regression, p<0.001). The outcome and morbidity of ISDE survivors were not related to the surgical method used (p>0.05). The only factor that significantly affected the morbidity of ISDE was level of consciousness at the time of surgery (multiple logistic regression, p<0.001). We conclude that CBOP and evacuation of the empyema is a better surgical method for ISDE than burr hole/s and drainage. Wide cranial opening and empyema evacuation improves neurological status, gives better clearance of the empyema and reduces the need for reoperation. Level of consciousness at the time of presentation is a predictor of the morbidity of ISDE. Thus, aggressive surgical treatment should occur as early as possible, before the patient deteriorates.
