    PURPOSE: The purpose of this study was to investigate the association between long-term intranasal steroid use and intraocular pressure (IOP) elevation.

    PATIENTS AND METHODS: In total, 100 eyes from 50 patients on long-term intranasal steroids (>2 y) for allergic rhinitis and 90 eyes from 45 controls were included in this study. Patients on other forms of steroids and risk factors for glaucoma were excluded. IOP was measured and nonmydriatic stereoscopic optic disc photos were taken for each eye. The vertical cup-to-disc ratio and the status of the optic disc were evaluated.

    RESULTS: The mean IOP for intranasal steroids group was significantly higher (15.24±2.31 mm Hg) compared to the control group (13.91±1.86 mm Hg; P=0.000). However, there were no significant differences in the vertical cup-to-disc ratio and the status of glaucomatous optic disc changes between the groups.

    CONCLUSIONS: Prolonged use of intranasal steroids cause statistical significant increase in IOP in patients with allergic rhinitis although no significant glaucomatous disc changes were seen. We suggest patients on long-term use of intranasal steroid have a yearly eye examination to be monitored for IOP elevation and those with additional risk factors for glaucoma is closely monitored for glaucoma.

    Phenytoin (PHT) is a common cause of severe cutaneous adverse reactions (SCARs), including Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN) and drug reaction with eosinophilia and systemic symptoms (DRESS). Although HLA-B*15:02 is associated with PHT-induced SJS/TEN (PHT-SJS/TEN) in Han Chinese and Thais, the genetic basis for susceptibility to PHT-induced SCARs (PHT-SCAR) in other populations remains unclear. We performed a case-control association study by genotyping the human leukocyte antigen (HLA)-B alleles of 16 Malay PHT-SCAR patients (13 SJS/TEN and 3 DRESS), 32 PHT-tolerant controls and 300 healthy ethnicity-matched controls. A novel genetic biomarker, HLA-B*15:13, showed significant association with PHT-SJS/TEN (53.8%, 7/13 cases) (odds ratio (OR) 11.28, P=0.003) and PHT-DRESS (100%, 3/3 cases) (OR 59.00, P=0.003) when compared with PHT-tolerant controls (9.4%, 3/32 controls). We also confirmed HLA-B*15:02 association with PHT-SJS/TEN (61.5%, 8/13 cases vs 21.9%, 7/32 controls; OR 5.71, P=0.016) when compared with PHT-tolerant controls. These alleles may serve as markers to predict PHT-SCAR in Malays.

    The mistletoe fig (Ficus deltoidea) is frequently found in several areas of the world, and primarily functions as houseplant or an ornamental shrub. The plant is discovered indigenous generally in Asia tropical region for example Indonesia, Philippines, Malaysia, and Thailand. Scientific studies on the effect of plant growth regulators on cells production from this plant are vital as optimization of cells production may result in effective production of secondary products characterization and output. The growth of cell suspension cultures from this plant shows sigmoidal property. In this work, we model the effect of the plant growth regulator 2,4-dichlorophenoxyacetic acid (2,4-D) on the growth kinetics of the cells from this plant according to the modified Gompertz model. The coefficient of determination showed good agreement between experimental and predicted data with values ranging from 0.97-0.98. The results showed that 2,4-D at 2 mg/L was optimal for achieving the highest cells growth rate. It is anticipated that the growth parameter constants extracted from the modelling exercise will be helpful in the future for additional secondary modelling on the effect of media conditions as well as other factors on cells growth.

    Ficus deltoidea (or commonly known as mistletoe fig) in various parts of the world mainly
    serves as an ornamental shrub or houseplant and found native mainly in Asia tropical region,
    for example, Malaysia, Indonesia, Philippines and Thailand. Studies on the effect of plant
    growth regulators on cells production from this plant is important as optimization of cells
    production can lead to efficient production of secondary products characterization and
    production. An important aspect of the sigmoidal cells growth curve is the growth parameters.
    In this work, we model the effect of picloram (4-amino-3,5,6-trichloropicolinic acid) on the
    growth kinetics of the cell suspension cultures of Ficus deltoidea according to the modified
    Gompertz model. The adjusted coefficient of determination showed good agreement between
    experimental and predicted data with values ranging from 0.97-0.99. Parameters obtained from
    the fitting exercise were maximum cells growth rate (μm), lag time (l) and maximal cells
    production (Ymax). The results showed that picloram at concentrations of 3 mg/L and above
    were optimal for giving the highest cells growth rate measured as PCV (packed cell volume).
    The parameter growth rate obtained from the modelling exercise will be helpful for additional
    secondary modelling implicating the consequence of media conditions as well as other factors
    on the effect of picloram on the growth rate of cell suspension from this plant.

    Introduction: Loneliness is a major social problem among the elderly that has gone through almost all countries in the world. Even though loneliness is seen as the health-threatening but, the loneliness assessment is still marginal- ized in routine medical check-up for every senior citizen. Methods: This is a sequential explanatory mixed-method study with a cross-sectional community survey was conducted among 380 community residence elderly followed by in-depth interview among eight respondents. This study aims to measure the loneliness and to explore loneliness experienced by older adults. Data were collected using the short form of Social and Emotional Loneliness Scale for Adults (SELSA-S) and data were analysed using SPSS (version 23.0) for windows. In the qualitative study, eight re- spondents were interviewed face-to-face and data were analysed using thematic analysis Braun and Clarke (2006). Results: The results revealed that 37.2% of respondents had loneliness. Bivariate analyses showed the age, level of education and marital status were significantly associated with loneliness. The results of multiple logistic regression discovered gender (OR=1.75,95%CI:1.04-2.93; p=0.03) and marital status (OR=2.24,95%CI: 1.33-3.77;p= 0.002) were a significant predictor of loneliness. In a qualitative study, the result showed two main themes were recorded as causes of loneliness among the elderly are ‘changing the lifestyles’ and ‘ineffective communications. The sub-themes included ‘loss of someone who significant’, ‘limited physical mobility’, ‘all alone’ and ‘no support’. Conclusion: The issue of loneliness needs critically to be addressed even though Malaysia has not reached the ‘aging population’, but the loneliness prevalence experienced by the older adult has similarities with other countries that have attained that status.

    The genomics and genetic information of Malaysian rice (Oryza sativa L.) and their mutant lines are very limited. Therefore, the genome resequencing of two drought and submergence tolerant mutant rice lines, NMR152 and NMR151 and their parental line, MR219 was performed using Illumina Hi Seq 4000 platform with 20x sequencing coverage to facilitate the identification of causal mutations and the discovery of candidate variants of single nucleotide polymorphisms (SNPs) from their genome. The identification of SNPs would be useful in developing functional SNP markers related to drought and submergences resistance traits. Genome sequence data (FASTQ format) of the mutants and their parental line have been deposited into the National Center for Biotechnology Information (NCBI) database under sequence read archive (SRA) data with accessions SRR20824202 (NMR152), SRR20995190 (NMR151) and SRR21043964 (MR219), respectively.

    A single dose pharmacokinetic study of a combined antimalarial formulation of mefloquine, sulphadoxine and pyrimethamine (Fansimef) has been performed in 10 healthy adult male Malaysian volunteers. The dose consisted of two tablets containing 250 mg mefloquine base, 500 mg sulphadoxine base and 25 mg pyrimethamine base each. Plasma concentrations of mefloquine and pyrimethamine were measured by GC-ECD, those of sulphadoxine by h.p.l.c. Time to peak concentrations (mean +/- s.d. for mefloquine (5.70 +/- 0.95 h), sulphadoxine (3.75 +/- 2.03 h) and pyrimethamine (3.30 +/- 1.98 h) were similar to those observed by others after administration of the single compounds. This was also true for elimination half-lives (t1/2). The t1/2s for mefloquine, sulphadoxine and pyrimethamine were 387 +/- 98 h, 255 +/- 61 h and 114 +/- 42 h, respectively.
