    BACKGROUND/AIMS: The association between facial injury characteristics and mild traumatic brain injury (mTBI) is unclear. The aim of this study was to determine the association between facial injury characteristics and mTBI.

    METHODS: A retrospective review utilizing patients' medical records at the Universiti Kebangsaan Malaysia Medical Centre (UKMMC), Kuala Lumpur, Malaysia, was performed. Records of subjects with a history of facial trauma from 1 April 2012 to 31 December 2012 were selected. Various levels and surgical speciality records were reviewed to ascertain the diagnoses of facial injuries and mTBI.

    RESULTS: A total of 348 medical records with the diagnosis of facial injuries were included. The prevalence of mTBI among these patients was 41.4% (95% CI 36.2-46.6). The majority of the patients with mTBI were in the age group of 18-25 years old. Patients with or without facial lacerations that were located on the lower face had a significant association with mTBI, P=.001, compared to other types of soft tissue injury. In addition, a statistically significant association was observed between facial fractures occurring to the middle face and mTBI, P=.018. Pearson chi-square test also showed statistically significant association between the severity of facial injuries and mTBI, P=.018.

    CONCLUSIONS: Mild traumatic brain injury should be suspected in patients with facial injuries and particularly those with lower face lacerations, midface fractures, moderate to severe facial injury and presence of multiple injuries.

    This paper proposes an effective global path planning technique for cellular-connected UAVs to enhance the reliability of unmanned aerial vehicles' (UAVs) flights operating beyond the visual line of sight (BVLOS). Cellular networks are considered one of the leading enabler technologies to provide a ubiquitous and reliable communication link for UAVs. First, this paper investigates a reliable aerial zone based on an extensive aerial drive test in a 4G network within a suburban environment. Then, the path planning problem for the cellular-connected UAVs is formulated under communication link reliability and power consumption constraints. To provide a realistic optimization solution, all constraints of the optimization problem are defined based on real-world scenarios; in addition, the presence of static obstacles and no-fly zones is considered in the path planning problem. Two powerful intelligent optimization algorithms, the genetic algorithm (GA) and the particle swarm optimization (PSO) algorithm, are used to solve the defined optimization problem. Moreover, a combination of both algorithms, referred to as PSO-GA, is used to overcome the inherent shortcomings of the algorithms. The performances of the algorithms are compared under different scenarios in simulation environments. According to the statistical analysis of the aerial drive test, existing 4G base stations are able to provide reliable aerial coverage up to a radius of 500 m and a height of 85 m. The statistical analysis of the optimization results shows that PSO-GA is a more stable and effective algorithm to rapidly converge to a feasible solution for UAV path planning problems, with a far faster execution time compared with PSO and GA, about two times. To validate the performance of the proposed solution, the simulation results are compared with the real-world aerial drive test results. The results comparison proves the effectiveness of the proposed path planning method in suburban environments with 4G coverage. The proposed method can be extended by identifying the aerial link reliability of 5G networks to solve the UAV global path planning problem in the current 5G deployment.

    Blockchain introduces challenges related to the reliability of user identity and identity management systems; this includes detecting unfalsified identities linked to IoT applications. This study focuses on optimizing user identity verification time by employing an efficient encryption algorithm for the user signature in a peer-to-peer decentralized IoT blockchain network. To achieve this, a user signature-based identity management framework is examined by using various encryption techniques and contrasting various hash functions built on top of the Modified Merkle Hash Tree (MMHT) data structure algorithm. The paper presents the execution of varying dataset sizes based on transactions between nodes to test the scalability of the proposed design for secure blockchain communication. The results show that the MMHT data structure algorithm using SHA3 and AES-128 encryption algorithm gives the lowest execution time, offering a minimum of 36% gain in time optimization compared to other algorithms. This work shows that using the AES-128 encryption algorithm with the MMHT algorithm and SHA3 hash function not only identifies malicious codes but also improves user integrity check performance in a blockchain network, while ensuring network scalability. Therefore, this study presents the performance evaluation of a blockchain network considering its distinct types, properties, components, and algorithms' taxonomy.

    Anterosuperior temporomandibular joint dislocation is rare. Manual reduction of such dislocation is difficult especially when treatment is delayed. Therefore, it has an increased likelihood of needing surgical intervention to achieve reduction. The authors report a case of an anterosuperior temporomandibular dislocation in a 19-year-old male following a motor vehicle accident. Difficulties were encountered in reducing the dislocation necessitating surgically assisted reduction. The long-term outcome following management by condylotomy is reported. This present report also reviews the literature regarding the classification and management of this uncommon dislocation.

    To identify gender difference in safe and unsafe practice of pesticide handling in tobacco farmers of Malaysia, we conducted a 20-item questionnaire interview on storage of pesticide (4 questions), mixing of pesticide (3 questions), use of personal protective equipment and clothing while spraying pesticide (7 questions), activities during and after spraying of pesticide (5 questions), and maintenance of pesticide sprayer (1 question) in 496 tobacco farmers (395 males and 101 females) in Bachok District, Kelantan, Malaysia. Duration of employment was significantly longer in females than those in males (p<0.001). In addition, proportion with no formal education in females was significantly higher than those in males (p<0.05). The following eight common factors were extracted from the 20 questionnaires by principal components factor analysis after varimax rotation in all farmers: (1) use of personal protective equipment, (2) unsafe work habit, (3) reading and following instructions on pesticide label, (4) security, storage and disposal of pesticide container, (5) safe work habit, (6) proper handling of pesticide and maintenance of pesticide sprayer, (7) use of personal protective clothing, and (8) safe handling of pesticide. Results of analysis of covariance for the eight factor scores of all male and female farmers, controlling for educational level and duration of employment, showed that: (1) factor scores for use of personal protective equipment (p<0.001), use of personal protective clothing (p<0.001) and safe work habit (p<0.001) in females were significantly lower than those in males; (2) conversely, factor scores for reading and following instruction on pesticide label (p<0.001) and proper handling of pesticide and maintenance of pesticide sprayer (p<0.01) in males were significantly lower than those in females; and (3) there were no significant differences in other three factor scores (p>0.05). We therefore conclude that: (1) for female tobacco farmers, choice of personal attire tend to result in lower scores on use of personal protective equipment and personal protective clothing while personal hygiene practices result in lower score on safe work habit; and, (2) for male tobacco farmers, the lower scores on reading and following instruction on pesticide label and mixing pesticide and maintenance of pesticide sprayer in good condition suggests that they were not primarily involved in these activities. It is postulated that these differences in safe and unsafe practices of pesticide handling across gender is related to the choice of personal attire, personal hygiene practices and division of labour within farming households which in turn is influenced by prevailing sociocultural norms in the community.

    OBJECTIVE: This study examined the relationships between psychosocial work factors and risk of WRMSDs among public hospital nurses in the Klang Valley, Malaysia.

    METHODS: We conducted a cross-sectional study among 660 public hospital nurses. A self-administered questionnaire was used to collect data on the occurrence of WRMSDs according to body regions, socio-demographic profiles, occupational information and psychosocial risk factors. 468 questionnaires were returned (response rate of 71%), and 376 questionnaires qualified for subsequent analysis. Univariate analyses were applied to test for mean and categorical differences across the WRMSDs; multiple logistic regression was applied to predict WRMSDs based on the Job Strain Model's psychosocial risk factors.

    RESULTS: Over two thirds of the sample of nurses experienced discomfort or pain in at least one site of the musculoskeletal system within the last year. The neck was the most prevalent site (48.94%), followed by the feet (47.20%), the upper back (40.69%) and the lower back (35.28%). More than 50% of the nurses complained of having discomfort in region one (neck, shoulders and upperback) and region four (hips, knees, ankles, and feet).     This paper describes the preliminary work required to understand cultural differences in palliative care in the United Kingdom and three countries in the Asia-Pacific region, in preparation for a cross-country study. The study is intended to address cultural understandings of palliative care, the role of the family in end of life care, what constitutes good care and the ethical issues in each country. Suggestions are then made to shape the scope of the study and to be considered as outcomes to improve care of the dying in these countries. It is anticipated that the method used to achieve consensus on cross-country palliative care issues will be both qualitative and quantitative. Identifying key priorities in the delivery and quality measures of palliative care will involve participants in focus groups, a Delphi survey and in the development of clinical indicators towards creating standards of palliative care common to the Asian Pacific region.

    The teaching profession is an occupation at high risk for stress. This research attempted to determine the prevalence of stress and the associated factors contributing to stress among teachers in Malaysia. A cross-sectional study was conducted on 580 secondary school teachers in Kota Bharu District. The instrument used to carry out the study was adopted and modified from the Depression, Anxiety and Stress Scale (DASS 21) and Job Content Questionnaire (JCQ). The questionnaire consisted of two parts: Part I consisting non-job factors (socio-demographic characteristics) and Part II consisting of psychosocial factors contributing to stress. Simple and multiple linear regression analysis were carried out. The prevalence of stress was reported as 34.0%. Seventeen point four percent of teachers experienced mild stress. Age, duration of work and psychological job demands were significantly associated with stress level. This study indicates job-related factors did not contribute much to stress among secondary school teachers. Non-job-related factors should be further studied to determine methods for stress reduction in teachers in Malaysia.

    Traumatic pseudoaneurysm involving the maxillary artery is rare. Owing to its anatomic location, internal maxillary artery is usually protected by its surrounding structures. Formation of pseudoaneurysm usually takes place after several weeks to months of the initial injury. In this case, we reported a pseudoaneurysm arising from left internal maxillary artery following blunt injuries within 3 hours after a road accident and the treatment with endovascular embolization with titanium coils prior to open reduction and internal fixation of the fractured mandibles.

    Channel rendezvous is an initial and important process for establishing communications between secondary users (SUs) in distributed cognitive radio networks. Due to the drawbacks of the common control channel (CCC) based rendezvous approach, channel hopping (CH) has attracted a lot of research interests for achieving blind rendezvous. To ensure rendezvous within a finite time, most of the existing CH-based rendezvous schemes generate their CH sequences based on the whole global channel set in the network. However, due to the spatial and temporal variations in channel availabilities as well as the limitation of SUs sensing capabilities, the local available channel set (ACS) for each SU is usually a small subset of the global set. Therefore, following these global-based generated CH sequences can result in extensively long time-to-rendezvous (TTR) especially when the number of unavailable channels is large. In this paper, we propose two matrix-based CH rendezvous schemes in which the CH sequences are generated based on the ACSs only. We prove the guaranteed and full diversity rendezvous of the proposed schemes by deriving the theoretical upper bounds of their maximum TTRs. Furthermore, extensive simulation comparisons with other existing works are conducted which illustrate the superior performance of our schemes in terms of the TTR metrics.

    INTRODUCTION: We aim to study the diagnostic value of electrocardiogram (ECG) in cardiac tamponade.

    METHODS: This study was a single centre, retrospective casecontrol study. We recruited 42 patients diagnosed with cardiac tamponade of various aetiologies confirmed by transthoracic echocardiography and 100 controls between January 2011 and December 2015. The ECG criteria of cardiac tamponade we adopted was as follows: 1) Low QRS voltage in a) the limb leads alone, b) in the precordial leads alone or, c) in all leads, 2) PR segment depression, 3) Electrical alternans, and 4) Sinus tachycardia.

    RESULTS: Malignancy was the most common causes of cardiac tamponade, the two groups were of similar proportion of gender and ethnicity. We calculated the sensitivity (SN), specificity (SP), positive predictive value (PPV), and negative predictive value (NPV) of each ECG criteria. Among the ECG abnormalities, we noted the SN of 'low voltage in all chest leads' (69%), 'low voltage in all limb leads' (67%) and 'sinus tachycardia' (69%) were higher as compared to 'PR depression' (12%) and 'electrical alternan' (5%). On the other hand, 'low voltage in all chest leads' (98%), 'low voltage in all leads' (99%), 'PR depression' (100%) and 'electrical alternans' (100%) has highest SP.

    CONCLUSION: Our study reaffirmed the findings of previous studies that electrocardiography cannot be used as a screening tool for diagnosing cardiac tamponade due to its low sensitivity. However, with clinical correlation, electrocardiography is a valuable adjuvant test to 'rule in' cardiac tamponade because of its high specificity.

    Currently, smart farming is considered an effective solution to enhance the productivity of farms; thereby, it has recently received broad interest from service providers to offer a wide range of applications, from pest identification to asset monitoring. Although the emergence of digital technologies, such as the Internet of Things (IoT) and low-power wide-area networks (LPWANs), has led to significant advances in the smart farming industry, farming operations still need more efficient solutions. On the other hand, the utilization of unmanned aerial vehicles (UAVs), also known as drones, is growing rapidly across many civil application domains. This paper aims to develop a farm monitoring system that incorporates UAV, LPWAN, and IoT technologies to transform the current farm management approach and aid farmers in obtaining actionable data from their farm operations. In this regard, an IoT-based water quality monitoring system was developed because water is an essential aspect in livestock development. Then, based on the Long-Range Wide-Area Network (LoRaWAN®) technology, a multi-channel LoRaWAN® gateway was developed and integrated into a vertical takeoff and landing drone to convey collected data from the sensors to the cloud for further analysis. In addition, to develop LoRaWAN®-based aerial communication, a series of measurements and simulations were performed under different configurations and scenarios. Finally, to enhance the efficiency of aerial-based data collection, the UAV path planning was optimized. Measurement results showed that the maximum achievable LoRa coverage when operating on-air via the drone is about 10 km, and the Longley-Rice irregular terrain model provides the most suitable path loss model for the scenario of large-scale farms, and a multi-channel gateway with a spreading factor of 12 provides the most reliable communication link at a high drone speed (up to 95 km/h). Simulation results showed that the developed system can overcome the coverage limitation of LoRaWAN® and it can establish a reliable communication link over large-scale wireless sensor networks. In addition, it was shown that by optimizing flight paths, aerial data collection could be performed in a much shorter time than industrial mission planning (up to four times in our case).

    The aim of this study was to describe the clinical features and expression of bcl-2, cyclin D1, p53, and proliferating cell nuclear antigen (PCNA) antibodies in syndromic (nevoid basal cell carcinoma syndrome [NBCCS]) and nonsyndromic patients diagnosed with keratocystic odontogenic tumor (KCOT).

    METHODS: This descriptive study comprised 5 patients of KCOT associated with NBCCS and 8 patients of nonsyndromic type treated in the Department of Oral Maxillofacial Surgery, Universiti Kebangsaan Malaysia Medical Centre between years 1998 and 2011. The clinical features (site, size, treatment, and recurrence), demographic characteristics, and immunohistochemistry results using antibodies of bcl-2, cyclin D1, p53, and PCNA were examined. The association of the antibody expression and the type of KCOT was analyzed using Fisher exact test.

    RESULTS: Altogether there were 13 patients, 5 with syndromic KCOT (1 patient met 3 major criteria of NBCCS) and 8 with sporadic KCOT. The age range for syndromic KCT was 11 to 21 years (mean 16.00 years, SD 4.36) and 10 to 54 years (median 24.50 years, interquartile range 19.00) for the nonsyndromic KCOT. Tumor recurrence occurred in 3 patients (7.7%); 1 patient from the syndromic and 2 patients from the nonsyndromic. The most positive expression was observed in PCNA for both the syndromic and nonsyndromic samples and the least positive expression involved the p53.

    CONCLUSION: PCNA, bcl-2 protein, and cyclin D1 expressions could be useful in evaluating the proliferative activity of the tumor and the aggressiveness of the clinical presentation; however, the authors would propose for larger sample size research for more definitive results.

    OBJECTIVE: Vietnamese spend hours travelling on the road using their motorcycles. Their helmets are exposed continuously to sunlight and rain. The objectives of this study were to determine the association between the effect of photo-oxidative degradation (POD) of the outer shells and helmet age on helmet damage. The micro-structural change of the outer shell was also investigated.

    METHODS: This was a prospective, cross sectional study recruiting injured motorcyclists from Hanoi, Vietnam hospital. The participants were interviewed by a trained researcher. The participants' helmets were collected post-crash. Initially, the helmets were examined for their type and external characteristics. A 3 cm × 3 cm cut was made on the helmet in the impacted and non-impacted areas (control). These areas were investigated for evidence of POD and presence of micro-cracks and material disintegration. 50 participants were enrolled. Sources of information included questionnaire and laboratory analyses. The helmet factors of interest were age of the helmet, exposure of helmet to sunlight and rain (duration/day) and history of previous impact. Laboratory analyses included Fourier Transform Infra Red (FTIR) for degradation and scanning electron microscopy (SEM) for micro-structural examination.

    RESULTS: Majority of the helmets was the open-face type, 40 (80.0%). 31 (62.0%) helmets aged less than three years (LTY) and 19 (38.0%) were three years old or more (MTY). 19 (61.3%) of the LTY helmets and 12 (63.2%) MTY helmets showed evidence of POD. The duration of helmet exposure to sunlight was between 93 to 6570 hours (mean 2347.74 hours; SD 1733.39). The SEM showed 15 helmets (30%) with micro-fractures, 21 helmets (42.0%) with material disintegration. Prolonged uv exposure to the ABS helmets resulted in changes in the helmet material in the form of material disintegration and microcracks and this association was statistically significant (p = 0.03).

    CONCLUSION: POD occurs due to routine exposure to the ultraviolet light. Prolonged uv exposure affects outer shell surface material integrity.

    The use of wireless sensor networks (WSNs) in modern precision agriculture to monitor climate conditions and to provide agriculturalists with a considerable amount of useful information is currently being widely considered. However, WSNs exhibit several limitations when deployed in real-world applications. One of the challenges faced by WSNs is prolonging the life of sensor nodes. This challenge is the primary motivation for this work, in which we aim to further minimize the energy consumption of a wireless agriculture system (WAS), which includes air temperature, air humidity, and soil moisture. Two power reduction schemes are proposed to decrease the power consumption of the sensor and router nodes. First, a sleep/wake scheme based on duty cycling is presented. Second, the sleep/wake scheme is merged with redundant data about soil moisture, thereby resulting in a new algorithm called sleep/wake on redundant data (SWORD). SWORD can minimize the power consumption and data communication of the sensor node. A 12 V/5 W solar cell is embedded into the WAS to sustain its operation. Results show that the power consumption of the sensor and router nodes is minimized and power savings are improved by the sleep/wake scheme. The power consumption of the sensor and router nodes is improved by 99.48% relative to that in traditional operation when the SWORD algorithm is applied. In addition, data communication in the SWORD algorithm is minimized by 86.45% relative to that in the sleep/wake scheme. The comparison results indicate that the proposed algorithms outperform power reduction techniques proposed in other studies. The average current consumptions of the sensor nodes in the sleep/wake scheme and the SWORD algorithm are 0.731 mA and 0.1 mA, respectively.

    The mandibulotomy is a procedure that was developed to improve access in tumour resection. This study aimed to investigate the complications associated with mandibulotomy and analyze factors that could affect the risks of developing these complications. The hospital records of all patients who underwent a mandibulotomy as part of their tumour ablative surgery at two major centres in Malaysia were reviewed retrospectively. Demographic, clinical, and complications data were recorded and analyzed. Early postoperative complications occurred in 46.5% of the patients and post-radiation therapy complications in 16.1%. Wound dehiscence (27.9%) and inferior alveolar nerve injury (25.6%) were the common early postoperative complications. Dental injuries (9.7%) and plate exposure/infection (9.7%) were the common post-radiation therapy complications. Furthermore, inferior alveolar nerve injury and early abscess formation were significantly associated with the site of the mandibulotomy. The T-stage of a tumour but not the site of mandibulotomy was significantly associated with tumour margin clearance. Mandibulotomy does pose an added risk of complications for a patient undergoing tumour surgery. The benefits of mandibulotomy in terms of gaining margin clearance could not be proven in this study. The site of mandibulotomy appears to increase the risk of developing an inferior alveolar nerve injury.

    Orbital fractures pose specific challenge in its surgical management. One of the greatest challenges is to obtain satisfactory reconstruction by correct positioning of orbital implant. Intraoperative computed tomography (CT) scan may facilitate this procedure. The aim of this study was to describe the early use of intraoperative CT in orbital fractures repair in our center. The authors assessed the revision types and rates that have occurred with this technique. With the use of pre-surgical planning, optical intraoperative navigation, and intraoperative CT, the impact of intraoperative CT on the management of 5 cases involving a total number of 14 orbital wall fractures were described. There were 6 pure orbital blowout wall fractures reconstructed, involving both medial and inferior wall of the orbit fracturing the transition zone and 8 impure orbital wall fractures in orbitozygomaticomaxillary complex fracture. 4 patients underwent primary and 1 had delayed orbital reconstruction. Intraoperative CT resulted in intraoperative orbital implant revision, following final navigation planning position, in 40% (2/5) of patients or 14% (2/14) of the fractures. In revised cases, both implant repositioning was conducted at posterior ledge of orbit. Intraoperative CT confirmed true to original reconstruction of medial wall, inferior wall and transition zone of the orbit. Two selected cases were illustrated. In conclusion, intraoperative CT allows real-time assessment of fracture reduction and immediate orbital implant revision, especially at posterior ledge. As a result, no postoperative imaging was indicated in any of the patients. Long-term follow-ups for orbital fracture patients managed with intraoperative CT is suggested.

    As nuclear technology evolves, and continues to be used in various fields since its discovery less than a century ago, radiation safety has become a major concern to humans and the environment. Radiation monitoring plays a significant role in preventive radiological nuclear detection in nuclear facilities, hospitals, or in any activities associated with radioactive materials by acting as a tool to measure the risk of being exposed to radiation while reaping its benefit. Apart from in occupational settings, radiation monitoring is required in emergency responses to radiation incidents as well as outdoor radiation zones. Several radiation sensors have been developed, ranging from as simple as a Geiger-Muller counter to bulkier radiation systems such as the High Purity Germanium detector, with different functionality for use in different settings, but the inability to provide real-time data makes radiation monitoring activities less effective. The deployment of manned vehicles equipped with these radiation sensors reduces the scope of radiation monitoring operations significantly, but the safety of radiation monitoring operators is still compromised. Recently, the Internet of Things (IoT) technology has been introduced to the world and offered solutions to these limitations. This review elucidates a systematic understanding of the fundamental usage of the Internet of Drones for radiation monitoring purposes. The extension of essential functional blocks in IoT can be expanded across radiation monitoring industries, presenting several emerging research opportunities and challenges. This article offers a comprehensive review of the evolutionary application of IoT technology in nuclear and radiation monitoring. Finally, the security of the nuclear industry is discussed.

    OBJECTIVES: Natural enzymes mouthwash has been proposed as salivary substitutes to treat xerostomia. This study aims to evaluate the efficacy of the mouthwash to treat xerostomia.

    MATERIALS AND METHODS: A double-blind, parallel group randomised control clinical trial involving N = 49 adult participants with xerostomia was carried out. Intervention group received natural enzymes moisturising mouthwash (with active ingredients lactoferrin, lysozyme, lactoperoxidase and glucose oxidase); while control group received benzydamine mouthwash. Mouthwashes were repacked, labelled with specific code, and were given to participants by third-party. Subjects were instructed to rinse with the mouthwash 4 times per day at a specific period, for 2 weeks. Symptoms of xerostomia were assessed using Xerostomia Inventory at day 0 and 14; together with the assessment of Clinical Oral Dryness Score (CODS), and measurement of resting and stimulated salivary flow rate.

    RESULTS: 48 participants completed the clinical follow-up, and n = 1 had lost of follow-up. From the 48 participants, n = 23 received natural enzymes mouthwash, while n = 25 received benzydamine mouthwash. Intervention group achieved reduction in symptoms of xerostomia from baseline. Intervention group also showed significantly better improvements in the cognitive perception of dry mouth and oromotor function such as chewing, swallowing and speech of the participants; and reduction in waking up at night to drink water (p
