    PURPOSE: This study examined the prevalence of financial toxicity (FT) and associated factors among urologic cancer patients. The association between FT and health-related quality of life (HRQoL) was also investigated.

    METHODS: A total of 429 respondents diagnosed with urologic cancers (prostate cancer, bladder and renal cancer) from Sarawak General Hospital and Subang Jaya Medical Centre in Malaysia were interviewed using a structured questionnaire. Objective and subjective FT were measured by catastrophic health expenditure (healthcare-cost-to-income ratio greater than 40%) and the Personal Financial Well-being Scale, respectively. HRQoL was measured with the Functional Assessment of Cancer Therapy - General 7 Items scale.

    RESULTS: Objective and subjective FT were experienced by 16.1 and 47.3% of the respondents, respectively. Respondents who sought treatment at a private hospital and had out-of-pocket health expenditures were more likely to experience objective FT, after adjustment for covariates. Respondents who were female and had a monthly household income less than MYR 5000 were more likely to experience average to high subjective FT. Greater objective FT (OR = 2.75, 95% CI 1.09-6.95) and subjective FT (OR = 4.68, 95% CI 2.63-8.30) were associated with poor HRQoL.

    CONCLUSIONS: The significant association between both objective and subjective FT and HRQoL highlights the importance of reducing FT among urologic cancer patients. Subjective FT was found to have a greater negative impact on HRQoL.

    This research aims to study the wet torrefaction (WT) and saccharification of sorghum distillery residue (SDR) towards hydrochar and bioethanol production. The experiments are designed by Box-Behnken design from response surface methodology where the operating conditions include sulfuric acid concentration (0, 0.01, and 0.02 M), amyloglucosidase concentration (36, 51, and 66 IU), and saccharification time (120, 180, and 240 min). Compared to conventional dry torrefaction, the hydrochar yield is between 13.24 and 14.73%, which is much lower than dry torrefaction biochar (yield >50%). The calorific value of the raw SDR is 17.15 MJ/kg, which is significantly enhanced to 22.36-23.37 MJ/kg after WT. When the sulfuric acid concentration increases from 0 to 0.02 M, the glucose concentration in the product increases from 5.59 g/L to 13.05 g/L. The prediction of analysis of variance suggests that the best combination to maximum glucose production is 0.02 M H2SO4, 66 IU enzyme concentration, and 120 min saccharification time, and the glucose concentration is 30.85 g/L. The maximum bioethanol concentration of 19.21 g/L is obtained, which is higher than those from wheat straw (18.1 g/L) and sweet sorghum residue (16.2 g/L). A large amount of SDR is generated in the kaoliang liquor production process, which may cause environmental problems if it is not appropriately treated. This study fulfills SDR valorization for hydrochar and bioenergy to lower environmental pollution and even achieve a circular economy.

    Colourants, micropollutants and heavy metals are regarded as the most notorious hazardous contaminants found in rivers, oceans and sewage treatment plants, with detrimental impacts on human health and environment. In recent development, algal biomass showed great potential for the synthesis of engineered algal adsorbents suitable for the adsorptive management of various pollutants. This review presents comprehensive investigations on the engineered synthesis routes focusing mainly on mechanical, thermochemical and activation processes to produce algal adsorbents. The adsorptive performances of engineered algal adsorbents are assessed in accordance with different categories of hazardous pollutants as well as in terms of their experimental and modelled adsorption capacities. Due to the unique physicochemical properties of macroalgae and microalgae in their adsorbent forms, the adsorption of hazardous pollutants was found to be highly effective, which involved different mechanisms such as physisorption, chemisorption, ion-exchange, complexation and others depending on the types of pollutants. Overall, both macroalgae and microalgae not only can be tailored into different forms of adsorbents based on the applications, their adsorption capacities are also far more superior compared to the conventional adsorbents.

    PURPOSE: To study the surgical and oncological outcomes of endoscopic endonasal transpterygoid nasopharyngectomy (EETN) in salvaging locally recurrent nasopharyngeal carcinoma (NPC).

    METHOD: This was a retrospective clinical record review study carried out at a tertiary centre from June 2013 until May 2017. A total of 55 locally recurrent NPC patients (rT1-rT4) underwent EETN performed by single skull base surgeon with curative intention with postoperative adjuvant chemotherapy but without postoperative radiotherapy.

    RESULTS: There were 44 (80.0%) males and 11 (20.0%) females, with mean age of 52.5 years. The mean operating time was 180 min (range 150-280 min). 85% (47/55) of patients achieved en bloc tumour resection. 93% (51/55) of patients obtained negative microscopic margin based on postoperative histopathological evaluation. Intraoperatively, one (1.8%) patient had internal carotid artery injury which was successfully stented and had recovered fully without neurological deficit. There were no major postoperative complications reported. During a mean follow-up period of 18-month (range 12-48 months) postsurgery, five patients (9.1%) had residual or recurrence at the primary site. All five patients underwent re-surgery. One patient at rT3 passed away 6 months after re-surgery due to distant metastasis complicated with septicaemia. The 1-year local disease-free rate was 93% and the 1-year overall survival rate was 98%.

    CONCLUSIONS: EETN is emerging treatment options for locally recurrent NPC, with relatively low morbidity and encouraging short-term outcome. Long-term outcome is yet to be determined with longer follow-up and bigger cohort study. However, a successful surgical outcome required a very experienced team and highly specialised equipment.

    Algal biomass is known as a promising sustainable feedstock for the production of biofuels and other valuable products. However, since last decade, massive amount of interests have turned to converting algal biomass into biochar. Due to their high nutrient content and ion-exchange capacity, algal biochars can be used as soil amendment for agriculture purposes or adsorbents in wastewater treatment for the removal of organic or inorganic pollutants. This review describes the conventional (e.g., slow and microwave-assisted pyrolysis) and newly developed (e.g., hydrothermal carbonization and torrefaction) methods used for the synthesis of algae-based biochars. The characterization of algal biochar and a comparison between algal biochar with biochar produced from other feedstocks are also presented. This review aims to provide updated information on the development of algal biochar in terms of the production methods and the characterization of its physical and chemical properties to justify and to expand their potential applications.
