    BACKGROUND: Although many smokers use electronic cigarettes (e-cigarettes) to quit smoking, most continue to smoke while vaping. This dual use might delay cessation and increase toxicant exposure. We aimed to test the efficacy of a self-help intervention designed to help dual users to quit smoking.

    METHODS: In this three-arm randomised controlled trial we recruited individuals in the USA using Facebook and multimedia advertisements. Included participants were 18 years or older, smoked at least weekly in the preceding year, and vaped at least weekly in the preceding month. We used computer generated randomisation with balanced-permuted blocks (block size 10, with 2-4-4 ratio) to allocate participants to assessment only (ASSESS group), generic smoking cessation self-help booklets (GENERIC group), or booklets targeting dual users (eTARGET group). Individuals in the generic or targeted intervention groups received monthly cessation materials for 18 months, with assessments every 3 months for 24 months. The main outcome was self-reported 7-day point-prevalence smoking abstinence at each assessment point. All randomly allocated participants were included in primary analyses using generalised estimating equations for each of 20 datasets created by multiple imputation. Analysis of the χ2s produced an F test. The trial is registered with ClinicalTrials.gov, NCT02416011, and is now closed.

    FINDINGS: Between July 12, 2016, and June 30, 2017, we randomly assigned 2896 dual users (575 to assessment, 1154 to generic intervention, and 1167 to targeted self-help). 7-day point-prevalence smoking abstinence increased from 14% at 3 months to 42% at 24 months (F7,541·7=67·1, p<0·0001) in the overall sample. Targeted self-help resulted in higher smoking abstinence than did assessment alone throughout the treatment period (F1,973·8=10·20, p=0·0014 [α=0·017]). The generic intervention group had abstinence rates between those of the assessment and targeted groups, but did not significantly differ from either when adjusted for multiple comparisons (GENERIC vs eTARGET F1,1102·5=1·79, p=0·18 [α=0·05]; GENERIC vs ASSESS F1,676·7=4·29, p=0·039 [α=0·025]). Differences between study groups attenuated after the interventions ended.

    INTERPRETATION: A targeted self-help intervention with high potential for dissemination could be efficacious in promoting smoking cessation among dual users of combustible cigarettes and e-cigarettes.

    STATEMENT OF PROBLEM: A comprehensive bibliometric analysis to determine different aspects of the Journal of Prosthetic Dentistry is lacking.

    PURPOSE: The purpose of this bibliometric study was to analyze the characteristics of the Journal of Prosthetic Dentistry between 1970 and 2019.

    MATERIAL AND METHODS: The Web of Science Core Collection was used to retrieve 9 categories of the Journal of Prosthetic Dentistry, including keywords and terms used, cited documents published, the countries and organizations of the authors, references, and sources cited during this period. Data were exported to a software program and analyzed for each 10-year period and for the entire 50 years. The highest 10 in each category were reported. Co-occurrence, couthorships, and linkage were also reported.

    RESULTS: A total of 11 989 records were reached by the search on the Web of Science Core Collection database; of which, 10 638 (92.9%) were included in the analysis. Articles made up 91.1%, of all records, with 217 review documents (1.8%). The most productive decade was 1980 to 1989 with 2936 documents. The total number of citations of all documents (available period 1980 to 2019) including self-citations was 155 112. During the period 1970 to 2019, 14 837 terms were used. The total number of keywords was 4933 (available period 1990 to 2019). There were 15 382 authors, 82 countries, and 2113 organizations identified in articles published in the Journal of Prosthetic Dentistry during this period, with most from the United States. There were 43 027 authors, 95 324 references, and 14 594 sources cited in the Journal of Prosthetic Dentistry during the period surveyed.

    CONCLUSIONS: This bibliometric analysis provided a comprehensive overview of the impactful role of the Journal of Prosthetic Dentistry in contemporary dentistry, particularly in the field of prosthodontics.

    BACKGROUND: World Health Organization has reported fifty countries have now detected the new coronavirus (B.1.1.7 variant) since a couple of months ago. In Indonesia, the B.1.1.7 cases have been found in several provinces since January 2021, although they are still in a lower number than the old variant of COVID-19. Therefore, this study aims to create a forecast analysis regarding the occasions of COVID-19 and B.1.1.7 cases based on data from the 1st January to 18th March 2021, and also analyze the association between meteorological factors with B.1.1.7 incidences in three different provinces of Indonesia such as the West Java, South Sumatra and East Kalimantan.

    METHODS: We used the Autoregressive Moving Average Models (ARIMA) to forecast the number of cases in the upcoming 14 days and the Spearman correlation analysis to analyze the relationship between B.1.1.7 cases and meteorological variables such as temperature, humidity, rainfall, sunshine, and wind speed.

    RESULTS: The results of the study showed the fitted ARIMA models forecasted there was an increase in the daily cases in three provinces. The total cases in three provinces would increase by 36% (West Java), 13.5% (South Sumatra), and 30% (East Kalimantan) as compared with actual cases until the end of 14 days later. The temperature, rainfall and sunshine factors were the main contributors for B.1.1.7 cases with each correlation coefficients; r = -0.230; p < 0.05, r = 0.211; p < 0.05 and r = -0.418; p < 0.01, respectively.

    CONCLUSIONS: We recapitulated that this investigation was the first preliminary study to analyze a short-term forecast regarding COVID-19 and B.1.1.7 cases as well as to determine the associated meteorological factors that become primary contributors to the virus spread.

    MXene based nanomaterial is an uprising two-dimensional material gaining tremendous scientific attentions due to its versatile properties for the applications in electronic devices, power generation, sensors, drug delivery, and biomedicine. However, the cytotoxic effects of MXene still remained a huge concern. Therefore, stringent analysis of biocompatibility of MXene is an essential requirement before introduction to human physiological system. Several in vitro and in vivo toxicological studies have been reported to investigate the interactions between MXenes with living organisms such as microbes, mammalian cells and animal models. The biological response and cytotoxicity reported were dependent on the physicochemical properties of MXene. The biocompatibility and cytotoxicity of MXene were dependent on size, dose, and surface coating. This review demystifies the in vitro and in vivo biocompatibility studies associated with MXene. Various methods proposed to mitigate the cytotoxicity of MXene for in vivo applications were revealed. The machine learning methods were developed to predict the cytotoxicity of experimentally synthesized MXene compounds. Finally, we also discussed the current research gaps of applying MXenes in biomedical interventions.

    Brachytherapy is commonly used in treatment of cervical, prostate, breast and skin cancers, also for oral cancers, typically via the application of sealed radioactive sources that are inserted within or alongside the area to be treated. A particular aim of the various brachytherapy techniques is to accurately transfer to the targeted tumour the largest possible dose, at the same time minimizing dose to the surrounding normal tissue, including organs at risk. The dose fall-off with distance from the sources is steep, the dose gradient representing a prime factor in determining the dose distribution, also representing a challenge to the conduct of measurements around sources.     Present work investigated the effects of processing (homogenization, sterilization) and cold storage on physicochemical properties, in vitro digestion and Caco-2 cellular uptake of bovine milk. Extreme heat sterilization and low temperature storage have significant impact on particle size and phospholipidome of bovine milk. In addition, cold storage of bovine milks led to formation of β' polymorphs crystals and endothermic peak with Toffset higher than body temperature. Processing and cold storage also increased the initial digestibility but reduced the overall digestibility of bovine milk. This might be related to the decreased particle size of the milk fat globules, changed in the phospholipidome of the MFGM and formation of β' polymorphs crystals in frozen milk. It is interesting to note that PE has relatively faster digestion meanwhile SM has relatively slower digestion. HTST milk which demonstrated lesser changed in terms of phospholipidome demonstrated highest cellular uptakes of most fatty acids.

    Floating plastic debris was investigated in the transition region in the North Pacific between 141°E and 165°W to understand its transportation process from Asian coast to central subtropical Pacific. Distribution was influenced primarily by the current system and the generation process of the high concentration area differed between the western and eastern areas. West of 180°, debris largely accumulated around nearshore convergent area and was transported by eddies and quasi-stationary jet from south to the subarctic region. The average was 15% higher than that previously reported in 1989, suggesting an increase in plastic debris in 30 years. East of 180°, debris concentrated in the calm water downstream of the Kuroshio Extension Bifurcation with considerably high concentration (505,032 ± 991,989 pieces km-2), due to the accumulation of small transparent film caused by calm weather conditions, suggesting a further investigation on small plastic (<1 mm) in the subsurface depth in the subtropical North Pacific.

    Angiotensin-converting enzyme (ACE) and its homologue, ACE2, are commonly allied with hypertension, renin-angiotensin-aldosterone system pathway, and other cardiovascular system disorders. The recent pandemic of COVID-19 has attracted the attention of numerous researchers on ACE2 receptors, where the causative viral particle, SARS-CoV-2, is established to exploit these receptors for permitting their entry into the human cells. Therefore, studies on the molecular origin and pathophysiology of the cell response in correlation to the role of ACE2 receptors to these viruses are bringing novel theories. The varying level of manifestation and importance of ACE proteins, underlying irregularities and disorders, intake of specific medications, and persistence of assured genomic variants at the ACE genes are potential questions raising nowadays while observing the marked alteration in response to the SARS-CoV-2-infected patients. Therefore, the present review has focused on several raised opinions associated with the role of the ACE2 receptor and its impact on COVID-19 pathogenesis.

    Severe acute respiratory syndrome corona virus-2 (SARS-CoV-2) due to novel coronavirus disease 2019 (COVID-19) has affected the global society in numerous unprecedented ways, with considerable morbidity and mortality. Both direct and indirect consequences from COVID-19 infection are recognized to give rise to cardio- and cerebrovascular complications. Despite current limited knowledge on COVID-19 pathogenesis, inflammation, endothelial dysfunction, and coagulopathy appear to play critical roles in COVID-19-associated cerebrovascular disease (CVD). One of the major subtypes of CVD is cerebral small vessel disease (CSVD) which represents a spectrum of pathological processes of various etiologies affecting the brain microcirculation that can trigger subsequent neuroinflammation and neurodegeneration. Prevalent with aging, CSVD is a recognized risk factor for stroke, vascular dementia, and Alzheimer's disease. In the background of COVID-19 infection, the heightened cellular activations from inflammations and oxidative stress may result in elevated levels of microthrombogenic extracellular-derived circulating microparticles (MPs). Consequently, MPs could act as pro-coagulant risk factor that may serve as microthrombi for the vulnerable microcirculation in the brain leading to CSVD manifestations. This review aims to appraise the accumulating body of evidence on the plausible impact of COVID-19 infection on the formation of microthrombogenic MPs that could lead to microthrombosis in CSVD manifestations, including occult CSVD which may last well beyond the pandemic era.

    Large vertebrates are extremely sensitive to anthropogenic pressure, and their populations are declining fast. The white rhinoceros (Ceratotherium simum) is a paradigmatic case: this African megaherbivore has suffered a remarkable decline in the last 150 years due to human activities. Its subspecies, the northern (NWR) and the southern white rhinoceros (SWR), however, underwent opposite fates: the NWR vanished quickly, while the SWR recovered after the severe decline. Such demographic events are predicted to have an erosive effect at the genomic level, linked to the extirpation of diversity, and increased genetic drift and inbreeding. However, there is little empirical data available to directly reconstruct the subtleties of such processes in light of distinct demographic histories. Therefore, we generated a whole-genome, temporal data set consisting of 52 resequenced white rhinoceros genomes, representing both subspecies at two time windows: before and during/after the bottleneck. Our data reveal previously unknown population structure within both subspecies, as well as quantifiable genomic erosion. Genome-wide heterozygosity decreased significantly by 10% in the NWR and 36% in the SWR, and inbreeding coefficients rose significantly by 11% and 39%, respectively. Despite the remarkable loss of genomic diversity and recent inbreeding it suffered, the only surviving subspecies, the SWR, does not show a significant accumulation of genetic load compared to its historical counterpart. Our data provide empirical support for predictions about the genomic consequences of shrinking populations, and our findings have the potential to inform the conservation efforts of the remaining white rhinoceroses.

    Objectives: A history of severe hypoglycemia (SH) is associated with cardiovascular (CV) events among patients with type 2 diabetes mellitus (T2DM). In this study, we compared the severity of atherosclerotic coronary artery disease (ACAD) in T2DM patients with and without a history of SH.

    Methodology: We conducted a comparative case-control study involving 28 T2DM patients with a history of SH within the last 5 years with no documented ACAD, and matched them with 28 T2DM patients with no history of SH. All subjects underwent coronary artery calcium scoring (CACS) with or without coronary computed tomographic angiography (CCTA) to evaluate the severity of ACAD.

    Results: A history of SH in T2DM was associated with a higher prevalence of significant ACAD (79% versus 46%, p=0.026). A high CACS (≥100) was seen in a greater number of patients with a history of SH compared to those without (75% versus 43%, p=0.029). Similarly, there was a higher prevalence of obstructive CAD in those with a history of SH compared to those without (72% versus 39%, p=0.036). Median C-reactive protein level was also higher among patients with a history of SH (0.41 mg/dL versus 0.16 mg/dL, p=0.029).

    Conclusion: In patients with T2DM, a history of SH is significantly associated with ACAD compared to those without SH. A history of SH warrants screening for ACAD.

    Extramedullary hematopoiesis (EMH) is a rare cause of adrenal mass. We present a 44-year-old woman who has thalassaemia intermedia, referred to Endocrinology clinic for huge adrenal mass. Along with a paraspinal lesion discovered in this patient, the leading diagnosis was EMH. The patient was treated with hypertransfusion and hydroxyurea, which led to a reduction in the size of the right adrenal mass and paraspinal mass. This case highlights the challenges in managing this rare condition. Although EMH is a rare cause of adrenal mass, the diagnosis must be considered in any patient with a history of a congenital hemolytic disorder, to avoid unnecessary surgical procedures.

    Pituitary stalk lesions can represent a wide range of pathologies. The exact cause is often unknown due to hesitancy to proceed with biopsy. We present a 16-year-old adolescent who presented with delayed puberty, short stature and bilateral cryptorchidism. He was found to have a thickened pituitary stalk of uncertain etiology with partial hypopituitarism (gonadotrophin and growth hormone deficiency) on further assessment. The presence of bilateral cryptorchidism and micropenis represents lack of "mini puberty," a phenomenon of activation of the hypothalamic-pituitary-gonadal (HPG) axis in-utero or within the first few months of life.1 These key clinical features have been useful to establish an early temporal relationship and suggest a congenital origin of disease. This enabled a more conservative approach of surveillance to be employed as opposed to invasive pathological examination with pituitary stalk biopsy.

    Cushing's syndrome due to ectopic adrenocorticotrophic hormone (ACTH) secretion is uncommon, accounting for 9 to 18% of cases; approximately 10% of ACTH producing tumours are caused by thymic carcinomas.1 We describe a young lady who presented with Cushing's syndrome secondary to a primary neuroendocrine tumour (NET) arising from the thymus. She had surgical resection of her primary tumour with remission of her Cushing's syndrome however subsequently went on to have locoregional recurrence followed by distant metastases to her bilateral ovaries. She underwent 6 surgeries including bilateral adrenalectomy and had 3 cycles of chemotherapy over the course of the 8 years since her diagnosis. Due to the rarity and highly aggressive nature of this disease, we highlight the need for a multidisciplinary team approach and use of multiple modalities in the management of our patient. Timely use of bilateral adrenalectomy particularly in young patients is important to prevent further complications and facilitate other treatment modalities.

    Giant parathyroid adenoma (GPA) is defined as adenoma larger than 3.5 g. Twenty-one cases of parathyroid mass >3.5 g in patients with primary hyperparathyroidism who underwent parathyroidectomy in Hospital Putrajaya, Malaysia were identified. Most cases presented with nephrolithiasis. Two cases are reported as parathyroid cancer. GPA has significantly higher serum calcium and iPTH levels and can be asymptomatic. Parathyroid carcinoma patients are frequently symptomatic, with large tumors. Differentiating GPA from parathyroid cancer is important as it determines the subsequent surgical intervention.

    INTRODUCTION: Coronavirus disease-2019 (COVID-19) pandemic has overloaded the healthcare system beyond its functional capacity. Late referral to higher levels of care may be one of the factors associated with higher mortality. Therefore, we aimed to find simple demographic and laboratory parameters which predict the requirement of admission to a critical care unit.

    MATERIALS AND METHODS: A case-control study was undertaken in adult age population >18 years, admitted in a dedicated COVID hospital in South India. A total of 50 patients with severe disease (cases) were compared with 143 mild or asymptomatic cases (controls). Those demographic and laboratory parameters that were found to be significant on univariate analysis were used for multiple logistic regression analysis.

    RESULTS: Univariate analysis of demographic and laboratory data showed higher age, male sex, presence of diabetes mellitus, higher values of C-reactive protein, ferritin, D-dimer, neutrophil-lymphocyte ratio (NLR), and lactate dehydrogenase to be significantly associated with cases. Multivariate logistic regression analysis of these significant variables showed NLR and ferritin to be the independent predictors of the requirement of admission to a critical care unit. The receiver-operating characteristic curve showed an NLR value of 5.2 and a ferritin value of 462 μg/L that were able to predict the requirement of admission in critical care units.

    CONCLUSION: High ferritin and NLR were independent predictors of the requirement of admission in critical care units. NLR is a simple tool that can be used in resource-limited settings for triage and early referral to higher levels of care.

    Background: Coronavirus disease 2019 (COVID-19), caused by the severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2), has turned into a global pandemic with multitudinous health impacts.

    Main body: In light of the higher vulnerability of men to COVID-19 than women, there is rising concerns on the impact of SARS-CoV-2 infection on male fertility and possibilities of seminal contamination and transmission. The pandemic has attributed to the brief suspension of many fertility clinics and pathology laboratories, though many remained functional. Few reports reflect that SARS-CoV-2 can contaminate the semen of COVID-19 patients as well as that of recovering patients. The viral invasion into the testis may be due to the disrupted anatomical barriers of the testis by the inflammatory responses, and the persistence of the virus in the semen may be facilitated by the testicular immune privilege. Since SARS-CoV-2 is an enveloped RNA virus, it is also theoretically possible that this virus can remain viable in the semen samples even after cryopreservation with liquid nitrogen.

    Conclusion: The present review emphasizes the possibilities of seminal dissemination of SARS-CoV-2 and thereby the chances of its sexual transmission. These perceptions and predictions are to facilitate immediate necessary actions to improvise the standard precautionary procedures for laboratory practices, including semen analysis or processing the semen sample for fertility treatments.

    Seagrass habitats are considered to be some of the most biodiverse ecosystems on the planet and safeguard some ecologically and economically important fauna, amongst which are some globally threatened species, including dugong. Malaysian seagrass ecosystems are not widespread, but their existence supports some significant marine fauna. A rigorous zooplankton study was conducted from May 2016 to February 2017, in the seagrass habitat of Lawas, Sarawak, Malaysia, to examine their temporal composition and diversity, together with their ecological influences. A total of 45 zooplankton species from 13 significant groups were recorded in the seagrass habitat. The population density of zooplankton ranged between 2,482 ind/m³ and 22,670 ind/m³ over three different seasons. A single zooplankton copepod was found to be dominant (47.40%), while bivalves were the second largest (31.8%) group in terms of total abundance. It was also noticed that the average relative abundance (0.62) and important species index (62.08) of copepods were higher than for other groups that exist in the seagrass meadow, whereas copepod Parvocalanus crassirostris showed both the highest average relative abundance (0.41) and the highest important species index (41.15). The diversity (H') and richness index of the intermediate season were found to be highest due to favourable physico-chemical conditions. Within the referred seasonal cluster, the wet and dry seasons were almost similar in terms of species abundance, while the intermediate season was distinct, with high species diversity backed by ANOSIM analysis results. Copepod and bivalves formed one group with a common similarity level of 0.80. The CCA (Canonical Correspondence Analysis) model established that abiotic factors, especially turbidity, NO2, rainfall, dissolved oxygen and pH were significantly correlated with abundance of individual groups of zooplankton. Zooplankton assemblage and abundance in Lawas were found to be very rich in multiple seasons, indicating that the productivity of uninterrupted seagrass habitat might be high and the system rich in biodiversity.

    The skin secretion of amphibians is known for its high content of bioactive compounds. These bioactive compounds are essential for the advancement of biomedical industries. Four wild green paddy frogs, Hylarana erythraea, were collected from various habitat types within the Langkawi Archipelago. These frogs' skin secretions were collected, extracted, and analysed for their protein compounds together with biomedical potentials using liquid chromatography-mass spectrometry (LC-MS). The total protein concentration of H. erythraea skin secretions was determined as 0.269 mg/mL. Based on the UniProt (Anura) database, we identified 29 proteins. These proteins were categorised as antimicrobial (AMP) (38%), followed by hormone (17%), enzyme (17%), unreviewed proteins (17%), structural proteins (7%), and regulatory proteins (4%). The AMPs identified were from the family of esculentin-1, esculentin-2, brevinin-1, and frenatin-4, while the hormones belonged to the cholecystokinin group. The enzymes detected were adenylate cyclase 9, the suppressor of tumorigenicity 14 protein homolog, and the HGF activator. The structural proteins belonged to toe pad keratin 2 and Krt5.7 proteins, while the single regulatory protein is CCR4-NOT transcription complex subunit 6-like. These proteins have a wide range of biomedical importance, such as wound healings, facilitate digestions, anti-tumours, and anti-cancer effect.

    INTRODUCTION: Secondhand (SHS) smoke exposure has caused various health problems. Therefore, continuous monitoring of SHS exposure is important to determine the efficacy of various anti-tobacco measure implemented. The study aims to compare the prevalence and factor(s) associated with SHS exposure among secondary school-going adolescents in Malaysia during 2012 and 2017.

    METHODS: We derived data from the Global School Health Survey (GSHS) 2012 and GSHS 2017, which was carried out in Malaysia using multistage sampling to select representative samples of secondary school-going adolescents. Both surveys used similar questionnaires to measure SHS exposure. Descriptive and multivariate logistic regression was used to determine the prevalence and factors associated with SHS exposure.

    RESULTS: Approximately four in ten respondents were exposed to SHS in the past week in both surveys (41.5% in GSHS 2012 and 42.0% in GSHS 2017, respectively). Both surveys revealed a significantly higher SHS exposure among respondents who smoked than among non-smokers and higher among males compared to females. The likelihood of SHS exposure in both surveys was also similar, with a higher likelihood of SHS exposure among smoking adolescents and non-smoking adolescents who had at least one smoking parent/guardian, regardless of their own smoking status. Male adolescents had a higher risk of SHS exposure compared to their female counterparts. Meanwhile, SHS risk also increased with age, regardless of smoking status.

    CONCLUSIONS: Our findings suggested that there were no changes in the prevalence of SHS exposure and recorded only a slight change in the factors associated with exposure to SHS among school-going adolescents in Malaysia between the years 2012 and 2017. A more pro-active, extensive and comprehensive programme should be implemented to address the problem of SHS exposure. Parents should be advised to stop smoking or abstain from smoking in the presence of their children, and smoking cessation interventions are necessary for smoking adolescents and their parents.
