    OBJECTIVES: To highlight a case of nasopharyngeal Respiratory Epithelial Adenomatoid Hamartoma (REAH) in a female patient with chronic nasal block.

    METHODS: A single case report of a female patient with the diagnosis of REAH, detailing her presenting symptoms, clinical findings, management and follow up.

    RESULTS: Histopathological assessment of the excised nasopharyngeal polyp was consistent with a diagnosis of REAH with a discussion on the disease and its current literature reviews.

    CONCLUSION: The incidence of REAH within the nasopharynx remain rare with only few cases described in literature, especially in females.

    Arthrobacter is a coryneform bacterium in the family of Micrococcaceae. Arthrobacter species isolated from hostile environments are capable of producing interesting bioactive compounds, some of which may be a new class of antibiotics. Here, we present the complete genome sequence of Arthrobacter sp. ES1 isolated from Schirmacher Oasis in East Antarctica. Genomic DNA sequencing was performed using the Illumina MiSeq sequencer. Arthrobacter sp. ES1 has a genome size of 3,964,927 bp and a GC content of 65.73%. The raw genome sequences have been deposited in the NCBI Sequence Read Archive database under the accession number, SRR20664316.

    The motor circuit in Parkinson's disease (PD) involves the basal ganglia, thalamus, motor cortex, and cerebellum. Hence, subthalamic nucleus (STN) or globus pallidus internus deep brain stimulation is commonly used in treating refractory Parkinson's patients. During the procedure, the local field potential (LPF) is commonly made along the trajectory of the STN. Two cases were assessed, where an electroencephalographic recording at the sensorimotor cortices was also performed with and without stimulation at the optimal STN electrode site. The 'on' stimulation state associated with clinical improvement correlated with a marked reduction in the late theta (7.5 Hz), alpha (10.5 Hz) (Mu wave), and beta (20 Hz) wave power. Besides, more synchronized and coherent brainwaves were noted when the stimulation was 'on'.

    Accidental foreign body (FB) ingestion is common in the elderly, particularly edentulous and denture wearers. The most commonly ingested FBs are food-related, including fish and chicken bones. While small FBs can pass through the gastrointestinal tract without any complications, large or irregular-shaped FBs usually cause complications. These complications include choking, ulceration, perforation, fistula, abscess formation, or even death. Ingestion of a large chicken bone that reaches the anal canal without causing injury is extremely rare. We present a rare case of accidental chicken bone (xiphoid process) ingestion that manifested itself by projecting from the anus while defecating. Interestingly, the patient did not experience any food choking or abdominal pain, nor did she have any type of dementia. However, the patient could not grind food properly due to the loss of her upper molar teeth, which made her develop a habit of rapid swallowing. On examination, the chicken bone was seen within the anus with a sharp edge embedded in the mucosal wall. While a plain pelvic X-ray failed to display the FB, a pelvic CT scan with a 3D illustration showed its dimensions (5.0 x 2.5 x 3 cm). A plain pelvic CT scan confirmed the presence of the bone-contrast FB within the anal canal without injuring the surrounding anatomical structures. The FB was gently manipulated and successfully extracted after administering 5 ml of lidocaine gel enema. The patient was counseled regarding proper chewing habits, and she was referred to a dental clinic for a professional assessment. Seven days later, the patient was seen again at the clinic for a follow-up and was found to have an uneventful recovery. Maintaining good dental health and proper eating habits, as well as increasing awareness among edentulous individuals and denture wearers, are required to prevent accidental FB ingestions.

    Lycium ruthenicum, commonly known as black goji berry, is a rich anthocyanin source containing a high amount of monoacylated anthocyanins. This study investigates the effect of different extraction methods to extract anthocyanins from black goji berry for food application. Different hot water extraction conditions were applied to investigate the effect of specific substrate: solvent ratio (1:15 and 1:20 (w/v)), extraction time (30 and 60 min) and extraction temperature (40, 50 and 60 °C) on the extraction yield, total anthocyanin content (TAC) and the total phenolic content (TPC) of the anthocyanin extracts. Best hot water extraction conditions for obtaining an anthocyanin extract with high TAC (13.8 ± 1.14 mg CGE/g), TPC (69.7 ± 2.50 mg of GAE/g), and extraction yield (48.3 ± 3.25%) consuming less solvent, time and heat were substrate: solvent ratio of 1: 15 (w/v), extraction temperature of 50 °C, and extraction time of 30 min. The effect of pectinase, ultrasound, and microwave on hot water extraction of anthocyanins from black goji berry was investigated using the best conditions for hot water extraction. Pectinase-assisted extraction [1.5% (w/v) pectinase, substrate: solvent ratio of 1:15 (w/v) at 50 °C for 30 min] was the best extraction method to extract black goji berry anthocyanins demonstrating higher extraction yield, TAC, TPC, and the highest percentage of petunidin-3-O-(trans-p-coumaroyl)-rutinoside-5-O-glucoside.

    A lot of people suffer from disability and death due to unintentional road accidents, which also result in the loss of a significant amount of financial assets. Several essential features of Advanced Driver Assistance Systems (ADAS) are being incorporated into vehicles by researchers to prevent road accidents. Lane marking detection (LMD) is a fundamental ADAS technology that helps the vehicle to keep its position in the lane. The current study employs Deep Learning (DL) methodologies and has several research constraints due to various problems. Researchers sometimes encounter difficulties in LMD due to environmental factors such as the variation of lights, obstacles, shadows, and curve lanes. To address these limitations, this study presents the Encode-Decode Instant Segmentation Network (EDIS-Net) as a DL methodology for detecting lane marking under various environmental situations with reliable accuracy. The framework is based on the E-Net architecture and incorporates combined cross-entropy and discriminative losses. The encoding segment was split into binary and instant segmentation to extract information about the lane pixels and the pixel position. DenselyBased Spatial Clustering of Application with Noise (DBSCAN) is employed to connect the predicted lane pixels and to get the final output. The system was trained with augmented data from the Tusimple dataset and then tested on three datasets: Tusimple, CalTech, and a local dataset. On the Tusimple dataset, the model achieved 97.39% accuracy. Furthermore, it has an average accuracy of 97.07% and 96.23% on the CalTech and local datasets, respectively. On the testing dataset, the EDIS-Net exhibited promising results compared to existing LMD approaches. Since the proposed framework performs better on the testing datasets, it can be argued that the model can recognize lane marking confidently in various scenarios. This study presents a novel EDIS-Net technique for efficient lane marking detection. It also includes the model's performance verification by testing in three different public datasets.

    MXene-based nanofluids are novel trends with improved dispersion stability and thermophysical characteristics over previously established nanofluids. In the present work, the thermohydraulic characteristics of a double pipe heat exchanger (DPHEX) with a Therminol55(TH55)/MXene + Al2O3 nanofluid and various geometrically shaped (triangular, rectangular) ribbed twisted tape (TT) inserts are numerically investigated using the ANSYS Fluent interface. A counter flow arrangement with three different types of inserts (RRTT, TRTT, TT) and TH55/MXene + Al2O3 nanofluid at 0.20 wt% are studied inside the heat exchanger. Adding ribbed inserts to the conventional TT insert enhances the turbulence intensity by creating extra vortices. The thermal boundary layer grows thinner due to increased axial and radial velocity. Due to the substantial flow obstruction, adding ribs increases the overall pressure drop between the inlet and outlet. The maximum increase in Nu is 11.04 % using nanofluid instead of water, whereas the combination of insert and nanofluid exhibited up to 105 % enhancement for rectangular-ribbed TT compared to the plain tube. Nevertheless, the pressure decrease is found to be maximum in rectangular-ribbed TT because of significant flow disruption. This was likewise true with triangular-ribbed TT and TT insert. According to the PEC assessment, the RRTT insert had a maximum PEC value of 1.67 greater than TRTT and traditional TT for both TH55 and nanofluid flowing inside the tube. To summarize, the combination of TH55/MXene + Al2O3 RRTT insert may be a promising choice for improving heat exchanger performance in a new generation efficient thermal system.

    The ongoing global pandemic of COVID-19 has devastatingly influenced the environment, society, and economy around the world. Numerous medical resources are used to inhibit the infectious transmission of the virus, resulting in massive medical waste. This study proposes a sustainable and environment-friendly method to convert hazardous medical waste into valuable fuel products through pyrolysis. Medical protective clothing (MPC), a typical medical waste from COVID-19, was utilized for co-pyrolysis with oil palm wastes (OPWs). The utilization of MPC improved the bio-oil properties in OPWs pyrolysis. The addition of catalysts further ameliorated the bio-oil quality. HZSM-5 was more effective in producing hydrocarbons in bio-oil, and the relevant reaction pathway was proposed. Meanwhile, a project was simulated to co-produce bio-oil and electricity from the co-pyrolysis of OPWs and MPC from application perspectives. The techno-economic analysis indicated that the project was economically feasible, and the payback period was 6.30-8.75 years. Moreover, it was also environmentally benign as its global warming potential varied from -211.13 to -90.76 kg CO2-eq/t. Therefore, converting MPC and OPWs into biofuel and electricity through co-pyrolysis is a green, economic, and sustainable method that can decrease waste, produce valuable fuel products, and achieve remarkable economic and environmental benefits.

    The electro/photocatalytic CO2 reduction reaction (CO2RR) is a long-term avenue toward synthesizing renewable fuels and value-added chemicals, as well as addressing the global energy crisis and environmental challenges. As a result, current research studies have focused on investigating new materials and implementing numerous fabrication approaches to increase the catalytic performances of electro/photocatalysts toward the CO2RR. MXenes, also known as 2D transition metal carbides, nitrides, and carbonitrides, are intriguing materials with outstanding traits. Since their discovery in 2011, there has been a flurry of interest in MXenes in electrocatalysis and photocatalysis, owing to their several benefits, including high mechanical strength, tunable structure, surface functionality, high specific surface area, and remarkable electrical conductivity. Herein, this review serves as a milestone for the most recent development of MXene-based catalysts for the electrocatalytic and photocatalytic CO2RR. The overall structure of MXenes is described, followed by a summary of several synthesis pathways classified as top-down and bottom-up approaches, including HF-etching, in situ HF-formation, electrochemical etching, and halogen etching. Additionally, the state-of-the-art development in the field of both the electrocatalytic and photocatalytic CO2RR is systematically reviewed. Surface termination modulation and heterostructure engineering of MXene-based electro/photocatalysts, and insights into the reaction mechanism for the comprehension of the structure-performance relationship from the CO2RR via density functional theory (DFT) have been underlined toward activity enhancement. Finally, imperative issues together with future perspectives associated with MXene-based electro/photocatalysts are proposed.

    Anogenital Warts (AGWs) are benign proliferations caused by Human Papillomavirus (HPV) infection on the genital or anal areas. Various therapeutic options are available for the treatment of AGWs but there is no best or ideal therapy, and the recurrence of AGWs is significantly high. A promising new therapy that is currently being evaluated is immunotherapy with the intralesional Bacillus Calmette-Guérin (BCG) vaccine. Two cases of a 23-year-old woman and a 41-year-old man were presented with manifestations of condyloma acuminata type AGWs. The patients were immunocompetent and received single dose intralesional BCG vaccine on the largest lesion. Clinical improvements of AGWs lesions were noted starting on the 14th day after receiving therapy by the disappearance of some lesions with no recurrence and side effects. Intralesional BCG vaccine activates the immune system, treats other AGWs lesions that do not receive an intralesional injection, and also prevents recurrence. Although the intralesional BCG vaccine is effective for treating AGWs, further evaluation is still needed for its recurrence.

    In today's fast-shifting climate change scenario, crops are exposed to environmental pressures, abiotic and biotic stress. Hence, these will affect the production of agricultural products and give rise to a worldwide economic crisis. The increase in world population has exacerbated the situation with increasing food demand. The use of chemical agents is no longer recommended due to adverse effects towards the environment and health. Biocontrol agents (BCAs) and biostimulants, are feasible options for dealing with yield losses induced by plant stresses, which are becoming more intense due to climate change. BCAs and biostimulants have been recommended due to their dual action in reducing both stresses simultaneously. Although protection against biotic stresses falls outside the generally accepted definition of biostimulant, some microbial and non-microbial biostimulants possess the biocontrol function, which helps reduce biotic pressure on crops. The application of synergisms using BCAs and biostimulants to control crop stresses is rarely explored. Currently, a combined application using both agents offer a great alternative to increase the yield and growth of crops while managing stresses. This article provides an overview of crop stresses and plant stress responses, a general knowledge on synergism, mathematical modelling used for synergy evaluation and type of in vitro and in vivo synergy testing, as well as the application of synergism using BCAs and biostimulants in reducing crop stresses. This review will facilitate an understanding of the combined effect of both agents on improving crop yield and growth and reducing stress while also providing an eco-friendly alternative to agroecosystems.

    The incorporation of additional functional groups such as bismuth nanoparticles (Bi NPs) into magnetite nanoparticles (Fe3O4 NPs) is critical for their properties modification, stabilization, and multi-functionalization in biomedical applications. In this work, ultrasound has rapidly modified iron oxide (Fe3O4) NPs via incorporating their surface through coating with Bi NPs, creating unique Fe3O4@Bi composite NPs. The Fe3O4@Bi nanocomposites were synthesized and statistically optimized using an ultrasonic probe and response surface methodology (RSM). A face-centered central composite design (FCCD) investigated the effect of preparation settings on the stability, size, and size distribution of the nanocomposite. Based on the numerical desirability function, the optimized preparation parameters that influenced the responses were determined to be 40 ml, 5 ml, and 12 min for Bi concentration, sodium borohydride (SBH) concentration, and sonication time, respectively. It was found that the sonication time was the most influential factor in determining the responses. The predicted values for the zeta potential, hydrodynamic size, and polydispersity index (PDI) at the highest desirability solution (100%) were -45 mV, 122 nm, and 0.257, while their experimental values at the optimal preparation conditions were -47.1 mV, 125 nm, and 0.281, respectively. Dynamic light scattering (DLS) result shows that the ultrasound efficiently stabilized and functionalized Fe3O4NPs following modification to Fe3O4@Bi NPs, improved the zeta potential value from -33.5 to -47.1 mV, but increased the hydrodynamic size from 98 to 125 nm. Energy dispersive spectroscopy (EDX) validated the elemental compositions and Fourier transform infrared spectroscopy (FTIR) confirmed the presence of Sumac (Rhus coriaria) compounds in the composition of the nanocomposites. The stability and biocompatibility of Fe3O4@Bi NPs were improved by using the extract solution of the Sumacedible plant. Other physicochemical results revealed that Fe3O4NPs and Fe3O4@Bi NPs were crystalline, semi-spherical, and monodisperse with average particle sizes of 11.7 nm and 19.5 nm, while their saturation magnetization (Ms) values were found to be 132.33 emu/g and 92.192 emu/g, respectively. In vitro cytotoxicity of Fe3O4@Bi NPs on the HEK-293 cells was dose- and time-dependent. Based on our findings, the sonochemical approach efficiently produced (and RSM accurately optimized) an extremely stable, homogeneous, and biocompatible Fe3O4@Bi NPs with multifunctional potential for various biomedical applications.

    OBJECTIVE: The present study aimed to investigate the relationship between olfactory sulcus (OS) depth and olfactory function considering age and gender and to provide normative data on OS depth in a population with normal olfactory function.

    MATERIALS AND METHODS: OS depth was obtained using T1 magnetic resonance imaging scans. Participants (mean age ± sd = 57 ± 16 years, ranging from 20 to 80 years) were screened for olfactory function using the Sniffin' Sticks Screening 12 test. They were divided into an olfactory dysfunction group (n = 604) and a normosmia group (n = 493). Participants also completed questionnaires measuring depression, anxiety and quality of life.

    RESULTS: The right OS was deeper than the left side in all age groups. On the left side, women had deeper OS compared with men, exhibiting a higher degree of symmetry in left and right OS depth in women. Variance of olfactory function was largely determined by age, OS depth explained only minor portions of this variance. Normative data for minimum OS depth was 7.55 mm on the left and 8.78 mm on the right for participants aged between 18 and 35 years (n = 144), 6.47 mm on the left and 6.99 mm on the right for those aged 36-55 years (n = 120), and 5.28 mm on the left and 6.19 mm on the right for participants older than 55 years (n = 222).

    CONCLUSION: Considering the limited resolution of the presently used T1 weighted MR scans and the nature of the olfactory screening test, OS depth explained only minor portions of the variance of olfactory function, which was largely determined by age. Age-related normative data of OS depth are presented as a reference for future work.

    In this work, chitin (Ch) was chemically extracted from wild mushrooms and then grafted to polyaniline (PANI) to form a composite (Ch-g-PANI) to detect ammonia (NH3) gas. The Ch-g-PANI was comprehensively characterized using Scanning electron microscopy (SEM), elemental mapping, thermogravimetric analysis (TGA), and Fourier transform infrared spectroscopy (FTIR) and UV-Vis spectroscopy. The NH3 gas detection optimization was evaluated using Box-Behnken Design. Typically, physical factors such as (A)film layer, (B)loading %, and (C)contact time were investigated and validated through the analysis of variance (ANOVA). The ANOVA revealed that dual interactions between (A)film layer - (C)contact time, and (B)loading % - (C)contact time are among the significant factors. By considering these significant interactions, the highest sensitivity was obtained when (A)film layer (3), (B)loading (5 %), and (C)contact time (10 min) in NH3 gas detection. Then, the optimized Ch-g-PANI was tested in the linear range of NH3 gas concentration from 10 to 50 ppm, which resulted in a linear calibration curve with R2 = 0.994 and a detection limit of 15.03 ppm. Sensor performances showed that Ch-g-PANI films possess high selectivity for NH3 gas among the common interfering gases and the film can be reused for up to 6 cycles. Therefore, the new mushroom-sourced Ch-g-PANI is an inexpensive and economical sensor in the NH3 gas sensor field.

    AIM: To investigate the existence of a bidirectional temporal relationship between periodontal condition and glycaemic status.

    MATERIALS AND METHODS: This longitudinal study included 2198 participants with mean age 43.4 ± 7.7 years, who underwent dental examinations in Yokohama, Japan, at two time points, 2003-2004 and 2008-2009, at an interval of 5 years. Periodontal condition was assessed by the mean value of probing pocket depth (PPD) and clinical attachment level (CAL). Glycaemic status was assessed by fasting glucose and glycated haemoglobin (HbA1c).

    RESULTS: The cross-lagged panel models showed the effect of HbA1c at baseline on mean PPD at follow-up (β = 0.044, p = .039). There was a marginal effect of fasting glucose on the mean PPD (β = 0.037, p = .059). It was similar to the effect of fasting glucose or HbAlc on mean CAL. However, in the opposite direction, no effect of mean PPD or CAL at baseline on fasting glucose or HbAlc at follow-up was identified.

    CONCLUSIONS: This study demonstrated a unidirectional relationship between glycaemic status and periodontal condition. The study population, however, had mostly mild periodontitis. Future studies are needed to investigate the effect of periodontal condition on glycaemic status in patients with severe periodontitis.

    Jellyfish stings are the most common cause of marine envenomation in humans. Various species of box jellyfish have been identified around Penang Island, Malaysia, and these include multitentacled and four-tentacled box jellyfish (class Cubozoa). The typical syndrome following envenomation from these jellyfish has been poorly documented, posing a greater challenge when managing an unidentified jellyfish sting from Penang Island. We report a case of a 32-y-old man from Penang Island who was stung by an unidentified jellyfish while walking into the sea. The patient reported that he felt an immediate and severe electric current‒like pain over both thighs, left flank, and left forearm, followed by chest discomfort and breathlessness. Vinegar was applied over the affected areas, and he was rushed to a hospital, where he was treated with analgesia, steroids, and antihistamine. He refused hospitalization and was discharged against medical advice. He then presented to a noncoastal hospital 377 km away in Kuala Lumpur on the following day with severe pain over the affected sites as well as chest discomfort, shortness of breath, and abdominal cramps. The electrocardiograph demonstrated features of Wolff-Parkinson-White. Serial blood test results showed elevated creatine kinase but normal troponin I levels. The patient was managed symptomatically over a period of 4 d and was discharged with cardiology follow-up. Appropriate health-seeking behavior needs to be emphasized. This case report provides an opportunity to document the signs and symptoms of envenomation from possibly an undescribed jellyfish species near the coastal waters of Penang Island.

    INTRODUCTION: A scaled-up treatment cascade with direct-acting antiviral (DAA) therapy is necessary to achieve global WHO targets for hepatitis C virus (HCV) elimination in Malaysia. Recently, limited access to sofosbuvir/daclatasvir (SOF/DAC) is available through compulsory licensing, with access to sofosbuvir/velpatasvir (SOF/VEL) expected through voluntary licensing due to recent agreements. SOF/VEL has superior clinical outcomes but has higher drug acquisition costs compared to SOF/DAC. A stratified treatment cascade might be the most cost-efficient approach for Malaysia whereby all HCV patients are treated with SOF/DAC except for patients with cirrhosis who are treated with SOF/VEL.

    METHODS: This study aimed to conduct a 5-year budget impact analysis of the proposed stratified treatment cascade for HCV treatment in Malaysia. A disease progression model that was developed based on model-predicted HCV epidemiology data was used for the analysis, where all HCV patients in scenario A were treated with SOF/DAC for all disease stages while in scenario B, SOF/DAC was used only for non-cirrhotic patients and SOF/VEL was used for the cirrhotic patients. Healthcare costs associated with DAA therapy and disease stage monitoring were included to estimate the downstream cost implications.

    RESULTS: The stratified treatment cascade with 109 in Scenario B was found to be cost-saving compared to Scenario A. The cumulative savings for the stratified treatment cascade was USD 1.4 million over 5 years.

    DISCUSSION: A stratified treatment cascade with SOF/VEL was expected to be cost-saving and can result in a budget impact reduction in overall healthcare expenditure in Malaysia.

    INTRODUCTION: Healthcare workers (HCWs) have been continually exposed to patients with COVID-19 and are at higher risk of contracting the disease. Their psychological health is important for overall wellbeing and productivity, which could lead to a reduction in human errors during the pandemic crisis. This study aimed to measure the level of concerns, work practices, adequacy of preventive measures among HCWs, and the impacts on their life and work, including mental health status during the second wave of the COVID-19 pandemic in Malaysia.

    METHODS: An online questionnaire was distributed randomly to 1,050 HCWs from the Ministry of Health facilities in the Klang Valley who were involved directly in managing or screening COVID-19 cases from May to August 2020. The questionnaire was divided into five domains, which were concerns, impact on life and work, practice, perceived adequacy of preventive measures, and Revised Impact of Event Scale (IES-R). Logistic regression was used to identify sociodemographic predictors of the five domains.

    RESULTS: A total of 907 respondents (86.4%) participated in this survey. Approximately half of the respondents had a low concern (50.5%), most of them had a good practice (85.1%), with 67.5% perceiving there were adequate preventive measures, and they perceived the outbreak had a low impact (92%) on their life and work. From the IES-R domain, 18.6% of respondents potentially suffered from post-traumatic stress disorder (PTSD).

    CONCLUSION: During the second wave of the COVID-19 outbreak in Malaysia, HCWs practiced high levels of precautions and preventive measures because they were aware of the risk of infection as an occupational hazard. With the adequate implementation of policy and control measures, the psychological wellbeing of the majority HCWs remained well and adequately supported.

    INTRODUCTION: The theory of planned behavior (TPB) has dominated the limited number of green hotel visitation studies; however, those studies' findings are often inconclusive or even controversial. Thus, research needs to move beyond using the TPB to measure consumers' intention and behavior, and to experiment with alternative theoretical frameworks to explain behavioral change. Value-belief-norm theory of environmentalism (VBN) proposed that various facets of values can influence individuals' beliefs, subsequently effecting their moral obligations, ultimately, their pro-environmental behaviors. Hence, this study aims to examine the relationship between value components (i.e., biospheric, altruistic, collectivistic), beliefs (i.e., explicit and implicit attitude), norms (i.e., social and personal norm), and green purchase intention to visit green hotels.

    METHODS: An online survey of convenience sampling technique was adopted for data collection. A total of 373 valid questionnaires were subjected to descriptive analysis, and confirmatory factor analysis and structural equation modeling were performed for the testing of the hypotheses.

    RESULTS: The results suggested that biospheric and collectivistic value positively influence explicit environmental attitude while altruistic value positively influences intrinsic environmental attitude, but negatively influences extrinsic environmental attitude. Social norm was shown to have a positive impact on personal norm and green purchase intention. Furthermore, implicit environmental attitude was shown to influence personal norm and intention, while personal norm positively influences green purchase intention to visit green hotels.

    DISCUSSION: This study provided an alternative perspective on the selection of green hotels among consumers based on value-belief-norm theory in the tourism literature. These empirical findings would greatly benefit green hotel managers and other key stakeholders in the hospitality industry.
