Background: The Medical Faculty of the International Islamic University Malaysia (IIUM) runs an undergraduate medical programme which is a combination of the traditional and the newer trends of medical curriculum. The IIUM curriculum in Phase I (Years 1, 2: preclinical) is integrated organ system based, with lectures being the main method of curriculum delivery and also incorporating problem-based learning (PBL) as one of the teaching-learning approaches. Methods: The aim of this study is to examine the effectiveness of PBL teaching-learning method in IIUM in achieving the aims of the medical school as viewed by the students. A total of 287 students from Phase I and Phase II (Year 3: clinical) participated in this study at the end of academic session 2006/2007. The questionnaires distributed consisted of 33 items with four different aspects of PBL being evaluated namely; i) PBL objectives, ii) assessment of performance of the facilitators, iii) assessment of the PBL packages and iv) preferred teaching-learning approach. Results: An average of 86.4 % of students agreed that PBL sessions achieved its aims of preparing students for professional training in the clinical years. Also approximately 80% of students felt that the facilitators fulfilled their roles as PBL tutors. Students were generally satisfied with the PBL packages prepared. However about 50% of Year 2 students did not look forward to PBL sessions and only about 20% of preclinical students wanted more PBL sessions to be scheduled. This latter finding differed as 60% of the clinical (Year 3) students wanted more PBL sessions to be implemented. A significant proportion of the students also believed that they needed some prior information about the PBL case in question in order to generate lively exchange of ideas in the first sessions. Preclinical students preferred lectured-based approach as compared to PBL in contrast to the clinical (Year 3) students of whom 57% (62) preferred the PBL teaching-learning approach. Nearly 80% of the Year 3 students found the exposure to PBL sessions during their preclinical years beneficial during the clinical year. Discussion: Although our findings supported the effectiveness of PBL as one of the teaching-learning approaches (as perceived by our students) there are a few areas of concern. These include believing that a prior knowledge of the topic in question is needed to generate a lively discussion; the group not being motivated; the group members confining themselves to individual assignments only; PBL not being supportive of on-going self- assessment; the consequence of a facilitator being too dominant; and cases constructed do not reflect real clinical scenarios. Despite agreeing to its effectiveness preclinical students still preferred lecture-based approach to the PBL as they are more confident with the knowledge as obtained through the former learning modality. Conclusion: PBL is an effective learning strategy and it contributes to the teaching-learning process of IIUM undergraduate preclinical medical programme which implements a non-Problem Based Curriculum. We are of the opinion that a combination of lecture sessions and PBL best suit the IIUM medical students.

INTRODUCTION: Calcaneal fractures are caused by high energy trauma and mostly are intra-articular fractures. Nondisplaced intra-articular calcaneal fracture (IACF) can be treated non-operatively. However, displaced intra-articular need to be reduced and fixed anatomically to facilitate early ankle rehabilitation and minimize functional impairment. This study was done to find out the outcome of the IACF patients who underwent operative treatment.

METHODS: 62 patients with IACF were selected in this study and had been followed up from June 2009 to May 2013. They were placed into two groups; the operative treated and non-operative treated groups. Bilateral ankle lateral view plain radiographs were taken for comparison of the Bohler and Gissane angles. Both groups of patients were assessed by the Maryland Foot Score (MFS) and the SF-36v2 general health survey questionnaire. The ability of the patients to perform activity of daily living (ADL) and /or return to work (RTW) was assessed as well.

RESULTS: The operative treatment group of displaced IACF patients achieved no significant better scores in the mean MFS and SF-36v2 mean scores as compared to non operated cases. There was no difference in RTW between the 2 groups, but earlier ADL was recorded in the operated group. However, this study had found 5 associated factors which causes major effect to the patients' outcome to treatment.

CONCLUSIONS: The patient's compliance with post-operative rehabilitation regimen were found to be significantly related with the outcomes.

OBJECTIVES: Traditionally, scoliosis surgery is aimed at attaining a fused, balanced and painless spine. With improvement in surgical and instrumentation techniques, spine surgeons strive to achieve higher degree of Cobb's angle and rib hump correction with the idea of greater patient satisfaction. The aim of this study was to determine the patients' satisfaction using SRS-22 questionnaire and their correlations with the radiographic changes.

MATERIALS AND METHODS: A prospective study was conducted in a tertiary referral cen re using the Scoliosis Research Society-22 (SRS-22) questionnaire during the patients' annual follow up, betwee February to April 2014. Thirtyseven patients who met the inclusion criteria were enrolled.

RESULTS: The mean pre-operative Cobb's angles were 57.8o ± 12.7o and mean post-operative angle of 20.0o ± 10.4o, resulting in average correction of 65.9 ± 14.4%. Mean preoperative rib hump was 61.1 ± 15.4 mm with mean postoperative rib hump of 15.8 ± 17.8 mm, resulting in average reduction of 77.7 ± 23.7%. Mean of post-operative total SRS score was 4.1 ± 0.5. Using Spearman rank correlation, the percentage of Cobb's angle correction versus the SRS-22 score showed correlation of 0.17 (P=0.33) while the percentage of rib hump reduction versus SRS-22 score showed a correlation of 0.11 (P=0.53).

CONCLUSION: In this study, the average total SRS-22 score was 4.1 ± 0.5 (range, 3.1-4.9) post-operatively indicating very high satisfaction rate overall. Despite attempts at greater curve correction and rib hump reduction, there is no direct correlation between patient satisfaction and radiographic parameters.

Objectives: To study relationship of depressive symptoms to hypertension. Methods: A cross sectional study was carried out on respondents from Kampung Awal. Hypertension was defined as self-reported or by measurement using a mercury sphymomanometer and depressive symptoms were evaluated by CES-D scale. Results: A total 103 respondents were studied. The male:female ratio was 1:1.5 with mean age of 41.8 years. Majority had formal education up to secondary school. 45% were government servant and over 95% subjects had income of RM1000 or less per each family member per month. 21.4% (n=22) self-reported to have hypertension and 7.8%(n=91) met the criteria for incident hypertension. 7.8% met the criteria for both. Mean score based on CES-D was 13.08. 32.0% subjects had depression. 54 subjects reported never experienced any strain for past 6 months and 24 reported to have 2 or more .61 subjects were overweight with mean value of BMI=26.19 kg/m2. Self-reported hypertension and incident hypertension were strongly associated with age but only selfreported hypertension was strongly associated with overweight. However, there was no significant association between hypertension with chronic strains and depressive symptoms. Conclusions: Depressive symptomatology was not associated with hypertension in this survey. Women had a significant risk in having depressive symptoms. Age =45 years & overweight were significant risks for hypertension. A prospective study will be more useful in the future to find out the association between depression and hypertension.

Study on motivational readiness for change is crucial to promote understanding of behavioural change among Methadone Maintenance Tretment (MMT) patients. A widely used method recently is via Stages of Change and Treatment Eagerness Scale for drug abusers (SOCRATES-8D). The aims of this study were to; 1) determine the general level of readiness for change, 2) assess differences in terms of readiness to change (RtC) based on socio-demography and clinical characteristics and 3) compare RtC with different health-related quality of life (HRQOL) levels. Methods: A convenient sample of MMT volunteers from Terengganu, Malaysia was enrolled. The SOCRATES-8D was administered (3 subscales; Likert-type responses 1-5; higher score, better RtC). Data was analysed using SPSS 15, employing descriptive statistics and non-parametric tests for score comparisons. Results: The mean age of 55 Malay respondents was 37.0 years, male (98.2%), = lower secondary qualification (65.5%) and addiction period > 15 years (52.7%). Generally the Recognition level was "low", Ambivalence and Taking Steps were moderately-rated. Most of them within unsatisfactory levels of RtC. Abusers with < 20 months treatment were significantly "more ready" for behavioural changes (p0.05). Conclusion: Findings demonstrated that patients were rather ready to adopt positive behavioural changes regardless of their sociodemographic backgrounds. Thus continuous efforts and psychosocial support from various authorities should be geared towards enhanced readiness as part of ensuring the success of MMT programme in the future.

This two-phase study examined the prevalence of depressive disorders among married working women in a sub-urban area in Malaysia. The Malay version of the General Health Questionnaire-12 (GHQ-12) was used for initial screening. Using multistage cluster sampling, there were 700 subjects from 25 workplaces that were approached, of which 307 filled out GHQ-12 in the initial phase. The prevalence of psychological distress (GHQ 2/3 cut off) is 22.8% (n=70, 95% CI 18.1-27.5). In the second phase, all 49 consenting patients who were screened positive for severe psychological distress (GHQ 3/4 cut off point) were telephone-interviewed by a trained psychiatrist with the Malay version of Mini International Neuropsychiatric Interview (M.I.N.I) to diagnose depressive disorders. The prevalence of depressive disorders was 5.5% (n=17, 95% CI 3.5-7.5). The prevalence of depressive disorders is comparable to those found in the working population worldwide.

Objective: To validate the Malay version of Berlin Questionnaire (BQ) as a tool to screen for patients at risk of obstructive sleep apnea (OSA) in primary care Background: Most patients with OSA are unrecognised and untreated. Thus, the BQ has been used as a tool to screen for patients at risk for OSA. However, this tool has not been validated in Malay version. Materials and Methods: A parallel back-to-back translation method was applied to produce the Malay version (Berlin-M). The Malay version was administered to 150 patients in a tertiary respiratory medical centre.  Concurrent validity of the Berlin-M was determined using the Apnea Hypopnea Index (AHI) as the gold standard measure. The test-retest reliability and internal consistency of the Berlin-M were determined. Results: Most patients were males (64.0%) and majority of them were Malays (63.3%). Based on the sleep study test, 121 (84.0%) were classified as high risk while 23 (16.0%) as low risk using the Apnea Hypopnea Index (AHI) ≥5 as the cutoff point. The test–retest reliability Kappa value showed a good range between 0.864 – 1.000. The Cronbach’s alpha of BQ was 0.750 in category 1 and 0.888 in category 2. The sensitivity and specificity were 92% and 17% respectively. Conclusion: The BQ showed high sensitivity (92%) but low specificity (17%). Therefore, though the Berlin-M is useful as a screening tool, it is not a confirmatory diagnostic tool.

Objective: Globally, there is growing evidence of stigmatization of people with mental illnesses and mental illness is more stigmatizing than physical illnesses. Therefore, this study aimed to determine the attitudes of university students towards persons with mental illness. Methods: A cross-sectional study was conducted among Management and Science University students during the academic year 2012. Prior ethical approval was taken for conducting the study. A total of 300 medical and health science students was recruited in this study and the response rate was 93%. The survey questionnaire was adopted from a previous study with 15 statements about attitude towards mental health illness and sociodemographic characteristics. Classes were chosen randomly and students also randomly selected. The inclusion criterion for this research was medical and health science students while the exclusion criterion was students from other faculties, staff and lecturers. Data was analyzed with the Statistical Package for the Social Sciences version 13.0. Descriptive statistics were used for the analysis of the basic demographics and survey items of the questionnaire. T-test and multiple linear regression were used to explore the relationship between the socio-demographic and the attitude. Results: A total number of 279 students participated in this study. The majority of them were female, age 20 years and above, single, Malay, from International Medical School (IMS) and from urban areas (68.5%, 88.2%, 96.4%, 58.1%, 60.2%, 77.8% respectively). In this study, the majority of the participants showed a moderate to good attitude towards people with mental illness. Univariate analysis showed that marital status influenced the attitudes of university students towards people with mental illness (p=0.015). In multivariate analysis, multiple linear regression showed that gender, marital status, smoking and drinking alcohol significantly influenced the attitude of university students towards people with mental illness (p=0.014, p=0.012; p=0.009, p=0.013; respectively). Conclusion: This study showed that the undergraduate university students had a moderate to good attitude towards people with mental illness. Gender, marital status, smoking and drinking alcohol significantly influenced the attitude of undergraduate university students towards people with mental illness. The community placement programme was first introduced to our medical students in 2007. The objectives of this community service programme are to enable students to explore and understand the importance of various skills such as leadership, teamwork and interest towards community services, as well as to improve their skills in those areas for becoming better students and future doctors. The first year medical students were tasked to plan, organize and implement activities in selected communities such as disabled people, orphans, neglected elderly, the poor, HIV positive single mothers and children. Amongst the activities conducted were sharing experiences and thoughts, games, donation and ‘gotong-royong’. The objective of this study was to evaluate the students’ perception on the successfulness of the community placement programme in building their professional qualities. A cross-sectional study was conducted among the first year medical students using a self-administered questionnaire. The questionnaire consists of 5 elements of professional qualities such as leadership and team work skills, interest towards community services, volunteerism and empathy. Self-reflection sessions were also held to explore the learning points gained. A total of 147 students answered the questionnaires. The students rated the overall programme as highly useful (80.6%) and as achieving the objectives (80.1%). They perceived that this programme helped them to improve their personal and professional skills such as leadership (70.0%), team work (71.4%), interest towards community services (87.1%), volunteerism (85.0%), and empathy (89.1%). Self-reflection revealed that the programme made them realize the role of doctors in a community, appreciate the spirit of teamwork and helped them to understand the need of vulnerable groups. As a conclusion, this programme was well-accepted and perceived as assisting medical students to build professional qualities to become caring and competent doctors.

Exposure to traffic air pollutant have shown a significant health effect on respiratory systems and decreased in lung function among traffic policemen. The main objective of this study was to determine the relationships between personal exposure levels to PM2.5 and respiratory health among traffic policemen working at Traffic Police Station in Kuala Lumpur and general duty policemen attached to Police Headquarters, Bukit Aman. A cross sectional comparative study was conducted among 50 traffic policemen from Traffic Police Station Kuala Lumpur and 50 general duty policemen from Police Headquarters Bukit Aman as comparative group. A purposive sampling method was used to select the respondents based on inclusive criteria such as age between 25 to 60 years, no history of respiratory disease and working not less than 3 years as traffic policemen. Questionnaire based on ATS (1978) was used to collect information on socio-demographic and respiratory symptoms. Spirometer (Spirolab II Model) was used to perform lung function tests. Personal Air Sampling Pump (Aircheck 52) was used to measure personal exposure level to PM2.5. The mean exposure level of PM2.5 among the traffic policemen was 22.33 ± 8.54μg/m³ compared to only 5.60 ± 4.29μg/m³ for comparative group. There was a significant difference in all lung function parameters between the exposed group and comparative group. From the finding, it shows that there was significant relationship between working duration (years) and lung function parameters among both exposed and comparative group. The result from this research shows that traffic policemen were determined as having lower lung function parameters due to their nature of work and the environment. Also, there was a significant association between exposure to fine particle (PM2.5) and lung function among the exposed group. Finding from this study indicated that exposure to elevated concentration level to traffic related air pollutant was the risk factors in the development of respiratory diseases as shown by the higher prevalence of respiratory symptoms and the reduction in lung function among traffic policemen.
