    The case notes of twelve jaundiced patients, on whom percutaneous transhepatic cholangiography (PTC) were performed, are reviewed. PTC was carried out to differentiate the patients with intrahepatic cholestasis from those with extrahepatic biliary obstruction, and to identify the site and nature of the block. In eleven cases, the biliary trees were visualised, with the sites of obstruction in those present demonstrated and confirmed at subsequent laparotomies. There was no serious side effect from the procedure. PTC in our hands has proved an invaluable aid in the investigation of the icteric patients.

    The role of multiple pleural biopsies in malignant pleural effusion to achieve better diagnostic yield was evaluated. The specific diagnosis was established in 43.8 percent (14/32) of cases on first biopsy, and improved to 93.8 percent (30/32) of cases by three biopsies. The contribution of multiple biopsies was stressed.

    A review of our experience of 916 cases ofpartial bilateral vasectomy performed under local anaesthesia at the University Hospital, Kuala Lumpur during the
    period between January 1971 and June 1980 is presented. Minor post-operative complications occurred in 5.8% of cases with a low post-operative infection rate of 1.2%. In 35% ofpatients, persistence of non-motile sperms in the semen were seen after at least ten ejaculations in each instance. In four of these cases non-motile sperms continued to persist into the sixth month of follow-up even after at least 50 ejaculations in each instance.

    The relevant investigations and management of a case of alpha-thalassaemia major suspected antenatally is discussed. The value of ultrasonically guided cordocentesis in the definite diagnosis of this condition is emphasised in the management of this pregnancy. We believe that this is the first time such a procedure has been done in this country.

    Eleven consecutive cases of simple transposition of great arteries were palliated by balloon atrial septostomy, The arterial saturation was improved and the gradient across the atrial septum was reduced or abolished. Improvement in the clinical status was achieved in all the patients. There was no mortality and morbidity was mild. A description of the technique as well as a brief review of the anatomy and haemodynamics of complete transposition of great arteries are the aims of this paper.

    The indirect hemagglutination (IHA) test done with turkey red cells was applied to 173 serum samples obtained from patients and persons exposed to Wuchereria bancrofti and Brugia malayi in endemic areas of Peninsular Malaysia. A crude extract of adult worms of the rat filaria, Breinlia booliati, was used as the antigen. When a titer of 1:16 was taken as negative, positive IHA test rates in sera from microfilaria-negative persons in endemic areas, microfilaremic cases, and patients with clinical filariasis were 13%, 75%, and 80%, respectively. Results of the IHA test correlated well with results obtained with the indirect fluorescent technique.

    Comparative field efficacy studies of four mosquito coil formulations containing active ingredient of d-allethrin (0.19 or 0.28 w/w) and d-transallethrin (0.12 or 0.16% w/w) and blank coils without active ingredient were carried out in living rooms (mean size 54.1 m3) of residential houses in a squatter area in Butterworth, Malaysia. The major indoor biting mosquitos collected in the test site were that of Culex quinquefasciatus Say (84.7%). Mean percentage reduction of blank coils, coils with 0.19 and 0.28% d-allethrin and coils with 0.12 and 0.16% d-transallethrin were 29.0, 71.7, 70.9, 75.0 and 72.6%, respectively. The use of coils as a mean of personal protection against mosquitos is discussed.

    Integrated nutrient management systems using plant residues and inorganic P fertilizers have high potential for increasing crop production and ensuring sustainability in the tropics, but their adoption requires in-depth understanding of nutrient dynamics in such systems. This was examined in a highly weathered tropical soil treated with green manures (GMs) and P fertilizers in two experiments conducted in the laboratory and glasshouse. The treatments were factorial combinations of the GMs (Calopogonium caeruleum, Gliricidia sepium, and Imperata cylindrica) and P fertilizers (phosphate rocks [PRs] from North Carolina, China, and Algeria, and triple superphosphate) replicated thrice. Olsen P, mineral N, pH, and exchangeable K, Ca, and Mg were monitored in a laboratory incubation study for 16 months. The change in soil P fractions and available P was also determined at the end of the study. Phosphorus available from the amendments was quantified at monthly intervals for 5 months by 33P-32P double isotopic labeling in the glasshouse using Setaria sphacelata as test crop. The GMs were labeled with 33P to determine their contribution to P taken up by Setaria, while that from the P fertilizers was indirectly measured by labeling the soil with 32P. The P fertilizers hardly changed Olsen P and exchangeable cations during 16 months of incubation. The legume GMs and legume GM+P did not change Olsen P, lowered exchangeable Ca, and increased exchangeable K about threefold (4.5 cmol[+]kg(-1) soil) in the first 4 months, even as large amounts of NH4-N accumulated (approximately 1000 mg kg soil(-1)) and soil pH increased to more than 6.5. Afterwards, Olsen P and exchangeable Ca and Mg increased (threefold) as NH4+-N and soil pH declined. The legume GMs also augmented reversibly sorbed P in Al-P and Fe-P fractions resulting in high residual effect in the soil, while fertilizer-P was irreversibly retained. The GMs increased PR-P utilization by 40 to over 80%, mobilized soil P, and markedly enhanced uptake of N, K, Ca, and Mg. Thus GMs+PRs is an appropriate combination for correcting nutrient deficiencies in tropical soils.

    The adsorption of Cu(II) ions from aqueous solution by chitosan and chitosan/PVA beads was studied in a batch adsorption system. Chitosan solution was blended with poly(vinyl alcohol) (PVA) in order to obtain sorbents that are insoluble in aqueous acidic and basic solution. The adsorption capacities and rates of Cu(II) ions onto chitosan and chitosan/PVA beads were evaluated. The Langmuir, Freundlich and BET adsorption models were applied to describe the isotherms and isotherm constants. Adsorption isothermal data could be well interpreted by the Langmuir model. The kinetic experimental data properly correlated with the second-order kinetic model, which indicates that the chemical sorption is the rate-limiting step. The Cu(II) ions can be removed from the chitosan and chitosan/PVA beads rapidly by treatment with an aqueous EDTA solution. Results also showed that chitosan and chitosan/PVA beads are favourable adsorbers.

    We report here a case of a kidney transplant recipient in whom the ureter was initially implanted into the peritoneum. Excessive ultrafiltration volume and reversal of serum vs dialysate creatinine ratio when the patient was recommenced on continuous ambulatory peritoneal dialysis first suggested the diagnosis which was subsequently confirmed by a plain abdominal x-ray demonstrating placement of ureteric stent in the peritoneum. This rare complication was successfully corrected with surgical re-implantation of ureter into the bladder and 5 years later, the patient remains well with good graft function.

    This article reviews the state of adverse drug reaction monitoring in five Asian/Pacific Rim countries (Australia, Japan, Malaysia, New Zealand and Singapore). Each country has an active pharmacovigilance programme managed by a national regulatory agency. Current methods for assessing risks and current methods used for risk management and communication are compared with the 'tools' used by the US FDA. Major positive attributes of the programmes in all five countries include active involvement of independent expert clinical advisory committees in identifying and evaluating risks through the assessment of reports of serious and unusual reactions, and regular communications about risks from the national agencies to doctors and pharmacists by means of pharmacovigilance bulletins. Most components of the risk-management toolbox are currently used, in some instances without legislated support. Variations in the way risk-management tools are implemented within individual national health systems are illustrated.

    Radium concentrations in 470 samples of the various types of waste from oil and gas industries were analysed using gamma spectrometers. The results showed that the radium concentration varied within a wide range. The highest mean 226Ra and 228Ra concentrations of 114,300 and 130,120 Bq/kg, respectively, were measured in scales. Overall, 75% of the waste, mostly sludge and extraction residue lies within the normal range of radium concentration in soils of Malaysia. However, some platform sludge can have radium concentration up to 560 Bq/kg.

    Separation of 1,2(2,3)- and 1,3-positional isomers of diacylglycerols (DAG) from vegetable oils by reversed-phase high-performance liquid chromatography (RP-HPLC) is investigated. The method is based on isocratic elution using 100% acetonitrile and UV detection at 205 nm. The following elution order of DAG molecular species is identified: 1,3-dilinolein < 1,2-dilinolein < 1,3-dimyristin < 1-oleoyl-3-linoleoyl-glycerol < 1,2-dimyristoyl-rac-glycerol < 1(2)-oleoyl-2(3)-linoleoyl-glycerol < 1-linolenoyl-3-stearoyl-glycerol < 1(2)-linolenoyl-2(3)-stearoyl-glycerol < 1,3-diolein < 1-palmitoyl-3-oleoyl-glycerol < 1,2-dioleoyl-sn-glycerol < 1(2)-palmitoyl-2(3)-oleoyl-glycerol < 1-linoleoyl-3-stearoyl-glycerol < 1,3-dipalmitin < 1(2)-linoleoyl-2(3)-stearoyl-glycerol < 1-oleoyl-3-stearoyl-glycerol < 1,2-dipalmitoyl-rac-glycerol < 1-palmitoyl-3-stearoyl-sn-glycerol < 1,3-distearin < 1,2-distearoyl-rac-glycerol. Linearity is observed over three orders of magnitude. Limits of detection and quantitation range 0.2-0.7 microg/mL for 1,3-dilinolein to 0.6-1.9 microg/mL for 1,2-dioleoyl-sn-glycerol, respectively. Precision and accuracy of the method are also demonstrated. The method is developed to separate mixtures of DAG molecular species produced from edible oils.

    This study was undertaken to screen the filamentous fungi isolated from its relevant habitats(wastewater, sewage sludge and sludge cake) for the bioconversion of domestic wastewater sludge. A total of 35 fungal strains were tested against wastewater sludge (total suspended solids, TSS 1%-5% w/w) to evaluate its potentiality for enhancing the biodegradability and dewaterability using liquid state bioconversion(LSB) process. The strains were divided into five groups i.e. Penicillium, Aspergillus, Trichoderma, Basidiomycete and Miscellaneous, respectively. The strains WWZP1003, SCahmA103, SCahmT105 and PC-9 among their respective groups of Penicillium, Aspergillus, Trichoderma and Basidiomycete played potential roles in terms of separation (formation of pellets/flocs/filaments), biodegradation(removal of COD) and filtration (filterability) of treated domestic wastewater sludge. The Miscellaneous group was not considered due to its unsatisfactory results as compared to the other groups. The pH value was also influenced by the microbial treatment during fermentation process. The filterability of treated sludge was improved by fungal treatment, and lowest filtration time was recorded for the strain WWZP1003 and SCahmA103 of Penicillium and Aspergillus groups respectively compared with other strains.
