    In the crystal structure of 2-bromo-beclometasone dipropionate [systematic name: (8S,9R,10S,11S,13S,14S,16S,17R)-2-bromo-9α-chloro-11-hydr-oxy-10,13,16-trimethyl-3-oxo-17-[2-(propion-yloxy)acet-yl]-6,7,8,9,10,11,12,13,14,15,16,17-dodeca-hydro-3H-cyclo-penta-[a]phenanthren-17-yl propionate], C(28)H(36)BrClO(7), the six-membered ring with the 1,4-diene-3-one composition is planar (r.m.s. deviations = 0.03 and 0.04 Å for the two independent mol-ecules), whereas the remaining six-membered rings have chair conformations. Each of the independent mol-ecules self-associates via O-H⋯O(propionate) hydrogen bonding, generating a supra-molecular chain running along the b axis. The crystal is twinned, with the monoclinic unit cell emulating an orthorhomic crystal system; the major twin component constitutes approximately 60%.

    In the crystal structure of (8S,9R,10S,11S,13S,14S,16S,17R)-9α-bromo-11-hydr-oxy-10,13,16-trimethyl-3-oxo-17-[2-(propion-yloxy)acet-yl]-6,7,8,9,10,11,12,13,14,15,16,17-dodeca-hydro-3H-cyclo-penta-[a]phenanthren-17-yl propionate monohydrate, C(28)H(37)BrO(7)·H(2)O, which has a 9α-Br atom in place of the 9α-Cl atom of monohydrated beclometasone dipropionate, one six-membered ring is planar (r.m.s. deviation = 0.02 Å) owing to its 1,4-diene-3-one composition, whereas the two other six-membered rings each have a chair conformation. The organic mol-ecule and water mol-ecules engage in hydrogen-bonding inter-actions, generating a helical chain running along the c axis of the ortho-rhom-bic unit cell.

    The title compound, C(28)H(34)Cl(2)O(7), is a derivative of the glucocorticoid steroid beclomethasone dipropionate. It features an expoxide linkage [angle at oxygen = 96.6 (2)°]. The dichlorocyclohexenone ring adopts an envelope conformation, with the C atom bearing the two Cl substituents representing the flap. The dichloro-methyl C atom deviates by 0.471 (4) Å from the plane defined by the other five atoms, whose maximum r.m.s. deviation is 0.04 Å.

    In the title compound, C(18)H(21)IO, the cyclo-hexane ring adopts a chair conformation, whereas the cyclo-pentane ring and the ten-membered tetra-line portions each adopt an envelope conformation. For the five-membered ring, the methine C atom deviates by 0.638 (4) Å (r.m.s. of the four other atoms is 0.005 Å) and for the ten-membered ring, the methine C atom constituting the flap deviates by 0.671 (3) Å (r.m.s. of the other nine atoms is 0.066 Å).

    At present, approximately 25%of drugs in modern pharmacopoeia are derived from plant sources (phytomedicines) that can be developed for the treatment of diseases and disorders. Many other drugs are synthetic analogues built on the prototype compounds isolated from plants. Cocos nucifera Linn. (Arecaceae), which is commonly known as coconut, is a plant possessing a lot of potential as an ingredient in traditional medicines for the treatment of metabolic disorders and particularly as an anti-inflammatory, antimicrobial and analgesic agent. This review emphasizes on the recent literature and research findings that highlight the significant biological activities of C. nucifera Linn. such as its anti-inflammatory, antimicrobial and analgesic properties. This review can help researchers keen on exploiting the therapeutic potential of C. nucifera Linn. which may motivate them to further explore their commercial viability.

    Current traveling wave ultrasonic motor (TWUSM) utilizes comb-teeth structure as deflection amplifier. The position of the stator neutral axis to the stator contact surface is one of the factors that influences the deflection amplifier. Stator deflection directly effects on motor performance. In this study, the modification of the comb-teeth stator design is proposed to see its effect on motor efficiency. The modification is done so that the neutral axis position is further distance from the stator top contact surface. The proposed solution is to remove a selected mass element from the comb-teeth structure. Modeling, simulation and experimental work of the proposed concept is carried out utilizing Shinsei USR60 as the chosen TWUSM. The modeling and analyses are conducted through multi-physic finite element simulation MSC Marc Mentat. The results of the analyses and experimental work reveal that the modified comb-teeth stator increases the position of the neutral axis from the stator top surface. Due to the neutral axis shifting, the results also confirm that the proposed modified motor has higher efficiency compared to the non-modified motor.

    INTRODUCTION: Being a houseman in Malaysian hospitals can be very stressful. Stress can affect decision making to a great extent while addressing the needs of the patient in an emergency setting. This necessitated a study to find out the main sources of stress among housemen.
    METHODS: This was a cross-sectional descriptive study carried out among 55 housemen using a questionnaire on sources of stress. The data was analysed using SPSS version 17.
    RESULTS: A total of 16 factors leading to stress were studied among the housemen. Poor work and social life balance, annoying non-clinical personnel and medico-legal threats were among the top causes of stress.
    CONCLUSION: The factors leading to stress among housemen should be considered for effective working of the hospital and improving the workplace atmosphere for the housemen.

    Seven cases of analgesic nephropathy due to excessive ingestion of paracetamol are reported. None of these patients had been taking any other analgesic. All had radiological features of papillary necrosis. With the increasing use of paracetamol as a mild analgesic it is necessary to be aware of the possibility that paracetamol may induce analgesic nephropathy.

    Thirty-four case notes of patients in acute renal failure on whom intravenous urograms (IVU) were performed, were reviewed. All patients received high doses of Conray 420 or Urotrast. Delayed films up to 48 hours were routinely done in patients suspected of having obstruction. There were no side effects of the intravenous urogram examination. IVU with high dose contrast material and nephrogram are helpful in differentiating the cause of the acute renal failure, and in excluding outflow obstruction.

    A prospective study was performed on patients admitted to the medical and renal wards of General Hospital, Kuala Lumpur. Over a period of 14 months from 1 January 1982, 12 new cases of analgesic nephropathy (AN) were documented. Contrary to the experience in the West and in Australia, AN in Malaysia tends to have a male preponderance and occurs even in the younger age groups. The common analgesics abused are paracetamol, Chap Kaki Tiga and Chap Harimau. The main reasons for analgesic abuse are headache and arthritis. In addition to radiological features of renal papillary necrosis patients have the other manifestations of the disease such as peptic ulceration, anaemia, neuro-psychiatric disorders and ischaemic heart disease.

    This study intended to validate customer inspiration (CI)in Malaysian/developing country context. Data were collected from two different respondents for two studies - from Millennial customers of the auto industry and Generation Z customers of the smartphone industry. The survey conducted through a standardized and structured questionnaire. The variables of the both studies were customer-defined market orientation (MO) (customer orientation, competitor orientation, and interfunctional coordination), CI (inspired-by and inspired-to), and customer loyalty (CL). This research strategy, in terms of quantity, is descriptive and correlational. Statistical analysis of the data was carried out, using ADANCO 2.0. The finding of the study suggests all results of data 1 and data 2 were significant, and CI mediates the sub-constructs of MO with CL.

    Curcumin is a small organic molecule with pleiotropic biological activities. However, its multiple structural-pharmacokinetic challenges prevent its development into a clinical drug. Various structural modifications have been made to improve its drug profile. In this review, we focus on the methods adopted in the synthesis of asymmetric curcumin derivatives and their biological activities and forecast the future of this exciting class of compounds in the field of medicine.

    BACKGROUND: Oral health is integral to systemic health. There is a growing body of evidence of an association between periodontal and systemic diseases. The aim of the study was to evaluate the awareness of dentists regarding link between oral and systemic health.

    MATERIALS AND METHODS: Data was collected using a self-administered pilot-tested questionnaire. Dentists awareness about link between oral and systemic link was assessed on five point likert scale. Data was entered and analysed using SPSS.

    RESULTS: Of the 588 dentists, 500 completed the questionnaire (response rate 85.03%). About 93% of the participants (mean age 25.82 ± 4.21 years) agreed that oral health was associated with systemic health. Most dentists were aware of a connection between periodontal disease and diabetes (84.4%) and heart disease (70.2%). Similarly, 85.6% believed in the negative impact of oral disease on the quality of life of patients. More female than male dentists were aware of the relationship between periodontal disease and adverse pregnancy outcomes, diabetes, and rheumatoid arthritis (P < 0.001). Most dentists (97%) believed that more patients would seek oral care if they were aware of the oral-systemic link. After adjustments, private dentists were 4.65 times more likely than public dentists to believe in improving access to oral care with increased patient awareness of the oral-systemic connection (P = 0.011).

    CONCLUSIONS: Most dentists were aware of the oral-systemic link. They believed that patients' access to oral care would improve if they were aware of a connection between oral and systemic health. Therefore, patients should be informed of the oral-systemic link to improve their oral health.

    The IEEE 802.11ah standard relies on the conventional distributed coordination function (DCF) as a backoff selection method. The DCF is utilized in the contention-based period of the newly introduced medium access control (MAC) mechanism, namely restricted access window (RAW). Despite various advantages of RAW, DCF still utilizes the legacy binary exponential backoff (BEB) algorithm, which suffers from a crucial disadvantage of being prone to high probability of collisions with high number of contending stations. To mitigate this issue, this paper investigates the possibility of replacing the existing exponential sequence (i.e., as in BEB) with a better pseudorandom sequence of integers. In particular, a new backoff algorithm, namely Pseudorandom Sequence Contention Algorithm (PRSCA) is proposed to update the CW size and minimize the collision probability. In addition, the proposed PRSCA incorporates a different approach of CW freezing mechanism and backoff stage reset process. An analytical model is derived for the proposed PRSCA and presented through a discrete 2-D Markov chain model. Performance evaluation demonstrates the efficiency of the proposed PRSCA in reducing collision probability and improving saturation throughput, network throughput, and access delay performance.

    Climate change-induced spatial and temporal variability of stremflow has significant implications for hydrological processes and water supplies at basin scale. This study investigated the impacts of climate change on streamflow of the Kurau River Basin in Malaysia using a Climate-Smart Decision Support System (CSDSS) to predict future climate sequences. For this, we used 25 reliazations consisting from 10 Global Climate Models (GCMs) and three IPCC Representative Concentration Pathways (RCP4.5, RCP6.0 and RCP8.5). The generated climate sequences were used as input to Soil and Water Assessment Tool (SWAT) to simulate projected changes in hydrological processes in the basin over the period 2021-2080. The model performed fairly well for the Kurau River Basin, with coefficient of determination (R2), Nash-Sutcliffe Efficiency (NSE) and Percent Bias (PBIAS) of 0.65, 0.65 and -3.0, respectively for calibration period (1981-1998) and 0.60, 0.59 and -4.6, respectively for validation period (1996-2005). Future projections over 2021-2080 period show an increase in rainfall during August to January (relatively wet season, called the main irrigation season) but a decrease in rainfall during February to July (relatively dry season, called the off season). Temperature projections show increase in both the maximum and minimum temperatures under the three RCP scenarios, with a maximum increase of 2.5 °C by 2021-2080 relative to baseline period of 1976-2005 under RCP8.5 scenario. The model predicted reduced streamflow under all RCP scenarios compared to the baseline period. Compared to 2021-2050 period, the projected streamflow will be higher during 2051-2080 period by 1.5 m3/s except in February for RCP8.5. The highest streamflow is predicted during August to December for both future periods under RCP8.5. The seasonal changes in streamflow range between -2.8% and -4.3% during the off season, and between 0% (nil) and -3.8% during the main season. The assessment of the impacts of climatic variabilities on the available water resources is necessary to identify adaptation strategies. It is supposed that such assessment on the Kurau River Basin under changing climate would improve operation policy for the Bukit Merah reservoir located at downstream of the basin. Thus, the predicted streamflow of the basin would be of importance to quantify potential impacts of climate change on the Bukit Merah reservoir and to determine the best possible operational strategies for irrigation release.

    Collaborative beamforming (CBF) with a finite number of collaborating nodes (CNs) produces sidelobes that are highly dependent on the collaborating nodes' locations. The sidelobes cause interference and affect the communication rate of unintended receivers located within the transmission range. Nulling is not possible in an open-loop CBF since the collaborating nodes are unable to receive feedback from the receivers. Hence, the overall sidelobe reduction is required to avoid interference in the directions of the unintended receivers. However, the impact of sidelobe reduction on the capacity improvement at the unintended receiver has never been reported in previous works. In this paper, the effect of peak sidelobe (PSL) reduction in CBF on the capacity of an unintended receiver is analyzed. Three meta-heuristic optimization methods are applied to perform PSL minimization, namely genetic algorithm (GA), particle swarm algorithm (PSO) and a simplified version of the PSO called the weightless swarm algorithm (WSA). An average reduction of 20 dB in PSL alongside 162% capacity improvement is achieved in the worst case scenario with the WSA optimization. It is discovered that the PSL minimization in the CBF provides capacity improvement at an unintended receiver only if the CBF cluster is small and dense.

    Percutaneous pinning after closed reduction is commonly used to treat supracondylar fractures of the humerus in children. Minor pin tract infections frequently occur. The aim of this study was to prevent pin tract infections using a rubber stopper to reduce irritation of the skin against the Kirschner (K) wire following percutaneous pinning. Between July 2011 and June 2012, seventeen children with closed supracondylar fracture of the humerus of Gartland types 2 and 3 were treated with this technique. All patients were treated with closed reduction and percutaneous pinning and followed up prospectively. Only one patient, who was a hyperactive child, developed pin tract infection due to softening of the plaster slab. We found using the rubber stopper to be a simple and inexpensive method to reduce pin tract infections following percutaneous pinning.
