    Edesonfilaria cynocephali n. sp., a parasite of Cynocephalus variegatus taylori (Thomas) in Malaysia, is described. Makifilaria Krishnasamy et coll., 1981 is placed in synonymy with Edesonfilaria and the new combination E. inderi (Krishnasamy et coll., 1981) n. comb. is proposed. Edesonfilaria and the closely related genus Macacanema constitute a small evolutionary line of Filariae with a hyperspecialized oesophagus (the glandular portion lacks lumen); the line is restricted to the Indo-Malaysian region and occurs in arboreal Dermopterans, Chiropterans and Primates.

    Studies carried out on the occupational exposure to paraquat of plantation workers in Malaysia comprised quantitative estimates of dermal and respiratory exposure of knapsack spray operators, carriers, and rubber tappers operating under their normal working conditions. Spray operators have been shown to be dermally exposed to paraquat by walking through recently sprayed vegetation and into their own spray, regular adjustment and unblocking of spray nozzles and leakage, and overfilling of knapsack spray tanks. Carriers also received measurable dermal exposure from walking through recently sprayed vegetation and accidental spillage when carrying and loading. The infrequent and negligible dermal exposure of tappers resulted from walking through recently sprayed vegetation. Determinations of the total airborne paraquat concentrations in the breathing zone show that spray operators and carriers are exposed to an order of 1% or less of the current TLV for respirable paraquat. No paraquat was detected in the breathing zones of tappers working in simultaneously sprayed blocks. The calculated ranges of dermal and respiratory exposures, when compared with published data on both the exposure to, and the toxicity of, paraquat, indicate that there should be no toxicological risk to any of the three groups studied as a result of using paraquat.

    Recent work with samples of black and white urban American women showed a clear behavioral sequence relating age at menarche to age at first intercourse to age at first birth. This paper shows that the linking of ages at menarche, intercourse, marriage, and first birth is a pattern which occurs in very diverse cultures. We present confirmatory data from the United States, Belgium, and Pakistan, and from Malay and Chinese women in Malaysia. We interpret our findings as indicating a biological process leading to (a) social interpretations of readiness for reproduction, and (b) persisting biological differences between early and late maturing women.

    This paper reviews the pattern of glomeruler disease from 163 renal biopsies performed at the Institute of Urology and Nephrology. Nephrotic syndrome formed the largest group of patients. There is a high prevalence of SLE nephritis in our community. The histopathologic findings in our series were comparable to those from Western countries except for the lower incidence of membranous and membranoproliferative glomerulonephritis.

    Radioisotope detection and localisation of myocardial infarction is discussed. Its clinical value and pitfalls are also discussed. The clinical application of this safe, simple, sensitive, repeatable, reproducible and non-invasive method in Malaysian patients performed during the period October 1978 to April 1979 at the University Hospital is reviewed. The main value of 99mTc labelled phosphate scan is in the demonstration and localisation of recent myocardial infarctions in patients where the electrocardiogram or serum enzymes changes are unhelpful.

    Spirometry was performed on 614 female subjects ranging in age from 13 to 69 years and comprising all the main races in Malaysia. They were divided into six age categories. Mean forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1) were 2.51 +/- 0.02 and 2.31 +/- 0.02l, respectively. Both FVC and FEV1 correlated negatively with age. Regression analysis revealed an age-related decline in FVC of 220 ml per decade of life. Multiple stepwise regression of the data for the prediction of an individual's FVC above the age of 20 years gave an equation: FVC(l) = 0.0312 (height)-0.022 (age)-1.64. Predicted FVC values derived from equations based on other populations were considerably higher than the observed mean in this study. Our study, therefore, reemphasizes the need to be cautious when applying formulae derived from one population to another. Grossly erroneous conclusions may be reached unless predicted equations for lung-function tests for a given population group are derived from studies based on the same population group.

    Dengue fever, Dengue hemorrhagic fever and Dengue shock syndrome within the dengue complex is a sinister disease of great public health importance and continues to ravage children, young adults and the aged in Malaysia. The history of the disease is traced for over the years and the changing pattern of clinical presentation are noted. Various hospital based studies have been compared and the pathognomonic features of the disease in Malaysia are highlighted.

    A 37 year old female who presented with a history of foreign body ingestion ten months previously was found on examination to have a retropharyngeal swelling. An initial differential of foreign body granuloma or retropharyngeal tuberculous abscess was considered. The usefulness of the lateral neck radiograph in demonstrating retropharyngeal pathology and the supplementary role of the computed tomography scan in confirming and elucidating the identity of the radio-opacity is highlighted. This presentation is a timely reminder of the need for a simple inexpensive lateral neck radiograph in situations of doubt as to persisting foreign body impaction.

    A 27 year old housewife developed thrombotic thrombocytopenic purpura during the twelfth week of pregnancy. She had partial response to initial plasma infusion and subsequent plasmapheresis. However, her clinical course was complicated by the development of severe pancytopenia the consequence of a hypocellular marrow. She succumbed to septicaemic shock one month after diagnosis. The development of hypocellular marrow in thrombotic thrombocytopenic purpura has not been reported before.

    We reviewed our data from 122 records of patients taking phenytoin for the treatment of various types of epilepsy and selected 15 (age range 10-43 years old) who were on phenytoin alone to calculate Michaelis-Menten pharmacokinetic parameters. The average Vm and Km for this age group was found to be 8.45 mg/kg/day and 6.72 mg/litre, respectively. Km was independent of age and weight. Vm correlated well with weight but there was no relationship with age.

    126 Malaysian children, 65 boys and 61 girls from higher income families were followed-up regularly from birth to six years of age in the University Hospital, Kuala Lumpur. The study confirms the observations of previous studies that growth velocity of head circumference is most rapid during the first few months of infancy and then decreases so that by the fifth year of life the increment is minimal. It also confirms the fact that boys have bigger head circumferences than girls. The paper also presents the head circumference distance and velocity percentile charts which can be used to monitor the head circumference of Malaysian children.

    In experiments conducted over a four-year period the effect of pasture type (Setaria sphacelata and a Brachiaria decumbens/Leucaena leucocephala mixture), management system (rotational grazing and cut and carry) and level of concentrate supplement (0, 4 and 6 kg fresh weight/cow per day) on milk production in smallholder dairy units was examined. All units were 1 ha in size and stocked with five Sahiwal X Friesian cows. Milk yields/ha were higher from rotational grazing and the brachiaria/leucaena pasture when compared with the cut and carry system and the setaria pasture respectively. In subsequent experiments smallholder units were based on brachiaria/leucaena and rotational grazing. A supplement of 4 kg fresh weight of concentrate (11 MJ/kg DM and 150 g/kg DM CP) increased milk yield/ha from 7,760 to 13,045 kg while in a third trial milk yield/ha was further increased from 14,148 to 16,760 kg when concentrate level was raised from 4 to 6 kg fresh weight/day. The results indicate that smallholder dairy units in Malaysia could be economically viable and competitive with other agricultural enterprises such as rubber and oil palm.

    An 18-year-old Malay lady was treated with high dose steroids for three and a half years for idiopathic thrombocytopaenic purpura. At 21 years, after a series of relapses, a splenectomy was carried out. In addition, two nodules at the hilum of the spleen were also removed. Histological examination of these nodules revealed features of Kaposi's sarcoma. Kaposi's sarcoma resulting from immunosuppression by corticosteroids is rare. Our patient is the first such case reported in Malaysia.

    Monitoring systems are useful epidemiological instruments for assessing the problem of drug abuse. The rapid growth of the drug dependence problem in Malaysia led to increased awareness of the need for a system for continuous monitoring of the situation. Preliminary work on the design of an appropriate monitoring system was initiated in 1976. A fully integrated national reporting system was established in 1978, linking all public services and agencies coming into contact with drug-dependent persons, including law enforcement agencies, drug abuse treatment and rehabilitation centres, and social and welfare institutions. The information system included a mechanism for systematic gathering, processing, analysing and presenting essential data on the prevention, control and management of drug abuse problems. It also included reporting on drug-related events, such as hospitalizations and arrests, as well as data on known drug-dependent persons and new cases of dependence. The system has been used for routine monitoring of the extent, trends, patterns and other characteristics of drug abuse problems in Malaysia, providing basic information for policy-making and programme planning. On the basis of data generated by the system, it was estimated that the prevalence rate of drug-dependent persons per 100,000 population increased from 84.3 in 1976 to 754.6 in 1986. It was estimated that there were 119,001 drug-dependent persons in Malaysia in 1986.
