    One of the main characteristics of urbanization in Asia is the very rapid increase in population movement from rural to urban centers. This phenomenon has led to changing population structure, its composition and lifestyles in the cities and its fringes. As a consequent of population pressure on urban system and infrastructure, compounded by the nature of the composition of the in-migrant population, the urban concentrates are faced with several social and socio-economic problems. Although there has been a lot of interests among researchers to study the causes and effects or urbanization, there is a vacuum in the area of health implications. Planners and administrators usually give priority to the physical aspects of the urban and urbanities. Social problems and health implications thereof receives very little attention either at the level of administration or research. This paper therefore is a brave attempt to focus and draw some attention to this neglected area by looking at selected social problems and the health consequences.

    Malaysian Borneo has a lower population density and is an area known for its lush rainforests. However, changes in pollutant profiles are expected due to increasing urbanisation and commercial-industrial activities. This study aims to determine the variation of surface O3concentration recorded at seven selected stations in Malaysian Borneo. Hourly surface O3data covering the period 2002 to 2013, obtained from the Malaysian Department of Environment (DOE), were analysed using statistical methods. The results show that the concentrations of O3recorded in Malaysian Borneo during the study period were below the maximum Malaysian Air Quality Standard of 100ppbv. The hourly average and maximum O3concentrations of 31 and 92ppbv reported at Bintulu (S3) respectively were the highest among the O3concentrations recorded at the sampling stations. Further investigation on O3precursors show that sampling sites located near to local petrochemical industrial activities, such as Bintulu (S3) and Miri (S4), have higher NO2/NO ratios (between 3.21 and 5.67) compared to other stations. The normalised O3values recorded at all stations were higher during the weekend compared to weekdays (unlike its precursors) which suggests the influence of O3titration by NO during weekdays. The results also show that there are distinct seasonal variations in O3across Borneo. High surface O3concentrations were usually observed between August and September at all stations with the exception of station S7 on the east coast. Majority of the stations (except S1 and S6) have recorded increasing averaged maximum concentrations of surface O3over the analysed years. Increasing trends of NO2and decreasing trends of NO influence the yearly averaged maximum of O3especially at S3. This study also shows that variations of meteorological factors such as wind speed and direction, humidity and temperature influence the concentration of surface O3.

    The effect of metals on environmental health is well documented and monitoring these and other pollutants is considered an important part of environmental management. Developing countries are yet to fully appreciate the direct impacts of pollution on aquatic ecosystems and as such, information on pollution dynamics is scant. Here, we assessed the temporal and spatial dynamics of stream sediment metal and nutrient concentrations using contaminant indices (e.g. enrichment factors, pollution load and toxic risk indices) in an arid temperate environment over the wet and dry seasons. The mean sediment nutrient, organic matter and metal concentration were highest during the dry season, with high values being observed for the urban environment. Sediment contaminant assessment scores indicated that during the wet season, the sediment quality was acceptable, but not so during the dry season. The dry season had low to moderate levels of enrichment for metals B, Cu, Cr, Fe, Mg, K and Zn. Overall, applying the sediment pollution load index highlighted poor quality river sediment along the length of the river. Toxic risk index indicated that most sites posed no toxic risk. The results of this study highlighted that river discharge plays a major role in structuring temporal differences in sediment quality. It was also evident that infrastructure degradation was likely contributing to the observed state of the river quality. The study contributes to our understanding of pollution dynamics in arid temperate landscapes where vast temporal differences in base flow characterise the riverscape. Such information is further useful for contrasting sediment pollution dynamics in aquatic environments with other climatic regions.

    Rapid urbanisation in Malaysian cities poses risks to the health of residents. This study aims to estimate the relative risk (RR) of major air pollutants on cardiovascular and respiratory hospitalisations in Kuala Lumpur. Daily hospitalisations due to cardiovascular and respiratory diseases from 2010 to 2014 were obtained from the Hospital Canselor Tuanku Muhriz (HCTM). The trace gases, PM10 and weather variables were obtained from the Department of Environment (DOE) Malaysia in consistent with the hospitalisation data. The RR was estimated using a Generalised Additive Model (GAM) based on Poisson regression. A "lag" concept was used where the analysis was segregated into risks of immediate exposure (lag 0) until exposure after 5 days (lag 5). The results showed that the gases could pose significant risks towards cardiovascular and respiratory hospitalisations. However, the RR value of PM10 was not significant in this study. Immediate effects on cardiovascular hospitalisations were observed for NO2 and O3 but no immediate effect was found on respiratory hospitalisations. Delayed effects on cardiovascular and respiratory hospitalisations were found with SO2 and NO2. The highest RR value was observed at lag 4 for respiratory admissions with SO2 (RR = 1.123, 95% CI = 1.045-1.207), followed by NO2 at lag 5 for cardiovascular admissions (RR = 1.025, 95% CI = 1.005-1.046). For the multi-pollutant model, NO2 at lag 5 showed the highest risks towards cardiovascular hospitalisations after controlling for O3 8 h mean lag 1 (RR = 1.026, 95% CI = 1.006-1.047), while SO2 at lag 4 showed highest risks towards respiratory hospitalisations after controlling for NO2 lag 3 (RR = 1.132, 95% CI = 1.053-1.216). This study indicated that exposure to trace gases in Kuala Lumpur could lead to both immediate and delayed effects on cardiovascular and respiratory hospitalisations.

    Introduction: Due to rapid urbanization, Malaysia is currently undergoing extensive amount of construction activities and significant rise in the construction waste. Improper construction wastes management practices have led to rise in the waste open dumping sites which may significantly affect the environment and public health. This study was undertaken to assess air pollution in such a site and the impact to the respiratory health of children studying in a nearby school. Methods: A comparative cross-sectional study was conducted in Sungai Buloh (exposed) and Kuala Selangor (comparative). Concentration of PM10 was monitored at the open dumping site (n=15), a nearby primary school (n=45) and a comparative primary school (n=12). Parents who gave consent (n=229) answered a questionnaire related to child’s respiratory health whereas their children participated in lung function assessment. Results: Concentration of PM10 at the dumping site, exposed school and comparative school was 0.245±0.048mg/ m3 , 0.270±0.020mg/m3 and 0.051±0.016mg/m3 respectively, with the first two significantly exceeded the 24-hour Malaysian Air Quality Standard (MAQS) for PM10 (0.150mg/m3 ).     The pace of urbanization in Peninsular Malaysia was slower in the most recent intercensal interval, 1957 to 1970, than in the previous period, 1947 to 1957. Most of the small change in the rural-urban balance from 1957 to 1970 appears due to the growth of towns into the urban classification rather than to a redistribution of population into the previous urban settlements. A number of towns in Peninsular Malaysia do show exceptional growth from 1957 to 1970, but there seems to be no clear relationship between a city's size and its subsequent growth. The rural areas on the outskirts of the largest cities do show rapid growth, especially the periphery of the capital city. It appears that neither the classic model of urbanization based upon Western experience nor the over-urbanization thesis explain the urbanization process in Peninsular Malaysia.

    INTRODUCTION: In this era of globalization, urbanization, and rapidly expanding unhygienic food corners across the globe, the incidence of food poisoning is very common nowadays.

    AIM: The aim of our study was to investigate the perceptions and readiness of schoolteachers to accept notifications on food poisoning as a part of education to the students.

    METHODOLOGY: A descriptive cross-sectional study was carried out with the help of a validated questionnaire for data collection. Our research involved schoolteachers from both primary and secondary schools in Muar. The questionnaire was pretested among the eligible trainee teachers and yielded an internal consistency reliability coefficient (c = Cronbach's alpha) of 0.082. This study was conducted from October 29, 2017, to December 14, 2018, in Muar. Our sample size was 259. Ethical consent was obtained from the Institution Ethical Committee.

    RESULTS: A total of 259 schoolteachers from both primary and secondary schools in Muar were included in this study. In our study, 81.1% of the teachers responded that they can easily educate their students about food poisoning. Most of them (93.1%) were ready to receive notifications on food poisoning in any mode, and about 72% of the teachers preferred WhatsApp as their mode of receiving notification. The least (1.2%) preferred mode of notification was LINE (a social app). Teachers' willingness to disseminate the information regarding food poisoning was also higher (98.5%).

    CONCLUSIONS: We concluded that majority of the schoolteachers had a good perception and were ready to receive the notifications on food poisoning through WhatsApp as a part of education to the students.

    The importance of rainfed areas and animal agriculture on productivity enhancement and food security for economic rural growth in Asia is discussed in the context of opportunities for increasing potential contribution from them. The extent of the rainfed area of about 223 million hectares and the biophysical attributes are described. They have been variously referred to inter alia as fragile, marginal, dry, waste, problem, threatened, range, less favoured, low potential lands, forests and woodlands, including lowlands and uplands. Of these, the terms less favoured areas (LFAs), and low or high potential are quite widely used. The LFAs are characterised by four key features: i) very variable biophysical elements, notably poor soil quality, rainfall, length of growing season and dry periods, ii) extreme poverty and very poor people who continuously face hunger and vulnerability, iii) presence of large populations of ruminant animals (buffaloes, cattle, goats and sheep), and iv) have had minimum development attention and an unfinished wanting agenda. The rainfed humid/sub-humid areas found mainly in South East Asia (99 million ha), and arid/semi-arid tropical systems found in South Asia (116 million ha) are priority agro-ecological zones (AEZs). In India for example, the ecosystem occupies 68% of the total cultivated area and supports 40% of the human and 65% of the livestock populations. The area also produces 4% of food requirements. The biophysical and typical household characteristics, agricultural diversification, patterns of mixed farming and cropping systems are also described. Concerning animals, their role and economic importance, relevance of ownership, nomadic movements, and more importantly their potential value as the entry point for the development of LFAs is discussed. Two examples of demonstrated success concern increasing buffalo production for milk and their expanded use in semi-arid AEZs in India, and the integration of cattle and goats with oil palm in Malaysia. Revitalised development of the LFAs is justified by the demand for agricultural land to meet human needs e.g. housing, recreation and industrialisation; use of arable land to expand crop production to ceiling levels; increasing and very high animal densities; increased urbanisation and pressure on the use of available land; growing environmental concerns of very intensive crop production e.g. acidification and salinisation with rice cultivation; and human health risks due to expanding peri-urban poultry and pig production. The strategies for promoting productivity growth will require concerted R and D on improved use of LFAs, application of systems perspectives for technology delivery, increased investments, a policy framework and improved farmer-researcher-extension linkages. These challenges and their resolution in rainfed areas can forcefully impact on increased productivity, improved livelihoods and human welfare, and environmental sustainability in the future.

    Malaysia inherits a highly subsidized tax-based public healthcare system complemented by a fee-for-service private sector. Population health in Malaysia has considerably improved since independence using a relatively small amount of gross domestic product (~4%). Brain drain of highly specialized personnel, growth in healthcare spending, demographic and disease pattern changes and increase in patients' demands and expectations towards better medical care are exerting pressure on the sustainability of the system to continuously provide efficient and effective services at relatively low cost. Malaysia has adopted and implemented some of the quality use of medicine concepts such as National Essential Medicine List, health technology assessment and promotion of generic medicines in their health policy, but so far the results may not be optimal. Activities to further promote these strategies are needed for successful implementation to achieve more positive and sustained beneficial outcomes. Better strategic planning, management and collaboration between various stakeholders, considering the needs and barriers of the strategies, are important to ensure effective implementation of the strategies. More emphasis should be placed upon more equitable and rational distribution of healthcare resources to cater for rapid urbanization. Additionally, a sustainable health financing structure that is more progressive and does not encourage moral hazard should be established. In conclusion, Malaysia has achieved good outcomes in population health with relatively low financial inputs since independence. However, changes in the overall environment have created issues which would threaten the long-term viability of the healthcare system if not tackled properly. The numbers of internationally trialled strategies could be used to deal with these challenges. In addition, coordinated implementation of these strategies and effective engagement and communication between various stakeholders are necessary to further strengthen the Malaysian healthcare system effectively.

    The practice of development planning does not empower the local authority to prepare a development
    plan, as planning and development control is under the jurisdiction of the State Planning Authority
    (SPA). Rapid growth in the housing sector shows a positive indicator towards urbanization in Sarawak.
    This study showed that the current centralised process is fair to all parties, for it ensures equilibrium in
    all areas. Beside that, the study also found that there is some confusion in the roles played by the
    involved parties. The study also identified few problems that often occur, among them complicated
    procedures, slow approval process, bureaucracy, and risks in planning and marketing. However, the
    practice of development planning is concerned with aspects quality and quantity so as not to burden the
    purchaser. The state government, through the SPA, controls the price of housing to ensure equilibrium
    and to avoid the occurrence of abandoned housing projects.

    Arthropod-borne viruses (arboviruses) are important causes of human disease nearly worldwide. All arboviruses circulate among wild animals, and many cause disease after spillover transmission to humans and agriculturally important domestic animals that are incidental or dead-end hosts. Viruses such as dengue (DENV) and chikungunya (CHIKV) that have lost the requirement for enzootic amplification now produce extensive epidemics in tropical urban centers. Many arboviruses recently have increased in importance as human and veterinary pathogens using a variety of mechanisms. Beginning in 1999, West Nile virus (WNV) underwent a dramatic geographic expansion into the Americas. High amplification associated with avian virulence coupled with adaptation for replication at higher temperatures in mosquito vectors, has caused the largest epidemic of arboviral encephalitis ever reported in the Americas. Japanese encephalitis virus (JEV), the most frequent arboviral cause of encephalitis worldwide, has spread throughout most of Asia and as far south as Australia from its putative origin in Indonesia and Malaysia. JEV has caused major epidemics as it invaded new areas, often enabled by rice culture and amplification in domesticated swine. Rift Valley fever virus (RVFV), another arbovirus that infects humans after amplification in domesticated animals, undergoes epizootic transmission during wet years following droughts. Warming of the Indian Ocean, linked to the El Niño-Southern Oscillation in the Pacific, leads to heavy rainfall in east Africa inundating surface pools and vertically infected mosquito eggs laid during previous seasons. Like WNV, JEV and RVFV could become epizootic and epidemic in the Americas if introduced unintentionally via commerce or intentionally for nefarious purposes. Climate warming also could facilitate the expansion of the distributions of many arboviruses, as documented for bluetongue viruses (BTV), major pathogens of ruminants. BTV, especially BTV-8, invaded Europe after climate warming and enabled the major midge vector to expand is distribution northward into southern Europe, extending the transmission season and vectorial capacity of local midge species. Perhaps the greatest health risk of arboviral emergence comes from extensive tropical urbanization and the colonization of this expanding habitat by the highly anthropophilic (attracted to humans) mosquito, Aedes aegypti. These factors led to the emergence of permanent endemic cycles of urban DENV and CHIKV, as well as seasonal interhuman transmission of yellow fever virus. The recent invasion into the Americas, Europe and Africa by Aedes albopictus, an important CHIKV and secondary DENV vector, could enhance urban transmission of these viruses in tropical as well as temperate regions. The minimal requirements for sustained endemic arbovirus transmission, adequate human viremia and vector competence of Ae. aegypti and/or Ae. albopictus, may be met by two other viruses with the potential to become major human pathogens: Venezuelan equine encephalitis virus, already an important cause of neurological disease in humans and equids throughout the Americas, and Mayaro virus, a close relative of CHIKV that produces a comparably debilitating arthralgic disease in South America. Further research is needed to understand the potential of these and other arboviruses to emerge in the future, invade new geographic areas, and become important public and veterinary health problems.

    Purpose: The goal of this study was to assess the effectiveness of healthcare spending among the leading Asian economies.

    Methods: We have selected a total of nine Asian nations, based on the strength of their economic output and long-term real GDP growth rates. The OECD members included Japan and the Republic of Korea, while the seven non-OECD nations were China, India, Indonesia, Malaysia, Pakistan, the Philippines, and Thailand. Healthcare systems efficiency was analyzed over the period 1996-2017. To assess the effectiveness of healthcare expenditure of each group of countries, the two-way fixed effects model (country- and year effects) was used.

    Results: Quality of governance and current health expenditure determine healthcare system performance. Population density and urbanization are positively associated with a healthy life expectancy in the non-OECD Asian countries. In this group, unsafe water drinking has a statistically negative effect on healthy life expectancy. Interestingly, only per capita consumption of carbohydrates is significantly linked with healthy life expectancy. In these non-OECD Asian countries, unsafe water drinking and per capita carbon dioxide emissions increase infant mortality. There is a strong negative association between GDP per capita and infant mortality in both sub-samples, although its impact is far larger in the OECD group. In Japan and South Korea, unemployment is negatively associated with infant mortality.

    Conclusion: Japan outperforms other countries from the sample in major healthcare performance indicators, while South Korea is ranked second. The only exception is per capita carbon dioxide emissions, which have maximal values in the Republic of Korea and Japan. Non-OECD nations' outcomes were led by China, as the largest economy. This group was characterized with substantial improvement in efficiency of health spending since the middle of the 1990s. Yet, progress was noted with remarkable heterogeneity within the group.
