Two types of palm oil and sal fat based cocoa butter equivalents, namely fCBE (produced by using co-fractionation method) and mCBE (produced by using conventional method) were prepared. Results showed that the fCBE had triglyceride composition and solidification characteristics closer to the Malaysian cocoa butter than the mCBE produced at the same yield percentage. Increasing acetone washing time had little effect on the fCBE if compared to the effect of increasing palm olein to sal fat blend ratio. Co-fractionation technique increase the compatibility between CBE component triglycerides. Thus, more palm oil can be incorporated in the preparation and the process can be carried out at not low temperature as compared to the conventional method.
Over a two year period, 323 livers were examined using ultrasound. Majority of these cases were icteric and ultrasound could distinguish obstructive from non-obstructive jaundice. Primary and secondary liver tumours were also detected. Liver abscesses, and radiolucent gallstones were picked up by ultrasound, the areas under study being scanned using standard methods as outlined by various ultrasonographists.
A 22-year-old male with underlying end-stage renal failure was referred to our center for a malpositioned dialysis catheter. Imaging showed the tip of the catheter to be placed in the brachiocephalic trunk; the patient was, however, asymptomatic. Surgical removal of the malpositioned catheter followed, with no postoperative complications.
Accurate corneal wavefront aberration measurements are essential in determining patient’s suitability, vision outcomes and patient satisfaction in laser refractive surgeries. This study aimed to evaluate the reliability measurement of higher-order aberration (HOA) using corneal videokeratograph Atlas 9000. Materials and method: 38 eyes of 19 participants were recruited in this study. Comprehensive eye examination was done to ensure all participants eligibility, and any conditions in which obstruct the central cornea were excluded. Corneal wavefront aberrations were measured based on Zernike polynomials. In assessing repeatability, three repetitive measurements in five minutes in a single session were taken by 2 experience examiners. For reproducibility measurement, similar approach was done with the time-interval between measurements was set at one week. Bland-Altman, limits of agreement (LoA) and intraclass correlation coefficients (ICCs) were used to evaluate the reliability measurement. Results: Bland-Altman and LoA findings revealed no significant differences for both repeatability and reproducibility measurement. For repeatability testing, the mean differences for vertical trefoil, oblique trefoil and spherical aberration were -0.096 ± 0.493, 0.001 ± 0.048, 0.008 ± 0.035, 0.004 ± 0.029, 0.010 ± 0.053 with LoA of 1.930, 0.188, 0.138, 0.114, and 0.208 respectively. Intraclass correlation coefficient (ICC) excellent reliability of 0.841 for all parameters. Likewise, reproducibility testing showed similar findings with the mean difference were -0.018 ± 0.091, 0.016 ± 0.061, -0.0004 ± 0.036, -0.002 ± 0.042, 0.003 ± 0.026, with LoA of 0.356, 0.24, 0.141,0.164, and 0.102 respectively. Intraclass correlation coefficient (ICC) shows excellent reliability of more than 0.9 for all parameters. LoA of less than 1.0 were observed in all measurements (except for repeatability of vertical trefoil) indicates high consistency of the measurements. Conclusions: Corneal videokeratograph Atlas 9000 provides excellent HOA measurement reliability.
The middle cerebral artery (MCA) dot sign is an important radiological sign in patients presenting with acute ischemic stroke (AIS). If identified and intervened early, a good clinical outcome may be achieved.
A combined similarity-numerical solution of the magnetohydrodynamic boundary layer slip flow of an electrically conducting non-Newtonian power-law nanofluid along a heated radiating moving vertical plate is explored. Our nanofluid model incorporates the influences of the thermophoresis and the Brownian motion. The basic transport equations are made dimensionless first and then suitable similarity transformations are applied to reduce them into a set of nonlinear ordinary differential equations with the associated boundary conditions. The reduced equations are then solved numerically. Graphical results for the non-dimensional flow velocity, the temperature and the nanoparticles volume fraction profiles as well as for the friction factor, the local Nusselt and the Sherwood numbers are exhibited and examined for various values of the controlling parameters to display the interesting aspects of the solutions. It was found that the friction factor increases with the increase of the magnetic field (M), whilst it is decreased with the linear momentum slip parameter (a). The linear momentum slip parameter (a) reduces the heat transfer rates and the nanoparticles volume fraction rates. Our results are compatible with the existing results for a special case.
Tuberculous granuloma in the middle ear is an unusual entity. Herein, we report a case with short presentation of otitis media with mastoid abscess but with a CT scan showing widespread bone destruction. The cause was determined to be middle ear tuberculosis. Awareness of this entity is important, as it may cause a delay in referral to an otorhinolaryngology specialist and, subsequently, a delay in initiating treatment. Therefore, it should be considered in the differential diagnosis, especially when the usual treatment fails to produce the desired result.
The experiments were carried out to evaluate the protein-sparing effect in Heteropneustes fossilis fry (0.751 ± 0.01 g) fed for 90 days with six isocaloric diets containing 45, 40 or 35% of casein-protein and 25, 30 or 35% of glucose/dextrin, combined properly as the work purpose. The highest weight gain was recorded in fry fed with 35% dextrin and 35% crude protein level. Interestingly, the value of SGR (2.950±0.017) and PER (1.793±0.03) were significantly (p<0.05) increased with reducing protein level from 45 to 35% and with increasing carbohydrate level from 25% to 35%, respectively. However, the poorest growth was recorded in the fry fed with glucose containing diets than dextrin-containing diets. Protein deposition was significantly (p<0.05) lower in fry fed with the glucose diets at the protein level of 45% than those fed with 40 or 35%, indicating that H. fossilis utilize dextrin easily than glucose. Increase of dextrin content from 35 to 45% did not reduce (p<0.05) the weight gain, even reducing the dietary protein from 45 to 35%. This fact suggests that carbohydrate spare protein by using dextrin as energy source.
Myelin oligodendrocyte glycoprotein-antibodies (MOG-IgG) are associated with acquired inflammatory demyelinating syndromes, seen predominantly in children and young adults. The overlapping clinical and radiological features of the heterogenous spectrum of demyelinating central nervous system (CNS) diseases makes the detection of MOG-IgG antibodies important for prognosis and treatment decisions. Herein, we describe the occurrence of MOG-IgG associated disease presenting as acute disseminated encephalomyelitis (ADEM), with spinal MRI findings of centrally located long cord lesion in a 14-month old child.
Sensorineural hearing loss following trauma is a common finding in daily clinical practice and usually associated with a poor prognosis. Our case illustrates a patient who was involved in motor vehicle accident sustaining bilateral severe to profound sensorineural hearing loss but subsequently recovered fully after two years. Unless there is clear trauma to the cochlea or auditory nerve, a substantial duration of follow up is needed in the treatment of such cases.
This work is to investigate the diagnostic value of a deep learning-based magnetic resonance imaging (MRI) image segmentation (IS) technique for shoulder joint injuries (SJIs) in swimmers. A novel multi-scale feature fusion network (MSFFN) is developed by optimizing and integrating the AlexNet and U-Net algorithms for the segmentation of MRI images of the shoulder joint. The model is evaluated using metrics such as the Dice similarity coefficient (DSC), positive predictive value (PPV), and sensitivity (SE). A cohort of 52 swimmers with SJIs from Guangzhou Hospital serve as the subjects for this study, wherein the accuracy of the developed shoulder joint MRI IS model in diagnosing swimmers' SJIs is analyzed. The results reveal that the DSC for segmenting joint bones in MRI images based on the MSFFN algorithm is 92.65%, with PPV of 95.83% and SE of 96.30%. Similarly, the DSC for segmenting humerus bones in MRI images is 92.93%, with PPV of 95.56% and SE of 92.78%. The MRI IS algorithm exhibits an accuracy of 86.54% in diagnosing types of SJIs in swimmers, surpassing the conventional diagnostic accuracy of 71.15%. The consistency between the diagnostic results of complete tear, superior surface tear, inferior surface tear, and intratendinous tear of SJIs in swimmers and arthroscopic diagnostic results yield a Kappa value of 0.785 and an accuracy of 87.89%. These findings underscore the significant diagnostic value and potential of the MRI IS technique based on the MSFFN algorithm in diagnosing SJIs in swimmers.
