    We report a case of Griscelli Syndrome (GS). Our patient initially presented with a diagnosis of haemophagocytic lymphistiocytosis (HLH). Subsequent microscopic analysis of the patient's hair follicle revealed abnormal distribution of melanosomes in the shaft, which is a hallmark for GS. Analysis of RAB27A gene in this patient revealed a homozygous mutation in exon 6, c.550C>T, p.R184X . This nonsense mutation causes premature truncation of the protein resulting in a dysfunctional RAB27A. Recognition of GS allows appropriate institution of therapy namely chemotherapy for HLH and curative haemotopoeitic stem cell transplantation.

    Network connectivity and link quality information are the fundamental requirements of wireless sensor network protocols to perform their desired functionality. Most of the existing discovery protocols have only focused on the neighbor discovery problem, while a few number of them provide an integrated neighbor search and link estimation. As these protocols require a careful parameter adjustment before network deployment, they cannot provide scalable and accurate network initialization in large-scale dense wireless sensor networks with random topology. Furthermore, performance of these protocols has not entirely been evaluated yet. In this paper, we perform a comprehensive simulation study on the efficiency of employing adaptive protocols compared to the existing nonadaptive protocols for initializing sensor networks with random topology. In this regard, we propose adaptive network initialization protocols which integrate the initial neighbor discovery with link quality estimation process to initialize large-scale dense wireless sensor networks without requiring any parameter adjustment before network deployment. To the best of our knowledge, this work is the first attempt to provide a detailed simulation study on the performance of integrated neighbor discovery and link quality estimation protocols for initializing sensor networks. This study can help system designers to determine the most appropriate approach for different applications.

    A wireless sensor network is a large collection of sensor nodes with limited power supply and constrained computational capability. Due to the restricted communication range and high density of sensor nodes, packet forwarding in sensor networks is usually performed through multi-hop data transmission. Therefore, routing in wireless sensor networks has been considered an important field of research over the past decade. Nowadays, multipath routing approach is widely used in wireless sensor networks to improve network performance through efficient utilization of available network resources. Accordingly, the main aim of this survey is to present the concept of the multipath routing approach and its fundamental challenges, as well as the basic motivations for utilizing this technique in wireless sensor networks. In addition, we present a comprehensive taxonomy on the existing multipath routing protocols, which are especially designed for wireless sensor networks. We highlight the primary motivation behind the development of each protocol category and explain the operation of different protocols in detail, with emphasis on their advantages and disadvantages. Furthermore, this paper compares and summarizes the state-of-the-art multipath routing techniques from the network application point of view. Finally, we identify open issues for further research in the development of multipath routing protocols for wireless sensor networks.

    Because students' ability to use statistics, which is mathematical in nature, is one of the concerns of educators, embedding within an e-learning system the pedagogical characteristics of learning is 'value added' because it facilitates the conventional method of learning mathematics. Many researchers emphasize the effectiveness of cognitive apprenticeship in learning and problem solving in the workplace. In a cognitive apprenticeship learning model, skills are learned within a community of practitioners through observation of modelling and then practice plus coaching. This study utilized an internet-based Cognitive Apprenticeship Model (i-CAM) in three phases and evaluated its effectiveness for improving statistics problem-solving performance among postgraduate students. The results showed that, when compared to the conventional mathematics learning model, the i-CAM could significantly promote students' problem-solving performance at the end of each phase. In addition, the combination of the differences in students' test scores were considered to be statistically significant after controlling for the pre-test scores. The findings conveyed in this paper confirmed the considerable value of i-CAM in the improvement of statistics learning for non-specialized postgraduate students.

    A modular typed dielectric barrier discharge (DBD) device is designed and tested for air and water remediation. The module is made of a number of DBD tubes that can be arranged in series or parallel. Each of the DBD tubes comprises inner electrode enclosed with dielectric barrier and arranged as such to provide a gap for the passage of gases. Non-thermal plasma generated in the gap effectively creates gaseous chemical reactions. Its efficacy in the remediation of gas stream containing high NOx, similar to diesel emission and wastewater containing latex, are presented. A six tubes DBD module has successfully removed more than 80% of nitric oxide from the gas stream. In another arrangement, oxygen was fed into a two tubes DBD to generate ozone for treatment of wastewater. Samples of wastewater were collected from a treatment pond of a rubber vulcanization pilot plant. The water pollution load was evaluated by the chemical oxygen demand (COD) and biological oxygen demand (BOD5) values. Preliminary results showed some improvement (about 13%) on the COD after treatment and at the same time had increased the BOD5 by 42%. This results in higher BOD5/COD ratio after ozonation which indicate better biodegradability of the wastewater.

    BACKGROUND: Late acute cellular rejection (LACR) is associated with poorer graft outcomes and non-adherence. Non-adherence to tacrolimus can be indirectly assessed by the intra-patient variability (IPV) of tacrolimus trough levels. The threshold of IPV associated with rejection is not known.

    METHODS: We conducted a case-control study comparing 25 patients with biopsy-proven LACR against 25 stable controls matched for age group, primary diagnosis and time post-transplant.     Network lifetime and energy efficiency are crucial performance metrics used to evaluate wireless sensor networks (WSNs). Decreasing and balancing the energy consumption of nodes can be employed to increase network lifetime. In cluster-based WSNs, one objective of applying clustering is to decrease the energy consumption of the network. In fact, the clustering technique will be considered effective if the energy consumed by sensor nodes decreases after applying clustering, however, this aim will not be achieved if the cluster size is not properly chosen. Therefore, in this paper, the energy consumption of nodes, before clustering, is considered to determine the optimal cluster size. A two-stage Genetic Algorithm (GA) is employed to determine the optimal interval of cluster size and derive the exact value from the interval. Furthermore, the energy hole is an inherent problem which leads to a remarkable decrease in the network's lifespan. This problem stems from the asynchronous energy depletion of nodes located in different layers of the network. For this reason, we propose Circular Motion of Mobile-Sink with Varied Velocity Algorithm (CM2SV2) to balance the energy consumption ratio of cluster heads (CH). According to the results, these strategies could largely increase the network's lifetime by decreasing the energy consumption of sensors and balancing the energy consumption among CHs.

    Software-defined networking (SDN) is a networking architecture with improved efficiency achieved by moving networking decisions from the data plane to provide them critically at the control plane. In a traditional SDN, typically, a single controller is used. However, the complexity of modern networks due to their size and high traffic volume with varied quality of service requirements have introduced high control message communications overhead on the controller. Similarly, the solution found using multiple distributed controllers brings forth the 'controller placement problem' (CPP). Incorporating switch roles in the CPP modelling during network partitioning for controller placement has not been adequately considered by any existing CPP techniques. This article proposes the controller placement algorithm with network partition based on critical switch awareness (CPCSA). CPCSA identifies critical switch in the software defined wide area network (SDWAN) and then partition the network based on the criticality. Subsequently, a controller is assigned to each partition to improve control messages communication overhead, loss, throughput, and flow setup delay. The CPSCSA experimented with real network topologies obtained from the Internet Topology Zoo. Results show that CPCSA has achieved an aggregate reduction in the controller's overhead by 73%, loss by 51%, and latency by 16% while improving throughput by 16% compared to the benchmark algorithms.

    With the rapid development of Internet of Things (IoT) technology, Terminal Devices (TDs) are more inclined to offload computing tasks to higher-performance computing servers, thereby solving the problems of insufficient computing capacity and rapid battery consumption of TD. The emergence of Multi-access Edge Computing (MEC) technology provides new opportunities for IoT task offloading. It allows TDs to access computing networks through multiple communication technologies and supports more mobility of terminal devices. Review studies on IoT task offloading and MEC have been extensive, but none of them focus on IoT task offloading in MEC. To fill this gap, this paper provides a comprehensive and in-depth understanding of the algorithms and mechanisms of multiple IoT task offloading in the MEC network. For each paper, the main problems solved by the mechanism, technical classification, evaluation methods, and supported parameters are extracted and analyzed. Furthermore, shortcomings of current research and future research trends are discussed. This review will help potential and new researchers quickly understand the panorama of IoT task offloading approaches in MEC and find appropriate research paths.

    Introduction: Many reports on investigations and treatments in UTI, however little, have been mentioned with regard to electrolyte abnormalities. Secondary pseudohypoaldosteronism (PHA) in UTI, though less common, is a known association. Features include hyponatremia and concomitant hyperkalemia. Objectives: We aim to highlight these uncommon sequelae in UTI to avoid incorrect diagnosis and unnecessary investigations. Study Design: Clinical data of patients admitted and referred to a pediatric nephrologist at the University Malaya Medical Center between May 2019 and October 2020 were collated and elaborated. Results and Discussion: We report three infants with hyponatremia and hyperkalemia during UTI episodes. Two infants were known to have posterior urethral valve (PUV) before the onset of UTI and one infant had UTI, which led to investigations confirming the diagnosis of bladder vaginal fistula. The electrolyte derangements were temporary and resolved within 48 to 72 h of treatment with intravenous fluid and appropriate antibiotic therapy. Out of three, only one had a hormonal study, which confirms PHA. Reduced aldosterone activity could be due to absolute reduction in aldosterone titer or lack of aldosterone responsiveness at tubular (other tissues) level. In the latter, aldosterone titer is elevated. The infant in our cohort who had hormonal evaluation had the mentioned electrolyte abnormalities with a markedly elevated aldosterone titer. This demonstrated defective action of the hormone at the level of mineralocorticoid receptor. Although the remaining two infants had no confirmatory hormonal study, all of them recovered within 48 h of hospital admission, after receiving appropriate management for the primary problem, which was UTI. We observed a slower recovery of hyponatremia in relation to hyperkalemia, but none of these infants required salt replacement upon discharge. Conclusion: Infants with severe UTI and deranged electrolytes should be screened for structural abnormality and vice versa. Not all infants require hormonal screening, but those who required prolonged salt replacement or showed involvement of other systems warrant further evaluation.

    Background: Adrenal insufficiency can result from impaired functions at all levels of hypothalamic-pituitary-adrenal (HPA) axis. We here studied risk factors associated with adrenal insufficiency in children receiving prolonged exogenous steroid treatment for nephrotic syndrome. Method:We performed low-dose Synacthen tests (LDSTs, 0.5 μg/m2) in children with steroid-sensitive nephrotic syndrome 4-6 weeks after discontinuation of the corticosteroid therapy. We measured early morning serum cortisol levels at baseline and at intervals of 10, 20, 30, and 60 min following the stimulation test. We defined normal HPA axis stimulation responses as those with peak cortisol cut-off values >550 nmol/L. Result:We enrolled 37 children for this study research. All children enrolled had normal early morning cortisol levels. However, 13 (35.1%) demonstrated HPA axis suppression (by LDST) 4-+6 weeks after discontinuation of oral prednisolone. Nephrotic syndrome diagnosed before 5 years of age (OR, 0.75; 95% CI, 0.57-0.99; p = 0.043), and steroid-dependence [OR, 5.58; 95% confidence interval (CI), 1.06-29.34; p = 0.042] were associated with increased risk of developing adrenal suppression after steroid discontinuation. Conclusion:HPA axis suppression, may go unnoticed without proper screening. A normal early morning cortisol level (275-555 nmol/L) does not exclude adrenal insufficiency in children with steroid-sensitive nephrotic syndrome. Further screening with LDSTs, particularly in children younger than 5 years at diagnosis, may be warranted.
