    To investigate the safety climate and knowledge, attitude and practice (KAP) on occupational safety and health (OSH)in the manufacturing industry, also to determine the association between safety climate factors and KAP of safety among manufacturing worker. A cross-sectional study was conducted among 59 respondents from two manufacturing plants located in Gebeng, Kuantan, Pahang. Most of the respondents were Malay (91.5%) and male (96.6%). Participants were administered a set of questionnaires (Cronbach alpha=0.674) that measured the safety climate as perceived by the workers towards their supervisor and KAP of the workers regarding safety-related matters at the workplace. Self-administered questionnaires consisted of 5 points Likert scale used to measure each of the items of safety climate and KAP. The scales for safety climate and KAP were probed using 16 items and 17 items in the questionnaires, respectively. The results were analysed using a non-parametric test, which is Spearman’s rho correlations and descriptive statistics. Bivariate analysis was performed.     Transport of fuel is essential to ensure supplies are delivered as per requested by the industrial sites or other demands. Numerous accidents have been reported and recorded in which loss of containment of hazardous chemicals occurred and led to disastrous outcomes. This paper presents the analysis of Boiling Liquid Expanding Vapour Explosion (BLEVE) due to loss of containment for Liquefied Petroleum Gas (LPG) road tankers. The main objective of this paper is to evaluate the potential consequences resulting from overpressure blast and thermal radiation of tankers carrying LPG to the people and the surrounding. The aim is also to compare the outcomes obtained from PHAST software simulator 8.11 with that of established mathematical model. Malaysia North-south Expressway (NSE) was selected as the location of the incident. The volume, weather parameters and properties of LPG were identified. It was found that the effect of BLEVE on people and structures was catastrophic. The results obtained from the mathematical model were similar with that modelled using PHAST software simulator.

    Medical professionals work in a constantly stressful environment. The nature of the profession requires routine extended working hours, but little is known about the effect of long working hours on selective physiological measures. A cross-sectional feasibility study was conducted from 1 October 2017 until 30 April 2018 on medical doctors in a tertiary health centre in northwest Malaysia. There was a total of 55 study participants recruited.     Digital Millennials (i.e., people born from 1980 to 2004) have often been portrayed as quick to embrace emerging technologies. The envisioned future of Industry 4.0 entails the blending of hardware, software, and people to complete work; and given their affinity for digital resources, technology, and social networking applications, the Millennials could be regarded as a much-needed asset in this future workforce. However, systematic research on how this cohort understands and reacts to safety issues surrounding the use of technologies has been lacking. In particular, behaviours such as impulsivity, online disinhibition, and risk-taking among them remain unclear. The present study investigated this gap with a sample of 203 Millennials of various nationality groups. Using the short version of the Urgency, Premeditation, Perseverance, Sensation Seeking, and Positive Urgency (Short UPPS-P) scale, the Revised Online Disinhibition Scale, and the Online Risky Behaviour scale, the results indicated that the most significant predictors of online risk-taking behaviours are lack of premeditation, positive urgency, and toxic disinhibition. Interestingly, though not hypothesised, was gender as a significant predictor of the outcome. The findings implied that these variables might be the most relevant target areas for interventions that could modify risky behaviours in cyberspace. The study’s findings are discussed in relation to the key challenges associated with e-safety where the future trends would be heading towards the gig economy and crowd-working.

    Solar thermal system or solar water heater system is one of the applications used to produce hot water in the residential sector. This paper describes HAZOP analysis and reliability assessment to evaluates the potential hazard and system probability for the closed loop solar thermal system applied for the residential area. Hazard identification for the main system components is analyzed while Fault Tree Analysis (FTA), Reliability Block Diagram (RBD) and Weibull distributions performed to determine the reliability for the overall system. The result shows that there are 49 potential hazards for the system with failure probability at 0.23822 and the reliability is 0.9693. Subsequently, this study determined the potential hazards for the system which can be anticipated by the residential consumer for the safety aspect. Furthermore, the evaluated reliability result shows that the application of closed loop type solar water heater system at residential premises is highly recommended due to its long lasting operational condition.

    Industries in Malaysia are entering a period of major disruption caused by new technologies such as Artificial Intelligent, Robotics, Blockchain, Nanotechnology as well as Building Information Modelling (BIM) and the Internet of Things (IoT). In this fourth industrial revolution where information is generated and exchanged at a rapid and huge scale, its reliability is of paramount importance. The success of Occupational Safety & Health Management System (OSHMS) is highly dependent on the reliability of the information gathered and used, where a large number of intermediaries authenticate the information to establish trust between the stakeholders. Blockchain technology is able to do verification by virtue of secured distributed storage brings about a paradigm shift in the way we establish trust. This paper gives an overview of the potential use of Blockchain technology for Occupational Safety & Health Management System. The discussions focused on the benefits and challenges of implementing the Blockchain technology in OSHMS. The conclusion is drawn based on the strength in the characteristics provided by the Blockchain technology itself.

    Radon exists naturally in the air. It can accumulate inside the buildings which may affect the indoor air quality. Radon is a radioactive gas that produces alpha particles during decay time. The alpha particles might cause harm to the human lungs and stomach. Inhalation of radon is one of the causes of lung cancer disease. Samples of inhaled radon in different rooms at the workplace were taken hourly through a passive diffusion chamber. The detection method was done using Alpha Spectrometry. The short term measurement was applied in the study to monitor the average weekly radon reading in different rooms in the Medical Imaging Laboratory of the University of Selangor (UNISEL). All tested rooms showed the existence of radon gas with different concentrations. Some of them showed the maximum reading of radon concentration which was higher than the radon action level of 148Bq/m3 or 4pCi/l. Their weekly average of radon concentration is contributing almost 50% of the accumulated radon concentration in the laboratory. It is highly recommended that monitoring the concentration of radon in indoor air is performed to ensure it is at a safe and healthy level.

    he practice of 5S has been adopted by many organizations as a method to organize the workplace towards reducing waste and improving productivity. Moreover, the practice also promotes a safe and efficient environment in organisations. As safety performance of the workplace is among the key issues in most industries including universities to become world class organizations, this study seeks to address empirically the impact of 5S practice on safety management performance. In this context, a survey questionnaire is employed and distributed to 60 employees of the selected university to examine the employees’ perception on the adoption of 5S practice as well as the impact of 5S practice towards safety management performance. The questionnaires were validated by a pilot test with an acceptable value of Cronbach's alpha. Overall findings highlight there was positive perceptions toward 5S practice among the respondents. The results also revealed that 5S Audit has a very strong positive correlation with safety management performance. Furthermore, 5S Training and Top Management Commitment were (2) factors that strongly correlated to safety management performance. These factors are highly important in contributing toward the success of safety management performance. Additionally, since most of the employees were not exposed with Occupational Safety and Health (OSH), this study reveals moderate negative correlation between compliance to OSH Requirements and safety management performance.

    The Department of Occupational Safety and Health Malaysia (DOSH) is the authority responsible to safeguard the occupational safety, health and welfare of workers in Malaysia. Occupational noise-related hearing disorders are the leading type of occupational diseases recorded by DOSH every year. Occupational Health Doctor (OHD) is a competency recognized by DOSH and their scope of duties in industries is currently confined to conduct medical surveillance for workers with hazardous chemical exposure and confined space medical fitness examination. The duties of OHD are good to be expanded by empowering more of their roles in industrial activities involving other legislations under DOSH. The noise exposure regulation under the Factories and Machinery Act (FMA) 1967 has been governing the worker’s exposure to hazardous industrial noise and preventing noise-induced hearing loss (NIHL) since 1989. However, the provisions of the regulation need to be strengthened in some essential medical elements of the Hearing Conservation Programme (HCP) for a comprehensive prevention of NIHL at the workplace. Recently enacted Occupational Safety and Health (Noise Exposure) Regulations 2019 offer a wider coverage of workers in ten sectors of industries applicable under the Occupational Safety and Health Act (OSHA) 1994. The current regulatory requirements for management of workplace noise have many improvements compared to the existing law. Enhancement in the medical requirements of industrial audiometry is made prominent by involving OHDs to interpret audiograms and conduct medical examination for workers. The reporting of occupational noise-related hearing disorders to DOSH is outlined better in the new regulation. The occurrence of NIHL and other related hearing disorders are expected to reduce eventually after the introduction of Occupational Safety and Health (Noise Exposure) Regulations 2019. The OHDs will play a pivotal role in industrial audiometry and prevention of hearing disorders among the working population.

    The purpose of this study is to validate Sugar Craving Assessment Tool (MySCAT) among type II diabetes mellitus patients. A total of 168 respondents were recruited to participate in this cross sectional study. It was conducted in out-patient health clinics in Kuala Terengganu and Kuantan. Patients’ sugar craving and dietary behavior were measures using structured questionnaires which were MySCAT, three-day dietary recall and demographic data. The interview sessions were conducted by a dietician. MySCAT internal consistency test had a Cronbach’s alpha value of 0.75 and showed a significant correlation (r=0.56, p < 0.001) with actual sugar intake collected via three days diet recall. ROC analysis reported a cut-off point for MySCAT as 16.5; the sensitivity value of 0.83 and 1-specificity of 0.38. 62% of respondents were categorized as cravers and 38% as non-cravers. Male and female repondents had no significant differences in craving status. The mean intake of sugar was 46 g/day (SD= 2.26), respondents had a relatively high intake of sugar in comparison to their recommendation. This study found that MySCAT provides an easy efficient tool which is sensitive enough to identify those with sugar craving problem. It also provides an overview of patients’ dietary intake and points out their problem with dietary intake compliance. We suggest MySCAT as an important tool that can assist dietitians in their consultation session.

    In Malaysia, breast cancer is the commonest cancer among women indicating that early diagnosis and screening practice is important to increase the survival rate. Breast self-examination (BSE), clinical breast examination (CBE) and mammography are the main screening practices for breast cancer. Knowledge and awareness of breast cancer can play an important role in aiding early detection of breast cancer. Little is known about the knowledge of breast cancer and screening practices among the non-academic staff in IIUM, Kuantan. As such this study aims to determine the knowledge and screening practices of breast cancer amongst the non-academic staff at the International Islamic University Malaysia (IIUM), Kuantan Campus. A cross-sectional study was conducted on 100 nonacademic staff from ten different departments in IIUM, Kuantan from March to April 2017. Data were collected using a self-administered questionnaire on knowledge of breast cancer and screening practices. The relationship between socio-demographic characteristics and knowledge of breast cancer and breast cancer screening practices were carried out using chi-square. No significance difference was found between knowledge of breast cancer and socio-demographic factors. However significant relationships were found between the age of respondents and BSE as well as the level of education of respondents and CBE. Most non-academic staff possessed an average level of knowledge on breast cancer and screening practices due to possibly ease of access to information. Low performance of breast screening practices was indicated possibly due to lack of awareness, misconception of the need and benefits derived from such screening practices.

    Gynura procumbens, commonly known as ‘sambung nyawa’ in Malaysia has been used
    traditionally as remedies for anti-inflammatory, anti-hyperlipidimic and anti-hyperglycemic. The
    purpose of the present study was to qualitatively evaluate the antimicrobial effects of Gynura procumbens extracts. effects of the sample were determined by disc diffusion method against two bacteria and two fungi namely E. coli, S. aureus, C. albicans and S. cerevisiae. The results showed that the acidic extract of G. procumbens has positive reactions towards E. coli, S. aureus and C. albicans with the presence of zone of inhibition at the concentration of 150 mg/mL. Following the positive reaction, the minimum inhibitory concentration (MIC) of the acidic extract was then evaluated by broth dilution method. The MIC of E. coli and S. aureus were determined at concentration 37.5 mg/mL and 75 mg/mL for C. albicans. It indicated that acidic extracts at lower concentration could inhibit the bacteria, but high concentration of extracts was required in the inhibition of the fungi. It can be concluded that, the present study proves that there is potential of antimicrobial effects in Gynura procumbens leaves extracts.

    The purpose of this study was to translate and adapt the Vestibular Rehabilitation Benefit Questionnaire (VRBQ) into Malay (My-VRBQ). This is to add on to the limited number of
    vestibular questionnaire available in Malay version. After the permission from the original
    author was obtained, the process of adaptation began with forward-translation by the author with
    audiological background and a linguistic expert. Then, the process of backward translation into
    English were completed by three bilinguals who were proficient in both Malay and English. The final version of the draft was evaluated for its content validity, which was conducted by five professionals with audiological or speech pathology backgrounds. Additionally, its face validity was determine amongst 32 respondents with diverse backgrounds. Ten participants diagnosed with vestibular problems were recruited in the My-VRBQ for internal consistency validation process. They were enlisted from two hospitals in the East Coast of Malaysia. Based on the comments and
    suggestions made by the validators; some changes in terms of the appropriateness of the instructions, items and the sentence structure were made. In the consistency analysis, the My-VRBQ revealed good internal consistency based on Cronbach alpha values (0.77 to 0.96). A translated and validated My-VRBQ has the potential to be used clinically to document vestibular rehabilitation benefits. Nevertheless, future studies are encouraged to further support the present study findings.

    This paper aimed to describe variation in apical corneal curvature between unilateral primary pterygium and normal adults utilizing simulated-K and corneal irregularity measurement corneal indices. A total of 100 participants comprise 50 unilateral primary pterygium eyes from 50 patients and 50 normal adults were recruited in this study. Diagnosis and classification of primary pterygium were done by a consultant ophthalmologist (KMK). Standard optometric examinations were performed in all participants. Simulated-K (SimK) and corneal irregularity measurement (CIM) was objectively measured using a corneal topographer. Three measurements based on best image quality for SimK and CIM were taken by single operator in a same visit. Difference for both SimK and CIM parameters between primary pterygium and normal groups were determined via independent T-test. Overall mean and standard deviation (n = 120) of SimK and CIM were found higher in primary pterygium group (9.06 ± 4.49 D and 11.48 ± 3.12) compared to normal (1.63 ± 0.67 D and 0.62 ± 0.24) respectively. Independent T-test results showed significance difference in SimK and CIM values between primary pterygium groups and normal (both P< 0.001). Both SimK and CIM corneal indices can be an important tool in describing and predicting changes on the corneal curvature due to pterygium progression. However, it is worth to note that the detectability of changes in anterior corneal curvature is limited to 5 mm of central corneal curvature.

    Most studies on predictors of vocational outcomes are cross-sectional and results are varied. This study aimed to examine the vocational rates of patients with first-episode psychosis (FEP), identify factors predicting a lack of engagement in age-appropriate roles, and evaluate the predictive ability of a model with baseline sociodemographic information and 2-year symptom and functioning trajectories on vocational outcomes. The Singapore Early Psychosis Intervention Program (EPIP) has maintained a standing database on patient clinico-demographic information. The primary outcome, vocational status, was operationalized as "meaningfully employed", that is, being gainfully employed or engaged in an age-appropriate role, and "unemployed". Using logistic regression, the predictive ability of the proposed model was evaluated. Vocational data was available for 1177 patients accepted into EPIP between 2001 and 2012. At the end of two years in the service, 829 (70.4%) patients were meaningfully employed and 348 (29.6%) patients were unemployed. The binary logistic regression model on the prediction of 2-year vocational outcomes yielded an AUC of 0.759 (SE = 0.016, p-value

    Understanding the excited-state dynamics and conformational relaxation in thermally activated delayed fluorescence (TADF) molecules, including conformations that potentially support intramolecular through-space charge transfer, can open new avenues for TADF molecular design as well as elucidate complex photophysical pathways in structurally complex molecules. Emissive molecules comprising a donor (triphenylamine, TPA) and an acceptor (triphenyltriazine, TRZ) bridged by a second donor (9,9-dimethyl-9-10-dihydroacridin, DMAC, or phenoxazine, PXZ) are synthesized and characterized. In solution, the flexibility of the sp3-hybridized carbon atom in DMAC of DMAC-TPA-TRZ, compared to the rigid PXZ, allows significant conformational reorganization, giving rise to multiple charge-transfer excited states. As a result of such a reorganization, the TRZ and TPA moieties become cofacially aligned, driven by a strong dipole-dipole attraction between the TPA and TRZ units, forming a weakly charge-transfer dimer state, in stark contrast to the case of PXZ-TPA-TRZ where the rigid PXZ bridge only supports a single PXZ-TRZ charge transfer (CT) state. The low-energy TPA-TRZ dimer is found to have a high-energy dimer local triplet state, which quenches delayed emission because the resultant singlet CT local triplet energy gap is too large to mediate efficient reverse intersystem crossing. However, organic light-emitting diodes using PXZ-TPA-TRZ as an emitting dopant resulted in external quantum efficiency as high as 22%, more than two times higher than that of DMAC-TPA-TRZ-based device, showing the impact that such intramolecular reorganization and donor-acceptor dimerization have on TADF performance.

    Introduction: This is a proto-type product which is based on Frankincense essential oil and hydrosol. Methods: Three oleo gum resin species, namely; Boswellia carterii (BC), Boswellia frereana (BF), and Commiphora myrrha (CM) of Burceraceae family were extracted for their essential oil by hydro-distillation. They were screened for their poten-tial of anti-cariogenic activity by in-vitro experimental study of two main bacterial species (Streptococcus mutans and Lactobacillus spp), which are considered the main cause of dental and mouth diseases. Results: Methanol and acetone extracts of the three plants inhibited the growth of the bacteria. However, BF-methanol extract shows the greatest inhibition followed by BC and CM respectively. Hence, the obtained result encourages proceeding further thorough investigation to benefit the positive outcomes of these plant extracts in terms of introducing new potential antimicrobial formulations, such as mouth wash which can be used for mouth cleansing and protection from the diseases such as mouth ulcers, gingivitis, sinusitis, glandular fever and brucellosis as well as dental caries. This result can be converted to Boswellia Mouthwash Essential Oil (BosMEO) and Boswellia Mouthwash Hydrosol (BosMoHy) based products. This new plant extract product can be exploited for further research for its potential used as moth infection natural treatments such as mouth ulcers, gingivitis, sinusitis, glandular fever, brucellosis as well as respi-ratory problem. It is free of synthetic chemicals, organic, natural, plant based, and halal with no major health side effects. Conclusion: Plant-based product which is free from synthetic chemicals and with minimal side effects will satisfy its quality efficiency.

    Introduction: Benzimidazole analogues are bicyclic compounds that had been synthesized comprising the fusion of benzene and imidazole. It gains interest in research as it poses numerous therapeutic potential such as anti-ulcer, anti-malarial, anti-helminthic, anti-fungal, anti-inflammatory, and anti-cancer. Hence, this work aims to screen novel benzimidazole analogues using MTT assay for potential anti-proliferation activities on gastric cancer, which is the second cause of cancer-related death. Methods: MTT assay was conducted following standard protocol on HGT-1 gastric cancer cells. Cells were seeded and allowed to attach overnight before being introduced with various con-centration of benzimidazole analogues up to 72 hours and the optical density of the MTT was recorded using 560 nm wavelength. Two-Way ANOVA was used to analyse all data, followed by post-hoc Tukey test and the structure analysis relationship was analysed using MTT result. Results: From five analogues, only compound 4 showed an-ti-proliferation activity with IC50 8.212 ± 0.813 μM at 72 hours. Compound 4 had hydroxyl group at ortho- and para- position and remarkably, compound 2 which contained the hydroxyl group at ortho- and meta- position together with compound 5 which contained the combination of meta- and para- induced proliferation on gastric cancer. Conclusion: Different position of hydroxyl group on the benzene ring gives different activities on gastric cancer and from the experiment, only compound 4 had the anti-proliferative activity.

    ntroduction: Nasopharyngeal carcinoma (NPC) is a prevalent cancer among human population in Southern China, Hong Kong and Southeast Asia. In Malaysia, NPC is the fourth most common cancer in both sexes, predominantly in the Chinese. Epstein-Barr virus (EBV) infection is known to be highly associated with NPC. Fibroblast growth factor receptor-4 (FGFR4) is part of the family of tyrosine kinase receptors that regulate cell survival, differentiation and pro-liferation. The binding of FGFR4 ligands such as fibroblasts growth factors (FGFs) has been shown to activate various oncogenic signalling pathway including MAPK, Ras and PI3K-Akt pathways. In the past, FGFR4 has been shown to promote tumorigenesis and tumour progression in various cancers such as liver, colon, breast and pancreatic and gastric cancers. However, its role in NPC establishment and pathogenesis is under-explored. This study aimed to evaluate the FGFR4 expression in NPC using various cell lines and its potential as a therapeutic target for NPC treat-ment by gene silencing. Methods: The basal FGFR4 level of NPC (EBV-positive: C666-1 and EBV-negative: HONE1 and HK1) and nasopharyngeal epithelial (NPE) normal (NP69 and NP460) cell lines was determined by western blot analysis and RT-qPCR. FGFR4 level at different time points (0, 24, 48, and 72 hours) in HONE1 and C666-1 cell lines were determined by western blot analysis. Luminescence-based assay was performed to determine the cell prolifer-ation of NPC cells in correlation with the FGFR4 expression. NPC cells were then treated with the optimised FGFR4 siRNA or FGFR inhibitor, BLU-9931 and the silencing/ inhibition of FGFR4 expression was confirmed by western blot analysis. The effect of FGFR4 inhibition on the cell proliferation and aggressiveness of NPC cells was then investigat-ed through wound healing assay and invasion marker analysis. Results: Out of the five tested cell lines, HONE1 and C666-1 highly expressed FGFR4, NP69 showed very low expression while HK1 and NP460 did not express FGFR4. In the time-point study, the FGFR4 level of HONE1 and C666-1 peaked at 24-48 hours which is the exponential phase of cells. Following that, the FGFR4 level decreased corresponding to the decreased cell growth rate due to the nutrient deprivation. siRNA experiments showed that 6.25nM of four siRNAs (5, 6, 9 and 10) could effectively target and silence the FGFR4 expression of HONE1, but not in C666-1 even up to 250nM was tested. When BLU-9931 was used, only modest inhibition was observed in both cells at 3uM. Compared to the untreated control, FGFR4-inhibited HONE1 exhibited decreased cell proliferation rate. Cell migration and invasion capabilities of HONE1 were also significantly reduced following the FGFR4 silencing, suggesting the potential of utilising FGFR4 as the therapeutic target. Conclusion: FGFR4 is highly expressed in C666-1 (EBV-positive) and HONE1 (initially EBV-positive, but lost EBV genome in subsequent in vitro passage) NPC cells, but not in EBV-negative HK1 NPC cell and normal NPE cells. FGFR4 gene silencing effectively inhibited the cell proliferation, migration and invasive potentials of NPC cell line. These findings highlight the therapeutic value of targeting FGFR4 for NPC treatment. Further investigations are war-ranted to reveal the molecular mechanism and the possible role of EBV in regulating FGFR4 pathway.

    Introduction: Leptospirosis is a re-emerging zoonotic disease caused by Leptospira bacteria. The clinical manifes-tations of leptospirosis include mild-fever to a severe or even fatal. Increased levels of inflammatory cytokines pro-duced in response to the Leptospira infection by the host immune system were hypothesized as among the causes of severity in leptospirosis. Besides the classical presentation with the triad of febrile, jaundice, and renal failure, patients with leptospirosis also can pose with predominant sign and symptoms of pulmonary involvement. This study aimed to compare the levels of TNF-α, IL-1b, IL-4, IL-6, IL-8, IL-10, IL-12, IL-13, IL-17A, IL-18, and IL-22 In the plasma of samples of leptospirosis patients with and without pneumonia. Methods: Circulating cytokine levels in plasma were measured in seventeen patients hospitalized and diagnosed with leptospirosis in Malaysia (January 2016 – December 2017) and nineteen healthy individuals as controls. Patients were categorized into leptospirosis without pneumonia (n=12) and with pneumonia (n=5). Cytokine was measured using SimplePlexTM assays (San Jose, CA, USA). Measurement was performed in triplicate and statistical analysis was conducted using Graphpad® Prism v6 (San Diego, CA, USA). Results: Elevation of plasma TNF-α, IL-6, IL-8, IL-10, IL-18, and IL-22 levels were observed among leptospirosis patients with pneumonia compared to without, although no statistical differences were observed between these two groups. Conclusion: There are no significant differences observed between the levels of plasma TNF-α, IL-6, IL-8, IL-10, IL-18, and IL-22 in patients with pneumonia compared to without.
