    Introduction: Pertussis is known to cause infection and reinfection to everyone irrespective of ages and countries. Therefore, adults do require vaccination for protection against pertussis infection especially the HCW. However, the pertussis vaccine coverage is low among HCW due to low-risk perception. Protection Motivation Theory (PMT) is one of the most cited theories to explain risk perception and intention to change. Therefore, we developed a questionnaire based on the subconstructs of the PMT to assess the acceptance of the pertussis vaccine amongst the HCWs in Sabah and Sarawak. The motive of this study is to validate this questionnaire to see its validity and reli- ability. Method: Data was collected using a self-administered questionnaire via an online survey (Monkey Survey). The questionnaire was given to 250 HCW. Items that were identified as a problem were modified to increase reli- ability. Further validation was done among 853 HCWs working in various parts of Sabah and Sarawak. Results: The Cronbach alpha of the overall construct of PMT during the first pilot study was 0.66 and improved to 0.82. Principal components factor analysis using varimax rotations showed that the first four factors explained 28%, 2%, 9% and 5% of the variance respectively. Both the one level and two-level modelling indicated that it’s a good fit model. Conclu- sion: The study instrument that was developed for the study has been tested and proven to be relevant to assess the risk perception of an HCW towards pertussis.

    Introduction: Iron deficiency anaemia (IDA) is endemic especially in the under-developed and developing countries and is a major public health concern. Improving nutrition is one of the ways to alleviate this condition. Consumption of locally available and affordable food such as date palm and goat milk which are rich in iron is one of the ways to overcome IDA. This study is aimed at evaluating the effect of date palm and goat milk supplementation on hae- matological parameters and iron bioavailability in IDA rats. Methods: 24 male Wistar rats were randomly divided into normal control and IDA group. The normal control was fed with normal diet and water ad libitum while the IDA group were fed on iron-deficient diet for two weeks to induce iron deficiency. The IDA rats were further divided into subgroups; each being supplemented with date palm, goat milk, a combination of date palm and goat milk, and ferrous fumarate as positive control. Blood were collected after 28 days for haematological parameters and iron profile determination. Iron bioavailability was assessed using the haemoglobin regeneration efficiency (HRE) index. Data was analysed by Student T Test and ANOVA using SPSS 23.0 software with p value < 0.05 considered as sta- tistically significant.     Introduction: The outbreaks of foodborne diseases have been linked to the consumption of contaminated seafood. This research aims to screen the bacteria from the sea cucumbers Acaudina molpadioides collected from Pulau Langkawi. Methods: A total of 22 sea cucumber samples were collected randomly from Pulau Langkawi, Kedah, Malaysia. The samples were isolated and identified for the presence of bacteria using the conventional culture-based method. Presumptive bacteria colonies were subjected to various biochemical and antimicrobial susceptibility tests. Results: There were no bacterial growth in Hektoen Enteric (HE) agar and Thiosulphate-Citrate-Bile Salt (TCBS) agar. Positive samples were isolated from MacConkey (MAC) agar with 6 samples were Staphylococcus spp. (27.27%), 14 samples were Proteus spp. (63.63%) and 2 samples were Bacillus spp. (9.01%). Among these isolates, highest resistance was found against Ampicillin (45%) followed by Tetracycline (40%). Conclusion: The results indicate that the sea cucumbers Acaudina molpadioides were contaminated with potential bacteria. There is a need for adequate consumer protection measures.

    Introduction: Proper distribution of human resources is an important factor ensuring high-quality performance and sustained service quality. The aim of this study was determining the workload pressure among medical officers in health clinics (HCs) in Kelantan. Method: A record review survey was conducted between January and April 2019 using human resources data for 2018 involving HCs in Kelantan. It included all the HCs in Kelantan and excluded community clinics. Workload pressure was determined using a tool known as Workload Indicator of Staffing Needs, developed by World Health Organization. A high workload pressure was defined as a ratio between required and acquired medical officers of less than 1. The data were presented descriptively using as frequencies and percentages. Results: All 85 HCs in Kelantan were involved in the study; 90% (9/10) of the Kelantan districts recorded high work- load pressure. Moreover, 68.2% (58/85) HCs had high workload pressure. Tanah Merah, Tumpat, Pasir Mas, and Kota Bharu had the most HCs with high workload pressure, and most such HCs were found in areas with a high-den- sity population, requiring huge coverage. Conclusion: The Kelantan State Health Department should develop better human resource distribution strategies to ensure the sustainability of quality care in HCs.

    Introduction: Diabetes is a chronic metabolic disease and noted to be incidence is intensifying globally and contem- plated as epidemic. The study is aimed to assess the coronary artery disease risk profile associated diabetes mellitus patient and to identify the clinical pharmacist care services in the management and to control the risk burden in the clinical practice. Method: A prospective observational study was conducted among the consecutive patients of coronary artery disease associated diabetic patients in a tertiary care teaching hospital over 6 months period. A sam- ple of 150 patients was recruited in the study. Data analysis was done with graph pad prism software 5.01. Results: The present study revealed that coronary artery disease in diabetes was more prevalent in age group between 41-50 years. About 54.66% patients with hyperlipidemia were at risk to develop the coronary artery disease complication. Glycated hemoglobin test was detected in 40% of the patient showing abnormal levels and around 43.33% of patient had an abnormal fasting blood sugar level. The study showed only 32% of patients was prescribed Insulin & oral hy- poglycemic agents and 13 % were treated with statins. Conclusion: It could be concluded that the causative factors should be controlled and treated with an early need for amalgamation of clinical pharmacist care services with the health care team on life style modification counseling could ultimately improve the patient health outcomes and also lowers progression of coronary artery disease risk complications among diabetic patients.

    Introduction: Unequal retinal image size (RIS) or aniseikonia is usually related with anisometropia. Higher dif- ferences of RIS may manifest symptoms such as dizziness, headache or disorientation. In worst case might cause suppression that leads to amblyopia. Current study aims to evaluate the consistency of aniseikonia measurement in Smart Optometry smartphone application among myopic, hyperopic, and astigmatic simulated anisometropia and real anisometropia groups. Methods: Fifteen real anisometropes (refractive error; -0.50 until -6.00 diopters; D) and fifteen emmetropes (refractive error: -0.25 until +0.50D) were recruited. Real anisometropes wore their habitual spectacle correction while each emmetropes were fitted using soft contact lenses of +4.00DS, -4.00DS and -4.00DC with base curve 8.6 and total diameter 14.2mm in random order to mimic myopic-, hyperopic- and astigmat- ic-anisometropia before testing. Participants with any ocular disease and binocular vision problem were excluded. The consistency of aniseikonia measurement was determined in two visits, separated by at least 24-hour interval. Three repetitive measurements were taken in each visit. Results: Independent t-test and paired t-test showed that real and simulated anisometropia gave insignificant aniseikonia percentage, p>0.05. ICC findings revealed moder- ate-to-good agreement for all simulated and real groups. Bland Altman analysis between two visits exhibited good agreement among all simulated group; myopic (mean difference 0.2047; 95%CI:-1.1386-1.549), hyperopic (mean difference 0.2200; 95%CI:-0.9286-1.3686) and astigmatic (mean difference 0.2533; 95%CI:-0.7114-1.2180). Real anisometropes demonstrated good agreement with bias value of 0.2247(95%CI:-0.9162-1.3656) using Bland Altman plot. Conclusion: Smart Optometry application provides consistent measurement of aniseikonia regardless any types of anisometropia.

    Introduction: In this study, we evaluated the kinematics of lower limb during early and late phases of 2km time trial on stationary rowing ergometer among Malaysian male rowers. Methods: Seventeen national-level rowers voluntari- ly participated. Three dimensional lower limb kinematics data were collected to represent the first 400 m (i.e., early) and the last 400m (i.e., late) phases of 2km time trial on a stationary ergometer. The kinematics data at sagittal, frontal and transverse planes of dominant leg during catch and finish positions were compared across early and late phases of the time trial using paired T-test. Results: The kinematics of lower limb joints at three planes were not significantly different during early versus late phases of 2km time trial among male senior rowers except for hip flexion at finish (p=0.411), ankle rotation at catch (p=0.779) and ankle abduction at finish (p=0.677). Conclusion: Lower limb kine- matics particularly the hip flexion, ankle rotation and ankle abduction may change across early and late phases of 2km time trial due to fatigue. Coaches and rowers should monitor these motions during fatiguing rowing piece and develop necessary injury prevention measures.

    Purpose: The purpose of this study was to determine the rates of, and factors associated with, eyecare utilization and spectacle affordability among Singaporeans with vision impairment (VI).

    Methods: We included adults with VI from their second visit of the Singapore Epidemiology of Eye Disease Study. Data on eyecare utilization and spectacle affordability were collected. Low eyecare utilization was defined as no eye check ever or eye checks not even once per year in reference to at least once per year. Difficulty affording glasses was defined as glasses being rated as expensive in reference to not expensive.

    Results: There were 985 adults (14.5%; 415 Malays, 260 Indian, and 310 Chinese; mean age [SD]: 69.5 [10.2] years; 55.4% women) with VI who answered the above questions, were included. Of these, 624 (63.4%) wore glasses. The rates of low eyecare utilization and difficulty affording eyeglasses were 31% and 63%, respectively. Compared to Chinese (23.8%) and Indians (18.8%), Malays (57.4%) had the highest rates of low eyecare utilization (P < 0.001), and most difficulty affording eyeglasses (47.2% vs. 26.1% and 26.6% in Chinese and Indians, respectively; P < 0.001). Younger age, low socioeconomic status, absence of diabetes, absence of self-reported eye conditions, and poor vision were independently associated with low eyecare utilization, whereas older age and female sex was associated with difficulty affording glasses.

    Conclusions: In this multi-ethnic population with VI, almost one-third had low eyecare utilization and nearly two-thirds reported difficulty affording eyeglasses.

    Translational Relevance: This will inform strategies, such as tailored eyecare utilization awareness campaigns and awareness of available subsidy schemes for at-risk Singaporeans, such as Malays.

    OBJECTIVES: Chronic wounds are common, costly and impair quality of life, yet epidemiological data are scarce. We aimed to estimate the incidence trend of a multiethnic Asian population.

    DESIGN: Retrospective cohort study.

    SETTING: Singapore's nationwide claims database.

    PARTICIPANTS: Singaporeans and permanent residents.

    OUTCOMES: Patients were identified by International Classification of Disease, Ninth Revision, Australian Modification (ICD-9-AM) and ICD-10-AM codes from all admissions between 2000 and 2017, and categorised according to aetiology: venous, arterial, diabetic and pressure. Comorbidities were extracted from a national database of Charlson Comorbidity Index scores.

    RESULTS: Between 2000 and 2017, 124 023 wound-related claims among 86 631 patients were identified. Age-specific rate (ASR) and age-adjusted incidence rates of all wounds increased over 18 years, with greatest increases among those aged ≥80. In 2017, the median age of patients was 74 (IQR 63-84). Half were male (51%). 70% were ethnic Chinese, 15% Malay and 9% Indian. In 2017, the crude incidence rate (CIR) was 15 per 100 000 persons (95% CI 14 to 16) for venous wounds, 56 (95% CI 53 to 58) for arterial, 168 (95% CI 164 to 173) for diabetic and 183 (95% CI 179 to 188) for pressure wounds. The CIR of any chronic wound was 296 (95% CI 291 to 301). ASRs were greatest in patients aged ≥80: 92 (95% CI 74 to 112) for venous, 478 (95% CI 436 to 522) for arterial, 1791 (95% CI 1710 to 1876) for diabetic, 3647 (95% CI 3530 to 3766) for pressure and 4277 (95% CI 4151 to 4407) for any wound. Compared with the Chinese, Indians had thrice the ASRs of venous and arterial wounds and double the ASR of diabetic wounds. Malays had double the ASRs of arterial and diabetic wounds.

    CONCLUSIONS: Chronic wounds are common in the elderly with significant ethnic disparities in this Asian cohort. With the incidence expected to rise with ageing populations, it is crucial to address health disparities and evaluate utilisation and cost to inform clinical practice and health policy.

    DNA strand displacement is an attractive, enzyme-free target hybridization strategy for nano-biosensing. The target DNA induces a strand displacement reaction by replacing the pre-hybridized strand that is labeled with gold nanoparticles (AuNPs). Thus, the amount of displaced-AuNP-labeled strand is proportional to the amount of target DNA in the sample. The use of a magnetogenosensing technique to isolate the target DNA allows for a simple, one-pot detection approach, which minimizes possible carry-over contamination and pipetting errors. We sought a proof-of-concept for this technology in its ability to detect DNA-equivalent of hepatitis E virus (HEV), which causes acute viral hepatitis for which rapid and simple diagnostic methods remain limited. Signal detection was done via visual observation, spectrophotometry, and electrochemistry. The sensor demonstrated good sensitivity with detection limits of 10 pM (visual), 10 pM (spectrophotometry) and 1 fM (electrochemical). This sensor also exhibited high specificity for real target amplicons and could discriminate between perfect and mismatched sequences. Lyophilized biosensor reagents stored at 4 °C, 25 °C, and outdoor ambient temperature, were stable for up to 90, 50, and 40 days, respectively. The integration of magnetic separation and target DNA-induced strand displacement reaction in a dry reagent form makes the sensing platform easy-to-use and suitable for field settings.

    Background: Chronic pain is a growing public health concern affecting 1.5 million people in Canada. In particular, it is a concern among the expanding immigrant population, because immigrant groups report higher pain intensity than non-immigrants. In 2011, the Indian population became the largest visible minority group and continues to be the fastest growing. Though the prevalence of chronic pain among Canadian Indians is unknown, research has found a higher prevalence among Indian women than men in India, Malaysia, Singapore, and the United Kingdom, with women reporting more severe pain. An understanding of how pain is experienced by this particular group is therefore important for providing culturally sensitive care.

    Aims: This study explores the lived experiences of chronic pain among immigrant Indian women in Canada.

    Methods: Thirteen immigrant Indian women participated in one-on-one interviews exploring daily experiences of chronic pain.

    Results: Using thematic analysis informed by van Manen's phenomenology of practice, four themes emerged: (1) the body in pain, (2) pain in the context of lived and felt space, (3) pain and relationships, and (4) pain and time. Women revealed that their experiences were shaped by gender roles and expectations enforced through culture. Specifically, a dual gender role was identified after immigration, in which women had to balance traditional household responsibilities of family labor and care alongside employment outside the home, exacerbating pain.

    Conclusions: This research uncovers the multifaceted nature of chronic pain and identifies factors within the sociocultural context that may place particular groups of women at greater risk of living with pain.

    When examining spontaneously recovered memories of childhood sexual abuse, victims report that there had been periods in which they had forgotten the abuse. However, there are sometimes people with whom the victim had spoken about the abuse during the period in which the victim had supposedly forgotten the abuse, suggesting the victim had not forgotten the abuse but the prior recall of the abuse. The underestimation of previous knowledge is termed the forgot-it-all-along effect. The goal of the present study was replicating the results of a laboratory study that had provided a theoretical understanding for the forgot-it-all-along effect by showing that people have difficulties remembering "remembering" when the memory had previously been recalled in a different context. The effect was replicated by using the same neutral context sentences, suggesting the finding was robust. We also extended the experimental design by using positive and negative context sentences, but it did not become smaller when the positive sentences provided the different context or larger when the negative sentences provided the different context. Although the sample sizes were sufficiently large to provide statistical power for the forgot-it-all-along effect, they may not have been sufficiently large to observe the moderation effects of emotional context.

    Centella asiatica is notable for its wide range of biological activities beneficial to human health, particularly its cognitive enhancement and neuroprotective effects. The α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptors are ionotropic glutamate receptors mediating fast excitatory neurotransmission essential in long-term potentiation widely thought to be the cellular mechanism of learning and memory. The method of whole-cell patch-clamp was used to study the effect of the acute application of Centella asiatica extract on the α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor-mediated spontaneous excitatory postsynaptic currents in the entorhinal cortex of rat brain slices. The respective low dose of test compounds significantly increased the amplitude of spontaneous excitatory postsynaptic currents while having no significant effects on the frequency. The findings suggested that Centella asiatica extract increased the response of α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptors at the postsynaptic level, revealing the potential role of Centella asiatica in modulating the glutamatergic responses in the entorhinal cortex of rat brain slices to produce cognitive enhancement effects.

    BACKGROUND: The World Health Organization recommends continuation with the failing second-line regimen if third-line option is not available. We investigated treatment outcomes among people living with HIV in Asia who continued with failing second-line regimens compared with those who had treatment modifications after failure.

    METHODS: Treatment modification was defined as a change of two antiretrovirals, a drug class change or treatment interruption (TI), all for >14 days. We assessed factors associated with CD4 changes and undetectable viral load (UVL <1,000 copies/ml) at 1 year after second-line failure using linear and logistic regression, respectively. Survival time was analysed using competing risk regression.

    RESULTS: Of the 328 patients who failed second-line ART in our cohorts, 208 (63%) had a subsequent treatment modification. Compared with those who continued the failing regimen, the average CD4 cell increase was higher in patients who had a modification without TI (difference =77.5, 95% CI 35.3, 119.7) while no difference was observed among those with TI (difference =-5.3, 95% CI -67.3, 56.8). Compared with those who continued the failing regimen, the odds of achieving UVL was lower in patients with TI (OR=0.18, 95% CI 0.06, 0.60) and similar among those who had a modification without TI (OR=1.97, 95% CI 0.95, 4.10), with proportions of UVL 60%, 22% and 75%, respectively. Survival time was not affected by treatment modifications.

    CONCLUSIONS: CD4 cell improvements were observed in those who had treatment modification without TI compared with those on the failing regimen. When no other options are available, maintaining the same failing ART combination provided better VL control than interrupting treatment.

    Neuroinflammation is associated with neuronal cell death and could lead to chronic neurodegeneration. This study investigated the neuroprotective potential of virgin coconut oil (VCO) against lipopolysaccharide (LPS)-induced cytotoxicity of neuroblastoma SK-N-SH cells.     This study was conducted to determine the effects of plyometric training on power, speed and agility on under 16 athletes. Thirty-two males from various background of sports were randomly selected as subjects. Subjects were divided into two groups which are intervention (N=16) and control (N=16). Subjects performed training for five weeks. Test batteries used includes 30-m sprint, 30 feet shuttle run and vertical jump to look at the effects of plyometric training given. Data that were obtained was analysed using T-test. Analysis showed that treatment group manage to improve performance in all the three tests while no significant differences between pre and post-test among control group for all the three tests. Based on the result, plyometric training that were given could improve power, speed and agility.

    This study is fundamental in looking to validate the agreement of Self-Assesment Instrument of Outdoor Competency (OCL-oMR) among the Co-curriculum Center Coaches in Malaysia. The instrument are newly developed by the researcher . The Inventory Responses –oMR (IR-oMR) are purposely to evaluate and determine the goodness of self-assesement instrument of outdoor competency (OCL-oMR) among co-curriculum center coaches in Malaysia. By using the correlation & percentage, the analysis were used. N=10 of head coaches of co-curriculum Center were selected to be a sampels. These data is a secondary data that researcher used in the main research. But as a secondary data, its really important to researcher to identify and justify the newly instrument of self assesment of outdoor competency (OCL-oMR). Findings shown contents validity r=.82 were recorded and the language validity were shown r=.83. Meanwhile, anothers supporting data were used percentage of agreement of Inventory Responses –Omr (IR-oMR) toward the Self-Assesment Instrument of Outdoor Competency (OCL-oMR) among the Co-curriculum Center Coaches in Malaysia. Overall, from these findings, researcher found that’s the Inventory Responses – oMR (IR-oMR) shown that the Self-Assesment Instrument of Outdoor Competency (OCL-oMR) among the Co-curriculum Center Coaches in Malaysia are valid instrument to measure the competency level of outdoor education coaches in co-curriculum center in Malaysia and the Inventory Responses – oMR (IR-oMR) are significantly toward the outdoor competency (OCL-oMR).

    Kajian ini adalah bertujuan untuk meneliti hubungan antara pengambilan kafein dan tahap keagresifan dalam kalangan atlet Kor SUKSIS. Kajian ini berbentuk kaedah tinjauan dengan menggunakan borang soal selidik berkaitan pengambilan kafein dan tahap keagresifan The Judgements About Moral Behavior in Youth Sport (JAMBYSQ). Peserta kajian terdiri daripada 80 orang atlet sukan berpasukan SUKSIS yang dipilih secara rawak. Data telah dianalisis menggunakan ujian statistik deskriptif, ujian t tidak bersandar dan korelasi Pearson. Hasil kajian mendapati bahawa terdapat perbezaan signifikan (p = 0.014) bagi tahap kekerapan pengambilan kafein antara atlet lelaki (2.00 ± 0.87) dan atlet perempuan (1.50 ± 0.76). Selain itu, bagi tahap keagresifan pula tidak terdapat perbezaan signifikan antara atlet lelaki (3.37 ± 0.60) dan atlet perempuan (3.23 ± 0.66) serta tidak terdapat hubungan yang signifikan antara pengambilan kafein dengan tahap keagresifan semasa bersukan (r = 0.024, n = 80, p =0.831). Kesimpulannya, walaupun kekerapan pengambilan kafein berbeza mengikut jantina tetapi ia tidak mempengaruhi tahap keagresifan dalam kalangan atlet Kor SUKSIS. Oleh itu, terdapat keperluan untuk mengkaji dos kafein yang sesuai yang dapat mencetus tahap keagresifan dalam kalangan atlet

    This research was undertaken to determine the physical characteristics among successful soldiers participating in the Basic Commando Course series 1/AK 2014 for 12 weeks, at Sungai Udang Camp, Malacca. A total of 37 male soldiers who had passed the commando practice test were selected to participate in this research, with special approval from the Malaysian Armed Forces Training Base. The anthropometric data of the body and fitness levels, were taken before, during and after the entire duration of the training. The successful commando candidates (commando trainees) aged 22.3±2.85 years, with a mean height of 1.71±0.03 m, mean weight of 60.76±5.18 kg, mean BMI of 22.02±1.38 kg/m2, and mean waist circumference of 68.92±2.48 cm. All physical fitness parameters showed a decrease in the level of physical fitness from the beginning to the end of the study period. In conclusion, the Malaysian commando selection training for twelve weeks produced a significant negative impact on the level of fitness of the military personnel involved. These study findings demonstrate the need for a specific recovery program after the commando’s training session, for the welfare of members and to ensure that the physical preparedness of the trainees has returned back to its pre-training maximum level.
