    The aimed of this study is to estimate the entrance surface dose (ESD) of routine chest X-ray (CXR) examination and to compare the direct and indirect measurement. This study also extended to relate with the patients body thickness and established lifetime attributable risk (LAR) of the examinations. A total of 101 CXR examinations in posteroanterior (PA) projections were selected
    as subjects and data, such as patient habitus and scanning acquisition parameters were recorded. The mean ESD value obtained from the TLD-100 and the software calculation was 0.31 mGy and 0.17 mGy, respectively. The percentage deviation obtained ranged from 25.5% to 61.3%. In comparison with the national diagnostic reference level (DRL), the mean values of ESD obtained from this study were lower by a factor of 3. Furthermore, it was observed that the mean absorbed dose of the adrenals, kidneys, lungs, oesophagus, and heart walls were lower as compared to the ESD value. The examinations were only associated with very low risk of cancer incidence and mortality. Hence, the results of this study suggested a need for standardisations of the personnel training to perform X-ray examinations according to ALARA principles.

    This research was focused on the thermoluminescence (TL) response of commercially produced single-mode telecommunication optical fibre manufactured by INOCORP (Canada). The fibres were either in the form of pure silica (SiO2) or as SiO2 doped with Ge or Al at concentrations appropriate for total internal reflection, as required for telecommunication purposes. Each of these INOCORP fibres had a core diameter of 125 ± 0.1 μm. It was noted that dopant concentration was not included among the data provided in the accompanying product data sheet. A particularly important parameter for obtaining the highest TL yield in this study was the dopant concentration of the SiO2 fibre. The dopants tended to diffuse during the production of the optical fibre. To obtain this parameter, proton induced X-ray emission (PIXE) analysis was utilised. PIXE while having limited depth resolution could unambiguously identify elements and analyse trace elements with a detection limit approaching μg g–1. For Al-doped fibres, dopant concentrations in the range of 0.98 – 2.93 mol% had been estimated, the equivalent range for Ge-doped fibres was 0.53 – 0.71 mol%. A linear dose response was observed following 2.5 MeV proton irradiation for Ge- and Al-doped fibres for up to 7 min exposure.

    A rare case of diffuse malignant peritoneal mesothelioma in a 71 year-old Malay man with no previous history of asbestos or radiation exposure is described. The clinical manifestation was a large abdominal mass. At laparotomy he was found to be in the advanced stage of the disease. The tumour was not resectable and patient was sent home. He gradually deteriorated and died three months after diagnosis was made. The magnetic resonance imaging (MRI) features of peritoneal mesothelioma which has not been previously reported are described.

    The adolescence period of life comes along with changes and challenges in terms of physical and cognitive development. In this hectic period, many adolescents may suffer more from various risk factors such as low socioeconomic status, substance abuse, sexual abuse and teenage pregnancy. Findings indicate that such disadvantaged backgrounds of Malaysian adolescent students lead to failure or underachievement in their academic performance. This narrative review scrutinises how some of these students are able to demonstrate academic resilience, which is satisfactory performance in cognitive or academic tasks in spite of their disadvantaged backgrounds. The review stresses the need for developing a caregiving relationship model for at-risk adolescent students in Malaysia. Such a model would allow educators to meet the students' needs for enhancing thinking skills, counteracting risk factors and demonstrating academic resilience.

    A novel microgels were polymerized using styrene (St), methyl methacrylate (MMA), acrylamide (AAm), and acrylic acid (AAc) monomers in the presence of N,N'-methylenebisacrylamide (MBA) cross-linker. Pre-emulsified monomer was first prepared followed by polymerizing monomers using semi-batch emulsion polymerization. Fourier Transform Infrared Spectroscopy (FTIR) and (1)H Nuclear Magnetic Resonance (NMR) were used to determine the chemical structure and to indentify the related functional group. Grafting and cross-linking of poly(acrylamide-co-acrilic acid)-grafted-poly(styrene-co-methyl methacrylate) [poly(AAm-co-AAc)-g-poly(St-co-MMA)] microgels are approved by the disappearance of band at 1300 cm(-1), 1200 cm(-1) and 1163 cm(-1) of FTIR spectrum and the appearance of CH peaks at 5.5-5.7 ppm in (1)H NMR spectrum. Scanning Electron Microscope (SEM) images indicated that poly(St-co-MMA) particle was lobed morphology coated by cross-linked poly(AAm-co-AAc) shell. Furthermore, SEM results revealed that poly(AAm-co-AAc)-g-poly(St-co-MMA) is composite particle that consist of "raspberry"-shape like structure core. Internal structures of the microgels showed homogeneous network of pores, an extensive interconnection among pores, thicker pore walls, and open network structures. Water absorbency test indicated that the sample with particle size 0.43 μm had lower equilibrium water content, % than the sample with particle size 7.39 μm.

    Influence of Nd(3+) concentration on the optical and thermoluminescence (TL) properties of melt-annealed synthesized 10 Na2 O: 20 Li2 O: (70-x) B2 O3 : xNd2 O3 , where 0.1≤ x ≤0.7 (LNB) glasses are determined. The absence of sharp peaks in X-ray diffraction patterns confirms the amorphous nature of the prepared glasses. The photoluminescence spectra under 800 nm laser excitations at room temperature exhibit three prominent peaks centred at 538, 603 and 675 nm corresponding to the transitions of (4) G7/2 → (4) I9/2 , [(4) G7/2 → (4) I11/2 , (4) G5/2 → (4) I9/2 ] and [(4) G7/2 → (4) I13/2 , (4) G5/2 → (4) I11/2 ], respectively. The TL glow curve exhibits a prominent peak (Tm ) at 180°C. The best performance of the prepared glass was found at 0.5 mol% of Nd2 O3 . We achieved a good linearity of TL response against dose between 0.5 to 4.0 Gy. The calculated value of the effective atomic number, Zeff , is 7.55 which is nearly tissue equivalent (Zeff = 7.42). These promising features demonstrate the capability of the aforementioned glass to be used as a radiation dosimeter. Copyright © 2015 John Wiley & Sons, Ltd.

    The paper presents the thermoluminescence (TL) response of strontium tetraborate glass subjected to electron irradiations at various Dy2O3 concentrations ranging from 0.00 to 1.00mol%. All glass samples exhibited single broad peak with maximum peak temperature positioned at 170-215°C. The optimum TL response was found at Dy2O3 concentration 0.75mol%. This glass showed good linearity and higher sensitivity for 7MeV compared to 6MeV electrons. Analysis of kinetic parameters showed that the glasses demonstrate second order kinetic.

    A 17 year old Malay student who is a known case of synovial sarcoma of left elbow (treated with an above elbow amputation) presented with duodenal obstruction. We report an unusual case of gastrointestinal tract metastases from synovial sarcoma. To our knowledge, there is no previous such report in the literature. The computed tomography (CT) and magnetic resonance imaging (MRI) features are described.

    Nicardipine has been shown to have an anti-atherogenic effect in rabbits given a 2% cholesterol diet. Current evidence suggests that lipid peroxidation plays an important role in atherogenesis. This study examines the effect of nicardipine on lipid peroxidation in rabbits given a 2% cholesterol diet, 8 of these rabbits given nicardipine 0.5 mg/kg twice daily intramuscularly for ten weeks while the remaining untreated 6 were controls. After ten weeks, serum malondialdehyde in the control group was significantly higher compared to their baseline levels (P < 0.05). However, there was no increase in serum malondialdehyde in the nicardipine group after 10 weeks. The area of Sudan IV positive intimal lesions (atherosclerotic plaques) were significantly decreased (P < 0.01) in the treated group compared to the control group. The aortic tissue content of cholesterol and diene conjugates were also decreased in the nicardipine group (P < 0.01). These findings suggest a possible link between nicardipine and lipid peroxidation in mediating its antiatherogenic effects.

    The spectrum of beta-thalassemia mutations in Malaysia has been determined in 45 beta-thalassemia chromosomes using dot blot hybridization of the polymerase chain reaction amplified DNA and direct DNA sequencing. Eleven different molecular defects, including those previously detected in Chinese, Asian Indians, and American blacks, and a novel frameshift mutation causing beta zero-thalassemia were detected. Since this novel mutation, a T deletion in codon 15 creates a new restriction site for EcoRII enzyme; the mutation could be detected by EcoRII digestion of the appropriate amplified fragment. The results of the present study provide additional information on the molecular heterogeneity of beta-thalassemia in this population. We also demonstrated the nonradioactive detection method of the beta-thalassemia mutation based upon the digoxigenin-labeled oligonucleotide probes.

    Radiation scattered throughout the room during fluoroscopy-guided interventional (FGI) procedures was quantified at different locations using nanoDot optically stimulated luminescence dosemeters. All the tube angulation imaging shows that the radiation spectrum resembled a single peak distribution. The left anterior oblique 90° shows the highest single peak distribution (28.65 mSv/h). The single peak distribution for standard anteroposterior, left anterior oblique 45° and right anterior oblique 45° imaging was 13.32, 22.99 and 17.40 mSv/h, respectively. All tube angulation shows that the position of the interventional radiologist experienced a higher radiation level compared to other staffs. The doses of radiation varied widely around the perimeter of the patient's table and changed in accordance to imaging angles during procedures. Knowledge pertaining to radiation exposure levels is integral in order to avoid adverse risks, particularly among staff.

    Influenza-like illnesses (ILI) were defined as a triad of cough, sore throat and fever. The aim of this study is to determine the changes in knowledge, perceptions and practices towards preventive measures of succumbing to ILI before and after hajj among Malaysian hajj pilgrims.Methodology: An open-labeled randomized control trial study performed on five hundred Malaysian hajj pilgrims in the year 2013. After validated pre-hajj questionnaires completed, 250 subjects were given health education, benzalkonium-based handrub and demonstration of the handrub usage together with pamphlets and labelled as an intervention group. Another 250 subjects from another flight batch were given the same questionnaires without health education with unmedicated hand lotion as a control group. The knowledge, perception, practices and compliances to the handrub practice in both groups were compared with the pre-hajj results. Results: No significant difference between pre and post hajj knowledge and practice among hajj pilgrims in intervention group. Pre hajj perception score [ M (IQR) = 30 (5.00), Z= -2.441, p < 0.05 ] is much higher compared to post hajj perception [ M (IQR) = 29 (6.00), Z = -2.441, p < O.O5 ]. No significant difference between pre and post hajj knowledge, perception and practices among hajj pilgrims in control group. Hand hygiene compliance was not improved with provision of health education and free handrub supply. Conclusion: No significant changes in knowledge, perceptions and practices of Malaysian hajj pilgrims pre and post-hajj in both groups. Provision of handrub did not improved hajj pilgrims compliance.

    Introduction: Osteoporosis has frequently been regards as a disease of women only. However, men are also at risk of developing osteoporosis. We aimed to evaluate the knowledge of osteoporosis among men and its associated factors.
    Materials and method: A cross-sectional study of 245 male patients aged 50 and above was conducted in the primary care clinic, University Kebangsaan Malaysia Medical Centre. The research instrument used in this study was a validated self-administered questionnaire which consists of socio-demographic data and 16 items of knowledge of osteoporosis among men.
    Results: The mean score for knowledge was 7.78± 3.32 out of 16 points. Most of the subjects (83 percent) knew the definition of osteoporosis but were not aware that osteoporosis was a silent disease. Only a quarter of subjects (38 percent) recognized family history as a risk factors for osteoporosis. Despite 78 percent of subjects answer correctly regarding importance of calcium intake in their diet however only 7 percent
    of them knew the recommended dosage of daily calcium intake. Knowledge of osteoporosis was found to be significantly associated with education and total monthly household income.
    Conclusion: Findings indicate limited knowledge of osteoporosis among the subjects. Public education of osteoporosis among men is important to increase their knowledge. Meanwhile, primary care doctors should also emphasize and educate men regarding osteoporosis to enhance their knowledge of the disease.

    The demand for new glass hosts with emergent properties is constantly growing for various miniaturized applications. Thus, some new types of Sm2O3-activated strontium-telluro-alumino-magnesium-borate glasses with copper oxide nanoparticles (CuONps) insertion were made using melt-quenching approach. The obtained glasses were characterized to determine the effects of Sm2O3 concentration changes on their thermal, structural, and luminescence properties. XRD, FTIR, and HRTEM analyses of the samples verified their glassy nature, presence of different functional units and CuONps, respectively. DTA analysis showed excellent thermal stability of these glasses with a stability factor as much as 125 °C. The optical absorption properties of the glasses were highly sensitive to Sm3+ concentrations variation. The Judd-Ofelt intensity parameters of the glasses with and without CuONps followed the trend of Ω6˃Ω4˃Ω2 and Ω4˃Ω6˃Ω2, respectively. These glasses displayed 4 visible luminescence bands at 561, 598, 645, and 705 nm corresponding to 4G5/2 → 6H5/2, 4G5/2 → 6H7/2, 4G5/2 → 6H9/2, and 4G5/2 → 6H11/2 transitions in Sm3+. In addition, branching ratio above 80% and stimulated emission cross-section up to 135.82 × 10-23 cm2 were achieved. The stimulated emission cross-section and optical gain were enhanced due to the inclusion of CuONps in the glasses. These glasses may be useful for solid state laser and optical amplifier development.

    This paper reports an intense up-conversion luminescence from Dy3+-doped strontium-telluro-alumino-magnesium-borate glasses for the first time. The samples were made via the melt-quenching method and characterized to determine the influence of various CuO nanoparticles contents change on their up-conversion emission traits. Absorption spectral data were used to calculate the Judd-Ofelt intensity parameters. The sample without CuO nanoparticles revealed two intense photoluminescence up-conversion emission peaks at 478 and 570 nm. In addition, CuO nanoparticles-activated sample displayed about 1.4-fold up-conversion emission intensity improvement due to strong light absorption in the visible to the infrared region at 799 nm excitation. The stimulated emission cross-section of the CuO nanoparticles-activated glasses was increased from 102.4 × 10-23 to 1301.1 × 10-23 cm2 (nearly 10-fold amplification) while the branching ratio was reduced to 66.9%. Thus, CuO nanoparticles as an additive in the current glass matrix enhanced the up-conversion emission and strengthened the associated nonlinear optical properties. CIE 1931 color matching revealed the influence of CuO in modifying the up-conversion color coordinates, thereby improving the white color purity. The achieved up-conversion emission coupled with the color tunability of the proposed glasses may be advantageous for the up-conversion UV tunable laser making.

    BACKGROUND: Breastfeeding is widely recognized as the optimal and natural method of feeding infants. However, there are obstacles that can limit exclusive breastfeeding practices during the first 6 months of an infant's life. This study aimed to determine the prevalence of unsuccessful exclusive breastfeeding and its associated factors among mothers who work as healthcare providers at Universiti Sains Malaysia Hospital.

    METHODS: A cross-sectional study was conducted among 295 medical doctors and staff nurses from June to December 2015. Simple random sampling was applied. The data were collected using a self-administered questionnaire and analyzed using IBM SPSS ver. 22.0 (IBM Corp., Armonk, NY, USA). Descriptive statistics and logistic regression analysis were performed.

    RESULTS: The prevalence of unsuccessful exclusive breastfeeding among the study participants was 58.3%. Mothers who preferred formula milk (odds ratio [OR], 4.40; 95% confidence interval [CI], 1.45-13.31) delivered via lower segment cesarean section (OR, 2.31; 95% CI, 1.07-4.98) and produced inadequate breast milk (OR, 4.06; 95% CI, 2.40- 6.89) were significantly associated with unsuccessful exclusive breastfeeding.

    CONCLUSION: The prevalence of unsuccessful exclusive breastfeeding among the study participants was high. Maternal characteristics such as preference towards formula milk, mode of delivery and adequacy of breast milk must be assessed to prevent unsuccessful exclusive breastfeeding among healthcare providers.

    This review aims to provide an insight into human learning processes by examining the role of cognitive and emotional unconscious processing in mentally integrating visual and verbal instructional materials. Reviewed literature shows that conscious mental integration does not happen all the time, nor does it necessarily result in optimal learning. Students of all ages and levels of experience cannot always have conscious awareness, control, and the intention to learn or promptly and continually organize perceptual, cognitive, and emotional processes of learning. This review suggests considering the role of unconscious learning processes to enhance the understanding of how students form or activate mental associations between verbal and pictorial information. The understanding would assist in presenting students with spatially-integrated verbal and pictorial instructional materials as a way of facilitating mental integration and improving teaching and learning performance.

    This paper describes a preliminary study of the thermoluminescence (TL) response of doped SiO(2) optical fibres subjected to (241)AmBe neutron irradiation. The TL materials, which comprise Al- and Ge-doped silica fibres, were exposed in close contact with the (241)AmBe source to obtain fast neutron interactions through use of measurements obtained with and without a Cd filter (the filter being made to entirely enclose the fibres). The neutron irradiations were performed for exposure times of 1-, 2-, 3-, 5- and 7-days in a neutron tank filled with water. In this study, use was also made of the Monte Carlo N-particle (MCNP) code version 5 (V5) to simulate the neutron irradiations experiment. It was found that the commercially available Ge-doped and Al-doped optical fibres show a linear dose response subjected to fast neutrons from (241)AmBe source up to seven days of irradiations. The simulation performed using MCNP5 also exhibits a similar pattern, albeit differing in sensitivity. The TL response of Ge-doped fibre is markedly greater than that of the Al-doped fibre, the total absorption cross section for Ge in both the fast and thermal neutrons region being some ten times greater than that of Al.

    Ion beams are used in radiotherapy to deliver a more precise dose to the target volume while minimizing dose to the surrounding healthy tissue. For optimum dose monitoring in ion-beam therapy, it is essential to be able to measure the delivered dose with a sensitivity, spatial resolution and dynamic range that is sufficient to meet the demands of the various therapy situations. Optical fibres have been demonstrated by this group to show promising thermoluminescence properties with respect to photon, electron and proton irradiation. In particular, and also given the flexibility and small size of optical fibre cores, for example 125.0+/-0.1 microm for the Al- and Ge-doped fibres used in this study, these fibres have the potential to fulfill the above requirements. This study investigates the thermoluminescence dosimetric characteristics of variously doped SiO(2) optical fibres irradiated with alpha particles from (241)Am. Following subtraction of the gamma contribution from the above source, the thermoluminescence characteristics of variously doped SiO(2) optical fibres have been compared with that of TLD-100 rods. The irradiations were performed in a bell jar. Of related potential significance is the effective atomic number, Z(eff) of the fibre, modifying measured dose from that deposited in tissues; in the present work, a scanning electron microscope and associated energy dispersive X-ray spectroscopy facility have been used to provide evaluation of Z(eff). For Ge-doped fibres, the effective atomic numbers value was 11.4, the equivalent value for Al-doped fibres was 12.3. This paper further presents results on dose response and the glow curves obtained. The results obtained indicate there to be good potential for use of variously doped SiO(2) optical fibres in measuring ion-beam doses in radiotherapeutic applications.
