A comparative pilot study was conducted to determine the difference in the reduction of total serum bilirubin in a group of infants who had phototherapy at home compared to an in-patient group on hospital phototherapy. Eighteen infants with unconjugated hyperbilirubinaemia who fitted the selection criteria were put under the mobile home unit (Bluelite Portable Light) placed in the home. A control group of 18 infants with the same matching characteristics had intense phototherapy in the hospital using a unit with top and bottom light sources. The infants were matched for race, starting total serum bilirubin level, birth weight (up to 250 grams) and age of baby at initiation of phototherapy (up to one-day difference). It was observed that the mean daily decrease in serum bilirubin concentration was significantly more in the home group as compared to the hospital group (t=2.95, df=17, P<0.05). The mean duration of treatment was significantly less for the home group as compared to the hospital group (t=2.84, df=17, P<0.05). None of the infants who had home phototherapy were re-hospitalized. Phototherapy related complications were mild and comparable in both groups. The result suggests that home phototherapy is safe and effective in bringing down the concentration of serum bilirubin for term babies with uncomplicated hyperbilirubinaemia.
The fracture behaviour represents the most critical issue in the automotive and aerospace engine fields. Thus, the objective of this study was to estimate and analyze the crack criteria by using the
Mathematical laws that were limited in E 1820 standard and the results affirmed by applying the numerical solutions of ANSYS to estimate the fracture toughness value KIC, besides the energy release rate of biomass composite. The specimens were prepared from different percentage of kenaf mat (KM) and unsaturated polyester resin (UP) 20% KM – 80% UP and 40% KM – 60% UP, respectively, as well the other composite properties which were calculated using the stress-strain data. The fracture characterizations of this composite were carried out using the compact tension (CT) specimen that was commonly used to determine Mode-I fracture properties. The fracture toughness has been found to be independent of pre-crack length. Meanwhile, the tests were performed at room temperature. The numerical simulations of the ANSYS model results demonstrated a good agreement between the experiments computed results of the fracture toughness. The fracture toughness KIC of 20% KM – 80% UP and 40% KM – 60% UP was equivalent to 0.76 MPa√m and 2.0 MPa√m, respectively. Thus, the fracture propagation is dependent on the fibre percentage of the composite. On the other hand, there are unlimited mechanisms of crack paths derived from randomly kenaf mat packs, particularly in the frontal process zone of crack tip.
In this study, a simple, selective and sensitive method, for spectrophotometric determination of As(III) with gallocyanine as the sensitive reagent was developed. The wavelength of an analytical measurement, for the determination of As (III), using gallocyanine was at 630 nm with an optimum response at pH 2. The RSD for the reproducibility of 100 ppm As (III) was 2.3%. The LOD was 0.04 ppm with linear dynamic range in As(III) concentration of 0.2 - 1.5 ppm. The developed method has been validated against Atomic Absorption Spectrophotometry (AAS). The interference study of several metal ions was carried out and it revealed that that Mn (II) ion was interfered the most.
BACKGROUND: The aim of this study was to examine Malaysian contributions in the field of biomedical and health sciences.
METHODS: In this study, 3697 publications affiliated to Malaysian addresses from the sci database between 1990 and 2005 were sampled. This study also explored publication productivity trends, authorship and collaboration pattern, core journals used, and citations obtained.
RESULTS: Main contributions were journal articles (73.3%). Most authors (63.7%) contributed only one article and 16.1% produced over 30-68 publications. Multi-authored works were the norm. The productive authors were named either first or second in publications. There were active collaborations with authors from Asia-Pacific countries (35%) and Europe (30%). The majority of publications were contributed by institutions of higher learning (87%). Core journals used follow quite close to Bradford's zonal ratios of 44:152:581. The active research areas were identified. About 71.3% of publications received citations especially those published from 1995 to 1999.
CONCLUSION: This study helped librarians identify active researchers, active research areas and journals relevant to biomedical and health sciences researchers and useful when producing reports to university management and planning medical collection policies and deciding on journal subscriptions and cancellations.
The management of depression in the primary care setting should ideally take a biological, psychological, and sociological approach. Antidepressants are the most commonly used biological agents in the treatment of depression. Psychological therapies and psychosocial interventions improve the outcome of treatment when combined with pharmacotherapy. Clinical depression is treatable and thus efforts should be made to alleviate the suffering of patients with depression.
We analysed 580 articles (original articles only) published in Medical Journal of Malaysia between 2004 and 2008, the resources referenced by the articles and the citations and impact received. Our aim was to examine article and author productivity, the age of references used and impact of the journal. Publication data was obtained from MyAIS database and Google Scholar provided the citation data. From the 580 articles analyzed, contributors mainly come from the hospitals, universities and clinics. Contributions from foreign authors are low. The useful lives of references cited were between 3 to 11 years. ISI derived Impact factor for MJM ranged between 0.378 to 0.616. Journal self-citation is low. Out of the 580 sampled articles, 76.8% have been cited at least once over the 5 years and the ratio of total publications to citations is 1: 2.6.
The cost of polyhydroxyalkanoates (PHAs) can be reduced by improving their productivity and recovery. In this study, we attempted to obtain a high cell density culture from a 13 L bioreactor and subsequently improved the recently developed biological recovery process using mealworms to obtain the PHA granules. A cell dry weight of 161 g/L containing 68-70 wt% P(3HB) was obtained. The freeze-dried cells contained a significant amount of mineral salts from the culture medium which reduced the cells' palatability for the mealworms. A simple washing procedure with water was sufficient to remove the residual mineral salts and this improved the cells' consumption by up to 12.5% of the mealworms' body weight. As a result, one kilogram of mealworms consumed 125 g of the washed cells daily and 87.2 g of feacal pellets were recovered, which was almost twice the weight of the unwashed cells. In addition, it also improved the purity of the PHA in the faecal pellets to a value <90% upon washing with water to remove the water-soluble compounds. This study has demonstrated a significant improvement in the production and recovery of PHA. In addition, the resulting mealworms showed a significant increase in protein content up to 79% and a decrease in fat content down to 8.3% of its dry weight.
One of the commonest psychological problems that a clinician would encounter in primary care is depression. The prevalence of depression is high in women, the elderly and those with underlying physical problems or during the postpartum period. The spectrum of clinical presentations is wide and somatic complaints are more common in primary care clinics. Depression may present as a primary disorder and co-morbidity with other psychological problems or physical illnesses is high. A good clinical interview is an important form of assessment and a quick screening of depression can be done with the administration of proper rating scales, such as the Patient Health Questionnaire, Hamilton Depression Rating Scale or Geriatric Depression Scale. Repeated use of the same scale in a patient would help the clinician to monitor the progress objectively.
Objectives: The objectives of this study were to ascertain the prevalence of Indonesians obstetrics immigrant and to assess the fetal maternal outcome. Methodology: A prospective cohort study design was used to analyse 54 consecutive Indonesians obstetrics immigrant compared to 56 Malay women. Chi square and student t test were used where appropriate, p < 0.05 was considered to be of statistical significance. Results: There was a reducing trend in the incidence admission of Indonesians from 10.5 in 1999 to 6.5 % in 2002. The maternal mortality ratio showed an increasing trend from 1999 (40.0/100000) to 2001 (162.9/100000) but decreased to 5.8/100000 in 2002. The majority was between 20-40 years old, multiparous and booked, which was similar to the Malay population. Most Malays were in occupational class 1 to 3 and the husband has secondary and tertiary education compared to the Indonesians who were in class 4 and 5 and the husband has primary and no formal education. Significantly more immigrants were housewives compared to Malays who were working women. There was no significant difference in the antenatal complications. There were no significant difference in terms of delivery and most delivered vaginally. The perinatal outcome in terms of gestation, birth weight, Apgar score and admission to neonatal ICU were similar in both populations and there was no perinatal mortality. In conclusion, the incidence admission of Indonesian immigrant was on the decreasing trend. The outcome of these patients managed in MHKL was similar to the Malay population. Further studies with enrollment of a larger number of patients should be carried out to ascertain the significance of these findings.
This study was conducted for the development of the green protection garments. For this purpose, laminate composite material was developed from Kevlar 29-ramie-unsaturated polyester resin. The aim of this study was to develop a solid body armour that meets the specific requirements of ballistic resistance. This composite is subjected to high impact loading. The target was shot using gas gun machine that is supported by camera hardware to capture the projectile speed. In order to achieve the goal of the research, several experiments were conducted with the aim to estimate the ballistic limit, maximum energy absorption, composite failure mode, life time rupture, target geometry, and environmental effect. The results of these experiments indicated that the maximum ballistic limit validated at impact speed is in the range of 250 m/s to 656.8 m/s for the second protection level. The targets are improved in term of the impact response with the increase in the relative humidity, i.e. the range of 50% ± 20%, whereas, reduction of resistance results in the increase of temperature. The range of temperatures was between 20oC and 70oC. A limited delamination was generated under multiple shots. Targets geometry plays a major role in increasing the impact response. Hence, the results present a high resistant impact for pairs from the panels with total thickness arrived to 15
mm ± 3 mm. This body armour is one of the most economical armour products, in which common materials are used in its production, particularly to reduce the amount of Kevlar, and this could further lead to a decrease in its production cost. On the other hand, this armour meets the ballistic threats under 623 m/s of 15 mm ± 3 mm target thickness and 837.5 m/s of 25 mm ± 2.mm. Thus, the armour is equivalent to the third level of protective ballistic limits in the National Institute of Justice (NIJ) standards.
Introduction: Assessment is an integral aspect of
teaching. One-best-answer (OBA) items, if properly
constructed are able to drive learning. In-house OBA
items are notoriously poorly-constructed. The role of
a central vetting committee is to review test items and
ensure that they adhere to expected standards. Hence,
the objective of this audit is to determine whether
central vetting has improved the construct quality of
OBA items.
Methods: We audited the psychiatry end-of posting
OBA items from before and after central vetting to
compare the quality of the items before and after
central vetting was instituted. Quality was evaluated
on appropriateness of test content, items with higher
cognition and items without flaws. A standard was not
set for this first audit.
Results: Seventy six of 181 psychiatry OBAs items
retrieved from 2011 to August 2012 had undergone
first level (department) vetting only and the remainder
105 (58.0%) had two levels of vetting; department and
central vetting committee (CVC).
Appropriateness of content increased from 92.1% to
98.1%. Items with higher order thinking doubled from
21.1% to 42.9%. Items with clinical scenario increased
by 8.4% to 78.1%. Logical ordering of options however,
remained around 50%.
Two-level vetting markedly reduced problematic
lead-in questions (67.1 to 13.3%), non-homogenous
options (42.1 to 9.5%), vague and implausible options
(39.5 to 6.7%), and spelling and grammar mistakes
(19.7 to 5.7%).
Conclusion: Two-level vetting had improved the
quality of OBAs and should be continued. This could
be enhanced by training all Faculty on writing quality
OBA items and careful selection and empowerment
of CVC members. A re-audit is to be conducted after
Faculty training.
This study aimed to investigate the impact of nonionic surfactants on the efficacy of fluorine degradation by Polyporus sp. S133 in a liquid culture. Fluorene was observed to be degraded in its entirety by Polyporus sp. S133 subsequent to a 23-day incubation period. The fastest cell growth rate was observed in the initial 7 days in the culture that was supplemented with Tween 80. The degradation process was primarily modulated by the activity of two ligninolytic enzymes, laccase and MnP. The highest laccase activity was stimulated by the addition of Tween 80 (2443U/L) followed by mixed surfactant (1766U/L) and Brij 35 (1655U/L). UV-vis spectroscopy, TLC analysis and mass spectrum analysis of samples subsequent to the degradation process in the culture medium confirmed the biotransformation of fluorene. Two metabolites, 9-fluorenol (λmax 270, tR 8.0min and m/z 254) and protocatechuic acid (λmax 260, tR 11.3min and m/z 370), were identified in the treated medium.
Latest progress in the liquid crystal (LC) field related to azo molecules incorporated into natural product- based moieties for the improvement of LC texture and mesomeric phases has received great interest among researchers. A LC containing natural product-based moieties i.e. menthol, kojic acid, cholesterol and chalcone with stable azo and azobenzene scaffolds with specific optical tunability, has been widely used in photo-active materials such as Liquid Crystal Display (LCD), LC films, smart windows and other devices. This review discusses the influence of azobenzene, a renowned photo-responsive and stable LC scaffold, in mesogenic phases due to photo-isomerization and optical switching. The incorporation of mesomeric phases of natural product moieties to azo molecules has improved the properties of LC, i.e, from the nematic phase to the smectic phase with proper magnetic field alignment. Natural product-based LC can be useful in numerous applications, especially practical electronic or optic devices such as optical image storage, display devices, solar cells, optical switching.
Chemical modification of medicines from natural product-based molecules has become of interest in recent years. In this study, a series of halogenated azo derivatives 1a-d were synthesised via coupling reaction, followed by Steglich esterification with aspirin (a natural product derivative) to form azo derivatives 2a-d. While, halogenated azo-aspirin 3a-d were synthesised via direct coupling reaction of aspirin and diazonium salt. Bacteriostatic activity was demonstrated against E. coli and S. aureus via turbidimetric kinetic method. Compound 3a-d showed excellent antibacterial activities against E. coli (MIC 75-94 ppm) and S. aureus (MIC 64-89 ppm) compared to ampicillin (MIC 93 and 124 ppm respectively), followed by 1a-d and 2a-d. The presence of reactive groups of -OH, N=N, C=O and halogens significantly contribute excellent interaction towards E. coli and S. aureus. Molecular dockings analysis of 3a against MIaC protein showed binding free energy of -7.2 kcal/mol (E. coli) and -6.6 kcal/mol (S. aureus).
Epilepsy, a chronic disorder of brain characterised by a predisposition to generate epileptic seizures, has an effect on the psychosocial well-being of sufferers. Measuring the quality of life (QOL) of people with epilepsy (PWE) is increasingly recognized as an important component of clinical management. QOL measures differ between countries and there is limited information regarding PWE in Malaysia. The aim of this study was to determine the health related QOL and its relationship with the presence of seizures in PWE at a Malaysian tertiary referral center.
Increased disposal of heavy metals, including lead (II) (Pb(II)) into the environment calls for a reliable and sustainable solution. In this study, nano-sized biochar from sago activated sludge was proposed for the removal of Pb(II). Sago activated sludge was pyrolysed in a tube furnace followed by a chemical activation to yield nano-sized particles ranging from 45 to 75 nm. The nano-sized biochar obtained was characterised and the influence of pH (2 – 10), initial Pb(II) concentration (1 – 5 mg/L), contact time (30 – 90 mins) and adsorbent dosage (0.1 – 0.5 g) was investigated in a batch adsorption study. Response surface methodology (RSM) approach with central composite design (CCD) was used as statistical tools to optimize the adsorption process by relating the mutual interactions among all studied variables. Characterisation of the prepared adsorbent showed that large surface area was observed on sludge activated carbon (78.863 m2 /g) compared with sludge biochar (8.044 m2 /g) and sludge biomass (1.303 m2 /g). The batch adsorption best fitted the Langmuir isotherm (maximum adsorption capacity, Q0 = 3.202 × 10-3 mg/g, R-squared value = 0.9308). The RSM indicated that the optimum Pb(II) removal (99.87%) was at 0.5 g of adsorbent, 5 mg/L initial concentration and 30 min contact time. This study is significant because utilisation of sago effluent will reduce sago manufacturing waste by conversion into a value-added product as adsorbent to adsorb Pb(II) in wastewater.
This study aims to determine the prevalence of denture wearing among elderly and to compare the oral health related quality of life (OHRQoL) between elderly with dentures and those without dentures. This cross sectional study involved 506 randomly selected edentulous elderly in the district of Badang, Kota Bharu, Kelantan. Consented participants were interviewed in the Kelantanese dialect at their homes by a single trained interviewer using the short version Oral Health Impact Profile [S-OHIP(M)] which had been translated into the Malay language, tested and validated for use in Malaysian population. Denture wearing and self rated denture status was noted. The prevalence of denture wearing was 46.2% (95% CI=41.83, 50.70). There was a significant difference in sex, smoking status, self perceived treatment need and self perceived satisfaction between denture wearers and non denture wearers. During a 24-month period, 21 children with acute bacterial meningitis were identified and studied. The majority of the children was from low socio-economic group and the male:female sex ratio was equal. Seventeen children (81%) were aged twelve months or below. In 15 (71.5%) of the children. Haemophilus influenzae type b was recovered, while Streptococcus pneumoniae was isolated from 4 children. Neisseria spp and Salmonella spp were identified respectively in each of the other two cases. The case fatality was four (19.0%) with nine others (42%) exhibiting neurological sequelae. Except for the Salmonella spp strain that was resistant to the cephalosporin, the rest of the bacterial species were sensitive to the commonly used antibiotics. As Haemophilus influenzae type b is still the most prevalent cause of acute bacterial meningitis, it is therefore strongly recommended that the national immunisation programme in this country should include the vaccine for it in our effort to minimise the mortality and morbidity caused by this organism.
This paper presents the evaluation of integrated partial match query in Geographic Information Retrieval (GIR). To facilitate the evaluation, Kuala Lumpur tourist related data is used as test collection and is stored in SuperWeb, a map server. Then the map server is customized to enhance its query capability to recognize word in partial or case sensitive between layers of spatial data. Query keyword is tested using the system and results are evaluated using experiments on sample data. Findings show that integrated partial match query provides more flexibility to tourist in determining search results.
Myasthenia gravis (MG) is a rare autoimmune disorder
characterised by fluctuating and variable combination
of muscle weakness and fatigue. Most cases are due to
T-cell mediated autoantibodies against post-synaptic
acetylcholine receptors (AChR-Ab), thus preventing
acetylcholine from binding and signalling skeletal
muscle to contract.1
The annual incidence is 7-23 new cases per million.1
It can occur at any age but with two peaks; an earlyonset
(20-40 years) female-predominant and a late-onset
(60-80 years) male-predominant peak. MG is classified
into ocular and generalised (80%). More than half the
patients initially present with ptosis and diplopia but half
will progress to generalised disease with involvement of
bulbar, limb and respiratory weakness. Those presenting
as generalised MG can also develop eye signs later.1
It is important to recognise MG early because it is
highly treatable. Untreated disease leads to permanent
weakness.2 Treatment reduces mortality from lifethreatening
myasthenic crisis.1,3 Misdiagnosis leads to
potentially harmful interventions and inappropriate
management.4,5 Diagnosis in late-onset MG is easily
missed2,3,4,5 because of overlapping symptoms with
other diseases common in the elderly. We report a case
of delay and misdiagnosis in an elderly patient with
