    White matter (WM) lesions are diffuse WM abnormalities that appear as hyperintense (bright) regions in cranial magnetic resonance imaging (MRI). WM lesions are often observed in older populations and are important indicators of stroke, multiple sclerosis, dementia and other brain-related disorders. In this paper, a new automated method for WM lesions segmentation is presented. In the proposed method, the presence of WM lesions is detected as outliers in the intensity distribution of the fluid-attenuated inversion recovery (FLAIR) MR images using an adaptive outlier detection approach. Outliers are detected using a novel adaptive trimmed mean algorithm and box-whisker plot. In addition, pre- and postprocessing steps are implemented to reduce false positives attributed to MRI artifacts commonly observed in FLAIR sequences. The approach is validated using the cranial MRI sequences of 38 subjects. A significant correlation (R=0.9641, P value=3.12×10(-3)) is observed between the automated approach and manual segmentation by radiologist. The accuracy of the proposed approach was further validated by comparing the lesion volumes computed using the automated approach and lesions manually segmented by an expert radiologist. Finally, the proposed approach is compared against leading lesion segmentation algorithms using a benchmark dataset.

    BACKGROUND: Electrical signals are generated inside human brain due to any mental or physical task. This causes activation of several sources inside brain which are localized using various optimization algorithms.

    METHODS: Such activity is recorded through various neuroimaging techniques like fMRI, EEG, MEG etc. EEG signals based localization is termed as EEG source localization. The source localization problem is defined by two complementary problems; the forward problem and the inverse problem. The forward problem involves the modeling how the electromagnetic sources cause measurement in sensor space, while the inverse problem refers to the estimation of the sources (causes) from observed data (consequences). Usually, this inverse problem is ill-posed. In other words, there are many solutions to the inverse problem that explains the same data. This ill-posed problem can be finessed by using prior information within a Bayesian framework. This research work discusses source reconstruction for EEG data using a Bayesian framework. In particular, MSP, LORETA and MNE are compared.

    RESULTS: The results are compared in terms of variational free energy approximation to model evidence and in terms of variance accounted for in the sensor space. The results are taken for real time EEG data and synthetically generated EEG data at an SNR level of 10dB.

    CONCLUSION: In brief, it was seen that MSP has the highest evidence and lowest localization error when compared to classical models. Furthermore, the plausibility and consistency of the source reconstruction speaks to the ability of MSP technique to localize active brain sources.

    Brain herniation is generally thought to be unlikely to occur in newborns due to the presence of the patent fontanelles and cranial sutures. A review of the literature published from 1993 to 2008 via MEDLINE search revealed no reports on neonatal brain herniation from intracranial tumour. We report a preterm Malay male infant born via elective Caesarean section for antenatally diagnosed intracerebral tumour, which subsequently developed herniation. Cerebral magnetic resonance imaging showed features that were compatible with a large complex intracranial tumour causing mass effect and gross hydrocephalus. Tumour excision was scheduled when the infant was two weeks old. Unfortunately, on the morning of the surgery, he developed signs of brain herniation and had profuse tumour haemorrhage during the attempted excision. Histopathological examination revealed an embryonal tumour, possibly an atypical rhabdoid/teratoid tumour. This case illustrates that intracranial tumours in newborns can herniate and should therefore be closely monitored.

    Different approaches to gray and white matter measurements in magnetic resonance imaging (MRI) have been studied. For clinical use, the estimated values must be reliable and accurate when, unfortunately, many techniques fail on these criteria in an unrestricted clinical environment. A recent method for tissue clusterization in MRI analysis has the advantage of great simplicity, and it takes the account of partial volume effects. In this study, we will evaluate the intensity of MR sequences known as T1-weighted images in an axial sliced section. Intensity group clustering algorithms are proposed to achieve further diagnosis for brain MRI, which has been hardly studied. Subjective study has been suggested to evaluate the clustering group intensity in order to obtain the best diagnosis as well as better detection for the suspected cases. This technique makes use of image tissue biases of intensity value pixels to provide 2 regions of interest as techniques. Moreover, the original mathematic solution could still be used with a specific set of modern sequences. There are many advantages to generalize the solution, which give far more scope for application and greater accuracy.

    A computer-aided detection auto-probing (CADAP) system is presented for detecting breast lesions using dynamic contrast enhanced magnetic resonance imaging, through a spatial-based discrete Fourier transform. The stand-alone CADAP system reduces noise, refines region of interest (ROI) automatically, and detects the breast lesion with minimal false positive detection. The lesions are then classified and colourised according to their characteristics, whether benign, suspicious or malignant. To enhance the visualisation, the entire analysed ROI is constructed into a 3-D image, so that the user can diagnose based on multiple views on the ROI. The proposed method has been applied to 101 sets of digital images, and the results compared with the biopsy results done by radiologists. The proposed scheme is able to identify breast cancer regions accurately and efficiently.

    The aim of this article is to provide a perspective on the relative importance and contribution of different imaging modalities in patients with valvular heart disease. Valvular heart disease is increasing in prevalence across Europe, at a time when the clinical ability of physicians to diagnose and assess severity is declining. Increasing reliance is placed on echocardiography, which is the mainstay of cardiac imaging in valvular heart disease. This article outlines the techniques used in this context and their limitations, identifying areas in which dynamic imaging with cardiovascular magnetic resonance and multislice CT are expanding.

    It is known from our previous studies that kiwifruits, which are used in common human diet, have preventive properties of coronary artery disease. This study describes a combination of (1)H NMR spectroscopy, multivariate data analyses and fluorescence measurements in differentiating of some kiwifruit varieties, their quenching and antioxidant properties. A total of 41 metabolites were identified by comparing with literature data Chenomx database and 2D NMR. The binding properties of the extracted polyphenols against HSA showed higher reactivity of studied two cultivars in comparison with the common Hayward. The results showed that the fluorescence of HSA was quenched by Bidan as much as twice than by other fruits. The correlation between the binding properties of polyphenols in the investigated fruits, their relative quantification and suggested metabolic pathway was established. These results can provide possible application of fruit extracts in pharmaceutical industry.

    Digital breast tomosynthesis (DBT) is a fairly recent breast imaging technique invented to overcome the challenges of overlapping breast tissue. Ultrasonography (USG) was used as a complementary tool to DBT for the purpose of this study. Nonetheless, breast magnetic resonance imaging (MRI) remains the most sensitive tool to detect breast lesion. The purpose of this study was to evaluate diagnostic performance of DBT, with and without USG, versus breast MRI in correlation to histopathological examination (HPE). This was a retrospective study in a university hospital over a duration of 24 months. Findings were acquired from a formal report and were correlated with HPE. The sensitivity of DBT with or without USG was lower than MRI. However, the accuracy, specificity and PPV were raised with the aid of USG to equivalent or better than MRI. These three modalities showed statistically significant in correlation with HPE (p < 0.005, chi-squared). Generally, DBT alone has lower sensitivity as compared to MRI. However, it is reassuring that DBT + USG could significantly improve diagnostic performance to that comparable to MRI. In conclusion, results of this study are vital to centers which do not have MRI, as complementary ultrasound can accentuate diagnostic performance of DBT.

    INTRODUCTION: We aimed to compare the degree of bowel distension and image quality between pineapple juice and different mannitol concentrations, as well as patients' acceptance and side effects of these different magnetic resonant enterography (MRE) oral contrast agents.

    MATERIALS AND METHODS: Seventy-five participants underwent MRE as an initial investigation or follow-up for inflammatory bowel disease. A systematic sampling method was used to divide the participants into three different groups: group 1 received 6.7% mannitol concentration, group 2 received 3.3% mannitol concentration and group 3 received pineapple juice as an oral contrast agent during their MRE examination. The degree of bowel distension on MRE images was assessed by a radiologist by measuring the bowel diameter from inner wall to inner wall at specified levels, while qualitative analysis was evaluated based on the presence of artefacts. All patients were asked to score their acceptance of the oral contrast and were asked about side effects such as diarrhoea, abdominal discomfort and vomiting.

    RESULTS: All patients were able to completely ingest 1.5L of oral contrast. The mean diameter of bowel distension was 2.1cm in patients who received 6.7% mannitol concentration, 2.0cm in patients who received 3.3% mannitol concentration and 1.6 cm in patients who received pineapple juice. Twothirds of patients who received 6.7% mannitol and 3.3% mannitol solutions had good-quality MRE images, but 68% of patients who received pineapple juice had poor-quality MRE images. Twenty-four patients (96%) who received pineapple juice rated it as slightly acceptable and acceptable but only 12 patients (48%) who received 6.7% mannitol solution rated it as slightly acceptable and acceptable. Eighty-eight percent of patients who received 6.7% mannitol solution experienced at least one form of side effect as compared to 44% of patients who received 3.3% mannitol solution and 18% of patients who received pineapple juice.

    CONCLUSION: Optimum small bowel distension and good image quality can be achieved using 3.3% mannitol concentration as an oral contrast agent. Increase in mannitol concentration does not result in significant improvement of small bowel distension or image quality but is instead related to poorer patient acceptance and increased side effects. Pineapple juice is more palatable than mannitol and produces satisfactory small bowel distension. However, the small bowel distension is less uniform when using pineapple juice with a considerable presence of artefacts. Mannitol, 3.3% concentration, is therefore recommended as an endoluminal contrast agent for bowel in MRE.

    INTRODUCTION: Motion and pulsation artifacts are the most prominent types of artifacts in Magnetic Resonance Imaging (MRI) of the shoulder. Therefore, this study examined the Periodically Rotating Overlapping Parallel Lines with Enhanced Reconstruction (PROPELLER) technique with small flex coil (SFC) and dedicated shoulder coil (DSC) for the reduction of motion and pulsation artifacts. The signalto- noise ratio (SNR) and contrast-to-noise ratio (CNR) of the standard proton density fat saturation (PDFS) pulse sequence and the PROPELLER proton density fat saturation (PROPELLER PDFS) pulse sequence were also evaluated.

    MATERIALS AND METHODS: Eighteen (18) participants who met the inclusion and exclusion criteria were scanned using a standard non-contrast MRI shoulder protocol including the PDFS pulse sequence and the PROPELLER PDFS pulse sequence using a small flex coil and a dedicated shoulder coil. Two experienced musculoskeletal (MSK) radiologists evaluated and graded the presence of artifacts on the MR images and the SNR and CNR were measured quantitatively.

    RESULTS: The non-parametric Wilcoxon Signed Rank test revealed a significant reduction in motion and pulsation artifacts between the PROPELLER PDFS pulse sequence and the standard PDFS pulse sequence. In addition, the nonparametric Mann-Whitney U test revealed that the mean rank of SNR for the standard sequence was statistically significant when compared to the PROPELLER sequence for both coil types. The CNR of the PROPELLER sequence was statistically significant between fat-fluid, bone-fluid, bonetendon, bone-muscle, and muscle-fluid when using SFC and DSC.

    CONCLUSION: This study proved that the PROPELLER-PDFS pulse sequence effectively eliminates motion and pulsation artifacts, regardless of the coils utilised. The PROPELLERPDFS pulse sequence can therefore be implemented into the standard MRI shoulder procedure.

    In this paper, an automatic computer-aided detection system for breast magnetic resonance imaging (MRI) tumour segmentation will be presented. The study is focused on tumour segmentation using the modified automatic seeded region growing algorithm with a variation of the automated initial seed and threshold selection methodologies. Prior to that, some pre-processing methodologies are involved. Breast skin is detected and deleted using the integration of two algorithms, namely the level set active contour and morphological thinning. The system is applied and tested on 40 test images from the RIDER breast MRI dataset, the results are evaluated and presented in comparison to the ground truths of the dataset. The analysis of variance (ANOVA) test shows that there is a statistically significance in the performance compared to the previous segmentation approaches that have been tested on the same dataset where ANOVA p values for the evaluation measures' results are less than 0.05, such as: relative overlap (p = 0.0002), misclassification rate (p = 0.045), true negative fraction (p = 0.0001) and sum of true volume fraction (p = 0.0001).

    Medical Image Quality Assessment (IQA) plays an important role in assisting and evaluating the development of any new hardware, imaging sequences, pre-processing or post-processing algorithms. We have performed a quantitative analysis of the correlation between subjective and objective Full Reference - IQA (FR-IQA) on Magnetic Resonance (MR) images of the human brain, spine, knee and abdomen. We have created a MR image database that consists of 25 original reference images and 750 distorted images. The reference images were distorted with six types of distortions: Rician Noise, Gaussian White Noise, Gaussian Blur, DCT compression, JPEG compression and JPEG2000 compression, at various levels of distortion. Twenty eight subjects were chosen to evaluate the images resulting in a total of 21,700 human evaluations. The raw scores were then converted to Difference Mean Opinion Score (DMOS). Thirteen objective FR-IQA metrics were used to determine the validity of the subjective DMOS. The results indicate a high correlation between the subjective and objective assessment of the MR images. The Noise Quality Measurement (NQM) has the highest correlation with DMOS, where the mean Pearson Linear Correlation Coefficient (PLCC) and Spearman Rank Order Correlation Coefficient (SROCC) are 0.936 and 0.938 respectively. The Universal Quality Index (UQI) has the lowest correlation with DMOS, where the mean PLCC and SROCC are 0.807 and 0.815 respectively. Student's T-test was used to find the difference in performance of FR-IQA across different types of distortion. The superior IQAs tested statistically are UQI for Rician noise images, Visual Information Fidelity (VIF) for Gaussian blur images, NQM for both DCT and JPEG compressed images, Peak Signal-to-Noise Ratio (PSNR) for JPEG2000 compressed images.

    The purpose of this study was to develop realistic phantom models of the intracellular environment of metastatic breast tumour and naïve brain, and using these models determine an analysis metric for quantification of CEST MRI data that is sensitive to only labile proton exchange rate and concentration. The ability of the optimal metric to quantify pH differences in the phantoms was also evaluated. Novel phantom models were produced, by adding perchloric acid extracts of either metastatic mouse breast carcinoma cells or healthy mouse brain to bovine serum albumin. The phantom model was validated using 1 H NMR spectroscopy, then utilized to determine the sensitivity of CEST MRI to changes in pH, labile proton concentration, T1 time and T2 time; six different CEST MRI analysis metrics (MTRasym , APT*, MTRRex , AREX and CESTR* with and without T1 /T2 compensation) were compared. The new phantom models were highly representative of the in vivo intracellular environment of both tumour and brain tissue. Of the analysis methods compared, CESTR* with T1 and T2 time compensation was optimally specific to changes in the CEST effect (i.e. minimal contamination from T1 or T2 variation). In phantoms with identical protein concentrations, pH differences between phantoms could be quantified with a mean accuracy of 0.6 pH units. We propose that CESTR* with T1 and T2 time compensation is the optimal analysis method for these phantoms. Analysis of CEST MRI data with T1 /T2 time compensated CESTR* is reproducible between phantoms, and its application in vivo may resolve the intracellular alkalosis associated with breast cancer brain metastases without the need for exogenous contrast agents.

    Endoscopic endonasal techniques have recently become the method of choice in dealing with cerebrospinal fluid leak involving the anterior cranial fossa. However, most surgeons prefer an intracranial approach when leaks involve the middle cranial fossa. This case report illustrates the possibilities of using endoscopic techniques for cerebrospinal fluid leaks involving the middle fossa.

    Ethylmalonic encephalopathy is a rare metabolic disease presenting in infancy with developmental delay, acrocyanosis, petechiae, chronic diarrhea and early death. The biochemical characteristics of this autosomal recessive disease are urinary organic acid abnormalities. Recently it has been found to be caused by mutations in the ETHE1 gene, located on Ch19q13. Only about 30 patients have been reported, and we describe two additional cases. The first patient showed a typical clinical picture and biochemical abnormalities, with additional atypical clinical features. Neuroimaging studies showed extensive changes. A new homozygous mutation in exon 3 of the ETHE1 gene was found. The second patient was not investigated genetically; however besides the typical clinical picture and biochemical profile he was found to have cytochrome C oxidase deficiency.
