    The objective of this prospective study was to report on the prevalence of hearing impairment in the neonatal unit population. From 15 February 2000 to 15 March 2000 and from 15 February 2001 to 15 May 2001, 401 neonates were screened using transient evoked otoacoustic emissions (TEOAE) followed by second-stage screening of those infants who failed the initial test. Eight (2 per cent) infants failed one ear and 23 (5.74 per cent) infants failed both ears, adding up to 7.74 per cent planned for second-stage screening. Five out of 22 infants who came for the follow up failed the screening, resulting in a prevalence of hearing impairment of 1 per cent (95 per cent confidence interval [95% CI]: 0.0-2.0). Craniofacial malformations, very low birth weight, ototoxic medication, stigmata/syndromes associated with hearing loss and hyperbilirubinaemia at the level of exchange tranfusion were identified to be independent significant risk factors for hearing impairment, while poor Apgar scores and mechanical ventilation of more than five days were not. In conclusion, hearing screening in high-risk neonates revealed a total of 1 per cent with hearing loss. The changes in the risk profile indicate improved perinatal handling in a neonatal population at risk for hearing disorders.

    Myxoid neurofibroma is benign tumours of perineural cell origin that arise from elements in the peripheral nervous system. We report a case of a 60-year-old female patient presented with history of right ear mass which was slowly growing. Her primary complaint was cosmetic deformity but hearing loss was also present. The mass was excised and histologically revealed a myxoid neurofibroma. To the authors' knowledge, this is the first time that a myxoid neurofibroma arising from external auditory canal is reported. It should be included in the differential diagnosis of a mass originating from this location.

    Medicinal herb preparations prescribed for specific treatment purposes were purchased from markets and were analyzed by instrumental neutron activation analysis with k0 standardization. Then, 500-700 mg of each sample was pelletized under a pressure of six tones and irradiated together with monitors for alpha and neutron flux ratio determinations for about 6 h in a thermal flux of 2.29 x 10(12) n/cm2/s. The accuracy of the method was established by analyzing standard reference materials. Twenty-nine elements, Ag, As, Au, Ba, Br, Ca, Ce, Co, Cr, Cs, Eu, Fe, Hf, K, La, Mn, Mo, Na, Rb, Sb, Sc, Se, Sm, Sr, Th, U, Yb, and Zn, were measured in all the samples, and Hg was detected in some samples, with good accuracy and reproducibility. The concentration of elements determined was found to vary depending on the composition of the herbs used. Although the trend linking the element of the medicinal plants to its curative abilities could not be clearly determined, this study showed that the toxic elements found in the samples were below the levels prescribed by health regulations. Nevertheless, such data are important to understand the pharmacological action and the exact mechanisms of action and formation of active constituents for each medicinal plant and to decide the dosage of the herbs used in the final formulation.

    Gastric cancer in the young adult is rare and has been said to be more aggressive than gastric cancers of the older age group. Its unique association with pregnancy is even rarer. However, they have similar complications of haemorrhage, obstruction and perforation. We report a 27 year old lady at 16 weeks gestation who presented with a perforated malignant gastric ulcer and carcinomatosis peritonei. Reviewing the literature, we realised that such complication of a gastric cancer occurring in a pregnant young adult has not been previously documented.

    Fifty black tiger shrimp Penaeus monodon from commercial cultivation ponds in Malaysia were examined by Tdt-mediated dUTP nick-end labeling (TUNEL) fluorescence assay and agarose gel electrophoresis of DNA extracts for evidence of DNA fragmentation as an indicator of apoptosis. From these specimens, 30 were grossly normal and 20 showed gross signs of white spot syndrome virus (WSSV) infection. Of the 30 grossly normal shrimp, 5 specimens were found to be positive for WSSV infection by normal histology and by nested polymerase chain reaction (PCR) analysis. All of the specimens showing gross signs of WSSV infection were positive for WSSV by normal histology, while 5 were positive by nested PCR only (indicating light infections) and 15 were positive by 1-step PCR (indicating heavy infections). Typical histological signs of WSSV infection included nuclear hypertrophy, chromatin condensation and margination. None of the 25 grossly normal shrimp negative for WSSV by 1-step PCR showed any signs of DNA fragmentation by TUNEL assay or agarose gel electrophoresis of DNA extracts. The 10 specimens that gave PCR-positive results for WSSV by nested PCR only (i.e., 5 grossly normal shrimp and 5 grossly positive for WSSV) gave mean counts of 16 +/- 8% TUNEL-positive cells, while the 25 specimens PCR positive by 1-step PCR gave mean counts of 40 +/- 7% TUNEL-positive cells. Thus, the number of TUNEL positive cells present in tissues increased with increasing severity of infection, as determined by gross signs of white spots on the cuticle, the number of intranuclear inclusions in histological sections, and results from single and nested PCR assays. DNA extracts of PCR-positive specimens tested by agarose gel electrophoresis showed indications of DNA fragmentation either as smears or as 200 bp ladders. Given that DNA fragmentation is generally considered to be a hallmark of apoptosis, the results suggested that apoptosis might be implicated in shrimp death caused by WSSV.

    One new genus of the tribe Ledrini, Yelahanka gen. nov. is described with Petalocephala granulosa Distant as its type species. The genus is characterised by the flexing of the forewing laterad of a strongly carinate or keeled outer claval vein and the claval veins fused in the distal two thirds. Seven new species, Yelahanka canaraica sp. nov. (India: Karnataka), Y. kodaiensis sp. nov. (India: Tamil Nadu), Y. montana sp. nov. (India: Tamil Nadu), Y. nepalica sp. nov. (Nepal), Y. sikkimensis sp. nov. (India: Sikkim), Y. shillongensis sp. nov. (India: Meghalaya) and Y. trifida sp. nov. (India: Himachal Pradesh, Arunachal Pradesh) are described and illustrated. The following new combinations are proposed: Yelahanka bainbriggei (Distant) comb. nov., Y. granulosa (Distant) comb. nov., Y. punctata (Walker) comb. nov. and Y. tabulata (Distant) comb. nov.; originally punctata was placed in the genus Ledra Fabricius and remaining species were placed in Petalocephala Stål. Petalocephala bainbriggei Distant 1916 is treated as a junior synonym of Y. granulosa (Distant 1910) syn. nov. and P. tabulata Distant 1908 is treated as a junior synonym of Yelahanka punctata (Walker 1851) syn. nov. Y. granulosa is newly recorded from Africa (Tanzania and Kenya) and Malaya. Relationships of the new genus with other genera of Ledrini are discussed and a key to species of the genus is also provided.

    Scrotal circumference (SC) and testicular volume (TV) were measured in 65 swamp buffalo bulls ranging in age from 7 to 60 months and weighing 130 kg to 560 kg. Ages and body weight (BW) were recorded for each male at the time of measurement to find out if they correlated with SC and TV. SC and TV increased linearly and correlated significantly with age and BW (SC vs age: r=0.74, p<0.01; SC vs BW: r=0.88, p<0.01; TV vs BW: r=0.82, p<0.01). SC measurements ranged from 15.1+/-1.1 cm to 24.0+/-0.4 cm for ages ranging from 10.0+/-0.6 to 48.5+/-6.3 months, revealing that testicular size in swamp buffaloes was very much smaller than domestic cattle. The SC norms distributed with age would be useful in the evaluation of swamp buffalo males for breeding soundness.

    The blue phase liquid crystal (BPLC) is a highly ordered liquid crystal (LC) phase found very close to the LC-isotropic transition. The BPLC has demonstrated potential in next-generation display and photonic technology due to its exceptional properties such as sub-millisecond response time and wide viewing angle. However, BPLC is stable in a very small temperature range (0.5-1 °C) and its driving voltage is very high (∼100 V). To overcome these challenges recent research has focused on solutions which incorporate polymers or nanoparticles into the blue phase to widen the temperature range from around few °C to potentially more than 60 °C. In order to reduce the driving voltage, strategies have been attempted by modifying the device structure by introducing protrusion or corrugated electrodes and vertical field switching mechanism has been proposed. In this paper the effectiveness of the proposed solution will be discussed, in order to assess the potential of BPLC in display technology and beyond.

    Hybrid tilapias (Oreochromis niloticus x O. mossambicus) were studied to determine gastric emptying and gastrointestinal transit time. The fish were divided into two groups. All fish were fasted for 12 h. The first experiment consisted of 11 fish fed with commercial food pellets and then administration of barium sulfate directly into the stomach using a blunt-end needle. Fish were then radiographed at different time intervals. The second experiment consisted of eight fish, which were given only barium sulfate after 12 h of fasting. In the first experiment, the stomach emptied completely, ranging from 4 to 15h postcontrast medium administration, whereas the gastrointestinal transit time ranged between 10 and 26 h. As for the second experiment, the contrast medium was still retained in the stomach in 50% of the fish after 24 h. The study did not proceed beyond 24 h as fish were considered stressed after this period of time. Thus, no data for the gastrointestinal transit time was obtained.

    We have studied residual limb length inequality following femoral shaft fractures in 62 children. From 61.2% of the children who had shortening of more than 1 cm at union, 34.21% still maintained the shortening at the completion of study. The longitudinal femoral overgrowth occurred significantly during the first 18 months of the fracture in 77.4% of the children, with an average of 1.17 cm. Children with proximal-third fractures and those who sustained the fractures before eight years of age have higher capability to correct the limb length disparity.

    Plasmasitoma ekstramedulari adalah komplikasi myeloma berbilang yang sangat
    jarang berlaku. Kami melaporkan kes seorang wanita berumur 56 tahun yang
    menghidap myeloma berbilang yang telah mengalami bengkak pada mata kiri. Ia
    menyebabkan bola mata beliau tersembul, luka pada kornea dan juga kemerosotan
    penglihatan. Biopsi tisu menunjukkan perebakan plasma sel neoplastik yang positif
    kepada CD138 dan rantai ringan Kappa yang menepati ciri-ciri plasmasitoma.
    Setelah melalui rawatan radio-kemoterapi, pembengkakan mata susut dengan
    ketara sekali namun kemerosotan penglihatan beliau tidak berubah.

    Fluids with different properties would exhibit different flow behaviour in any multiphase flow system at a given operating
    condition. Therefore, an in-depth knowledge of the operational and flow behaviour of any known fluid properties in a
    multiphase flow system of either liquid-liquid two-phase flow (oil and water) or gas-liquid-liquid three-phase flow (gas,
    oil and water) would be helpful in designing of pipelines and optimization of the production, separation, transportation
    and distribution systems, as may be found in oil and gas and allied petro-chemical industries. This paper presents the
    experimental observation of the flow patterns and water holdup for a two-phase low viscosity oil-water flow in horizontal
    pipes. The test fluids comprised of tap water and 2D-diesel which has a density of 832 kg/m3
    , viscosity of 3.24 mPa.s,
    surface tension of 0.030 N/m and flash point of 79°C. A total of 30 runs has been accomplished and the experimental
    results showed three different flow patterns identified as stratified flow (ST), stratified flow with mixing interface (ST&MI)
    and water-in-oil dispersed flow (), with superficial velocities of oil and water in the ranges of 0.32 - 0.87 m/s (Vos) and
    0.20 - 0.90 m/s (Vws), respectively. However, analysis of water holdup in the commingled flows of the test fluids showed
    its dependency on the fluid flow patterns and superficial velocity of water.

    Embryonal rhabdomyosarcoma of external ear canal is very rare, even in the pediatric population. We report an embryonal rhabdomyosarcoma of external acoustic canal, occurring in a 12-year-old Chinese girl who presented with left ear canal mass. Wide local excision of the mass was done under local anesthesia. Histopathological examination revealed the diagnosis. She then underwent 20 cycles of radiotherapy. CECT scan post treatment showed clearance of tumor cell. Now, patient is disease free for 5 years. We believe that early diagnosis followed by complete resection of the tumor with clear margin and radiotherapy improves the prognosis of the disease.

    AIMS: This study aims to compare microvascular endothelial function between vitamin D-deficient and nondeficient groups of patients with diabetic nephropathy. Serum levels of the inflammatory marker high-sensitivity C-reactive protein (hsCRP) were also measured.

    METHODS: This prospective cross-sectional study involved 70 patients with diabetic nephropathy; 40 were categorized into the group with nondeficient serum 25-hydroxyvitamin D levels [25(OH)D >50 nmol/l], whereas 30 patients were categorized to the group with deficient serum 25(OH)D (<50 nmol/l). Microvascular endothelial function was determined using laser Doppler fluximetry and the process of iontophoresis. Acetylcholine and sodium nitroprusside were used to determine endothelium-dependent and independent vasodilatation.

    RESULTS: Mean age of patients was 56.7 ± 3.8 years; 50 were men, whereas 20 were women. Mean serum 25(OH)D in the vitamin D-nondeficient group was 69.4 ± 2.9 nmol/l; the level in the vitamin D-deficient group was 42.1 ± 1.3 nmol/l, P < 0.001. Endothelium-dependent vasodilatation was lower in the vitamin D-deficient group compared with the vitamin D-nondeficient group (23.6 ± 2.7 versus 37.3 ± 3.8 arbitrary units, P = 0.004). No significant differences were observed between the two groups in their hsCRP levels, mean age, estimated glomerular filtration rate, systolic blood pressure (SBP) and diastolic blood pressure (DBP) and glycosylated haemoglobin.

    CONCLUSION: Microvascular endothelial function was significantly reduced in diabetic nephropathy patients with deficient vitamin D levels compared with those with nondeficient levels.

    Nurse managers have the burden of experiencing frequent ethical issues related to both their managerial and nursing care duties, according to previous international studies. However, no such study was published in Malaysia. The purpose of this study was to explore nurse managers' experience with ethical issues in six government hospitals in Malaysia including learning about the way they dealt with the issues.

    Dural exposure may occur during the course of thinning the tegmen tympani and tegmen mastoideum in mastoid procedure. If large area of dura is exposed or lacerated, cerebrospinal fluid and brain herniation may enter the mastoid cavity. We report a case of a patient with injured dura mater and tegmen mastoideum during mastoidectomy for chronic suppurative otitis media with cholesteatoma managed by using DuraGen. The dura mater and tegmen defect healed totally showing the success of the procedure. A collagen matrix like DuraGen is an option for repairing dural tear in mastoid region.

    OBJECTIVE: To determine the diagnostic accuracy of the two questions with help question (TQWHQ) in the Malay language. The two questions are case-finding questions on depression, and a question on whether help is needed was added to increase the specificity of the two questions.
    METHOD: This cross sectional validation study was conducted in a government funded primary care clinic in Malaysia. The participants included 146 consecutive women patients receiving no psychotropic drugs and who were Malay speakers. The main outcome measures were sensitivity, specificity, and likelihood ratios of the two questions and help question.
    RESULTS: The two questions showed a sensitivity of 99% (95% confidence interval 88% to 99.9%) and a specificity of 70% (62% to 78%), respectively. The likelihood ratio for a positive test was 3.3 (2.5 to 4.5) and the likelihood ratio for a negative test was 0.01 (0.00 to 0.57). The addition of the help question to the two questions increased the specificity to 95% (89% to 98%).
    CONCLUSION: The two qeustions on depression detected most cases of depression in this study. The questions have the advantage of brevity. The addition of the help question increased the specificity of the two questions. Based on these findings, the TQWHQ can be strongly recommended for detection of depression in government primary care clnics in Malaysia. Translation did not apear to affect the validity of the TQWHQ.

    The mechanism and enantioselectivity of the organocatalytic Diels-Alder reaction were computationally investigated by density functional theory at the B3LYP/6-31G(d) level of theory. The uncatalyzed Diels-Alder reaction was also studied to explore the effect of the organocatalyst on this reaction in terms of energetics, selectivity, and mechanism. The catalyzed reaction showed improved endo/exo selectivity, and the free energy of activation was significantly lowered in the presence of the catalyst. Both uncatalyzed and catalyzed reactions exhibited concerted asynchronous reaction mechanism with the degree of asynchronicity being more evident in the presence of the catalyst. The Corey's experimentally derived predictive selection rules for the outcome of the organocatalytic Diels-Alder reaction were also theoretically analyzed, and an excellent agreement was found between experiment and theory.
