BACKGROUND AND AIM: A critical causative factor of oxidative stress and inflammation leading to several skin complications is ultraviolet-B (UVB) irradiation. Lignosus rhinocerus (LR), or tiger milk mushroom, is native to Southeast Asia. Cold water extract of an LR cultivar, TM02® (xLr®) is a promising anti-oxidant and anti-inflammatory source. However, the effects of xLr® on UVB-induced photoaging have never been elucidated.

EXPERIMENTAL PROCEDURE: This study investigated the protective effects of xLr® and its high, medium, and low molecular weight (HLR, MLR, and LLR, respectively) fractions against UVB irradiation using in vivo Caenorhabditis elegans (C. elegans) model.

RESULTS AND CONCLUSION: The investigation revealed a significant lifespan extension of xLr® and its fractions in UVB-irradiated C. elegans, which could be mediated by the regulation of genes associated with anti-oxidant (daf-16 and sod-3) and apoptosis (cep-1, hus-1, ced-13, and egl-1) pathways. xLr® significantly reduced the ROS production in C. elegans and increased the DAF-16 nuclear translocation compared to untreated worms. Additionally, the SOD-3 expression was increased in the xLr®-treated worms. Hence, it suggests that the different components in xLr® work synergistically to protect against UVB irradiation. Our findings may be beneficial for the application of xLr® as a treatment against UVB-induced cellular damage and photoaging.

BACKGROUND: Planned gene therapies for retinitis pigmentosa (RP) depend on viable photoreceptors for efficacy. Understanding disease severity at presentation, and drivers that influence time to presentation is important when planning interventions. We examined features that influence RP severity at initial presentation.

METHODS: Multi-centre retrospective cohort study of RP patients at initial presentation. Disease severity was scored using ellipsoid zone (EZ) width on SD-OCT and logistic regression used to determine risk factors for advanced disease at presentation.

RESULTS: A total of 146 unrelated RP patients were included. Median age at onset and presentation was 40.5 (range 1-74) and 50.1 (range 3.9-81.8), respectively. Severe disease (<5° of remaining EZ width) was present in 28.1% of cases at presentation. Patients with family history of RP had greater odds of severe disease (OR 3.29, 95% CI 1.56, 6.95; p = 0.002), while male gender, race, age, syndromic features, and socioeconomic status did not. Patients with affected siblings (median EZ width 6.2°; p = 0.01), but not affected parents (median EZ width 9.4°; p = 0.99), presented with severe EZ loss compared to patients without family history (median EZ width 13.1°). Patients with affected siblings had delayed presentation (≥5 years; OR 5.76, 95% CI 1.817, 18.262; p = 0.003) compared to patients without family history.

CONCLUSIONS: Family history influences the stage of disease at which RP patients initially seek ophthalmology review. This has implications for patient counselling and the number of patients who may benefit from future therapies.

This study investigated the effects of pineapple peel powder with varied chemical profiles and sonication-assisted polyphenol biotransformation during fermentation on the quality characteristics of yogurt products. It aimed at exploring the feasibility of sonication-assisted fermentation to enhance the physicochemical properties, control post-acidification, and improve antioxidant activities in yogurts fortified with polyphenol-rich pineapple peel powder. Targeted analysis showed that polyphenol-rich pineapple dietary fiber obtained by ultrasonication-assisted extraction (NPFU) exhibited the slowest rates of acidification, highest antioxidant capacity, and lowest degree of whey separation at 21.67 %. Sonication pretreatments significantly increased transformation of free phenolic acids derived from pineapple peel fiber during fermentation, particularly increasing the accumulation of ferulic acid, caffeic acid and 5-hydroxyflavone, revealing the positive effects of sonication-mediated fermentation in promoting the hydrolysis of conjugated phenolics into free fractions. Yogurts fortified with pineapple peel fiber displayed significantly higher antioxidant activities (p

OBJECTIVES: Periodontal disease is a significant public health concern among older adults due to its relationship with tooth loss and systemic health disease. However, there are numerous barriers that prevent older adults from receiving routine dental care, highlighting the need for innovative screening tools at the community level. This pilot study aimed first, to evaluate the accuracy of GumAI, a new mHealth tool that uses AI and smartphones to detect gingivitis, and the user acceptance of personalized oral hygiene instructions provided through the new tool, among older adults in day-care community centers.

METHODS: Participants were invited from 3 day-care community centers. Intraoral photographs were captured and assessed by both GumAI (test) and a panel consisting of 2 calibrated periodontists and a dentist (benchmark). Mean sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), accuracy, and F1 score were calculated to determine GumAI's diagnostic performance in comparison to the benchmark. User acceptance with this tool was assessed using 2 Rasch Theory-based 5-point Likert-type questions.

RESULTS: 44 participants were recruited out of 80 invited older adults. GumAI demonstrated a sensitivity of 0.93 and specificity of 0.50 compared to the panel's assessments, with a PPV of 0.90 and NPV of 0.56. The accuracy and F1 scores were 0.85 and 0.91, respectively. All participants expressed high acceptance of the process.

CONCLUSION: GumAI demonstrates high sensitivity, PPV, accuracy, and F1 score compared to the panel's assessments but falls relatively short in specificity and NPV. Despite this, the tool was highly accepted by older adults, indicating its potential to enhance gingivitis detection and oral hygiene management in community settings. Further refinements are necessary to improve specificity and validate usability measures.

CLINICAL RELEVANCE: This study may pave the way for broader applications of mHealth systems in community settings, enabling greater health coverage and addressing oral health disparities.

BACKGROUND: Gastric accommodation (GA) testing is gaining clinical recognition as novel and minimally invasive modalities emerge. We investigated the feasibility of hybrid nuclear imaging volumetry (SPECT/CT) and combined high-resolution manometry-nutrient drink test (HRM-NDT) to assess GA.

METHODS: In this non-randomized pilot study, [99mTc]NaTcO4 gastric SPECT/CT (250 mL protocol) and proximal gastric HRM-NDT (~60 mL/min protocol) were performed separately within 30 days using Ensure Gold test meal (1.05 kcal/mL; Abbott). GA parameters were measured, and their preliminary associations were examined using Spearman's ρ and Hoeffding's D correlation tests. Data were presented as median ± normalized median absolute deviation.

KEY RESULTS: Twenty healthy, asymptomatic individuals (11 females; 23.5 ± 2.2 years, 23.7 ± 2.2 kg/m2) completed both procedures without serious adverse events and interrupted sessions. The accommodation volume and postprandial-to-fasting volume ratio from SPECT/CT were 325.8 ± 28.5 mL and 5.31 ± 1.28, respectively. During HRM-NDT, the nadir-intragastric pressure (IGP) was -6.6 ± 3.6 mmHg at an ingested volume of 360.0 ± 177.9 mL, and the area-under-curve of IGP was -1566.0 ± 1596.8 mmHg·mL. The maximum tolerated volume for reaching satiety/maximum discomfort was 450.0 ± 177.9 mL, and the area-under-curve of satiation score was 900.0 ± 266.9 satiation-unit·mL. The area-under-curve of IGP showed significant associations with maximum tolerated volume (ρ: -0.702; D: 0.234) and the area-under-curve of satiation score (D: 0.119): all p

BACKGROUND: Truancy is correlated with variety of unfavorable outcomes that can negatively impact adolescents' development and well-being. We investigated the prevalence of truancy and its associated factors among school-going adolescents in Malaysia in 2022.

METHODS: We used secondary data from the National Health and Morbidity Survey 2022. It is a cross-sectional study with a multistage stratified cluster sampling study design including 32,290 students aged 13 to 17 years old. It utilized the self-administered questionnaire from the validated Malaysian Global School-based Health Survey (GSHS). The associations between the truancy and its associated factors were examined using multivariate logistic regression, a complex sample analysis with sampling weightage.

RESULTS: The prevalence of truancy was 25.6% (95% CI=24.25, 26.98). In the multivariate analysis, truancy was significantly associated with older age groups (P<0.001), adolescents with separated/ divorced/ widowed parents (P<0.001), those with current any tobacco product use (P<0.001), those with ever drug use (P<0.001), those who have been physically attacked (P<0.001) and being bullied (P<0.001).

CONCLUSION: Gender, age, parent marital status, and adolescents with risk behavior were associated with truancy. This information may provide evidence for the implementation of effective prevention and intervention strategies to reduce truancy among these school-going adolescents.

BACKGROUND: Palliative Care, Geriatrics and Emergency physicians are exposed to death, terminally ill patients and distress of patients and their families. As physicians bear witness to patients' suffering, they are vulnerable to the costs of caring-the emotional distress associated with providing compassionate and empathetic care to patients. If left unattended, this may culminate in burnout and compromise professional identity. This study aims to provide a better understanding of suffering across various practice settings and specialties to guide the design of support frameworks for physicians and their patients.

METHODS: From August 2023 to September 2024, semi-structured interviews were conducted with sixteen Palliative Care, 12 Geriatrics and 13 Emergency physicians from various hospitals in Malaysia. Interview transcripts were analyzed using both inductive and deductive qualitative analyses.

RESULTS: Data analysis revealed three key domains: (1) living and dying well, (2) definition of suffering, and (3) impact of patient suffering on physicians.

CONCLUSION: Physicians' concepts of a good life and death frame their notions of suffering beyond the antithesis of a good life. Suffering is found to be distress at a loss of control, independence and dignity, alongside the presence of physical, emotional and existential distress. Witnessing patient suffering predisposes to physician suffering as they question their goals and roles in patient care. Our findings underscore the need for host organizations, hospitals and clinical departments to invest more in the care of their physicians. We believe these findings ought to be applicable to many resource-limited nations and other health care professionals beyond Malaysian shores.

BACKGROUND: The optimal control of type 2 diabetes (T2D) is defined by the innate mastery of self-management behaviours. This study is designed to condense the lived experiences of people with T2D in relation to factors 'exterior' to themselves into a universal self-management inventory (Assessment of Self-Management Questionnaire in Diabetes Mellitus-External Reality; ASQ-DM-EX).

METHODS: We collected responses to an online and physical survey from people living with T2D through a quantitative cross-sectional study. First, we tested the contextual validity and cultural preciseness of ASQ-DM-EX prototype through a preliminary pilot testing phase. These processes culminated in the development of a 91-item version of the questionnaire which was disseminated widely to evaluate the predictive strength of the ASQ-DM-EX.

RESULTS: All constructs within ASQ-DM-EX showed internal consistencies within good to excellent range (Cronbach's ⍺ = 0.70-0.94), except for the Influence construct (Cronbach's ⍺ = 0.33). An increase in ASQ-DM-EX scores was associated with a reduction in HbA1c control (r = -0.17, P

We report a first case of ceftazidime-resistant pediatric melioidosis involving a previously healthy seven-year-old boy who presented with right lobar pneumonia complicated with a 5-cm lung abscess. Ceftazidime was initiated on Day-6 of admission when Burkholderia pseudomallei (ceftazidime-susceptible, minimum inhibitory concentration [MIC] 1.0 mcg/mL) was isolated from blood. Despite ceftazidime therapy at the recommended dosage, he developed fulminant septic shock and respiratory failure on Day-18 of hospitalization, requiring invasive ventilation, hemodynamic support, and continuous renal replacement therapy. His antibiotic was empirically escalated to meropenem; ceftazidime-resistant B. pseudomallei (MIC 32 mcg/mL) was subsequently isolated from blood and endotracheal secretions. He improved after two weeks of intensive care and was discharged well after two months of hospitalization. Our literature review on ceftazidime-resistant B. pseudomallei infection indicates that acquired resistance is a rare but potentially lethal treatment-related complication. All melioidosis patients should be carefully monitored during treatment with ceftazidime (or other β-lactams) for the development of antimicrobial resistance.

Thermal cycling failure of solder joints in surface-mount passive components represents a core bottleneck limiting the reliability of next-generation high-frequency communication systems. Conventional microwave dielectric ceramics exhibit low coefficients of thermal expansion (CTE, 5-12 ppm/°C), causing severe thermal mismatch with commercial SAC305 lead-free solder (21.9 ppm/°C) and copper conductors, which generates enormous thermal stress and induces solder joint cracking during temperature cycling. Herein, we report an intrinsically high-CTE microwave dielectric ceramic Na3P3O9 with a CTE of 22.25 ppm/°C, matching almost perfectly with SAC305 solder. At its optimal sintering temperature of 610 °C, this material simultaneously possesses an ultralow permittivity (εr = 4.88) and ultralow dielectric loss (on the order of 10-4). The physical origins of its excellent comprehensive properties are elucidated via crystal structure analysis and chemical bond theory. Finite element simulations reveal that under the standard electronic packaging thermal cycling condition of -40 to 125 °C, the thermal stress of solder joints on Na3P3O9 is only 54% of that on commercial A6M and 30% of that on conventional alumina. A Ku-band broadband dielectric resonator antenna designed using this material achieves an impedance bandwidth of 3.1 GHz and an in-band total efficiency of >87.2%, fully covering the transmit and receive bands of fixed-satellite and broadcasting satellite services and enabling transmit-receive integration for low Earth orbit satellite terminals. This work provides a high-CTE dielectric material system for addressing the solder joint reliability issue in high-frequency passive components.

Delayed antibiotics increase mortality in bloodstream infection (BSI). Direct-from-blood-culture disk diffusion antibiotic susceptibility testing (dAST) forecasts susceptibility earlier than conventional susceptibility testing (cAST). The study aimed to evaluate dAST performance and its impact on antibiotic adjustment in BSI. In this Malaysian single-center prospective study, dAST was performed and interpreted according to breakpoints by Clinical and Laboratory Standards Institute (CLSI) guidelines. The turnaround time (TAT), categorical agreements (CA), and predictive values of dAST for susceptibility by cAST were determined among positive blood cultures (PBC) between November 2022 and November 2023. The active and WHO AWaRe antibiotics administered before and after dAST results were compared. Of the 318 PBCs, the median of TAT was earlier than cAST by 35 hours. The CA for 3,561 organism-antibiotics combinations was 91.5%, with 3.6% very major errors (VME), 3.3% major errors (ME), and 5.2% minor errors (mE). The dAST achieved CA above 90% for Staphylococcus aureus with cefoxitin (98.5%), Pseudomonas aeruginosa with ceftazidime (100%), and Acinetobacter baumannii with ampicillin/sulbactam (100%). For Enterobacterales, most combinations were above 90%, including ampicillin (95.1%), ceftriaxone (95.7%), and meropenem (95.7%), though those of the aminopenicillin/inhibitor combinations were above 80%. Most errors were attributed to mEs. Among 159 BSI, the prescribing of active antibiotics improved significantly following dAST (73.0% versus 89.3%, P < 0.001) but not WHO Access antibiotics (35.8% versus 35.2%, P = 0.188). dAST had good CAs for most antibiotics, allowing earlier improvement in active antibiotics. The modest change in WHO Access antibiotics prescribing reveals practice gaps in need of antimicrobial stewardship.

IMPORTANCE: Global deaths attributable to antimicrobial resistance are rising. Hence, rapid susceptibility testing is essential for timely antibiotic de-escalation to mitigate antimicrobial resistance (AMR) development from exposure to broad-spectrum antibiotics. Compared to the costly advanced technology, direct disk diffusion from blood culture (diffusion antibiotic susceptibility testing [dAST]) is an affordable method that can be quickly adopted. However, the reliability of dAST in informing susceptibility was mainly reported from Western countries and scarcely from other regions, including Southeast Asia, where the AMR burden is high. This study from Malaysia adds insights into the performance of dAST and the potential to apply it in similar resource-limited settings from the same region. Furthermore, assessing the dAST's influence on antibiotic prescribing identifies the gap in implementation to guide areas of improvement for optimizing clinical utility.

Polymer blends have attracted significant research interest due to their potential use as power cable insulating materials. Specifically, polypropylene (PP) blends offer improved dielectric properties over conventional crosslinked polyethylene insulating materials attributable to PP's high melting temperatures, hence high rated voltages. Despite numerous promising findings have been reported regarding the potential application of PP blends as power cable insulating materials, there have been relatively less investigations into the dielectric effects of incorporating nanofillers into PP blends. The current work therefore explores the influence of calcined magnesia (MgO) nanofiller on the structure and dielectric properties of PP blended with ethylene-octene copolymer (EOC). Nanofiller-wise, calcination of MgO does not significantly affect the structure of MgO, albeit that water-related molecules are removed from MgO. Upon adding the calcined MgO to the PP/EOC blend, the breakdown performance of the PP/EOC/MgO blend nanocomposites becomes jeopardized, especially under the direct current field. This is primarily attributed to the presence of residue water molecules within the PP/EOC/MgO blend nanocomposites, even after MgO calcination. Although the addition of the calcined MgO to the PP blend does not result in favorable dielectric properties, the findings suggest that nanostructuration of PP blends could be further explored to pave the way for the development of nanostructured PP blends for use in advanced power cable insulation applications.

BACKGROUND: Aldosterone, produced in the zona glomerulosa (ZG) by aldosterone synthase (CYP11B2), regulates sodium balance and blood pressure. In adult adrenals, CYP11B2-expressing ZG cells are sparse, likely reflecting chronic suppression of the renin-angiotensin system under high-salt conditions. Whether reduced aldosterone synthesis directly influences ZG cell behavior remains unclear. This study examined the impact of CYP11B2 inhibition on human adrenal cell fate and function.

METHODS: CYP11B2 was inhibited in HAC15 cells using siRNA (siCYP11B2) or baxdrostat, a selective CYP11B2 inhibitor. Forty-eight hours post-treatment, steroid hormone levels, RNA sequencing profiles, and functional outputs (EZMTT metabolic assay, xCELLigence real-time cell analysis, and Annexin V/SYTOX flow cytometry for apoptosis/necrosis) were evaluated.

    RESULTS: siCYP11B2 reduced aldosterone production by 70% ± 2.9% (n = 18, p

BACKGROUND: Access to neurosurgical care in remote regions remains challenging because of the uneven distribution of specialists, transportation limitations, and infrastructure constraints. For many years, patients from East Coast Sabah relied on transfer to Queen Elizabeth Hospital, Kota Kinabalu, for emergency and elective neurosurgical care, often requiring long-distance evacuation. This study describes the phased development of a decentralised neurosurgical service in Tawau and evaluates associated regional service trends.

METHODS: This descriptive historical vignette and service evaluation reviewed aggregated neurosurgical data from Tawau Hospital, Duchess of Kent Hospital (Sandakan), and Lahad Datu Hospital from 2019 through April 2026. Service activity was analysed across three phases: the flying surgeon model, rotational specialist postings, and permanent specialist deployment. Referrals, admissions, outpatient attendance, operative activity, interhospital transfers, and mortality were evaluated descriptively.

RESULTS: The East Coast Sabah Neurosurgical Service evolved from a monthly visiting service to rotational specialist coverage and, ultimately, a permanent Tawau-based hub in 2025. Referrals, outpatient attendance, and operative workload increased progressively across the three phases. Mean annual referrals increased from 1,388 to 1,802 and reached an annualised 2,130 during early Phase 3. Annual outpatient attendance increased from 1,117 to 1,675, with an annualised total of 2,526 visits. Operative volume increased from 202 to 366, with an annualised total of 374 procedures, while the proportion of elective procedures rose from 1.9% to 16.5%. Crude inpatient mortality rates were 5.8%, 4.9%, and 3.2% across the three phases. Following establishment of the permanent hub, emergency transfers from Tawau decreased from approximately 35 per year during Phase 2 to an annualised 18 per year during early Phase 3, corresponding to a crude administrative estimate of annual government cost avoidance of approximately RM425,000 to RM680,000 through reduced emergency air evacuations.

CONCLUSION: The phased decentralisation of neurosurgical services in Tawau was associated with expanded local capacity, improved continuity of care, and reduced reliance on transfers to a tertiary referral centre for selected neurosurgical conditions.

INTRODUCTION: Small intestinal bacterial overgrowth (SIBO) is prevalent in irritable bowel syndrome (IBS), but its' association with other functional gastrointestinal disorders (FGIDs) is less certain. This study aimed to explore SIBO in a multi-racial Asian population with various FGIDs compared to non-FGID controls.

METHODOLOGY: Consecutive Asian adults with Rome III diagnosed common FGIDs (functional dyspepsia/FD, IBS and functional constipation/FC) and non-FGID controls were subjected to glucose breath testing, with hydrogen (H2) and methane (CH4) levels determined.

RESULTS: A total of 244 participants (FGIDs n = 186, controls n = 58, median age 45 years, males 36%, Malay ethnicity 76%) were recruited. FGIDs had a higher prevalence trend of SIBO compared to controls (16% FGIDs vs. 10% controls, p = 0.278) with 14% in FD, 18% in IBS and 17% in FC. Compared to controls, SIBO was associated with diarrhoea-predominant IBS (IBS-D) (24% vs. 10%, P = 0.050) but not with other types of FGIDs. IBS-D remained an independent predictor of SIBO (OR = 2.864, 95% CI 1.160-7.071, p = 0.023) but not PPI usage nor history of diabetes (both p > 0.050) at multivariate analysis. Compared to controls, SIBO in IBS-D was associated with an elevated H2 level (≥ 20 ppm from baseline) (18% vs. 3%, p = 0.017), but not CH4 levels (≥ 10 ppm) (9% vs. 7%, p = 0.493). In addition, no difference was found in the prevalence of methane-positive SIBO between chronic constipation (constipation-predominant IBS and FC) compared to controls (9% vs. 7%, P = 0.466).

CONCLUSION: SIBO is prevalent amongst multi-ethnic Asian adults with and without FGIDs. Amongst various FGIDs, only IBS-D is significantly associated with SIBO.

This study examined the prevalence, pattern, and predictors of death-related distress among 156 cancer patients in a multi-ethnic Malaysian setting. The main concerns related to the dying process and its consequences, particularly prolonged dying, becoming a burden to others, the impact of death on loved ones, and unfinished life goals. Moderate-or-greater distress affected 39% of patients for prolonged dying and approximately one-third for the other major concerns. Multivariable analysis showed that greater death-related distress was independently associated with younger age, mild functional decline reflected by ECOG performance status 1, higher anxiety, and greater suffering. These factors collectively explained a substantial proportion of variation in distress scores. The findings indicate that death-related distress is common among cancer patients and may not be limited to those with advanced physical impairment. Routine screening should therefore focus particularly on younger patients, those showing early functional deterioration, and individuals experiencing significant anxiety or global suffering.

Understanding particle dispersion characteristics in indoor environments is crucial for revising infection prevention guidelines through optimized engineering control. The secondary wake flow induced by human movements can disrupt the local airflow field, which enhances particle dispersion within indoor spaces. Over the years, researchers have explored the impact of human movement on indoor air quality (IAQ) and identified noteworthy findings. However, there is a lack of a comprehensive review that systematically synthesizes and summarizes the research in this field. This paper aims to fill that gap by providing an overview of the topic and shedding light on emerging areas. Through a systematic review of relevant articles from the Web of Science database, the study findings reveal an emerging trend and current research gaps on the topic titled Impact of Human Movement in Indoor Airflow (HMIA). As an overview, this paper explores the effect of human movement on human microenvironments and particle resuspension in indoor environments. It delves into the currently available methods for assessing the HMIA and proposes the integration of IoT sensors for potential indoor airflow monitoring. The present study also emphasizes incorporating human movement into ventilation studies to achieve more realistic predictions and yield more practical measures. This review advances knowledge and holds significant implications for scientific and public communities. It identifies future research directions and facilitates the development of effective ventilation strategies to enhance indoor environments and safeguard public health.

The present study examines the effect of medical staff's turning movements on particle concentration in the surgical zone and settlement on the patient under single large diffuser (SLD) ventilation. A computational domain representing the operating room (OR) was constructed using computer-aided design (CAD) software. The airflow and particle models were validated against the published data before conducting the case studies. The airflow in the OR was simulated using an RNG k-ε turbulence model, while the dispersion of the particles was simulated using a discrete phase model based on the Lagrangian approach. A user-defined function (UDF) code was written and compiled in the simulation software to describe the medical staff member's turning movements. In this study, three cases were examined: baseline, SLD 1, and SLD 2, with the air supply areas of 4.3 m2, 5.7 m2, and 15.9 m2, respectively. Results show that SLD ventilations in an OR can reduce the number of dispersed particles in the surgical zone. The particles that settled on the patient were reduced by 41% and 39% when using the SLD 1 and SLD 2 ventilations, respectively. The use of the larger air supply area of SLD 2 ventilation in the present study does not significantly reduce the particles that settle on a patient. Likewise, the use of SLD 2 ventilation may increase operating and maintenance costs.
