    INTRODUCTION: Infections linked to orthopedic trauma are common complications that place a significant strain on the healthcare system. Immediate identification of the infection and its severity is essential for providing effective treatment.

    METHOD: C-reactive Protein (CRP) is a commonly used inflammatory marker in orthopedic surgery and has proven to be a valuable biomarker for diagnosing and monitoring infections. Specifically, CRP aids in the early identification of postoperative infections. This research work has focused on developing a highly sensitive CRP biosensor using iron oxide nanomaterial-modified dielectric sensors.

    RESULT: Gold Urchin (GU)-conjugated aptamers and antibodies were used as probes and attached to the electrode via amine linkers. The aptamer-GU-antibody-modified electrode detected CRP at concentrations as low as 1 pg/mL, with an R2 value of 0.9942. Furthermore, CRP-spiked serum exhibited an increase in current response at all concentrations of CRP, indicating selective detection of CRP. Additionally, control experiments using complementary sequences of the aptamer, relevant proteins, and non-immune antibodies did not enhance the current responses, confirming the specific identification of CRP.

    CONCLUSION: The sensing strategy has enabled the detection of CRP at its lowest levels, facilitating the identification of infections during orthopedic surgery and subsequent treatment.

    Since the Tactical Games Model (TGM) was adopted and popularised by Oslin, Griffin, and Mitchell, research interest in this model has surged, increasing its application in teaching and research. However, much-existing research is fragmented and lacks a comprehensive review. This systematic review aims to fill that gap by thoroughly analysing the literature on TGM within the context of physical education, highlighting current trends and developments. We systematically searched four online databases, identifying 38 relevant articles for inclusion, which were then evaluated across eight domains following PRISMA guidelines. The analysis indicates a steady increase in TGM-focused research within physical education, particularly at the K-12 level. However, there is a notable absence of studies addressing higher education, as well as teachers and coaches. The review also found that the majority of TGM research is concentrated on team sports and invasion games, with limited exploration of other sports. The research objectives often centred on extrinsic factors, such as game performance, skill level, and physical activity (PA) level, as well as intrinsic factors, intrinsic motivation, stress, and enjoyment. The TGM has shown varying levels of success in improving these factors. Despite the recognised benefits of TGM in physical education, its impact on cognitive aspects and decision-making remains underexplored. This systematic review may serve as a valuable resource for educators and researchers, supporting the broader application of TGM across different areas of physical education and potentially leading to enhanced student outcomes.

    Tranexamic acid is an antifibrinolytic drug that is widely used during surgery, but there are concerns about its thromboembolic effects. We aimed to investigate the effect of prophylactic intravenous tranexamic acid on thromboembolic outcomes in patients undergoing non-cardiac surgery. The MEDLINE, EMBASE and Cochrane Central Register of Controlled Trials were searched. Randomised controlled trials comparing intravenous tranexamic acid with placebo or no treatment in patients undergoing non-cardiac surgery were included. The primary outcome was a composite of peri-operative cardiovascular thromboembolic events, defined as any deep vein thrombosis, pulmonary embolism, myocardial ischaemia/infarction or cerebral ischaemia/infarction. A total of 191 randomised controlled trials (40,621 patients) were included in the review. The primary outcome occurred in 4.5% of patients receiving intravenous tranexamic acid compared with 4.9% of patients in the control group. Our analysis showed that there was no difference between groups for composite cardiovascular thromboembolic events (risk ratio 1.02, 95%CI 0.94-1.11, p = 0.65, I2 0%, n = 37,512). This finding remained robust when sensitivity analysis was performed with continuity correction and in studies with a low risk of bias. However, in trial sequential analysis, our meta-analysis only achieved 64.6% of the required information size. There was no association between intravenous tranexamic acid and seizure rate or mortality rate within 30 days.     In the context of global climate change and the influence of human activities, the concept of "sponge city" is put forward to realize the purification, collection, and reuse of rainwater. The effective evaluation of LID facilities in sponge cities is of great guiding significance for the promotion and construction of sponge cities. IFMS (Integrated Flood Modeling System) Urban was selected to construct the rainstorm simulation. LID parameters were added to simulate the improvement of urban waterlogging after the construction of sponge city. A reasonable disaster loss assessment method was used to calculate the disaster mitigation benefit brought by the construction of sponge city. Through the comparison of the inundation situation before and after LID facilities' construction, it can be concluded that the mitigation effect of LID facilities on the overall inundation area of the city decreases with the increase of rainfall recurrence period, with the maximum reduction rate reaching 13.63% in the 5-year recurrence period and the minimum reduction rate of 11.06% in the 50-year recurrence period. LID facilities have a better disaster reduction effect for rainfall events with a small recurrence period than for rainfall events with a large recurrence period.

    Pretreatment of waste-activated sludge (WAS) is an effective way to destabilize sludge floc structure and release organic matter for improving sludge digestion efficiency. Nonetheless, information on the impact of WAS pretreatment on digestion sludge microbiomes, as well as mechanistic insights into how sludge pretreatment improves digestion performance, remains elusive. In this study, a genome-centric metagenomic approach was employed to investigate the digestion sludge microbiome in four sludge digesters with different types of feeding sludge: WAS pretreated with 0.25 mol/liter alkaline/acid (APAD), WAS pretreated with 0.8 mol/liter alkaline/acid (HS-APAD), thermally pretreated WAS (thermal-AD), and fresh WAS (control-AD). We retrieved 254 metagenome-assembled genomes (MAGs) to identify the key functional populations involved in the methanogenic digestion process. These MAGs span 28 phyla, including 69 yet-to-be-cultivated lineages, and 30 novel lineages were characterized with metabolic potential associated with hydrolysis and fermentation. Interestingly, functional populations involving carbohydrate digestion were enriched in APAD and HS-APAD, while lineages related to protein and lipid fermentation were enriched in thermal-AD, corroborating the idea that different substrates are released from alkaline/acid and thermal pretreatments. Among the major functional populations (i.e., fermenters, syntrophic acetogens, and methanogens), significant correlations between genome sizes and abundance of the fermenters were observed, particularly in APAD and HS-APAD, which had improved digestion performance.IMPORTANCE Wastewater treatment generates large amounts of waste-activated sludge (WAS), which consists mainly of recalcitrant microbial cells and particulate organic matter. Though WAS pretreatment is an effective way to release sludge organic matter for subsequent digestion, detailed information on the impact of the sludge pretreatment on the digestion sludge microbiome remains scarce. Our study provides unprecedented genome-centric metagenomic insights into how WAS pretreatments change the digestion sludge microbiomes, as well as their metabolic networks. Moreover, digestion sludge microbiomes could be a unique source for exploring microbial dark matter. These results may inform future optimization of methanogenic sludge digestion and resource recovery.

    Covalent triazine frameworks (CTFs) have attracted tremendous attention with respect to their rich nitrogen content, functional triazine units, and high porosity. However, efficient and simple preparation of CTF monoliths is still a challenge. Here, we propose a novel and facile approach for the in situ preparation of CTF aerogels. Trifluoromethanesulfonic acid was used as the solvent and catalyst, which could not only promote the polymerization reaction to form a gel but also protonate the CTFs. By virtue of the simplicity and efficiency of the strategy, a series of macroscopic CTF aerogels with tunable density were obtained by adjusting the monomer concentration. Due to the porous and partially crystalline structure, the as-produced macroscopic CTF aerogels behaved with good mechanical and thermal insulation performances. This finding thus offers an effective and easily scalable approach toward fabricating macroscopic CTF aerogels and broadens the applications of CTFs in a variety of domains.

    To maintain the versatility of a hydrogel, extensive modifications are necessary, particularly to overcome the daunting mechanical trait of this material. In agriculture especially, achieving the desired balance between strength and high water absorption ability with this polymer is a significant challenge. Therefore, this study used and evaluated both carboxymethyl cellulose (CMC) mesofiber (CMCF) and CMC-chitosan mesofiber (CMC/CHF) as a reinforcing agent at varying concentrations in the widely known regenerated cellulose hydrogel. These fibers were fined and revamped as mesofiber before being integrated into the cellulose solution for crosslinking and formation stages. The hydrogel filled with 2 wt% mesofiber, especially CMC/CHF exhibited the highest storage modulus value (3300 Pa), compression strength (0.315 MPa), and thermal stability, showing the resistivity of this composite towards external pressure. Morphologically, the distribution of smaller pores within the mesofiber-reinforced hydrogel improved along with the water absorption ability. The composite hydrogels, however, demonstrated lower transparency compared to the plain hydrogel due to the high loading of CMCF and complex CMC/CHF. The utilization of CMC/CHF is especially successful and effective in enhancing the resulting composite's mechanical strength and hydrophilicity. Thus, it is expected to be beneficial as a planting medium that provides both functionality and vitality.

    Microsatellites are the most popular markers for parentage assignment and population genetic studies. To meet the demand for international comparability for genetic studies of Asian seabass, a standard panel of 28 microsatellites has been selected and characterized using the DNA of 24 individuals from Thailand, Malaysia, Indonesia and Australia. The average allele number of these markers was 10.82 +/- 0.71 (range: 6-19), and the expected heterozygosity averaged 0.76 +/- 0.02 (range: 0.63-1.00). All microsatellites showed Mendelian inheritance. In addition, eight standard size controls have been developed by cloning a set of microsatellite alleles into a pGEM-T vector to calibrate allele sizes determined by different laboratories, and are available upon request. Seven multiplex PCRs, each amplifying 3-5 markers, were optimized to accurately and rapidly genotype microsatellites. Parentage assignment using 10 microsatellites in two crosses (10 x 10 and 20 x 20) demonstrated a high power of these markers for revealing parent-sibling connections. This standard set of microsatellites will standardize genetic diversity studies of Asian seabass, and the multiplex PCR sets will facilitate parentage assignment.

    BACKGROUND & AIMS: Trends in long-term mortality rates for viral hepatitis in East and Southeast Asia have been rarely reported. The aim of our study was to explore the long-term trends in viral hepatitis mortality rates in East and Southeast Asian countries between 1987 and 2015 and provide predictions of mortality to 2030.

    METHODS: We obtained viral hepatitis mortality data from the WHO Mortality Database for six East and Southeast Asian countries between 1987 and 2015. We produced choropleth maps of viral hepatitis mortality rates in 1987 and 2015 in East and Southeast Asia to illustrate geographic variations. We made predictions of mortality rates for each included country until the year 2030 using a series of joinpoint models.

    RESULTS: Viral hepatitis mortality rates declined in China (the average annual percent change (AAPC) = -5.1%, 95% CI: -7.5, -2.6), Singapore (AAPC = -5.4%, 95% CI: -7.5, -3.2), and the Philippines (AAPC = -3.4%, 95% CI: -4.9, -1.8). In contrast, Japan, the Republic of Korea, and Malaysia have experienced increasing trends in mortality rates, followed by decreasing trends. Our predictions indicate that all countries will experience slight to moderate downward trends until 2030.

    CONCLUSION: Favourable decreasing trends have been noted in East and Southeast Asian countries, which may not only inform the control and management of viral hepatitis in this region but also guide the prevention of viral hepatitis deaths in another region with a similar viral hepatitis epidemic.

    OBJECTIVE: To evaluate predictors of difficult intubation in patients with obstructive sleep apnoea (OSA).

    METHODOLOGY: Prospective series of 405 OSA patients (350 males/55 females) who had upper airway surgery. Procedures included functional endoscopic sinus surgery, septoplasty, turbinate reduction, palate/tonsil surgery, and/or tongue base surgery. Intubation difficulty (ID) was assessed using Mallampati grade, Laryngoscopic grade (Cormack and Lehane), and clinical parameters including BMI, neck circumference, thyromental distance, jaw adequacy, neck movements and glidescope grading.

    RESULTS: Mean age was 41.6 years old; mean BMI 26.6; mean neck circumference 44.5cm; mean Apnea Hypopnea Index (AHI) was 25.0; and mean LSAT 82%. The various laryngeal grades (based on Cormack and Lehane), grade 1 - 53 patients (12.9%), grade 2A - 127 patients (31.0%), grade 2B - 125 patients (30.5%), grade 3 - 93 patients (22.7%) and grade 4 - seven patients (1.7%); hence, 24.4% had difficulties in intubation. Parameters that adversely affected intubation were, age of the patient, opening of mouth, retrognathia, overbite, overjet, limited neck extension, thyromental distance, Mallampati grade, and macroglossia (p<0.001). Body mass index (BMI) (p=0.087), neck circumference (p=0.645), neck aches (p=0.728), jaw aches (p=0.417), tonsil size (p=0.048), and AHI (p=0.047) had poor correlation with intubation. BMI-adjusted for Asians and Caucasians, showed that Asians were more likely to have difficulties in intubation (adjusted OR = 4.6 (95%Confidence Interval: 1.05 to 20.06) (p=0.043), compared to the Caucasian group.

    CONCLUSION: This study illustrates that difficult intubation can be predicted pre-surgery in order to avert any anaesthetic morbidity.

    Head and neck cancer is a heterogeneous disease, originating in the squamous cells lining the larynx (voice box), mouth, pharynx (throat), nasal cavity and salivary glands. Head and neck cancer diagnosis at the later stage is greatly influencing the survival rate of the patient. It makes a mandatory situation to identify this cancer at the earlier stages of development with a suitable biomarker. Squamous cell carcinoma antigen (SCC-Ag) is a circulating serum tumour biomarker, and the elevated level has been found in the head and neck cancer patients and highly correlated with the tumour volume. The present research was carried out to detect and quantify the level of SCC-Ag on titanium oxide (TiO2)-modified interdigitated electrode sensor (IDE) by SCC-Ag antibody. The detection of SCC-Ag was found at the level of 100 fM, while it was improved to 10 fM when the antibody was conjugated with gold nanostar, representing a 10-fold improvement. Interestingly, this enhancement in sensitivity is 1000-folds higher than other substrates. Moreover, the specificity analysis was carried out using two different control proteins and noticed that the antibody only recognised SCC-Ag, indicating the specific detection on IDE-TiO2 sensing surface.

    Toxoplasma gondii, a ubiquitous pathogen that infects nearly all warm-blooded animals and humans, can cause severe complications to the infected people and animals as well as serious economic losses and social problems. Here, one local strain (TgPIG-WH1) was isolated from an aborted pig fetus, and the genotype of this strain was identified as ToxoDB #3 by the PCR RFLP typing method using 10 molecular markers (SAG1, SAG2, alternative SAG2, SAG3, BTUB, GRA6, L358, PK1, C22-8, C29-2 and Apico). A comparison of the virulence of this isolate with other strains in both mice and piglets showed that TgPIG-WH1 was less virulent than type 1 strain RH and type 2 strain ME49 in mice, and caused similar symptoms to those of ME49 such as fever in piglets. Additionally, in piglet infection with both strains, the TgPIG-WH1 caused a higher IgG response and more severe pathological damages than ME49. Furthermore, TgPIG-WH1 caused one death in the 5 infected piglets, whereas ME49 did not, suggesting the higher virulence of TgPIG-WH1 than ME49 during piglet infection. Experimental infections indicate that the virulence of TgPIG-WH1 relative to ME49 is weaker in mice, but higher in pigs. This is probably the first report regarding a ToxoDB #3 strain from pigs in Hubei, China. These data will facilitate the understanding of genetic diversity of Toxoplasma strains in China as well as the prevention and control of porcine toxoplasmosis in the local region.

    Tetracycline is a potentially hazardous residual antibiotic detected in various sewages. High concentration (mg/L) of tetracycline is found in pharmaceutical/hospital wastewater and wastewater derived from livestock and poultry. So far, only antibiotics in μg/L level have been reported in granulation of aerobic sludge during wastewater treatment, but its effects in high concentration are rarely reported. In this study, the influence of tetracycline in high concentration (∼2 mg/L) on the formation of granular sludge, structure, and metabolic function of the microbial community during the granulation of aerobic sludge was investigated to improve the understanding of the aerobic granular sludge formation under high-level of tetracycline. The role of extracellular polymers substances (EPSs) derived from granular sludge in the granulation and tetracycline removal process was also investigated, showing that tetracycline improved the relative hydrophobicity, flocculability and protein/polysaccharide ratio of EPSs, accelerating the granulation of sludge. Succession of microbial communities occurred during the domestication of functional bacteria present in the sludge and was accompanied with regulation of metabolic function. The addition of tetracycline lead to an increase of tetracycline-degrading bacteria or antibiotic resistance genus. Those findings provide new perspectives of the influence of tetracycline on aerobic sludge granulation and the removal mechanism of tetracycline.

    The humpback puffer, Tetraodon palembangensis, is a poisonous freshwater pufferfish species mainly distributed in Southeast Asia (Thailand, Laos, Malaysia and Indonesia). The humpback puffer has many interesting biological features, such as inactivity, tetrodotoxin production and body expansion. Here, we report the first chromosome-level genome assembly of the humpback puffer. The genome size is 362 Mb, with a contig N50 value of ∼1.78 Mb and a scaffold N50 value of ∼15.8 Mb. Based on this genome assembly, ∼61.5 Mb (18.11%) repeat sequences were identified, 19,925 genes were annotated, and the function of 90.01% of these genes could be predicted. Finally, a phylogenetic tree of ten teleost fish species was constructed. This analysis suggests that the humpback puffer and T. nigroviridis share a common ancestor 18.1 million years ago (MYA), and diverged from T. rubripes 45.8 MYA. The humpback puffer genome will be a valuable genomic resource to illustrate possible mechanisms of tetrodotoxin synthesis and tolerance.

    BACKGROUND: The incidence of postsurgical venous thromboembolism is thought to be low in Asian ethnic populations.

    OBJECTIVE: We studied the incidence of deep-vein thrombosis (DVT) in Asian patients undergoing major orthopedic surgery of the lower limbs.

    PATIENTS/METHODS: We performed a prospective epidemiological study in 19 centers across Asia (China, Indonesia, South Korea, Malaysia, Philippines, Taiwan, and Thailand) in patients undergoing elective total hip replacement (THR), total knee replacement (TKR) or hip fracture surgery (HFS) without pharmacological thromboprophylaxis. The primary endpoint was the rate of DVT of the lower limbs documented objectively with bilateral ascending venography performed 6-10 days after surgery using a standardized technique and evaluated by a central adjudication committee unaware of local interpretation.

    RESULTS: Overall, of 837 Asian patients screened for this survey, 407 (48.6%, aged 20-99 years) undergoing THR (n = 175), TKR (n = 136) or HFS (n = 96) were recruited in 19 centers. DVT was diagnosed in 121 of 295 evaluable patients [41.0%, (95% confidence interval (CI): 35.4-46.7)]. Proximal DVT was found in 30 patients [10.2% (7.0-14.2)]. Total DVT and proximal DVT rates were highest in TKR patients (58.1% and 17.1%, respectively), followed by HFS patients (42.0% and 7.2%, respectively), then THR patients (25.6% and 5.8%, respectively). DVT was more frequent in female patients aged at least 65 years. Pulmonary embolism was clinically suspected in 10 of 407 patients (2.5%) and objectively confirmed in two (0.5%).

    CONCLUSIONS: The rate of venographic thrombosis in the absence of thromboprophylaxis after major joint surgery in Asian patients is similar to that previously reported in patients in Western countries.

    The COVID-19 outbreak led to the discovery of SARS-CoV-2 in sewage; thus, wastewater treatment plants (WWTPs) could have the virus in their effluent. However, whether SARS-CoV-2 is eradicated by sewage treatment is virtually unknown. Specifically, the objectives of this study include (i) determining whether a mixed matrixed membrane (MMM) is able to remove SARS-CoV-2 (polycarbonate (PC)-hydrous manganese oxide (HMO) and PC-silver nanoparticles (Ag-NP)), (ii) comparing filtration performance among different secondary treatment processes, and (iii) evaluating whether artificial neural networks (ANNs) can be employed as performance indicators to reduce SARS-CoV-2 in the treatment of sewage. At Shariati Hospital in Mashhad, Iran, secondary treatment effluent during the outbreak of COVID-19 was collected from a WWTP. There were two PC-Ag-NP and PC-HMO processes at the WWTP targeted. RT-qPCR was employed to detect the presence of SARS-CoV-2 in sewage fractions. For the purposes of determining SARS-CoV-2 prevalence rates in the treated effluent, 10 L of effluent specimens were collected in middle-risk and low-risk treatment MMMs. For PC-HMO, the log reduction value (LRV) for SARS-CoV-2 was 1.3-1 log10 for moderate risk and 0.96-1 log10 for low risk, whereas for PC-Ag-NP, the LRV was 0.99-1.3 log10 for moderate risk and 0.94-0.98 log10 for low risk.     Traumatic cervical spinal cord injury (TCSCI) often causes varying degrees of motor dysfunction, common assessed by the International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI), in association with the American Spinal Injury Association (ASIA) Impairment Scale. Accurate prediction of motor function recovery is extremely important for formulating effective diagnosis, therapeutic and rehabilitation programs. The aim of this study is to investigate the validity of a novel nested ensemble algorithm that uses the very early ASIA motor score (AMS) of ISNCSCI examination to predict motor function recovery 6 months after injury in TCSCI patients. This retrospective study included complete data of 315 TCSCI patients. The dataset consisting of the first AMS at ≤ 24 h post-injury and follow-up AMS at 6 months post-injury was divided into a training set (80%) and a test set (20%). The nested ensemble algorithm was established in a two-stage manner. Support Vector Classification (SVC), Adaboost, Weak-learner and Dummy were used in the first stage, and Adaboost was selected as second-stage model. The prediction results of the first stage models were uploaded into second-stage model to obtain the final prediction results. The model performance was evaluated using precision, recall, accuracy, F1 score, and confusion matrix. The nested ensemble algorithm was applied to predict motor function recovery of TCSCI, achieving an accuracy of 80.6%, a F1 score of 80.6%, and balancing sensitivity and specificity. The confusion matrix showed few false-negative rate, which has crucial practical implications for prognostic prediction of TCSCI. This novel nested ensemble algorithm, simply based on very early AMS, provides a useful tool for predicting motor function recovery 6 months after TCSCI, which is graded in gradients that progressively improve the accuracy and reliability of the prediction, demonstrating a strong potential of ensemble learning to personalize and optimize the rehabilitation and care of TCSCI patients.

    BACKGROUND: This multicenter, three-armed, parallel, single-blind randomized controlled trial (RCT) primarily aims to compare the efficacy of virtual reality exposure therapy (VRET) with that of acceptance and commitment therapy (ACT) and treatment as usual (TAU) to depreciate the degree of alcohol craving among alcohol use disorder patients who have undergone in-patient detoxification across four timelines (t0 = baseline prior to intervention, t1 = 4 weeks after baseline, t2 = 12 weeks after baseline, and t3 = 24 weeks after baseline). The secondary aims of this RCT are to compare the efficacy of VRET with that of ACT and TAU to alleviate the severity of alcohol use disorder, dissipate comorbid depressive and anxiety symptoms, and normalize event-related potential (ERP) in electroencephalogram (EEG) monitoring across the four timelines.

    METHODS: Initially, after 2 weeks of in-patient detoxification, 120 patients with alcohol use disorder will be randomized into three groups (VRET, ACT, and TAU control groups) via stratified permuted block randomization in a 1:1:1 ratio. Baseline assessment (t0) commences, whereby all the participants will be administered with sociodemographic, clinical, and alcohol use characteristics questionnaire, such as Alcohol Use Disorder Identification Test (AUDIT), Penn Alcohol Craving Scale (PACS), Hamilton Anxiety Rating Scale (HAM-A), and Hamilton Depression Rating Scale (HAM-D), while event-related potential (ERP) detection in electroencephalogram (EEG) will also be carried out. Then, 4 weeks of VRET, ACT, and non-therapeutic supportive activities will be conducted in the three respective groups. For the subsequent three assessment timelines (t1, t2, and t3), the alcohol use characteristic questionnaire, such as AUDIT, PACS, HAM-D, HAM-A, and ERP monitoring, will be re-administered to all participants.

    DISCUSSION: As data on the effects of non-pharmacological interventions, such as VRET and ACT, on the treatment of alcohol craving and preventing relapse in alcohol use disorder are lacking, this RCT fills the research gap by providing these important data to treating clinicians. If proven efficacious, the efficacy of VRET and ACT for the treatment of other substance use disorders should also be investigated in future.
