    Background: Pneumonia is defined as the inflammation of parenchyma of the lung. It is a substantial cause of morbidity and mortality in childhood throughout the world. The incidence of pneumonia in children under the age of five years is 0.29 episodes per child-year, which equates 151.8 million cases annually in developing countries. Objective: To determine the risk factors for complicated pneumonia.

    Material and Methods: This case-control study conducted in Medical Unit III, The Children’s Hospital & Institute of Child Health Lahore. Out of total of 180 cases of pneumonia, 100 were labeled as complicated pneumonia (case) and 80 were labeled as uncomplicated pneumonia (control). Complicated pneumonia included pneumonia with associated complications. Detail history was taken in both groups and recorded on predesigned proforma. Data was analyzed by SPSS 20. Quantitative risk factors like child age, maternal age and father age were analyzed by mean and standard deviation. However qualitative risk factors like method of feeding, malnutrition, immunization, anaemia, and non-vaccination were analyzed by applying chi-square test and finding odd ratios.

    It has been reported in the literature that the perception of crime at the neighbourhood level inversely affects the walking behaviour of individuals. On the other hand, the gated neighbourhoods are considered safe from crime, however, there is a lack of research on the association of the perception of crime and walking in gated neighbourhoods. Therefore, the objectives of the study reported in this paper were to investigate the association between the perception of crime and walking in gated and non-gated neighbourhoods. A questionnaire was used to collect the data on walking and the perception of crime in 16 neighbourhoods of Karachi Pakistan, 8 out of which were gated. Independent sample t-test and gamma tests were used for the data analysis. The results show that although there is a lower perception of crime in the gated neighbourhoods, yet the inhabitants of gated neighbourhoods engage in less physical activity comprising of walking. In spite of a greater perception of crime in non-gated neighbourhoods, higher values of walking were reported by the residents of those neighbourhoods. Therefore, it has been concluded that there does not exist a definite relationship between the perception of crime and walking behaviour at the neighbourhood level and the perceived safety from crime claimed by the proponents of the gated neighbourhoods does not encourage walking among the residents.

    This study accounts for the Hicks neutral technical change in a calibrated model of climate analysis, to identify the optimum level of technical change for addressing climate changes. It demonstrates the reduction to crop damages, the costs to technical change, and the net gains for the adoption of technical change for a climate-sensitive Pakistan economy. The calibrated model assesses the net gains of technical change for the overall economy and at the agriculture-specific level. The study finds that the gains of technical change are overwhelmingly higher than the costs across the agriculture subsectors. The gains and costs following technical change differ substantially for different crops. More importantly, the study finds a cost-effective optimal level of technical change that potentially reduces crop damages to a minimum possible level. The study therefore contends that the climate policy for Pakistan should consider the role of technical change in addressing climate impacts on the agriculture sector.

    The well-established emissions-growth debate relies on the symmetric nexus between CO2 emissions and economic growth, thereby ignoring a fundamental component of macro economy in the form of asymmetric relation. This paper considers how CO2 emissions respond asymmetrically to changes in economic growth. While utilizing both linear and nonlinear time series approaches for an environmentally exposed country, Pakistan over the period 1971-2018, we find convincing evidence that CO2 emissions rise more rapidly during negative shocks to economic growth than increase during economic expansions. Thus, contrary to what has previously been reported, the effect is strong as holds both at short run and long run. This is partly due to the increase in informal sector as GDP declines. Our estimated results show that accounting for the shadow economy results a higher magnitude of CO2 emissions due to decrease in economic growth, thus question the traditional symmetric decoupling of economic growth and CO2 emissions. The estimated results are robust to alternative estimators such as fully modified least squares (FMOLS) and dynamic OLS (DOLS). Thus, the findings of this study call for a re-thinking on climate policy design that rarely pays attention to the aforementioned outcomes due to fall in economic growth.

    This novel research is an argumentative subject which was needed to be addressed and to fill this gap, the author examined the effect of financial development, information and communication technology, and institutional quality on CO2 emission in Pakistan by using quantile autoregressive distributed lag (QARDL) model. The data were obtained for the period from 1995Q1 to 2018Q4. In the long run, GDP and institutional quality have a positive impact on CO2 emission when this emission is already high, which shows that if the GDP and institutional quality increases, the CO2 emission also increases. Moreover, financial development and ICT has a negative impact on CO2 emission irrespective of emission level that whether it is high or low in the country, which shows that if financial enhancement and ICT increases, carbon emission decreases. The study also supported the EKC hypothesis in Pakistan.

    This study examined the effect of air pollution on the initial return of IPOs in Pakistan. Cross-sectional data were used to examine 102 listed IPOs on Pakistan Stock Exchange between 1996 and 2019. Ordinary least squares and quantile least squares were employed to examine the influence of air pollution on IPO initial returns. Lastly, stepwise regression was utilised for additional analysis. According to the findings, in the presence of high air pollution, IPO initial returns also increase due to higher uncertainty. The findings demonstrate that air pollution intensifies a company's information environment and financial uncertainty. Therefore, addressing environmental challenges is critical for both public health and capital formation. This study's findings will increase companies' awareness of the economic effect of air pollution, particularly in a country where air pollution is strictly regulated. This study provides businesses with an economic reason to reduce their pollution levels, and it can also help regulators pass environmental laws that are aimed at addressing this issue.

    BACKGROUND: Precision medicine (PM) is in great progressive stages in the West and allows healthcare practitioners (HCPs) to give treatment according to the patient's genetic findings, physiological and environmental characteristics. PM is a relatively new treatment approach in Pakistan Therefore, it is important to investigate the level of awareness, attitude, and challenges faced by oncology physicians while practicing PM for various therapies, especially cancer treatment.

    OBJECTIVES: The present study aims to explore the level of awareness, attitude, and practice of PM in Pakistan along with the challenges faced by the oncologists for the treatment of cancer using the PM approach.

    METHODS: Phenomenology-based qualitative approach was used. Face-to-face in-depth interviews were conducted using the purposive sampling approach among oncologists in Lahore, Pakistan. The data were analyzed using thematic content analysis to identify themes and sub-themes.

    RESULTS: Out of 14 physicians interviewed 11 were aware of PM. They were keen on training to hone their skills and agreed on providing PM. Oncologists believed PM was expensive and given to affluent patients only. Other impeding factors include cost, lack of knowledge, and drug unavailability.

    CONCLUSIONS: Despite basic knowledge and will to practice, resource and cost constraints were marked as significant barriers. Additional training programs and inclusion into the curriculum may help to pave the way to PM implementation in the future. In addition, health authorities and policymakers need to ensure a cheaper PM treatment can be made available for all cancer patients.

    Present climate change consists of global warming that is caused by the emission of greenhouse gases, generally carbon dioxide. The study examines the pollution haven, pollution halo, and environmental Kuznets curve for a number of Asian countries during the period of 1985 to 2020. Outcomes suggest that urbanization, gross domestic product per capita, energy consumption, and foreign direct investment inflow have positive effects, while gross domestic product square, foreign direct investment square, and tourism have negative effects on emissions of carbon dioxide. Furthermore, findings support the validity of the environmental Kuznets curve, pollution haven, and pollution halo hypothesis for the selected Asian countries. We also find robust results of rationality of the environmental Kuznets curve hypothesis for Pakistan, Bangladesh, India, China, Indonesia, Korea, Japan, Malaysia, Vietnam, and Singapore; of pollution haven hypothesis for Bangladesh, China, Indonesia, Japan, Pakistan, and Singapore; and of pollution halo hypothesis for Bangladesh, China, Indonesia, Japan, Pakistan, and Singapore.

    Organic food has gained much importance due to consumers' rising environmental and health concerns. Purchase intention of organic food has been explored widely, but the repurchase intention of organic food has gained little attention among researchers. So, it has become important to explore repurchase intention among generation Z; a generation considered more educated and aware of rising environmental concerns. Generation Z is more tech-savvy and brand conscious, so its impact on repurchase intention through consumer satisfaction has been explored. The data in this paper was collected from 400 respondents through a structured questionnaire in Islamabad, Pakistan. We used the PLS-SEM approach for data analysis and results; we found that social media influence and brand purchase impact brand awareness and positively impact brand awareness on consumer satisfaction. Moreover, it is also found that consumer satisfaction positively impacts the repurchase intention of organic food. Our study found that Generation Z has a strong social media influence, so marketers' managers must consider and address the issues when consumers consider social media for their concerns and suggestion.

    INTRODUCTION: This study aimed to determine the predictors of quality of life (QOL) among persons with paraplegic spinal cord injury (SCI) after discharge from the hospital to the community in Pakistan, based on the International Classification of Functioning (ICF) components, including participation, impairments of body function/structures, personal factors, and environmental factors.

    MATERIALS AND METHODS: A cross-sectional study was conducted with, one hundred and forty individuals with paraplegic SCI, who met the inclusion and exclusion criteria and attended an outpatient rehabilitation clinic. The impairment of body function/structures of participants was assessed using the American Spinal Injury Association (ASIA) Scale, which classified them as A, B, C, D, or E. A set of questionnaire survey forms was used to collect sociodemographic information, occupational participation, environmental factors, and QOL by using a demographic questionnaire, World Health Organization Disability Assessment Schedule 2.0 (WHODAS-II), Craig Hospital Inventory of Environmental Factors (CHIEF) scale and World Health Organization Quality of Life (WHOQOL) BREF form respectively.

    RESULTS: The results showed that occupational participation was the strongest predictor of QOL among persons with paraplegic SCI (β=-0.586, p<0.001). In the second step, variables representing body function/structure factors (ASIA-A, B, C, D, E) were added, and the overall model explained 40.7% of the variance in QOL. In the third step, personal factors (age groups, gender, marital status, level of education, and rehabilitation duration) were added, and the overall model explained 51.4% of the variance in QOL. In the final step, environmental factors (CHIEF 12 Items scale) were added, but they did not significantly explain the model.

    CONCLUSION: The findings suggest that occupational participation was found to be the most significant predictor of QOL among individuals with paraplegic SCI. Body function/structure factors, personal factors, and environmental factors were also significant predictors, but to a lesser extent. The findings of this study can inform healthcare professionals and policymakers in developing interventions and, policies targeting occupational participation, and personal factors that may be effective to improve the QOL of individuals with paraplegic SCI in Pakistan.

    BACKGROUND: Ventilator associated pneumonia (VAP) rates in Asia are several times above those of US. The objective of this study is to identify VAP risk factors.

    METHODS: We conducted a prospective cohort study, between March 27, 2004 and November 2, 2022, in 279 ICUs of 95 hospitals in 44 cities in 9 Asian countries (China, India, Malaysia, Mongolia, Nepal, Pakistan, Philippines, Sri Lanka, Thailand, Vietnam).

    Introduction: Anxiety and depression in people living with HIV/AIDS (PLWHA) can lead to non-adherence to antiretroviral therapy (ART), morbidity, and mortality. Therefore, assessing the stigma, social support, and other determinants of anxiety and depression in PLWHA are important for developing further interventions. Methods: An institution-based cross-sectional study was conducted in 505 PLWHA, approached through systematic sampling, who paid routine visits to the ART center, Pakistan Institute of Medical Sciences (PIMS), Islamabad. Data was collected by pretested validated hospital anxiety and depression scale (HADS). Version 26 of the SPSS was used to apply Logistic regression analysis to identify determinants, and the 95% confidence interval (CI) adjusted odds ratio (AOR) was calculated to assess the magnitude of the relationships. Results: In PLWHA, the prevalence of co-morbid depression and anxiety was 80%. Separately, 89.9% had depression, and 80.3% had anxiety. Use of illicit drugs [AOR = 1.87, 95% CI (1.01, 3.27)], low social support [AOR = 1.21, 95% CI (1.02, 2.25)], being male [AOR = 2.21, 95% CI (1.11, 5.49)], and HIV related stigma [AOR = 2.48, 95% CI (1.25, 6.02)] were significant predictors of depression. Having detectable viral load [AOR = 3.04, 95% CI (1.04, 8.86)], young age [AOR = 5.31, 95% CI (1.19, 29.39)], no formal education [AOR = 21.78, 95% CI (4.03, 117.62)], low [AOR = 1.70, 95% CI (1.12, 6.93)] or moderate [AOR = 2.20, 95% CI (1.79, 6.09)] social support, illicit drugs addiction [AOR = 1.17, 95% CI (1.03, 2.55)], and HIV stigma [AOR = 54.3, 95% CI (21.20, 139.32)] had a remarkable association with anxiety. Conclusions: Given the high prevalence of anxiety and depression among PLWHA, the Pakistan Ministry of Health should focus more on monitoring mental health, expanding mental health services, and developing interventions based on identified factors to treat depression and anxiety among PLWHA.

    BACKGROUND: The pharmacist's role shifts from dispensing to bedside care, resulting in better patient health outcomes. Pharmacists in developed countries ensure rational drug use, improve clinical outcomes, and promote health status by working as part of a multidisciplinary team of healthcare professionals. However, clinical pharmacist services on healthcare utilization in low-and middle-income countries (LMICs) like Pakistan are unclear. As a result, we aim to systematically review pharmacists' clinical roles in improving Pakistani patients' therapeutic, safety, humanistic, and economic outcomes.

    METHODS: We searched PubMed, Scopus, EMBASE, CINAHL, and Cochrane Library for relevant articles published from inception to 28th February 2021. All authors were involved in the screening and selection of studies. Original studies investigating the therapeutic, humanistic, safety, and economic impact of clinical pharmacists in Pakistani patients (hospitalised or outpatients) were selected. Two reviewers independently assessed the risk of bias in studies, and discrepancies were resolved through mutual consensus. All of the included studies were descriptively synthesised, and PRISMA reporting guidelines were followed.

    RESULTS: The literature search found 751 articles from which nine studies were included; seven were randomized controlled trials (RCTs), and two were observational studies. Three RCTs included were having a low risk of bias (ROB), two RCTs were having an unclear ROB, while two RCTs were having a high ROB. The nature of clinical pharmacist interventions included one or more components such as disease-related education, lifestyle changes, medication adherence counselling, medication therapy management, and discussions with physicians about prescription modification if necessary. Clinical pharmacist interventions reduce medication-related errors, improve therapeutic outcomes such as blood pressure, glycemic control, lipid control, CD4 T lymphocytes, and renal functions, and improve humanistic outcomes such as patient knowledge, adherence, and health-related quality of life. However, no study reported the economic outcomes of interventions.

    CONCLUSIONS: The findings of the studies included in this systematic review suggest that clinical pharmacists play important roles in improving patients' health outcomes in Pakistan; however, it should be noted that the majority of the studies have a high risk of bias, and more research with appropriate study designs is needed.

    OBJECTIVES: Strengthening of emergency care systems, including prehospital systems, can reduce death and disability. We aimed to identify perspectives on barriers and facilitators relating to the development and implementation of a prehospital emergency care system assessment tool (PEC-SET) from prehospital providers representing several South and Southeast (SE) Asian countries.

    DESIGN: We conducted a qualitative study using focus group discussions (FGD) informed by the Consolidated Framework for Implementation Research (CFIR). FGDs were conducted in English, audioconferencing/videoconferencing was recorded, transcribed verbatim and coded using an inductive and deductive approach. Participants suggested specific elements to be measured within three main 'pillars' of disease conditions proposed by the research team of the tool being developed (cardiovascular, trauma and perinatal emergencies).

    SETTING: We explored the perspectives of medical directors in six low-income and middle-income countries (LMICs) in South and SE Asia.

    PARTICIPANTS: A total of 16 participants were interviewed (1 Vietnam, 4 Philippines, 4 Thailand, 5 Malaysia, 1 Indonesia and 1 Pakistan) as a part of 4 focus groups.

    RESULTS: Themes identified within the four CFIR constructs included: (1) Intervention characteristics: importance of developing an contextually specific tool, need for generalisability, trialling in one geographical area or with one pillar before expanding; (2) Inner setting: data transfer barriers, workforce shortages; (3) Outer setting: underdevelopment of EMS nationally; need for further EMS system development prior to implementing a tool and (4) Individual characteristics: lack of buy-in by prehospital personnel. Elements proposed by participants included both process and outcome measures.

    CONCLUSIONS: Through the CFIR framework, we identified several themes which can provide a basis for codeveloping a PEC-SET for LMICs with local stakeholders. This work may inform development of quality improvement tools in LMIC PEC systems.

    Snakebite envenoming (SBE) is a priority Neglected Tropical Disease listed by the World Health Organization. South Asia is heavily affected, and virtually all countries in the region import polyvalent antivenom products from India for clinical use. The imported antivenoms, however, have suboptimal effectiveness due to geographical venom variation. Recently, a domestic bivalent product, named Pakistani Viper Antivenom (PVAV) has been developed specifically for Pakistani vipers, Echis carinatus sochureki and Daboia russelii. As a bivalent viperid antivenom, it is unknown yet if PVAV exhibits higher immunological binding and neutralization activities against viper venoms from distant locales compared with polyvalent antivenoms manufactured in India. This study thus examined the preclinical efficacy of PVAV against venoms of Western Russell's Vipers and Saw-scaled Viper subspecies from selected locales in the Indian subcontinent. PVAV generally outperformed the commonly used VINS polyvalent antivenom (VPAV, manufactured in India) in binding toward venoms, and showed superior or comparable neutralization efficacy against the venom procoagulant and hemorrhagic effects of Saw-scaled Vipers as well as Russell's Vipers from Pakistan and Sri Lanka. Based on normalized potency values, PVAV is far more potent than VPAV in neutralizing the lethality of all viper venoms, except that of the Indian Russell's Viper. The study shows conserved antigenicity of toxins responsible for major toxicity across these viperid venoms, and suggests the feasible production of a viper-specific antivenom with higher potency and broader geographical utility for the region.

    The phytosociological survey was conducted during 2018-2020. The research area was classified into five ecological zones based on habitat, physiognomy and species composition. Pc-Ord software was used for cluster analysis and four vegetation communities were established. The Quercus baloot-Quercus incana community is situated in Sair at an altitude of 1196 (mean ± SE) m altitude with a 14.1 ± 0 slope angle and contains eleven tree species. The Pinus wallichiana- Ailanthus altissima community had a relatively small number of tree species reported in Shakawlie at 1556 (mean ± SE) with a 17.5 ± 0 slope angle. The Pinus wallichiana- Quercus incana community is distributed in Wali Kandao and Mangi Kandao at altitudes of 2030.5 (mean ± SE) m and the slope angle was 19.2 ± 1.4. This community possesses a total of twenty-one tree species and is highly diverse. Similarly, the Populus alba - Platanus orientalis group was present in Banr Pate, with an altitude of 1613 (mean ± SE) m and a 16.3 slope angle. The principal component analysis (PCA) and non-metric multidimensional scaling (NMS) ordination methods were applied to study the relationships between ecological and soil variables with trees species. The NMS ordination of axis 1 was significantly correlated with Sand% (p<0.2), Nitrogen% (p<0.1) and Pb (mg/kg) (r= 0.876751, p<0.05), while the ordination of axis 2 was significantly correlated with Silt% (p<0.2), Sand% (p<0.2), Organic matter% (p<0.2), K (mg/kg) (r=0.882433, p<0.02), Fe (mg/kg)(r=0.614833, p<0.2), Ca (mg/kg) (r=0.721712, p< 0.2) and Zn (mg/kg) (r=0.609545, p<0.2). Similarly, the PCA ordination of axis 1 revealed that it was significantly correlated with phosphorus, calcium and slope angle, while that of axis 2 was significantly correlated with altitude, zinc and manganese.

    BACKGROUND: Asia consists of diverse nations with extremely variable health care systems. Integrated real-world data (RWD) research warehouses provide vast interconnected data sets that uphold statistical rigor. Yet, their intricate details remain underexplored, restricting their broader applications.

    OBJECTIVE: Building on our previous research that analyzed integrated RWD warehouses in India, Thailand, and Taiwan, this study extends the research to 7 distinct health care systems: Hong Kong, Indonesia, Malaysia, Pakistan, the Philippines, Singapore, and Vietnam. We aimed to map the evolving landscape of RWD, preferences for methodologies, and database use and archetype the health systems based on existing intrinsic capability for RWD generation.

    METHODS: A systematic scoping review methodology was used, centering on contemporary English literature on PubMed (search date: May 9, 2023). Rigorous screening as defined by eligibility criteria identified RWD studies from multiple health care facilities in at least 1 of the 7 target Asian nations. Point estimates and their associated errors were determined for the data collected from eligible studies.

    RESULTS: Of the 1483 real-world evidence citations identified on May 9, 2023, a total of 369 (24.9%) fulfilled the requirements for data extraction and subsequent analysis. Singapore, Hong Kong, and Malaysia contributed to ≥100 publications, with each country marked by a higher proportion of single-country studies at 51% (80/157), 66.2% (86/130), and 50% (50/100), respectively, and were classified as solo scholars. Indonesia, Pakistan, Vietnam, and the Philippines had fewer publications and a higher proportion of cross-country collaboration studies (CCCSs) at 79% (26/33), 58% (18/31), 74% (20/27), and 86% (19/22), respectively, and were classified as global collaborators. Collaboration with countries outside the 7 target nations appeared in 84.2% to 97.7% of the CCCSs of each nation. Among target nations, Singapore and Malaysia emerged as preferred research partners for other nations. From 2018 to 2023, most nations showed an increasing trend in study numbers, with Vietnam (24.5%) and Pakistan (21.2%) leading the growth; the only exception was the Philippines, which declined by -14.5%. Clinical registry databases were predominant across all CCCSs from every target nation. For single-country studies, Indonesia, Malaysia, and the Philippines favored clinical registries; Singapore had a balanced use of clinical registries and electronic medical or health records, whereas Hong Kong, Pakistan, and Vietnam leaned toward electronic medical or health records. Overall, 89.9% (310/345) of the studies took >2 years from completion to publication.

    CONCLUSIONS: The observed variations in contemporary RWD publications across the 7 nations in Asia exemplify distinct research landscapes across nations that are partially explained by their diverse economic, clinical, and research settings. Nevertheless, recognizing these variations is pivotal for fostering tailored, synergistic strategies that amplify RWD's potential in guiding future health care research and policy decisions.

    The wild edible plants offer a valuable resource for food and human nutrition. The current study aimed to analyze the proximate composition and ethno-medicinal profiles of twenty selected wild edible plants from the Malakand District, Pakistan. The proximate composition (moisture, ash, crude protein, crude fat, and dietary fiber) was analyzed following Association of Official Agricultural Chemists (AOAC) protocols. Ethno-medicinal profiles were obtained through semi-structured interviews with local informants. Mineral elements were analyzed using Energy Dispersive X-ray Fluorescence (EDX) Spectroscopy. The normalized variation matrix and centered log ratio (CLR) biplot were used to assess linear associations between nutritional components. The results revealed significant variations in the nutritional components and mineral contents among the studied plant species. Higher crude carbohydrates (64.48%) were scrutinized in Ziziphus jujuba, followed by proteins (19.90%) in leaves of Solanum nigrum, fiber (19.50%) in Caralluma tuberculata and fats (6.12%) in Mentha spicata. A maximum energy value of 332.34 Kcal/100g was calculated for leaves of Solanum nigrum. The concentrations of carbon (C) in Mentha longifolia were (60.20%), followed by oxygen (46.06%) in Ziziphus jujuba, potassium (10.80%) in Rumex dentatusand nitrogen (7.98%) in Nasturtium officinale. The centered log ratio biplot confirms strong relationships between moisture, energy, carbohydrates, fibers, and ash content, while protein and fat concentrations exhibit a separate pattern. The findings provide valuable insights into the compositional data analysis of edible plant species, suggesting interdependencies among carbohydrate, energy, moisture content, and fibers. Additionally, the results of our current study showed that all of these selected wild edible plants have the potential to provide humans with important macronutrients and elements.

    With the ongoing digital transformation, the internet, mobile, social media and other computer technologies are being increasingly used in the targeting, recruitment, transportation and exploitation of human trafficking victims. The current study is the first of its kind which uses a qualitative method to comprehensively investigate the role of technology in the lived experiences of human trafficking victims in Pakistan. This qualitative study was carried out with a phenomenological approach in two provinces of Pakistan. Data was collected using in-depth interviews with 13 victims who were selected using purposive and snowball sampling methods. The data was then analyzed using Braun and Clarke's thematic analysis approach. The data analysis results were divided into four main themes and ten sub-themes. The main themes are: Recruitment approaches, transportation process, exploitation process, and mental health consequences. The analysis show that traffickers extensively use technology across all phases of human trafficking. This study aims to support a range of relevant institutional stakeholders, including law enforcement agencies, legislative bodies, policymakers, civil society and SDG (2030) goals, in Pakistan and globally, in their efforts to address and enhance the learning and awareness level regarding related technologies in order to improve investigation in combating technological based human trafficking.
