    The future hospital is a resilient, physical learning facility featuring digital enhancement and leveraging an ecosystem of platforms for the Internet of Things (IoT) and analytics, achieving patient-centric care delivery via multidisciplinary healthcare provider teams coordinated to meet patients' medical, psychological, social and economic needs. It exists in a just ecosystem that assimilates the care spectrum from healthy living, the prevention of disease to acute care and the rehabilitation of patients recuperating from illnesses. It will take some time for these future hospitals to be built or for current hospitals to evolve and/or transform, but efforts to spread wisdom among the stakeholders, healthcare providers and patients must start now. The development of the digital components can also begin today, as can competency building for the healthcare providers who will be staffing these future hospitals, ensuring that they are equipped with competent staff employing patient-centric care processes that cater to patients' current and future needs.

    In this paper, a Semantic Web service matchmaker called UltiMatch-NL is presented. UltiMatch-NL applies two filters namely Signature-based and Description-based on different abstraction levels of a service profile to achieve more accurate results. More specifically, the proposed filters rely on semantic knowledge to extract the similarity between a given pair of service descriptions. Thus it is a further step towards fully automated Web service discovery via making this process more semantic-aware. In addition, a new technique is proposed to weight and combine the results of different filters of UltiMatch-NL, automatically. Moreover, an innovative approach is introduced to predict the relevance of requests and Web services and eliminate the need for setting a threshold value of similarity. In order to evaluate UltiMatch-NL, the repository of OWLS-TC is used. The performance evaluation based on standard measures from the information retrieval field shows that semantic matching of OWL-S services can be significantly improved by incorporating designed matching filters.

    Expert system is a system that emulates experts to aid in decision making. This system can be applied in various categories such as diagnosis, prediction, interpretation, and others. Expert System to Diagnose Dengue Fever is a web-based system which is integrated with prolog language in order to provide rules for dengue fever detection. The aims of this research are to study dengue fever symptoms and other illnesses related to the fever, to design an inference engine, and to build an expert system. The challenges faced while developing this expert system were the complexity of prolog codes and their integration with the web development. In this system, rules were developed by prolog language which define dengue fever and accuracy based on input from the user. This system is expected to aid users in self-detecting early symptoms of dengue fever before seeing the doctors.

    A literate and numerate population is the goal of any modern industrialized society. Literacy and mathematics skills carry the means by which children are equipped for the education processes on which their future will depend. Deaf and hard of hearing students' reading and mathematics skills are lower than that of others due to their inability. Before enhancing their literacy and mathematics skills, their standard of literacy and mathematics skills should first be identified. For this reason, the Malaysian Ministry of Education initiated the Literacy and Numeracy Screening (LINUS) program in 2009. However, problems arose in the assessment method of LINUS screening for these students since the LINUS screening method does not accommodate these students' situation and needs. Therefore, the researchers introduced internet-based Literacy and Mathematics Assessment (iLiMA) prototype that can overcome those problems. In the iLiMA prototype, sign language instruction video is used to standardize the assessment method in order to ensure that non-bias assessment could be established. The methodology used to develop this system is the Evolutionary Process Model - Prototype. The iLiMA prototype usability was assessed with the Computer System Usability Questionnaire (CSUQ) and conducted by using web-based survey method. The results indicate that the iLiMA prototype is usable and teachers are satisfied with it. Finally, the iLiMA prototype which had the potential to accommodate deaf and hard of hearing students to get a standardized and non-bias literacy and mathematics assessment was developed.

    Breast Self-Examination (BSE) is a process whereby women examine their breasts regularly to detect any abnormal swelling or lumps in order to seek prompt medical attention. The purpose of this study was to investigate the knowledge and practice of BSE among female non-medical students in UKM, Bangi. A cross-sectional study was conducted using self-administered and validated questionnaire among 364 students. The results showed that only 37.1% performs BSE and 45% of the students have good knowledge. Age, marital status, using internet and pamphlets as source of information, personal, and family history of BC, were significantly associated with knowledge level of students. Practice of BSE was significantly associated with knowledge level. From multivariate analysis, BSE was more likely to be done among students with family history, students who using internet and pamphlet as sources of information, also among students with good knowledge. As conclusions, the practice of BSE is inadequate among the respondents in spite of most of them had heard about BSE. The results suggest the need of providing continuing educational programs to increase the knowledge level on BSE which in turn will have a positive effect on students to practice and motivate to perform BSE.

    Lack of parking space has become major contributing factor to traffic congestion
    which may affect the environmental condition due to incalculable amounts of wasted
    fuel and carbon emission. Congestion and parking are interrelated as looking for
    parking space may create additional delay and impair local circulation. The proposed
    automatic parking system which embedded with radio frequency identification (RFID)
    and internet of things (IoTs) module will introduce a monitoring platform for the
    parking space. This intelligent parking system makes it possible for the drivers to
    obtain information beforehand through their mobile hand phone application or any
    display available. This paper describes the mechanical design of automatic indoor
    parking space with the advancement of sensor networks application known as RFID.
    The design criterias were studied to develop a parking prototype which dealing with
    number of degree of freedom as well as movement mechanism. The result presented
    at the end of this paper was evaluated from the engineering analysis using the
    developed parking prototype to meet system performance as well as design safety
    factor.

    OBJECTIVE: Thalassaemia is the most common inherited blood disorder in Malaysia. This study aims to report the current status of thalassaemia in Malaysia and provide a comprehensive understanding of the disease through data obtained from the Malaysian Thalassaemia Registry.

    DESIGN: Data were extracted from the Malaysian Thalassaemia Registry, a web-based system accessible to enrolled users through www.mytalasemia.net.my.

    SETTING: The Malaysian Thalassaemia Registry data was recorded from reports obtained from 110 participating government and university hospitals in Malaysia.

    PARTICIPANTS: The patients were those attending the 110 participating hospitals for thalassaemia treatment.

    INTERVENTION: Data were collected from the Malaysian Thalassaemia Registry from 2007 until the fourth quarter of 2018.

    PRIMARY OUTCOME MEASURE: 7984 out of 8681 patients with thalassaemia registered in the Malaysian Thalassaemia Registry were reported alive.

    RESULTS: Majority of the patients were reported in the state of Sabah (22.72%); the largest age group affected was 5.0-24.9 years old (64.45%); the largest ethnic group involved was Malay (63.95%); and the major diagnosis was haemoglobin E/β-thalassaemia (34.37%). From the 7984 patients, 56.73% were on regular blood transfusions and 61.72% were on chelation therapy. A small fraction (14.23%) has undergone splenectomy, while the percentage of patients with severe iron overload (serum ferritin ≥5000 µg/L) reduced over time. However, cardiac complications are still the main cause of death in patients with thalassaemia.

    CONCLUSION: Data gathered into the registry can be used to understand the progression of the disorder, to monitor iron overload management and to improve the outcomes of treatment, to enhance preventive strategies, reduce healthcare burden and improve the quality of life. Sustainability of the Malaysian Thalassaemia Registry is important for surveillance of thalassaemia management in the country and help the national health authorities to develop more effective policies.

    BACKGROUND: In Malaysia, the Sugar-Sweetened Beverages (SSBs) tax was announced during the parliament's 2019 Budget Speech. The tax was slated to be enforced by April 2019 but was later postponed to July 2019. The announcement has since generated significant media coverage and public feedback.

    OBJECTIVE: This study presents a qualitative and quantitative cross-sectional study using netnography to examine how Malaysian online news articles responded to the SSBs tax after the announcement and postimplementation.

    METHODS: Online news articles published on popular online news platforms from November 2018 to August 2019 were downloaded using NCapture and imported into NVivo for analysis using the inductive approach and thematic content analysis following the initial SSBs implementation announcement.

    RESULTS: A total of 62 news articles were analyzed. Most of the articles positively portrayed the SSBs tax (46.8%) and highlighted its health impacts (76%). There were 7 key framing arguments identified in the articles. The positive arguments revolved around incentivizing manufacturers to introduce healthier products voluntarily, positive health consequences, the tax's impact on government revenue, and the use of the generated revenue toward beneficial social programs. The opposing arguments included increased operating costs to the manufacturer, the increased retail price of drinks, and how the SSBs tax is not a robust solution to obesity. The top priority sector considered in introducing the tax was the health perspective, followed by economic purposes and creating policies such as regulating the food and drinks industry.

    CONCLUSIONS: The majority of online news articles positively reported the implementation of the SSBs tax in Malaysia. This suggests media played a role in garnering support for the health policy. As such, relevant bodies can use negative findings to anticipate and reframe counteracting arguments opposing the SSBs tax.

    BACKGROUND: Vaccine hesitancy is a threat in combating vaccine-preventable diseases. It has been studied extensively in the Western countries but not so among Asian countries.

    OBJECTIVES: To assess the test-retest reliability of the Parent Attitudes about Childhood Vaccines (PACV) questionnaire in Malay language; to determine the prevalence of vaccine hesitancy among parents and its associations with parents' socio-demographic characteristics.

    METHODS: Forward and backward translation of PACV in Malay language was carried out. The reliability of the Malay-PACV questionnaire was tested among parents with children. The same questionnaire was used to study vaccine hesitancy among parents in a tertiary hospital in Kuala Lumpur. Information pertaining to socio-demographic characteristics, sources of information regarding vaccination and vaccine hesitancy were collected. Associations between vaccine hesitancy with socio-demographic factors were tested using Multivariable Logistic Regression.

    RESULTS: The Spearman correlation coefficient and Cronbach alpha for total PACV was 0.79 (p<0.001) and 0.79 respectively. The intra-class correlation coefficients of the subscales ranged from 0.54 to 0.90 demonstrating fair to excellent reliability. A total of 63 (11.6%) parents were noted to be vaccine hesitant. In the univariate analyses, vaccine hesitancy was associated with unemployed parents, parents who were younger, had fewer children and non-Muslim. In the multivariate model, pregnant mothers expecting their first child were four times more likely to be vaccine hesitant compared to those who already had one or more children (aOR: 3.91, 95% CI: 1.74-8.79) and unemployed parents were also more likely to be vaccine hesitant (aOR: 1.97, 95% CI: 1.08-3.59). The internet (65.6%) was the main source of information on vaccination followed by brochures (56.9%).

    CONCLUSION: The Malay-PACV questionnaire is reliable to be used. The prevalence of vaccine hesitancy among the multi-ethnic Malaysians was comparable with other populations. Pregnant mothers expecting their first child and unemployed parents were found to be more vaccine hesitant.

    The importance of incorporating medical (or health) informatics into the education of medical students and medical practitioners is being increasingly recognised. The advances in information and communication technology and the pervasion of the Internet into everyday life have important implications for healthcare services and medical education. Students and practitioners should learn to utilise biomedical information for problem solving and decision making based on evidence. The extensive introduction of electronic health information systems into hospitals and clinics and at the enterprise level in Malaysia and elsewhere is driving a demand for health professionals who have at least basic skills in and appreciation of the use of these technologies. The essential clinical informatics skills have been identified and should be incorporated into the undergraduate medical curriculum. It is recommended that these be introduced in stages and integrated into existing programmes rather than taught as a separate module. At the same time, medical schools should support the integration of e-learning in the educational process in view of the numerous potential benefits.

    Web 2.0 technologies such as wikis, podcasts/vodcasting, blogs and semantic portals could be quite effective tools in e-learning for health professionals. If effectively deployed, such tools can offer a way to enhance students', clinicians' and patients' learning experiences, and deepens levels of learners' engagement and collaboration within medical learning environments. However, Web 2.0 requires simplicity of use as well as integration with modern web technologies. This article presents a Web 2.0 telemedical portal, which provides a social community-learning paradigm from the desk of the physician, the student, the hospital administrator, or the insurer. The presented portal utilises RESTful web services and techniques like content syndication, mushups and Asynchronous JavaScript API and XML (AJAX). The designed portal is based on the Apache Cocoon RESTful framework for sharing Digital Imaging and Communications in Medicine (DICOM) medical case studies. Central to this article is the integration between Cocoon and AJAX. The proposed AJAX-Cocoon portal utilises a JSP portlet architecture, which manages the interaction dynamics and overcomes the shortcomings of the JSR 168 and WSRP 1.0 standards.

Even though, IoT provides the effective protocols for maintaining the information, several intermediate attacks and intruders trying to access the health information which in turn reduce the privacy, security and reliability of the entire health care system in internet environment. As a result and to solve the issues, in this research Learning based Deep-Q-Networks has been introduced for reducing the malware attacks while managing the health information. This method examines the medical information in different layers according to the Q-learning concept which helps to minimize the intermediate attacks with less complexity. The efficiency of the system has been evaluated with the help of experimental results and discussions.

    Internet Gaming Disorder (IGD) has been placed under the conditions for further study segment in DSM-5. The purpose of the current study was to develop a preliminary psychosocial model as a reference for providing appropriate intervention, particularly for adolescents with IGD. A total of 5290 adolescents from secondary schools in seven states in Malaysia were recruited by using proportionate random sampling. A standardized set of validated questionnaires such as DASS-21, BIS-11, and IGDS9-SF were distributed to participants that fulfilled the inclusion criteria. The prevalence of IGD among Malaysian adolescents was 3.5%. The bivariate analysis summarized that factors such as stress, impulsivity, gender, time spent using the internet, and relationship with parents and friends; have statistically significant associations with IGD. The logistic regression model revealed that adolescents with IGD were 9 times more likely to experience extremely severe stress (p < 0.001). Several psychosocial factors were associated strongly with IGD in the current study, however, mental health shows the most significant issues among adolescents with IGD. Immediate intervention through a psychological approach to internet gaming is needed from parents, schools, and also respective stakeholders. IGD may become one of the addictions diseases that cause deterioration in many aspects of an adolescent's future life without serious intervention.

    The Internet has opened pathways for youth to find sexual health information which was not easily available to them in the past. Studies have shown that seeking sexual health information online may potentially influence an individuals' decision-making to change their sexual health behaviours. However, there is a gap in research on the associations of seeking online sexually transmitted infection (STI) information with STI prevention among young people, particularly in Malaysia. This study investigated the associations of seeking STI information online with the intentions of condom use among young adult online users in Malaysia. A cross-sectional online survey was conducted among Malaysian youth aged 18-25 years old who were recruited through Facebook. In total, 1530 respondents completed the survey, identifying 874 respondents who had sought STI information online. The majority of respondents had intentions to use condoms as protection against STI. Respondents who sought online STI information were significantly more likely to have the intention to use condoms compared to respondents who did not seek online STI information (OR = 1.33, 95% CI = 1.01-1.76, p = 0.040). Online STI information has the potential to increase access to STI information among young online users in Malaysia amid the stigma surrounding sex-related issues. Providing accurate STI information online from reliable sources may equip young people who have Internet access with awareness of sexual health protective behaviours such as condom use. It is recommended to facilitate Internet access to reach sectors of the population that currently do not have access, as the Internet is useful in STI prevention.

    Background The Internet is an important resource of information about sexually transmissible infections (STIs) among young people. Little is known of the facilitators of online STI information seeking. This study aimed to identify the factors associated with online STI information seeking among youth in Malaysia.

    METHODS: A cross-sectional online survey was conducted among Malaysian youth aged 18-25 years who were recruited through social media.

    RESULTS: Among 1530 respondents who completed the survey, 57.1% sought online STI information in the past 12 months mostly from general Internet websites, health websites, and Facebook. Respondents' overall STI knowledge was low. Young people who had higher STI knowledge (OR = 2.47, 95% CI = 1.87-3.25, P = 0.000), had intention-to-seek online STI information (OR = 2.02, 95% CI = 1.23-3.30, P = 0.000), identified as homosexual or bisexual (OR = 1.67, 95% CI = 1.08-2.57, P = 0.020), experienced STI symptoms (OR = 1.51, 95%CI = 1.02-2.22, P = 0.040), were in a relationship (OR = 1.42, 95% CI = 1.08-1.86, P = 0.012) and had high perceived quality of online STI information (OR = 1.41, 95%CI = 1.09-1.83, P = 0.009) were more likely to seek online STI information.

    CONCLUSIONS: Findings suggest the need to increase STI knowledge and perceived quality of online STI information among Malaysian youth. Young people with low STI knowledge, who are heterosexual, sexually active without STI symptoms and single would benefit from reliable sources of online STI information for STI prevention.

    Objective: This study aims to investigate the public pattern in seeking breast cancer screening information in
    Malaysia using Google Trends. Methods: The Google Trends database was evaluated for the relative Internet search
    popularity of breast cancer and screening-related search terms from 2007 to 2018. Results: Result showed downward
    trends in breast cancer search, whereas mammogram and tomosynthesis search fluctuated consistently. A significant
    increment was found during Pink October month. Breast cancer search term achieved the highest popularity in the east
    coast of Malaysia with [x2 (5, N=661) = 110.93, P<0.05], whereas mammogram attained the highest search volume in
    central Malaysia [x2 (4, N=67) = 18.90, P<0.05]. The cross-correlation for breast cancer was moderate among northern
    Malaysia, Sabah, and Sarawak (0.3 ≤ rs ≤ 0.7). Conclusion: Public interest trend in breast cancer screening is strongly
    correlated with the breast cancer awareness campaign, Pink October. Breast cancer screening should be promoted in
    the rural areas in Malaysia.

    BACKGROUND: The impact of web-based formative assessment practices on performance of undergraduate medical students in summative assessments is not widely studied. This study was conducted among third-year undergraduate medical students of a designated university in Malaysia to compare the effect, on performance in summative assessment, of repeated computer-based formative assessment with automated feedback with that of single paper-based formative assessment with face-to face feedback.
    METHODS: This quasi-randomized trial was conducted among two groups of undergraduate medical students who were selected by stratified random technique from a cohort undertaking the Musculoskeletal module. The control group C (n = 102) was subjected to a paper-based formative MCQ test. The experimental group E (n = 65) was provided three online formative MCQ tests with automated feedback. The summative MCQ test scores for both these groups were collected after the completion of the module.
    RESULTS: In this study, no significant difference was observed between the mean summative scores of the two groups. However, Band 1 students from group E with higher entry qualification showed higher mean score in the summative assessment. A trivial, but significant and positive correlation (r(2) = +0.328) was observed between the online formative test scores and summative assessment scores of group E. The proportionate increase of performance in group E was found to be almost double than group C.
    CONCLUSION: The use of computer based formative test with automated feedback improved the performance of the students with better academic background in the summative assessment. Computer-based formative test can be explored as an optional addition to the curriculum of pre-clinical integrated medical program to improve the performance of the students with higher academic ability.

    The need for cloud services has been raised globally to provide a platform for healthcare providers to efficiently manage their citizens' health records and thus provide treatment remotely. In Iraq, the healthcare records of public hospitals are increasing progressively with poor digital management. While recent works indicate cloud computing as a platform for all sectors globally, a lack of empirical evidence demands a comprehensive investigation to identify the significant factors that influence the utilization of cloud health computing. Here we provide a cost-effective, modular, and computationally efficient model of utilizing cloud computing based on the organization theory and the theory of reasoned action perspectives. A total of 105 key informant data were further analyzed. The partial least square structural equation modeling was used for data analysis to explore the effect of organizational structure variables on healthcare information technicians' behaviors to utilize cloud services. Empirical results revealed that Internet networks, software modularity, hardware modularity, and training availability significantly influence information technicians' behavioral control and confirmation. Furthermore, these factors positively impacted their utilization of cloud systems, while behavioral control had no significant effect. The importance-performance map analysis further confirms that these factors exhibit high importance in shaping user utilization. Our findings can provide a comprehensive and unified guide to policymakers in the healthcare industry by focusing on the significant factors in organizational and behavioral contexts to engage health information technicians in the development and implementation phases.

    The IntFOLD server provides a unified resource for the automated prediction of: protein tertiary structures with built-in estimates of model accuracy (EMA), protein structural domain boundaries, natively unstructured or disordered regions in proteins, and protein-ligand interactions. The component methods have been independently evaluated via the successive blind CASP experiments and the continual CAMEO benchmarking project. The IntFOLD server has established its ranking as one of the best performing publicly available servers, based on independent official evaluation metrics. Here, we describe significant updates to the server back end, where we have focused on performance improvements in tertiary structure predictions, in terms of global 3D model quality and accuracy self-estimates (ASE), which we achieve using our newly improved ModFOLD7_rank algorithm. We also report on various upgrades to the front end including: a streamlined submission process, enhanced visualization of models, new confidence scores for ranking, and links for accessing all annotated model data. Furthermore, we now include an option for users to submit selected models for further refinement via convenient push buttons.     The revolution generated by the Internet of Things (IoT) has radically changed the world; countless objects with remote sensing, actuation, analysis and sharing capabilities are interconnected over heterogeneous communication networks. Consequently, all of today's devices can connect to the internet and can provide valuable information for decision making. However, the data collected by different devices are in different formats, which makes it necessary to develop a solution that integrates comprehensive semantic tools to represent, integrate and acquire knowledge, which is a major challenge for IoT environments. The proposed solution addresses this challenge by using IoT semantic data to reason about actionable knowledge, combining next-generation semantic technologies and artificial intelligence through a set of cognitive components that enables easy interoperability and integration for both legacy systems and emerging technologies, such as IoT, to generate business value in terms of faster analytics and improved decision making. Thus, combining IoT environments with cognitive artificial intelligence services, COSIBAS builds an abstraction layer between existing platforms for IoT and AI technologies to enable cognitive solutions and increase interoperability across multiple domains. The resulting low-cost cross platform supports scalability and the evolution of large-scale heterogeneous systems and allows the modernization of legacy infrastructures with cognitive tools and communication mechanisms while reusing assets.
