    The aim of the study was to determine the effect of pro-phylactic low dose dopamine infusion on renal function in ventilated premature newborns with respiratory dis-tress syndrome (RDS). A prospective, randomised con-trolled trial was conducted, using low dose dopamine [2.5μg/kg/min] in the treatment of preterm babies with gestational age 28-36 weeks requiring mechanical ventilation for RDS within six hours of age. Thirty-six babies were enrolled and 19 babies were randomly assigned to the treatment groups. The renal function after 72 hours for the treatment and control groups respectively were: urine output (ml/kg/hour) 3.3±0.4 and 3.0±0.3 [p=0.55], urine specific gravity 1006±0.6 and 1006±1.0 [p=0.68], fractional excretion of sodium 4.1±0.8 and 2.6±0.4 [p=0.10], fractional excretion of potassium 37.44 ± 5.6 and 16.49 ± 2.2 [p=0.001], glomerular filtration rate (ml/day/1.72m2) 16±2.6 and 25.6±4.5 [p=0.06]. There were no significant differ-ences in the frequency of hypotension, oliguria and sep-sis between the two groups. There were seven deaths (36.8%) in the treatment group (six due to sepsis and one due to prematurity) and two deaths (11.8%) in the control group (both due to sepsis) (p = 0.13). In con-clusion prophylactic low-dose dopamine infusion did not improve the renal function in ventilated premature babies with respiratory distress syndrome. The results of this study do not support the routine use of prophylac-tic low-dose dopamine in ventilated preterm babies with respiratory distress syndrome.

    Objective: An in vitro assessment of MG-63 human osteosarcoma cells' alkaline phosphatase (ALP) activity when in contact with calcium hydroxide powder (CH), paste (CHP) and grey mineral trioxide aggregate (MTA). Methods: MG-63 cells were seeded to the three selected materials for durations of 0.25, 0.5, 1, 24, 48 and 72 hours. BCIP-NBT assay was used and ALP activity quantified using ELISA reader at 410 nm. Results: The overall analysis for ALP activity indicated significant interaction between test materials and control (maintenance medium). Subsequently, the test materials were paired and analysed for initial (0.25, 0.5, 1 hour) and delayed response (24, 48 and 72 hours). During the initial response, CH exhibited an increased ALP activity compared to MTA. This interaction was not dependant on duration. The delayed response exhibited elevated ALP activity with CHP when compared to MTA and CH. The interaction of CHP was dependant on duration. Conclusion: All three materials exhibited increased ALP activity.

    Crowdsourcing has changed the way people conduct business. It provides access to work, and employers can source for the best talent, at the best price, with the shortest turnaround time. Research so far has focussed on crowdsourcing implementation. Hence, there is a need to conduct research that can contribute towards crowdsourcing sustainability. Thus, the objectives of this paper are to identify current practices of crowdsourcing in Malaysia and the challenges that face it. A conceptual model for crowdsourcing sustainability ecosystem is then proposed. This study adopted the case-study approach. Two crowdsourcing platforms were examined in the case study. Two techniques were used to obtain the data: observation and interview. Observation was carried out to observe how the crowdsourcing platforms worked. The interviews helped to uncover current practices, challenges in using crowdsourcing and identification of sustainability factors. It is hoped that the proposed conceptual model will facilitate better planning of the ecosystem supporting crowdsourcing and ensure sustainable growth for crowdsourcing. Future research into crowdsourcing can test the proposed conceptual model to validate its components.

    Adnexal masses are growths that form near the uterus; the majority being ovarian tumours. Although
    there is no established population-screening tool for detecting ovarian cancer, ultrasound and contrastenhanced
    computed tomography (CECT) are useful imaging tools in the management of adnexal masses.
    Our study aimed to determine the characteristics of malignant adnexal masses on CECT scan and to
    describe common pitfalls in diagnosis of ovarian cancer when interpreting images. We also determined the
    sensitivity and specificity of diagnosing ovarian cancer using CECT. A retrospective study was conducted
    in Hospital Serdang using data from all patients who underwent CECT scan and detected with adnexal
    masses, and had histopathological examination correlation from January 2013 until January 2015. Out of
    the 64 cases analysed; the majority of malignant lesions were serous carcinoma of the ovary (40%). The
    CECT scan characteristics, tumour consistency of mixed type, presence of wall enhancement, septations,
    ascites and peritoneal nodule/omental caking were significantly associated with ovarian malignancy

    Clostridium septicum infection has been shown to have a strikingly high association with either bowel or blood malignancies. The infection may be fatal if unrecognised. We report a case of C. septicum bacteremia in a man diagnosed with acute myeloid leukaemia.

    This article is a review of the latest information on the prevalence of G. lamblia in South Asia, South East Asia and Far East, characterizing the current endemic situation within these regions. Around 33 published papers from 2002-2007 were collected on G. lamblia. The included countries were Nepal, Bangladesh, India, Cambodia, Vietnam, Malaysia, Philippines, Indonesia, Thailand, Republic of Korea, and China. Only five published papers were discarded because data was extracted before 2002-2007 or they are not included within our regions, emphasizing more on G. lamblia in animals, or performed at extensive molecular level. The prevalence of G. lamblia varied markedly between studies illustrating higher levels in the urban than in the rural areas, more among poor communities, slightly higher in males than in females with age range of 2-5-year-old children, and among university students, old-aged people, HIV-positive patients, and gastric carcinoma patients. Though G. lamblia is not a life-threatening parasite, nevertheless, it is still considered as the most common water-borne diarrhea-causing disease. It is important to understand the etiology, frequency, and consequences of acute diarrhea in children. Routine surveillance such as bi-annual follow-up treatments, treating G. duodenalis cysts and other protozoa oocysts detected in ground water sources, and continuous health education are the most preventive measures.

    Gas-Liquid chromatography (GLC) was used to detect the presence of isocaproic acid produced by Clostridium difficile from 54 stool samples grown in cycloserine-cefoxitin broth. Isocaproic acid was detected in 12 samples of which 5 were confirmed to be Clostridium difficile by culture and biochemical tests. The detection of isocaproic acid by GLC together with the presence of presumptive colonies on primary selective culture media provides a more rapid laboratory diagnosis for Clostridium difficile.

    Three bacterial species of Clostridium (septicum, tertium and sporogenes) were identified to produce extracellular proteases cleaving IgA to Fab and Fc fragments, as demonstrated by SDS-PAGE and immunoelectrophoretic procedures. These enzymes acted on monometric IgA1 paraproteins and normal serum IgA1 but had no activity on IgA2 paraproteins and intact secretory IgA1 from human colostrum. Their action on polyclonal serum IgA1 suggested the absence of neutralizing anti-clostridial IgA protease activity. Although the enzymes were shown not to act on secretory IgA1, they were, however, able to digest free alpha-heavy chains of the dimeric IgA molecules. Susceptibility of the alpha-heavy chain to the proteases was more likely due to the change to a more accessible conformation than because of the absence of neutralizing anti-enzymic activity.

    Introduction: Colorectal cancer is the second most common cause of death in women, and the third most common cause in men. The main treatments available following surgery are chemotherapy and radiotherapy which are not always effective and have side effects. Modern medicine aims to develop targeted cancer therapies that are efficient and cause less side effects to patients. However, this approach requires a thorough understanding of the molecular events that cause cancer cell to grow and divide in order to identify suitable targets. The process of translating the findings into clinical studies can be high cost and technically demanding. However, development of a tissue microarray (TMA), allows immunohistochemical (IHC) staining of multiple cases simultaneously, thereby greatly reducing costs and time. Methods: A TMA was produced from approximately 400 cases of colorectal cancer, along with collection of associated clinical and pathological data. Sections from the TMA were tested for quality by staining with haematoxylin and eosin (H & E), in addition to IHC markers to molecularly classify the colon cancers. Results: The cores from the 384 cases of cancer were successfully transferred to 18 recipient TMA blocks. H & E staining showed good morphological preservation of the cases, reflecting the tumour in the donor blocks. IHC testing was able to successfully classify cases into distinct molecular groupings. Conclusions: The development of a TMA of colorectal cancers provides a valuable tool for the efficient and subsequent molecular classification of colorectal cancer using immunohistochemistry.

    Background: The cytokine cascade in the immunopathogenesis of malaria infection had been widely studied.
    However, their specific association with survival and severe infection remained obscure.

    Methods: The study investigated the cytokine profiles and histopathological features of malaria in the severe
    infection and survival models by using male ICR mice and male Sprague Dawley rats respectively.

    Results: The severe model, the infected ICR mice, exhibited a high parasitemia with 100% mortality after
    peak parasitemia at day 5 post-infection. The survival model, the infected Sprague Dawley rats, showed
    mild parasitemia with full recovery by day 14 of infection. Both severe and survival models showed similar
    histopathological severity during peak parasitemia. The severe model produced highly elevated levels of proinflammatory
    cytokines, TNF-α and IL-1α, and low levels of the anti-inflammatory cytokine, IL-4; while the
    survival model showed low levels of TNF-α and IL-1α with high levels of IL-4.

    Conclusion: There were differences in the pathogenesis of the severe and survival models of malaria infection.
    These could be a basis for immunotherapy of malaria in the future.

    This work has focused on the co-pyrolysis of sugarcane waste (SW) with polyethylene terephthalate (PET) to gain insight on its thermal decomposition, product distribution, kinetics, and synergistic effect. SW and PET were blended at different ratios (100:0, 80:20, 60:40, 40:60, and 0:100), and the Coats-Redfern method was used to determine the kinetics parameters. To ascertain the synergistic effect between SW and PET, product yields and composition of chemicals were compared with the synergistic effect of the individual components of pyrolysis. The bio-oil yield was significant at 60% of PET, with a difference of 19.41 wt% compared to the theoretical value. The synergistic impact of SW:PET on ester formation and acid compound inhibition was the most dominant at the 60:40 ratio. The kinetics analysis revealed that the diffusion mechanism, power law, and order of reactions were the most probable reaction models that can explain the pyrolysis of SW, and PET, and their blends. The resultant co-pyrolysis oil contained slightly larger hydrogen and carbon contents with low oxygen, and sulphur, and nitrogen contents, which improved the quality of the bio-oil. The results of this work could be used as a guide in selecting proper reaction conditions with optimal synergy during the co-pyrolysis process.

    A systematic investigation of the X-Ge-CY(3) (X = H, F, Cl, Br, and I; Y = F, Cl, Br, and I) species is carried out using density functional theory. The basis sets used for all atoms (except iodine) in this work are of double-ζ plus polarization quality with additional s- and p-type diffuse functions, and denoted DZP++. Vibrational frequency analyses are performed to evaluate zero-point energy corrections and to determine the nature of the stationary points located. Predicted are four different forms of neutral-anion separations: adiabatic electron affinity (EA(ad)), zero-point vibrational energy corrected EA(ad(ZPVE)), vertical electron affinity (EA(vert)), and vertical detachment energy (VDE). The electronegativity (χ) reactivity descriptor for the halogens (X = F, Cl, Br, and I) is used as a tool to assess the interrelated properties of these germylenes. The topological position of the halogen atom bound to the divalent germanium center is well correlated with the trend in the electron affinities and singlet-triplet gaps. For the expected XGeCY(3) structures (X = H, F, Cl, Br, and I; Y = F and Cl), the predicted trend in the electron affinities is well correlated with simpler germylene derivatives (J. Phys. Chem. A 2009, 113, 8080). The predicted EA(ad(ZPVE)) values with the BHLYP functional range from 1.66 eV (FGeCCl(3)) to 2.20 eV (IGeCF(3)), while the singlet-triplet splittings range from 1.28 eV (HGeCF(3)) to 2.22 eV (FGeCCl(3)). The XGeCY(3) (Y = Br and I) species are most often characterized by three-membered cyclic systems involving the divalent germanium atom, the carbon atom, and a halogen atom.

    BACKGROUND: The relatedness between the linear equations of thermodynamics and QSAR was studied thanks to the recently elucidated crystal structure complexes between sulfonamide pterin conjugates and dihydropteroate synthase (DHPS) together with a published set of thirty- six synthetic dapsone derivatives with their reported entropy-driven activity data. Only a few congeners were slightly better than dapsone.

    OBJECTIVE: Our study aimed at demonstrating the applicability of thermodynamic QSAR and to shed light on the mechanistic aspects of sulfone binding to DHPS.

    METHODS: To this end ligand docking to DHPS, quantum mechanical properties, 2D- and 3D-QSAR as well as Principle Component Analysis (PCA) were carried out.

    RESULTS: The short aryl substituents of the docked pterin-sulfa conjugates were outward oriented into the solvent space without interacting with target residues which explains why binding enthalpy (ΔH) did not correlate with potency. PCA revealed how chemically informative descriptors are evenly loaded on the first three PCs (interpreted as ΔG, ΔH and ΔS), while chemically cryptic ones reflected higher dimensional (complex) loadings.

    CONCLUSION: It is safe to utter that synthesis efforts to introduce short side chains for aryl derivatization of the dapsone scaffold have failed in the past. On theoretical grounds we provide computed evidence why dapsone is not a pharmacodynamic lead for drug profiling because enthalpic terms do not change significantly at the moment of ligand binding to target.

    A 19-year-old Malay male succumbed to a septicaemia caused by Chromobacterium violaceum 11 days after onset of illness. The organism is a common soil saprophyte and may be considered as contaminant on culture. It is essential to recognise its clinical significance in purulent processes so that appropriate therapy can be instituted. We report the first fatal case of Chromobacterium violaceum infection in the University Hospital, Kuala Lumpur.

    The in vitro activity of teicoplanin and A16686, two new glycopeptide antibiotics was determined against 196 isolates of anaerobic bacteria. The activity of teicoplanin and A16686, in comparison with that of vancomycin, clindamycin, erythromycin and fusidic acid was 2 to 16 times higher against the gram positive anaerobes, namely, Propionibacterium acnes, Clostridium perfringens, Clostridium difficile, Clostridium species, Peptococcus species and Peptostreptococcus species. However, Bacteroides fragilis was resistant to teicoplanin and A16686 while Bacteroides melaninogenicus and Bacteroides bivius were found to be sensitive.

    This review aimed to assess the usefulness of various magnetic resonance imaging (MRI) techniques for the quantification of neonatal white matter myelination. The Scopus, PubMed, and Web of Science databases were searched to identify studies following the PRISMA (preferred reporting items for systematic reviews and meta-analyses) statement using quantitative MRI techniques to examine samples collected from neonates to quantify myelin. Twelve studies were ultimately included. The results demonstrated that in validation studies, relaxometry is the most frequently explored approach (83.33%), followed by magnetization transfer imaging (8.33%) and a new automatic segmentation technique (8.33%). Synthetic MRI is recommended for quantifying myelin in neonates because of several advantages that outweigh a few negligible limitations.
