    Inadvertent perioperative hypothermia (IPH) is a common problem, despite advancements in a variety of warming systems. The use of a resistive heating blanket (RHB) is a common but costly approach to patient warming. We have introduced the use of a heat-band in our centre as a cost-effective alternative to the RHB for patient warming. The efficacy of the heat-band in preventing IPH during laparotomy for gynaecological surgeries was compared with that of the RHB.

    BACKGROUND: The idea of launching an internet-based self-management program for patients with diabetes led us to do a cross-sectional study to find out about the willingness, interest, equipment, and level of usage of computer and internet in a medium- to low-social class area and to find the feasibility of using e-telemonitoring systems for these patients.

    METHODS: A total of 180 patients with type 2 diabetes participated in this study and fulfilled the self-administered questionnaire in Diabetes Clinic of Primary Medical Center of University Kebangsaan Malaysia Medical Centre; the response rate was 84%. We used the universal sampling method and assessed three groups of factors including sociodemographic, information and communication technology (ICT), willingness and interest, and disease factors.

    RESULTS: Our results showed that 56% of the patients with diabetes were interested to use such programs; majority of the patients were Malay, and patients in the age group of 51-60 years formed the largest group. Majority of these patients studied up to secondary level of education. Age, education, income, and money spent for checkup were significantly associated with the interest of patients with diabetes to the internet-based programs. ICT-related factors such as computer ownership, computer knowledge, access to the internet, frequency of using the internet and reasons of internet usage had a positive effect on patients' interest.

    CONCLUSION: Our results show that among low to intermediate social class of Malaysian patients with type 2 diabetes, more than 50% of them can and wanted to use the internet-based self-management programs. Furthermore, we also show that patients equipped with more ICT-related factors had more interest toward these programs. Therefore, we propose making ICT more affordable and integrating it into the health care system at primary care level and then extending it nationwide.

    BACKGROUND: This study aims to estimate the prevalence and explore the predictors for post-stroke cognitive impairment at the community level in Malaysia.

    METHODS: A total of 50 stroke patients aged 29 to 81-year-old were included in this study. A face to face interview was conducted to gather the demographic and clinical data. Subsequently, assessments including Barthel ADL Index (BI), Addenbrooke's Cognitive Examination-Revised (ACE-R) and Hospital Anxiety and Depression Scale (HADS) were administered to the subjects.

    RESULTS: The results showed that the prevalence of cognitive impairment was 76% among the studied populations. The subjects' race (Fisher's value= 9.56, P < 0.05) and education level (Fisher's value = 7.29, P < 0.05) were significantly associated with the cognitive status. The depression score was significantly higher in cognitively impaired group [t (48) = -4.42, P < 0.001] while the Barthel Index score was significantly lower in cognitively impaired group (median = 18.00, P < 0.05). The univariate logistic analysis demonstrated that Chinese (OR 7.33, 95% CI = 1.61-33.51), lower education level (OR 9.33, 95% CI = 0.89-97.62), right sided lesion (OR 0.29, 95% CI = 0.06-1.54), left face weaknesses (OR 0.40, 95% CI 0.09-1.83), high cholesterol (OR 0.45, 95% CI = 0.12-1.75), depression (OR 2.16, 95% CI = 0.85-1.35), and Barthel Index (OR 0.79, 95% CI = 0.57-1.10) were significant predictors. Finally, multivariate logistic regression verified that depression was the only significant predictor of post-stroke cognitive impairment (OR 2.03, 95% CI = 1.20-3.45).

    CONCLUSION: In conclusion, the prevalence of cognitive impairment in this study was higher than other community based studies and depression was a risk factor for cognitive impairment.

    This study explores the experience of both learners and a teacher during a team-based learning (TBL) session. TBL involves active learning that allows medical students to utilise their visual, auditory, writing and kinetic learning styles in order to strengthen their knowledge and retain it for longer, which is important with regard to applying basic sciences in clinical settings. This pilot study explored the effectiveness of TBL in learning medical genetics, and its potential to replace conventional lectures. First-year medical students (n = 194) studying at Universiti Kebangsaan, Malaysia, during 2014/2015 were selected to participate in this study. The topic of 'Mutation and Mutation Analysis' was selected, and the principles of TBL were adhered to during the study. It was found that the students' performance in a group readiness test was better than in individual readiness tests. The effectiveness of TBL was further shown in the examination, during which the marks obtained were tremendously improved. Collective commentaries from both the learners and the teacher recommended TBL as another useful tool in learning medical genetics. Implementation strategies should be advanced for the benefit of future learners and teachers.

    The purpose is to describe a case of traumatic right extracranial internal carotid artery (EICA) pseudoaneurysm, which is a rare entity and the evolution of treatment from surgery to minimally invasive intervention by endovascular stenting and coiling. We reported a case of traumatic right EICA pseudoaneurysm who presented with multiple cranial nerve palsies. Multiple radiological examinations [including magnetic resonance imaging (MRI) with angiogram, computed tomography angiogram (CTA), and digital subtraction angiogram (DSA)] demonstrated right EICA pseudoaneurysm. The pseudoaneurysm was successfully treated with endovascular stenting and coiling. EICA pseudoaneurysm is a rare entity, and open surgery was the gold standard of treatment. Current technology allows endovascular stenting and coiling of pseudoaneurysm as an alternative treatment. It is minimally invasive, associated with lesser complications, better recovery and a shorter hospital stay.

    BACKGROUND: The main aim of this study is to utilize the geographical information system (GIS) software and perform the spatial analysis in relation to clinical data for road traffic injury (RTI) pediatric cases attending the emergency department.
    METHODS: The study sample included pediatric patients (age less than 18 years) with road-related injuries within a district in Malaysia who attended emergency departments of two tertiary hospitals within the district. In addition to injury, pre-hospital care and outcome data, the coordinate of the locations were obtained by the ambulance paramedics by using portable handheld GPS unit brand Garmin(®) model GPS 72 H. The data was transferred into the excel format which in turn underwent GIS analysis by using ARCGIS(®) (by ESRI) software version 10.1 licensed to the study institution.
    RESULTS: A total of 102 (24.8%) of all motor vehicle crash (MVC) victims involved the pediatric age group (age 18 years and below). The mean (SD) age of the pediatric victims was 14.30 years (SD 3.830). Male comprised of 68 (66.7%) of the cases. Motorcyclists [88 (88.0%)] were the most common type of victims involved. Interestingly, the majority of the severely injured victims [75 (73%)] sustained the RTI on roads with maximum speed limit of 60 km/hour. The mean (SD) length of hospital stay was 7.83 days (5.59).
    CONCLUSION: The pediatric related road traffic injury in Malaysia causes significant health and social burden in the country. This study showed both important clinical and geographical factors that need to be taken into consideration for future preventive action.

    Functional genomics research can give us valuable insights into bacterial gene function. RNA Sequencing (RNA-seq) can generate information on transcript abundance in bacteria following abiotic stress treatments. In this study, we used the RNA-seq technique to study the transcriptomes of the opportunistic nosocomial pathogen Pseudomonas aeruginosa PAO1 following heat shock. Samples were grown at both the human body temperature (37 °C) and an arbitrarily-selected temperature of 46 °C. In this work using RNA-seq, we identified 133 genes that are differentially expressed at 46 °C compared to the human body temperature. Our work identifies some key P. aeruginosa PAO1 genes whose products have importance in both environmental adaptation as well as in vivo infection in febrile hosts. More importantly, our transcriptomic results show that many genes are only expressed when subjected to heat shock. Because the RNA-seq can generate high throughput gene expression profiles, our work reveals many unanticipated genes with further work to be done exploring such genes products.

    Tanjung Selongor and Pantai Balok (State Pahang) are the only two places known for spawning activity of the Malaysian horseshoe crab - Tachypleus gigas (Müller, 1785) on the east coast of Peninsular Malaysia. While the former beach has been disturbed by several anthropogenic activities that ultimately brought an end to the spawning activity of T. gigas, the status of the latter remains uncertain. In the present study, the spawning behavior of T. gigas at Pantai Balok (Sites I-III) was observed over a period of thirty six months, in three phases, between 2009 and 2013. Every year, the crab's nesting activity was found to be high during Southwest monsoon (May-September) followed by Northeast (November-March) and Inter monsoon (April and October) periods. In the meantime, the number of female T. gigas in 2009-2010 (Phase-1) was higher (38 crabs) than in 2010-2011 (Phase-2: 7 crabs) and 2012-2013 (Phase-3: 9 crabs) for which both increased overexploitation (for edible and fishmeal preparations) as well as anthropogenic disturbances in the vicinity (sand mining since 2009, land reclamation for wave breaker/parking lot constructions in 2011 and fishing jetty construction in 2013) are responsible. In this context, the physical infrastructure developments have altered the sediment close to nesting sites to be dominated by fine sand (2.5Xφ ) with moderately-well sorted (0.6-0.7σφ), very-coarse skewed (-2.4SKφ), and extremely leptokurtic (12.6Kφ) properties. Also, increased concentrations of Cadmium (from 4.2 to 13.6 mg kg(-1)) and Selenium (from 11.5 to 23.3 mg kg(-1)) in the sediment, and Sulphide (from 21 to 28 µg l(-1)) in the water were observed. In relation to the monsoonal changes affecting sheltered beach topography and sediment flux, the spawning crabs have shown a seasonal nest shifting behaviour in-between Sites I-III during 2009-2011. However, in 2012-2013, the crabs were mostly restricted to the areas (i.e., Sites I and II) with high oxygen (5.5-8.0 mg l(-1)) and moisture depth (6.2-10.2 cm). In view of the sustained anthropogenic pressure on the coastal habitats on one hand and decreasing horseshoe crabs population on the other, it is crucial to implement both conservation and management measures for T. gigas at Pantai Balok. Failing that may lead to the loss of this final spawning ground on the east coast of P. Malaysia.

    The natural behaviour of the honeybee has attracted the attention of researchers in recent years and several algorithms have been developed that mimic swarm behaviour to solve optimisation problems. This paper introduces an artificial bee colony (ABC) algorithm for the vehicle routing problem with time windows (VRPTW). A Modified ABC algorithm is proposed to improve the solution quality of the original ABC. The high exploration ability of the ABC slows-down its convergence speed, which may due to the mechanism used by scout bees in replacing abandoned (unimproved) solutions with new ones. In the Modified ABC a list of abandoned solutions is used by the scout bees to memorise the abandoned solutions, then the scout bees select a solution from the list based on roulette wheel selection and replace by a new solution with random routs selected from the best solution. The performance of the Modified ABC is evaluated on Solomon benchmark datasets and compared with the original ABC. The computational results demonstrate that the Modified ABC outperforms the original ABC also produce good solutions when compared with the best-known results in the literature. Computational investigations show that the proposed algorithm is a good and promising approach for the VRPTW.

    Methods for in silico screening of large databases of molecules increasingly complement and replace experimental techniques to discover novel compounds to combat diseases. As these techniques become more complex and computationally costly we are faced with an increasing problem to provide the research community of life sciences with a convenient tool for high-throughput virtual screening on distributed computing resources.

    A method based on extended cubic B-spline is proposed to solve a linear system of second-order boundary value problems. In this method, two free parameters, [Formula: see text] and [Formula: see text], play an important role in producing accurate results. Optimization of these parameters are carried out and the truncation error is calculated. This method is tested on three examples. The examples suggest that this method produces comparable or more accurate results than cubic B-spline and some other methods.

    In recent years, there has been major interest in the exposure to physical therapy during rehabilitation. Several publications have demonstrated its usefulness in clinical/medical and human machine interface (HMI) applications. An automated system will guide the user to perform the training during rehabilitation independently. Advances in engineering have extended electromyography (EMG) beyond the traditional diagnostic applications to also include applications in diverse areas such as movement analysis. This paper gives an overview of the numerous methods available to recognize motion patterns of EMG signals for both isotonic and isometric contractions. Various signal analysis methods are compared by illustrating their applicability in real-time settings. This paper will be of interest to researchers who would like to select the most appropriate methodology in classifying motion patterns, especially during different types of contractions. For feature extraction, the probability density function (PDF) of EMG signals will be the main interest of this study. Following that, a brief explanation of the different methods for pre-processing, feature extraction and classifying EMG signals will be compared in terms of their performance. The crux of this paper is to review the most recent developments and research studies related to the issues mentioned above.

    Microbial contamination in food system poses risk towards public health. The usage of synthetic
    and chemical preservatives to prevent the contamination has become a growing concern due
    to the presence of deleterious and harmful substances that can cause environment and health
    problems in prolonged exposure. Thus, there are needs to overcome this problem by using
    natural products as food preservatives. In this study, the antimicrobial activities of methanolic
    Cymbopogon citratus (lemongrass) extracts were tested against five foodborne pathogens,
    namely Bacillus cereus, Escherichia coli O157:H7, Klebsiella pneumoniae, Staphylococcus
    aureus and Candida albicans. The susceptibility test, minimum inhibitory concentrations
    (MIC) and minimum bactericidal concentration (MBC) or minimum fungicidal concentration
    (MFC) were conducted using the broth microdilution techniques as described by Clinical and
    Laboratory Standard Institute (CLSI). C. citratus extract showed antimicrobial activity against
    all tested foodborne pathogens; B. cereus, E. coli O157:H7, K. pneumoniae, S. aureus and C.
    albicans with the inhibition zone of 12 mm, 7.5 mm, 11 mm, 10 mm and 9 mm, respectively.
    The MIC of C. citratus extract against B. cereus, E. coli O157:H7, K. pneumoniae, S. aureus
    and C. albicans was 0.08 mg/ml, 0.63 mg/ml, 0.04 mg/ml, 0.31 mg/ml, and 0.16 mg/ml,
    respectively, while the MBC or MFC was 1.25 mg/ml, 2.50 mg/ml, 2.50 mg/ml, 1.25 mg/
    ml and 1.25 mg/ml, respectively. Time–kill curves were determined to assess the correlation
    between MIC and bactericidal activity of C. citratus extract at concentrations ranging from 0×
    MIC to 4× MIC. The bactericidal endpoint for B. cereus, E. coli O157:H7, S. aureus and C.
    albicans was at 4× MIC after 2 h, 4× MIC after 2 h, 4× MIC after 30 min and 4× MIC after 4
    h, respectively whereas K. pneumoniae was not completely killed after 4 hours of incubation at
    4× MIC. The potent antimicrobial activity of C. citratus extract may support its usage as natural
    antimicrobial agent

    Fish can live healthier in aquarium with good water quality than they do in the wild. Maintaining
    the quality of the water in fish facility is needed to avoid fluctuation of physicochemical
    parameter values and contamination with pathogenic microorganisms that may cause serious
    illness or even death among the fish. Contamination of the water, especially with animal
    pathogens which are also pathogenic to human may pose health risk to those who are handling
    or in direct contact with the water and fish in the facility. Therefore, there is a need to assess the
    water quality and the risk associated with microorganisms in the water and the cultured animals.
    The aim of this study was to determine the water quality with regard to the physicochemical
    and microbiological parameters as well as the risk associated with bacteria in the water of the
    fish facility. Samples of water from the water source and also from aquariums in the fish facility
    were collected and analyzed. The water samples were plated on nutrient agar for bacterial
    enumeration then bacterial colonies growing on the agar plates were randomly picked and
    purified. (GTG)5
    -PCR analysis was carried out to analyse the heterogeneity of the genome of the
    bacterial isolated and a dendrogram was constructed from the (GTG)5
    -PCR profile to determine
    the genotypic group of the bacterial isolates. The risk associated with the bacteria from the
    water was analyzed with respect to their antibiotic resitance. The result of this study revealed
    that the (GTG)5
    -PCR analysis was able to group the bacteria into 2 main genotypic clusters
    which were further grouped into several sub-clusters. From the dengrogram, 12 representative
    isolates were selected and identified using 16S rRNA sequencing. The identification confirmed
    the presence of Aeromonas veronii (8 isolates), Aeromonas jandaei (2 isolates), Plesiomonas
    shigelloides (1 isolate) and Pseudomonas alcaligene (1 isolate) from the water samples. All
    of the isolates exhibited resistant towards ampicillin, penicillin and gentamicin. This study
    revealed that the water from the fish facility harboured genetically diverse antibiotic resistance
    bacteria which may pose health risk to the fish and also to those who are in direct contact
    with the contaminated water and fish in the facility. Therefore, water in fish facility should be
    monitored regularly and handled with caution.

    As the society begin to realize the importance of combating antimicrobial resistance, going
    back to silver might be the solution. Silver has been known for its potential antimicrobial
    activity since ancient times and, the development of nanoparticles has increased its potential
    into becoming an antimicrobial agent that can be applied in broad-spectrum. Antimicrobial
    resistance has spread into an irrepressible manner which requires drastic action plan as a number
    of pathogenic bacteria began to acquire resistance genes. Methicillin Resistant Staphylococcus
    aureus (MRSA) is one of the earliest reported resistant clones which is the center of this study.
    This study focused on the dissemination and evolution of MRSA on its resistance towards
    antibiotics. Disc Diffusion Test was employed to create the antibiograms of MRSA isolates. All
    isolates showed resistance towards amoxicillin, ampicillin, cefazolin, oxacillin and penicillin.
    In contrast, all isolates were susceptible towards erythromycin. The findings also discovered
    isolates that were vancomycin-resistant (66.7%) and vancomycin-intermediate (33.3%). As the
    efficacy of antibiotic treatment is at a question, we also investigated on the antimicrobial activity
    of colloidal silver in the hope as an alternative treatment. Shiga Toxin producing Escherichia
    coli (STEC) and MRSA (ATCC 33591) was tested using modified Quantitative suspension
    test for the evaluation of bactericidal activity for chemical disinfectants and antiseptics based
    on BS EN 1276:2009. The outcome of this study indicated that the colloidal silver is working
    effectively against STEC and MRSA (ATCC 33591), showing killing percentages well above
    99.0% at 4 minutes and 8 minutes of contact. Vancomycin-resistant S. aureus (VRSA) and
    Vancomycin-intermediate S. aureus (VISA) were also tested and the results indicated that
    VISA had higher killing percentages at 4 minutes (99.83%) and 8 minutes (99.85%) compared
    to VRSA at 4 minutes (96.72%) and 8 minutes (98.35%). This opens a solution to the rising
    problem of antimicrobial resistance.

    The wide application of microalgae in the field of wastewater treatment and bioenergy source has improved research studies in the past years. Microalgae represent a good source of biomass and bio-products which are used in different medical and industrial activities, among them the production of high-valued products and biofuels. The present review focused on greywater treatment through the application of phycoremediation technique with microalgae and presented recent advances in technologies used for harvesting the microalgae biomass. The advantages and disadvantages of each method are discussed. The microbiological aspects of production, harvesting and utilization of microalgae biomass are viewed.

    Hendra virus (HeV) and Nipah virus (NiV) are members of the genus Henipavirus, within the family Paramyxoviridae. Nipah virus has caused outbreaks of human disease in Bangladesh, Malaysia, Singapore, India and Philippines, in addition to a large outbreak in swine in Malaysia in 1998/1999. Recently, NiV was suspected to be a causative agent of an outbreak in horses in 2014 in the Philippines, while HeV has caused multiple human and equine outbreaks in Australia since 1994. A swine vaccine able to prevent shedding of infectious virus is of veterinary and human health importance, and correlates of protection against henipavirus infection in swine need to be better understood. In the present study, three groups of animals were employed. Pigs vaccinated with adjuvanted recombinant soluble HeV G protein (sGHEV) and challenged with HeV, developed antibody levels considered to be protective prior to the challenge (titers of 320). However, activation of the cell-mediated immune response was not detected, and the animals were only partially protected against challenge with 5×10(5) PFU of HeV per animal. In the second group, cross-neutralizing antibody levels against NiV in the sGHEV vaccinated animals did not reach protective levels, and with no activation of cellular immune memory, these animals were not protected against NiV. Only pigs orally infected with 5×10(4) PFU of NiV per animal were protected against nasal challenge with 5×10(5) PFU of NiV per animal. This group of pigs developed protective antibody levels, as well as cell-mediated immune memory. Peripheral blood mononuclear cells restimulated with UV-inactivated NiV upregulated IFN-gamma, IL-10 and the CD25 activation marker on CD4(+)CD8(+) T memory helper cells and to lesser extent on CD4(-)CD8(+) T cells. In conclusion, both humoral and cellular immune responses were required for protection of swine against henipaviruses.

    Composition of atherosclerotic arterial walls is rich in lipids such as cholesterol, unlike normal arterial walls. In this study, we aimed to utilize this difference to diagnose atherosclerosis via multispectral fluorescence imaging, which allows for identification of fluorescence originating from the substance in the arterial wall.
