    In recent years, water resources management has become more complicated and controversial due to the impacts of various factors affecting hydrological systems. System Dynamics (SD) has in turn become increasingly popular due to its advantages as a tool for dealing with such complex systems. However, SD also has some limitations. This review contains a comprehensive survey of the existing literature on SD as a potential method to deal with the complexity of system integrated modeling, with a particular focus on the application of SD to the integrated modeling of water resources systems. It discusses the limitations of SD in these contexts, and highlights a number of studies which have applied a combination of SD and other methods to overcome these limitations. Finally, our study makes a number of recommendations for future modifications in the application of SD methods in order to enhance their performance.

    Auriculotherapy has been extensively used for chronic spontaneous urticaria in China. However, the evidence of its effectiveness and safety for the treatment of chronic spontaneous urticaria is insufficient. Hence, we conducted this study to compare auriculotherapy or auriculotherapy joint treatment with Western medicine for the cure of chronic spontaneous urticaria. This meta-analysis of seven randomized controlled trials showed that auriculotherapy or auriculotherapy joint treatment was significantly superior to Western medicine in curing clinical signs and symptoms of chronic spontaneous urticaria [odds ration (OR), 2.61; 95% confidence interval (CI), 1.54-4.43; p = 0.0004) and also better in total effect rate (OR, 3.81; 95% CI, 2.07-7.01; p<0.0001). But, auriculotherapy or auriculotherapy joint treatment was similar to Western medicine in improving clinical signs and symptoms of chronic spontaneous urticaria (OR, 0.74; 95% CI, 0.35-1.56; p = 0.42). Auriculotherapy or auriculotherapy joint treatment was safer than Western medicine for curing chronic spontaneous urticaria (OR, 0.26; 95% CI, 0.09-0.80; p = 0.02). Auriculotherapy alone or auriculotherapy joint treatment appears to be more effective and safer than Western medicine that contains antihistamines in the treatment of chronic spontaneous urticaria. However, these findings should be interpreted with caution due to the unclear risk bias of methodological quality, and further studies with large-scale, better, and more rigorously designed protocol are necessary to prove these findings.

    Two azide-terminated oligoethylene oxide spacered glycolipids have been synthesized, and their assembly behavior has been studied in comparison to the corresponding base surfactants. The results suggest potential of the Guerbet lactoside-based compound for targeted drug delivery, while a coiling of the ethylene oxide linker disfavors the application of the glucoside.

    With the rapid growth of agricultural areas globally, forest birds increasingly encounter fragmented landscapes in which forest patches are surrounded by an agricultural plantation matrix, yet how birds respond behaviourally to this fragmentation is poorly understood. Information on microhabitat requirements of birds is scarce, but nevertheless essential to predicting adaptation of bird species to the patchy landscapes. We investigated foraging patterns of three tropical insectivorous birds, Green Iora Aegithina viridissima, Pin-striped Tit-Babbler Macronus gularis and Chestnut-winged Babbler Cyanoderma erythropterum, to determine whether they vary in foraging methods in different forest patches. Our study area encompassed old-logged lowland forest; one continuous forest and three forest patches. Observations were performed for 15 days every month for a period of 13 months. Information on foraging height, substrate, attack manoeuvres, and foliage density was collected independently for each foraging bird individual. All three species used different foraging substrates and attack manoeuvres in different habitat types. The Green Iora frequently used lower strata when foraging in forest patches as opposed to continuous forest, while the Pin-striped Tit-Babbler tended to forage in more dense vegetation in patches. Only Chestnut-winged Babbler displayed complete foraging plasticity across all study parameters. Different habitat features (e.g., edges, microclimates) between continuous forest and forest patches significantly influenced the foraging strategies of the study species. These changes in foraging strategies suggest that some Malaysian forest birds (e.g. generalist species) can respond behaviourally to fragmentation and habitat loss. Although continuous forest has critically important characteristics that need to be conserved, remnant forest patches are also important as ecological movement corridors and foraging grounds for birds.

    PURPOSE: Low muscle strength is central to geriatric physical disabilities and mortality. The purpose of the present study was to examine handgrip strength (HGS) and its associated factors among community-dwelling older people in Sri Lanka.

    METHODS: A cross-sectional study was conducted in the Kandy district using multistage sampling. A total of 999 older people were recruited, with a female preponderance. Data were collected using interviewer-administered questionnaires on demographic characteristics, depression, and physical activity. Anthropometric measurements including weight, height, mid-upper arm circumference, calf circumference, and HGS were recorded. Complex sample general linear model was used to examine the association between HGS and its associated factors.

    RESULTS: The mean highest HGS of the study group was 12.56 kg (95% confidence interval: 11.94-13.19). Male older people had a higher HGS (17.02, 95% confidence interval: 15.55-18.49 kg) than females (10.59, 95% confidence interval: 10.12-11.06 kg). For both men and women, older age was associated with lower HGS, while mid-upper arm circumference was associated with better HGS. Diabetes mellitus, vegetarian diet, and alcohol consumption were associated with HGS for women only.

    CONCLUSION: Men had a higher HGS compared with women. Age, mid-upper arm circumference, diabetes mellitus, vegetarian diet, and alcohol consumption were factors associated with HGS among community-dwelling older people in Kandy district, Sri Lanka. HGS can be used as a feasible strategy to improve health status of older people by community health nurses.

    Purpose: Renal cell carcinoma (RCC) has the potential for cure with surgery when diagnosed at an early stage. Kidney injury molecule-1 (KIM-1) has been shown to be elevated in the plasma of RCC patients. We aimed to test whether plasma KIM-1 could represent a means of detecting RCC prior to clinical diagnosis.Experimental Design: KIM-1 concentrations were measured in prediagnostic plasma from 190 RCC cases and 190 controls nested within a population-based prospective cohort study. Cases had entered the cohort up to 5 years before diagnosis, and controls were matched on cases for date of birth, date at blood donation, sex, and country. We applied conditional logistic regression and flexible parametric survival models to evaluate the association between plasma KIM-1 concentrations and RCC risk and survival.Results: The incidence rate ratio (IRR) of RCC for a doubling in KIM-1 concentration was 1.71 [95% confidence interval (CI), 1.44-2.03, P = 4.1 × 10-23], corresponding to an IRR of 63.3 (95% CI, 16.2-246.9) comparing the 80th to the 20th percentiles of the KIM-1 distribution in this sample. Compared with a risk model including known risk factors of RCC (age, sex, country, body mass index, and tobacco smoking status), a risk model additionally including KIM-1 substantially improved discrimination between cases and controls (area under the receiver-operating characteristic curve of 0.8 compared with 0.7). High plasma KIM-1 concentrations were also associated with poorer survival (P = 0.0053).Conclusions: Plasma KIM-1 concentrations could predict RCC incidence up to 5 years prior to diagnosis and were associated with poorer survival.     Background: The World Health Organization (WHO) declares Chikungunya (CHIK) infection to be endemic in South Asia. Despite its first outbreak in Pakistan, no documented evidence exists which reveals the knowledge or awareness of healthcare students and workers (HCSW) regarding CHIK, its spread, symptoms, treatment and prevention. Since CHIK is an emergent infection in Pakistan, poor disease knowledge may result in a significant delay in diagnosis and treatment. The current study was aimed to evaluate the awareness and knowledge of CHIK among HCSW.

    Methods: A cross-sectional study was conducted among HCSW from teaching institutes and hospitals in seven provinces of Pakistan. We collected information on socio-demographic characteristics of the participants and their knowledge by using a 30-item questionnaire. The cumulative knowledge score (CKS) was calculated by correct answers with maximum score of 22. The relationship between demographics and knowledge score was evaluated by using appropriate statistical methods.

    Results: There were 563 respondents; mean age 25.2 ± 5.9 years with female preponderance (62.5%). Of these, 319 (56.7%) were aware of CHIK infection before administering the survey. The average knowledge score was 12.8 ± 4.1 (% knowledge score: 58.2%). Only 31% respondents had good disease knowledge while others had fair (36.4%) and poor (32.6%) knowledge. Out of five knowledge domains, domain III (vector, disease spread and transmission) and V (prevention and treatment) scored lowest among all i.e. percent score 44.5% and 54.1%, respectively. We found that socio-demographic characteristics had no influence on knowledge score of the study participants.

    Conclusion: Approximately one-half of participants were not aware of CHIK infection and those who were aware had insufficient disease knowledge. Findings of the current study underscore the dire need of educational interventions not only for health care workers but also for students, irrespective to the discipline of study.

    Microwave imaging is the technique to identify hidden objects from structures using electromagnetic waves that can be applied in medical diagnosis. The change of dielectric property can be detected using microwave antenna sensor, which can lead to localization of abnormality in the human body. This paper presents a stacked type modified Planar Inverted F Antenna (PIFA) as microwave imaging sensor. Design and performance analysis of the sensor antenna along with computational and experimental analysis to identify concealed object has been investigated in this study. The dimension of the modified PIFA radiating patch is 40 × 20 × 10 mm³. The reflector walls used, are 45 mm in length and 0.2-mm-thick inexpensive copper sheet is considered for the simulation and fabrication which addresses the problems of high expenses in conventional patch antenna. The proposed antenna sensor operates at 1.55⁻1.68 GHz where the maximum realized gain is 4.5 dB with consistent unidirectional radiation characteristics. The proposed sensor antenna is used to identify tumor in a computational human tissue phantom based on reflection and transmission coefficient. Finally, an experiment has been performed to verify the antenna's potentiality of detecting abnormality in realistic breast phantom.

    Globally, breast cancer is reported as a primary cause of death in women. More than 1.8 million new breast cancer cases are diagnosed every year. Because of the current limitations on clinical imaging, researchers are motivated to investigate complementary tools and alternatives to available techniques for detecting breast cancer in earlier stages. This article presents a review of concepts and electromagnetic techniques for microwave breast imaging. More specifically, this work reviews ultra-wideband (UWB) antenna sensors and their current applications in medical imaging, leading to breast imaging. We review the use of UWB sensor based microwave energy in various imaging applications for breast tumor related diseases, tumor detection, and breast tumor detection. In microwave imaging, the back-scattered signals radiating by sensors from a human body are analyzed for changes in the electrical properties of tissues. Tumorous cells exhibit higher dielectric constants because of their high water content. The goal of this article is to provide microwave researchers with in-depth information on electromagnetic techniques for microwave imaging sensors and describe recent developments in these techniques.

    The preparation of activated carbon using palm kernel shells as the precursor (PKSAC) was successfully accomplished after the parametric optimization of the carbonization temperature, carbonization holding time, and the ratio of the activator (H₃PO₄) to the precursor. Optimization at 500 °C for 2 h of carbonization with 20% H₃PO₄ resulted in the highest surface area of the activated carbon (C20) of 1169 m² g-1 and, with an average pore size of 27 Å. Subsequently, the preparation of shape-stabilized phase change material (SSPCM-C20) was done by the encapsulation of n-octadecane into the pores of the PKSAC, C20. The field emission scanning electron microscope images and the nitrogen gas adsorption-desorption isotherms show that n-octadecane was successfully encapsulated into the pores of C20. The resulting SSPCM-C20 nano-composite shows good thermal reliability which is chemically and thermally stable and can stand up to 500 melting and freezing cycles. This research work provided a new strategy for the preparation of SSPCM material for thermal energy storage application generated from oil palm waste.

    A compact ultrawideband (UWB) antenna based on a hexagonal split-ring resonator (HSRR) is presented in this paper for sensing the pH factor. The modified HSRR is a new concept regarding the conventional square split-ring resonator (SSRR). Two HSRRs are interconnected with a strip line and a split in one HSRR is introduced to increase the electrical length and coupling effect. The presented UWB antenna consists of three unit cells on top of the radiating patch element. This combination of UWB antenna and HSRR gives double-negative characteristics which increase the sensitivity of the UWB antenna for the pH sensor. The proposed ultrawideband antenna metamaterial sensor was designed and fabricated on FR-4 substrate. The electrical length of the proposed metamaterial antenna sensor is 0.238 × 0.194 × 0.016 λ, where λ is the lowest frequency of 3 GHz. The fractional bandwidth and bandwidth dimension ratio were achieved with the metamaterial-inspired antenna as 146.91% and 3183.05, respectively. The operating frequency of this antenna sensor covers the bandwidth of 17 GHz, starting from 3 to 20 GHz with a realized gain of 3.88 dB. The proposed HSRR-based ultrawideband antenna sensor is found to reach high gain and bandwidth while maintaining the smallest electrical size, a highly desired property for pH-sensing applications.

    Microwave breast imaging has been reported as having the most potential to become an alternative or additional tool to the existing X-ray mammography technique for detecting breast tumors. Microwave antenna sensor performance plays a significant role in microwave imaging system applications because the image quality is mostly affected by the microwave antenna sensor array properties like the number of antenna sensors in the array and the size of the antenna sensors. In this paper, a new system for successful early detection of a breast tumor using a balanced slotted antipodal Vivaldi Antenna (BSAVA) sensor is presented. The designed antenna sensor has an overall dimension of 0.401λ × 0.401λ × 0.016λ at the first resonant frequency and operates between 3.01 to 11 GHz under 10 dB. The radiating fins are modified by etching three slots on both fins which increases the operating bandwidth, directionality of radiation pattern, gain and efficiency. The antenna sensor performance of both the frequency domain and time domain scenarios and high-fidelity factor with NFD is also investigated. The antenna sensor can send and receive short electromagnetic pulses in the near field with low loss, little distortion and highly directionality. A realistic homogenous breast phantom is fabricated, and a breast phantom measurement system is developed where a two antennas sensor is placed on the breast model rotated by a mechanical scanner. The tumor response was investigated by analyzing the backscattering signals and successful image construction proves that the proposed microwave antenna sensor can be a suitable candidate for a high-resolution microwave breast imaging system.

    Streptococcus suis (S. suis) is a gram-positive bacterial pathogen in pigs which can cause serious infections in human including meningitis, and septicaemia resulting in serious complications. There were discrepancies between different data and little is known concerning associated risk factors of S. suis. A systematic review and meta-analysis was conducted to investigate on S. suis infection risk factors in human. We searched eight relevant databases using the MeSH terms "Streptococcus suis" OR "Streptococcus suis AND infection" limited in human with no time nor language restriction. Out of 4,999 articles identified, 32 and 3 studies were included for systematic review and meta-analysis respectively with a total of 1,454 Streptococcus suis cases reported. S. suis patients were generally adult males and the elderly. The mean age ranged between 37 to 63 years. Meningitis was the most common clinical manifestation, and deafness was the most common sequelae found among survivors followed by vestibular dysfunction. Infective endocarditis was also noted as among the most common clinical presentations associated with high mortality rate in a few studies. Meta-analyses categorized by type of control groups (community control, and non-S. suis sepsis) were done among 850 participants in 3 studies. The combined odd ratios for studies using community control groups and non-S. Suis sepsis as controls respectively were 4.63 (95% CI 2.94-7.29) and 78.00 (95% CI 10.38-585.87) for raw pork consumption, 4.01 (95% CI 2.61-6.15) and 3.03 (95% CI 1.61-5.68) for exposure to pigs or pork, 11.47, (95% CI 5.68-23.14) and 3.07 (95% CI 1.81-5.18) for pig-related occupation and 3.56 (95% CI 2.18-5.80) and 5.84 (95% CI 2.76-12.36) for male sex. The results were found to be significantly associated with S. suis infection and there was non-significant heterogeneity. History of skin injury and underlying diseases were noted only a small percentage in most studies. Setting up an effective screening protocol and public health interventions would be effective to enhance understanding about the disease.

    Background and Objectives: Medical schools are to develop integrated medical curricula because the term 'integrated curriculum' has grown up and flourished globally and it has become mandatory to align the medical education with the global concept in Pakistan. This paper aims to present a guideline to design an undergraduate integrated medical curriculum.

    Methods: Various themes are used to develop integrated curriculum which are basic medical science, simulation skills, clinical science, personality development, research, entrepreneurship and pre specialization. Each theme is subdivided, termed a module and its contents primarily focus on particular aspect.

    Results: Knowledge, skill and attitude, embodied in themes or modules, are planted in specific way that they have horizontal as well as vertical integration. There is no boundary of various traditional disciplines in template of five years curriculum. For example, diagnosis is a theme which carries contents from medicine, surgery, orthopedics etc.

    Conclusion: The blueprint introduced in this paper would help medical educators to draft integrated medical curricula for those institutions which intend to switch their medical programs from traditional to integrated one.

    Objectives: Studies have shown that periodontal disease and Rheumatoid Arthritis (RA) shared similar pathogenesis. Anti-Cyclic Citrullinated Peptide anibodies (anti-CCP) has recently been used for diagnosis of RA. Thus, this study aimed to assess the levels of anti-CCP antibodies and periodontal status in RA patients.
    Methods: Forty four RA patients were included in this study. The blood samples were analysed for anti-CCP levels. Plaque Score (PS), Gingivitis Score (GS), Probing Pocket Depth (PPD) and Clinical Attachment Loss (CAL) were recorded for assessment of periodontal status. Relevant clinical information was obtained from medical records.
    Results: Mean anti-CCP level was 180.8 ± 290.3 Unit/ml. The results showed that 27.3% patients had poor oral hygiene (PS >60%; mean anti-CCP 84.22 ± 167.51 Unit/ml), 52.3% had generalized gingivitis (mean anti-CCP 145.07 ± 269.17 Unit/ml), and 20.5% had mean CAL of >3mm (mean anti-CCP 56.81 ± 119.02 Unit/ml). None of patients presented with deep PPD > 4mm. The levels of anti-CCP showed no significant association with periodontal status (p=0.27).
    Conclusion: Most RA patients were positive for anti-CCP antibodies and presented with generalized gingivitis. Oral hygiene education should be reinforced in RA patients to prevent further progression of periodontal disease. Nevertheless, studies with larger sample size should be carried out to obtain more conclusive findings.

    Malaysia is facing an increasing trend in industrial solid waste generation due to industrial development. Thus, there is a paramount need in taking practical actions and measurements to move toward sustainable industrial waste management. The main aim of this study is to assess practicing solid waste minimization by manufacturing firms. Analysis showed that majority of firms (92%) dispose of their wastes rather than utilize other sustainable waste management options.     Many organisms use chemicals to deter enemies. Some spiders can modify the composition of their silk to deter predators from climbing onto their webs. The Malaysian golden orb-weaver Nephila antipodiana (Walckenaer) produces silk containing an alkaloid (2-pyrrolidinone) that functions as a defense against ant invasion-ants avoid silk containing this chemical. In the present study, we test the generality of ants' silk avoidance behavior in the field. We introduced three ant species to the orb webs of Nephila clavipes (Linnaeus) in the tropical rainforest of La Selva, Costa Rica. We found that predatory army ants (Eciton burchellii Westwood) as well as non-predatory leaf-cutting ants (Atta cephalotes Linnaeus and Acromyrmex volcanus Wheeler) avoided adult N. clavipes silk, suggesting that an additional species within genus Nephila may possess ant-deterring silk. Our field assay also suggests that silk avoidance behavior is found in multiple ant species.

    The ability to control the subcellular localization of nanoparticles within living plants offers unique advantages for targeted biomolecule delivery and enables important applications in plant bioengineering. However, the mechanism of nanoparticle transport past plant biological membranes is poorly understood. Here, a mechanistic study of nanoparticle cellular uptake into plant protoplasts is presented. An experimentally validated mathematical model of lipid exchange envelope penetration mechanism for protoplasts, which predicts that the subcellular distribution of nanoparticles in plant cells is dictated by the particle size and the magnitude of the zeta potential, is advanced. The mechanism is completely generic, describing nanoparticles ranging from quantum dots, gold and silica nanoparticles, nanoceria, and single-walled carbon nanotubes (SWNTs). In addition, the use of imaging flow cytometry to investigate the influence of protoplasts' morphological characteristics on nanoparticle uptake efficiency is demonstrated. Using DNA-wrapped SWNTs as model nanoparticles, it is found that glycerolipids, the predominant lipids in chloroplast membranes, exhibit stronger lipid-nanoparticle interaction than phospholipids, the major constituent in protoplast membrane. This work can guide the rational design of nanoparticles for targeted delivery into specific compartments within plant cells without the use of chemical or mechanical aid, potentially enabling various plant engineering applications.
