    Transboundary emissions of smoke-haze from land and forest fires have recurred annually during the dry period (June to October, over the past few decades) in South East Asia. Hazardous air quality has been recorded in Malaysia during these episodes. Agricultural practices such as slash-and-burn of biomass and peat fires particularly in Sumatera and Kalimantan, Indonesia, have been implicated as the major causes of the haze. Past findings have shown that a diversity of microbes can thrive in air including in smoke-haze polluted air. In this study, metagenomic data were generated to reveal the diversity of microorganisms in air during days with and without haze. Air samples were collected during non-haze (2013A01) and two haze (2013A04 and 2013A05) periods in the month of June 2013. DNA was extracted from the samples, subjected to Multiple Displacement Amplification and whole genome sequencing (Next Generation Sequencing) using the HiSeq 2000 Platform. Extensive bio-informatic analyses of the raw sequence data then followed. Raw reads from these six air samples were deposited in the NCBI SRA databases under Bioproject PRJNA662021 with accession numbers SRX9087478, SRX9087479 and SRX9087480.

    The genome data of Streptomyces sp. FH025 comprised of 8,381,474 bp with a high GC content of 72.51%. The genome contains 7035 coding sequences spanning 1261 contigs. Streptomyces sp. FH025 contains 57 secondary metabolite gene clusters including polyketide synthase, nonribosomal polyketide synthase and other biosynthetic pathways such as amglyccycl, butyrolactone, terpenes, siderophores, lanthipeptide-class-iv, and ladderane. 16S rRNA analysis of Streptomyces sp. FH025 is similar to the Streptomyces genus. This whole genome project has been deposited at NCBI under the accession JAFLNG000000000.

    Objective: Primary hypophysitis refers to the isolated inflammation of the pituitary gland not associated with other secondary causes. Among its histopathologic subtypes, xanthomatous is the rarest.

    Methods: We describe a 22-year-old woman with xanthomatous hypophysitis (XH), its clinical progression over 8 years as well as the treatment effects of prednisolone and azathioprine. Our patient was first referred for severe short stature and delayed puberty at the age of 14 years.

    Results: Investigations revealed multiple pituitary deficiencies. Magnetic resonance imaging showed a pituitary mass whereby a partial resection was performed. A full resection was not feasible due to the location of the mass. The histopathologic analysis of the tissue was consistent with XH. The results of secondary workout for neoplasm, infection, autoimmune, and inflammatory disorders were negative. After surgery, a progressive enlargement of the mass was observed. Two courses of prednisolone were administered with a significant reduction in the mass size. Azathioprine was added due to the unsustained effects of prednisolone when tapered off and the concern of steroid toxicity with continued use. No further increase in the mass size was noted after 6 months on azathioprine.

    Conclusion: Glucocorticoid and immunotherapy are treatment options for XH; however, more cases are needed to better understand its pathogenesis and clinical progression.

    Interleukin (IL)-6 is an emerging therapeutic target in myocardial infarction (MI). IL-6 has 2 distinct signaling pathways: trans-signaling, which mediates inflammation, and classic signaling, which also has anti-inflammatory effects. The novel recombinant fusion protein sgp130Fc achieves exclusive trans-signaling blockade, whereas anti-IL-6 antibodies (Abs) result in panantagonism. In a rat model of reperfused MI, sgp130Fc, but not anti-IL-6-Ab, attenuated neutrophil and macrophage infiltration into the myocardium, reduced infarct size, and preserved cardiac function 28 days after MI. These data demonstrate the efficacy of exclusive IL-6 trans-signaling blockade and support further investigation of sgp130Fc as a potential novel therapy in MI.

    The discovery of different antimicrobial agents has revolutionized the treatment against a variety of infections for many decades, but the emergence of antimicrobial resistance require rigorous measures, even amid the coronavirus disease 2019 (COVID-19) pandemic. This retrospective study aimed to examine the consumption of antibiotics in patients with COVID-19 admitted into the five hospitals in the province of Punjab, Pakistan. We collected data on the consumption of antibiotics, classified using the World Health Organization (WHO) AWaRe (Access, Watch, and Reserve), within two months-August and September, 2020, and the corresponding months in 2019. Consumption of antibiotics was presented as daily define dose (DDD) per 100 occupied bed-days. Eight different classes of antibiotics were prescribed to patients with COVID-19 without culture tests being performed, with the prescribing of antibiotics of the Watch category was especially prevalent. The consumption of antibiotics was higher during the COVID-19 pandemic compared to the pre-pandemic period: the consumption of azithromycin increased from 11.5 DDDs per 100 occupied bed-days in 2019 to 17.0 DDDs per 100 occupied bed-days in 2020, while the consumption of ceftriaxone increased from 20.2 DDDs per 100 occupied bed-days in 2019 to 25.1 DDDs per 100 occupied bed-days in 2020. The current study revealed non-evidence-based utilization of antibiotics among patients with COVID-19 admitted into the hospitals in Pakistan. Evidently, the current COVID-19 pandemic is a public health threat of notable dimensions which has compromised the ongoing antimicrobial stewardship program, potentially leading to the emergence of antimicrobial resistance among pathogens.

    PURPOSE: This study aims to describe the employment status and pattern among survivors of traumatic brain injury after motor vehicle accidents, and to explore the effects of demographic, injury variables and concomitant injuries on the employment status .

    METHOD: A retrospective analyses of 370 medical reports written for patients who sustained traumatic brain injury from motor vehicle accidents was conducted. To establish the employment pattern, the pre-injury employment history was compared to the latest employment status documented. Types and severity of concomitant injuries were rated according to Abbreviated Injury Scale criteria. All significant variables were further analyzed using logistic regression to explore predictors of employment.

    RESULTS: Up to 87% of the patients sustained concomitant injuries, with more than two-thirds (72%) scoring ≤ 2 on the Abbreviated Injury Scale. One hundred and eighty-two patients (49.2%) successfully returned to work. Among those who returned to work, 34% returned to former employment with pre-injury job description. Severity of traumatic brain injury, length of acute hospital stay, ambulation status and cognitive status were found to be significant predictive factors for employment status post traumatic brain injury. Presence of concomitant extremity injuries was found to influence the employment pattern among traumatic brain injury survivors.

    CONCLUSION: The return to work rate was somewhat low and was not influenced by presence of concomitant injuries. .

    AIM: To evaluate the dental pulp response to a novel mineral trioxide aggregate containing phosphorylated pullulan (MTAPPL) in rats after direct pulp capping.

    METHODS: Ninety-six cavities were prepared in the maxillary first molars of 56 male Wistar rats. The dental pulps were intentionally exposed and randomly divided into four groups according to the application of pulp capping materials: MTAPPL; phosphorylated pullulan (PPL); a conventional MTA (Nex-Cem MTA, NCMTA; positive control); and Super-Bond (SB; negative control). All cavities were restored with SB and observed for pulpal responses at 1-, 3-, 7- and 28-day intervals using a histological scoring system. Statistical analysis was performed using Kruskal-Wallis and Mann-Whitney U-test with Bonferroni's correction, and the level of significance was set at 0.05. DMP1 and CD34 antigen were used to evaluate odontoblast differentiation and pulpal vascularization, respectively.

    RESULTS: On day 1, mild inflammatory cells were present in MTAPPL and NCMTA groups; fewer inflammatory cells were present in the PPL, whereas SB was associated with a mild-to-moderate inflammatory response.     Space is a resource that is constantly being depleted, especially in mega-cities. Underground workspaces (UGS) are increasingly being included in urban plans and have emerged as an essential component of vertical cities. While progress had been made on the engineering aspects associated with the development of high-quality UGS, public attitudes toward UGS as work environments (ie, the public's design concerns with UGS) are relatively unknown. Here, we present the first large-scale study examining preferences and attitudes toward UGS, surveying close to 2000 participants from four cities in three continents (Singapore, Shanghai, London, and Montreal). Contrary to previous beliefs, air quality (and not lack of windows) is the major concern of prospective occupants. Windows, temperature, and lighting emerged as additional important building performance aspects for UGS. Early adopters (ie, individuals more willing to accept UGS and thus more likely to be the first occupants) across all cities prioritized air quality. Present results suggest that (perceived) air quality is a key building performance aspect for UGS that needs to be communicated to prospective occupants as this will improve their attitudes and views toward UGS. This study highlights the importance of indoor air quality for the public.

    IMPORTANCE: Knowledge and attitude influence compliance and individuals' practices. The risk and protective factors associated with high compliance to these preventive measures are critical to enhancing pandemic preparedness.

    OBJECTIVE: This survey aims to assess differences in mental health, knowledge, attitudes, and practices (KAP) of preventive measures for COVID-19 amongst healthcare professionals (HCP) and non-healthcare professionals.

    DESIGN: Multi-national cross-sectional study was carried out using electronic surveys between May-June 2020.

    SETTING: Multi-national survey was distributed across 36 countries through social media, word-of-mouth, and electronic mail.

    PARTICIPANTS: Participants ≥21 years working in healthcare and non-healthcare related professions.

    MAIN OUTCOME: Risk factors determining the difference in KAP towards personal hygiene and social distancing measures during COVID-19 amongst HCP and non-HCP.

    RESULTS: HCP were significantly more knowledgeable on personal hygiene (AdjOR 1.45, 95% CI -1.14 to 1.83) and social distancing (AdjOR 1.31, 95% CI -1.06 to 1.61) compared to non-HCP. They were more likely to have a positive attitude towards personal hygiene and 1.5 times more willing to participate in the contact tracing app. There was high compliance towards personal hygiene and social distancing measures amongst HCP. HCP with high compliance were 1.8 times more likely to flourish and more likely to have a high sense of emotional (AdjOR 1.94, 95% CI (1.44 to 2.61), social (AdjOR 2.07, 95% CI -1.55 to 2.78), and psychological (AdjOR 2.13, 95% CI (1.59-2.85) well-being.

    CONCLUSION AND RELEVANCE: While healthcare professionals were more knowledgeable, had more positive attitudes, their higher sense of total well-being was seen to be more critical to enhance compliance. Therefore, focusing on the well-being of the general population would help to enhance their compliance towards the preventive measures for COVID-19.

    BACKGROUND/AIMS: To examine how measuring adherence at 3 weeks by self-report and pill counts compares to measurements at 7 weeks in a pre-randomization run-in period.

    METHODS: Study within a trial of an international parallel group randomized controlled trial (RCT) that compares spironolactone to placebo. Adults receiving dialysis enter an 8-week active run-in period with spironolactone. Adherence was assessed by both self-report and pill counts in a subgroup of participants at both 3 weeks and 7 weeks.

    RESULTS: 332 participants entered the run-in period of which 166 had complete data. By self-report, 146/166 (94.0%) and 153/166 (92.2%) had at least 80% adherence at 3 and 7 weeks respectively (kappa = 0.27 (95% C.I. 0.16 to 0.38). By pill counts, the mean (SD) adherence was 96.5% (16.1%) and 92.4% (18.2%) at 3 and 7 weeks respectively (r = 0.32) with a mean (SD) difference of 3.1% (17.8%) and a 95% limit of agreement from -31.7% to +37.9%. The proportion of adherent participants by self-report and pill counts at 3 weeks agreed in 87.4% of participants (McNemar's p-value 0.58, kappa 0.11, p = 0.02) and at 7 weeks agreed in 92.2% (McNemar's p-value 0.82, kappa 0.47, p

    Nano/microplastics (NPs/MPs), a tiny particle of plastic pollution, are known as one of the most important environmental threats to marine ecosystems. Wastewater treatment plants can act as entrance routes for NPs/MPs to the aquatic environment as they breakdown of larger fragments of the plastic component during the treatment process; therefore, it is necessary to remove NPs/MPs during the wastewater treatment process. In this study, understanding the effect of water shear force on the fragmentation of larger size MPs into smaller MPs and NPs and their removal by air flotation and nano-ferrofluid (i.e., magnetite and cobalt ferrite particle as a coagulant) and membrane processes were investigated as a proof-of-concept study. It is found that a two-blade mechanical impeller could fragment MPs from 75, 150 and 300 μm into mean size NPs/MPs of 0.74, 1.14 and 1.88 μm, respectively. Results showed that the maximum removal efficiency of polyethylene, polyvinyl chloride and polyester was 85, 82 and 69%, respectively, in the air flotation process. Increasing the dose of behentrimonium chloride surfactant from 2 to 10 mg/L improved the efficiency of the air flotation process for NPs/MPs removal. It is also found that the removal efficiency of NPs/MPs by the air flotation system depends on solution pH, size, and types of NPs/MPs. This study also found a less significant removal efficiency of NPs/MPs by both types of ferrofluid used in this study with an average removal of 43% for magnetite and 55% for cobalt ferrite. All three plastics tested had similar removal efficiency by the nano-ferrofluid particles, meaning that this removal technique does not rely on the plastic component type. Among all the process tested, both ultrafiltration and microfiltration membrane processes were highly effective, removing more than 90% of NPs/MPs fragment particles. Overall, this study has confirmed the effectiveness of using air flotation and the membrane process to remove NPs/MPs from wastewater.

    Alcohol is a global risk factor for road trauma. Although drink driving has received most of the scholarly attention, there is growing evidence of the risks of alcohol-impaired walking. Alcohol-impaired pedestrians are over-represented in fatal crashes compared to non-impaired pedestrians. Additionally, empirical evidence shows that alcohol intoxication impairs road-crossing judgements. Besides some limited early research, much is unknown about the global prevalence and determinants of alcohol-impaired walking. Understanding alcohol-impaired walking will support health promotion initiatives and injury prevention. The present investigation has three aims: (1) compare the prevalence of alcohol-impaired walking across countries; (2) identify international groups of pedestrians based on psychosocial factors (i.e., Theory of Planned Behaviour (TPB) and perceptions of risk); and (3) investigate how segments of pedestrians form their intention for alcohol-impaired walking using the extended TPB (i.e. subjective norm, attitudes, perceived control, and perceived risk). A cross-sectional design was applied. The target behaviour question was "have you been a pedestrian when your thinking or physical ability (balance/strength) is affected by alcohol?" to ensure comparability across countries. Cluster analysis based on the extended TPB was used to identify groups of countries. Finally, regressions were used to predict pedestrians' intentions per group. A total of 6,166 respondents (Age M(SD) = 29.4 (14.2); Males = 39.2%) completed the questionnaire, ranging from 12.6% from Russia to 2.2% from Finland. The proportion of participants who reported never engaging in alcohol-impaired walking in the last three months ranged from 30.1% (Spain) to 83.1% (Turkey). Four groups of countries were identified: group-1 (Czech Republic, Spain, and Australia), group-2 (Russia and Finland), group-3 (Japan), and group-4 (final ten countries including Colombia, China, and Romania). Pedestrian intentions to engage in alcohol- impaired walking are predicted by perceptions of risk and TPB-psychosocial factors in group-1 and group-4. Favourable TPB-beliefs and low perceived risk increased alcohol-impaired walking intentions. Conversely, subjective norms were not significant in group-2 and only perceived risk predicted intention in group-3. The willingness of pedestrians to walk when alcohol-impaired differs significantly across the countries in this study. Perceived risk was the only common predictor among the 16 countries.

    Copper (Cu) ion in wastewater is considered as one of the crucial hazardous elements to be quantified. This research is established to predict copper ions adsorption (Ad) by Attapulgite clay from aqueous solutions using computer-aided models. Three artificial intelligent (AI) models are developed for this purpose including Grid optimization-based random forest (Grid-RF), artificial neural network (ANN) and support vector machine (SVM). Principal component analysis (PCA) is used to select model inputs from different variables including the initial concentration of Cu (IC), the dosage of Attapulgite clay (Dose), contact time (CT), pH, and addition of NaNO3 (SN). The ANN model is found to predict Ad with minimum root mean square error (RMSE = 0.9283) and maximum coefficient of determination (R2 = 0.9974) when all the variables (i.e., IC, Dose, CT, pH, SN) were considered as input. The prediction accuracy of Grid-RF model is found similar to ANN model when a few numbers of predictors are used. According to prediction accuracy, the models can be arranged as ANN-M5> Grid-RF-M5> Grid-RF-M4> ANN-M4> SVM-M4> SVM-M5. Overall, the applied statistical analysis of the results indicates that ANN and Grid-RF models can be employed as a computer-aided model for monitoring and simulating the adsorption from aqueous solutions by Attapulgite clay.

    Heavy metals in water and wastewater are taken into account as one of the most hazardous environmental issues that significantly impact human health. The use of biochar systems with different materials helped significantly remove heavy metals in the water, especially wastewater treatment systems. Nevertheless, heavy metal's sorption efficiency on the biochar systems is highly dependent on the biochar characteristics, metal sources, and environmental conditions. Therefore, this study implicates the feasibility of biochar systems in the heavy metal sorption in water/wastewater and the use of artificial intelligence (AI) models in investigating efficiency sorption of heavy metal on biochar. Accordingly, this work investigated and proposed 20 artificial intelligent models for forecasting the sorption efficiency of heavy metal onto biochar based on five machine learning algorithms and bagging technique (BA). Accordingly, support vector machine (SVM), random forest (RF), artificial neural network (ANN), M5Tree, and Gaussian process (GP) algorithms were used as the key algorithms for the aim of this study. Subsequently, the individual models were bagged with each other to generate new ensemble models. Finally, 20 intelligent models were developed and evaluated, including SVM, RF, M5Tree, GP, ANN, BA-SVM, BA-RF, BA-M5Tree, BA-GP, BA-ANN, SVM-RF, SVM-M5Tree, SVM-GP, SVM-ANN, RF-M5Tree, RF-GP, RF-ANN, M5Tree-GP, M5Tree-ANN, GP-ANN. Of those, the hybrid models (i.e., BA-SVM, BA-RF, BA-M5Tree, BA-GP, BA-ANN, SVM-RF, SVM-M5Tree, SVM-GP, SVM-ANN, RF-M5Tree, RF-GP, RF-ANN, M5Tree-GP, M5Tree-ANN, GP-ANN) are introduced as the novelty of this study for estimating the heavy metal's sorption efficiency on the biochar systems. Also, the biochar characteristics, metal sources, and environmental conditions were comprehensively assessed and used, and they are considered as a novelty of the study as well. For this aim, a dataset of sorption efficiency of heavy metal was collected and processed with 353 experimental tests. Various performance indexes were applied to evaluate the models, such as RMSE, R2, MAE, color intensity, Taylor diagram, box and whiskers plots. This study's findings revealed that AI models could predict heavy metal's sorption efficiency onto biochar with high reliability, and the efficiency of the ensemble models is higher than those of individual models. The results also reported that the SVM-ANN ensemble model is the most superior model among 20 developed models. The predictive model proposed that heavy metal's efficiency sorption on biochar can be accurately forecasted and early warning for the water pollution by heavy metal.

    Polychlorinated dibenzo-p-dioxins (PCDDs) and polychlorinated dibenzofurans (PCDFs) are toxic compounds derived from anthropogenic sources that stay in the environment for long periods. Ambient air has become the most important pathway for the transfer of PCDDs/PCDFs from emission sources to the environment. This review intends to summarise the information available on atmospheric PCDDs/PCDFs in the countries of Southeast Asia to provide a detailed description of the trends in PCDDs/PCDFs emissions, key sources, and levels in urban, rural, and industrial air as reported in peer-reviewed literature since 2000 and by the United Nations Environment Programme. As the largest country in Southeast Asia, Indonesia is the major PCDDs/PCDFs emitter, accounting for 72.81% of the total release of PCDDs/PCDFs in the air from all available inventories in this region, while Brunei Darussalam is the lowest emitter, contributing to less than 0.02%. Open burning processes have become the largest source of ambient PCDDs/PCDFs in the region (69.62%), followed by waste incineration (10.69%), and ferrous and non-ferrous metal production (8.78%). PCDDs/PCDFs levels in rural areas ranged between 10 and 38 fg TEQ m-3; however, where open burning waste has occurred, the levels rose to 12-29 times higher. In urban areas, ambient levels were 15 times greater than in rural areas, varying from 23 to 565 fg TEQ m-3. Atmospheric concentrations near industrial palm oil and waste incinerator sites were between 64 and 1530 fg TEQ m-3. The non-cancer risk of ambient exposure to PCDDs/PCDFs through inhalation is low among populations near facilities emitting PCDDs/PCDFs. The lack of local technical capacity, the high economic costs, and the lack of established human resource capacities have been the major challenges in conducting ambient PCDDs/PCDFs studies in most countries in the region.

    Seagrasses have the ability to contribute towards climate change mitigation, through large organic carbon (Corg) sinks within their ecosystems. Although the importance of blue carbon within these ecosystems has been addressed in some countries of Southeast Asia, the regional and national inventories with the application of nature-based solutions are lacking. In this study, we aim to estimate national coastal blue carbon stocks in the seagrass ecosystems in the countries of Southeast Asia including the Andaman and Nicobar Islands of India. This study further assesses the potential of conservation and restoration practices and highlights the seagrass meadows as nature-based solution for climate change mitigation. The average value of the total carbon storage within seagrass meadows of this region is 121.95 ± 76.11 Mg ha-1 (average ± SD) and the total Corg stock of the seagrass meadows of this region was 429.11 ± 111.88 Tg, with the highest Corg stock in the Philippines (78%). The seagrass meadows of this region have the capacity to accumulate 5.85-6.80 Tg C year-1, which accounts for $214.6-249.4 million USD. Under the current rate of decline of 2.82%, the seagrass meadows are emitting 1.65-2.08 Tg of CO2 year-1 and the economic value of these losses accounts for $21.42-24.96 million USD. The potential of the seagrass meadows to the offset current CO2 emissions varies across the region, with the highest contribution to offset is in the seagrass meadows of the Philippines (11.71%). Current national policies and commitments of nationally determined contributions do not include blue carbon ecosystems as climate mitigation measures, even though these ecosystems can contribute up to 7.03% of the countries' reduction goal of CO2 emissions by 2030. The results of this study highlight and promote the potential of the southeast Asian seagrass meadows to national and international agencies as a practical scheme for nature-based solutions for climate change mitigation.

    BACKGROUND: The 2013 Malaysian Clinical Practice Guidelines on the Management of Osteoarthritis (OA) recommend a linear step-up approach to manage knee OA. However, patients with knee OA often require a multimodal approach to address OA-related pain symptoms and functional limitations. This consensus aimed to provide doctors with an updated set of evidence-based, clinical experience-guided recommendations to manage knee OA.

    METHODS: A multi-speciality expert panel consisting of nine Malaysian physicians from different healthcare settings who manage a diverse OA patient population was convened. Using a combination of the ADAPTE process and modified Delphi method, the panel reviewed current evidence on the management of knee OA and synthesised a set of nine recommendations on the management of knee OA, supported by an algorithm that summarises the consensus' core messages.

    RESULTS: A multimodal intervention strategy is the mainstay of OA management and the choice of any single or multimodal intervention may vary over the course of the disease. Overall, a non-pharmacological core treatment set of patient education, weight loss and exercise is recommended for all patients. When pharmacotherapy is indicated, symptomatic slow-acting drugs for osteoarthritis are recommended at the early stage of disease, and they can be paired with physical therapy as background treatment. Concurrent advanced pharmacotherapy that includes non-steroidal anti-inflammatory drugs, intraarticular injections and short-term weak opioids can be considered if patients do not respond sufficiently to background treatment. Patients with severe symptomatic knee OA should be considered for knee replacement surgery. Management should begin with specific treatments with the least systemic exposure or toxicity, and the choice of treatment should be determined as a shared decision between patients and their team of healthcare providers.

    CONCLUSIONS: This consensus presents nine recommendations that advocate an algorithmic approach in the management of patients living with knee OA. They are applicable to patients receiving treatment from primary to tertiary care providers in Malaysia as well as other countries.

    INTRODUCTION: Citrus hystrix is widely used by Indonesians as a traditional medicine for gastrointestinal diseases, including Salmonella spp. infection. We investigated the antibacterial activity of the ethanolic peel extract of C. hystrix against Salmonella typhimurium.

    METHODS: The antibacterial activity was evaluated both in vitro and in vivo. The minimum inhibitory concentration (MIC) of the extract was determined at a concentration of 0.625% by agar dilution assay. Later, the in vivo antibacterial activity was examined by the administration of 16mg of the extract daily for three consecutive days in a mouse model infected with S. typhimurium.

    RESULTS: The bacterial loads of S. typhimurium in the ileum, liver, and spleen decreased after 24h of administration of the extract (p=0.00008, p=0.00084, and p=0.00003, respectively).

    CONCLUSION: The ethanolic peel extract of C. hystrix shows antibacterial activity against S. typhimurium, indicating the potential of C. hystrix as an effective treatment for Salmonella spp. infection.
