    Chronic hyperglycemia increases oxidative stress, activates inflammatory pathways and reduces nerve growth factor (NGF) among diabetic patients, which contribute to development of diabetic peripheral neuropathy (DPN). Tocotrienol-Rich Vitamin E (Tocovid) possesses potent antioxidant and anti-inflammatory properties which are postulated to target these pathogeneses in order to ameliorate DPN. This study aims to evaluate the effects of Tocovid on nerve conduction parameters and serum biomarkers among diabetic patients. This multicenter, prospective, randomized, double-blind, placebo-controlled clinical trial was conducted on 80 eligible participants. The intervention group (n = 39) was randomly allocated to receive 200 mg of Tocovid twice a day, and the control group (n = 41) received placebo twice a day. At the end of eight weeks, the nerve conduction parameters, as assessed by nerve conduction study, as well as serum biomarkers (NGF, malondialdehyde, vascular cell adhesion molecule 1, tumor necrosis factor receptor 1 and thromboxane B2) were compared between the two groups. Compared to placebo, Tocovid significantly improves the nerve conduction velocities of all nerves (+1.25 m/s, interquartile range [IQR] 3.35, p < 0.001, median nerve; +1.60 m/s, IQR 1.80, p < 0.001, sural nerve; +0.75 m/s, IQR 2.25, p < 0.001, tibial nerve). Meanwhile, the levels of serum NGF were significantly higher in the Tocovid group as compared to placebo at eight weeks post-intervention. Participants receiving Tocovid illustrated highly significant improvement in terms of nerve conduction velocities for all nerves tested after eight weeks of supplementation. In addition, Tocovid supplementation elevated the levels of serum NGF, in which its increase is postulated to reflect enhanced neuronal functions. This novel finding suggests that Tocovid could be a disease-modifying agent targeting serum NGF to improve nerve conduction velocities.

    Diabetic peripheral neuropathy (DPN) is the most common microvascular complication of diabetes that affects approximately half of the diabetic population. Up to 53% of DPN patients experience neuropathic pain, which leads to a reduction in the quality of life and work productivity. Tocotrienols have been shown to possess antioxidant, anti-inflammatory, and neuroprotective properties in preclinical and clinical studies. This study aimed to investigate the effects of tocotrienol-rich vitamin E (Tocovid SuprabioTM) on nerve conduction parameters and serum biomarkers among patients with type 2 diabetes mellitus (T2DM). A total of 88 patients were randomized to receive 200 mg of Tocovid twice daily, or a matching placebo for 12 months. Fasting blood samples were collected for measurements of HbA1c, renal profile, lipid profile, and biomarkers. A nerve conduction study (NCS) was performed on all patients at baseline and subsequently at 2, 6, 12 months. Patients were reassessed after 6 months of washout. After 12 months of supplementation, patients in the Tocovid group exhibited highly significant improvements in conduction velocity (CV) of both median and sural sensory nerves as compared to those in the placebo group. The between-intervention-group differences (treatment effects) in CV were 1.60 m/s (95% CI: 0.70, 2.40) for the median nerve and 2.10 m/s (95% CI: 1.50, 2.90) for the sural nerve. A significant difference in peak velocity (PV) was also observed in the sural nerve (2.10 m/s; 95% CI: 1.00, 3.20) after 12 months. Significant improvements in CV were only observed up to 6 months in the tibial motor nerve, 1.30 m/s (95% CI: 0.60, 2.20). There were no significant changes in serum biomarkers, transforming growth factor beta-1 (TGFβ-1), or vascular endothelial growth factor A (VEGF-A). After 6 months of washout, there were no significant differences from baseline between groups in nerve conduction parameters of all three nerves. Tocovid at 400 mg/day significantly improve tibial motor nerve CV up to 6 months, but median and sural sensory nerve CV in up to 12 months of supplementation. All improvements diminished after 6 months of washout.

    Sebanyak 6 jenis sampel nuget ikan komersial yang dipasarkan di Malaysia telah ditentukan komposisi proksimat (kandungan air, protein, lemak, abu dan karbohidrat), warna, kehilangan memasak, keupayaan memegang air, aktiviti air dan pH. Hasil analisis menunjukkan terdapat perbezaan bererti (p<0.05) bagi komposisi proksimat dan ciri-ciri warna, kehilangan memasak dan keupayaan memegang air. Kandungan air, protein, lemak, abu dan karbohidrat masing-masing berada dalam julat 58.97-64.58%, 9.30-11.60%, 0.14-5.23%, 1.95-2.84% dan 22.81-23.69%. Nilai L*, a* dan b* bagi warna, masing-masing adalah antara 49.72-56.80, 16.64-19.57 dan 38.40-59.58. Walau bagaimanapun tiada perbezaan yang bererti (p>0.05)untuk nilai aktiviti air dan pH antara sampel nuget ikan.

    OBJECTIVE: The aim of this study was to determine risk factors and prevalence of diabetic neuropathy (DN) among type II diabetic patients in Malaysian hospital setting.
    SUBJECTS AND METHODS: a observational prospective longitudinal follow up study design was selected, total no of respondents were 1077 type 2 diabetes mellitus outpatients recruited via attended the diabetes clinics at Hospital Universiti Sains Malaysia (HUSM) in Kelantan. The diagnosis of neuropathy was confirmed by nerve conduction studies. Logistic regression analysis was used to assess the independent variables that affect the development of neuropathy.
    RESULTS: The prevalence of nephropathy is 54.3%. Longitudinal logistic regression identified four predictive variables on the development and progression of diabetic neuropathy that are: duration of diabetes, retinopathy, HbA1c at second visit, and creatinine clearance third visit.
    CONCLUSION: Findings of this study show high prevalence of diabetic neuropathy. HbA1c and creatinine clearance are two modifiable risk factors for the development of diabetic neuropathy.

    Pembinaan model geometri berbantukan komputer (CAGD) dengan titik data yang mempunyai ketakpastian adalah sukar dan mencabar. Dalam kertas ini, pembinaan model splin-B kabur sebagai perwakilan matematik bagi lengkung dengan data ketakpastian menggunakan titik kawalan kabur dan titik kawalan penyahkaburan dibincangkan. Lengkung splin-B kabur atau splin-B penyahkaburan kubik untuk masalah data ketakpastian akan diperihalkan dengan menggunakan kaedah penghampiran splin-B kubik yang ditakrif menerusi titik kawalan kabur dan titik kawalan penyahkaburan. Bagi menyelesaikan masalah mengenai titik data ketakpastian pula, kaedah pengkaburan dan penyahkaburan titik data berkomponen kabur (penyahkaburan) beserta modelnya diperkenalkan. Bagi menguji tahap keberkesanan model, beberapa contoh lengkung simulasi data tersebut juga dibincangkan.

    Suatu kajian mengenai pemencilan dan pengenalpastian mikroflora bakteria dari kolam air panas telah dilakukan di kolam air panas Bentong, Pahang. Kebanyakan bakteria yang dipencil dari kolam air panas adalah dari Bacillus sp. kerana ia mempunyai ciri adaptasi yang membolehkannya hidup di kawasan suhu yang tinggi dengan pembentukan endospora. Objektif kajian adalah untuk mengenal pasti spesies bakteria selain daripada spesies Bacillus sp. yang boleh dipencilkan dari kolam air panas. Tiga buah kolam air panas (A, B dan C) dikenal pasti dengan julat suhunya antara 36°C-52°C dan nilai pH antara 7.88-8.65. Kajian ini telah berjaya memencilkan dua pencilan bakteria kokus Gram positif dan tiga spesies basilus Gram negatif. Spesies Gram positif tersebut adalah dari genus Streptococcus dan Staphylococcus, manakala bagi Gram negatif pula terdapat masalah dalam pengenalpastian dan hanya dikenali sebagai X, XI and XII.

    Objektif kajian ini adalah untuk menunjukkan pembangunan kaedah berangka untuk menilai keboleharapan cerun tanah dan menentukan kebarangkalian permukaan gelonsoran genting. Fungsi prestasi dirumuskan menggunakan kaedah had keseimbangan Bishop mudah untuk mengira indeks keboleharapan. Indeks keboleharapan yang ditakrifkan oleh Hasofer dan Lind digunakan sebagai indeks ukuran keselamatan. Pencarian permukaan gelonsoran genting secara kebarangkaliannya yang berkaitan dengan indeks keboleharapan terendah dirumuskan sebagai masalah pengoptimuman.
    Seterusnya pengoptimuman kelompok zarah digunakan untuk mengira indeks keboleharapan Hasofer dan Lind dan kebarangkalian permukaan kegagalan genting. Penggunaan algorithma ini ditunjukkan melalui tiga contoh berangka daripada kajian terdahulu. Keputusan menunjukkan bahawa kaedah yang dicadangkan berupaya untuk memberikan penyelesaian yang lebih baik bagi analisis keboleharapan cerun tanah berbanding dengan kaedah yang dilaporkan dalam kajian-kajian terdahulu.

    Peningkatan jumlah kehamilan luar nikah dalam kalangan remaja perempuan sehingga awal dewasa membimbangkan ramai pihak. Kemahilan luar nikah bukan sahaja boleh mengundang kepada risiko jangkitan HIV/AIDS, malah ia juga boleh membawa kepada masalah lain seperti pembuangan bayi dan pengguguran. Kajian ini dilakukan adalah untuk melihat sejauhmana tahap ganguan kognitif, kemurungan, kesunyian, penghargaan kendiri dan sokongan sosial dalam kalangan wanita hamil tanpa nikah. Dapatan kajian menunjukkan bahawa subjek kajian mempunyai tahap sokongan sosial yang sederhana, gangguan kognitif yang rendah; kemurungan, kesunyian dan penghargaan kendiri yang sederhana. Hal ini menunjukkan bahawa wanita hamil tanpa nikah ini masih kurang mendapat sokongan sosial yang sewajarnya dan cenderung untuk mengalami kemurungan dan kesunyian yang berpanjangan. Cadangan intervensi turut dibincangkan di akhir perbincangan kajian.

    Penilaian anestetik pra-operasi adalah penting semasa penjagaan perioperatif. Matlamat utama kajian ini adalah untuk menilai tahap kepuasaan pesakit dan faktor-faktor yang mempengaruhi kepuasaan pesakit terhadap klinik pra-anestetik. Sejumlah 304 orang pesakit yang menghadiri klinik pra-anestetik telah terlibat dalam kajian ini. Kajian kepuasaan selidik dwibahasa (Bahasa Inggeris dan Bahasa Melayu) dengan berjumlah 18 soalan menguji faktor-faktor bukan penyedia (kemudahan klinik, kesesuaian masa menunggu) dan faktor-faktor penyedia (perkhidmatan doktor, pegawai kaunter, jururawat dan kakitangan sokongan) telah dikaji. Kajian kepuasaan selidik menggunakan format skala Likert lima mata skor sangat setuju/tidak setuju telah digunakan untuk menilai tahap kepuasaan pesakit. Secara keseluruhan, peratusan kepuasaan pesakit terhadap penilaian pra-anestetik dilaporkan sebagai 98.7% dengan skor purata untuk perkhidmatan doktor (4.49 + 0.60), pegawai kaunter (4.48 + 0.61), kakitangan sokongan (4.47 + 0.62), kemudahan klinik dan kesesuaian masa menunggu (4.40 + 0.62).     Pengkomposan tandan buah kosong (EFB) dan sisa efluen kilang (POME) merupakan pendekatan lestari bagi menukarkan EFB dan POME kepada baja kompos. Pengkomposan adalah satu proses biologi yang memerlukan pengawalan yang optimum agar nutrien yang terhasil adalah tinggi dan stabil. Cabaran utama yang dihadapi oleh industri kompos ialah untuk menghasilkan kompos yang bernutrien tinggi dan stabil. Objektif kajian ini ialah untuk menganalisis prestasi jangka panjang bagi loji kompos terpilih melalui kandungan nutrien kompos yang dihasilkan. Penyelidikan ini mengkaji prestasi tiga tahun bagi empat loji kompos di Malaysia iaitu loji kompos Merotai, loji kompos Binuang, loji kompos Tanah Merah dan loji kompos Elphil. Prestasi kandungan nutrien yang diambil kira termasuklah nitrogen, kalium, nisbah karbon kepada nitrogen, kandungan kelembapan dan agregat kompos. Sebanyak 1,465 set data nutrien dianalisis secara deskriptif. Secara keseluruhannya, prestasi kandungan nutrien kesemua loji kompos yang dikaji berjaya memenuhi keperluan minimum industri sawit. Namun terdapat kes kompos yang dihasilkan mengandungi nutrien kalium yang tidak mencapai keperluan minimum industri dan kandungan kelembapan kompos yang tinggi.     BACKGROUND: Diabetic peripheral neuropathy (DSPN), a major form of diabetic neuropathy, is a complication that arises in long-term diabetic patients. Even though the application of machine learning (ML) in disease diagnosis is a very common and well-established field of research, its application in diabetic peripheral neuropathy (DSPN) diagnosis using composite scoring techniques like Michigan Neuropathy Screening Instrumentation (MNSI), is very limited in the existing literature.

    METHOD: In this study, the MNSI data were collected from the Epidemiology of Diabetes Interventions and Complications (EDIC) clinical trials. Two different datasets with different MNSI variable combinations based on the results from the eXtreme Gradient Boosting feature ranking technique were used to analyze the performance of eight different conventional ML algorithms.

    RESULTS: The random forest (RF) classifier outperformed other ML models for both datasets. However, all ML models showed almost perfect reliability based on Kappa statistics and a high correlation between the predicted output and actual class of the EDIC patients when all six MNSI variables were considered as inputs.

    CONCLUSIONS: This study suggests that the RF algorithm-based classifier using all MNSI variables can help to predict the DSPN severity which will help to enhance the medical facilities for diabetic patients.

    Transistor berasaskan karbon tiub nano (CNTFET) telah dihasilkan melalui kaedah pertumbuhan langsung menggunakan alkohol pemangkin pemendapan wap kimia. Sifat struktur dan ciri elektrik peranti telah dikaji. FET ini menunjukkan operasi ambikutub dan ia selaras dengan jenis-Schottky FET. Ia telah didapati bahawa ketinggian halangan Schottky yang wujud antara sentuhan elektrod dan CNT, boleh termodulat oleh aplikasi berkesan daripada voltan pincang (VDS) dan voltan get (VGS). Aplikasi voltan pincang sahaja tidak mudah untuk merendahkan ketinggian halangan Schottky dan ketebalan halangan Shottky, tetapi ia berkait rapat dengan aplikasi voltan get. Bagi konfigurasi peranti dalam kajian ini, keberkesanan medan boleh dikaitkan dengan VGS:VDS = 10:-1. Peningkatan arus didapati disebabkan oleh pengurangan tenaga pengaktifan. Kaitan yang jelas antara kesan voltan pincang, voltan get dan tenaga pengaktifan telah diperhati dalam kajian ini.

    Satu model matematik dibina bagi menilai pengurusan aktiviti pelancongan rekreasi di Wetland Putrajaya. Kajian ini menggunakan kaedah pengaturcaraan gol (PaG) dan perisian LINDO 6.1.untuk menyelesaikan masalah pelbagai objektif bagi memaksimumkan anggaran keuntungan aktiviti dan bilangan peserta yang terlibat. Tujuh aktiviti di Wetland Putrajaya yang terletak di bawah pengurusan Perbadanan Putrajaya telah dipilih sebagai kes kajian. Data dan maklumat rekod tahun 2008 dijadikan sebagai anggaran untuk kos dan bilangan peserta. Hasil kajian mendapati pihak Wetland Putrajaya boleh mencapai keuntungan lebih 40% daripada jumlah kos dan matlamat untuk memaksimumkan bilangan peserta bagi aktiviti yang terlibat juga tercapai.

    Diabetic amyotrophy also known as Bruns-Garland syndrome is diabetic neuropathy subtype affecting the lumbosacral nerve roots and peripheral nerves. There is an ongoing debate on the pathophysiology behind this condition whether is it ischaemic, metabolic (hyperglycaemia) or inflammatory. A 36-year-old man with uncontrolled diabetes mellitus complained of unprovoked weight lost and right proximal thigh discomfort with weakness for one week duration. In neurological examination, his right hip flexion was at medical research council (MRC) grade 3, right hip extension MRC grade 4, his knee and ankle flexion and extension are normal (MRC grade 5). The muscle tones and reflexes were normal. Sensation and proprioception were intact bilaterally. Nerve conduction study (NCS) showed markedly reduced amplitude of the compound muscle action potentials and sensory nerve action potentials, while conduction velocities show only mild slowing. He was started on a course of oral prednisolone 10 mg daily and improved gradually. At three months follow-up, his right lower limb power has recovered fully and he can walk without any assistance. Diabetic amyotrophy was confirmed by suggestive clinical features supported by electrophysiological findings of the affected nerves. This condition is due to metabolic derangement and vasculopathy or immune mediated nerve injury. So, the healthcare providers should be aware about this rare complication of diabetes.

    This study was undertaken to clinically estimate the prevalence of diabetic peripheral neuropathy amongst patients attending an outpatient clinic and to evaluate their risk factors for developing peripheral neuropathy. It was a cross-sectional study of 134 diabetes mellitus patients who attended the Primary Care Clinic, University Hospital, Kuala Lumpur. The patients were interviewed for their demographic data, past and present medical/surgical history, social history, personal habits and symptoms of peripheral neuropathy. Foot examination and clinical neurological tests were conducted and the presence of peripheral neuropathy was assessed. The main outcome measures were the Neuropathy Symptom Score and the Neuropathy Disability Score. The prevalence of diabetic peripheral neuropathy was found to be 50.7%. Peripheral neuropathy was related to the age of the patient and the duration of diabetes but did not seem to be significantly related to diabetic control. To conclude, there was a high prevalence of peripheral neuropathy amongst the diabetics in this study. These patients developed peripheral neuropathy at a younger age and shorter duration of diabetes compared to a similar study that was done in the UK.

    Despite the availability of various clinical trials that used different diagnostic methods to identify diabetic sensorimotor polyneuropathy (DSPN), no reliable studies that prove the associations among diagnostic parameters from two different methods are available. Statistically significant diagnostic parameters from various methods can help determine if two different methods can be incorporated together for diagnosing DSPN. In this study, a systematic review, meta-analysis, and trial sequential analysis (TSA) were performed to determine the associations among the different parameters from the most commonly used electrophysiological screening methods in clinical research for DSPN, namely, nerve conduction study (NCS), corneal confocal microscopy (CCM), and electromyography (EMG), for different experimental groups. Electronic databases (e.g., Web of Science, PubMed, and Google Scholar) were searched systematically for articles reporting different screening tools for diabetic peripheral neuropathy. A total of 22 studies involving 2394 participants (801 patients with DSPN, 702 controls, and 891 non-DSPN patients) were reviewed systematically.     With the pathophysiology not clearly understood and fewer than 130 cases having been reported in the literature, diabetic papillopathy presents a special challenge to the ophthalmologist. We report a case of a young patient with more than 12 years of type 1 diabetes mellitus (T1DM) on insulin with poor compliance to treatment who presented with sudden bilateral loss of vision. Ocular examination, fluorescence angiography (FA) and systemic signs were conclusive of diabetic papillopathy. His fasting blood sugar level was high and serum glycosylated haemoglobin (HbA1c) indicated a long term fluctuating blood glucose control. His vision initially improved with treatment, but later deteriorated with tight glycemic control.

    Poor glycaemic control and the duration of diabetes mellitus are known to accelerate development and progression of neuropathy. Diabetic co-morbidities: hypertension and hyperlipidaemia, have been postulated to associate with development of neuropathy. A diabetic foot with low temperature and frequent exposure to low temperature environment has recently been hypothesized to be at higher risk to develop early neuropathy. This cross-sectional study is undertaken to identify risk factors for diabetic neuropathy and the association between foot temperature and development of diabetic neuropathy by using simple clinical examination in the outpatient setting. From April 18, to April 30, 2005, universal sampling method was used to select 134 diabetic patients (type 1 or type 2 for >1 year) with peripheral neuropathy. Excluded are those with chronic alcoholism, drug-induced neuropathy, dietary history of vitamin B deficiency and family history of porphyria and hereditary sensorimotor neuropathy. The patient's duration of diabetes, glycaemic control status and the presence of co-morbids: hypertension and hyperlipidemia, were recorded. The temperature of the foot was measured by using thermo buddy. Of 134 patients representing Malaysian ethnic distribution with an equal number of males and females, 20.1% were in the age group of 61 to 65 years and, 85.1% and 67.9% belonged to lower socioeconomic and educational groups respectively. Associations between diabetic neuropathy and glycaemic control (p = 0.018) and duration of diabetes (p < 0.05) were significant. However, hypertension, hyperlipidaemia and low foot temperature were not significantly associated with development of diabetic neuropathy. Poor glycaemic control is significantly associated with diabetic neuropathy. Foot temperature alteration is merely an effect of autonomic neuropathy with a cold foot is attributed to co-existing peripheral arterial disease.

    A 68-year-old diabetic and hypertensive woman presented with chronic autonomic diarrhoea, syncope and palpitations which were associated with QT prolongation and recurrent episodes of torsade de pointes. She was on glibenclamide, indapamide and probucol (for type V hyperlipidaemia). Despite intravenous infusions of potassium, lignocaine and amiodarone, the unstable rhythm persisted. However, intravenous magnesium sulphate with small doses of intravenous propranolol terminated the torsade de pointes. She was stabilised but following discharge she relapsed, and upon re-admission, succumbed to intractable ventricular fibrillation. Early recognition and aggressive treatment of this condition is emphasised. Multiple aggravating factors ie autonomic diarrhoea resulting in severe potassium and magnesium depletion, kaliuretic effect of indapamide, probable QT prolongation associated with diabetic autonomic neuropathy and probucol; probable underlying coronary artery disease and heightened emotional and sympathetic discharge could have contributed to this very unstable ventricular arrhythmia and sudden death.

    Kekasaran permukaan satah ketakselanjaran adalah parameter penting untuk penilaian kestabilan struktur pengorekan dalam jasad batuan kerana ia mempengaruhi tingkah-laku ricihan dan nilai sudut geseran puncak satah tersebut. Beberapa kaedah boleh digunakan untuk menentukan kekasaran permukaan ketakselanjaran. Kertas ini memperkenalkan satu pendekatan yang mudah dalam bentuk tiga (3) persamaan polinominal untuk mengkorelasikan sudut geseran puncak ketakselanjaran (fp) batuan granit segar, terluluhawa sedikit dan terluluhawa sederhana dengan pekali kekasaran kekar, (JRC). Persamaan polinominal ketakselanjaran batuan granit segar ialah fp1 = -0.071JRC2 + 3.56JRC + 35.6. Persamaan polinominal ketakselanjaran batuan granit terluluhawa sedikit dan terluluhawa sederhana masing-masing ialah fp2 = -0.087JRC2 + 4.32JRC + 34.1 dan fp3 = -0.133JRC2 + 5.08JRC + 29. Hasil kajian ini memberikan satu kaedah alternatif penganggaran nilai fp satah ketakselanjaran daripada penentuan nilai PKK pada permukaan ketakselanjaran batuan granit di lapangan.
