The importance of epidemiology and epidemilogical knowledge of plantation health and disease in the planning of health care & effective management of a plantation is highlighted. The results of the survey of endemic diseases in the estates in Peninsular Malaysia are presented and compared with national disease patterns. The disease patterns in the plantations are similar to those for the country in general. Differences that exist are due to differences in the ecology of the plantations. The health effects of ecological changes consequent on development and progress are referred to. Most of the endemic communicable diseases encountered in the plantations can be prevented and controlled through the improvement of the micro environment of the plantations and the utilisation of simple available appropriate technologies for health care and services.
A study was undertaken to evaluate the efficiency of health services at two maternal and child health (MCH) clinics in Kuala Lumpur. The time spent for individual patient care by the clinic staff was used as an indicator for the evaluation. The main objective was to derive a model of ideal operation time for specific patient care activities at a MCH clinic. Primary data was collected through systematic random sampling of patients between 25/7/1985 and 31/7/1985. Secondary data was obtained from available clinic records for July, 1985. The time a patient spent at the clinic and the time she spent at each examination station and waiting between stations was recorded with digital watches. Results show tha patients spent 80% of their time at the clinics waiting. The actual time spent on each patient was found to be considerably less than the ideal time measured under ideal conditions. Various recommendation are presented in the paper to reduce waiting time and extend examination time. The ideal patient capacity for the two clinics was also calculated.
Socioeconomic development in Malaysia, over the past few decades, has led to the improvement and expansion of the public healthcare system. This system has provided universal access to a low-priced package of comprehensive health care leading Malaysia to claim to have achieved universal health coverage (UHC). However, the Malaysian health landscape is changing rapidly. Provision of private care has grown especially in large urban towns, mainly in response to public demand. Thus far, private care has been predominantly bought and utilised by the rich but because of differentials in quality of care between the public and private sector, unabated expansion of the private health sector has the potential to adversely affect universal access to care. This effect may be accentuated in the coming years by demographic changes in the country specifically by the ageing of the population. This paper is intended to highlight challenges to UHC in Malaysia in the face of the changing health landscape in the country and to offer some suggestions as to how these challenges can be met.
In 2018, our team collected aerosols samples from five poultry farms in Malaysia. Influenza D virus was detected in 14% of samples. One sample had an 86.3% identity score similar to NCBI accession number MH785020.1. This is the first molecular sequence of influenza D virus detected in Southeast Asia from a bioaerosol sample. Our findings indicate that further study of role of IDV in poultry is necessary.
Obsessive-compulsive disorder (OCD) is a common anxiety disorder which can be chronic and sustained. An OCD sufferer experiences intrusive and repetitive thoughts, impulses, and behaviors, which ultimately cause extreme discomfort. We report a case of a patient that primarily presented with lower urinary tract symptoms who was subsequently treated with antibiotics. Nonetheless, the symptoms persisted. In subsequent consultations, the patient clarified the compulsive nature of his symptoms and was treated as a case of OCD. Therefore, it is crucial for physicians to correctly identify the nature of the symptoms to manage the disorder properly and to avoid unnecessary consultation and treatment. To the best of our knowledge, this is the first report of other presentations of OCD.
Bronchial asthma causes great morbidity and mortality worldwide. Certain occupations especially those exposed to known triggers of asthma such as animal fur, dusts or solvents may trigger asthma attacks in a previously undiagnosed individual or worsen its’ control in a known asthmatic. This is especially true for adult-onset asthma. This may in turn the health of the affected workers and affect their productivity. Affected workers may be given job reassignment and eligible for medical compensation from Social Security Organisation (SOSCO). This case report will look at how two individuals in very distinct occupation were diagnosed with suspected occupational asthma.
Acanthaceae merupakan famili tumbuhan angiosperma di bawah order Lamiales yang terdiri daripada sekurang-kurangnya 4000 spesies sama ada spesies tropika atau subtropika. Spesies daripada famili ini ditemui di pelbagai habitat dan mempunyai pelbagai morfologi serta corak taburan geografi. Walau bagaimanapun, maklumat mengenai ciri anatomi bagi Acanthaceae masih dangkal sehingga ke hari ini. Objektif kajian ini ialah untuk mengenal pasti jenis trikom yang hadir pada permukaan epidermis adaksial dan abaksial sepal dan juga petal bunga bagi beberapa spesies terpilih daripada Acanthaceae di Semenanjung Malaysia. Kajian ini melibatkan pengumpulan sampel di lapangan, penyediaan spesimen baucer, teknik kajian epidermis petal, cerapan di bawah mikroskop cahaya dan juga cerapan di bawah mikroskop imbasan elektron. Tiga puluh jenis trikom dicerap dalam kajian ini dan daripada jumlah tersebut, 23 jenis trikom dicerap hadir pada permukaan epidermis petal manakala 17 jenis trikom dicerap hadir pada permukaan epidermis sepal. Jenis trikom yang direkodkan ialah trikom ringkas unisel dan ringkas multisel, trikom kelenjar kapitat dan kelenjar peltat serta juga trikom berlengan. Keputusan kajian ini menunjukkan kehadiran dan jenis trikom pada permukaan sepal dan petal mempunyai nilai taksonomi yang berguna untuk tujuan pembezaan dan pengecaman spesies. Maklumat ciri morfologi trikom yang diperoleh daripada kajian ini merupakan maklumat baharu ciri anatomi bunga bagi Acanthaceae.
The stability of the limestone cliff at Gunung Kandu, Gopeng, Perak, Malaysia was assessed based on the Slope Mass
Rating (SMR) system on 53 cross sections of the Gunung Kandu hill slopes. The slopes of Gunung Kandu were identified
as class I (very good) to IV (poor). The kinematic analysis showed that 12 out of 53 hill slopes of Gunung Kandu were
identified as having potential wedge, planar and toppling failures. The assessment showed that the stability of the western
flanks can be classified as stable to unstable with the probability of failure from 0.2 to 0.6. The stability of the eastern and
southern flanks range from very stable to partially stable with the probability of failure from 0.0 to 0.4. While the stability
of northern flanks are from very stable to stable with the probability of failure of 0.0 - 0.2. This systematic approach
offers a practical method especially for large area of rock slope stability assessment and the results from probability of
failure values will help engineers to design adequate mitigation measures.
The conventional use of aminoglycoside antibiotics has several disadvantages including the need for regular pre- and post-dose assaying and the risks of toxicity. Achieving a therapeutic and non-toxic serum concentration may be difficult in many patients especially those with severe sepsis. Correct timing of doses and assays is essential, but this is often difficult to achieve. Many of these difficulties may be remedied by the use of once daily dosing. This dosing schedule appears to be equally effective as the conventional method and i s also less toxic. A restropective study on a case of recurent salmonellosis in a 3 month old child due to Salmonella bovismorbificans and Salmonella matopeni was carried out using pulsed field gel electrophoresis (PFGE) and antibiotic susceptibility analysis. PFGE clearly distinguished the two serovars and that the recurrent infection was shown to associated with variant forms of Salmonella bovismorbificans. The chromosomal changes detected among the seqential isolates of Salmonella bovismorbificans appeared to be associated with varing antibiogram patterns. The study also showed that the recurrent infection in the patient could be related to prolonged antibiotic therapy. A case of chronic fatigue syndrome (CFS) like illness was identified recently. Diagnosis CFS is commonly used in the western countries but not in Malaysia or other parts of Asia. It is probably because the diagnosis of neurasthenia has gradually disappeared especially in the United States and United Kingdom. Neurasthenia is dropped in the DSM-111 and DSM-1V but is still retained in the 10th International Classification of Disease. This paper is reporting the case and discussing the definition of chronic fatigue syndrome. (JUMMEC 2000; 2:103-104).
A common practice in psychiatry when treating patients is the concurrent administration of anticholinergics along with antipsychotics, either to prevent or treat extrapyramidal syndrome reactions from occurring. However, most antipsychotics have inherent anticholinergic properties themselves. Therefore, this subtype of these patients have a higher than usual risk of developing anticholinergic side-effects, of which the central nervous manifestations can mimic psychosis, and may cloud judgement on patients’ progress towards their treatment.
We evaluated the use of Desflurane anaesthesia in this prospective observational audit in the University of Malaya Medical Centre Day Surgery Unit. Fifty ASA I-II unpremedicated day surgery patients received fentanyl and propofol induction after preoxygenation. Desflurane was introduced initially at 2% and the concentration was increased gradually to 4%, then 6%, 8% and 10% in nitrous oxide and oxygen. Patients breathed spontaneously throughout the surgery. Desflurane was switched off at the end of surgery and patients breathed 100% oxygen. The haemodynamic effect, perioperative complications and recovery profiles were recorded. Systolic arterial pressure and heart rate decreased after induction of anaesthesia but returned to baseline value at discharge. Adverse airway event such as coughing and postoperative nausea and vomiting are two unwanted complications. KEYWORDS: Desflurane, day surgery, propofol induction
Sexual health is defined as part of reproductive health and includes healthy sexual development; equitable and responsible relationships and sexual fulfillment; and freedom from illness, disease, disability, violence and other harmful practices related to sexuality. For this project on Health Problems of Migrant Workers, a section on sexual health was incorporated based on a special concern over sexually transmitted diseases (STDs), and high-risk behaviours. Access to healthcare is recognised to be a problem with minority or marginalised groups, including adolescents and migrant populations, especially undocumented migrants. This exacerbates the issue of control and management of STDs. The Sexual Health section covered experience with sexual interourse, and a limited number of questions related to sex partners and condom use among those who are sexually experienced.
