    BACKGROUND: There is very little literature available on the reliability of the rapid prototyping technology in the production of three-dimension (3D)-printed surgical guides for accurate implant placement.

    OBJECTIVES: The aim of the study was to evaluate the deviation of implant placement performed with a surgical guide fabricated by means of the rapid prototyping technique (the PolyJet™ technology).

    MATERIAL AND METHODS: Twenty sheep mandibles were used in the study. Pre-surgical cone-beam computed tomography (CBCT) scans were acquired for the mandibles by using the Kodak 9000 3D cone-beam system. Two implants with dimensions of 4 mm in diameter and 10 mm in length were virtually planned on the 3D models of each mandible by using the Mimics software, v. 16.0. Twenty surgical guides were designed and printed using the PolyJet technology. A total of 40 implants were placed using the surgical guides, 1 on each side of the mandible (2 implants per mandible). The post-surgical CBCT scans of the mandibles were performed and superimposed on the pre-surgical CBCT scans. The amount of deviation between the virtually planned placement and the actual implant placement was measured, and a descriptive analysis was done.

    RESULTS: The results showed that the mean deviation at the implant coronal position was 1.82 ±0.74 mm, the mean deviation at the implant apex was 1.54 ±0.88 mm, the mean depth deviation was 0.44 ±0.32 mm, and the mean angular deviation was 3.01 ±1.98°.

    CONCLUSIONS: The deviation of dental implant placement performed with a 3D-printed surgical guide (the PolyJet technology) is within the acceptable 2-millimeter limit reported in the literature.

    BACKGROUND: Unoptimized protocols, including a miscentered position, might affect the outcome of diagnostic in CT examinations. In this study, we investigate the effects of miscentering position during CT head examination on the signal-to-noise ratio (SNR) and contrast-to-noise ratio (CNR).

    METHOD: We simulate the CT head examination using a water phantom with a standard protocol (120 kVp/180 mAs) and a low dose protocol (100 kVp/142 mAs). The table height was adjusted to simulate miscentering by 5 cm from the isocenter, where the height was miscentered superiorly (MCS) at 109, 114, 119, and 124 cm, and miscentered inferiorly (MCI) at 99, 94, 89, and 84 cm. Seven circular regions of interest were used, with one drawn at the center, four at the peripheral area of the phantom, and two at the background area of the image.

    INTRODUCTION: Autopsy is one of the most important approaches to identify clearly the exact cause of death, whether it was due to natural causes, sudden death, or traumatic. Various studies have been done in different countries regarding ways to improve the diagnosis during autopsy. The imaging approach is one of the methods that has been used to complement autopsy findings and to enhance the diagnosis for achieving the most accurate post-mortem diagnosis. The aim of this study is to identify the role of imaging modalities that complement routine autopsy and correlate the findings of diagnostic imaging that can help improve the accuracy of diagnosing the cause of death.

    METHODS: We sourced articles from Scopus, Ovid and PubMed databases for journal publications related to post-mortem diagnostic imaging. We highlight the most relevant full articles in English that explain the type of modality that was utilised and the added value it provided for diagnosing the cause of death.

    RESULTS: Minimally invasive autopsies assisted by imaging modalities added a great benefit to forensic medicine, and supported conventional autopsy. In particular the role of post mortem computed tomography (PMCT), post mortem computed tomography angiography (PMMR) and positron emission tomography computed tomography (PMCTA) that have incremental benefits in diagnosing traumatic death, fractures, tissue injuries, as well as the assessment of body height or weight for corpse identification.

    CONCLUSION: PMCT and PMMR, with particular emphasis on PMCTA, can provide higher accuracy than the other modalities. They can be regarded as indispensable methods that should be applied to the routine autopsy protocol, thus improving the findings and accuracy of diagnosing the cause of death.

    Pyomyoma (suppurative leiomyoma) is a rare complication of uterine leiomyoma. It results from infarction and infection of a leiomyoma. It may lead to diagnostic and therapeutic difficulties with potential complications such as bacteraemia, uterine rupture, compressive effect to adjacent structures and even death. We report a case of pyomyoma based on the Ultrasound (US) and Computed Tomography (CT) findings and literature review on the subject.

    Ultrasound technology progressed through the 1960’s from simple A-mode and B-mode scans to today’s M-mode and Doppler two dimensional (2-D) and even three dimensional (3-D) systems. Modern ultrasound imaging has its roots in sonar technology after it was first described by Lord John Rayleigh over 100 years ago on the interaction of acoustic waves with media. Tomography technique was developed as a diagnostic tool in the medical area since the early of 1970’s. This research initially focused on how to retrieve a cross sectional images from living or non-living things. After a decade, the application of tomography systems span into the industrial area. However, the long exposure time of medical radiation-based method cannot tolerate the dynamic changes in industrial process two phase liquid/gas flow system.. An alternative system such as a process tomography system, can give information on the nature of the flow regime characteristic. The overall aim of this paper is to investigate the use of a small scale ultrasonic tomography method based on ultrasonic transmission mode tomography for online monitoring of liquid/gas flow in pipe/vessel system through ultrasonic transceivers application. This non-invasive technique applied sixteen transceivers as the sensing elements to cover the pipe/vessel cross section. The paper also details the transceivers selection criteria, hardware setup, the electronic measurement circuit and also the image reconstruction algorithm applied. The system was found capable of visualizing the internal characteristics and provides the concentration profile for the corresponding liquid and gas phases.

    A 50-year-old Chinese man presented with sudden onset of painful right eye, diplopia, and redness associated with headache and deteriorating vision. Examination revealed obvious proptosis with elevated intraocular pressure. Computed tomography (CT) scan showed presence of retrobulbar haemmorhage. Emergency lateral canthotomy and cantholysis was performed followed by medical orbital decompression, resulting in improvements in visual acuity, and other ocular symptoms. The diagnosis of thrombosed orbital varices involving inferior ophthalmic vein was confirmed on radiological- angiographic study. To date, he is symptoms-free with good visual acuity. Immediate surgical decompression with lateral cantholysis for retrobulbar haemorrhage was effective in the treatment of retrobulbar haemorrhage.

    Saccular aneurysms associated with moyamoya disease are commonly located in the vertebrobasilar circulation. Anterior circulation aneurysm associated with moyamoya disease is uncommon and is usually treated by neurosurgical clipping. Objective: We report a succesful treatment using the endovascular approach in a case of ruptured anterior communicating artery aneurysm in unilateral moyamoya disease. Clinical Presentation: A 23 year old man presented with a 5 day history of headache, diplopia and fever. Computed Tomography (CT) scan and cerebral angiogram showed a bilobed anterior comunicating artery aneurysm. There was also severe M1 segment stenosis of the left middle cerebral artery with multiple collaterals, representing moyamoya vessels. Intervention: Treatment was done under general anesthesia and followed the standard practice for endovascular treatment. The aneurysm was occluded with three detachable platinum microcoils (Microplex®, Microvention®). Conclusion: Endovascular treatment can be a treatment option for ruptured anterior circulation saccular aneurysms associated with moyamoya disease.

    Monoclonal gammopathies result from an overproduction of a single abnormal clone of plasma cell
    or B lymphocyte that produce an immunologically homogenous immunoglobulin (Ig) commonly referred to as paraprotein or monoclonal (M) protein. The circulating M-protein may consist of an intact immunoglobulin, the light chain only, or (rarely) the heavy chain only. The heavy chain is from one of the five immunoglobulin classes G, A, M, D or E, while the light chain is either kappa (κ) or lambda (λ) in type. Accurate detection and quantitation of monoclonal immunoglobulins is important for the diagnosis and management of monoclonal gammopathies. We report a case of a 71 year old lady with a history of chronic gastritis and recurrent lower respiratory tract infection whereby no specific diagnosis was made until a computed tomography (CT) guided lung biopsy and orogastroduodenoscopy (OGDS) 5 years later from the onset of initial symptoms revealed pulmonary and gastric amyloidosis, respectively.

    The purpose of this article is to demonstrate the appearance of active TB lymphadenitis using multimodality imaging apparatus. Multi-modality diagnostic imaging tools, including chest radiograph, Ultrasound (US), Computed Tomographic Scan (CT), Magnetic Resonance Imaging (MRI), and integrated 18F-FDG Positron Emission Tomography/CT examination, were performed to demonstrate TB lymphadenitis in the neck and superior mediastinum of a 26 year old female patient. There was widening of superior mediastinum on chest radiograph. Meanwhile, the ultrasound carried out detected superficial cystic lesions in the cervical region. The MRI found multiple gadolinium enhanced cervical and mediastinal lymphadenophaties. Contrast enhanced CT found heterogeneous enhancing lymphadenopathies in the same anatomical region. FDG PET/CT demonstrated a high metabolic activity in all lesions, as demonstrated by conventional imaging
    modalities. Mycobacterium tuberculosis was isolated from 1ml aspirate using US guidance. Post treatment FDG PET CT scan demonstrated a complete metabolic remission of active lesions FDG PET CT can be used to demonstrate metabolic activity of active TB lesions in addition to guide clinicians in treating TB lesions.

    PURPOSE: Segmentation of liver tumours is an important part of the 3D visualisation of the liver anatomy for surgical planning. The spatial relationship between tumours and other structures inside the liver forms the basis of preoperative surgical risk assessment. However, the automatic segmentation of liver tumours from abdominal CT scans is riddled with challenges. Tumours located at the border of the liver impose a big challenge as the surrounding tissues could have similar intensities.

    METHODS: In this work, we introduce a fully automated liver tumour segmentation approach in contrast-enhanced CT datasets. The method is a multi-stage technique which starts with contrast enhancement of the tumours using anisotropic filtering, followed by adaptive thresholding to extract the initial mask of the tumours from an identified liver region of interest. Localised level set-based active contours are used to extend the mask to the tumour boundaries.

    RESULTS: The proposed method is validated on the IRCAD database with pathologies that offer highly variable and complex liver tumours. The results are compared quantitatively to the ground truth, which is delineated by experts. We achieved an average dice similarity coefficient of 75% over all patients with liver tumours in the database with overall absolute relative volume difference of 11%. This is comparable to other recent works, which include semiautomated methods, although they were validated on different datasets.

    CONCLUSIONS: The proposed approach aims to segment tumours inside the liver envelope automatically with a level of accuracy adequate for its use as a tool for surgical planning using abdominal CT images. The approach will be validated on larger datasets in the future.

    This work evaluated and compared the absorbed doses to selected organs in the head and neck region from the three image guided radiotherapy systems: cone-beam computed tomography (CBCT) and kilovoltage (kV) planar imaging using the On-board Imager® (OBI) as well as the ExacTrac® X-ray system, all available on the Varian Novalis TX linear accelerator. The head and neck region of an anthropomorphic phantom was used to simulate patients' head within the imaging field. Nanodots optically stimulated luminescent dosemeters were positioned at selected sites to measure the absorbed doses. CBCT was found to be delivering the highest dose to internal organs while OBI-2D gave the highest doses to the eye lenses. The setting of half-rotation in CBCT effectively reduces the dose to the eye lenses. Daily high-quality CBCT verification was found to increase the secondary cancer risk by 0.79%.

    Introduction: External laryngotracheal (ELT) trauma is rarely encountered in clinical practice. In most
    circumstances, this injury is overlooked by the primary attending team. Surgical management of ELT trauma
    is complicated, because there is no established management approach for this potentially life-altering, high
    morbidity injury. It is important for this injury to be identified early, as any delay in surgical intervention
    may result in poor airway and phonatory outcomes. The aim of surgical reconstruction is to minimise the
    above debilitating morbidities by restoring the main laryngeal functions as much as possible. Methods: We
    reviewed the outcomes of six surgical interventions for ELT trauma at Tengku Ampuan Afzan Hospital from
    June 2007 to June 2014. Clinical presentations, computed tomography (CT) scans features, intraoperative
    findings, and postoperative outcomes were evaluated. Results: All patients made a good recovery in terms of
    phonation except for one patient who had reduced speech function. After one year, one patient was still
    dependent on a fenestrated tracheostomy. This article describes the surgical reconstruction techniques used
    to achieve these positive outcomes. Stenting is helpful to aid healing and re-epithelialisation. Conclusion:
    Prompt recognition and non-traumatised airway control are essential for addressing laryngotracheal trauma.
    Subcutaneous emphysema is an important hallmark that should alert the attending physician to the
    possibility of ELT trauma. Immediate surgical intervention using appropriate techniques can produce
    favorable patient outcomes.

    AIM: To investigate the number of roots and root canal configurations using two coding systems and the root canal diverging and merging levels in extracted maxillary premolars in an Egyptian subpopulation using cone beam computed tomography (CBCT).

    METHODOLOGY: A total of 700 maxillary premolars were examined using CBCT in an Egyptian subpopulation. The number of roots was identified, and root canal configurations were classified according to Vertucci's classification and a new system for classifying root and canal morphology. In addition, the position where roots bifurcated and the levels where canals merged or diverged were identified. Fisher's exact test and independent t-test were used for statistical analysis, and the level of significance was set at 0.05 (P = 0.05).

    Ureteric and bladder injuries are uncommon, difficult to diagnose and rarely occur in isolation. Diagnosis is often delayed or missed at presentation. Therefore, high clinical suspicion and appropriate timing of computed tomography (CT) are of paramount importance. We report two cases (ureteropelvic junction avulsion and ruptured dome of bladder) whereby the presentations were subtle and would have been missed if not for high clinical suspicion. This article discusses the problems associated with these urologic injuries, as well as how to develop a high index of suspicion based on the pattern of anatomical disruption, mechanism of injury, physiological abnormality and comorbidity.

    Objectives: The risk of damaging the mandibular incisive canal (MIC) during surgery in the anterior mandible should not be overlooked. Hence, preoperative radiographic assessment is essential to avoid complications. This study was aimed to estimate the length of the MIC in the interforaminal safe zone, to analyse its course in relation to the lingual and the buccal cortical plates of the mandible using cone beam computed tomography (CBCT) scans and to relate the above findings to age, gender, dental status and Malaysian races.

    Methods: Retrospective analysis of 100 CBCT scans (n = 200) was performed on both sides of the mandible. Cross-sectional and panoramic images were reconstructed. The length of the MIC and the horizontal distances between the MIC and the buccal and the lingual cortical plates were measured at the three different points (starting, mid-, end points). Independent samples t-test and one-way ANOVA test were used to analyse the variation in the length and course of the MIC in gender, age, dental status and Malaysian races.

    Imaging modality has become increasingly important in hospital setting especially in cases of unknown site of infection/pyrexia of unknown origin (PUO) and osteomyelitis (OM). In recent years, nuclear imaging has been used and is known to deliver prompt and precise diagnoses of numerous infectious diseases. The purpose of the study is to detect and localise the site of infection using Tc99m-besilesomab and to assess the added contributions of single photon emission computed tomography/computed tomography (SPECT/CT) over planar scan in patients with PUO and OM. Methods: Tc99m-besilesomab with SPECT/CT were prospectively performed in 23 patients (eight males, 15 females) with suspected infection. True findings were diagnosed by both cold and hot spot in the scan with reference to positive blood or tissue cultures, or other additional imaging. Results: Tc99m-besilesomab managed to detect presence of infection with high sensitivity of 87.5% and specificity of 71.4%.     The use of computed tomography (CT) in emergency departments has increased over several decades, as physicians increasingly depend on imaging for diagnoses. Patients and medical personnel are put at risk due to frequent exposure to and higher levels of radiation, with very little evidence of improvements in outcomes. Here, we explore why CT imaging has a tendency to be overused in emergency departments and the obstacles that medical personnel face in ensuring patient safety. The solution requires cooperation from all emergency care stakeholders as well as the continuous education of doctors on how CT scans help in particular cases.

    Orbital fractures pose specific challenge in its surgical management. One of the greatest challenges is to obtain satisfactory reconstruction by correct positioning of orbital implant. Intraoperative computed tomography (CT) scan may facilitate this procedure. The aim of this study was to describe the early use of intraoperative CT in orbital fractures repair in our center. The authors assessed the revision types and rates that have occurred with this technique. With the use of pre-surgical planning, optical intraoperative navigation, and intraoperative CT, the impact of intraoperative CT on the management of 5 cases involving a total number of 14 orbital wall fractures were described. There were 6 pure orbital blowout wall fractures reconstructed, involving both medial and inferior wall of the orbit fracturing the transition zone and 8 impure orbital wall fractures in orbitozygomaticomaxillary complex fracture. 4 patients underwent primary and 1 had delayed orbital reconstruction. Intraoperative CT resulted in intraoperative orbital implant revision, following final navigation planning position, in 40% (2/5) of patients or 14% (2/14) of the fractures. In revised cases, both implant repositioning was conducted at posterior ledge of orbit. Intraoperative CT confirmed true to original reconstruction of medial wall, inferior wall and transition zone of the orbit. Two selected cases were illustrated. In conclusion, intraoperative CT allows real-time assessment of fracture reduction and immediate orbital implant revision, especially at posterior ledge. As a result, no postoperative imaging was indicated in any of the patients. Long-term follow-ups for orbital fracture patients managed with intraoperative CT is suggested.

    BACKGROUND: Cone Beam Computed Tomography (CBCT) is a reliable radiographic modality to assess trabecular bone microarchitecture. The aim of this study was to determine the effect of CBCT image reconstruction parameters, namely, the threshold value and reconstruction voxel size, on trabecular bone microstructure assessment.

    METHODS: Five sectioned maxilla of adult Dorper male sheep were scanned using a CBCT system with a resolution of 76 μm3 (Kodak 9000). The CBCT images were reconstructed using different reconstruction parameters and analysed. The effect of reconstruction voxel size (76, 100 and 200 μm3) and threshold values (±15% from the global threshold value) on trabecular bone microstructure measurement was assessed using image analysis software (CT analyser version 1.15).

    RESULTS: There was no significant difference in trabecular bone microstructure measurement between the reconstruction voxel sizes, but a significant difference (Tb.N = 0.03, Tb.Sp = 0.04, Tb.Th = 0.01, BV/TV = 0.00) was apparent when the global threshold value was decreased by 15%.

    CONCLUSIONS: Trabecular bone microstructure measurements are not compromised by changing the CBCT reconstruction voxel size. However, measurements can be affected when applying a threshold value of less than 15% of the recommended global value.
