    Vaccination-induced optic neuritis is not common. The development of optic neuritis following various vaccinations have been reported, suggesting a possible association between optic neuritis and vaccination. Of those reported cases, influenza vaccines have been the most common. Although rare, those patients who developed optic neuritis following HPV vaccination also presented with other central nervous system (CNS) demyelinating syndromes, especially following a booster dose. We present a rare case of simultaneous isolated bilateral optic neuritis following the first dose of an HPV vaccination in a young child. She received treatment with a systemic corticosteroid that resulted in a good clinical outcome without developing any demyelinating disease.

    Protein glycosylation, particularly N-linked glycosylation, is a complex posttranslational modification (PTM), which plays an important role in protein folding and conformation, regulating protein stability and activity, cell-cell interaction, and cell signaling pathways. This review focuses on analytical techniques, primarily MS-based techniques, to qualitatively and quantitatively assess N-glycosylation while successfully characterizing compositional, structural, and linkage features with high specificity and sensitivity. The analytical techniques explored in this review include LC-ESI-MS/MS and MALDI time-of-flight MS (MALDI-TOF-MS), which have been used to analyze clinical samples, such as serum, plasma, ascites, and tissue. Targeting the aberrant N-glycosylation patterns observed in MALDI-MS imaging (MSI) offers a platform to visualize N-glycans in tissue-specific regions. The studies on the intra-patient (i.e., a comparison of tissue-specific regions from the same patient) and inter-patient (i.e., a comparison of tissue-specific regions between different patients) variation of early- and late-stage ovarian cancer (OC) patients identify specific N-glycan differences that improve understanding of the tumor microenvironment and potentially improve therapeutic strategies for the clinic.

    Online shopping has been preferred over the conventional shopping methods as the results of hectic
    lifestyles nowadays. It is a convenient way to get groceries without having to queue at the counters.
    The objective of this research is to study the online shopping practices and the factors that affect it.
    Apart from that, it is also to identify the difference in online shopping behaviour among selected
    background factors (sex, social status, generation, ethnic, household income, education level and zone).
    The populations of this research include consumers of 17 years old and above whose chosen by
    convenience through mall intercept method. T-test result shows there is significant difference (t=-
    3.156, p=0.001) for online shopping practice between men and women. Meanwhile, based on ANOVA result, it shows that there is significant difference between mean scores of different generation [F(3,
    1298)= 3.475, p = 0.016], ethnics [F(4, 1291)= 3.678, p = 0.006] and household income [F (4, 1227)=
    6.935, p = 0.000]. However, there is no significant difference among mean scores for education level,
    zones and stratum (p>0.05). Apart from that, the result also indicates that only five factors taken into
    consideration before a particular customer does the online shopping which are safety, products offered,
    risk, trustworthy and customers experience. Thus, the sellers should approach their customers in
    different ways in order to meet the demand, hence expanding their bussiness.

    Mitral stenosis (MS) affects left atrial (LA) function as a result of hemodynamic and myocardial factors that causes significant symptoms and complications. Conventional echocardiographic methods have been practicing to see the improvement of left atrial function after successful percutaneous mitral balloon valvuloplasty (PMBV). Introduction of tissue doppler imaging allows direct and non-invasive measurement of myocardial velocities. The aim of the study was to evaluate LA functions after PMBV using colour tissue doppler imaging. This cross sectional study was performed in Cardiology department of Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from March 2014 to February 2015. Forty six (46) patients (28 females, mean age: 28.96±5.78 years) presenting with mitral valve stenosis who fulfilled the indications for PMBV were included in the study. Within 24 hours before PMBV, all the patients underwent colour tissue doppler study in addition to routine conventional echocardiographic examinations. Late diastolic velocities (A') measured at the septal and lateral annuli were recorded. All the measurements were repeated 24 hours after PMBV. The PMBV was done using the Inoue technique. After PMBV mitral valve areas (MVA) were significantly increased. Maximum and mean gradients, LA diameter, LA area, LA volume, systolic pulmonary arterial pressure and mean LA pressures were decreased while septal and lateral A' were significantly (p<0.001) increased. Lateral and septal A' velocities were correlated with MVA and inversely related to LA pressure measured invasively during PMBV. Tissue doppler velocities illustrated improvement of left atrial systolic function after PMBV in relation to decreased mean left atrial pressure and increased mitral valve area. Therefore, tissue doppler Imaging is a useful tool to detect improvement of left atrial systolic function after PMBV in patients with mitral stenosis.

    Mammary Paget's disease is clinically defined as skin inflammation of the nipple area and is an adenocarcinoma of the epidermis of the nipple. The pathogenesis of mammary Paget's disease is relatively unknown; nonetheless, there are two popular theories that support the underlying carcinoma and de novo carcinogenesis. For the attending medical practitioner, mammary Paget's disease poses a diagnostic and therapeutic dilemma, especially in the absence of a clinically palpable breast mass. We report a rare case of a 48-year-old Malay woman who presented at Hospital Universiti Sains Malaysia, Kelantan, Malaysia with the symptom of skin erosion on the left nipple and unresponsiveness to multiple topical treatments. A full evaluation and assessment of the patient were conducted, and mammary Paget's disease was diagnosed.

    Radiofrequency ablation (RFA) of a hepatic tumour is an established treatment option with an acceptable complication rate. Formation of a pseudoaneurysm after RFA of liver metastasis is an uncommon complication. We report the case of a 69-year-old female patient developing a hepatic pseudoaneurysm after RFA of liver metastasis. On a follow-up CT scan 6 weeks later, there was spontaneous resolution of the pseudoaneurysm. Hepatic pseudoaneurysms are usually treated owing to the risk of rupture. Invasive procedures or conservative management of an asymptomatic hepatic pseudoaneurysm is still the subject of debate. The spontaneous resolution of a hepatic pseudoaneurysm in our patient suggests that an asymptomatic pseudoaneurysm maybe observed for resolution instead of being treated at presentation.

    The application of ultrasound technology has been widely accepted in clinical settings, particularly in Obstetrics and Gynaecology. This is in light of its ability to detect early foetal malformations apart from enabling foetal monitoring throughout gestation. While ultrasonography is an imaging method that is regularly used in Obstetrics, it is questionable as to whether it is safe for foetuses. The purpose of this paper was to review the evidence regarding the thermal effects of ultrasound exposure on foetal development, particularly. It is hoped that the importance of prudent usage of prenatal ultrasonography will be impressed on clinicians and the public in order to avoid the unnecessary usage of ultrasonography when it is not medically indicated. This is so that the welfare of pregnant women will be looked after, besides contributing to the better health of the next generation by ensuring that the benefits outweigh the known risks or potential harms.

    Blood donation is important for lives saving and the need is unceasing. However, the shortage of blood supply
    is a common issue. While the public is the main source of blood donation, it is unknown whether they are
    aware about it. This study was carried out to determine the public's knowledge, perceptions and barriers
    towards blood donation. Atotal of 384 male and female respondents were conveniently selected to participate
    in this study. Data was collected through self-administered questionnaire and analyzed with SPSS software
    version 22.0. Most of the respondents (60.9%) had never donated blood but their knowledge towards blood
    donation was high (79.9%). Some misconceptions were indicated among respondents. Afraid of the needle
    prick, pain or discomfort were the top barriers among non-donors respondents (45.3%). The majority of the
    respondents believed that blood donation is a practice or selfless concern for the well-being of others (98.7%).

    The awareness of the Muslim community on the importance of understanding breastfeeding according
    to Islamic perspective needs to be emphasized in promoting the implementation of breastfeeding in the
    best way. This proved that breast milk is the major food after birth and a mother should know the right
    and the regulation of breastfeeding in order to be in line with Islamic law. This article aims to discuss
    the level of awareness among the Muslim community in Selangor regarding breast milk donation. Data
    were collected through a questionnaire distributed to 400 respondents involving three locations in
    Selangor, namely in the northern zone (Hulu Selangor district), central zone (Klang district) and
    southern zone (Sepang district). The findings showed that the respondents had a moderate level of
    awareness regarding milk donation laws with average mean value of 3.26. This finding shows that the
    Muslim community in Selangor are concerned about the rights and the regulation of breastfeeding.

    Human resource development comprises skills, abilities, creativity and talent are amongst factors in
    human capital as well as emulous aspect. Hence, the emphasis on human capital development and
    emulous aspect is important to become a good leader for family, community and country. It is also
    important in ensuring entrepreneurs can compete in market economy today and they are able to meet
    customer demand. Thus, the objectives are the emphasis on the factors that are able to contribute in
    improving human capital and emulous of women. This is because, the right factor will enable the government to carry out in accordance with the factors that have been identified. In obtaining the
    factors contribute to human capital development, the survey method was conducted on 145 respondents
    among Bumiputera women entrepreneurs in Melaka state and supported with qualitative data from 10
    informants. The findings through exploratory factor analysis found that there are four main factors that
    contribute to human capital development among Bumiputera women entrepreneurs which are
    education and training, experience, social support and creativity, while three main factors that
    contribute to emulous among Bumiputera women entrepreneurs are financial assistance, facilities and
    infrastructure and commitment. Thus, the government is advised to emphasize on education and
    training as well as financial assistance to improve their abilities on human capital and emulous that is
    appropriate to support the women entrepreneurs need to increase their performance.

    Although several studies have examined the relationships among attention, impulse control, gender, and academic achievement, most have focused on clinical samples and have considered only one or two academic subjects. This study investigated these relationships among typically developing children using general achievement measures (academic scores and grades). Our participants were 270 typically developing primary school students (142 boys and 128 girls) of different nationalities living in Malaysia, recruited with purposive sampling with a mean age of 9.75 years. We found that both attention and impulse control significantly predicted academic achievement. Girls had a higher level of attention and impulse control than boys, but gender was not a significant moderator between either attention or impulse control and academic achievement. We discuss the implications of these findings and the need for further research.

    Species of the bopyrid isopod genus Rhopalione Pérez, 1920, are parasites of Indo-West Pacific pinnotherid crabs. Unlike other bopyrid parasites of brachyurans that occupy the branchial chambers, however, species of Rhopalione (subfamily Pseudioninae) infest the abdominal cavity. Prior to the present study, four species of Rhopalione were recognized: R. atrinicolae Page, 1985 (New Zealand), R. incerta (Bonnier, 1900) (Madagascar), R. sinensis Markham, 1990 (East Asia), and R. uromyzon Pérez, 1920 (Persian Gulf). A fifth species of Rhopalione, from Perhentian Besar, Malaysia, is described herein, parasitic on the pinnotherid crab Serenotheres besutensis (Serène, 1967). Keys are provided to females and males of the species in the genus.

    Podolestes parvus sp. nov. is described and illustrated from both sexes from Sarawak, Malaysian Borneo (holotype male Maludam National Park, Betong Division, Sarawak, 10 vii 2012, deposited in RMNH). The new species is closest to P. atomarius Lieftinck, 1950 and is the smallest species yet known in its genus. A full set illustrations of P. atomarius is provided for comparison.     The 'Cephennomicrus group' of Cephenniini includes nine genera distributed mostly in SE Asia (but with some species known also from other continents, except for the Americas). Four new Oriental species are described: Cephennococcus penangensis sp. n. (W Malaysia), Cephennomicrus matangensis sp. n. (E Malaysia), Cephennomicrus selangorensis sp. n. (E Malaysia), and Trurlia nova sp. n. (E Malaysia). Cephennomicrus selangorensis is unique in having the pronotum in both sexes with broad lateral impressions demarcated from the disc by a sharp ridge, a modification slightly similar to that of only one (Australian) congener.

    Seven new species of the genus Deltoplastis Meyrick are described: D. acutangulata Wang et Yu, sp. nov., D. anatoliana Wang et Park, sp. nov., D. multidentalis Wang et Yu, sp. nov. and D. similihoristis Wang et Yu, sp. nov. from China; D. aculeata Wang et Yu, sp. nov. and D. spatuliunca Wang et Yu, sp. nov. from Malaysian Borneo; and D. ovidiscalis Park et Wang, sp. nov. from Vietnam. Deltoplastis horistis (Meyrick, 1910) is newly recorded in China and its female is described for the first time. Images of adults and genitalia of the new species are provided.

    Cranial defects lead to unesthetic appearance and are a constant source of apprehension to the patient. Meningioma with calvarial extension requires the excision of the involved bone for complete excision. Such total excision would leave behind a bony defect which would need reconstruction. Presurgical fabrication of acrylic flap helps in reconstruction of such cranial defect following complete excision in single stage, thereby decreasing the cost and morbidity of surgery. Further, it facilitates the reproduction of the contours, and the tissue bed is not exposed to the heat of polymerization or to the free monomer. The authors report a case of hyperostotic convexity meningioma in a middle-aged female where heat-cured acrylic resin alloplastic implant was prefabricated and used successfully.

    Several anatomical variations concerning the median nerve have surfaced in the medical literature. Among them, bifid median nerve or median nerve bifurcation with or without persistent median artery has been widely reported. Sporadic case reports describe median nerve trifurcation (trifid median nerve) as well. In the present report, we describe carpal tunnel syndrome manifestations in association with trifid median nerve unveiled incidentally under high-frequency musculoskeletal ultrasonogram in a lactating mother-a first in the medical literature.

    BACKGROUND/AIMS: No previous epidemiological study has investigated the prevalence and associated factors of traumatic dental injuries (TDIs) among Libyan children. Such information is required for the planning and evaluation of health services. The aim of this study was to assess the prevalence of TDIs and associated factors among 12-year-old schoolchildren in Benghazi, Libya.

    METHODS: Data for this study were collected as part of a comprehensive, cross-sectional survey investigating oral health status and treatment needs of 12-year-old schoolchildren in Benghazi, Libya, between December 2016 and May 2017. Sociodemographic information was collected through a dental health questionnaire. The children were assessed for oral health status, including TDIs according to modified World Health Organization (WHO) classification criteria, in their classroom by trained and calibrated examiners. Anthropometric measures, lip competence and overjet were all assessed and reported. History of TDIs was sought among those affected. Logistic regression models were applied for TDIs as an outcome variable. The statistical significance for all tests was ≤0.05.

    RESULTS: Data from 1134 participants were included in this study. TDIs were observed in 10.3% of the sample. Most of these TDIs were enamel fractures only (55.6%) and enamel and dentine fractures (35.9%). "Falling" was the most common cause of TDIs, accounting for 51% of cases. While increased overjet appeared to be associated with higher risk of TDIs (OR: 1.92; 95% CI: 1.29-2.86), being female (OR: 0.34; 95% CI: 0.22-0.53) and overweight (OR: 0.33; 95% CI: 0.13-0.83) were also associated with lower risk of having TDIs.

    CONCLUSIONS: This survey showed that a considerable proportion (10.3%) of 12-year-old Libyan children had TDIs, with relatively high unmet treatment needs. More efforts are required to develop effective prevention programmes and to enhance the provision of dental treatment of TDIs for Libyan children.

    OBJECTIVES: To validate the accuracy and reproducibility of linear measurements of three-dimensional (3D) images and to compare the measurements with the direct anthropometry method on cleft lip and palate (CLP) patients.

    MATERIALS AND METHODS: Nineteen linear facial measurements were derived from 16 standardized surface landmarks obtained from 37 cleft patients (20 males, 17 females; mean age 23.84 years, standard deviation ± 6.02). They were taken manually with calipers and were compared with the digitally calculated distance on the 3D images captured using the VECTRA-M5 360° Imaging System with pre-marked landmarks. Another pair of 19 linear measurements were computed on the 3D images 2 weeks apart for intra- and inter-observer agreements. Statistical analyses used were paired t test, the Bland-Altman analysis, and the intra-class correlation coefficient (ICC) index.

    RESULTS: Most of the linear measurements showed no statistically significant differences between the proposed method and direct anthropometry linear measurements. Nevertheless, bias of the 3D imaging system is present in the linear measurements of the nose width and the upper vermillion height. The measurements' mean biases were within 2 mm, but the 95% limit of agreement was more than 2 mm. Intra- and inter-observer measurements generally showed good reproducibility. Four inter-observer measurements, the upper and lower face heights, nose width, and pronasale to left alar base were clinically significant.

    CONCLUSIONS: Measurements obtained from this 3D imaging system are valid and reproducible for evaluating CLP patients.

    CLINICAL RELEVANCE: The system is suitable to be used in a clinical setting for cleft patients. However, training of the operator is strictly advisable.

    INTRODUCTION: Breast cancer metastasis to the gastrointestinal (GI) tract is rare and occurs more frequently in invasive lobular carcinoma. Patients may be asymptomatic or present with variable vague symptoms that may be mistakenly attributed to side effects of chemotherapy or other benign GI diseases. Treatment follows the principles of systemic disease and includes hormonal therapy, chemotherapy and signal transduction inhibitors, with surgical intervention indicated for complications such as obstruction, perforation and hemorrhage.

    PRESENTATION OF CASE: We present the case of a female patient with a history of invasive lobular breast carcinoma who had undergone mastectomy and axillary dissection, followed by chemoradiotherapy. Over the next nine years, she developed ovarian and bone metastases for which appropriate treatment was provided. A right iliac fossa mass was discovered during routine clinic review, though she remained asymptomatic. Computed tomography scan showed ileocecal intussusception. Histopathological examination of the right hemicolectomy specimen following emergency surgery confirmed metastatic invasive lobular carcinoma to the GI tract.

    DISCUSSION: GI tract metastasis may present 30 years after the primary breast cancer. Up to 20% of patients may be asymptomatic as shown by Montagna et al. When present, symptoms are commonly non-specific and vague. Histological diagnosis is challenging. GI metastasis typically appears as intramural infiltration of the bowel wall by small cells arranged in cords.

    CONCLUSION: It is important to maintain a suspicion for GI tract metastasis in breast cancer patients who present with abdominal mass or GI symptoms, as this aids in prompt institution of accurate and appropriate management.
