    RATIONALE: We report a rare case of paraneoplastic bullous pemphigoid associated with mantle cell lymphoma.

    PATIENTS CONCERNS: The patient presented with 5 months' history of generalized skin itchiness, night sweat and loss of weight. The skin manifestations started over the foot and hand area. However, he started to developed tense blisters over the face, trunk and limbs 3 days prior to this admission.

    DIAGNOSES: The skin biopsy report showed subepidermal bullae, in which the immunofluorescence findings in keeping with bullous pemphigoid. The peripheral blood immunophenotyping was suggestive of mantle cell lymphoma. Hence, a diagnosis of paraneoplastic bullous pemphigoid associated with mantle cell lymphoma was made.

    INTERVENTIONS: The patient was initiated with a cytoreduction chemotherapy.

    OUTCOMES: Unfortunately, patient's condition deteriorated further due to neutropenic sepsis and he succumbed after 2 weeks of intensive care.

    LESSONS: Bullous pemphigoid associated with mantle cell lymphoma are very rare. The presentation of bullous pemphigoid led to the detection of mantle cell lymphoma. Early diagnosis and appropriate treatment is crucial in managing this aggressive type of the disease. Both, bullous pemphigoid and mantle cell lymphoma had a parallel clinical course which suggests a paraneoplastic phenomenon in this reported case.

    There was a change in the pattern of substance usage among people who use substances during the COVID-19 pandemic period. This study aims to determine the effects of the COVID-19 pandemic on the pattern of substance usage among people who use drugs (PWUD) receiving treatment at the University Malaya Medical Centre (UMMC) as well as levels of anxiety and depression together with coping mechanisms and the factors affecting the pattern of substance use during COVID-19 pandemic period. A cross-sectional study was applied. The questionnaire used was the Mini-European Web Survey on Drugs (EWSD): COVID-19, Hospital Anxiety and Depression Scale (HADS), and Brief COPE Scale. In total, 130 PWUD were recruited. Of the participants, 36.2% of PWUD had not used/stopped the usage of illicit drugs/alcohol, 26.2% increased their usage, 20% decreased, and 14.6% used the same amount of illicit substances/alcohol during the COVID-19 pandemic period/restrictions. In addition, 28.5% of PWUD had an increased intention to seek professional support for drug counseling/treatment during the COVID-19 pandemic period. The prevalence anxiety and depression symptoms in PWUD according to HADS was 33% and 41.5%, respectively, with depression (p = 0.05) and isolation status (adjusted OR = 2.63, p < 0.05) being associated with an increase in alcohol/illicit substance use during the COVID-19 pandemic. PWUD who had increased their intention to seek professional support had significantly higher odds (adjusted OR = 4.42, p < 0.01) of reducing their alcohol/illicit substance use during the COVID-19 pandemic period. There were increased odds of maintaining alcohol/illicit substance usage among PWUD who practiced dysfunctional coping (adjusted OR = 3.87, p < 0.025) during the COVID-19 pandemic period. In conclusion, depression, isolation status, dysfunctional coping, and intention to seek professional support affected the pattern of alcohol/illicit substance use during the COVID-19 pandemic period. Strategies, substance rehabilitation/counseling, and proper mental health screening and the associated risk factors must be emphasized to prevent a further epidemic of substance use during the pandemic.

    BACKGROUND: Immune checkpoint inhibitors (ICIs) have increased overall survival (OS) in metastatic melanoma in all clinical trials to date. However, pivotal trial populations were not representative of the real-world population in New Zealand. Real-world treatment effectiveness studies help evaluate the value of public pharmaceutical expenditure.

    AIM: To determine the survival outcomes of New Zealand patients with unresectable or metastatic melanoma treated with pembrolizumab or nivolumab.

    METHODS: This is a national retrospective cohort study. Patients with advanced unresectable or metastatic melanoma who received publicly funded immune-checkpoint inhibitors (ICIs) from 2017 to 2019 were included. Individual patient data were extracted from national administrative databases. The primary endpoint was OS, and secondary endpoints included OS by age, duration of treatment, posttreatment survival, and 30-day mortality from last pharmaceutical claim.

    RESULTS: Five hundred ninety-seven patients were included, with a median follow-up of 25 months. One-year OS was 72%, the 2-year OS estimate was 60%, and median OS not reached. Survival did not differ by dichotomized age (≥70 vs. <70 year old), hazard ratio (HR) .94 (95% confidence interval (CI): .72-1.22; p = .62). Median duration of treatment was 9.0 months (95% CI: 7.9-10.1). Median post-treatment survival for the subgroup who had ceased treatment was 12.0 months (95% CI: 9.0-14.0). For the sample as a whole, the estimated 30-day mortality from last pharmaceutical claim was 15.7%.

    CONCLUSION: OS in our New Zealand real-world population is comparable to pivotal clinical trials and real-world data (RWD) from other countries. These findings support the achievement of health gains from use of ICI in advanced unresectable and metastatic melanoma.

    Interactomics is a branch of systems biology that deals with the study of protein-protein interactions and how these interactions influence phenotypes. Identifying the interactomes involved during host-pathogen interaction events may bring us a step closer to deciphering the molecular mechanisms underlying plant defence. Here, we conducted a systematic review of plant interactomics studies over the last two decades and found that while a substantial progress has been made in the field, plant-pathogen interactomics remains a less-travelled route. As an effort to facilitate the progress in this field, we provide here a comprehensive research pipeline for an in planta plant-pathogen interactomics study that encompasses the in silico prediction step to the validation step, unconfined to model plants. We also highlight four challenges in plant-pathogen interactomics with plausible solution(s) for each.

    This work presents the structural and adsorption properties of the CaCO3(-)-rich Corbicula fluminea shell as a natural and economic adsorbent to remove Cd ions from aqueous solutions under batch studies. Experiments were conducted with different contact times, various initial concentrations, initial solution pH and serial biosorbent dosage to examine the dynamic characterization of the adsorption and its influence on Cd uptake capacity. The characterization of the C. fluminea shell using SEM/EDX revealed that the adsorbent surface is mostly impregnated by small particles of potentially calcium salts. The dominant Cd adsorption mechanism is strongly pH and concentration dependent. A maximum Cd removal efficiency of 96.20% was obtained at pH 7 while the optimum adsorbent dosage was observed as 5 g/L. The Langmuir isotherm was discovered to be more suitable to represent the experimental equilibrium isotherm results with higher correlation coefficients (R(2) > 0.98) than Freundlich (R(2)

    The present study investigates the concentration of Pb, Cd, Ni, Zn, and Cu in the paddy field soils collected from Tumpat, Kelantan. Soil samples were treated with sequential extraction to distinguish the anthropogenic and lithogenic origin of Pb, Cd, Ni, Zn, and Cu. ELFE and oxidizable-organic fractions were detected as the lowest accumulation of Pb, Cd, Ni, Zn, and Cu. Therefore, all the heavy metals examined were concentrated, particularly in resistant fraction, indicating that those heavy metals occurred and accumulated in an unavailable form. The utilization of agrochemical fertilizers and pesticides might not elevate the levels of heavy metals in the paddy field soils. In comparison, the enrichment factor and geoaccumulation index for Pb, Cd, Ni, Zn, and Cu suggest that these heavy metals have the potential to cause environmental risk, although they present abundance in resistant fraction. Therefore, a complete study should be conducted based on the paddy cycle, which in turn could provide a clear picture of heavy metals distribution in the paddy field soils.

    Studies have proven that client satisfaction (CS) is greatly affected by the nature of construction practices on site. However, not much attention has been given to CS in projects that adopt green construction practices (GCPs). This study examines the effect of GCPs on CS, and the means by which CS can be achieved through health and safety performance (HSP). This study also analyses how the relationship between HSP and CS could vary depending on the level of GCPs adopted. A questionnaire with 27 items drawn from literature was used in collecting data from class A contractors in Nigeria. The study's hypotheses were tested using the partial least squares structural equation modelling (PLS-SEM) technique. The findings of the study reveal that GCPs have a positive effect on CS. HSP proved to only partially mediate the relationship between GCPs and CS. The findings also show that the effect of HSP on CS is moderated by GCPs with the stronger effect from projects with low adoption of GCPs. The study's findings highlight the pivotal role of HSP in ensuring CS in construction projects that adopt GCPs. This study makes practical contributions to the body of existing knowledge by highlighting the need for concerted efforts by contractors such as investing in health and safety training and ensuring site workers wear personal protective equipment aimed at adhering to acceptable health and safety standards. This can be achieved through provision of funds for safety activities, monitoring, and supervision of workers to guarantee HSP which translates to CS while adopting GCPs. However, the obstacles to these are the lack of safety culture by construction organizations, lack of enlightenment on the part of construction site workers, and poor safety climate on the construction project site. Additionally, the study provides greater insight on the practice of green construction among contractors by showing that the effect of HSP on CS would differ considerably on account of the level of GCPs adopted.

    The interaction and the interplay of climate change with oil palm production in the Southeast Asia region are of serious concern. This particularly applies in Malaysia due to its rank as the second largest palm oil producer in the world. The anthropogenic activities and the agroecological practices in oil palm plantation, including excessive use of fertilisers, bush fire due to land clearing, and cultivation on peatland, have exacerbated the effects of climate change featuring extreme events, drought, flooding, heatwave, as well as infestation of pest and diseases. These adverse impacts on oil palm production highlight the significance of deploying effective adaptation strategies. The study aims to examine the impact of climate change on oil palm production and identify the farmers' adaptation strategies to the impacts of climate change in Malaysia. This study was conducted a comprehensive review of the articles published from 2000 to 2021 in the contexts of climate change and oil palm production in Malaysia. The review shows that climate change has a range of impacts on the oil palm production in Malaysia. As a result, several adaptation options were identified, such as breeding of hybrid varieties that are tolerant and resistant to heat; sustainable management of soil; pit and tranches to enhance water management in plantation areas; minimal use of fertilisers, herbicides, and pesticides; zero burning; and minimum tillage. The reviewed studies recommended the following to mitigate the adverse impacts of climate change: sustainable national policy on climate change, conservation of the existing carbon stock, effective management of tropical rainforest biodiversity, afforestation for carbon sequestration, and reduction in greenhouse gas (GHG) emission.

    The recurrence of nasopharyngeal carcinoma (NPC) is either at the local primary site or at regional or distant metastases. However, an axillary metastasis is a rare entity in NPC. We highlighted a case of recurrent NPC that presented with axillary swelling as the main initial complaint. Clinical examinations showed enlarged left axillary lymph nodes and left cervical lymph nodes. Histopathological examination of the axillary lymph node biopsy confirmed the recurrence of NPC. The patient underwent palliative chemotherapy in view of the advanced stage of recurrent disease. A thorough clinical history and examination during surveillance are crucial for early diagnosis and better survival outcomes.

    BACKGROUND: Breast reconstruction after mastectomy is important in breast cancer care.

    MATERIALS AND METHODS: A cross-sectional study was designed to assess surgeons' and patients' perceptions toward breast reconstruction. Questionnaires were distributed to general and breast surgeons in East Coast Malaysian hospitals and Hospital Kuala Lumpur and to postmastectomy patients with and without breast reconstruction at the Hospital Universiti Sains Malaysia and Hospital Raja Perempuan Zainab II. The response rates were 82.5% for the surgeons (n = 33), 95.4% for the patients with reconstruction (n = 63), and 95.5% for the patients without reconstruction (n = 278).

    RESULTS: The median surgeon age and experience was 42 and 6 years, respectively. Each surgeon saw an average of 20 new breast cancer cases annually. Most surgeons (86.7%) discussed reconstruction options with their patients but had only referred an average of 4 patients for reconstruction during a 3-year period. Surgeons' concerns regarding the qualitative outcome increased the likelihood of a breast reconstruction discussion (β = 4.833; P = .044). The women who underwent breast reconstruction were younger (mean age, 42 vs. 50 years), were more often working (69.4% vs. 42.2%), and more often had previous awareness of the option (90.3% vs. 44.3%). The most common reasons for undergoing breast reconstruction were "to feel more balanced" (92.1%) and "surgeon's strong recommendation" (92.1%). Previous knowledge of breast reconstruction increased the likelihood of reconstruction (odds ratio, 5.805; P = .026). Although 70% of surgeons thought that patients would not be interested in reconstruction, only 37.9% of patients with previous awareness reported having no interest.

    CONCLUSION: The low reconstruction rate (20.6%) can be attributed to the low referral rate. Patients' likelihood to undergo reconstruction with their surgeon's recommendation and with previous awareness were reflective of the surgeons' strong influence on their patients. Thus, clarification of surgeons' hypothetical criticisms could conceivably increase the reconstructive surgery rate.

    BACKGROUND: The literature suggests that the medical community needs musicians to provide an insider's perspective to understand the physical and psychological dimensions of playing an instrument, and healthcare providers need to understand musicians' experiences in order to develop coping strategies. Compared with professional pianists, student pianists are a neglected group. However, student and professional pianists both want to maintain their playing careers and have the experience of giving up playing because of playing-related musculoskeletal disorder (PRMD). There are a few studies conducted on student pianists' experiences with PRMD, but none have been conducted in the Chinese context. Given the distinctive characteristics of higher music education in China and Chinese piano students, this study aims to investigate the lived experiences of tertiary student pianists with PRMD.

    METHODS: Phenomenology is the most suitable qualitative method for investigating lived experiences. This study employed a transcendental phenomenological approach to investigate the experiences of student pianists, collecting data through one-on-one interviews and focus group discussions. Since phenomenological research emphasizes the homogeneity of research subjects, all 25 participants in this study are tertiary student pianists from seven Chinese higher education institutions.

    RESULTS: Four themes and ten sub-themes were identified in this study. They are as follows: Theme one, Perceptions of PRMD, with sub-themes of body perceptions, negative thought, and emotional changes; Theme two, Complex Identity, with sub-themes of future pianists' identity, nuanced identity of student pianists, and the dual identity between student pianist and patient; Theme three, Coping Strategies, with sub-themes of self-regulation and actively seek help from social relations; Theme four, Influences and Meanings, with sub-themes of negative influences of PRMD and positive meanings of PRMD.

    CONCLUSION: This study explores the experiences of tertiary student pianists with PRMD, including their subjective thoughts and feelings. It also highlights the importance of understanding tertiary student pianists' experiences in developing health education and healthcare measures tailored to them.

    Lignocellulosic biomass contains lignin, an aromatic and oxygenated substance and a potential method for lignin utilization is achieved through catalytic conversion into useful phenolic and aromatic monomers. The application of monometallic catalysts for lignin hydrogenolysis reaction remains one of the major reasons for the underutilization of lignin to produce valuable chemicals. Monometallic catalysts have many limitations such as limited catalytic sites for interacting with different lignin linkages, poor catalytic activity, low lignin conversion, and low product selectivity. It is due to lack of synergy with other metallic catalysts that can enhance the catalytic activity, stability, selectivity, and overall catalytic performance. To overcome these limitations, works on the application of bimetallic catalysts that can offer higher activity, selectivity, and stability have been initiated. In this review, cutting-edge insights into the catalytic hydrogenolysis of lignin, focusing on the production of phenolic and aromatic monomers using bimetallic catalysts within an internal hydrogen donor solvent are discussed. The contribution of this work lies in a critical discussion of recent reported findings, in-depth analyses of reaction mechanisms, optimal conditions, and emerging trends in lignin catalytic hydrogenolysis. The specific effects of catalytic active components on the reaction outcomes are also explored. Additionally, this review extends beyond current knowledge, offering forward-looking suggestions for utilizing lignin as a raw material in the production of valuable products across various industrial processes. This work not only consolidates existing knowledge but also introduces novel perspectives, paving the way for future advancements in lignin utilization and catalytic processes.

    Located in one of the famous natural and cultural tourist destinations of northern Malaysia, Penang Hill serves as an ideal destination for both local and international tourists. The objective of this study was to understand and explore key stakeholders perspectives to enhance management for the aim of conservation, economic development, and recreation in Penang Hill. Data was collected from three key stakeholders of Penang Hill using semi-structured in-depth interviews. Stakeholders showed robust interests in environmental protection, cultural conservation, economic activities, awareness, and empowerment. This paper concludes by stating that Penang Hill is under strong management, and stakeholders are continuously engaged in the destination. The management authority collaborates with the key stakeholders of Penang Hill and incorporates their interests and viewpoint to ensure the ecotourism process goes smoothly and Penang Hill becomes one of the top ecotourism destinations.

    Swamp eel, Monopterus albus is one of the common fish in paddy fields, thus it is suitable to be a bio-monitor for heavy metals pollution studies in paddy fields. This study was conducted to assess heavy metals levels in swamp eels collected from paddy fields in Kelantan, Malaysia. The results showed zinc [Zn (86.40 μg/g dry weight)] was the highest accumulated metal in the kidney, liver, bone, gill, muscle and skin. Among the selected organs, gill had the highest concentrations of lead (Pb), cadmium (Cd) and nickel (Ni) whereas muscle showed the lowest total metal accumulation of Zn, Pb, copper (Cu), Cd and Ni. Based on the Malaysian Food Regulation, the levels of Zn and Cu in edible parts (muscle and skin) were within the safety limits. However, Cd, Pb and Ni exceeded the permissible limits. By comparing with the maximum level intake (MLI), Pb, Ni and Cd in edible parts can still be consumed. This investigation indicated that M. albus from paddy fields of Kelantan are safe for human consumption with little precaution.

    The current case study was conducted to identify the causes of environmental health issues in the office space associated with the existing Underfloor Air Distribution (UFAD) system in a high-rise office building in the tropics. The causes of the indoor environmental quality degradation are the key to resolve the environmental health issues. Thus, the parameters such as the indoor air temperature, relative humidity (RH), relative air velocity, carbon monoxide (CO), carbon dioxide (CO2), formaldehyde, total volatile organic compound (TVOC) and particulate matter (PM10) were evaluated in five office spaces. The results showed that the diffusers were not effective in creating air mixing at the occupied space. The RH has exceeded the threshold limit of 70%. The CO2 concentration has exceeded 1000 ppm, and the formaldehyde has exceeded 0.1 ppm. These indicate the poor design and maintenance of the building that lead to the degradation of indoor environmental quality. Long exposure to the poor indoor environmental quality will cause permanent health damages. The Indoor Air Quality (IAQ) management plan must be established and implemented in the ongoing basis to ensure the health of the occupants are safeguarded. As part of the plan, the practice to conduct an IAQ assessment once every 3 years is recommended to ensure the health and well-being of the occupants are safeguarded.

    IL-23 is a double-subunit cytokine that plays an important role in shaping the immune response. IL-23 was found to be associated with several autoinflammatory diseases by generating sustained inflammatory loops that lead to tissue damage. Antibody neutralization of IL-23 was proven to be effective in ameliorating associated diseases. However, antibodies as large proteins have limited tissue penetration and tend to elicit anti-drug antibodies. Additionally, anti-IL-23 antibodies target only one subunit of IL-23 leaving the other one unneutralized. Here, we attempted to isolate a recycling single domain antibody by phage display. One of IL-23 subunits, p19, was expressed in E. coli fused to Gamillus protein to stabilize the α-helix-only p19. To remove Gamillus binders, two biopanning methods were investigated, first, preselection with Gamillus and second, challenge with IL-23 then on the subsequent round challenge with p19-Gam. The isolation of calcium-dependent and pH-dependent recycling binders was performed with EDTA and citrate buffers respectively. Both methods of panning failed to isolate high-affinity and specific p19 recycling binders, while from the second panning method, a high affinity and specific p19 standard binder, namely H11, was successfully isolated. H11 significantly inhibited the gene expression of IL-17 and IL-22 in IL-23-challenged PBMCs indicating H11 specificity and neutralizing ability for IL-23. The new binder due to its small size can overcome antibodies limitations, also, it can be further engineered in the future for antigen clearance such as fusing it to cell penetrating peptides, granting H11 the ability to clear excess IL-23 and enhancing its potential therapeutic effect.

    OBJECTIVE(S): The effects of tamarind seeds (T) on lipid and carbohydrate metabolism in rats were studied. Rats were offered basal diet (BD) with T (2%, 4% or 8%) or without T.

    MATERIALS AND METHODS: Feeding and growth performance in rats were measured and samples of liver and blood were analyzed for glycogen content and levels of cholesterol and glucose respectively.
