A new Salmonella type is described, for which the name Salmonella seremban is proposed; it has the antigenic formula IX, XIII, XII 2, XIIa; i=l, 3, 5. It was the apparent cause of a number of human cases of food poisoning at Seremban, Malaya.
Background: Comparative efficacy of different pharmacist based interventions on glycemic control of type 2 diabetes patients is unclear. This review aimed to evaluate and compare the efficacy of different pharmacist based interventions on clinical outcomes of type 2 diabetes patients. Methods: A systematic search was conducted across five databases from date of database inception to September 2017. All randomized clinical trials evaluating the efficacy of pharmacist based interventions on type 2 diabetes patients were included for network meta-analysis (NMA). The protocol is available with PROSPERO (CRD42017078854). Results: A total of 43 studies, involving 6259 type 2 diabetes patients, were included. NMA demonstrated that all interventions significantly lowered glycosylated hemoglobin (HbA1c) levels compared to usual care, but there was no statistical evidence from this study that one intervention was significantly better than the other for reducing HbA1c levels. Pharmacist based diabetes education plus pharmaceutical care showed maximum efficacy for reducing HbA1c levels [-0.86, 95% CI -0.983, -0.727; p < 0.001]. Pharmacist based diabetes education plus pharmaceutical care was observed to be statistically significant in lowering levels of systolic blood pressure [-4.94; 95%CI -8.65, -1.23] and triglycerides levels [-0.26, 95%CI -0.51, -0.01], as compared to the interventions which involved diabetes education by pharmacist, and for body mass index (BMI) [-0.57; 95%CI -1.25, -0.12] in comparison to diabetes education by health care team involving pharmacist as member. Conclusion: The findings of this review demonstrate that all interventions had a significantly positive effect on HbA1c, but there was no statistical evidence from this study that one intervention was significantly better than the other for achieving glycemic control.Pharmacist based diabetes education plus pharmaceutical care showed maximum efficacy on HbA1c and rest of the clinical outcomes.
Introduction: This cross-sectional study aimed to determine association of age and gender with dental treatment need among first-visit patients at Dental Outpatient Clinic, IIUM Kuantan in 2016.
Materials and Methods: Ethical approval (IREC 762) was obtained from IIUM Human Ethics Committee. List of first-visit patients who attended clinic from 1st January to 31st December 2016 was retrieved to assess patient's age and gender. Dental treatment need was determined according to the normative need which was professionally defined by dental-officer-in-charge. All first-visit patients were included, except those attended Orthodontic department or with old Malaysian identification card or foreign passport. Included patients were classified into six age groups (≤14, 15-19, 20-34, 35-44, 45-64 and ≥ 65). Pearson's chi square test was used for statistical analysis using SPSS Version 20 software.
Results: From a total number of 2,713 patients (age range of 2 to 87), 1,210 (44.6%) were males and 1,503 (55.4%) were females. Age (p<0.001) and gender (p<0.001) were significantly associated with dental treatment need. Paediatric patients (≤14) majorly received paedodontic care (97.5%). Young-age patients (15-19, 20-34 and 35-44) mostly required conservative care (39.5%, 31.2% & 30.6%, respectively) while elder-age patients (45-64 and ≥ 65) have major need for prosthodontics (30.0% & 40.2%, respectively). Females outnumbered males in all age groups, except for the age group of ≥ 65 whereby the situation was the opposite. Females also outnumbered males in all types of treatment need, except for periodontics which was predominated by males.
Conclusion(s): Age and gender have significant associations with treatment need for dental care.

How to look for new drugs? What guidelines to use? Have we to continue investigations on plant and marine organisms? These questions arise frequently today. A pharmacological effect results from the addition of many effects at a molecular level, i.e. the interaction between a ligand and a receptor. As long as the chemical structure of this receptor remains unknown, studies of Nature's resources will yield the largest reservoir of new drugs. Nature provides our imagination with the pattern of novel biologically active molecules. Criteria classically used in the past to select plants for study were chemotaxonomy, ethnopharmacology or pharmacotaxonomy. Examples will be taken from personal experience, to illustrate work done according to the chemotaxonomical approach (Ochrosia and ellipticines), and the ethnopharmacological approach (antiinflammatory properties of Euphorbiaceae from New Caledonia). Taking into account that one of the major problems we have to face is the unsatisfactory classical pharmacological testing procedure, we have tried to set up a network grouping biologists and chemists. Among many results obtained, one concerns the use of the mammalian hypothalamo-pituitary system to screen effects of alkaloids extracted from Psychotria oleoides, a Rubiaceae collected in New Caledonia. Psycholeine exhibits an intriguing activity on GH release. Another result concerns the influence of a Labiatae extract on the adenylate cyclase system: 9 HODE extracted from Glechoma hederacea stimulates the basal level of enzyme activity in platelets, this activity being possibly involved in the folk uses claimed. Using the tubulin test to screen antimitotic activities of plant extracts, the biological activity of rhazinilam has been demonstrated as responsible for the antitubulin activity of a Malaysian plant, Kopsia singapurensis.(ABSTRACT TRUNCATED AT 250 WORDS)
Anterior crossbite is relatively a common presentation in the mixed dentition stage. If left untreated, it can lead to a host of problems and may complicate future orthodontic treatment. One of the major difficulties in performing anterior crossbite correction in young children is treatment compliance. In most cases, poor compliance is due to the unacceptability of the removable appliance used. This article describes three cases of successful correction of anterior crossbite of patients in mixed dentition using short-span wire-fixed orthodontic appliances. This sectional appliance provides an alternative method of correcting anterior crossbite of dental origin and offers many advantages compared to the use of removable appliances.
There is a large volume of published studies describing the adverse relationship between treatment non-adherence with tuberculosis treatment outcome. Non-adherence could result in increased risks of prolonged infectiousness, drug resistance, relapse cases and poor survival among tuberculosis patients. Nevertheless, few studies are to be found providing detailed on the reason of defaulting treatment among tuberculosis patients in Malaysia. Hence the goal of this paper is to find out the barriers and motivations factors that affect patients’ treatment compliance among our local tuberculosis patients. This is a qualitative study which included 12 in-depth interviews with tuberculosis non-compliance patients who were treated at Institute of Respiratory Medicine, Kuala Lumpur. All the conversations were recorded, transcribed and analysed by using thematic analysis. It was found that low knowledge, self-negative attitudes, traditional believes, negative perceptions towards health caregiver, drug side effects, stigma, financial problems, less family support and work commitments are the barriers that prevent the patients from religiously taking their anti-tuberculosis treatment. Meanwhile, factors that encourage them to continue their treatment were the believes of bad effects of the disease onto their lives and health, good relationship between patient and health caregiver and social support from people around them. In conclusion, non-adherence involved a dynamic influence of individual, socio-economic and treatment-related factors on the patients. The results presented here may facilitate improvement in the activities in promoting compliance among tuberculosis patients in the future which tailored to the patients’ specific needs.
Tuberculosis or TB is a major issue in Malaysia as the dramatic emerge of infection. In Mac 2016 (Until 15 April 2016)
3049 cases were reported and total cases were reported 7717 cases which increasing 902 (13.0%) cases at same month
2015 (6815 cases). Meanwhile, prevention and diagnosis should be started from children. Educational planning with
the collaboration from Ministry of Education (MOE) and Ministry of Health (MOH) should be intensive to ensure the
prevention and mechanism control of this disease can be cure at the beginning. The focus of the study was given on the
(1) transmission of tuberculosis (TB) in Malaysia (2) practice of prevention of TB (3) mechanism control in educational
institutions and (4) promotion health programmes in schools. The conclusion of this study is drawn on the premise of
providing plausible suggestion that will promote good governance for the Ministry of Education (MOE) and Ministry of
Health (MOH) to strengthening the current policy for a better future implementation.
The aims of this study were to demonstrate "order error" in the calculation of continuous relative phase (CRP) and to suggest two alternative methods-(i) constructing phase-plane portraits by plotting position over velocity; and (ii), the Hilbert transform-to rectify it. Order error is the change of CRP order between two degrees of freedom (e.g., body segments) when using the conventional method of constructing phase-plane portraits (i.e., velocity over position). Both sinusoidal and non-sinusoidal simulated signals as well as signals from human movement kinematics were used to investigate order error and the performance of the two alternative methods. Both methods have been shown to lead to correct results for simulated sinusoidal and non-sinusoidal signals. For human movement data, however, the Hilbert transform is superior for calculating CRP.
Primary osteosarcoma of the spine is indeed rare and only several sporadic cases have been reported. It tends to occur in a slightly older age group than those with appendicular skeleton tumours. We present here an unusual case of aggressive lumbosacral osteosarcoma in a young teenager complicated by extensive dural spread, skip lesions and intravascular extension. Although a histopathological examination is mandatory to establish the diagnosis, this case emphasises the need of imaging to ascertain the full extent of disease spread especially in deciding the type of treatment to be instituted and to evaluate the response to the treatment.
Management of hepatocellular carcinoma (HCC) generally follows EASL-EORTC clinical practise guideline. We report a case of a 50-year-old man who had multinodular HCC with lung metastases, whose management had deviated from the standard guideline but still survive 5 years after initial diagnosis.
Schwannomas are benign tumours arising from neurilemmal cells which forms the myelin sheath of peripheral nerves. It usually occurs in the head, but may be found in the brachial plexus and sciatic nerve. Common peroneal nerve schwannoma are rare. We report a case of a middle age gentleman who presented with pain and swelling over the right popliteal fossa with associated right radicular pain of the anterolateral leg and weakness of ankle dorsiflexion. Examination revealed a 3x2 cm lump behind the posterolateral aspect of the right knee with positiveTinel’s sign upon tapping of the lump, sensory deficit over the anterolateral aspect of the leg and the ankle dorsiflexors had a muscle power of grade 3. Magnetic resonance imaging (MRI) of the right leg revealed a well circumscribed, oval lesion located along the pathway of the common peroneal nerve homogenously hypointense on T1-weighted images and heterogeneously hyperintense on T2 weighted images compared to the muscles. The lesion was not surpressed on fat suppression sequences. Intra-operatively, we noted that the schwannoma was in continuity with the common peroneal nerve. The patient underwent excision of the schwannoma. Post operatively, the pain reduced remarkably but patient suffers from numbness and right foot drop as a complication of the tumour. This case highlights the rarity of common peroneal nerve schwannoma which presents with neuropathic symptoms complicated with right foot drop.
Plantar pressure, force and contact area information may provideinsights regarding stresses imparted to the foot when performing functional tasks. There is limited information regarding plantar pressure, force and contact area when carrying incremental loads (no load, 5 kg, 7.5 kg and 10 kg) using one hand between adults with and without low back pain (LBP). The aim of our study was to investigate the changes in the plantar pressure, force and contact area when carrying incremental loads (no load, 5 kg, 7.5 kg and 10 kg) using one hand between adults with and without low back pain (LBP). A total of 20 adults with non-specific LBP and 20 matched individuals without LBP were recruited according to the predefined recruitment criteria. Plantar pressure (PP), maximum force (MF) and contact area (CA were measured in standing position and during walking while carrying incremental loads (no load, 5 kg, 7.5 kg and 10 kg) using their right hand on a Matscan pressure assessment system. A two-way mixed analysis of variance (group× load) was conducted to analyse the data. No significant main effectof group was demonstrated on both the right and left foot during standing (PP: p = 0.74, p = 0.32; MF: p = 0.17, p = 0.67; CA: p = 0.25, p = 0.24) and walking (PP: p = 0.61, p = 0.48; MF: p = 0.19, p = 0.06; CA: p = 0.16, p = 0.26. Similarly, there was no interaction effect between the loads and groups on the PP (p = 0.89, p = 0.47), MF (p = 0.76, p = 0.83) and CA (p = 0.88, p = 0.20) on theright and left foot, respectively during standing. However, a significant interaction effect (p < 0.05), between the loads and groups was demonstrated on the PP, MF and CA on the left foot during walking. The results of our study suggest that stresses imparted to the foot alters during dynamic postures and this may be a compensatory mechanism. Plantar pressure, force and contact area were similar in adults both with and without LBP when standing and walking. Further biomechanical information that includes both kinematic and kinetic data in lumbopelvic and lower limbs in relation to the foot may be required to justify for prevention and management strategies among adults with LBP.
A meta-analysis was conducted to evaluate the prevalence of osteopenia/osteoporosis in human immunodeficiency virus (HIV)-infected individuals. The prevalence of osteopenia/osteoporosis in HIV-infected and antiretroviral therapy (ART)-treated individuals was significantly higher than respective controls. Evidence regarding bone loss within first year of HIV infection or ART initiation was preliminary.
Computer vision is applied in many software and devices. The detection and
reconstruction of the human skeletal structure is one of area of interest, where the
camera will identify the human parts and construct the joints of the person standing in
front. Three-dimensional pose estimation is solved using various learning approaches,
such as Support Vector Machines and Gaussian processes. However, difficulties in
cluttered scenarios are encountered, and require additional input data, such as
silhouettes, or controlled camera settings. The paper focused on estimating the threedimensional
pose of a person without requiring background information, which is
robust to camera variations. Each of the joint has three-dimensional space position and
matrix orientation with respect to the sensor. Matlab Simulink was utilized to provide
communication tools with depth camera using Kinect device for skeletal detection.
Results on the skeletal detection using Kinect sensor is analysed in measuring the
abilities to detect skeletal structure accurately, and it is shown that the system is able
to detect human skeletal performing non-complex basic motions in daily life.
Introduction: Clinical resolution of periodontitis (CRP) of type-2 diabetic patients with chronic periodontitis (T2DM-PD) after receiving non-surgical periodontal treatment (NSPT) has been reported in the previous studies. This study aimed to evaluate CRP of T2DM-PD under medicaldental coordinated care (M-DCC). Materials and Methods: A 6-months follow-up quasi-experimental study was conducted among 20 subjects who received M-DCC in 2016. M-DCC included standard diabetic care provided by medical professional from 3 health clinics and NSPT provided by periodontal specialists from two periodontal specialist clinics. Target glycemic control achievement (TGCA) HbA1c 6.5% was assessed at baseline and 6 months after NSPT. Clinical resolution of PD was measured in terms of BPE, BOP %, CAL(mm), PPD(mm), PPD 4mm, PPD =4 mm and PPD 6mm at baseline, 3 and 6 months. Paired simple t test and ANOVA F test were applied to infer clinical resolution of periodontitis and its relation to TGCA.
Results: Mean (SD) of average BPE at baseline, 3- and 6-months were 3.52(0.34), 3.12(0.33) and 3(0.45) with (p<0.05); average PPD(mm) were 3.33(0.5), 3.23(0.75) and 2.73(0.57) with (p<0.05); PPD(%) 4mm were 71.03(12.33), 82.77(9.9) and 85.85 (8.9) with (p<0.05); PPD(%) =4 mm were 27.94(11.9), 16.97(10.01) and 13.71(9.1) with (p<0.05); PPD(%) 6mm were 8.04(4.32), 2.66(2.3) and 1.87(2.32) with (p<0.05). Significant resolution of BPE, CAL(mm) and PPD(mm) was noticed among two subjects who has changed from uncontrolled TGCA to controlled TGCA.
Conclusion(s): CRP and TGCA results have verified the effectiveness of M-DCC. A further clinical control trial with adequate sample size needs to confirm the results of the present study.

Introduction: The objective of this study was to determine the prevalence and the pattern of hypodontia among orthodontic patients in Klinik Pergigian Jalan Sultan Mahmud, Kuala Terengganu.
Materials and Methods: A retrospective of 351 orthodontic patients record review study was conducted; comprised of 104 males and 247 females. The records were reviewed by trained dental officer between October 2016 and June 2017. The Chi-square and Fisher's Exact Tests were carried out for statistical analysis.
Results: The prevalence of hypodontia was 10.8%. Thirty-eight patients were found to have at least single missing tooth. A significant difference was found between males and females of having hypodontia (p = 0.048). No significant difference was found for hypodontia patients by races (p = 0.829). The maxillary lateral incisor was the most frequently missing tooth followed by mandibular second premolars and mandibular lateral incisors. Hypodontia cases observed tended to be more likely occurred in the upper left arch compared to the lower arch. The majority of hypodontia patients had single missing tooth, but rarely more than 3 missing teeth. Hypodontia patients had mainly Class III malocclusion, however no statistically significant was found (p= 0.081).
Conclusion(s): The prevalence of hypodontia of this study was within the range of what is being reported in the literature and showed gender significant. Hypodontia was mainly involved maxillary lateral incisors; more prevalent in the upper arch; and mostly observed in patients with Class III malocclusion in the study sample. Early detection and accurate diagnosis are essential. Therefore, alternative treatment modalities can be well planned under multidisciplinary team approach in restoring the aesthetic and function.
