    This paper investigates several asymptotic confidence interval estimates, based on the Wald, likelihood ratio and the score statistics for the parameters of a parallel two-component system model, with dependent failure and a time varying covariate, when data is censored. This model is an extension of the bivariate exponential model. The procedures are investigated via a coverage probability study using the simulated data. The results clearly indicate that the interval estimates, based on the likelihood ratio method, work better than any of the other two methods when dealing with the censored data.

    Determination of Length-weight Relationship (LWR) of any commercially important fish is crucial to validate the wild stock level, to predict their wellbeing in the natural habitat and for various sustainable fishery management practices. Liza subviridis (Valenciennes) is noted to be highly abundant along the coast of Parangipettai, South east coast of India. Hence, the present study was aimed to establish Length-weight relationship and condition factor of Greenback mullet, Liza subviridis (Valenciennes) occurring in Vellar estuary, Parangipettai (lat. 11 degrees 30' N, long. 79 degrees 46' E) using least square method. To determine the actual relationship between length and weight of L. subviridis exponent coefficient or equilibrium constant (b) and relative condition factor (Kn) analysis were adopted. The females were found to be heavier than males at similar length. The equilibrium constant 'b' was found to be 2.7106 in males and 2.8927 in females. The corresponding parabolic representation for male was W = 0.0462L(2.7106) and for female W = 0.0382L(2.8927). The equilibrium constant did not obey the cube law as it deviated significantly from 3 in the case of males. The relative condition factor around 1 and little over it revealed the well-being of L. subviridis in Parangipettai waters.

    Background and Purpose: Workplace bullying has been regarded as a serious phenomenon, particularly in health-care settings, due to its tendency to predispose health workers to serious psychological repercussions, job dissatisfaction, and turnover. Such consequences are costly to health systems and disruptive to the continuity of patient care. While global bullying literature in health settings grows, evidence on the magnitude of the problem from a Malaysian perspective is scarce. This study aimed to determine the prevalence of workplace bullying and its associated factors among health workers in a Malaysian public university hospital.

    Methods: This cross-sectional study was conducted from October to December 2019 among 178 hospital workers at the Hospital Canselor Tuanku Muhriz in Kuala Lumpur, Malaysia. The study utilized a self-administered questionnaire that consisted of items on sociodemographics, work characteristics, sources of bullying, and the validated Malay version of the 23-item Negative Acts Questionnaire - revised to determine the prevalence of bullying. Descriptive and inferential statistics were analyzed using SPSS 22.0. Statistical significance was set at P<0.05.

    Results: The prevalence of workplace bullying in this sample was 11.2%. Superiors or supervisors from other departments and colleagues were the main perpetrators. In the multivariate model, working for 10 years or less (aOR 4, 95% CI 1.3-12.3; P=0.014) and not being involved in patient care (aOR 5, 95% CI 2.5-10; P<0.001) were statistically significant attributes associated with workplace bullying.

    Conclusion: Workplace bullying in the current study was strongly associated with occupational characteristics, particularly length of service and service orientation of the workers. Hospital directors and managers could undertake preventive measures to identify groups vulnerable to bullying and subsequently craft appropriate coping strategies and mentoring programs to curb bullying.

    Introduction: Hepatitis C virus (HCV) infection is known as contributing to high morbidity and mortality globally. Major liver complications such as liver failure and liver cancer which can lead to fatality have been associated with persistent HCV infection. Globally, it is estimated that 5.6 million chronically infected HCV are among people who inject drugs (PWID). Malaysia has estimated that 59% HCV infections were among PWID. The aim of this study is to determine the prevalence of HCV infection and its predictors among PWID in Negeri Sembilan. Methods: A cross-sectional study based on random proportion to size sampling was conducted among 212 out of 1414 regis- tered Methadone Maintenance Therapy (MMT) clients with PWID attending health clinics in Negeri Sembilan from February 2018 to July 2018.     BACKGROUND: Primary care physicians (PCP) play an important role in detecting Familial Hypercholesterolaemia (FH) early. However, knowledge, awareness and practice (KAP) regarding FH among Malaysian PCP are not well established, and there was no validated tool to assess their FH KAP. Thus, the aim of this study was to adapt an FH KAP questionnaire and determine its validity and reliability among Malaysian PCP.

    METHODS: This cross-sectional validation study involved Malaysian PCP with ≥ 1-year work experience in the primary care settings. In Phase 1, the original 19-item FH KAP questionnaire underwent content validation and adaptation by 7 experts. The questionnaire was then converted into an online survey instrument and was face validated by 10 PCP. In Phase 2, the adapted questionnaire was disseminated through e-mail to 1500 PCP. Data were collected on their KAP, demography, qualification and work experience. The construct validity was tested using known-groups validation method. The hypothesis was PCP holding postgraduate qualification (PCP-PG-Qual) would have better FH KAP compared with PCP without postgraduate qualification (PCP-noPG-Qual). Internal consistency reliability was calculated using Kuder Richardson formula-20 (KR-20) and test-retest reliability was tested on 26 PCP using kappa statistics.

    RESULTS: During content validation and adaptation, 10 items remained unchanged, 8 items were modified, 1 item was moved to demography and 7 items were added. The adapted questionnaire consisted of 25 items (11 knowledge, 5 awareness and 9 practice items). A total of 130 out of 1500 PCP (response rate: 8.7%) completed the questionnaire.     BACKGROUND: Offspring of type 2 diabetes patients have an absolute risk of 20-40% of developing the condition. Type 2 diabetes patients should be encouraged to speak to their offspring regarding diabetes risk and prevention strategies. The Health Belief Model conceptualises that the higher the perceived risk, the more likely an individual will modify their behaviour. The objectives of this study were to i) determine the distribution of type 2 diabetes patients regarding their willingness to accept training to speak to their offspring, ii) determine the distribution of type 2 diabetes patients regarding their willingness to accept training based on the HBM and iii) to determine the factors associated with their willingness to accept training.

    METHODS: This was a cross-sectional study amongst type 2 diabetes patients attending two primary care clinics in Malaysia. Sociodemographic data and knowledge of diabetes risk factors were collected. The adapted, translated and validated Diabetes Mellitus in the Offspring Questionnaire-Malay version (DMOQ-Malay) was self-administered. Statistical analysis included descriptive statistics, univariate and multiple logistic regression (MLogR).

    RESULTS: A total of 425 participants were recruited. Of these, 61.6% were willing to accept training. In MLogR, six variables were found to be significantly associated with willingness to accept training. These were i) positive family history [Adj. OR 2.06 (95% CI: 1.27, 3.35)], ii) having the correct knowledge that being overweight is a risk factor [Adj. OR 1.49 (95%CI: 1.01, 2.29)], iii) correctly identifying age ≥ 40 years old as a risk factor [Adj. OR 1.88 (95%CI: 1.22, 2.90)], iv) agreeing that speaking to their offspring would help them to prevent type 2 diabetes [Adj. OR 4.34 (95%: 1.07, 17.73)], v) being neutral with the statement 'I do not have much contact with my offspring' [Adj. OR: 0.31 (95% CI: 0.12, 0.810] and vi) being neutral with the statement 'my offspring are not open to advice from me' [Adj. OR: 0.63 (95% CI: 0.31, 0.84].

    CONCLUSION: The majority of type 2 diabetes patients were willing to accept training to speak to their offspring to prevent diabetes. A training module should be designed to enhance their knowledge, attitude and skills to become family health educators.

    Suspended sediment load (SSL) estimation is a required exercise in water resource management. This article proposes the use of hybrid artificial neural network (ANN) models, for the prediction of SSL, based on previous SSL values. Different input scenarios of daily SSL were used to evaluate the capacity of the ANN-ant lion optimization (ALO), ANN-bat algorithm (BA) and ANN-particle swarm optimization (PSO). The Goorganrood basin in Iran was selected for this study. First, the lagged SSL data were used as the inputs to the models. Next, the rainfall and temperature data were used. Optimization algorithms were used to fine-tune the parameters of the ANN model. Three statistical indexes were used to evaluate the accuracy of the models: the root-mean-square error (RMSE), mean absolute error (MAE) and Nash-Sutcliffe efficiency (NSE). An uncertainty analysis of the predicting models was performed to evaluate the capability of the hybrid ANN models. A comparison of models indicated that the ANN-ALO improved the RMSE accuracy of the ANN-BA and ANN-PSO models by 18% and 26%, respectively. Based on the uncertainty analysis, it can be surmised that the ANN-ALO has an acceptable degree of uncertainty in predicting daily SSL. Generally, the results indicate that the ANN-ALO is applicable for a variety of water resource management operations.

    It is possible that different techniques used to measure axial length (AL) and anterior chamber depth (ACD) is the cause of discrepancy in refractive outcomes of cataract surgery. This study evaluated the agreement and repeatability of AL and ACD measurements using immersion and contact A-scan biometry techniques and compared the refractive outcomes from both techniques. Twenty four patients were evaluated for agreement and repeatability of AL and ACD measurements using the two different methods. The results were analyzed using Bland and Altman plots. Another 60 patients with age-related cataract were selected to compare the refractive outcomes between both methods. The IOL power was calculated using Sanders- Retzlaff- Kraff- Theoretical (SRK-T) equation. Refraction was determined between four to six weeks postoperatively and the results were analyzed using paired t-test. The results of this study showed good agreement between both techniques was noted with no significant difference detected between measurements (p > 0.05). Significant correlation was found in all parameters (AL: r = 0.99; p < 0.01, r = 0.99; p < 0.01) ACD: r = 0.91; p < 0.01, r = 0.97; p < 0.01). No significant difference in refractive outcomes of post cataract surgery was detected between the two techniques (p = 0.07). This study concludes that contact A-scan biometry and immersion techniques provide reliable results and should not be the cause of discrepancy in the refractive planned and outcome of cataract surgery.

    Background: The construct of satisfaction with life has been studied across various cultures through the Satisfaction with Life Scale. The Satisfaction with Life Scale (SWLS) has been validated across several populations and languages. There are no published psychometric properties of its Urdu version. Hence, the aim of this study was to ascertain the psychometric properties of the Urdu version of the SWLS among the Urdu speaking population of Pakistan. Methods: The SWLS has already been translated into Urdu, and the Urdu version is available on the author's website however there is no information about its psychometric properties. To establish the psychometric properties especially the factor structure of the already translated Urdu SWLS, the SWLS-Urdu was administered to Urdu speaking population residing in Pakistan. The statistical analyses (i.e., normality through skewness and kurtosis, Kaiser-Meyer-Olkin (KMO) and Bartlett's test of sphericity, and test and re-test reliability) were conducted through SPSS version 25.0. Structure Equation Modelling via maximum likelihood method of estimation was used to perform confirmatory factor analysis on the data using AMOS 20.0. The significance level was set at p < 0.05. Results: The study was completed by recruiting 120 participants from different universities in Lahore, Pakistan. The sample was equally divided between male and female participants. The mean age of participants was 22.7(3.6) years. Test of the adequacy of the sample through Kaiser-Mayer-Olkin showed KMO=0.88 and Bartlett's test of sphericity (p<0.001). The Cronbach's alpha reliability of the scale was 0.90 and Confirmatory Factor Analysis confirmed a one-factor model as a good fit with strong statistical evidence. No factorial group variances were noticed in male and female participants. Conclusion: This study shows that Urdu SWLS has sound psychometric properties, is linguistically and culturally acceptable, and equally useful in assessing satisfaction with life in the Urdu speaking population.

    Leptospirosis is an emerging disease, especially in countries with a tropical climate such as Malaysia. A dramatic increase in the number of cases has been reported over the last decade; however, information on the epidemiological trends of this disease is lacking. The objective of this study is to provide an epidemiological description of human leptospirosis cases over a 9-year period (2004-2012) and disease relationship with meteorological, geographical, and demographical information. A retrospective study was undertaken to describe the patterns of human leptospirosis cases and their association with intrinsic (sex, age, and ethnicity) and extrinsic (location, rainfall, and temperature) factors. Data was grouped according to age, sex, ethnicity, seasonality and geographical distribution, and analyzed using statistical tools to understand the influence of all the different factors on disease incidence. A total of 12,325 cases of leptospirosis were reported between 2004 and 2012 with an upward trend in disease incidence, with the highest in 2012. Three hundred thirty-eight deaths were reported with an overall case fatality rate of 2.74%, with higher incidence in males (9696; 78.7%) compared with female patients (2629; 21.3%), and overall male to female ratio of 3.69:1. Patients aged cohorts between 30-39 years old (16.22 per 100,000 population) had the highest disease incidence while the lowest incidence occurred between <1 to 9 years old (3.44 per 100,000 population). The average incidence was highest amongst Malays (10.97 per 100,000 population), followed by Indians (7.95 per 100,000 population). Stratification according to geographical distribution showed that the state of Malacca had the highest average disease incidence (11.12 per 100,000 population) followed by Pahang (10.08 per 100,000 population). The states of Terengganu, Kelantan, and Perak recorded similar rates of incidence (≈8.00 per 100,000 population), while Johor with the least number of reported cases (1.80 per 100,000 population). Positive relationships were recorded between the number of reported cases with the number of raining days per month and monthly average temperature (p-value<0.05). However, no significant association was noted between rainfall volume and number of reported Leptospirosis cases. This collaborative efforts between medical, academic and governmental institutions has enabled the construction of this comprehensive database that is essential to understand the disease trends in Malaysia and add insights into the prevention and control of this disease.

    Evidence based on average patient data, which occupies most of our present day information databases, does not fulfil the needs of individual patient-centred health care. In spite of the unprecedented expansion in medical information we still do not have the types of information required to allow us to tailor optimal care for a given individual patient. As our current information is chiefly provided in disconnected silos, we need an information system that can seamlessly integrate different types of information to meet diverse user group needs. Groups of certain individual medical learners namely patients, medical students and health professionals share the patient's need to increasingly interact with and seek knowledge and solutions offered by others (individual medical learners) who have the lived experiences that they would benefit to access and learn from. A web-based user-driven learning solution may be a stepping-stone to address the present problem of information oversupply in medicine that mostly remains underutilized, as it doesn't meet the needs of the individual patient and health professional user. The key to its success would be to relax central control and make local trust and strategic health workers feel more engaged in the project such that it is truly user-driven.

    The identification of recurrent founder variants in cancer predisposing genes may have important implications for implementing cost-effective targeted genetic screening strategies. In this study, we evaluated the prevalence and relative risk of the CHEK2 recurrent variant c.349A>G in a series of 462 Portuguese patients with early-onset and/or familial/hereditary prostate cancer (PrCa), as well as in the large multicentre PRACTICAL case-control study comprising 55,162 prostate cancer cases and 36,147 controls. Additionally, we investigated the potential shared ancestry of the carriers by performing identity-by-descent, haplotype and age estimation analyses using high-density SNP data from 70 variant carriers belonging to 11 different populations included in the PRACTICAL consortium. The CHEK2 missense variant c.349A>G was found significantly associated with an increased risk for PrCa (OR 1.9; 95% CI: 1.1-3.2). A shared haplotype flanking the variant in all carriers was identified, strongly suggesting a common founder of European origin. Additionally, using two independent statistical algorithms, implemented by DMLE+2.3 and ESTIAGE, we were able to estimate the age of the variant between 2300 and 3125 years. By extending the haplotype analysis to 14 additional carrier families, a shared core haplotype was revealed among all carriers matching the conserved region previously identified in the high-density SNP analysis. These findings are consistent with CHEK2 c.349A>G being a founder variant associated with increased PrCa risk, suggesting its potential usefulness for cost-effective targeted genetic screening in PrCa families.

    Determination of a minimum sample size required for a study is a major consideration which all researchers are confronted with at the early stage of developing a research protocol. This is because the researcher will need to have a sound prerequisite knowledge of inferential statistics in order to enable him/her to acquire a thorough understanding of the overall concept of a minimum sample size requirement and its estimation. Besides type I error and power of the study, some estimates for effect sizes will also need to be determined in the process to calculate or estimate the sample size. The appropriateness in calculating or estimating the sample size will enable the researchers to better plan their study especially pertaining to recruitment of subjects. To facilitate a researcher in estimating the appropriate sample size for their study, this article provides some recommendations for researchers on how to determine the appropriate sample size for their studies. In addition, several issues related to sample size determination were also discussed.

    Statistical validation is crucial for the clustering of unknown samples. This study aims to demonstrate how statistical techniques can be optimized using simulated heroin samples containing a range of analyte concentrations that are similar to those of the case samples. Eight simulated heroin distribution links consisting of 64 postcut samples were prepared by mixing one of two mixtures of paracetamol-caffeine-dextromethorphan at different proportions with eight precut samples. Analyte contents and compositional variation of the prepared samples were investigated. A number of data pretreatments were evaluated by associating the postcut samples with the corresponding precut samples using principal component analysis and discriminant analysis. Subsequently, combinations of seven linkage methods and five distance measures were explored using hierarchical cluster analysis. In this study, Ward-Manhattan showed better distinctions between unrelated links and was able to cluster all related samples in very close distance under the known links on a dendogram. A similar discriminative outcome was also achieved by 90 unknown case samples when clustered via Ward-Manhattan.

    Statistical classification remains the most useful statistical tool for forensic chemists to assess the relationships between samples. Many clustering techniques such as principal component analysis and hierarchical cluster analysis have been employed to analyze chemical data for pattern recognition. Due to the feeble foundation of this statistics knowledge among novice drug chemists, a tetrahedron method was designed to simulate how advanced chemometrics operates. In this paper, the development of the graphical tetrahedron and computational matrices derived from the possible tetrahedrons are discussed. The tetrahedron method was applied to four selected parameters obtained from nine illicit heroin samples. Pattern analysis and mathematical computation of the differences in areas for assessing the dissimilarity between the nine tetrahedrons were found to be user-convenient and straightforward for novice cluster analysts.

    Drug-related problems (DRPs) in the elderly include polypharmacy, potentially inappropriate medications, nonadherence, and drug-related falls. In this systematic review and meta-analysis, the prevalence of DRPs and complementary and alternative medicine (CAM) use among the Malaysian elderly was estimated. PubMed, Scopus, Web of Science, and Google Scholar databases were searched to identify studies published since their inception up to 24 August 2020. A random-effects model was used to generate the pooled prevalence of DRPs along with its corresponding 95% confidence interval (CI). The heterogeneity of the results was estimated using the I2 statistics, and Cochran's Q test and sensitivity analyses were performed to confirm the robustness of the results. We identified 526 studies, 23 of which were included in the meta-analysis. (n = 29,342). The pooled prevalence of DRPs among Malaysian elderly was as follows: (1) polypharmacy: 49.5% [95% CI: 20.5-78.6], (2) potentially inappropriate medications: 28.9% [95% CI: 25.4-32.3], (3) nonadherence to medications: 60.6% [95% CI: 50.2-70.9], and (4) medication-related falls 39.3% [95% CI: 0.0-80.8]. Approximately one in two Malaysian elderly used CAM. The prevalence of polypharmacy and potentially inappropriate medications among the Malaysian elderly population was high, calling for measures and evidence-based guidelines to ensure the safe medication use.

    Introduction: The prevalence of knee osteoarthritis is on the raise. This raise has been a huge financial burden to developed countries in treating the disease. Transcutaneous electrical nerve stimulation (TENS) is a cost-effective, easily available, and self-applicable mode of non-pharmacological pain relieve technique. Despite these advantages, the use, settings, and effectiveness of portable TENS are still poorly understood. The aim of this study is to determine the effectiveness of portable TENS at different frequencies in treating knee osteoarthritis.

    Materials and Methods: This is a single-center quasi-experimental study involving 100 patients seen in the outpatient department with knee osteoarthritis. They were randomly (computer generated) allocated into two arms (high frequency [H-F] or low frequency [L-F]). H-F is set at 100 Hz and L-F is set at 4 Hz. A baseline assessment is taken with the visual analog score (VAS), Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), Oxford Knee Score, and Lequesne index. They were instructed to self-administer the TENS therapy as per protocol and followed up at the 4th and 12th week to be reevaluated on the above scores.

    Results: The final results show that both H-F and L-F groups showed improvement in all parameters of the VAS, WOMAC index, Oxford Knee Score, and Lequesne index (73%). Only the pain component of Lequesne index, activities of daily living component of Lequesne index, total Lequesne index, and pain component of WOMAC index shows a statistically significant difference, favoring the H-F group. The H-F group yields a faster result; however, with time the overall effect remains the same in both groups.

    Conclusion: Both H-F and L-F groups show improvement in all the component of Lequesne index, Oxford Knee Score, WOMAC index, and VAS with no statistical difference between the two groups. Although H-F yields a faster result, not everyone is able to tolerate the intensity. Therefore, the selection of H-F or L-F should be done on case basis depending on the severity of symptoms, patient's expectation, and patient's ability to withstand the treatment therapy. Based on this 12th week follow-up, both groups will continue to improve with time. A longer study should be conducted to see it this improvement will eventually plateau off or continue to improve until the patient is symptom free.

    Unmanned Aerial Vehicle (UAV) is one of the latest technologies for high spatial resolution 3D modeling of the Earth. The objectives of this study are to assess low-cost UAV data using image radiometric transformation techniques and investigate its effects on global and local accuracy of the Digital Surface Model (DSM). This research uses UAV Light Detection and Ranging (LIDAR) data from 80 meters and UAV Drone data from 300 and 500 meters flying height. RAW UAV images acquired from 500 meters flying height are radiometrically transformed in Matrix Laboratory (MATLAB). UAV images from 300 meters flying height are processed for the generation of 3D point cloud and DSM in Pix4D Mapper. UAV LIDAR data are used for the acquisition of Ground Control Points (GCP) and accuracy assessment of UAV Image data products. Accuracy of enhanced DSM with DSM generated from 300 meters flight height were analyzed for point cloud number, density and distribution. Root Mean Square Error (RMSE) value of Z is enhanced from ±2.15 meters to 0.11 meters. For local accuracy assessment of DSM, four different types of land covers are statistically compared with UAV LIDAR resulting in compatibility of enhancement technique with UAV LIDAR accuracy.

    126 Malaysian children, 65 boys and 61 girls from higher income families were followed-up regularly from birth to six years of age in the University Hospital, Kuala Lumpur. Their physical growth, development, dietary and illness patterns were measured at each visit. The study confirms the observations of previous studies that boys are, on the average, heavier and taller than girls and that Asians are smaller in size with relatively shorter legs compared with children of European ancestry. These racial differences are due to a combination of genetic and environmental differences. Since there are genetic differences in the size and shape of children, standards applicable to the specific population should be used to obtain the best results when assessing the health of an individual child. The growth charts presented in this paper can be used as standards to monitor the growth of Asian infants and pre-school children.
