This study investigated the effect of different Co3O4-based catalysts on the catalytic decomposition of nitrous oxide (N2O) and on nitric oxide (NO) conversion. The experiments were carried out using various reaction temperatures, alkaline solutions, pH, mixing conditions, aging times, space velocities, impregnation loads, and compounds. The results showed that Co3O4 catalysts prepared by precipitation methods have the highest catalytic activity and N2O conversion, even at low reaction temperatures, while the commercial nano and powder forms of Co3O4 (CS) have the lowest performance. The catalysts become inactive at temperatures below 400 °C, and their activity is strongly influenced by the mixing temperature. Samples without stirring during the aging process have higher catalytic activity than those with stirring, even at low reaction temperatures (200-300 °C). The catalytic activity of Co3O4 PM1 decreases with low W/F values and low reaction temperatures. Additionally, the catalyst's performance tends to increase with the reduction process. The study suggests that cobalt-oxide-based catalysts are effective in N2O catalytic decomposition and NO conversion. The findings may be useful in the design and optimization of catalytic systems for N2O and NO control. The results obtained provide important insights into the development of highly efficient, low-cost, and sustainable catalysts for environmental protection.

In this study, palm oil mill effluent (POME) was treated using electrocoagulation, whereby the influencing factors including voltage, electrolysis time, and electrolyte amount were optimized to achieve the highest chemical oxygen demand (COD) and color removal efficiencies. Graphite was selected as electrode material due to its performance better compared to aluminum and copper. Response surface methodology (RSM) was carried out for optimization of the electrocoagulation operating parameters. The best model obtained using Box-Behnken design (BBD) were quadratic for COD removal (R2 = 0.9844), color reduction (R2 = 0.9412), and oil and grease removal (R2 = 0.9724). The result from the analysis of variance (ANOVA) was obtained to determine the relationship between factors and treatment efficiencies. The experimental results under optimized conditions such as voltage 14, electrolysis time of 3 h, and electrolyte amount of 13.41 g/L show that the electrocoagulation process effectively reduced the COD (56%), color (65%), and oil and grease (99%) of the POME treatment. The rock formation of late Cretaceous-Paleocene metapsammite and metagranite found across Luk Ulo Complex indicated boulders with diameter of approximately 1 m and rounded shape along Luk Ulo River, Indonesia. However, less research found on geochronology and geochemistry has been conducted in study area, and such rocks require comprehensive understanding of magmatism and tectonic environment of Central Java, Indonesia. Therefore, the main objective of this study is to address the geochemical and geochronological age histories across Central Java, Indonesia, using U-Pb zircon dating technique. Generally, most common types of rocks were observed which composed of hornblende and garnet-bearing metapsammite and metagranite. The geochemical study showed that protolith of rocks with hornblende was identified as Cordilleran granitoid (I-type), which originated from magmatic arc with basaltic differentiation. Furthermore, protolith of rocks containing garnet was categorized as Caledonian granitoid (S-type), which is caused by post-collisional orogeny. The cluster observations of magmatic zircons reveal their magmatic ages, which vary from 67.00 ± 1.2 to 69.10 ± 0.91 Ma (late Cretaceous), whereas ages of inherited zircons ranged from 100 ± 5 to 437 ± 13 Ma (early Cretaceous to Silurian). Estimated periods of partial melting were found between 100 ± 5 Ma and 118 ± 4 Ma (early Cretaceous). Comparing the zircon ages of Luk Ulo with the zircon ages from the Sundaland regions reveals that the age distribution patterns are incredibly similar; the peak ages dispersed between the Cretaceous and Triassic periods, as well as Sundaland region was the source of the materials.

Control of ticks and tick-borne pathogens is a priority for human and animal health. Livestock-holders extensively rely on acaricide applications for tick control. Different groups of acaricides including cypermethrin and amitraz have been consistently used in Pakistan. There has been a gap in understanding the susceptibility or resistance of Rhipicephalus microplus, the most prevalent tick in Pakistan, to acaricides. The present study aimed to molecularly characterize cypermethrin and amitraz targeted genes such as voltage-gated sodium channel (VGSC) and octopamine tyramine (OCT/Tyr) of R. microplus ticks in Khyber Pakhtunkhwa (KP), Pakistan to monitor the acaricides resistance. Tick specimens were collected from cattle and buffaloes in northern (Chitral, Shangla, Swat, Dir, and Buner), central (Peshawar, Mardan, Charsadda, Swabi, and Nowshera), and southern districts (Kohat, Karak, Lakki Marwat, Tank, and Dera Ismail Khan) of KP, Pakistan. Different concentrations of commercially available cypermethrin (10%) and amitraz (12.5%) were prepared for in vitro larval immersion tests (LIT). In LIT, the average mortality rate of immersed larvae was recorded that was increased gradually with an increase in the concentration of specific acaricide. The larvae's highest mortality rates (94.5% and 79.5%) were observed at 100-ppm of cypermethrin and amitraz, respectively. A subset of 82 R. microplus ticks was subjected to extract genomic DNA, followed by PCR to amplify partial fragments of VGSC (domain-II) and OCT/Tyr genes. The BLAST results of the consensus sequence of VGSC gene (domain-II) showed 100% identity with the acaricides susceptible tick sequence from the United States (reference sequence). Obtained identical sequences of OCT/Tyr genes showed maximum identity (94-100%) with the identical sequences reported from Australia (reference sequence), India, Brazil, Philippines, USA, South Africa, and China. Thirteen single nucleotide polymorphisms (10 synonymous and three non-synonymous) were observed at various positions of partial OCT/Tyr gene fragments. The SNP at position A-22-C (T-8-P) in OCT/Tyr gene has been linked to amitraz resistance in R. microplus ticks. Molecular analysis and LIT bioassay's findings indicate the availability of resistant R. microplus ticks in the KP region. To our understanding, this is the first preliminary study to monitor cypermethrin and amitraz resistance via molecular profiling of cypermethrin and amitraz targeted genes (VGSC and OCT/Tyr) in combination with in vitro bioassays (LIT) in R. microplus ticks from Pakistan.

INTRODUCTION: Necrotizing fasciitis (NF) is a rapidly progressive necrosis of the fascial layer with a high mortality rate. It is a life-threatening medical emergency that requires urgent treatment. Lack of skin finding in NF made diagnosis difficult and required a high clinical index of suspicion. The use of ultrasound may guide clinicians in improving diagnostic speed and accuracy, thus leading to improved management decisions and patient outcomes. This literature search aims to review the use of point-of-care ultrasonography in diagnosing necrotizing fasciitis.

METHOD: We searched relevant electronic databases, including PUBMED, MEDLINE, and SCOPUS, and performed a systematic review. Keywords used were "necrotizing fasciitis" or "necrotising fasciitis" or "necrotizing soft tissue infections" and "point-of-care ultrasonography" "ultrasonography" or "ultrasound". No temporal limitation was set. An additional search was performed via google scholar, and the top 100 entry was screened.

RESULTS: Among 540 papers screened, only 21 were related to diagnosing necrotizing fasciitis using ultrasonography. The outcome includes three observational studies, 16 case reports, and two case series, covering the period from 1976 to 2022.

CONCLUSION: Although the use of ultrasonography in diagnosing NF was published in several papers with promising results, more studies are required to investigate its diagnostic accuracy and potential to reduce time delay before surgical intervention, morbidity, and mortality.

BACKGROUND: Safety and efficacy concerns regarding coronavirus disease 2019 (COVID-19) vaccines are common among the public and have a negative impact on their uptake. We aimed to report the adverse effects currently associated with the vaccine in Pakistan to build confidence among the population for its adoption.

METHODS: A cross-sectional study was conducted in five districts of the Punjab province of Pakistan between January and March 2022. The participants were recruited using convenience sampling. All data were analyzed using SPSS 22.

RESULTS: We recruited 1622 people with the majority aged between 25-45 years. Of these, 51% were female, including 27 pregnant women and 42 lactating mothers. Most participants had received the Sinopharm (62.6%) or Sinovac (17.8%) vaccines. The incidences of at least one side effect after the first (N = 1622), second (N = 1484), and booster doses (N = 219) of the COVID-19 vaccine were 16.5%, 20.1%, and 32%, respectively. Inflammation/erythema at the injection site, pain at the injection site, fever, and bone/muscle pain were common side effects of vaccination. No significant differences were observed in the adverse effect scores between all demographic variables except for pregnancy (P = 0.012) after the initial dose. No significant association was observed between any variable and the side effect scores of the second and booster doses of the vaccine.

CONCLUSIONS: Our study showed a 16-32% prevalence of self-reported side effects after the first, second, and booster COVID-19 vaccinations. Most adverse effects were mild and transient, indicating the safety of different COVID-19 vaccines.

Microbial fuel cells (MFCs) seem to have emerged in recent years to degrade the organic pollutants from wastewater. The current research also focused on phenol biodegradation using MFCs. According to the US Environmental Protection Agency (EPA), phenol is a priority pollutant to remediate due to its potential adverse effects on human health. At the same time, the present study focused on the weakness of MFCs, which is the low generation of electrons due to the organic substrate. The present study used rotten rice as an organic substrate to empower the MFC's functional capacity to degrade the phenol while simultaneously generating bioenergy. In 19 days of operation, the phenol degradation efficiency was 70% at a current density of 17.10 mA/m2 and a voltage of 199 mV. The electrochemical analysis showed that the internal resistance was 312.58 Ω and the maximum specific capacitance value was 0.00020 F/g on day 30, which demonstrated mature biofilm production and its stability throughout the operation. The biofilm study and bacterial identification process revealed that the presence of conductive pili species (Bacillus genus) are the most dominant on the anode electrode. However, the present study also explained well the oxidation mechanism of rotten rice with phenol degradation. The most critical challenges for future recommendations are also enclosed in a separate section for the research community with concluding remarks.

Nanofluids have attracted significant attention from researchers due to their ability to significantly enhance heat transfer, especially in jet impingement flows, which can improve their cooling performance. However, there is a lack of research on the use of nanofluids in multiple jet impingements, both in terms of experimental and numerical studies. Therefore, further investigation is necessary to fully understand the potential benefits and limitations of using nanofluids in this type of cooling system. Thus, an experimental and numerical investigation was performed to study the flow structure and heat transfer behavior of multiple jet impingement using MgO-water nanofluids with a 3 × 3 inline jet array at a nozzle-to-plate distance of 3 mm. The jet spacing was set to 3, 4.5, and 6 mm; the Reynolds number varies from 1000 to 10,000; and the particle volume fraction ranges from 0% to 0.15%. A 3D numerical analysis using ANSYS Fluent with SST k-ω turbulent model was presented. The single-phase model is adopted to predict the thermal physical nanofluid. The flow field and temperature distribution were investigated. Experimental results show that a nanofluid can provide a heat transfer enhancement at a small jet-to-jet spacing using a high particle volume fraction under a low Reynolds number; otherwise, an adverse effect on heat transfer may occur. The numerical results show that the single-phase model can predict the heat transfer trend of multiple jet impingement using nanofluids correctly but with significant deviation from experimental results because it cannot capture the effect of nanoparticles.

The effect of a nontoxic chloride treatment on the crystallinity and optoelectrical characteristics of a CdSe thin film was studied. A detailed comparative analysis was conducted utilizing four molarities (0.01 M, 0.10 M, 0.15 M, and 0.20 M) of indium (III) chloride (InCl3), where the results showed a notable improvement in CdSe properties. The crystallite size of treated CdSe samples increased from 31.845 nm to 38.819 nm, and the strain in treated films dropped from 4.9 × 10-3 to 4.0 × 10-3, according to XRD measurements. The highest crystallinity resulted from the 0.10 M InCl3-treated CdSe films. The In contents in the prepared samples were verified by compositional analysis, and FESEM images from treated CdSe thin films demonstrated compact and optimal grain arrangements with passivated grain boundaries, which are required for the development of a robust operational solar cell. The UV-Vis plot, similarly, showed that the samples were darkened after treatment and the band gap of 1.7 eV for the as-grown samples fell to roughly 1.5 eV. Furthermore, the Hall effect results suggested that the carrier concentration increased by one order of magnitude for samples treated with 0.10 M of InCl3, but the resistivity remained in the order of 103 ohm/cm2, suggesting that the indium treatment had no considerable effect on resistivity. Hence, despite the deficit in the optical results, samples treated at 0.10 M InCl3 showed promising characteristics as well as the viability of treatment with 0.10 M InCl3 as an alternative to standard CdCl2 treatment.

Transcutaneous vaccination is one of the successful, affordable, and patient-friendly advanced immunization approaches because of the presence of multiple immune-responsive cell types in the skin. However, in the absence of a preferable facilitator, the skin's outer layer is a strong impediment to delivering biologically active foreign particles. Lipid-based biocompatible ionic-liquid-mediated nanodrug carriers represent an expedient and distinct strategy to permit transdermal drug delivery; with acceptable surfactants, the performance of drug formulations might be further enhanced. For this purpose, we formulated a lipid-based nanovaccine using a conventional (cationic/anionic/nonionic) surfactant loaded with an antigenic protein and immunomodulator in its core to promote drug delivery by penetrating the skin and boosting drug delivery and immunogenic cell activity. In a follow-up investigation, a freeze-dry emulsification process was used to prepare the nanovaccine, and its transdermal delivery, pharmacokinetic parameters, and ability to activate autoimmune cells in the tumor microenvironment were studied in a tumor-budding C57BL/6N mouse model. These analyses were performed using ELISA, nuclei and HE staining, flow cytometry, and other biological techniques. The immunomodulator-containing nanovaccine significantly (p < 0.001) increased transdermal drug delivery and anticancer immune responses (IgG, IgG1, IgG2, CD8+, CD207+, and CD103+ expression) without causing cellular or biological toxicity. Using a nanovaccination approach, it is possible to create a more targeted and efficient delivery system for cancer antigens, thereby stimulating a stronger immune response compared with conventional aqueous formulations. This might lead to more effective therapeutic and preventative outcomes for patients with cancer.

Novel solid-supported ionic liquid (Si-Sal-SSIL) was synthesized by immobilization of 1-methyl-3-(3-trimethoxysilylpropylimidazolium) salicylate [MTMSPI][Sal] ionic liquid onto the activated silica gel. First, the [MTMSPI][Sal] ionic liquid was derived from the reaction of a metathesis product of 1-methyl-3-(3-trimethoxysilylpropylimidazolium) chloride [MTMSPI][Cl] with sodium salicylate through an ion-exchanged reaction. [MTMSPI][Sal] was purified and characterized through ion-chromatography, CHN and Karl-Fischer titration analyses. Further characterizations on [MTMSPI][Sal] were carried out by 1H NMR and FTIR analyses. Si-Sal-SSIL was successfully prepared and confirmed through BET and solid-state NMR analyses. Si-Sal-SSIL showed better removal capacities towards Pb(ii) and Ni(ii) ions in comparison to native activated silica gel. Si-Sal-SSIL was then applied as solid adsorbent for an efficient removal of Pb(ii) and Ni(ii) from the aqueous solution. A series of batch sorption study were performed to explore the influence of parameters i.e., loading ratio of activated silica gel to [MTMSPI][Sal], pH, mixing time, initial concentration of analyte towards the adsorption of Pb(ii) and Ni(ii) ions onto Si-Sal-SSIL as a function of removal efficiency. Under optimized conditions, the sorption kinetics for removal of both metals agreed with pseudo-second order linear plots. The mechanism of Pb(ii) and Ni(ii) sorption by Si-Sal-SSIL gave good fits for Langmuir model.

The toxicity of histamine has attracted numerous researchers to develop a method for histamine determination purposes. The Food and Drug Administration (FDA) unequivocally prohibits the consumption of histamine above 50 mg·kg-1. Thus, an innovation in histamine detection in fish has been developed in this research. The investigation of the histamine level in fish has been conducted by using an electrochemical sensor approach and producing a polymer via molecularly imprinted polymer (MIP) on a screen-printed electrode. The technique was validated by assessing the shifts in electron shifting using the cyclic voltammetry (CV) approach and electrochemical impedance spectroscopy (EIS), whereas differential pulse voltammetry (DPV) was applied to validate the sensor method. The instruments showed a linear response ranging from 1-1000 nmol·L-1, with a detection limit of MIP/SPE at 1.765 nmol·L-1 and 709 nmol·L-1 for the NIP/SPE, respectively. The sensing technique was employed to determine the histamine level in selected samples at room temperature (25°C). The outcomes of this study indicated that the validated chemical sensor allowed accurate and precise detection of fish samples and can be categorized as a simple approach. The instrument is inexpensive and suitable for on-site detection.

INTRODUCTION: Melioidosis is a deadly endemic disease in northern Australia and Southeast Asia, including Sabah, Malaysia, which is caused by the bacterium Burkholderia pseudomallei. It contributes to high fatality rates, mainly due to misdiagnosis leading to the wrong treatment being administered to the patients. Local epidemiology and data on clinical features could assist clinicians during diagnosis and treatment. However, these details are still scarce, particularly in Sabah.

METHODS: A retrospective study of 246 culture-confirmed melioidosis cases in Queen Elizabeth Hospital, Sabah, Malaysia was performed between 2016 and 2018. The epidemiological data and clinical and laboratory findings were extracted and analysed.

RESULTS: The annual incidence of culture-confirmed melioidosis cases was estimated to be 4.97 per 100,000 people. The mean age of the patients was 50±15 years. Males and members of the Kadazan-Dusun ethnic group accounted for the majority of the melioidosis cases. The odds ratio analysis indicated that bacteraemic melioidosis in this region was significantly associated with fever (76%), and patients having at least one underlying illness (43%), including diabetes mellitus (32%). Sixty-eight patients (28%) succumbed to melioidosis. Contrary to what is known regarding factors that promote bacteraemic melioidosis, neither patients with fever nor patients with at least one comorbid disease, including diabetes mellitus, were significantly associated with death from melioidosis. There was no statistically significant difference between patients without comorbidities (24, 27%) and those with at least one comorbid disease (26, 25%), including diabetes mellitus (18, 23%). The odds ratios indicate that melioidosis mortality in this region is related to patients showing respiratory organ-associated symptoms (29%), bacteraemia (30%), and septic shock (47%). Burkholderia pseudomallei isolates in this study were highly susceptible to ceftazidime (100%), imipenem (100%), and trimethoprim-sulfamethoxazole (98%).

CONCLUSIONS: Information obtained from this study can be used by clinicians to recognise individuals with the highest risk of acquiring melioidosis, estimate an accurate prognosis, and provide effective treatment for melioidosis patients to reduce death from melioidosis.

Environmental issues such as environmental pollutions and climate change are the impacts of globalization and become debatable issues among academics and industry key players. One of the environmental issues which is air pollution has been catching attention among industrialists, researchers, and communities around the world. However, it has always neglected until the impacts on human health become worse, and at times, irreversible. Human exposure to air pollutant such as particulate matters, sulfur dioxide, ozone and carbon monoxide contributed to adverse health hazards which result in respiratory diseases, cardiorespiratory diseases, cancers, and worst, can lead to death. This has led to a spike increase of hospitalization and emergency department visits especially at areas with worse pollution cases that seriously impacting human life and health. To address this alarming issue, a predictive model of air pollution is crucial in assessing the impacts of health due to air pollution. It is also critical in predicting the air quality index when assessing the risk contributed by air pollutant exposure. Hence, this systemic review explores the existing studies on anticipating air quality impact to human health using the advancement of Artificial Intelligence (AI). From the extensive review, we highlighted research gaps in this field that are worth to inquire. Our study proposes to develop an AI-based integrated environmental and health impact assessment system using federated learning. This is specifically aims to identify the association of health impact and pollution based on socio-economic activities and predict the Air Quality Index (AQI) for impact assessment. The output of the system will be utilized for hospitals and healthcare services management and planning. The proposed solution is expected to accommodate the needs of the critical and prioritization of sensitive group of publics during pollution seasons. Our finding will bring positive impacts to the society in terms of improved healthcare services quality, environmental and health sustainability. The findings are beneficial to local authorities either in healthcare or environmental monitoring institutions especially in the developing countries.

Chemical dispersants are extensively used for marine oil spill remediation. However, the increased toxicity and low biodegradability of these dispersants restrict their employment in the marine environment. Hence, in this work, we have developed an eco-friendly formulation composed of an ionic liquid,1-butyl-3-methylimidazolium lauroyl sarcosinate [BMIM][Lausar] and sorbitan monooleate (Span) 80. Micellar and interfacial parameters, dispersion effectiveness, as well as the toxicity and biodegradability of the developed formulation were investigated. Micellar properties confirmed a high degree of synergism among the surfactant molecules and the formation of stable micelle. The dispersion effectiveness, at dispersant-to-oil ratio (DOR) of 1:25 (v/v), against three crude oils (Arab, Ratawi, and Doba) was assessed. We achieved a dispersion effectiveness of 68.49%, 74.05%, and 83.43% for Ratawi, Doba, and Arab crude oil, respectively, using a 70:30 (w/w) ratio of Span 80 to [BMIM][Lausar]. Furthermore, the results obtained from optical microscopy and particle size analysis (PSA) indicated that the oil droplet size decreased with higher DOR. Additionally, acute toxicity experiments were conducted on zebrafish (Danio rerio) using the developed formulation, confirming its non-toxic behavior, with LC50 values of 800 mg/L after 96 h. The formulation also exhibited high biodegradability, with only 25.01% of the original quantity remaining after 28 days. Hence, these results suggest that the new formulation has the potential to be a highly effective and environmentally friendly dispersant for oil spill remediation.

Osteoblasts, cells derived from mesenchymal stem cells (MSCs) in the bone marrow, are cells responsible for bone formation and remodeling. The differentiation of osteoblasts from MSCs is triggered by the expression of specific genes, which are subsequently controlled by pro-osteogenic pathways. Mature osteoblasts then differentiate into osteocytes and are embedded in the bone matrix. Dysregulation of osteoblast function can cause inadequate bone formation, which leads to the development of bone disease. Various key molecules are involved in the regulation of osteoblastogenesis, which are transcription factors. Previous studies have heavily examined the role of factors that control gene expression during osteoblastogenesis, both in vitro and in vivo. However, the systematic relationship of these transcription factors remains unknown. The involvement of ncRNAs in this mechanism, particularly miRNAs, lncRNAs, and circRNAs, has been shown to influence transcriptional factor activity in the regulation of osteoblast differentiation. Here, we discuss nine essential transcription factors involved in osteoblast differentiation, including Runx2, Osx, Dlx5, β-catenin, ATF4, Ihh, Satb2, and Shn3. In addition, we summarize the role of ncRNAs and their relationship to these essential transcription factors in order to improve our understanding of the transcriptional regulation of osteoblast differentiation. Adequate exploration and understanding of the molecular mechanisms of osteoblastogenesis can be a critical strategy in the development of therapies for bone-related diseases.Communicated by Ramaswamy H. The usage of cladocerans as non-model organisms in ecotoxicological and risk assessment studies has intensified in recent years due to their ecological importance in aquatic ecosystems. The molecular assessment such as gene expression analysis has been introduced in ecotoxicological and risk assessment to link the expression of specific genes to a biological process in the cladocerans. The validity and accuracy of gene expression analysis depends on the quantity, quality and integrity of extracted ribonucleic acid (RNA) of the sample. However, the standard methods of RNA extraction from the cladocerans are still lacking. This study evaluates the extraction of RNA from tropical freshwater cladocerans Moina micrura using two methods: the phenol-chloroform extraction method (QIAzol) and a column-based kit (Qiagen Micro Kit). Glycogen was introduced in both approaches to enhance the recovery of extracted RNA and the extracted RNA was characterised using spectrophotometric analysis (NanoDrop), capillary electrophoresis (Bioanalyzer). Then, the extracted RNA was analysed with reverse transcription polymerase chain reaction (RT-PCR) to validate the RNA extraction method towards downstream gene expression analysis. The results indicate that the column-based kit is most suitable for the extraction of RNA from M. micrura, with the quantity (RNA concentration = 26.90 ± 6.89 ng/μl), quality (A260:230 = 1.95 ± 0.15, A280:230 = 1.85 ± 0.09) and integrity (RNA integrity number, RIN = 7.20 ± 0.16). The RT-PCR analysis shows that the method successfully amplified both alpha tubulin and actin gene at 33-35 cycles (i.e. Ct = 32.64 to 33.48). The results demonstrate that the addition of glycogen is only suitable for the phenol-chloroform extraction method. RNA extraction with high and comprehensive quality control assessment will increase the accuracy and reliability of downstream gene expression, thus providing more ecotoxicological data at the molecular biological level on other freshwater zooplankton species.

Rapid outbreak of coronavirus disease 2019 has caused the implementation of the movement control order (MCO) which aimed to reduce the spread in Covid-19 infections. While some may find it easy to adjust to the new norm, others found it difficult to switch from their normal routines and habits as according to the MCO SOP. This resulted in a more frequent insomnia and depression that subsequently impacted their mental health. Insomnia and depression levels are examined in this study as they relate to the Covid-19 Pandemic and the MCO among Malaysian undergraduate health sciences students at the Faculty of Medicine and Health Sciences, UPM. Random sampling methods were utilised with consideration of inclusion and exclusion criteria. The Patient Health Questionnaire-9 (PHQ-9) and the Insomnia Severity Index (ISI) were the instrument packages used in this investigation. An internet platform was used to distribute the questionnaire. Based on the results, it is concluded that depression and insomnia are significantly correlated, with a p-value of 0.05. This study also revealed the link between the severity of insomnia and the severity of depression among UPM students studying health sciences. The percentage of students with depression and insomnia was rather high (54.9% and 33.9%, respectively), and this occurred during the second wave of Covid-19 cases in Malaysia.

Delirium, a common form of acute brain dysfunction, is associated with increased morbidity and mortality, especially in older patients. The underlying pathophysiology of delirium is not clearly understood, but acute systemic inflammation is known to drive delirium in cases of acute illnesses, such as sepsis, trauma, and surgery. Based on psychomotor presentations, delirium has three main subtypes, such as hypoactive, hyperactive, and mixed subtype. There are similarities in the initial presentation of delirium with depression and dementia, especially in the hypoactive subtype. Hence, patients with hypoactive delirium are frequently misdiagnosed. The altered kynurenine pathway (KP) is a promising molecular pathway implicated in the pathogenesis of delirium. The KP is highly regulated in the immune system and influences neurological functions. The activation of indoleamine 2,3-dioxygenase, and specific KP neuroactive metabolites, such as quinolinic acid and kynurenic acid, could play a role in the event of delirium. Here, we collectively describe the roles of the KP and speculate on its relevance in delirium.
