    Triage is a process by which patients are assessed, classified, and sorted based on their presenting complaint and clinical urgency, providing assurance for timely access to emergency care. The goal is to get the right person to the right place, in the right amount of time, for the right reason, and within the context of resource availability. In many countries, a standardized triage system, underpinned through the use of guidelines, is used to provide clinicians with support and guidance. Triage is a globally adopted principle, and although triage guidelines are used in many countries, no single system has been internationally adopted. This paper discusses the importance of how triage process standardization improves patient care, resource management, and benchmarking at local, national, and international levels by applying 5 internationally recognized triage systems to fictional case studies. Evaluation of similarities and differences in severity scores, with a gap analysis, occurs.

    Melioidosis is a potentially life-threatening infection caused by the Gram-negative bacillus Burkholderia pseudomallei. Mediastinal melioidosis has a range of clinical presentations, making it difficult to diagnose: we therefore reviewed the evidence on the clinical characteristics, radiological features and invasive diagnostic modalities or interventions. An electronic search was conducted on three databases (PubMed, SCOPUS, Google Scholar) from November to December 2022. The initial search yielded 120 results, of which 34 studies met the inclusion criteria, but only 31 full-texts were retrievable. Among these, 4 were cohort studies, 26 case reports or series and 1 a conference abstract. The four main themes covered were mediastinal melioidosis as a diagnostic dilemma, unexpected complications, invasive interventions or an accompanying thoracic feature. Radiological manifestations included matting, necrosis and abscess-like collection. Severe presentations of mediastinal melioidosis included superior vena cava obstruction, sinus tract formation and pericardial tamponade. Transbronchial needle aspiration was the most common invasive diagnostic modality. Further research is needed to understand the relationship between the thoracic features of melioidosis on patient prognosis, its relationship to melioidosis transmission and potential preventive measures.

    The food choices in childhood have high a probability of being carried through into their adulthood life, which then contributes to the risk of many non-communicable diseases. Therefore, there is a need to gather some information about children's views on foods which may influence their food choices for planning a related dietary intervention or programme. This paper aimed to explore the views of children on foods and the types of foods which are usually consumed by children under four food groups (snacks, fast foods, cereals and cereal products; and milk and dairy products) by using focus group discussions. A total of 33 school children aged 7-9 years old from Selangor and Kuala Lumpur participated in the focus groups. Focus groups were audio-taped, transcribed and analyzed according to the listed themes. The outcomes show that the children usually consumed snacks such as white bread with spread or as a sandwich, local cakes, fruits such as papaya, mango and watermelon, biscuits or cookies, tea, chocolate drink and instant noodles. Their choices of fast foods included pizza, burgers, French fries and fried chicken. For cereal products, they usually consumed rice, bread and ready-to-eat cereals. Finally, their choices of dairy products included milk, cheese and yogurt. The reasons for the food liking were taste, nutritional value and the characteristics of food. The outcome of this study may provide additional information on the food choices among Malaysian children, especially in urban areas with regard to the food groups which have shown to have a relationship with the risk of childhood obesity.

    Currently, many countries all over the world are facing the second wave of COVID-19. Therefore, this study aims to analyze the spatial distribution of COVID-19 cases, epidemic spread rate, spatial pattern during the first to the second waves in the South Sumatra Province of Indonesia. This study used the geographical information system (GIS) software to map the spatial distribution of COVID-19 cases and epidemic spread rate. The spatial autocorrelation of the COVID-19 cases was carried out using Moran's I, while the Pearson correlation was used to examining the relationship between meteorological factors and the epidemic spread rate. Most infected areas and the direction of virus spread were predicted using wind rose analysis. The results revealed that the epidemic rapidly spread from August 1 to December 1, 2020. The highest epidemic spread rate was observed in the Palembang district and in its peripheral areas (dense urban areas), while the lowest spread rate was found in the eastern and southern parts of South Sumatra Province (remote areas). The spatial correlation characteristic of the epidemic distribution exhibited a negative correlation and random distribution. Air temperature, wind speed, and precipitation have contributed to a significant impact on the high epidemic spread rate in the second wave. In summary, this study offers new insight for arranging control and prevention strategies against the potential of second wave strike.

    INTRODUCTION: There is increasing evidence that COVID-19 has unmasked the true magnitude of health inequity worldwide. Policies and guidance for containing the infection and reducing the COVID-19 related deaths have proven to be effective, however the extent to which health inequity factors were considered in these policies is rather unknown. The aim of this study is to measure the extent to which COVID-19 related policies reflect equity considerations by focusing on the global policy landscape around wearing masks and personal protection equipment (PPE).

    METHODS: A systematic search for published documents on COVID-19 and masks/PPE was conducted across six databases: PubMed, EMBASE, CINAHL, ERIC, ASSIA and Psycinfo. Reviews, policy documents, briefs related to COVID-19 and masks/PPE were included in the review. To assess the extent of incorporation of equity in the policy documents, a guidance framework known as 'PROGRESS-Plus': Place of residence, Race/ethnicity, Occupation, Gender/sex, Religion, Education, Socioeconomic status, Social capital, Plus (age, disability etc.) was utilized.

    RESULTS: This review included 212 policy documents. Out of 212 policy documents, 190 policy documents (89.62%) included at least one PROGRESS-plus component. Most of the policy documents (n = 163, 85.79%) focused on "occupation" component of the PROGRESS-plus followed by personal characteristics associated with discrimination (n = 4;2.11%), place of residence (n = 2;1.05%) and education (n = 1;0.53%). Subgroup analysis revealed that most of the policy documents (n = 176, 83.01%) were focused on "workers" such as healthcare workers, mortuary workers, school workers, transportation workers, essential workers etc. Of the remaining policy documents, most were targeted towards whole population (n = 30; 14.15%). Contrary to "worker focused" policy documents, most of the 'whole population focused' policy documents didn't have a PROGRESS-plus equity component rendering them equity limiting for the society.

    CONCLUSION: Our review highlights even if policies considered health inequity during the design/implementation, this consideration was often one dimensional in nature. In addition, population wide policies should be carefully designed and implemented after identifying relevant equity related barriers in order to produce better outcomes for the whole society.

    In recent years, there has been an immense amount of research into fall event detection. Generally, a fall event is defined as a situation in which a person unintentionally drops down onto a lower surface. It is crucial to detect the occurrence of fall events as early as possible so that any severe fall consequences can be minimized. Nonetheless, a fall event is a sporadic incidence that occurs seldomly that is falsely detected due to a wide range of fall conditions and situations. Therefore, an automated fall frame detection system, which is referred to as the SmartConvFall is proposed to detect the exact fall frame in a video sequence. It is crucial to know the exact fall frame as it dictates the response time of the system to administer an early treatment to reduce the fall's negative consequences and related injuries. Henceforth, searching for the optimal training configurations is imperative to ensure the main goal of the SmartConvFall is achieved. The proposed SmartConvFall consists of two parts, which are object tracking and instantaneous fall frame detection modules that rely on deep learning representations. The first stage will track the object of interest using a fully convolutional neural network (CNN) tracker. Various training configurations such as optimizer, learning rate, mini-batch size, number of training samples, and region of interest are individually evaluated to determine the best configuration to produce the best tracker model. Meanwhile, the second module goal is to determine the exact instantaneous fall frame by modeling the continuous object trajectories using the Long Short-Term Memory (LSTM) network. Similarly, the LSTM model will undergo various training configurations that cover different types of features selection and the number of stacked layers. The exact instantaneous fall frame is determined using an assumption that a large movement difference with respect to the ground level along the vertical axis can be observed if a fall incident happened. The proposed SmartConvFall is a novel technique as most of the existing methods still relying on detection rather than the tracking module. The SmartConvFall outperforms the state-of-the-art trackers, namely TCNN and MDNET-N trackers, with the highest expected average overlap, robustness, and reliability metrics of 0.1619, 0.6323, and 0.7958, respectively. The SmartConvFall also managed to produce the lowest number of tracking failures with only 43 occasions. Moreover, a three-stack LSTM delivers the lowest mean error with approximately one second delay time in locating the exact instantaneous fall frame. Therefore, the proposed SmartConvFall has demonstrated its potential and suitability to be implemented for a real-time application that could help to avoid any crucial fall consequences such as death and internal bleeding if the early treatment can be administered.

    Background: The objectives of this study were to investigate risk perceptions and travel intention among the general public in Taiwan during the COVID-19 outbreak. Methods: This study used a cross-sectional online survey to collect data. The questionnaire was disseminated via the social media platform (LINE and Facebook) to the general public. Results: A total of 3,237 complete responses were received, of whom 5.8% (95% CI 5.1-6.7) of the participants reported intent to travel to overseas countries with an apparent community spread and 5.5% (95% CI 4.7-6.3) reported intent to travel to other overseas countries in the next 1 month. A relatively higher proportion (46.5%; 95% CI 44.7-48.2) reported intention for domestic travelling. Participants who viewed travelling to only be risky for older adults or those with medical conditions (OR = 2.19; 95% CI 1.38-3.47) and who perceived that one will not get infected if one takes recommended precautionary measures (OR = 3.12; 95% CI 1.85-5.27) reported higher travelling intention to overseas countries with an apparent community spread. Conclusions: Overall, the findings suggest that risk perceptions were depicted as a strong influence of travel intentions.

    Due to the rapid increase in the use of electrical and electronic equipment (EEE) worldwide, e-waste has become a critical environmental issue for many governments around the world. Several studies have pointed out that failure to adopt appropriate recycling practices for e-waste may cause environmental disasters and health concerns to humans due to the presence of hazardous materials. This warrants the need for a review of the existing processes of e-waste management. In view of the growing e-waste generation in the Asia Pacific region and the importance of e-waste management, this study critically reviews previous research on e-waste generation and management practices of major e-waste producing nations (Australia, China, India, Indonesia, and Malaysia) in the Asia Pacific region, provides an overview of progress made and identifies areas for improvement. To fulfil the aims of this research, previous studies from 2005 to 2020 are collected from various databases. Accordingly, this study focuses on e-waste generation and environmental management of these countries. This study found that e-waste management practices of the selected countries need to be enhanced and recommends several best practices for effectively managing e-waste.

    In addition to social and environmental factors, individual personality traits have intricately linked with maladaptive behaviour. Thus, the purpose of this article was to review the link between individual personality traits and criminality. A systematic review was conducted to obtain information regarding the link between individual personality traits with criminal behaviour in the Sage, Web of Science, APA PsycNet, Wiley Online Library, and PubMed databases. The results indicate that individual personality traits that contribute towards criminality are (i) psychopathy; (ii) low self-control; and (iii) difficult temperament. As an overall impact, the review is expected to provide in-depth understanding of the link between individual personality traits and criminality; hence, greater consideration will be given to the dimension of personality as a notable risk factor of criminal behaviour.

    Machine learning algorithms (ML) are receiving a lot of attention in the development of predictive models for monitoring dengue transmission rates. Previous work has focused only on specific weather variables and algorithms, and there is still a need for a model that uses more variables and algorithms that have higher performance. In this study, we use vector indices and meteorological data as predictors to develop the ML models. We trained and validated seven ML algorithms, including an ensemble ML method, and compared their performance using the receiver operating characteristic (ROC) with the area under the curve (AUC), accuracy and F1 score. Our results show that an ensemble ML such as XG Boost, AdaBoost and Random Forest perform better than the logistics regression, Naïve Bayens, decision tree, and support vector machine (SVM), with XGBoost having the highest AUC, accuracy and F1 score. Analysis of the importance of the variables showed that the container index was the least important. By removing this variable, the ML models improved their performance by at least 6% in AUC and F1 score. Our result provides a framework for future studies on the use of predictive models in the development of an early warning system.

    Implementing asthma guideline recommendations is challenging in low- and middle-income countries. We aimed to explore healthcare provider (HCP) perspectives on the provision of recommended care. Twenty-six HCPs from six public primary care clinics in a semi-urban district of Malaysia were purposively sampled based on roles and experience. Focus group discussions were guided by a semi-structured interview guide and analysed thematically. HCPs had access to guidelines and training but highlighted multiple infrastructure-related challenges to implementing recommended care. Diagnosis and review of asthma control were hampered by limited access to spirometry and limited asthma control test (ACT) use, respectively. Treatment decisions were limited by poor availability of inhaled combination therapy (ICS/LABA) and free spacer devices. Imposed Ministry of Health programmes involving other non-communicable diseases were prioritised over asthma. Ministerial policies need practical resources and organisational support if quality improvement programmes are to facilitate better management of asthma in public primary care clinics.

    BACKGROUND: Siwak is a chewing stick used as an oral hygiene aid associated with Muslim communities across the globe since more than 1500 years ago. Used either exclusively or in conjunction with a regular toothbrush, there is evidence supporting its clinical effectiveness in plaque control, but adverse effects on periodontal health remains inconclusive.

    OBJECTIVE: This study aims to systematically review the wide range of data and literatures related to siwak practice and its effect on periodontal health.

    METHOD: The review was conducted based on scoping review techniques, searching literature in EBSCOHOST, PubMed, SCOPUS and Google scholar databases using the following search terms: "siwak' or 'miswak' or 'chewing stick" for intervention, and "periodontium or 'periodontal' or 'periodontal health' or 'periodontal disease" for outcome. Articles published between January 1990 to March 2021 and written in English language were included.

    RESULTS: A total of 721 articles collected from the search and 21 of them were eligible for the final analysis. Results of this study was described based on clinical and antibacterial reporting of siwak, method of siwak practice and its adverse effect on oral health. Siwak was found effective at removing dental plaque and improving periodontal health over time although its effect on subgingival microbiota was inconclusive. Presence of gingival recession and clinical attachment loss were much more commonly reported in siwak users, attributable to variations in the methods employed for tooth cleaning using the siwak.

    CONCLUSION: There is substantial evidence that the lack of standardised reporting for effective siwak use may have resulted in contradictory findings about its oral hygiene benefits and adverse effects. As such, future work on safe and effective siwak practice is to be advocated among its users.

    Malaysia encounters a consistent rise in the generation of solid waste and leachate on a daily basis. It should also be noted that leachate has a low degree of biodegradability (BOD5 /chemical oxygen demand [COD]), as shown by its BOD5 /COD ratio. Its high toxicity levels significantly threaten the environment, water bodies, and human well-being. High concentrations of COD, color, and ammoniacal nitrogen (NH3 -N) in leachate prevent this wastewater from being allowed to be discharged directly into the water body. Therefore, an effective process to remove the pollutant is desired. The aims of this study are to investigate the performance of ozonation with two metallic compounds, ZrCl4 and SnCl4 , and optimize their performance using response surface methodology (RSM). In this study, the performance of ozonation with ZrCl4 (O3 /ZrCl4 ) recorded better pollutant removals compared with the ozonation with tin tetrachloride (O3 /SnCl4 ), as seen in the removals of 99.8%, 93.5%, and 46.3% for color, COD, and NH3 -N, respectively. These removals were achieved by following the experimental model (optimum experiment condition) generated by RSM at O3 dosage of 31 g/m3 , COD and ZrCl4 dosage ratio (COD, mg/L/ZrCl4 , mg/L) of 1:1.35, with the pH solution of 8.78 and reaction time of 89 min. The R2 of each parameter for this model was recorded as 0.999 (COD), 0.999 (color), and 0.998 (NH3 -N), respectively. These data indicated that the model is well fitted as the predicted data by statistical calculation and in good agreement with the actual data. PRACTITIONER POINTS: The performance of O3 /ZrCl4 and O3 /SnCl4 was examined for remediate stabilized landfill leachate. The performance of O3 /ZrCl4 and O3 /SnCl4 was optimized using RSM, and a set of experimental models was generated and tested. O3 /ZrCl4 recorded the higher removal of COD, color, and NH3 -N compared with O3 /SnCl4 . At best condition, both methods recorded removal as 89% to 99.8% of pollutants in leachate and product clear effluent. This finding gives a new approach to treat landfill leachate effectively and efficiently.

    The exponential growth of human population and anthropogenic activities have led to the increase of global surface water contamination especially in river, lakes and ocean. Safe and clean surface water sources are crucial to human health and well-being, aquatic ecosystem, environment and economy. Thus, water monitoring is vital to ensure minimal and controllable contamination in the water sources. The conventional surface water monitoring method involves collecting samples on site and then testing them in the laboratory, which is time-consuming and not able to provide real-time water quality data. In addition, it involves many manpower and resources, costly and lack of integration. These make surface water quality monitoring more challenging. The incorporation of Internet of Things (IoT) and smart technology has contributed to the improvement of monitoring system. There are different approaches in the development and implementation of online surface water quality monitoring system to provide real-time data collection with lower operating cost. This paper reviews the sensors and system developed for the online surface water quality monitoring system in the previous studies. The calibration and validation of the sensors, and challenges in the design and development of online surface water quality monitoring system are also discussed.

    The circadian rhythm (CR) is a fundamental biological process regulated by the Earth's rotation and solar cycles. It plays a critical role in various bodily functions, and its dysregulation can have systemic effects. These effects impact metabolism, redox homeostasis, cell cycle regulation, gut microbiota, cognition, and immune response. Immune mediators, cycle proteins, and hormones exhibit circadian oscillations, supporting optimal immune function and defence against pathogens. Sleep deprivation and disruptions challenge the regulatory mechanisms, making immune responses vulnerable. Altered CR pathways have been implicated in diseases such as diabetes, neurological conditions, and systemic autoimmune diseases (SADs). SADs involve abnormal immune responses to self-antigens, with genetic and environmental factors disrupting self-tolerance and contributing to conditions like Systemic Lupus Erythematosus, Rheumatoid Arthritis, and Inflammatory Myositis. Dysregulated CR may lead to increased production of pro-inflammatory cytokines, contributing to the systemic responses observed in SADs. Sleep disturbances significantly impact the quality of life of patients with SADs; however, they are often overlooked. The relationship between sleep and autoimmune conditions, whether causal or consequential to CR dysregulation, remains unclear. Chrono-immunology investigates the role of CR in immunity, offering potential for targeted therapies in autoimmune conditions. This paper provides an overview of the connections between sleep and autoimmune conditions, highlighting the importance of recognizing sleep disturbances in SADs and the need for further research into the complex relationship between the CR and autoimmune diseases.

    Hikikomori is the Japanese term for the phenomenon of young adults isolating themselves from society and becoming reclusive. This study aims to examine the evolution and distribution of hikikomori, as well as identify the major players and institutions involved in hikikomori research. The searches, which were conducted in Scopus database identified 286 academic research works pertaining to hikikomori research. The findings indicate that hikikomori has attracted the attention of academic researchers in both the East and the West, with a consistent increase in research output since the Internet revolution over the past decade. The study highlights the potential for additional research on hikikomori and provides a benchmark for new researchers entering the field.

    The oral mucosa can be involved in a wide variety of mucocutaneous conditions that may present primarily in the mouth or affect other cutaneous or mucosal sites. Many of these conditions are immune mediated and typically present as inflammatory mucosal pathology. Patients experiencing such conditions usually seek medical evaluation and treatment due to the associated pain and discomfort and occasionally taste disturbance or dysphagia and the overall deterioration in the oral health-related quality of life. These conditions share some common features and there could be some overlapping in their clinical presentation, which can lead to delays in diagnosis and proper management of patients. Clinicians dealing with such disorders, including dermatologists, need to be aware of the oral manifestations of mucocutaneous conditions, their clinical features, underlying mechanisms, diagnostic approaches, and treatment options, as well as the recent advances in the research on these conditions. This review provides a comprehensive, evidence-based reference for clinicians, with updated insights into a group of immune mediated conditions known to cause oral mucosal pathology. Part one will cover oral lichen planus, erythema multiforme and systemic lupus erythematosus, while part two will cover pemphigus vulgaris and mucous membrane pemphigoid, recurrent aphthous stomatitis, in addition to the less common disorders linear IgA disease, dermatitis herpetiformis and epidermolysis bullosa.
