    Background and objective: A successful family physician program needs ongoing and full cooperation between people and the organizations in charge. Ensuring the satisfaction of family physicians through improvement of the underlying factors could motivate them to provide high-quality services. This study aimed to determine the family physicians' satisfaction level with the factors affecting the dynamism of the urban family physicians program in the Fars and Mazandaran provinces of Iran.

    Method: This cross-sectional study was carried out in urban areas in the Fars and Mazandaran provinces in 2016. The sample consisted of 143 and 96 family physicians, respectively, in Fars and Mazandaran provinces and was selected using the stratified random sampling method. Data were collected using a questionnaire and included both sociodemographic variables and factors assessing the family physicians' satisfaction levels. Each factor was scored based on a Likert scale from 0 to 5 points, and any satisfaction level higher than 3 out of 5 was equated with being satisfied.

    Results: The overall satisfaction levels among family physicians in Fars and Mazandaran provinces were 2.77±0.53 and 3.37±0.56, respectively, revealing a statistically significant difference between provinces (p<0.001). Moreover, the mean satisfaction scores for the performances of healthcare centers, insurance companies, specialists, healthcare workers, and the population covered were 2.78±0.1, 2.54±0.9, 2.52±0.8, 4.24±0.07, and 2.96±0.8, respectively. The family physicians' levels of satisfaction were significantly correlated with population size (p=0.02, r= -0.106), and willingness to stay in an urban family physician program (p<0.001, r= +0.398).

    Conclusion: This study revealed that family physicians exhibited a low level of satisfaction with the urban family physician program. Given the direct association between family physicians' satisfaction levels and retention in the program, it is expected that family physicians will no longer stay in the program, and it is likely to have subsequent executive problems.

    A cross·sectional study was done to determine the capacity of Public Health Inspectors in Selangor using a seh administered questionnaire and universal sampling, to determine the enforcement of public health legislation and its influencing factors. A total of 99 respondents from the District Health Offices and 100 respondents from the Local Health Departments participated. The level of enforcement is generally low in all the units except for the Food Unit and Sanitary and Hygiene Units. Factors influencing enforcement of Public Health Legislations are the unit where the respondents worlc and the length of service with the unit. Further analysis using multiple logistic regression, found that respondents from the Food Unit (OR ZZ; 95% C1: 7 .9-58.9 ) , being in the
    U8 category (OR 5; 95% C1: l.4» 17.6), service offour years and above in the same unit (OR 6; 95% C1: 2.2»13.8) , received service rewards (OR 3; 95% C1 : 1.2· 6.9); low exposure to bribery (OR 46; 95% C1: 2.3·100.0) and offered bribes while on the field (OR 3; 95% C1: 1.02» 4.8) have higher kvel enforcement performance. Attention must be given to the enforcement organization with respect to duration of service, service awards and monitoring of bribes exposure to help develop the enforcement of public health legislations.

    Work-related upper-extremity disorders (WRUEDs) are frequently occurring among workers in general. Various researches support the empirical results of the Autogenic factors related to such problems of health and safety. Research shows the association of different aspects of work organization strengthen the possibilities of work-related stress and other psychosocial factors, which may lead towards adverse health conditions. To review the previous work done for finding the association between psychosocial factors and upper extremity disorders. Findings from different studies have been reviewed to examine the psychosocial factors association in developing WRUEDs. Published studies, which were primarily focused on WRUEDs and psychosocial factors association, were reviewed. Various studies have shown the relationship between psychosocial factors and UEDs. Due to the different nature of epidemiology of the disorder and variable differences, findings from different studies contradicts. Finally, this review presents the limitations in existing studies, and further research has been proposed for future.

    Introduction: Family Environment Scale (FES) was developed by Moos1, has ten subscales and was categorized into three dimensions.Objective:The objective of this study was to develop factor structure for the Malay-translated version of the FES subscales. Methodology: The study used Malay translated version of FES by Khairani et. al2. This study was a multi centre, cross-sectional study, involving four secondary schools consisted of adolescents, aged between 12-17 years old and a total of 295 participants were enrolled in this study. Exploratory factor analyses was done across two groups of analysis set on the subscales with Cronbach’s alpha more than 0.50 and 0.53 respectively. Results: Two distinct factors were extracted across the four subscales consisted of Cohesion, Conflict and Organization in factor 1 and only Control in factor 2. Conclusion: The finding indicated that element of Cohesion, Conflict and Organization has a good indicator of a good family relationship.

    Introduction: Recently mobile communication technology had tremendous advances and became the priority for most of the users with some opponent opinions that highlight the accountability and the privacy issues in using these messaging applications. WhatsApp is scored to be the first top used messaging application worldwide that is used in 180 countries by about 1.5 billion population. This research aims to determine the staff reflections regarding the WhatsApp usage for organizational communication. Methods: This study is implied among the staff at the medical and health life science cluster, Management and Science University (MSU), Malaysia. A questionnaire was distributed to all the staff through an online google form. Responses were analysed by the ‘Statistical Package for Social Sciences’ (SPSS) Version 24.0. Results: Results showed that all the staff responded (100%) are using WhatsApp for organizational communication with variable degrees of satisfaction; 8.1% are extremely satisfied, 61.3 % are satisfied, 12.9% are not satisfied and 16.1 % are extremely not satisfied. Their level of satisfaction is significantly affected by the respondent’s age, benefit of usage, and the time of messaging. Conclusion: To ensure effectiveness of communication through WhatsApp, it should be through official groups and the organization should put specific announced rules for the time of usage and the content of messages to be posted to all members.

    Infrastructure damage due to land slide, fallen bridge and broken and submerged roads become the main constraint in providing good medical services to the flood victims and isolated places in the remote area. The health care provider has to face a huge challenge at delivering the medical services to the flood victims in Kluang district especially to the remote and isolated areas. This gives us a meaningful and valuable experience in managing such problem. From the true experience of the medical and health team and also the flood victims, few problems and major issues were detected. Other than the environmental factor, human error is another major area of concern of which the failure to interact with the District Flood Operation Centre leading to miscommunication resulting in delay of management of the patient. In smaller proportion, poor inter-agency collaboration and lacking of good equipment was also noted to be affecting the health care services. The issues raised here will hopefully be making better in managing disaster in the future.

    Each organization has a particular culture, due to personal interactions, with certain values shared by its members. Corporate culture is defined as ‘the set of shared, taken-for-granted implicit assumptions that a group holds and that determines how it perceives, thinks about, and reacts to its various environments’. Positive corporate culture is linked to increased staff alignment, advanced level of employee commitment, increased employee productivity, enhanced organizational effectiveness and increased profitability. Researchers claimed that most studies suggesting the associations between culture and performance are methodologically weak. Cultural transformation has been a big part of NHS reforms and health system redesign in United States to deliver improvements in quality and performance. Environment, market competition, technology advancement, information age and government policies will influence the cultural change within the organization. Undesirable culture might emerge if the organization does not act appropriately to manage its corporate culture. There are six critical success factors for the implementation of corporate culture changes: committed and effective leadership, clear definition of the desired goals, rigorous implementation of a change management model, effective mitigation of change resistance, active governance structure and a design model, and effective communication. Among the issues in implementing corporate culture within healthcare organizations are middle managers dilemma, cultural diversity and subcultures within the organization, size of healthcare organization and critical mass.

    Workplace bullying has drawn greater attention in the last one and half decades. Despite its recognition by many organizations and countries, it is still rife. Why is that so? Could it be that the root of the problem has not been addressed? Or, could it be due to difficulties and resistances in embarking preventive and control measures. In this paper, we will examine the possible causes of workplace bullying based on a proposed model. In depth discussion of the personal and organizational factors are made while the work group and societal factors are dealt with in brief. In summary, the root of workplace bullying is multi-factorial. Understanding the complexity and subtlety of workplace bullying is pertinent in the effort to prevent or curtail it.

    Introduction : Quality is an important aspect in health care delivery not only because it is one of the most important factors in individual and community health, but it also influences lives in improving lifespan, health status and also reduction in disease and the burden of diseases.
    Methodology : This is a systematic review on various papers, studies and articles based on studies and researches done by a few scholars, experts and organizations involved in quality, quality assessment and monitoring.
    Results : Quality in health care delivery involves six main aspects including access to services, suitability to the needs, effectiveness, equity, social acceptance and efficiency. Quality is taken from the perspective of the consumers and the providers. Some of the issues surrounding quality assessment are identifying and balancing between the difference perspective among the stakeholders, producing accountable framework of assessment and also finding suitable criteria for outcome assessment. Health care services in Malaysia are provided both by the government and the private sector with each sector has its own advantages, disadvantages and challenges.
    Conclusion : The challenges in improving and maintaining quality include balancing advances in science and technology with the available brainpower and human resources. Therefore all agencies should work together in order to provide the best health care delivery and to keep improving the quality in health care services.

    Background : The implementation of Occupational Safety and Health Management System (OSHMS) requires a level of measurement. The effectiveness of OSHMS implementation was influenced by inhibiting and supporting factors. The objective of this study was to determine the inhibiting and supporting factors towards the implementation of OSHMS in medical laboratories.
    Methodology : The implementation of Occupational Safety and Health Management System (OSHMS) requires a level of measurement. The effectiveness of OSHMS implementation was influenced by inhibiting and supporting factors. The objective of this study was to determine the inhibiting and supporting factors towards the implementation of OSHMS in medical laboratories.
    Result : This study showed the level of compliance to the OSHMS MS1722 guideline among medical laboratories who are MS ISO 15189 accredited & have a higher scores in overall measurement indicators OSHMS compared to those do not have SAMM accreditation. Private medical laboratories services have higher scores in overall measurement indicators OSHMS, compared to government medical laboratories. However, there was no significant difference (p>0.05) between medical laboratories which have ISO 9000 quality management system certification, compared to level of compliance to the OSHMS MS 1722 guideline among medical laboratories. There was also no significant difference (p>0.05) between the laboratories who had been operating for more than 10 years compared to those with less than 10 years, towards implementing the OSHMS. Results showed that majority of medical laboratories management perceived that difficulty and complexities of OSH legislation are the most important factor that prevent them from implementing OSHMS in their organization. The most important supporting factor, that medical laboratories management perceived as the most important to comply to the OSHMS were their moral responsibilities towards the community.
    Conclusion : In conclusion, the study shows there were many inhibiting and supporting factors that contribute towards effective implementation of OSHMS elements in workplace.

    Introduction : It is no doubt that the success of any health organization depends so closely on its managerial functions. To achieve this, the leader or manager as the core strategist of its organization must in all time be updated with the latest evidence-based information so that he or she can be easily operationalized his or her management function in more effective and sustainable manners. It depends largely on scientific literatures that published relevant articles within this scope. Unfortunately, management topics related to health care system is scattered published and this has indirectly affect manager to access the latest scientific documents.
    Methods : We examined the practice of well known international journal in health care namely New England Medical Journal (NEMJ) on its role in propagating latest health management topics to its prospective clients.
    Results : The result showed that a total of 31% health management topics were published throughout 2007 out of 1140 articles appeared. Of these, about 33% were confined to general health administration. The remaining articles were related to healthcare delivery practices (24%), medical ethics and legal matters (17% each), and manpower issues and training (9%).
    Conclusion : Focus on managerial related articles relatively low as compared to clinical and other evidence-based medicine that clearly dominated health management issues.

    Despite increasing evidence about the value and importance of breastfeeding, less than half of the world's infants and young children (aged 0-36 months) are breastfed as recommended. This Series paper examines the social, political, and economic reasons for this problem. First, this paper highlights the power of the commercial milk formula (CMF) industry to commodify the feeding of infants and young children; influence policy at both national and international levels in ways that grow and sustain CMF markets; and externalise the social, environmental, and economic costs of CMF. Second, this paper examines how breastfeeding is undermined by economic policies and systems that ignore the value of care work by women, including breastfeeding, and by the inadequacy of maternity rights protection across the world, especially for poorer women. Third, this paper presents three reasons why health systems often do not provide adequate breastfeeding protection, promotion, and support. These reasons are the gendered and biomedical power systems that deny women-centred and culturally appropriate care; the economic and ideological factors that accept, and even encourage, commercial influence and conflicts of interest; and the fiscal and economic policies that leave governments with insufficient funds to adequately protect, promote, and support breastfeeding. We outline six sets of wide-ranging social, political, and economic reforms required to overcome these deeply embedded commercial and structural barriers to breastfeeding.

    The e-PA system was developed to establish a monitoring and reporting process for academic adviser(PA) and student (advisee) to be used in the UiTM Kedah Academic Affairs Division (BHEA). The e-PA system is an online system that enhances the process of monitoring and reporting of PA-advisee activities. The e-PA system utilized the Design Thinking approach which is an approach in empowering innovation that was identified to value functionality. Therefore, the e-PA system are categorized in 3 phases which were: Hear (H) which was Emphatize-activity aiming to visualize and analyze users’ needs; Create (C)-this phase encompassed Define-and-Ideate-activities aiming to find inventions that meets the value of functionality; and Deliver (D) which covers the Prototype-and-Test-activities where the e-PA system was developed through the provision of prototypes and had been through 3 trials and finally tested with four target groups. The results of the e-PA system development had achieved significant reductions in terms of cost and time. In addition, the objectives of 25 percent innovation creation indication had also been achieved through the aspects of organizational function, customer needs, stakeholders, and current conditions and eco systems of the campus. The e-PA system could be a platform of communication to facilitate monitoring and reporting of PA-advisee activities for all stakeholders at UiTM Kedah Branch. The e-PA system can enhance PA's roles by helping BHEA to empower PA-advisee relationships, increase customer satisfaction, maximize students’ success in accordance with the aspirations of the Higher Education Ministry, and assist the campus to achieve a "green-university" status. This three-phase-interaction system is a precedent system in the monitoring and reporting of PA-advisors at UiTM Kedah Branch

    In Malaysia, there are 81 (as on February 15, 2013) higher education institutions including satellite branches of the foreign universities. In northern part of the Peninsular Malaysia, AIMST University is the first private not-for-profit university and aims to become a premier private university in the country and the region. The workshop described in this article was designed to develop and enhance the capacity of academic staff-in-leadership-role for the University. This type of workshops may be a good method to enhance the leadership qualities of the head of each unit, department, school and faculty in each university.

    The United States of America (USA) is one of the largest bilateral donors in the field of global health assistance. There are beneficiaries in 70 countries around the world. In 2015, the USA released US$638 million for the improvement of global health status by promoting family planning services. Unfortunately, in 2017, Trump administration reinstated Mexico City Policy/Global Gag Rule (GGR). This policy prevents non-US nongovernmental organizations (NGOs) from receiving US health financial assistance if they have any relationship with abortion-related services. This restriction pushed millions of lives into great danger due to the lack of comprehensive family planning services, especially lack of abortion-related services. This article has attempted to let the readers know about the impacts of GGR around the world and how global leaders are trying to overcome the harmful effects of this rule. Finally, it proposes some solutions to the impacts of the extension of Mexico City Policy.

    Improving access to medicines is a major healthcare challenge for low-income countries because the problem traverses health systems, society and multiple stakeholders. The Annapurna region of Nepal provides a valuable case study to investigate the interplay between medicines, society and health systems and their effects on access to and use of medicines. Government health facilities and international aid organizations provide some healthcare in the region, communities participate actively in healthcare organization and delivery, there is an important tourism sector and a mostly rural society. This study investigates access to and use of medicines through health facility and household-based studies using standardised tools and through a series of structured key informant interviews with various stakeholders in health. Overall, access to essential medicines at public health facilities was good, but this was not benefitting households as much as it should. People were using the private sector for medicines because of their perception about the quality and limited numbers of government-supplied free medicines. They utilised money from remittances and tourism, and subsidised healthcare from non-government organizations (NGOs) to access healthcare and medicines. A pluralistic healthcare system existed in the villages. Inappropriate use of medicines was found in households and was linked to the inadequate health system, socioeconomic and sociocultural practices and beliefs. Nevertheless, the often disadvantaged Dalit users said that they did not face any discrimination in access to health services and medicines. The government as the main stakeholder of health was unable to meet people's health services and medicines needs; however, health aid agencies and the local community supported these needs to some extent. This study shows that the interconnectedness between medicines, society and health systems impacts the way people access and use medicines. Improving access to medicines requires an improvement in public's perception about quality, actual coverage and appropriate use of medicines and health services via collaborative contributions of all stakeholders.

    Mental ill health affects individual well-being and national economic prosperity and makes up a substantial portion of the burden of disease globally, especially in the Asia-Pacific region. Integrating mental health into primary care is widely considered a key strategy to improve access to mental health care. Integration, however, is a complex process that needs to be addressed at multiple levels. A collaboration between the Asia-Pacific Economic Cooperation (APEC) Digital Hub for Mental Health and the World Organization of Family Doctors (WONCA) is described in this paper, which outlines a framework and next steps to improve the mental health of communities in APEC economies. This paper notes gaps related to the integration of mental health into primary care across the region and identifies enablers and current best practices from several APEC economies. The potential of digital technology to benefit primary mental health care for populations in the APEC region, including delivery of training programs for healthcare staff and access to resources for patients, is described. Finally, key next steps are proposed to promote enhanced integration into primary care and improve mental health care throughout the APEC region.

    AIMS: To examine the mediating role of employee well-being on the relationship between work-life balance practices, the need for achievement and intention to leave among nurses in Malaysia.

    BACKGROUND: Work-life balance practices are associated with employee perceptions of the need for achievement and well-being which subsequently influence their intention to leave the organization. This study contributes new knowledge to nursing studies on work-life balance in an Asian and Islamic society where the expectations for women are to focus on family rather than career.

    DESIGN: A cross-sectional, explanatory mixed methodology.

    METHODS: This is a two-phase study conducted between 2015-2017 with 401 nurses in East Malaysia. In Phase 1, researchers surveyed 379 nurses to test eight hypotheses and in Phase 2 researchers interviewed 22 nurses to explore the results of Phase 1.

    RESULTS: Phase 1 revealed job satisfaction mediates the relationship between work-life balance practices (e.g. flexibility and choice in working hours, supportive supervision), financial success, and intention to leave. However, life satisfaction and money as a motivator did not mediate such relationships. Phase 2 identified four important factors that cast light on survey results: working conditions of Malaysian nurses; inadequate compensation in the public healthcare sector; team-based practices; and pressure on senior nurses in both administrative and clinical roles.

    CONCLUSION: This is one of the first studies to investigate work-life balance issues among nurses in Malaysia. Outcomes of this study extend the debates on work-life balance and employee well-being in an Asian Islamic social context.

    IMPACT: The use of flexible working arrangements and collectivist teamwork approaches, improving compensation and employment benefits and eliminating the 'time-based job promotion' policy may help to mitigate work-life balance issues and intention to leave among nurses in Malaysia.

    There is a need to develop an Occupational Safety and Health Management System (OSHMS) Critical Success Factors (CSF) Monitoring among gas contractor in Malaysia. Critical Success Factors (CSF) can be used for future implementation, adaptation and practice for gas contractor work for both by clients and contractors in Malaysia. The main purpose of CSF implementation is to reduce the number of accident related with Occupational Safety and Health (OSH). The need for CSF is due to the requirement by law and standard that require companies to establish an arrangement related to the identified OSH risks. The objective of this study is to assess the established of OSHMS among gas contractor in Peninsular Malaysia and to identify the CSF imposed by these industries. A cross sectional study for eighty gas contractor companies using established questionnaire has been done. All data consolidated in order to determine the OSHMS and it CSF among gas contractor in Peninsular Malaysia that has registered with Department of Occupational Safety and Health. Established questionnaire are based on OSHMS MS 1722:2011 elements requirement No 1: Policy, No 2: Organizing, No 3: Planning and Implementation, No 4: Evaluation and No 5: Action for Improvement as parameters to come out with organization means and Z-scores. Descriptive statistic showed that element mean (standard deviation) score for policy is 75(6.6), for Organizing is 63(5.2), for Planning and Implementation is 59(9.4), for Evaluation is 66(5.1) and Action for Improvement is 63(1.3). Percentage of company that complies with the main element for Policy is 15%, for Organizing is 8.8%, for Planning and Implementation is 11.3%, for evaluation is 11.3% and for Action for improvement is 13.8%. Percentage of companies that partially comply with Policy element is distributed between 61.0% to 85.0% which is 76.3 %, for Organizing score is mainly distributed between 46.0% to 60.0% which is 62.5%, for planning and Implementation score for 80 companies distributed mainly in group score between 46.0 to 60.0% which is 56.3 %, for Evaluation the partially comply score is distributed in group score between 61.0% to 85.5% which is 41 % and for Action for Improvement partially comply score, the distribution is mainly distribute in range of 61.0% to 85.0% which is 62.5 %. Z-score for element policy is five points from policy mean, for organizing Z-score is three points from Organizing mean, for Planning and Implementation the Z-score is three points from Planning and Implementation mean, for Evaluation the Z-score is three points from evaluation mean and for Action for improvement Z-score is two to three points from Action for Improvement mean. Percentage of compliance with OSHMS MS 1722:2011 elements by gas contractors in almost main element and sub element are still low and can be further improved by focusing on all company element score for continual improvement of OSH elements compliances.

    Regulation on occupational safety and health in Malaysia had evolved from the prescriptive Factory and Machinery Act (1967) to a self-regulated Occupational Safety and Health Act (1994). However, from the authors’ observation the high standards of occupational safety and health culture that surpass the legal requirement were not widely practiced by small and medium enterprises (SMEs). The two main objectives of this study are: first, to identify and determine the level of conformity; and second, to investigate the reasons of nonconformity to Occupational Safety and Health Act (1994) regulation in SMEs involved the chemical industry sub-sectors. The survey questionnaire was distributed to 150 SMEs in chemical industry sub-sectors. Forty one of the survey questionnaires were completed and returned, giving a response rate of 27.3% for the survey. The survey results revealed that an overwhelming majority (92.7%) of the respondents from SMEs are likely not conforming to the basic requirement of Occupational Safety and Health Act (1994). In addition to this, the survey also found that only 3.1% of the management personnel can be considered competent in terms of knowledge, skill and ability in carrying out occupational safety and health regulation within their respective organization. While, 96.9% of the respondents that participated in the survey can be considered not competent. The authors hope the result of this survey could assist the relevant authorities in formulating a better policy and strategy for implementing occupational safety and health in SMEs involved in chemical industry sub-sectors.
