    The objective of this study was to assess the feasibility of the Sequencing Batch Reactor (SBR) system for implementation in Malaysia. Theoretical, field, laboratory investigations, and modelling simulations have been carried out. The results of the study indicated that the SBR system was robust, relatively cost-effective, and efficient under Malaysian conditions. However, the SBR system requires highly skilled operators and continuous monitoring. This paper also attempted to identify operating conditions for the SBR system, which optimise both the removal efficiencies and the removal rates. The removal efficiencies could reach 90-96% for COD, up to 92% for TN, and 95% for SS. An approach to estimate a full operational cycle time, to estimate the de-sludging rate, and to control the biomass in the sludge has also been developed. About 4 hours react time was obtained, as 2.25 hours of nitrification with aerated slow fill and 1.75 hour of denitrification with HAc addition as an additional carbon source. Inefficient settling was one of the problems that affect the SBR effluent quality. The settling time was one hour for achieving Standard B (effluent quality) and 2 hours for Standard A.

    Streptococcus zooepidemicus (SZ) is an aerotolerant bacteria and its ability to survive under reactive oxidant challenge raises the question of the existence of a defense system. Thus growth, hyaluronic acid (HA) and hydrogen peroxide (H2O2) production by SZ in the presence of increasing concentration of Mn2+ were studied. The results suggested that the tested strain supported growth and HA production in cultures treated with 1 and 10 mM of Mn2+ regardless of H2O2 presence in the medium. This showed that SZ have acquired elaborate defense mechanisms to scavenge oxygen toxicity and thus protect cells from direct and indirect effect of this radical. In contrast, cells treated with 25 mM Mn2+ were sensitive, in which, the HA production was reduced considerably. Thus showing that the oxygen scavenger systems of the cells may be fully saturated at this concentration.

    Children with bronchiolitis from Hospitals Universiti Sains Malaysia (HUSM) and Hospital Kota Bharu (HKB) were student using the Kristjansson Respiratory Score and the Wang Respiratory Score respectively. Saturation of oxygen (S(a)O(2)) was measured with a pulse-oximeter while the child is breathing room-air. Two observers assessed the respiratory scores in all children independently. The correlation between respiratory scores and S(a)O(2) was assessed using Spearman's Rho, and the inter-rater reliability of respiratory scores determined using intraclasscorelation coefficient. There were 29 children in HUSM and 25 from HKB with a median age of 8 months (IQR 4.5 months) and 9 months(IQR 7 months) respectively. In HUSM, the median Kristjansson Respiratory Score for both observers was 4 (IQR 2), and the median S(a)O(2) was 96% (IQR 3%). The correlation coefficient between the Kristjansson Respiratory Score and S(a)O(2) for the first observer was - 0.75 (p <0.001), and for the second observer -0.73 (p <0.001). In HKB, the median Wang Respiratory Score was also similar for both observers (median 4 IQR 4.5), and the median (IQR) for S(a)O(2) was 96% (2%). The correlation coefficient between the Wang Respiratory Score and S(a)O(2) for the first observer was -0.41 (p = 0.04) and for the second observer -0.43 (p = 0.03). The inter-rater reliability between the first and second observer was high for both the Kristjansson Respiratory (ICC 0.89) and the Wang Respiratory Scores (ICC 0.99). In conclusion the validity of the Kristjansson Respiratory Score was high whereas the validity of the Wang Respiratory Score was moderate in the assessment of the severity of acute bronchiolitis. Both respiratory scores and physical signs showed high agreement between observers. The Kristjansson Respiratory Score should be considered for use by medical personnel in the assessment of the severity of acute bronchiolitis in children.

    Prediction of mortality in patients with community-acquired pneumonia (CAP) can be assessed using clinical severity scores on admission to hospital. The clinical benefit of such tools is untested in Asian countries. The aim of this study was to determine the early adverse prognostic factors in patients hospitalized with CAP in Malaysia and to assess the usefulness of the British Thoracic Society (BTS) severity criteria.

    The effects of sulfide on the energy metabolism of Boleophthalmus boddaerti in normoxia and hypoxia were examined. The 24-, 48-, and 96-h LC50 values of sulfide for B. boddaerti with body weight ranging from 11.6 to 14.2 g were 0.786, 0.567, and 0.467 mM, respectively. The tolerance of B. boddaerti to sulfide was not due to the presence of a sulfide-insensitive cytochrome c oxidase. There was no accumulation of lactate in the muscle and liver of specimens exposed to sulfide in normoxia. In addition, the levels of ATP, AMP, and energy charge in both the muscle and the liver were unaffected. These results indicate that B. boddaerti was able to sustain the energy supply required for its metabolic needs via mainly aerobic respiration when exposed to sulfide (up to 0.4 mM) in normoxia. Exposure of B. boddaerti simultaneously to hypoxia and 0.2 mM sulfide for 48 h resulted in decreases in the ATP levels in the muscle and liver. However, the energy charge in both tissues remained unchanged, and the level of lactate accumulated in the muscle was too low to have any major contribution to the energy budget of the fish. Our results reveal that B. boddaerti possesses inducible mechanisms to detoxify sulfide in an ample supply or a lack of O2. In normoxia, it detoxified sulfide to sulfate, sulfite, and thiosulfate. There were significant increases in the activities of sulfide oxidase in the muscle and liver of specimens exposed to sulfide, with that in the liver being >13-fold higher than that in the muscle. However, in hypoxia, sulfide oxidase activity in the liver was suppressed in response to environmental sulfide. In such conditions, there were significant increases in the activities of sulfane sulfur-forming enzyme(s) in the muscle and liver that were not observed in specimens exposed to sulfide in normoxia. Correspondingly, there were no changes in the levels of sulfate or sulfite in the muscle or liver. Instead, B. boddaerti detoxified sulfide mainly to sulfane sulfur in hypoxia. In conclusion, B. boddaerti was able to activate different mechanisms to detoxify sulfide, producing different types of detoxification products in normoxia and hypoxia.

    A laboratory-scale study was undertaken to evaluate the liquid state bioconversion (LSB) in terms of biodegradation of microbially treated domestic wastewater sludge (biosolids) as well as its kinetics. The potential fungal strains and process factors developed from previous studies were used throughout the study. The results presented in this study showed that an effective biodegradation occurred with the biosolids (sludge cake) accumulated. The maximum biosolids (sludge cake) accumulated (93.8 g/kg of liquid sludge) enriched with the biomass protein (30.2 g/kg of dry biosolids), was achieved which improved the effluent quality by enhancing the removal of chemical oxygen demand (COD), reducing sugar (RS), soluble protein (SP), total dissolved solids (TDS), and total suspended solids (TSS). The higher reduction of specific resistance to filtration (SRF) was observed during bioconversion process. The kinetics results showed that the experimental data were better fitted for the biodegradation efficiency, and biosolids accumulation and biodegradation rate.

    The sorption of Cu(II) and Cd(II) from synthetic solution by powdered activated carbon (PAC), biomass, rice husk (RH) and activated rice husk (ARH) were investigate under batch conditions. After activated by concentrated nitric acid for 15 hours at 60-65 degrees C, the adsorption capacity for RH was increased. The adsorbents arranged in the increasing order of adsorption capacities to the Langmuir Q degree parameter were biomass > PAC > ARH > RH. The addition of adsorbents in base mix solution had increased the specific oxygen uptake rate (SOUR) activated sludge microorganisms with and without the presence of metals. The increased of SOUR were due to the ability of PAC and RH in reducing the inhibitory effect of metals on microorganisms and provide a reaction site between activated sludge microorganisms and substrates.

    The role of bioregeneration process in renewing the adsorbent surface for further adsorption of organics during simultaneous adsorption and biodegradation processes has been well recognized. The extent of bioregeneration of powdered activated carbon (PAC) as an adsorbent loaded with phenol, p-methylphenol, p-ethylphenol and p-isopropylphenol, respectively, in the simultaneous adsorption and biodegradation processes were quantitatively determined using oxygen uptake as a measure of substrate consumption. Bioregeneration phenomenon was also evaluated in the simultaneous adsorption and biodegradation processes under sequencing batch reactor (SBR) operation to treat synthetic wastewater containing 1200 mg l(-1) phenol and p-methylphenol, respectively. The SBR systems were operated with FILL, REACT, SETTLE, DRAW and IDLE periods in the ratio of 4:6:1:0.75:0.25 for a cycle time of 12 h. The results show that the percentage of desorption from loaded PAC decreased in the order phenol>p-methylphenol>p-ethylphenol>p-isopropylphenol. For the treatment of phenol and p-methylphenol in the SBR reactors, respectively, the simultaneous adsorption and biodegradation processes were able to produce a consistent effluent quality of COD < or = 100 mg l(-1) when the applied PAC dosage was 0.115 and 0.143 g PAC per cycle, respectively. When no further PAC was added, the treatment performance deteriorated to that of the case without PAC addition after 68 and 48 cycles of SBR operation, respectively, for phenol and p-methylphenol. This observation is consistent with the greater extent of bioregeneration for phenol-loaded PAC as compared to p-methylphenol-loaded PAC.

    On commercial pig production farms in South East (SE) Asia, the liquid effluent is often discharged into rivers. The discharge is a hazard to the environment and to the health of people using water from the river either for consumption or for irrigation. Therefore, a simple percolation biofilter for treatment of the liquid effluent was developed. Pig slurry was treated in test-biofilters packed with different biomass for the purpose of selecting the most efficient material, thereafter the efficiency of the biofilter was examined at farm scale with demo biofilters using the most efficient material. The effect of using "Effective Microorganisms" (EM) added to slurry that was treated with biofilter material mixed with Glenor KR+ was examined. Slurry treatment in the test-biofilters indicated that rice straw was better than coconut husks, wood shavings, rattan strips and oil palm fronds in reducing BOD. Addition of EM and Glenor KR+ to slurry and biofilter material, respectively, had no effect on the temperature of the biofilter material or on the concentrations of organic and inorganic components of the treated slurry. The BOD of slurry treated in test biofilters is reduced to between 80 and 637 mg O2 I(-1) and in the demo biofilter to between 3094 and 3376 mg O2 l(-1). The concentration of BOD in the effluent is related to the BOD in the slurry being treated and the BOD concentration in slurry treated in test biofilters was lower than BOD of slurry treated in demo biofilters. The demo biofilter can reduce BOD to between 52 an 56% of the original value, and TSS, COD (chemical oxygen demand) and ammonium (NH4+) to 41-55% of the original slurry. The treated effluent could not meet the standards for discharge to rivers. The composted biofilter material has a high content of nitrogen and phosphorus; consequently, the fertilizer value of the compost is high. The investments costs were 123 US dollar per SPP which has to be reduced if this method should be a treatment option in practise.

    OBJECTIVES: Anthropometric and physiological profiles of national sepak takraw players were determined.
    METHODS: Thirty nine players, specialising in the three playing positions (tekong/server, feeder, and killer/spiker) were divided into three age categories of under 15 (U15), under 18 (U18), and under 23 (U23) years of age. Height, weight, percent body fat (%bf), maximum oxygen consumption (Vo(2max)), range of motion (ROM), back and leg strength, and heart rate, for the estimation of oxygen consumption during matches, were recorded. Statistical analysis was performed using one way ANOVA for independent measurements and data are presented as mean+/-standard deviation.
    RESULTS: The U23 players were significantly taller and heavier with significantly better ROM of the neck, trunk, and ankle joints and back and leg strength than the U15 players. No significant difference was found in %bf between the three age categories. Mean maximum heart rate during exercise was significantly higher in the U15 group when compared to the U18 and U23 groups (p<0.05). Mean Vo2max was similar between the three groups. Estimated oxygen consumption during matches was 69.1%, 68.5%, and 56.4% of Vo2max in the killer, tekong, and the feeder groups, respectively.
    CONCLUSIONS: The mean height, body weight, and cardiopulmonary capacities of the players were within the Malaysian population norms, but were somewhat lower than those of players of other court games from other countries. %bf was also lower in these players. This study provides the much needed anthropometric and physiological data of sepak takraw players for further development of this sport.

    Municipal leachate was treated in an experimental unit of constructed wetlands of subsurface flow type. The parameters studied were organics (BOD and COD), solids and heavy metals (Zn, Ni, Cu, Cr and Pb). Using two types of emergent plants of Scirpus globulosus and Eriocaulon sexangulare, more than 80% removal was achieved for all the parameters. E. sexangulare removed organics and heavy metals better than Scirpus globulosus. A higher concentration of heavy metals in the influent did not change the removal efficiency.

    A two-stage anoxic transformation process, involving growth of biomass utilizing two types of different electron acceptors, namely nitrate and nitrite, has been observed. The present water quality modules established for sewer processes cannot account for the two-stage process. This paper outlines the development of a model concept that enables the two-stage anoxic transformation process to be simulated. The proposed model is formulated in a matrix form that is similar to the Activated Sludge Models and Sewer Process Model matrices. The model was successfully applied to simulate changes in nitrate and nitrite concentrations during anoxic transformations in the bulkwater phase of municipal wastewater.

    Cardiopulmonary exercise testing is a non-invasive physiological test which incorporates the conventional method of exercise stress test with a more advanced breath-to-breath ventilatory analysis. The physiological parameters obtained from the test help to illustrate the cardiovascular, respiratory and metabolic responses to physical exertion. Individual's functional capacity and aerobic fitness is reflected by the value of maximal oxygen consumption (VO2 max) obtained from the cardiopulmonary exercise test. This non-invasive and sophisticated test is regarded as a valuable assessment tool in research and clinical practice. Cardiopulmonary exercise test has been extensively utilized to define the mechanisms of exercise intolerance in various clinical disorders, to evaluate responses to therapy and indicate disease prognosis. Emerging data obtained from the use of the cardiopulmonary exercise testing in the research field, has led to its extensive clinical usage. It is now utilized as an integral part of the patients' clinical evaluation in the field of respiratory and cardiovascular medicine, sports medicine, surgery as well as occupational and rehabilitative medicine. It has a clinical role in assessing patient's functional capacity, monitoring disease progression and response to therapy, predicting prognosis, and perioperative morbidity and mortality, as well as constructing and monitoring training and rehabilitative programs. This article aims to give an overview of the physiological profiles obtained from cardiopulmonary exercise testing, its methodological aspects, as well as its utility in research and clinical practice.     Some chalcones, such as hydroxychalcones have been reported previously to inhibit major pro-inflammatory mediators such as nitric oxide (NO), prostaglandin E(2) (PGE(2)), tumor necrosis factor-alpha (TNF-alpha) and reactive oxygen species production by suppressing inducible enzyme expression via inhibition of the mitogen-activated protein kinase (MAPK) pathway and nuclear translocation of critical transcription factors. In this report, the effects of cardamonin (2',4'-dihydroxy-6'-methoxychalcone), a chalcone that we have previously isolated from Alpinia rafflesiana, was evaluated upon two cellular systems that are repeatedly used in the analysis of anti-inflammatory bioactive compounds namely RAW 264.7 cells and whole blood. Cardamonin inhibited NO and PGE(2) production from lipopolysaccharide- and interferon-gamma-induced RAW cells and whole blood with IC(50) values of 11.4 microM and 26.8 microM, respectively. Analysis of thromboxane B(2) (TxB(2)) secretion from whole blood either stimulated via the COX-1 or COX-2 pathway revealed that cardamonin inhibits the generation of TxB(2) via both pathways with IC(50) values of 2.9 and 1.1 microM, respectively. Analysis of IC(50) ratios determined that cardamonin was more COX-2 selective in its inhibition of TxB(2) with a ratio of 0.39. Cardamonin also inhibited the generation of intracellular reactive oxygen species and secretion of TNF-alpha from RAW 264.7 cells in a dose responsive manner with IC(50) values of 12.8 microM and 4.6 microM, respectively. However, cardamonin was a moderate inhibitor of lipoxygenase activity when tested in an enzymatic assay system, in which not a single concentration tested was able to cause an inhibition of more than 50%. Our results suggest that cardamonin acts upon major pro-inflammatory mediators in a similar fashion as described by previous work on other closely related synthetic hydroxychalcones and strengthens the conclusion of the importance of the methoxyl moiety substitution on the 4' or 6' locations of the A benzene ring.

    The flocculation performances of nine cationic and anionic polyacrylamides with different molecular weights and different charge densities in the treatment of pulp and paper mill wastewater have been studied. The experiments were carried out in jar tests with the polyacrylamide dosages range of 0.5-15 mg l(-1), rapid mixing at 200 rpm for 2 min, followed by slow mixing at 40 rpm for 15 min and settling time of 30 min. The effectiveness of the polyacrylamides was measured based on the reduction of turbidity, the removal of total suspended solids (TSS) and the reduction of chemical oxygen demand (COD). Cationic polyacrlyamide Organopol 5415 with very high molecular weight and low charge density is found to give the highest flocculation efficiency in the treatment of the paper mill wastewater. It can achieve 95% of turbidity reduction, 98% of TSS removal, 93% of COD reduction and sludge volume index (SVI) of 14 ml g(-1) at the optimum dosage of 5 mg l(-1). SVI values of less than 70 m lg(-1) are found for all polyacrylamide at their respective optimum dosage. Based on the cost evaluation, the use of the polyacrylamides is economically feasible to treat the pulp and paper mill wastewaters. This result suggests that single-polymer system can be used alone in the coagulation-flocculation process due to the efficiency of the polyacrylamide. Sedimentation of the sludge by gravity thickening with settling time of 30 min is possible based on the settling characteristics of the sludge produced by Organopol 5415 that can achieve 91% water recovery and 99% TSS removal after 30 min settling.

    The effect of initial dissolved oxygen concentration (IDOC) on Fenton's reagent degradation of a dyestuff, Reactive Black 5 was explored in this study. The study was designed, conducted and analysed based on Central Composite Rotatable Design using a 3-1 lab-scale reactor. The participation of O2 in the process was experimentally observed and appears to be affected by the dosage of the reagents used in the study. The IDOC was found to have a significant influence on the process. Reducing the IDOC from 7.5 mg l(-1) to 2.5 mg l(-1) increased the removal of TOC by an average of about 10%. Reduction of IDOC from 10 mg l(-1) to 0 mg l(-1) enhanced the TOC removal by about 30%. The negative influence of IDOC is likely to be caused by the competition between the O2 and the reagents for the organoradicals. A model describing the relationship between initial TOC removal, reagent dosage and IDOC has also been developed.

    OBJECTIVE: To revisit cut-off values of BMI, waist circumference (WC), and waist-to-stature ratio (WSR) based on their association with cardiorespiratory fitness (CRF). The derived cut-off points were compared with current values (BMI, 25.0 kg/m(2); WC, 80 cm) as recommended by the World Health Organization.
    RESEARCH METHODS AND PROCEDURES: Anthropometric indices were measured in a cross sectional study of 358 Singaporean female employees of a large tertiary hospital (63% Singaporean Chinese, 28% Malays, and 9% Indians). CRF was determined by the 1-mile walk test. Receiver operating characteristic curves were constructed to determine cut-off points.
    RESULTS: The cut-off points for BMI, WC, and WSR were 23.6 kg/m(2), 75.3 cm, and 0.48, respectively. The areas under the curve of BMI, WC, and WSR were 0.68, 0.74, and 0.74, respectively. For a given BMI, women with low CRF had higher WSR compared with women with high CRF.
    DISCUSSION: These findings provide convergent evidence that the cut-off points for Singaporean women were lower than the World Health Organization's criteria but were in good agreement with those reported for Asians.

    This study aimed to determine effects of 6-day progressive increase in salinity from 1 per thousand to 15 per thousand on nitrogen metabolism and excretion in the soft-shelled turtle, Pelodiscus sinensis. For turtles exposed to 15 per thousand water on day 6, the plasma osmolality and concentrations of Na+, Cl- and urea increased significantly, which presumably decreased the osmotic loss of water. Simultaneously, there were significant increases in contents of urea, certain free amino acids (FAAs) and water-soluble proteins that were involved in cell volume regulation in various tissues. There was an apparent increase in proteolysis, releasing FAAs as osmolytes. In addition, there might be an increase in catabolism of certain amino acids, producing more ammonia. The excess ammonia was retained as indicated by a significant decrease in the rate of ammonia excretion on day 4 in 15 per thousand water, and a major portion of it was converted to urea. The rate of urea synthesis increased 1.4-fold during the 6-day period, although the capacity of the hepatic ornithine urea cycle remained unchanged. Urea was retained for osmoregulation because there was a significant decrease in urea excretion on day 4. Increased protein degradation and urea synthesis implies greater metabolic demands, and indeed turtles exposed to 15 per thousand water had significantly higher O2 consumption rate than the freshwater (FW) control. When turtles were returned from 15 per thousand water to FW on day 7, there were significant increases in ammonia (probably released through increased amino acid catabolism) and urea excretion, confirming that FAAs and urea were retained for osmoregulatory purposes in brackish water.

    In Sarawak, pig farm operators are required to treat the wastewater containing pig waste and spilt food in oxidation ponds before discharge. However, information on the impact of this industry on surface water quality is lacking. Therefore, the objective of this study is to determine the impact of pig farm effluent on the water quality of Serin River and its tributaries. Results of analysis show that the tributary that received pond effluent has significantly higher mean of total suspended solids (TSS), biochemical oxygen demand (BOD5) chemical oxygen demand (COD), ammonia-nitrogen (NH3 -N), reactive phosphorus (SRP) and Escherichia coli (E. coli) concentrations when compared to those of the tributary that did not receive pond effluent. Comparisons between the stations upstream and downstream of the discharge point indicated that BOD5 and COD were significantly higher at the downstream station that received pond effluent. Dissolved oxygen (DO) was the lowest at the tributary receiving effluent from pig farms with a mean of 2.40 mg/l. According to the Interim Water Quality Standard of the Department of Environment, water quality at the tributary that received pig farm effluent falls into Class III whereas that of the other stations falls into Class II. It is recommended that further studies be conducted on the management of waste to explore the possibility of turning the waste into a resource so that water quality of rivers can remain pristine for drinking and recreation.
