    Introduction: Hypertension related morbidities and mortalities around the world show a gradual increase and early detection and prevention are advocated. The Database of Genomic Variants (DGV) has associated variation in DNA sequences called copy number variation (CNV) with susceptibility to common diseases. However, little is known about CNV role in essential hypertension. Thus, this study aimed to characterize the CNV esv27061 among prehypertensive and hypertensive young adults in Malaysia. Materials and method: In this comparative cross-sectional study, 104 subjects living in Kuantan who gave voluntary consent to participate are recruited and divided into three groups; control (43 subjects), prehypertensive (38 subjects) and mild hypertensive (23 subjects). An optimized droplet digital polymerase chain reaction (ddPCR) was used in the determination of CNV esv27061 in this study. Results: All subjects in the control (n=38; 88.4% gain), prehypertensive (n=33; 86.8% gain) and mild hypertensive (n=21; 91.3% gain) groups had CNV gain (copy number > 2) while 11.6% of control, 13.2% of prehypertensive and 8.7% of mild hypertensive subjects exhibited normal copies (copy number = 2). Conclusion: The present preliminary finding was consistent with the Database of Genomic Variants (DGV) which stated that CNV esv27061 showed more gain than loss.

    Tuberculosis (TB) is the second biggest killer disease after HIV. Therefore, early detection is vital to
    prevent its outbreak. This paper looked at an automated TB bacteria counting using Image Processing technique and Matlab Graphical User Interface (GUI) for analysing the results. The image processing algorithms used in this project involved Image Acquisition, Image Pre-processing and Image Segmentation. In order to separate any overlap between the TB bacteria, Watershed Segmentation techniques was proposed and implemented. There are two techniques in Watershed Segmentation which is Watershed Distance Transform Segmentation and Marker Based Watershed Segmentation. Marker Based Watershed Segmentation had 81.08 % accuracy compared with Distance Transform with an accuracy of 59.06%. These accuracies were benchmarked with manual inspection. It was observed that Distance Transform Watershed Segmentation has disadvantages over segmentation and produce inaccurate results. Automatic counting of TB bacteria algorithms have also been proven to be less time consuming, contains less human error and consumes less man-power.

    Colorectal cancer (CRC), gastroesophageal reflux disease (GERD), inflammatory bowel disease (IBD), and nonalcoholic fatty liver disease are considered important emerging diseases in the Asia Pacific (AP) region. The incidence rate of CRC is the highest among gastrointestinal cancers in the region surpassing that of gastric cancer. However, population CRC screening is limited by availability of adequate health resources and financing. GERD is a highly prevalent disease in AP with the prevalence of GERD symptoms and reflux esophagitis reported to be increasing. The usage of proton pump inhibitors has also been reported to be high. The incidence and prevalence of IBD is not as high as in the west but is now an increasingly recognizable disease in the AP region. Being a complicated disease, IBD will pose a huge financial burden with the increasing use of expensive biological drugs. In tandem with the exponential increase in obesity and diabetes mellitus in AP, nonalcoholic fatty liver disease will become the most important liver disease in the region in the coming years. These emerging diseases reflect the continued fast-paced socioeconomic development in the region with marked lifestyle changes and increasing affluence.

    Thalassaemia is a hereditary blood disorder that is becoming a major health problem all over the world. This chronic illness harms the quality of life of the sufferers by interrupting their physical activities, school performance and social life. Hence, this review takes aim to assess the factors affecting the quality of life of thalassaemia among paediatrics patients. A comprehensive electronic search was conducted by using PubMed, Google Scholar and Science Direct. The search was limited to those articles written in English language and by using Pediatrics Quality of Life Inventory (PedsQLTM) 4.0 generic core scale questionnaire only. This review notifies emerging knowledge regarding the factors affecting the quality of life among thalassaemia patients and its implications in the essential core domains for paediatrics health-related quality of life measurements: physical, emotional, social and school functioning. It also empowers a better understanding regarding thalassaemia and assists as a foundation for the development of the effective preventive strategies for it.

    Introduction: We present a case of broken peripheral intravenous catheter/cannula (PIVC), a well-known, underreported complication of PIVC placement. The fractured cannula could have resulted in intravascular foreign body retention, which is usually iatrogenic.

    Presentation of case: In this case, we conceded that both iatrogenic and self-infliction were culpable. The intoxicated, aggressive patient forcefully removed the inserted cannula after repeated attempts by medical personnel to place it. The same cannula was used for multiple attempts. After the location of the fractured catheter was reconfirmed with radiological imaging, venotomy and removal of the foreign body were performed.

    Conclusion: Due to potentially devastating consequences, early detection, adherence to standard operating procedures for peripheral venous access, management of aggressive patients, and meticulous teamwork must be upheld.

    The menace of cholera epidemic occurrence in Yemen was reported in early 2017. Recent reports revealed that an estimated 500,000 people are infected with cholera whereas 2,000 deaths have been reported in Yemen. Cholera is transmitted through contaminated water and food. Yemen is the least developed country among the Middle East countries in terms of wastewater and solid waste management. The population of Yemen is about 24.5 million and generates about 70-100 million m3 of sewage. An estimated 7% of the population has sewerage systems. It has been revealed that 31.2 million m3 of untreated sewage is used for irrigation purposes especially for vegetables and Khat trees. In addition, more than 70% of the population in Yemen has no potable water. They depend on water wells as a water source which are located close to sewage disposal sites. The present review focuses on the current status of water, wastewater as well as solid waste management in Yemen and their roles in the outbreak of cholera. Future prospects for waste management have been proposed.

    BACKGROUND: Cardiovascular disease remains a leading cause of death worldwide, with hypercholesterolemia being a major risk factor. Evidence-based consensus guidelines have recommended consideration of increasingly stringent cholesterol-lowering goals, yet most patients do not meet these targets. Coronary heart disease (CHD) event and mortality rates and mean serum cholesterol levels have declined in Singapore in recent years; however, certain groups remain at elevated risk.

    OBJECTIVE: To determine (i) proportions of patients with CHD in Singapore who achieved goals for serum low-density lipoprotein-cholesterol (LDL-C); and (ii) factors influencing goal attainment.

    METHODS: A historical cohort study was conducted using records from the Singapore Cardiac Databank, a national registry of CHD patients. Serum LDL-C goal attainment was assessed in 5174 survivors of acute myocardial infarction or coronary revascularization (i.e. coronary artery bypass graft surgery or percutaneous coronary interventions), of whom 3811 (73.7%) were at very high risk.

    RESULTS: At baseline, the mean patient age was 60.3 years, mean serum value of total cholesterol was 228 mg/dL, and mean LDL-C was 163 mg/dL. Of all CHD patients, approximately 70% did not achieve a serum LDL-C target of <100 mg/dL. Most patients receiving HMG-CoA reductase inhibitor (statin) regimens were treated initially with low- to medium-equipotency regimens and were never titrated to stronger regimens. The vast majority (approximately 94%) of patients at very high risk did not achieve the stringent serum LDL-C target of <70 mg/dL. Patients receiving higher potency statins were significantly more likely to achieve LDL-C goals, whereas those with higher baseline LDL-C levels or Malaysian ethnicity were less likely to achieve LDL-C goals.

    CONCLUSIONS: Most CHD patients in the large group of Singapore residents with CHD in the present study did not achieve recommended LDL-C targets. A more effective disease-management approach, including patient education concerning lifestyle modification (e.g. diet, physical activity), efforts to enhance medication adherence, and more effective, well tolerated therapies such as high-equipotency or high-dose statins and statin combination regimens, may be needed to improve achievement of consensus cholesterol targets. This is the first study of cholesterol goal attainment in a large group of Southeast Asians and serves as a baseline for future evaluations in Asian populations.

    Accurate identification of mosquitoes is crucial for the management and control of vector species. Although Singapore is a tropical country with high mosquito diversity, there are no identification keys specific to its mosquito fauna. An illustrated identification key to the genera of the adult female mosquitoes of Singapore is presented. Twenty genera are characterized and two genera found in Malaysia, Culiseta and Topomyia, are also included. The key will aid non-taxonomically trained operational officers in adult mosquito identification. The key to the genera provides the first crucial step in the process to species identification.

    Haemothorax refer to the bleeding in the pleural space. It is commonly due to iatrogenic, blunt or penetrating chest trauma. Non-traumatic haemothorax is a rare entity that can potentially lead to life threatening complications. The initial management of both traumatic and non-traumatic haemothorax includes resuscitation and stabilisation of the patient. We would like to present two cases of non-traumatic haemothorax secondary to an avulsed bullae vessel.

    This communication shares insights into the definition of disease progression and drug durability in type 2 diabetes. Disease progression may be defined as gradual worsening of beta-cell function, clinically observed as an increase in drug dosage, drug frequency or number of glucose lowering drugs needed to maintain HbA1c control; and/or a ≥0.5% rise in HbA1c, unexplained by acute, modifiable factors, while using the same drug regimen; and/or as the occurrence or worsening of cardiovascular or microvascular complications, in spite of standard care, over a pre-specified time period. Durability of a drug or a drug combination may be defined as its ability to postpone or delay progression of disease, in a safe and well tolerated manner. Thus, all drugs that are able to prevent disease progression (i.e., postpone loss of glycaemic control, need for intensification of therapy or onset or worsening of complications) may be termed 'durable'.

    Kajian ekologi nyamuk merupakan kajian yang penting dalam merangka sesuatu aktiviti kawalan penyakit bawaan nyamuk. Satu kajian ekologi nyamuk Aedes telah dijalankan di Senawang, Negeri Sembilan dari Julai 2009 hingga Febuari 2010. Senawang terletak di pinggir bandaraya Seremban yang merupakan kawasan perindustrian kecil dan sederhana dengan jumlah penduduk yang padat. Kajian ekologi vektor ini bertujuan menentukan komposisi nyamuk secara keseluruhan terutamanya kepadatan populasi Aedes, masa kemuncak gigitan nyamuk, mengenal pasti kehadiran tempat pembiakan nyamuk dan menentukan hubung kait antara nyamuk Aedes dengan faktor persekitarannya seperti suhu, kelembapan relatif, halaju angin dan curahan hujan. Pensampelan nyamuk Aedes menggunakan empat kaedah iaitu perangkap cahaya (CDC), tangkapan nyamuk berumpan kaki (BLC), ovitrap dan kajian Aedes. Terdapat dua spesies nyamuk di lokasi kajian yang didominasi oleh Ae. albopictus (93.29%) dan Ae. aegypti (0.11%). Terdapat dua masa kemuncak gigitan nyamuk Ae. albopictus iaitu awal pagi (0700-0900 pagi) dan lewat petang (jam 1700-1900). Suhu persekitaran, kelembapan relatif dan kelajuan angin didapati mempunyai kaitan dengan gigitan nyamuk Aedes pada perumah (p < 0.05). Taburan hujan didapati tidak mempunyai hubungan dengan kepadatan nyamuk Aedes (r = -0.137, n =6 , p > 0.05). Kepadatan nyamuk Ae. albopictus adalah tinggi di lokasi kajian dengan Indeks Ovitrap 100% bagi kesemua stesen. Kajian Aedes mendapati daripada 30 bekas yang positif larva, 93.33% daripadanya didominasi oleh Ae. albopictus. Kajian ini menunjukkan kepadatan Ae. albopictus adalah tinggi dan berpotensi membawa penyakit demam denggi dan chikungunya di lokasi kajian. Penjagaan persekitaran perlu ditekankan kepada masyarakat setempat untuk mengawal penyebaran penyakit-penyakit tersebut.

    Introduction: Entomological surveillance is crucial to determine the abundance of dengue vector and to evaluate breeding areas of Aedes aegypti and Aedes albopictus. The objective of this study is to determine the distribution and breeding preference for both Ae. aegypti and Ae. albopictus in dengue endemic areas. Materials and Methods: Ovitraps surveillance was conducted in two dengue endemic areas; AU2, Keramat and Seksyen 7, Shah Alam, Selangor. A minimum number of 100 ovitraps were deployed for 5 days in the study sites. Samples collected were brought back to the lab and all larvae recovered were identified to species level. Results: The ovitap index (OI) in both localities exceeded the transmission threshold of 10% with the OI recorded ranged from 42.3-79.8% in AU2, Keramat and 16.7-42.9% in Seksyen 7, Shah Alam. Ae. albopictus was the dominant species in AU2 Keramat with the highest ratio Ae. aegypti to Ae. albopictus recorded was 1.00:22.79. Nonetheless, in Seksyen 7, Shah Alam the difference in Ae. aegypti to Ae. albopictus ratio is not really prominent with 1.00:3.61 for ovitraps deployed outdoor and 3.40:1.00 for ovitraps set indoor. It was determined that single infestation of either Ae. aegypti or Ae. albopictus is more frequent for ovitraps deployed indoor and/or outdoor, respectively. It was also determined that mixed infestations were found in this study indicating that both species can oviposit in the same container. Conclusion: This study indicates that OI is still above transmission threshold in both study sites. While Ae. aegypti and Ae. albopictus remain as a dominant indoor and outdoor breeder, respectively, mixed breeding of Aedes species in a same container was also observed.

    INTRODUCTION: MACK-3 (combination of hoMa, Ast and CK18) was reported to be a good biomarker for the diagnosis of fibrotic non-alcoholic steatohepatitis (NASH). However, there is no external validation to date.

    AIM: To evaluate the accuracy of MACK-3 for the diagnosis of fibrotic NASH.

    METHODOLOGY: Consecutive adult non-alcoholic fatty liver disease (NAFLD) patients who had liver biopsy in a university hospital were included. MACK-3 was calculated using the online calculator using the following variables: fasting glucose, fasting insulin, aspartate aminotransferase (AST) and cytokeratin 18 (CK18). MACK-3 cut-offs ≤0.134 and ≥0.550 were used to predict absence and presence of fibrotic NASH, respectively. Histopathological examination of liver biopsy specimen was reported according to the NASH Clinical Research Network Scoring System.

    RESULTS: Data for 196 subjects were analysed. MACK-3 was good for diagnosis of fibrotic NASH (area under receiver-operating characteristics curve [AUROC] 0.80), comparable to the Fibrosis-4 index (FIB4) and the NAFLD fibrosis score (NFS) and superior to the BARD score and CK18. MACK-3 was good for diagnosis of active NASH (AUROC 0.81) and was superior to other blood fibrosis tests. The overall accuracy, percentage of subjects in grey zone, sensitivity, specificity, positive predictive value and negative predictive value of MACK-3 for diagnosis of fibrotic NASH was 79.1%, 46.9%, 100%, 43.8%, 43.1% and 100%, respectively, while for diagnosis of active NASH was 90.0%, 39.3%, 84.2%, 81.4%, 88.9% and 74.5%, respectively.

    CONCLUSION: MACK-3 is promising as a non-invasive test for active NASH and fibrotic NASH and may be useful to identify patients who need more aggressive intervention.
