    INTRODUCTION: The quality of information and efficiency in the practice and care environments are important aspects of nursing care. The use of a reliable and valid scale can monitor the quality of handover and provide information for continuous improvement of practice. This study aims to describe the perception of nurses, on the domains of quality of information, efficiency, interaction and support and patient involvement.

    METHOD: A cross-sectional descriptive study was conducted among 450 nurses from 37 wards in Hospital Kuala Lumpur. Nurses on shift duty were recruited by convenience sampling from the Medical, Surgery, Obstetrics & Gynaecology, Orthopaedic and Paediatric wards. Using a validated questionnaire (Handover Evaluation Scale), nurses self-rated their perceptions using a 7-point scale and provided open-ended responses to the strengths and challenges that they faced. Descriptive and inferential analyses were done while open-ended questions were summarised based on key themes.

    RESULTS: A total of 414 nurses completed the survey (92.0% response rate). Nurses had an overall mean (SD) perception score of 5.01 (SD 0.56). They perceived good interaction and support during handover and on the quality of information that they received, with mean scores of 5.54 (SD 0.79) and 5.19 (SD 0.69), respectively. There was an association between the departments where the nurses worked and their overall perceptions on nursing handover (p<0.001). Interruptions being the most common theme emerged from the open-ended section.

    CONCLUSION: Despite having substantial interaction and support amongst nurses, opportunities for improvements were noted. Improvements in the quality of handover information and reducing interruptions should be the main emphases as these were perceived to be essential in the current handover practices by nurses.

    The purpose of this study is to explore diabetes patients' views on various factors contributing to nonadherence to dietary therapy. Using a qualitative approach, 17 in-depth interviews were conducted with type 2 diabetes mellitus patients at Hospital Sultanah Bahiyah, Kedah state, Malaysia. All interviews were audio recorded, transcribed verbatim, and translated before analysis. Thematic content analysis was used and three main themes emerged. The main factors that affect diabetes patients' dietary adherence were "individual preference," "family support," and "social and cultural activities." Difficulty to change the existing meals, poor family support, practice of eating out, and social and cultural gatherings were among the factors that influenced diabetes patients' adherence toward dietary therapy. Most challenges were related to the social role of food in the multicultural population. Therefore, in addition to the family and community involvement, dietary therapy should be individualized to improve patient adherence.

    The practice of brachytherapy in unresectable tongue carcinoma is gaining popularity. However, this procedure poses specific anesthetic challenges, particularly challenges of airway sharing and a higher rate of difficult airway. We report a 74-year-old chronic smoker, chronic alcoholic with history of stroke, who had undergone brachytherapy for tongue carcinoma. Apart from a huge tongue tumor, he had an epiglottic mass but refused elective tracheostomy. This had led to a few critical states throughout the process of treatment, including a metabolic crisis due to thiamine deficiency and difficult airway crisis. To our best knowledge, there have been no reported case on a patient with vocal cord mass undergoing tongue brachytherapy. We hope sharing of this experience may aid the management of similar patients in future.

    A simple water iodizing system, which incorporates the Venturi principle in combination with the controlled release mechanism of a silicone-sodium iodide elastomer, for the iodization of rural piped-water supply in the control of endemic iodine deficiency has been developed and its effectiveness evaluated in three Iban longhouse villages in the iodine-deficient district of Lubok Antu, Sarawak. Urines were collected for iodine assays from women aged 15-40 years before and at 6 and 12 months after the connection of the iodinating device; goiter assessment was performed on the women at the start and end of the 1-year study. Water samples were collected for iodine assays at 2-weekly intervals. In all three villages, significant and sustained increases in median urinary iodine excretions, reaching levels recommended for an iodine-sufficient population, were observed; goitre prevalences were reduced in all the villages (by 22.6% to 35.8%). The iodine levels in the water ranged from 34 micrograms/l to 212 micrograms/L. In the control village, median urinary iodine excretions remained essentially unchanged but a small increase in goiter prevalence was observed. The iodized water was well received by the villagers and no adverse effects of water iodization were observed. The system functioned unattended throughout the one year period. The cost of providing supplemental iodine via the iodizing device is approximately 60 cents (U.S.) per family per year which is affordable by either the Government or the villagers. It is concluded that the iodizing system offers a new cost-effective strategy for the control of endemic iodine deficiency in Sarawak and may have applications in other areas with similar water sources.

    Methyl ester sulphonates (MES) have been considered as an alternative green surfactant for the detergent market. Investigation on the purification of methyl ester sulphonates (MES) with various carbon chains of C12, C14, C16 and C16-18 derived from palm methyl ester is of great interest. These MES powders have been repeatedly crystallized with ethanol and the purity of MES has increased to a maximum of 99% active content and 96% crystallinity index without changing the structure. These crystallized MES with high active content have 1.0% to 2.3% moisture content and retained its di-salt content in the range of 5%. The crystallized MES C16 and C16-18 attained excellent flow characteristics. Morphology, structural and its crystallinity analyses showed that the crystals MES had good solubility properties, stable crystal structure (β polymorphic) and triclinic lateral structure when it is in high active content. The brittleness of MES crystals increased from a β' to a β subcell. Crystal with high brittleness has the potential to ease production of powder, which leads to a reduction in the cost of production and improves efficiency.

    Ethylene glycol is a super commodity chemical and it has vital roles in various applications. Its co-production with other chemicals, such as ethylene carbonate and glycerol carbonate, has promised cheaper production cost. Its quantification presents a challenge as its contaminants, such as ethylene carbonate, produce a signal-reducing effect in flame ionized detector. The aim of this study is to evaluate external standard to quantify the composition of glycol mixture. Measurement system analysis was employed on the external standard method. Reliability of the external standard is statistically significant with low p-values, excellent capability indices, and high F-values. The external standard is found to have remarkable precision and trueness as both capability indices are mirroring each other. Furthermore, the capability analysis has a strong correlation with quality measurement. Based on capability indices, the limit of detection is recommended at S/N = 25 and the limit of quantification is recommended at S/N = 100 for a reliable measurement. A high degree of reliability is achieved coherently as almost all uncertainties of coefficients of variations are less than 5%. The established method was validated and successfully applied to glycol mixture at azeotropic distillation pilot plant.

    Prazosin (PRZ) and levonorgestrel (LNG) are widely used as an anti-disease drugs due to their biological activity in the human body. The frequent detection of these compounds in water samples requires alternative technologies for the removal of both compounds. After electrochemical degradation of PRZ and LNG, the parent compounds could be completely removed after treatment, but the identification and characterization of by-products are necessary as well. In this study, the effects of NaCl concentration and applied voltage were investigated during the electrochemical degradation process. The results revealed that the increase of NaCl concentration and applied voltage could promote the generation of hypochlorite OCl- and then enhance the degradation of PRZ and LNG. After initial study, 6V and 0.2g NaCl were selected for further experiments (96% and 99% removal of PRZ and LNG after 40min, respectively). Energy consumption was also evaluated and calculated for PRZ and LNG at 3, 6 and 8V. Solid phase extraction (SPE) method plays an important role in enhancing the detection limit of by-products. Furthermore, characterization and identification of chlorinated and non-chlorinated by-products were conducted using an accurate liquid chromatography-time of flight/mass spectrometry LC-TOF/MS instrument. The monitoring of products during the electrochemical degradation process was performed at 6V and 0.2g NaCl in a 50mL solution. The results indicated that two chlorinated products were formed during the electrochemical process. The toxicity of by-products toward E. coli bacteria was investigated at 37°C and 20hr incubation time.

    BACKGROUND: Melioidosis, a fatal infectious disease caused by Burkholderia pseudomallei, is increasingly diagnosed in tropical regions. However, data on risk factors and the geographic epidemiology of the disease are still limited. Previous studies have also largely been based on the analysis of case series data. Here, we undertook a more definitive hospital-based matched case-control study coupled with spatial analysis to identify demographic, socioeconomic and landscape risk factors for bacteremic melioidosis in the Kedah region of northern Malaysia.

    METHODOLOGY/PRINCIPAL FINDINGS: We obtained patient demographic and residential information and clinical presentation and medical history data from 254 confirmed melioidosis cases and 384 matched controls attending Hospital Sultanah Bahiyah (HSB), the main tertiary hospital of Alor Setar, the capital city of Kedah, during the period between 2005 and 2011. Crude and adjusted odds ratios employing conditional logistic regression analysis were used to assess if melioidosis in this region is related to risk factors connected with socio-demographics, various behavioural characteristics, and co-occurring diseases. Spatial clusters of cases were determined using a continuous Poisson model as deployed in SaTScan. A land cover map in conjunction with mapped case data was used to determine disease-land type associations using the Fisher's exact test deploying simulated p-values. Crude and adjusted odds ratios indicate that melioidosis in this region is related to gender (males), race, occupation (farming) and co-occurring chronic diseases, particularly diabetes. Spatial analyses of disease incidence, however, showed that disease risk and geographic clustering of cases are related strongly to land cover types, with risk of disease increasing non-linearly with the degree of human modification of the natural ecosystem.

    CONCLUSIONS/SIGNIFICANCE: These findings indicate that melioidosis represents a complex socio-ecological public health problem in Kedah, and that its control requires an understanding and modification of the coupled human and natural variables that govern disease transmission in endemic communities.

    BACKGROUND: Anxiety disorders are common mental health disorders with significant impact on the individual as well as burden on the country as a whole.
    METHODS: A systematic review of databases, reference lists, internet sources, and input from content experts revealed 42 studies that documented the prevalence of anxiety symptoms or disorders. 12 of these studies specifically evaluated anxiety disorders.
    RESULTS: 4 studies looked at the prevalence of anxiety disorders in the general population, whilst the remainder focused on selected population groups: university students (4 studies); substance abuse (3 studies); and victims of abuse (1 study). Studies in the general population showed that the prevalence of generalised anxiety disorder was 0.4-5.6%, mixed anxiety and depression were 3-5%, panic without agoraphobia 0.4%, phobia unspecified 0.5-%, and anxiety not-otherwise-specified 0.3-6.5%. We found significant variability in anxiety disorders in the studies in selected population groups. The variability could also have been affected by methodological factors within each study.
    CONCLUSION: This study provides a broad overview of the prevalence of anxiety disorders in Malaysia. More research is required to develop diagnostic instruments that are validated for local use and comparable with international standards. Reliable prevalence estimates are lacking within certain groups, e.g. those in rural, indigenous, migrant population groups and those exposed to natural disasters.

    Brooding rumination is positively associated with symptoms of both depression and posttraumatic stress disorder (PTSD). However, non-clinical cross-cultural research indicates that culture may influence these associations. This study aimed to examine the moderating effect of cultural group (Australian versus Malaysian) on the associations between brooding rumination and symptoms of depression and PTSD. European Australians (n = 109) and Malaysians of varying Asian heritages (n = 144) completed an online questionnaire containing the Hospital Anxiety and Depression Scale, PTSD checklist for DSM-5 and the Ruminative Response Scale-Short Form. First, Malaysian participants had higher brooding rumination than Australian participants. Second, higher levels of brooding rumination were positively associated with depression and PTSD symptom severity. Third, contrary to our expectations, cultural group did not moderate the relationships between brooding rumination and symptoms of depression and PTSD. If replicable, these results suggest that existing assessment and treatment approaches that target brooding rumination may apply to Malaysian individuals with depression and PTSD.

    The emergence of coronavirus disease has impacted human lives, one of which is economic disruption. Many Malaysian organisations have devised various crisis-response techniques, such as downsizing, laying off, retrenching, and combining. As a result, the number of Malaysians working in precarious jobs, which are defined by unpredictable and uncertain situations, has indirectly increased, hence increasing job insecurity. Therefore, maintaining psychological health is essential to safeguarding the mental health of employees. In the current working landscape, job security and work-life balance have commonly been deemed necessary in contributing to well-being among employees. As a result, the purpose of this study was to examine how work-life balance influences the relationship between job insecurity and psychological well-being among Malaysian precarious workers. It also fills a gap in the research by explaining the causal association between job insecurity and psychological well-being among precarious workers, as previous well-being studies have largely focused on employees with secure jobs. There were 442 responders collected using purposive and snowball sampling methods, and they were requested to complete the Job Insecurity Scale (JIS), Work-Life Balance, and Well-Being Index Scale (WHO-5). Job instability was negatively connected with work-life balance and psychological well-being. On the other hand, work-life balance was found to be positively related to psychological well-being. This supports the notion that work-life balance is a significant mediator in the relationship between job insecurity and psychological well-being. These findings emphasise that Malaysian precarious workers with job security could enhance their psychological well-being by achieving work-life balance.

    INTRODUCTION: Self-monitoring of blood glucose (SMBG) has been underutilised. We conducted an open-label, randomised controlled trial to assess the feasibility of introducing SMBG in primary care clinics in Malaysia.

    METHODS: This was an open-label, randomised controlled trial conducted in five public primary care clinics in Malaysia. Patients with type 2 diabetes mellitus (age range 35-65 years) not performing SMBG at the time of the study were randomised to receive either a glucometer for SMBG or usual care. Both groups of patients received similar diabetes care from the clinics.

    RESULTS: A total of 105 patients with type 2 diabetes mellitus were enrolled. Of these, 58 and 47 were randomised to intervention and control groups, respectively. After six months, the glycated haemoglobin (HbA1c) level in the intervention group showed a statistically significant improvement of 1.3% (p = 0.001; 95% confidence interval 0.6-2.0), relative to the control group that underwent usual care. The percentages of patients that reached the HbA1c treatment target of ≤ 7% were 14.0% and 32.1% in the control and intervention groups (p = 0.036), respectively.

    CONCLUSION: The usage of a glucometer improved glycaemic control, possibly due to the encouragement of greater self-care in the intervention group.

    Sago bark (SB) and empty fruit bunch (EFB) are available abundantly as agricultural waste in Sarawak. This study was conducted to investigate the physicochemical characteristics of SB and EFB as composting materials and used as a plant growth medium. The SB and EFB composts were prepared in a separate container by mixing chicken manure as compost accelerator and wood chips as a bulking agent in dry weight equivalent ratio (1:1:1). The maturity and stability of compost in 60-day composting periods were evaluated via physicochemical characterization of the composts in terms of pH, elemental content, total ash content, moisture content and nutrient analyses. The effect of the compost usage as growth medium was assessed towards water spinach and green mustard via seed germination and pot study. After 2 months, the colour of both composts was dark brown with an earthy smell. The acidic pH of the initial composting stage has changed into alkaline pH after 60 days of composting. Total NPK present in the SB and EFB composts were 0.96% and 1.21%, respectively. The germination index (GI) for the studied vegetables was above 100%, while the pot study showed that vegetables in compost media has higher growth compared to the control, after 14 days. SB and EFB are renewable waste which can be used as an excellent compost and able to improve the quality of the soil.

    Air surface temperature (AST) is a crucial importance element for many applications such as hydrology, agriculture, and climate change studies. The aim of this study is to develop regression equation for calculating AST and to analyze and investigate the effects of atmospheric parameters (O3, CH4, CO, H2Ovapor, and outgoing longwave radiation (OLR)) on the AST value in Iraq. Dataset retrieved from the Atmospheric Infrared Sounder (AIRS) at EOS Aqua Satellite, spanning the years of 2003 to 2016, and multiple linear regression were used to achieve the objectives of the study. For the study period, the five atmospheric parameters were highly correlated (R, 0.855-0.958) with predicted AST. Statistical analyses in terms of β showed that OLR (0.310 to 1.053) contributes significantly in enhancing AST values. Comparisons among selected five stations (Mosul, Kanaqin, Rutba, Baghdad, and Basra) for the year 2010 showed a close agreement between the predicted and observed AST from AIRS, with values ranging from 0.9 to 1.5 K and for ground stations data, within 0.9 to 2.6 K. To make more complete analysis, also, comparison between predicted and observed AST from AIRS for four selected month in 2016 (January, April, July, and October) has been carried out. The result showed a high correlation coefficient (R, 0.87 and 0.95) with less variability (RMSE ≤ 1.9) for all months studied, indicating model's capability and accuracy. In general, the results indicate the advantage of using the AIRS data and the regression analysis to investigate the impact of the atmospheric parameters on AST over the study area.

    This study was conducted to determine the effect of ethnicity on maternal, placental and neonatal parameters. Maternal, placental, and the newborn parameters were corrected for gestational age. The male:female sex ratio was 1:1.03. One hundred and forty-four freshly delivered placentae from 55 Malaysian, 51 Chinese, and 38 Indian normal healthy patients were collected and standard stereological methods used to estimate the placental parameters. Pearson's correlation, Spearman's correlation and 1-way ANOVA were used to test significance of differences. Placental surface area, placental weight and placental volume of Indians were lower than Malays (P< 0.05). Placental weight correlated significantly with neonatal length (r=0.527), birthweight (r=0.665), head circumference (r=0.371) and booking weight (r=0.193) while placental volume correlated with neonatal length (r=0.588), birthweight (r=0.688), head circumference (r=0.384), parity (r=0.202) and booking weight (r=0.219) at P< 0.05. Indian babies weight and length were less than Chinese and Malay babies (P< 0.05) while booking weight of Indian mothers was less than those of Chinese mothers (P< 0.05). Even after being corrected for booking weight, placental parameters of Indian patients were still significantly less than Malays and Chinese (P< 0.05).

    Suboptimal management of healthcare waste poses a significant concern that can be effectively tackled by implementing Internet of Things (IoT) solutions to enhance trash monitoring and disposal processes. The potential utilisation of the Internet of Things (IoT) in addressing the requirements associated with biomedical waste management within the Kaduna area was examined. The study included a selection of ten hospitals, chosen based on the criterion of having access to wireless Internet connectivity. The issue of biomedical waste is significant within the healthcare sector since it accounts for a considerable amount of overall waste generation, with estimates ranging from 43.62 to 52.47% across various facilities. Utilisation of (IoT) sensors resulted in the activation of alarms and messages to facilitate the prompt collection of waste. Data collected from these sensors was subjected to analysis to discover patterns and enhance the overall efficiency of waste management practices. The study revealed a positive correlation between the quantity of hospital beds and the daily garbage generated. Notably, hospitals with a higher number of beds were observed to generate a much greater amount of waste per bed. Hazardous waste generated varies by hospital, with one hospital leading in sharps waste (10.98 kgd-1) and chemical waste (21.06 kgd-1). Other hospitals generate considerable amounts of radioactive waste (0.60 kgd-1 and 0.50 kgd-1), pharmaceuticals, and genotoxic waste (16.19 kgd-1), indicating the need for specialised waste management approaches. The study sheds light on the significance of IoT in efficient waste collection and the need for tailored management of hazardous waste.

    This work reports a significant improvement in both the open-circuit voltage (VOC) and current density (J) of dye-sensitized solar cells (DSSCs) using gold nanorod-modified TiO2 nanoparticles (TiO2/AuNRs) together with a cobalt-imidazolate framework (ZIF-67) as an efficient photoanode. It was demonstrated that adding ZIF-67 (8 wt%) to TiO2 NPs increased the VOC by 160 mV and J by 2.5 times. This observation was described based on the significant increase in the amount of adsorbed dye in the presence of highly porous ZIF-67, which boosts the photoanode's light harvesting. Modifying TiO2 NPs with AuNRs also caused a remarkable enhancement in J (∼ 2.8 times), which can be explained via electron transfer between the TiO2 conduction band and AuNRs. It can result in a more efficient inhibiting effect on the interfacial charge recombination processes in TiO2/AuNRs/ZIF-67 because of the formation of a Schottky barrier at the interface between TiO2 and Au. These effects were confirmed by the reduction in the photoluminescence intensity of TiO2 in the presence of AuNRs. More reduction in the photoluminescence intensity was observed when ZIF-67 was added. The prepared photoanode showed an outstanding improvement in the overall efficiency of the DSSC (η) to 8.38% compared to the bare TiO2-based photoanode (1.83%). The notable improvement in the TiO2/AuNRs/ZIF-67 performance confirmed its practicality for high-efficiency DSSCs.

In the existing literature, power dispatch and demand side management schemes were implemented for various techno-economic objectives. In renewable energy-based distributed grids, power dispatch is strategic to system operations. However, demand side management is preferred, as it allows more options for customer participation and active management of energy in buildings. Moreover, the demand side management can simply follow supplies. This paper investigates the implications of demand side management as it affects planning and operations in renewable energy-based distributed grids. Integration of demand side management in customer-oriented plans such as the time-of-use and real-time-pricing on residential and commercial demands is conceptualised to ensure effective customer participation which maintains the valued comforts. Moreover, the optimised tariff integrated demand side management implementations based on the utility-initiated demand response programmes are envisaged to offset conflicting objectives of the economy and customer comforts within residential and commercial demands and are also viewed as a step towards efficient management of energy in buildings.

    Water bodies with the dye methylene blue pose serious environmental and health risks to humans. Therefore, the creation and investigation of affordable, potential adsorbents to remove methylene blue dye from water resources as a long-term fix is one focus of the scientific community. Food plants and other carbon-source serve as a hotspot for a wider range of application on different pollutants that impact the environment and living organisms. Here, we reviewed the use of treated and untreated biosorbents made from plant waste leaves for removing the dye methylene blue from aqueous media. After being modified, activated carbon made from various plant leaves improves adsorption performance. The range of activating chemicals, activation methods, and bio-sorbent material characterisation using FTIR analysis, Barunauer-Emmett-Teller (BET) surface area, scanning electron microscope (SEM-EDX), and SEM-EDX have all been covered in this review. It has been thoroughly described how the pH solution of the methylene blue dye compares to the pHPZC of the adsorbent surface. The presentation also includes a thorough analysis of the application of the isotherm model, kinetic model, and thermodynamic parameters. The selectivity of the adsorbent is the main focus of the adsorption kinetics and isotherm models. It has been studied how adsorption occurs, how surface area and pH affect it, and how biomass waste compares to other adsorbents. The use of biomass waste as adsorbents is both environmentally and economically advantageous, and it has been discovered to have exceptional color removal capabilities.
