There has been an increase in the use of therapeutic plasma exchange (TPE) in immune-mediated neurological disorders in recent years. However, accessibility and availability of TPE remains low and costly, especially for a country with limited healthcare funding like Malaysia. With expanding clinical indications in neurological disorders, and increasingly expensive conventional immunomodulatory treatment such as intravenous immunoglobulin and monoclonal antibodies, TPE remains an effective part of first or second-line treatment. In this article, we detailed the historical aspects of the use of TPE in neurological disorders in Malaysia over the last four decades and discussed the challenges behind the establishment of the first in-house neurology-driven TPE service in the country. Local TPE database from a national neurology centre in Kuala Lumpur over the past 20 years was analyzed. We observed a remarkable three folds increase in the use of TPE at our center over the past 10 years (total 131 TPE treatments) compared to a decade prior, with expanding clinical indications predominantly for central nervous system demyelinating disorders. Besides using membrane filtration method, centrifugal technique was adopted, providing new opportunities for other clinical beneficiaries such as a neurologist driven "in-house TPE unit". However, there were real world challenges, especially having to provide services with limited funding, human resources, and space. In addition, much has to be done to improve accessibility, availability, and sustainability of TPE services at our center and nationwide. Nevertheless, even with limited resources and support, it is possible with concerted efforts to work within the confines of these limitations to establish a safe, successful, and sustainable TPE service.

The COVID-19 pandemic has revolutionized the teaching pedagogy in higher education as universities are forecasted to increase investments in learning technology infrastructure to transition away from traditional teaching methods. Therefore, it is crucial to investigate whether academics intend to continually integrate learning technologies as part of a permanent pedagogical change beyond the COVID-19 pandemic. Drawing upon the Unified Theory of Acceptance and Use of Technology (UTAUT), and Expectation Confirmation Model (ECM), this study examines the salient determinants influencing the continuance intention of academics to use learning technologies in their teaching pedagogy during and after COVID-19. Primary data collected from a private university was analyzed using the partial least squares structural equation modelling technique (PLS-SEM). The findings revealed two sequential mediating relationships which serve as the mechanism linking the relationship between facilitating conditions and their continuance intention to use learning technologies during and beyond the COVID-19 pandemic.

Malaysia has adopted various strategies in developing its space sector. Indigenous space technologies would enable a sustainable growth of the space field and at the same time develop the strategic space technologies. Therefore, issues related to the current space research level are fundamentally crucial to be highlighted. Subsequently, the space focus areas can be derived in order to meet the expectations of the national and international space technology growth requirements, which are moving on to a stronger posture in R&D. In the absence of a strong R&D national space industry leadership, the Malaysian space sector remains in a traditional downstream mode of the world space technology supply chain ever since. The space technology supply chain can be divided into the ground segment and the space segment. This paper examines the current space research activities in Malaysia within the framework of the space technology supply chain. As a result, a preliminary gap in the overview of space research in Malaysia is established.

Modern molecular genetic methods, massive parallel sequencing in particular, allow for genotyping of various pathogens with the aim of their epidemiological marking and improvement of molecular epidemiological surveillance of actual infections, including cytomegalovirus infection. The aim of the study is to evaluate the next-generation sequencing (NGS) technology for genotyping clinical isolates of cytomegalovirus (CMV).

MATERIALS AND METHODS: The object of the study were samples of biological substrates (leukocyte mass, saliva, urine) taken from patients who underwent liver and kidney transplantation. Detection of CMV DNA was carried out by a real-time PCR using commercial diagnostic AmpliSense CMV-FL test systems (Central Research Institute for Epidemiology, Moscow, Russia). DNA extraction was performed using DNA-sorb AM and DNA-sorb V kits (Central Research Institute for Epidemiology) in accordance with manufacturer's manual. The quality of the prepared DNA library for sequencing was assessed by means of the QIAxcel Advanced System capillary gel electrophoresis system (QIAGEN, Germany). Alignment and assembly of nucleotide sequences were carried out using CLC Genomics Workbench 5.5 software (CLC bio, USA). The sequencing results were analyzed using BLAST of NCBI server.

RESULTS: CMV DNA samples were selected for genotyping. The two variable genes, UL55(gB) and UL73(gN), were used for CMV genotype determination, which was performed using NGS technology MiSeq sequencer (Illumina, USA). Based on the exploratory studies and analysis of literature sources, primers for genotyping on the UL55(gB) and UL73(gN) genes have been selected and the optimal conditions for the PCR reaction have been defined. The results of sequencing the UL55(gB) and UL73(gN) gene fragments of CMV clinical isolates from recipients of solid organs made it possible to determine the virus genotypes, among which gB2, gN4c, and gN4b were dominant. In some cases, association of two and three CMV genotypes has been revealed.

CONCLUSION: The application of the NGS technology for genotyping cytomegalovirus strains can become one of the main methods of CMV infection molecular epidemiology, as it allows for obtaining reliable results with a significant reduction in research time.

The spatial and temporal dynamics of daily ultraviolet index (UVI) for a period of 18 years (2004-2022) over the Indian state of Kerala were statistically characterised in the study. The UVI measurements used for the study were derived from the ultraviolet-B (UVB) irradiance measured by the Ozone Monitoring Instrument (OMI) of the AURA satellite and classified into different severity levels for analysis. Basic statistics of daily, monthly and seasonal UVI as well as Mann-Kendall (MK) statistical trend characteristics and the rate of change of daily UVI using Theil-Sen's slope test were also evaluated. A higher variability of UVI characteristics was observed in the Kerala region, and more than 79% of the measurements fell into the categories of very high and extreme UVI values, which suggests the need of implementation of appropriate measures to reduce health risks. Although the UVI measured during the study period shows a slight decrease, most of the data show a seasonal variation with undulating low and peak values. Higher UVI are observed during the months of March, April and September. The region also has higher UVI during the southwest monsoon (SWM) and summer seasons. Although Kerala region as a single whole unit, UVI show a non-significant decreasing trend (-0.83), the MK test revealed the increasing and decreasing trends of UVI ranging from -1.96 to 0.41 facilitated the delineation of areas (domains) where UVI are increasing or decreasing. The domain of UVI increase occupies the central and southern (S) parts, and the domains of decrease cover the northern (N) and S parts of the Kerala region. The rate of change of daily UVI in domain of increase and decrease shows an average rate of 0.34 × 10-5 day-1 and -2 × 10-5 day-1, respectively. The parameters (rainfall, air temperature, cloud optical depth (COD) and solar zenith angle (SZA)) that affect the strength of UV rays reaching the surface indicate that a cloud-free atmosphere or low thickness clouds prevails in the Kerala region. Overall, the study results indicate the need for regular monitoring of UVI in the study area and also suggest appropriate campaigns to disseminate information and precautions for prolonged UVI exposure to reduce the adverse health effects, since the study area has a high population density.

COVID'19 pandemic has devastated several industries and solar energy is no exception. In its economic relief package, Malaysia has announced approximately US$ 2.9 billion in expenditure for the installation of new grids, LED street lights and rooftop solar panels. The Government will also open the tender for a 1400 MW solar power project in the year 2020, which is expected to generate 5 billion ringgit (US$1.1 billion) in investments. As these measures are intended to sustain the existing growth of solar energy potential in the country, it is vital to assess its status quo. Hence, this paper aims to review the current status of renewable energy in Malaysia as well as the initiatives taken before the pandemic to promote solar photovoltaic (PV) technology to meet the energy demands through the low-carbon pathway.

Customer is considered as the king in the world of business. The issue of customer satisfaction in electronics home appliances has received greater attention from academics and practitioners. In other words, customer satisfaction is a vital consideration in marketing. With the development of technology, new and innovative electronic home appliances are available in the market. Customers purchase and use the costly electronic home appliances where the satisfaction issue is an important concern. In Bangladesh, working families find the electronic home appliance very necessary. Companies offer state-of- the-art appliances for customers' household works. Therefore, the study intends to investigate the effect of product quality (PQ), quality of service (SQ) and perceived value on customer satisfaction (CS). In addition, this study also seeks this relationship shaped by customer's perceived value (CPV) as a key mechanism and interacted by social media usage. A total of 300 households were selected on a judgmental basis from Dhaka city in Bangladesh using a structured questionnaire. Collected data were CB-SEM (AMOS-v24) and SPSS. The findings showed PQ and SQ have positive effects on CS; SQ affects, but PQ does not affect CPV. CPV has a mixing mediating effect on SQ and CS relationship and PQ and CS relationship. Importantly, the positive impact of PQ, SQ and CPV is greater on customers who exhibit higher social media use. The conceptual framework was buttressed by EDT theory. The study contributed to contextual and theoretical knowledge in regards to home appliances. The practicing managers can collect an insight of customer satisfaction for their business.

Wind energy has often been touted as one of the most reliable sources of renewable energy that should be used for people. Today, wind energy (mainly by propeller type wind turbines) produces less than one percent of the total energy used worldwide. Practically, a standard three-blade propellers efficiency of use of the wind energy is around twenty percents and this is due to its design and shape that use the wind lift force and a rotating turbine. In addition, these turbines are quite expensive due to the complex aerodynamic shape of the propellers which are made of composite materials. The new world boom for wind turbines obliges inventors to create new wind turbine designs that have high efficiency and are better than any known design. This paper proposes the new patented invention of the vane-type wind turbine which uses wind energy more efficiently and is only dependent on the acting area of the vanes. The vane wind turbine was designed to increase the output of a wind turbine that uses kinetic energy of the wind. Due to its high efficiency, simple construction and technology, the vane wind turbine can be used universally, apart from the fact that it is made from cheap materials. The new design of the vane-type wind turbine has quite small sizes than the propeller type one of same output power.

Objective: The present study aimed to assess the knowledge, attitude, and practice (KAP) of 3rd- and 4th-year medical, dental, and pharmacy students about hepatitis B (HB) infection at a private medical university, Malaysia.

Methods: A cross-sectional, questionnaire-based study was conducted among 482 medical, dental, and pharmacy students of 3rd- and 4th-year degree program of Asian Institute of Medicine, Science and Technology University to assess their KAP about HB infection using 34 prevalidated questions by convenient sampling method. A questionnaire was administered to the students, and their responses were measured at "yes" and "no" scale. Students' responses were entered in SPSS version 22, and quantitative analysis was performed using descriptive statistics and nonparametric tests at P < 0.05.

Findings: The medical, dental, and pharmacy students had good knowledge and practice with positive attitude about HB infection. Mann-Whitney U-test determined a significant difference in knowledge (P < 0.001) and practice (P < 0.001) scores between medical and pharmacy, attitude (P < 0.001) scores between medical and dental, and attitude (P < 0.001) and practice (P < 0.001) scores between pharmacy and dental students. Students' age was correlated with their attitude, practice, and KAP scores and family income with their knowledge, attitude, practice, and KAP scores.

Conclusion: Although students' knowledge and practice were good with positive attitude, all the students did not participate in health education program, screening, and vaccination of hepatitis B virus (HBV) infection which makes them more vulnerable to occupational HBV infection. Hence, it is recommended to organize a regular health education program for the students on screening and vaccination against HBV to prevent its infection.

Enterprises across the globe are facing increasing pressure to effectively utilize resources and reduce costs through green supply chain practices. Emerging technology, such as blockchain technology which enables green practices, has become a contemporary industrial paradigm. However, enterprises need to build green intellectual capital to implement blockchain technology, which can be key to realizing green supply chain practices. This research examines the impact of green intellectual capital (GIC) on blockchain technology and its role in implementing green manufacturing to achieve sustainability. Partial least squares structural equation modeling was utilized for assessing the proposed hypotheses, and cross-sectional data was accumulated from manufacturing firms. As per the results, GIC, which includes green human capital, green structural capital, and green relational capital has a crucial role in the implementation of blockchain technology. The outcomes also indicated that the adoption of blockchain technology significantly influences green manufacturing. Moreover, green manufacturing (GM) has a substantial role in improving business sustainability. This empirical research provides a deeper understanding of how GIC and blockchain technology contribute to the implementation of GM. This research also provides guidelines that managers, policymakers, and producers can use to facilitate the incorporation of GM practice into business activities.

Resource bound security solutions have facilitated the mitigation of spatio-temporal attacks by altering protocol semantics to provide minimal security while maintaining an acceptable level of performance. The Dynamic Window Secured Implicit Geographic Forwarding (DWSIGF) routing protocol for Wireless Sensor Network (WSN) has been proposed to achieve a minimal selection of malicious nodes by introducing a dynamic collection window period to the protocol's semantics. However, its selection scheme suffers substantial packet losses due to the utilization of a single distance based parameter for node selection. In this paper, we propose a Fuzzy-based Geographic Forwarding protocol (FuGeF) to minimize packet loss, while maintaining performance. The FuGeF utilizes a new form of dynamism and introduces three selection parameters: remaining energy, connectivity cost, and progressive distance, as well as a Fuzzy Logic System (FLS) for node selection. These introduced mechanisms ensure the appropriate selection of a non-malicious node. Extensive simulation experiments have been conducted to evaluate the performance of the proposed FuGeF protocol as compared to DWSIGF variants. The simulation results show that the proposed FuGeF outperforms the two DWSIGF variants (DWSIGF-P and DWSIGF-R) in terms of packet delivery.

Recently, Wireless Body Area Network (WBAN) has witnessed significant attentions in research and product development due to the growing number of sensor-based applications in healthcare domain. Design of efficient and effective Medium Access Control (MAC) protocol is one of the fundamental research themes in WBAN. Static on-demand slot allocation to patient data is the main approach adopted in the design of MAC protocol in literature, without considering the type of patient data specifically the level of severity on patient data. This leads to the degradation of the performance of MAC protocols considering effectiveness and traffic adjustability in realistic medical environments. In this context, this paper proposes a Traffic Priority-Aware MAC (TraPy-MAC) protocol for WBAN. It classifies patient data into emergency and non-emergency categories based on the severity of patient data. The threshold value aided classification considers a number of parameters including type of sensor, body placement location, and data transmission time for allocating dedicated slots patient data. Emergency data are not required to carry out contention and slots are allocated by giving the due importance to threshold value of vital sign data. The contention for slots is made efficient in case of non-emergency data considering threshold value in slot allocation. Moreover, the slot allocation to emergency and non-emergency data are performed parallel resulting in performance gain in channel assignment. Two algorithms namely, Detection of Severity on Vital Sign data (DSVS), and ETS Slots allocation based on the Severity on Vital Sign (ETS-SVS) are developed for calculating threshold value and resolving the conflicts of channel assignment, respectively. Simulations are performed in ns2 and results are compared with the state-of-the-art MAC techniques. Analysis of results attests the benefit of TraPy-MAC in comparison with the state-of-the-art MAC in channel assignment in realistic medical environments.

Unpredictable natural disasters, disease outbreaks, climate change, pollution, and war constantly threaten food crop production. Smart and precision farming encourages using information or data obtained by using advanced technology (sensors, AI, and IoT) to improve decision-making in agriculture and achieve high productivity. For instance, weather prediction, nutrient information, pollutant assessment, and pathogen determination can be made with the help of new analytical and bioanalytical methods, demonstrating the potential for societal impact such as environmental, agricultural, and food science. As a rising technology, biosensors can be a potential tool to promote smart and precision farming in developing and underdeveloped countries. This review emphasizes the role of on-field, in vivo, and wearable biosensors in smart and precision farming, especially those biosensing systems that have proven with suitably complex and analytically challenging samples. The development of various agricultural biosensors in the past five years that fulfill market requirements such as portability, low cost, long-term stability, user-friendliness, rapidity, and on-site monitoring will be reviewed. The challenges and prospects for developing IoT and AI-integrated biosensors to increase crop yield and advance sustainable agriculture will be discussed. Using biosensors in smart and precision farming would ensure food security and revenue for farming communities.

The emergence of wireless technologies such as WirelessHART and ISA100 Wireless for deployment at industrial process plants has urged the need for research and development in wireless control. This is in view of the fact that the recent application is mainly in monitoring domain due to lack of confidence in control aspect. WirelessHART has an edge over its counterpart as it is based on the successful Wired HART protocol with over 30 million devices as of 2009. Recent works on control have primarily focused on maintaining the traditional PID control structure which is proven not adequate for the wireless environment. In contrast, Internal Model Control (IMC), a promising technique for delay compensation, disturbance rejection and setpoint tracking has not been investigated in the context of WirelessHART. Therefore, this paper discusses the control design using IMC approach with a focus on wireless processes. The simulation and experimental results using real-time WirelessHART hardware-in-the-loop simulator (WH-HILS) indicate that the proposed approach is more robust to delay variation of the network than the PID.

This study demonstrated the potential of single chamber up-flow membrane-less microbial fuel cell (UFML-MFC) in wastewater treatment and power generation. The purpose of this study was to evaluate and enhance the performance under different operational conditions which affect the chemical oxygen demand (COD) reduction and power generation, including the increase of KCl concentration (MFC1) and COD concentration (MFC2). The results showed that the increase of KCl concentration is an important factor in up-flow membrane-less MFC to enhance the ease of electron transfer from anode to cathode. The increase of COD concentration in MFC2 could led to the drop of voltage output due to the prompt of biofilm growth in MFC2 cathode which could increase the internal resistance. It also showed that the COD concentration is a vital issue in up-flow membrane-less MFC. Despite the COD reduction was up to 96%, the power output remained constrained.

In our previous study, a novel alginate-based bilayer film for slow-release wound dressings was successfully developed. We found that alginate alone yielded poor films; however, the addition of gelatine had significantly enhanced the drug dispersion as well as the physical properties. Here, an investigation of the drug-polymer interactions in the bilayer films was carried out. Drug content uniformity test and microscopy observation revealed that the addition of gelatine generated bilayer films with a homogenous drug distribution within the matrix. The FTIR and XRD data showed an increase in film crystallinity which might infer the presence of drug-polymer crystalline microaggregates in the films. DSC confirmed the drug-polymer interaction and indicated that the gelatine has no effect on the thermal behaviour of the microaggregates, suggesting the compatibility of the drug and excipients in the bilayer films. In conclusion, the addition of gelatine can promote homogenous dispersion of hydrophobic drugs in alginate films possibly through the formation of crystalline microaggregates.

Medication reviews are effective in improving the quality of medication use among older people. However, they are conducted to various standards resulting in a wide range of outcomes which limit generalisability of findings arising from research studies. There also appear to be funding and time constraints, lack of data storage for quality improvement purposes, and non-standardised reporting of outcomes, especially clinically relevant outcomes. Furthermore, the coronavirus disease-19 (COVID-19) pandemic has restricted many face-to-face activities, including medication reviews. This article introduces a technology-enabled approach to medication reviews that may overcome some limitations with current medication review processes, and also make it possible to conduct medication reviews during the COVID-19 pandemic by providing an alternate platform. The possible advantages of this technology-enabled approach, legislative considerations and possible implementation in practice are discussed.
